Purpose of review: To critique the recent literature on telephone, correspondence-based, and computerized interventions for alcohol problems, which enhance or substitute for practitioner-delivered treatments. Recent findings: There is an unmet need for screening, assessment and intervention for alcohol problems, in part because of the difficulty in accessing such treatment within the current health care system. Research on the efficacy of correspondence or electronic (for example Internet-based) interventions is beginning to emerge. In the period 2003–2004 we identified nine acceptability or feasibility studies of these approaches and seven efficacy trials covering a wide range of settings. These modes of intervention are acceptable to patients and the public, and with careful planning, can be implemented in a variety of settings. Treatment trials demonstrate the efficacy of these interventions in reducing hazardous drinking by university students, in delaying initiation of heavy drinking in children and adolescents, and, intriguingly, in addressing insomnia among recovering alcoholics. Summary: There is strong support among potential users for alcohol interventions that employ telephone assistance, written correspondence, and the Internet. These new technologies offer the prospect of increasing the reach of interventions for problem drinking and being cost- effective alternatives or supplements to face-to-face health service delivery.

Background: The incidence of obesity is increasing; this is of major concern, as obesity is associated with cardiovascular disease, stroke, type 2 diabetes, respiratory tract disease, and cancer. Objectives/methods: This evaluation is of a Phase II clinical trial with tesofensine in obese subjects. Results: After 26 weeks, tesofensine caused a significant weight loss, and may have a higher maximal ability to reduce weight than the presently available anti-obesity agents. However, tesofensine also increased blood pressure and heart rate, and may increase psychiatric disorders. Conclusions: It is encouraging that tesofensine 0.5 mg may cause almost double the weight loss observed with sibutramine or rimonabant. As tesofensine and sibutramine have similar pharmacological profiles, it would be of interest to compare the weight loss with tesofensine in a head-to-head clinical trial with sibutramine, to properly assess their comparative potency. Also, as teso fensine 0.5 mg increases heart rate, as well as increasing the incidence of adverse effects such as nausea, drug mouth, flatulence, insomnia, and depressed mode, its tolerability needs to be further evaluated in large Phase III clinical trials.

This is a guidebook for clinicians on how to conduct assessment interviews with patients presenting with common psychological disorders. The orientation is behavioural and cognitive; so the book has wide applicability, as most clinicians explicitly or implicitly accept this combination of models as a useful basis for assessing and treating these problems. The problem areas covered are: fear and anxiety problems; depression, obesity; interpersonal problems; sexual dysfunction; insomnia; headache; and substance abuse.

Aim: To explore the lived experience of being a sole mother in Taiwan Background: The number of sole mothers in Taiwan has increased by 55 % in the last decade due to changes in the social and economic status of women (e.g. earlier divorce, the development of national policies for the protection of women, the rise of feminism, and changing work practices which have seen an increase in the number of women in the workforce) (Taiwan Department of Statistics, 2010). Issues confronting sole mothers as part of daily living involve inability to cope with daily life stressors, little social support, experiencing feelings of helplessness and hopelessness, and lack of self-confidence to assume responsibility for the physical and mental health needs of themselves and their children (Cairney, 2007; Loxton, Mooney & Young, 2006; Samuels-Dennis, 2006; Waldron et al., 1996). Although there have been a number of studies conducted concerning what it means to be a sole mother, few Taiwanese studies have been undertaken. In light of the absence of research on this topic from a Taiwanese perspective, this study was undertaken. Design:A descriptive phenomenological approach was used for this study. Methods: In-depth audio-taped interviews were conducted with 15 sole Taiwanese mothers. The audiotapes were later transcribed, translated into English, and then back translated into Chinese to ensure accuracy of participants‘ information. Colaizzi‘s phenomenological approach to analysis with one additional step (eight steps in all) informed the analytical process. Findings: The process of analysis identified six central themes: 1. Enduring the burdensome, 2. Survival means living day-by-day, 3. Living in the shadows of insomnia, depression and suicidal thoughts, 4. Living with rejection and social isolation, 5. Living with uncertainty, and 6. Transcending difficult times through being resilient. Conclusion: For the participants of this study, the lived world of Taiwanese sole mothers was replete with daily difficulties marked by isolation, loneliness, social disapproval and rejection. Feelings of sadness and dejection were their daily companions. However, amid their myriad hardships, the participants found strength and solace in their children and close friends. Rather than succumb to the pressures of being a sole mother, the participants forged new paths spurred on by their own hopes and dreams for a better future. The findings of this study have the potential to make significant contributions to extant knowledge concerning the lived experiences of sole mothers in Taiwan.

Background: When experiencing sleep problems for the first time, consumers may often approach community pharmacists for advice as they are easily accessible health care professionals in the community. In Australian community pharmacies there are no specific tools available for use by pharmacists to assist with the assessment and handling of consumers with sleep enquiries. Objective: To assess the feasibility of improving the detection of sleep disorders within the community through the pilot of a newly developed Community Pharmacy Sleep Assessment Tool (COP-SAT). Method: The COP-SAT was designed to incorporate elements from a number of existing, standardized, and validated clinical screening measures. The COP-SAT was trialed in four Australian community pharmacies over a 4-week period. Key findings: A total of 241 community pharmacy consumers were assessed using the COP-SAT. A total of 74 (30.7%) were assessed as being at risk of insomnia, 26 (10.7%) were at risk of daytime sleepiness, 19 (7.9%) were at risk of obstructive sleep apnea, and 121 (50.2%) were regular snorers. A total of 116 (48.1%) participants indicated that they consume caffeine before bedtime, of which 55 (47%) had associated symptoms of sleep onset insomnia. Moreover, 85 (35%) consumed alcohol before bedtime, of which 50 (58%) experienced fragmented sleep, 50 (58%) were regular snorers, and nine (10.6%) had apnea symptoms. The COP-SAT was feasible in the community pharmacy setting. The prevalence of sleep disorders in the sampled population was high, but generally consistent with previous studies on the general population. Conclusion: A large proportion of participants reported sleep disorder symptoms, and a link was found between the consumption of alcohol and caffeine substances at bedtime and associated symptoms. While larger studies are needed to assess the clinical properties of the tool, the results of this feasibility study have demonstrated that the COP-SAT may be a practical tool for the identification of patients at risk of developing sleep disorders in the community.

Background: Heart failure is a serious condition estimated to affect 1.5-2.0% of the Australian population with a point prevalence of approximately 1% in people aged 50-59 years, 10% in people aged 65 years or more and over 50% in people aged 85 years or over (National Heart Foundation of Australian and the Cardiac Society of Australia and New Zealand, 2006). Sleep disturbances are a common complaint of persons with heart failure. Disturbances of sleep can worsen heart failure symptoms, impair independence, reduce quality of life and lead to increased health care utilisation in patients with heart failure. Previous studies have identified exercise as a possible treatment for poor sleep in patients without cardiac disease however there is limited evidence of the effect of this form of treatment in heart failure. Aim: The primary objective of this study was to examine the effect of a supervised, hospital-based exercise training programme on subjective sleep quality in heart failure patients. Secondary objectives were to examine the association between changes in sleep quality and changes in depression, exercise performance and body mass index. Methods: The sample for the study was recruited from metropolitan and regional heart failure services across Brisbane, Queensland. Patients with a recent heart failure related hospital admission who met study inclusion criteria were recruited. Participants were screened by specialist heart failure exercise staff at each site to ensure exercise safety prior to study enrolment. Demographic data, medical history, medications, Pittsburgh Sleep Quality Index score, Geriatric Depression Score, exercise performance (six minute walk test), weight and height were collected at Baseline. Pittsburgh Sleep Quality Index score, Geriatric Depression Score, exercise performance and weight were repeated at 3 months. One hundred and six patients admitted to hospital with heart failure were randomly allocated to a 3-month disease-based management programme of education and self-management support including standard exercise advice (Control) or to the same disease management programme as the Control group with the addition of a tailored physical activity program (Intervention). The intervention consisted of 1 hour of aerobic and resistance exercise twice a week. Programs were designed and supervised by an exercise specialist. The main outcome measure was achievement of a clinically significant change (.3 points) in global Pittsburgh Sleep Quality score. Results: Intervention group participants reported significantly greater clinical improvement in global sleep quality than Control (p=0.016). These patients also exhibited significant improvements in component sleep disturbance (p=0.004), component sleep quality (p=0.015) and global sleep quality (p=0.032) after 3 months of supervised exercise intervention. Improvements in sleep quality correlated with improvements in depression (p<0.001) and six minute walk distance (p=0.04). When study results were examined categorically, with subjects classified as either "poor" or "good" sleepers, subjects in the Control group were significantly more likely to report "poor" sleep at 3 months (p=0.039) while Intervention participants were likely to report "good" sleep at this time (p=0.08). Conclusion: Three months of supervised, hospital based, aerobic and resistance exercise training improved subjective sleep quality in patients with heart failure. This is the first randomised controlled trial to examine the role of aerobic and resistance exercise training in the improvement of sleep quality for patients with this disease. While this study establishes exercise as a therapy for poor sleep quality, further research is needed to investigate the effect of exercise training on objective parameters of sleep in this population.

Background Sleep disturbances, including insomnia and sleep-disordered breathing, are a common complaint in people with heart failure and impair well-being. Exercise training (ET) improves quality of life in stable heart failure patients. ET also improves sleep quality in healthy older patients, but there are no previous intervention studies in heart failure patients. Aim The aim of this study was to examine the impact of ET on sleep quality in patients recently discharged from hospital with heart failure. Methods This was a sub-study of a multisite randomised controlled trial. Participants with a heart failure hospitalisation were randomised within six weeks of discharge to a 12-week disease management programme including exercise advice (n=52) or to the same programme with twice weekly structured ET (n=54). ET consisted of two one-hour supervised aerobic and resistance training sessions, prescribed and advanced by an exercise specialist. The primary outcome was change in Pittsburgh Sleep Quality Index (PSQI) between randomisation and week 12. Results At randomisation, 45% of participants reported poor sleep (PSQI≥5). PSQI global score improved significantly more in the ET group than the control group (–1.5±3.7 vs 0.4±3.8, p=0.03). Improved sleep quality correlated with improved exercise capacity and reduced depressive symptoms, but not with changes in body mass index or resting heart rate. Conclusion Twelve weeks of twice-weekly supervised ET improved sleep quality in patients recently discharged from hospital with heart failure.

In this paper, we identify two types of injustice as antecedents of abusive supervision and ultimately of subordinate psychological distress and insomnia. We examine distributive justice (an individual's evaluation of their input to output ratio compared to relevant others) and interactional injustice (the quality of interpersonal treatment received when procedures are implemented). Using a sample of Filipinos in a variety of occupations, we identify two types of injustice experienced by supervisors as stressors that provoke them to display abusive supervision to their subordinates. We examine two consequences of abusive supervision - subordinate psychological distress and insomnia. In addition, we identify two moderators of these relationships, namely, supervisor distress and subordinate self-esteem. We collected survey data from multiple sources including subordinates, their supervisors, and their partners. Data were obtained from 175 matched supervisor-subordinate dyads over a 6-month period, with subordinates' partners providing ratings of insomnia. Results of structural equation modelling analyses provided support for an indirect effects model in which supervisors' experience of unfair treatment cascades down the organization, resulting in subordinate psychological distress and, ultimately in their insomnia. In addition, results partially supported the proposed moderated relationships in the cascading model. © 2010 Taylor & Francis.

Sleep disturbance after mild traumatic brain injury (mTBI) is commonly reported as debilitating and persistent. However, the nature of this disturbance is poorly understood. This study sought to characterize sleep after mTBI compared with a control group. A cross-sectional matched case control design was used. Thirty-three persons with recent mTBI (1–6 months ago) and 33 age, sex, and ethnicity matched controls completed established questionnaires of sleep quality, quantity, timing, and sleep-related daytime impairment. The mTBI participants were compared with an independent sample of close-matched controls (CMCs; n=33) to allow partial internal replication. Compared with controls, persons with mTBI reported significantly greater sleep disturbance, more severe insomnia symptoms, a longer duration of wake after sleep onset, and greater sleep-related impairment (all medium to large effects, Cohen's d>0.5). No differences were found in sleep quantity, timing, sleep onset latency, sleep efficiency, or daytime sleepiness. All findings except a measure of sleep timing (i.e., sleep midpoint) were replicated for CMCs. These results indicate a difference in the magnitude and nature of perceived sleep disturbance after mTBI compared with controls, where persons with mTBI report poorer sleep quality and greater sleep-related impairment. Sleep quantity and timing did not differ between the groups. These preliminary findings should guide the provision of clearer advice to patients about the aspects of their sleep that may change after mTBI and could inform treatment selection.

OBJECTIVES To identify common genetic variants that predispose to caffeine-induced insomnia and to test whether genes whose expression changes in the presence of caffeine are enriched for association with caffeine-induced insomnia. DESIGN A hypothesis-free, genome-wide association study. SETTING Community-based sample of Australian twins from the Australian Twin Registry. PARTICIPANTS After removal of individuals who said that they do not drink coffee, a total of 2,402 individuals from 1,470 families in the Australian Twin Registry provided both phenotype and genotype information. MEASUREMENTS AND RESULTS A dichotomized scale based on whether participants reported ever or never experiencing caffeine-induced insomnia. A factor score based on responses to a number of questions regarding normal sleep habits was included as a covariate in the analysis. More than 2 million common single nucleotide polymorphisms (SNPs) were tested for association with caffeine-induced insomnia. No SNPs reached the genome-wide significance threshold. In the analysis that did not include the insomnia factor score as a covariate, the most significant SNP identified was an intronic SNP in the PRIMA1 gene (P = 1.4 x 10(-)(6), odds ratio = 0.68 [0.53 - 0.89]). An intergenic SNP near the GBP4 gene on chromosome 1 was the most significant upon inclusion of the insomnia factor score into the model (P = 1.9 x 10(-)(6), odds ratio = 0.70 [0.62 - 0.78]). A previously identified association with a polymorphism in the ADORA2A gene was replicated. CONCLUSIONS Several genes have been identified in the study as potentially influencing caffeine-induced insomnia. They will require replication in another sample. The results may have implications for understanding the biologic mechanisms underlying insomnia.

Tavoitteena oli tutkia 40-vuotiaiden miesten terveyskäyttäytymistä, terveysuskomuksia ja miesten saamaa terveysneuvontaa Helsingissä. 273 miestä vastasi kyselyyn ja osallistui terveystutkimuksiin. Terveydentilan perusteella miehet arvioitiin matalan (n=145) ja korkean (n=128) riskin ryhmiin. Khin neliö-testillä tutkittiin elämäntapa- ja riskitekijöitä koetun terveyden (hyvä, keskinkertainen/huono) luokissa ja verrattiin matalan ja korkean riskin ryhmiä em. tekijöiden osalta. Askeltavalla logistisella regressiomallilla analysoitiin tulosmuuttujia taustatekijöiden, terveyskäyttäytymisen, terveysuskomusten ja kliinisten riskitekijöiden avulla sekä arvioitiin oireiden ja vaivojen suhdetta koettuun terveydentilaan. Korkeassa riskissä olevien terveyttä seurattiin vuosina 2001–2004 analysoimalla mini-intervention vaikutusta terveysriskeihin ja elintapoihin varianssianalyysin avulla (ANOVA) (n=46). Matalasta vastausprosentista johtuen (39.6%), ei-vastanneiden aineistoa kerättiin käyttämällä syvähaastattelua (n=28) sekä puhelinkyselyä (n=40). Lopullinen aineisto koostui 341 miehestä. Tulokset osoittivat, että miehillä oli sydän- ja verisuonitautiriskejä. Kaksi kolmesta osallistuneista oli ylipainoisia tai lihavia, yli kolmanneksella vyötärönympärys oli ≥100 cm, ja yli 40%:llä oli diastolinen verenpaine ≥90 mmHg. Yli puolet tupakoi päivittäin ja 40% käytti alkoholia runsaasti. Ristiriitaisuutta ilmensi se, että huolimatta riskitekijöistä noin puolet miehistä koki terveydentilansa hyväksi. Sairauden tai vamman puute, hyvä suun terveydentila ja normaali vyötärönympärys olivat yhteydessä hyväksi koettuun terveydentilaan. Suora yhteys voitiin havaita omaisten tarjoaman neuvonnan ja vähäisen alkoholin käytön välillä. Masennus ja unettomuus olivat voimakkaasti yhteydessä loppuun palamiseen. Miehillä oli erilaisia fyysisiä ja psyykkisiä oireita, jotka korreloivat voimakkaasti masennuksen kanssa. Pieni määrä miehistä koki saaneensa terveysneuvontaa hoitohenkilökunnalta verrattuna perheenjäseniltä saatuun ohjaukseen. Korkeariskisten miesten (n=46) arvot parantuivat merkitsevästi lyhyellä aikavälillä. Kolesteroliarvoja lukuunottamatta ne palautuivat kolmen vuoden kuluttua alkumittausarvoja kohti. Laadullinen tutkimus osoitti, että “ei-vastanneet“ eivät osallistuneet projektiin, sillä he olivat oireettomia tai kiireisiä. Heillä todettiin samoja terveysriskejä kuin projektiin osallistuneilla. Syvähaastattelussa miehet toivat esille kokemuksiaan huolista, vihan tunteista, peloista ja yksinäisyydestä. Hoidonantajien on tärkeää ymmärtää ristiriidat miesten subjektiivisen ja objektiivisen terveydentilan välillä, mikä auttaa havaitsemaan esteitä terveyskäyttäytymiselle. Yhä enemmän tarvitaan yhteistyötä yksityisen ja julkisen terveydenhuollon välillä varmistamaan terveystottumusten jatkuminen miesten keskuudessa.

Background: Sporadic Creutzfeldt-Jakob disease (sCJD) is a rare neurodegenerative disorder in humans included in the group of Transmissible Spongiform Encephalopathies or prion diseases. The vast majority of sCJD cases are molecularly classified according to the abnormal prion protein (PrPSc) conformations along with polymorphism of codon 129 of the PRNP gene. Recently, a novel human disease, termed "protease-sensitive prionopathy", has been described. This disease shows a distinct clinical and neuropathological phenotype and it is associated to an abnormal prion protein more sensitive to protease digestion. Case presentation: We report the case of a 75-year-old-man who developed a clinical course and presented pathologic lesions compatible with sporadic Creutzfeldt-Jakob disease, and biochemical findings reminiscent of "protease-sensitive prionopathy". Neuropathological examinations revealed spongiform change mainly affecting the cerebral cortex, putamen/globus pallidus and thalamus, accompanied by mild astrocytosis and microgliosis, with slight involvement of the cerebellum. Confluent vacuoles were absent. Diffuse synaptic PrP deposits in these regions were largely removed following proteinase treatment. PrP deposition, as revealed with 3F4 and 1E4 antibodies, was markedly sensitive to pre-treatment with proteinase K. Molecular analysis of PrPSc showed an abnormal prion protein more sensitive to proteinase K digestion, with a five-band pattern of 28, 24, 21, 19, and 16 kDa, and three aglycosylated isoforms of 19, 16 and 6 kDa. This PrPSc was estimated to be 80% susceptible to digestion while the pathogenic prion protein associated with classical forms of sporadic Creutzfeldt-Jakob disease were only 2% (type VV2) and 23% (type MM1) susceptible. No mutations in the PRNP gene were found and genotype for codon 129 was heterozygous methionine/valine. Conclusions: A novel form of human disease with abnormal prion protein sensitive to protease and MV at codon 129 was described. Although clinical signs were compatible with sporadic Creutzfeldt-Jakob disease, the molecular subtype with the abnormal prion protein isoforms showing enhanced protease sensitivity was reminiscent of the "protease-sensitive prionopathy". It remains to be established whether the differences found between the latter and this case are due to the polymorphism at codon 129. Different degrees of proteinase K susceptibility were easily determined with the chemical polymer detection system which could help to detect proteinase-susceptible pathologic prion protein in diseases other than the classical ones.

A proposta primeira da dissertação Contra a Luz: insônia, prosa de ficção e Graciliano Ramos é a de investigar desdobramentos narrativos do tema da insônia na prosa de ficção da primeira metade do século XX. O primeiro capítulo, de um lado, traça um panorama histórico-literário da insônia da Idade Média ao século passado e, de outro lado, propõe algumas considerações de cunho psicanalítico sobre o tema. Pretende-se, assim, estabelecer alguns argumentos-chave que se desenvolverão ao longo dos capítulos subseqüentes, a saber: o de que a escuridão e o vazio noturno são altamente propícios à concentração na reflexão em detrimento da ação e que, portanto, possibilitam uma excepcional exploração da subjetividade dos personagens em questão. O segundo capítulo pensará tal situação no contexto da prosa de ficção moderna, a partir de breves estudos das obras Em busca do tempo perdido, de Marcel Proust; Livro do desassossego, de Fernando Pessoa (sob o heterônimo Bernardo Soares); Funes, o memorioso, de Jorge Luis Borges; e Buriti, de João Guimarães Rosa. Assim, serão expostas as maneiras pelas quais, nessas obras, as cenas de insônia mostram-se essenciais tanto à proposição de uma reflexão sobre a própria construção da narrativa, quanto permitem o aprofundamento psicológico dos personagens e o experimentalismo formal. Estes dois eixos permearão a Parte II da dissertação, que terá por foco a obra de Graciliano Ramos. O terceiro capítulo analisará a insônia do personagem Paulo Honório, no romance S.Bernardo, em relação à composição da narrativa feita por ele em suas noites em claro. O quarto capítulo, dedicado ao romance Angústia, investigará a instalação de um clima angustiado e de experimentações narrativas a partir das noites insones de Luís da Silva. Por fim, o quinto capítulo, abordando os contos Insônia e O relógio do hospital, traçará algumas conclusões sobre a função da insônia no estilo de Graciliano Ramos, e proporá também algumas considerações finais acerca de toda a dissertação

A menopausa, fenômeno fisiológico que ocorre em todas as mulheres, em média, aos 51 anos, é acompanhada em cerca de 80% dos casos de sintomas como fogachos, secura vaginal, irritabilidade e insônia, que interferem na qualidade de vida e na produtividade socioeconômica das mulheres, além de predispô-las a doenças crônico-degenerativas, como arteriosclerose, obesidade e distúrbios cardiovasculares. A terapia de reposição hormonal à base de estrógenos, que visa reduzir os incômodos da menopausa, está associada ao aumento do risco de câncer de mama e do endométrio, como foi demonstrado em estudos científicos. Considerando que as mulheres orientais, consumidoras de soja, apresentam doenças crônico-degenerativas e câncer em taxas inferiores às dos países ocidentais, as isoflavonas da soja têm sido testadas em estudos clínicos e experimentais, porém com obtenção de dados até contraditórios. O presente estudo investigou o efeito da administração crônica de isoflavonas de soja no útero, mamas e tecidos adiposo e ósseo de ratas ovariectomizadas. Quarenta ratas Wistar adultas foram distribuídas em quatro grupos experimentais: a) ovariectomizadas: grupo ISO, recebendo isoflavonas de soja (100mg/kg/dia/v.o.); b) ovariectomizadas: grupo BE, recebendo benzoato de estradiol (10g/kg/dia/s.c.); c) ovariectomizadas: grupo OVX, recebendo salina (0,1ml/100g/dia/v.o.); d) controles: grupo FO, recebendo salina (0,1ml/100g/dia/v.o.). Antes e durante os 90 dias de tratamento, foram analisados os esfregaços vaginais, para acompanhamento do ciclo estral, determinação do peso corporal e do consumo de ração semanal. Após esse período, os animais foram anestesiados e o sangue coletado para análise de estradiol e progesterona séricos, por radioimunoensaio; e lipidograma e glicose, por espectrofotometria. Posteriormente, os animais foram sacrificados e necropsiados, coletando-se o útero, mamas, gordura intra-abdominal e fêmur para macroscopia e pesagem. Os tecidos selecionados para o estudo foram corados em HE, analisados por microscopia óptica e histomorfometria, visando investigar alterações do crescimento celular (software V.S NIH Image-J; imagens digitais-optronicos CCD). No grupo tratado com isoflavonas, o peso corporal diminuiu em relação OVX, no qual ocorreu aumento de peso em comparação aos animais falso-operados. O exame macroscópico revelou que o útero diminuiu de peso nas ratas do grupo ISO, semelhante às do OVX. Além disso, a histopatologia das glândulas endometriais não mostrou alterações entre os grupos ISO e OVX. Contudo, o grupo BE apresentou proliferação glandular, pseudoestratificação epitelial, frequentes mitoses típicas, metaplasia escamosa, infiltrado eosinofílico e hidrométrio. A concentração de estradiol no grupo ISO foi semelhante à do OVX. Porém, no grupo BE, o estradiol e o peso uterino apresentaram-se aumentados em relação ao OVX. Não foram observadas diferenças na histomorfometria mamária entre os grupos. Houve redução no peso do tecido adiposo abdominal no grupo ISO, comparado com o OVX, sem identificação de alterações morfológicas significativas, apenas hipotrofia celular, confirmada pela histomorfometria. Apesar de não ter havido diferenças na concentração de glicose, colesterol total e triglicerídeos, entre os grupos, o colesterol-HDL apresentou aumento no grupo ISO. Não houve diferença na densitometria do fêmur entre os grupos avaliados. Esses resultados indicam que o tratamento crônico com isoflavonas de soja, na dose testada, não induz mudanças significativas no útero, mamas e tecidos adiposo e ósseo, sugerindo segurança no tratamento, sem risco para o desenvolvimento de câncer.

Trata-se de um estudo que possui como objeto as cargas de trabalho de enfermagem em hospital psiquiátrico e as repercussões para a saúde. O trabalho da enfermagem em hospital psiquiátrico tem particularidades ao se considerar a dinâmica do serviço e o cuidado de pacientes com diversos perfis de adoecimento psíquico; portanto com necessidades de cuidados e monitoramento extenuantes por parte da enfermagem durante 24 horas. Objetivos: identificar as cargas de trabalho vivenciadas pelos trabalhadores de enfermagem em um hospital psiquiátrico; descrever como as cargas de trabalho no hospital psiquiátrico afetam a saúde dos trabalhadores de enfermagem; analisar os mecanismos de enfretamento adotados pelos trabalhadores de enfermagem diante das cargas de trabalho no hospital psiquiátrico. Estudo qualitativo, exploratório e descritivo, tendo como campo um hospital psiquiátrico, situado no município do Rio de Janeiro. O projeto foi aprovado pelo Comitê de Ética em Pesquisa e protocolado com o n 619.262. A coleta de dados ocorreu no primeiro semestre de 2014, tendo participado do estudo 30 trabalhadores de enfermagem (enfermeiros, técnicos e auxiliares de enfermagem) a partir dos critérios de inclusão estabelecidos. Na coleta dos dados, realizada no primeiro semestre de 2014, trabalhou-se com a técnica de entrevista do tipo semiestruturada mediante um roteiro e na caracterização dos participantes, utilizou-se um instrumento autoaplicado. Após a transcrição dos depoimentos recorreu-se à análise de conteúdo do tipo temática, que apontou os seguintes resultados: as cargas presentes em hospital psiquiátrico, na visão dos trabalhadores, devem-se aos cuidados prestados aos pacientes de ordem subjetiva e objetiva, como também em relação à imprevisibilidade do quadro clínico, principalmente nas emergências psiquiátricas. Para o grupo, as cargas de trabalho, tanto físicas quanto psíquicas são intensificadas em função da precariedade das condições de trabalho, a ausência de poder do trabalhador frente a organização e ao volume de trabalho. As cargas acarretam o sofrimento psíquico evidenciado através de queixas como cansaço, nervosismo, incômodo, ansiedade, irritação e desgaste, dores musculares, insônia e cefaleia. Diante do sofrimento psíquico decorrente das cargas de trabalho o grupo elabora mecanismos de enfrentamento centrados na solução de problemas; momentos em que buscam a chefia e conversam com os pares. Quanto aos mecanismos de enfrentamento centrados na emoção, os trabalhadores, buscam afastar-se temporariamente do trabalho, procuram não pensar, não falar a respeito ou extravasam a tensão através de gestos e reclamações. Conclui-se que o hospital psiquiátrico possui cargas de trabalho específicas em função do tipo de trabalho realizado e clientela assistida que exigem dos trabalhadores de enfermagem observação e cuidados contínuos. Pelo fato de as cargas acarretarem prejuízos à saúde do trabalhador, há necessidade de uma política voltada para a prevenção e a promoção da saúde do grupo. Sugere-se a criação e manutenção de grupos de suporte aos trabalhadores com vistas a elaboração e discussão dos problemas enfrentados, assim como a continuidade dos estudos.
