Es ist alles Intrige], um daraus ein Motto für seine Analyse der gegenwãrtigen portugiesischen Gesellschaft zu entnechmen. Unfãhig, ihre herkõmmlichen Formen von Solidaritãt und gegenseitiger Hilfe wieder in Kraft zu stzen, sei die portugiesische Gesellschaft im chaotischen Ozean der Konkurrenz, der Arbeitslosigkeit und des Ausschlusses so tief gesunken, dass sie sich wegen der ihr gestellten Herausforderungen bedroht fühle. Der portugiesischen Gesellschaft würde es nicht gelingen, die Angst zu überwinden, die sie direkt aus der ehemaligen autoritãren und diktatorialen Gesellschaft als Erbe bekommen hatte, deren strukturelle Hauptstütze die Angst war - wie auch ein System, dessen Hauptziel es war, die Fõrderung der Kreativitãt , der schõpferischen und unternehmerischen Krãfte des sozialen, wirtschaftlichen Lebens zu unterdrücken; unter der Diktatur Salazars lebte man schon unter dem Einfluss der Gehorsamkeit, der Korruption, des Nicht-Teilnehmens, des Nicht-Einwerbens, des Nicht-Ereignens.

RESUMO: Este estudo procurou documentar a perspectiva (s) dos utentes de saúde mental e das associações de prestadores de cuidados sobre a prestação, o papel e a contribuição de serviços de saúde mental da comunidade tal como foram percebidos por um número de informadores-chave, incluindo os utentes do serviço mentais e os próprios prestadores de cuidados. O caso específico da Sociedade Saúde Mental do Gana (MEHSOG) foi o foco deste estudo. O modelo foi o de um estudo de caso, utilizando discussões de grupo e entrevistas com informadores-chave como instrumentos de recolha de dados. Estas ferramentas de colheita de dados foram complementadas por observações dos participantes e pela revisão de documentos da MEHSOG e dos vários grupos de apoio da comunidade de auto-ajuda que compõem a associação nacional. O estudo revelou que os utentes dos serviços de saúde mental e seus prestadores de cuidados constituem um importante grupo de partes interessadas na prestação de serviços de saúde mental da comunidade e no desenvolvimento de políticas que tenham em conta as necessidades e os direitos das pessoas com doença mental ou epilepsia. O envolvimento da MEHSOG promove a mobilização de membros e famílias relacionadas com a doença mental de beneficiar de serviços de tratamento bem organizados com um impacto significativo na melhoria da saúde e da participação dos utentes dos serviços e seus prestadores de cuidados primários em processos de tomada de decisão da família e na comunidade processos de desenvolvimento. Os utentes dos serviços por beneficiarem de tratamento, e os prestadores de cuidados primários, por se tornarem mais livres e menos sobrecarregados com a responsabilidade de cuidar, podem passar a envolver-se mais em atividades que melhoramo seu estado, o de suas famílias e das comunidades. A advocacia dos membros da MEHSOG para conseguir que a “Mental Health Bill” se transforme numa Lei foi também um desenvolvimento significativo resultante da participação ativa dos utentes do serviço em chamar a atenção para uma nova e inclusiva legislação de saúde mental para o Gana. Entre os fatores e oportunidades que permitiram aos utentes dos serviços de saúde mental e aos prestadores de cuidados primários de pessoas com doença mental apoiar activamente a prestação de serviços de saúde mental comunitária e o desenvolvimento de políticas conta-se a contribuição da sociedade civil do Gana, particularmente o movimento da deficiência, e os esforços anteriores de ONGs em saúde mental e dos profissionais de saúde mental para ter uma nova lei em saúde mental. Observámos um certo número de desafios e barreiras que actuam de forma a limitar a influência dos utentes dos serviços de saúde mental na provisão da saúde mental comunitária e no desenvolvimento de políticas. Entre elas o estigma social contra a doença mental e pessoas com doença mental ou epilepsia e seus cuidadores primaries é um factor chave. O estigma tem alterado a percepção e as análises do público em geral, especialmente dos profissionais de saúde e das autoridades políticas afetando a priorização dos problemas de saúde mental nas políticas e programas. Outro desafio foi a deficiente infra-estrutura disponível para apoiar serviços de saúde mentais que assegurem aos utentes permanecerem em bom estado de saúde e bem-estar para serem advogados de si próprios. A recomendação do presente estudo é que os movimentos de utentes dos serviços de saúde mental são importantes e que eles precisam de ser apoiados e encorajados a desempenhar o seu papel como pessoas com experiência vivida para contribuir para a organização e prestação de serviços de saúde mental, bem como para a implementação, monitorização e avaliação de políticas e programas. ------------------------------------ ABSTRACT: This study sought to document the perspective(s) of mental health users and care-givers associations in community mental health service provision and their role and contribution as it was perceived by a number of key informants including the mental service users and care-givers themselves. The specific case of the Mental Health Society of Ghana (MEHSOG) was the focus of this study. A case study approach was used to with Focus Group Discussions and Key Informants Interviews being the data collection tools that were used. These data collection tools were complemented by participant observations and review of documents of the MEHSOG and the various community self-help peer support groups that make up the national association. The study revealed that mental health service users and their care-givers constitute an important stakeholder group in community mental health service provision and development of policies that factor in the needs and rights of persons with mental illness or epilepsy. MEHSOG’s involvement in mobilising members and education families to come forward with the relations with mental illness to benefit from treatment services were well made a significant impact in improving the health and participation of service users and their primary carers in family decision-making processes and in community development processes. Service users, on benefiting from treatment, and primary care-givers, on becoming freer and less burdened with the responsibility of care, move on to engage in secure livelihoods activities, which enhanced their status in their families and communities. The advocacy MEHSOG members undertook in getting the mental health Bill become Law was also noted as significant development that was realised as a result of active involvement of service users in calling for a new and inclusive mental health legislation for Ghana. Enabling factors and opportunities that enabled mental health service users and primary care-givers of people with mental illness to actively support community mental health service provision and policy development is with the vibrant civil society presence in Ghana, particularly the disability movement, and earlier efforts by NGOs in mental health in Ghana long-side mental health professionals to have a new law in mental health. A number of challenges were also noted which were found to limit the extent to which mental health service users can be influential in community mental health service provision and policy development. Key among them was the social stigma against mental illness and people with mental illness or epilepsy and their primary carers. Stigma has affected perceptions, analyses of the general public, especially health practitioners and policy authorities that it has affected their prioritisation of mental health issues in policies and programmes. Another challenge was the poor infrastructure available to support enhanced mental health care services that ensure mental health service users remain in a good state of health and wellbeing to advocate for themselves. The recommendation from the study is that mental health service user movements are important and need to be supported and encouraged to play their role as persons with lived experience to inform organisation and provision of mental health services as well as design and implementation, monitoring and evaluation of policies and programes.

Des années 1780, quand surgit la question de l'émancipation des juifs, à la Première Guerre mondiale, qui dément l'optimisme de la perfectibilité du genre humain cultivé par la Bildung, le « long » XIXe siècle est la scène sur laquelle se déploient les efforts d'intégration des juifs dans la société et la culture allemandes, où la Bildung, intimement liée à l'esprit du protestantisme allemand qui l'a profondément marquée de son empreinte, tient lieu de médiation. Fil conducteur de ma recherche, la Bildung me permet de montrer en quoi son idéal est devenu un élément constitutif de l'identité des juifs allemands, en même temps qu'il cesse, sous les effets de la nationalisation d'une culture allemande devenue un outil au service d'un peuple particulier, d'être le projet, certes d'une communauté donnée, mais porteur d'universalisme. De fait, tout en adhérant à sa définition originale, les juifs ont su réinterpréter l'idée de Bildung en désamorçant l'alliance entre culture, germanité et nationalisme, afin de construire une nouvelle identité judéo-allemande qui réponde aux enjeux et aux exigences de la modernité ainsi qu'aux évolutions du temps, tout en visant à la reconnaissance des valeurs et du statut du judaïsme. Dans la mesure où cet idéal de la Bildung, sous les coups du nationalisme allemand, a perdu sa portée universelle pour, dans un processus de germanisation, devenir un instrument au service du projet nationaliste, les juifs vont progressivement se voir exclus de la nation allemande, quand bien même ou précisément parce qu'ils se sont identifiés à tel point au projet initial de la Bildung qu'ils en sont devenus les garants. From the 1780s, when the question of the emancipation of the Jews emerged, until World War I-a disappointment for those who were optimistic about cultivating a perfected humanity through Bildung (education)-the "long" nineteenth century is the stage on which the efforts to integrate the Jews into German society and culture took place. In this context, Bildung, which was decidedly bound to and profoundly marked by the German Protestant spirit, served as mediation. The underlying theme of Bildung in my research enables me to show how its ideal became the constitutive element of German Jewish identity. Concurrently, under the effects of the nationalization of German culture that became a tool in the service of a specific folk, the ideal of Bildung ceased to be a project that conveyed universal meaning. In fact, although the Jewish people agreed with its original definition, they succeeded in reinterpreting the idea of Bildung by neutralizing the alliance between culture, being German, and nationalism in order to elaborate a new German-Jewish identity in reply to the challenges and requirements of modernity and the evolution of society while still recognizing the values and status of Judaism. Inasmuch as the ideal of Bildung lost its universal significance for serving the nationalist project under the influence of German nationalism, the Jews were gradually excluded from the German folk, which took place despite, or precisely because, they identified to such an extent with the original aims of Bildung that they became the guarantors for it. Jahrhundert bildet die Kulisse der Integrationsbemühungen der Juden in die deutsche Gesellschaft und Kultur, von den 1780er Jahren, als die Frage nach der Judenemanzipation zutage kommt, bis zum Ersten Weltkrieg, der den Optimismus der menschlichen Verbesserungsfahigkeit durch die Bildung widerlegt. Die mit dem Geist des deutschen Protestantismus eng verbundene Bildung dient hier als Mediation. Der rote Faden der Bildung ermöglicht mir zu zeigen, inwiefern ihr Ideal wesentlich für die jüdische Identität geworden ist. Zur gleichen Zeit hat das Bildungsideal, unter der Wirkung der Nationalisierung der deutschen Kultur, die zum Werkzeug eines eigenartigen Volkes gemacht wurde, sein universales Wesen verloren. In der Tat, obwohl die Juden dem ursprünglichen Bildungsideal zustimmten, haben sie die Bildung neu interpretiert, indem sie die Verbindung zwischen Kultur, Germanentum und Nationalismus entschärft und eine neue deutsch-jüdische Identität gebildet haben, die den Herausforderungen und den Ansprüchen der Moderne sowie dem Gesellschaftswandel entsprach und gleichzeitig darauf abzielte, die Werte und den Status des Judentums zu anerkennen. BACKGROUND: The Adolescent Drug Abuse Diagnosis (ADAD) and Health of Nation Outcome Scales for Children and Adolescents (HoNOSCA) are both measures of outcome for adolescent mental health services. AIMS: To compare the ADAD with HoNOSCA; to examine their clinical usefulness. METHODS: Comparison of the ADAD and HoNOSCA outcome measures of 20 adolescents attending a psychiatric day care unit. RESULTS: ADAD change was positively correlated with HoNOSCA change. HoNOSCA assesses the clinic's day-care programme more positively than the ADAD. The ADAD detects a group for which the mean score remains unchanged whereas HoNOSCA does not. CONCLUSIONS: A good convergent validity emerges between the two assessment tools. The ADAD allows an evidence-based assessment and generally enables a better subject discrimination than HoNOSCA. HoNOSCA gives a less refined evaluation but is more economic in time and possibly more sensitive to change. Both assessment tools give useful information and enabled the Day-care Unit for Adolescents to rethink the process of care and of outcome, which benefited both the institution and the patients.

While flexible exchange rates facilitate stabilisation, exchange rate fluctuations can cause real volatility. This gives policy importance to the causal relationship between exchange rate depreciation and its volatility. An exchange rate may be expected to become more volatile when the underlying currency loses value. We conjecture that a reverse causation, which further weakens the currency, may be mitigated by price stability. Data from Ghana, Mozambique and Tanzania support this: depreciation makes exchange rate more volatile for all but volatility does not causes depreciation in Tanzania which has enjoyed a more stable inflation despite all countries adopting similar macro-policies since early 1990s.

Dans l'histoire européenne ont commencé à se former, à partir de la fin du XVIIIe siècle, des objets politiques inédits que l'on appelle maintenant des nations. Le mouvement a commencé en France et en Grande-Bretagne, pour se propager rapi- dement dans toute l'Europe, soutenu par les profonds bouleversements entraînés par la révolution industrielle et une timide démocratisation des sociétés. Au milieu de ce continent en pleine mutation se trouve un autre objet politique, au moins aussi étrange, vieille confédération d'États souverains sortie tout droit du Moyen-Âge et qu'on appelait encore il y a peu le «Corps helvétique». La Suisse, puisque c'est d'elle qu'il s'agit, est-elle affectée par ce vaste mouvement des nationalismes, et si oui comment ? Les nations européennes se construisent fondamentalement autour de quatre éléments : un État centralisé, un territoire, une histoire et un peuple. Dans leur pléni- tude, ces quatre éléments manquent à la Suisse. En fait d'État centralisé, on trouve plutôt un pouvoir central faible, débordé à la fois par des cantons ayant conservé de nombreuses prérogatives et par des associations privées redoutablement efficaces. Son territoire n'est que le résultat hasardeux de traités d'entraide successifs; son histoire est marquée par la désunion et les guerres; quant au peuple suisse, il faut avoir une imagination fertile pour en concevoir concrètement l'existence. C'est pourquoi il faut parler, à propos de la Suisse, d'une fêlure nationale. A défaut de peuple facilement identifiable, on construit des mythes politiques rassembleurs, parmi lesquels la neutralité armée, le consensus, le fédéralisme ou la démocratie. Mais cette mythologie tourne toujours un peu à vide, ce sera le propos de cet essai de le montrer.
