Résumé Introduction et but: Les accidents vasculaires cérébraux (AVC) ischémiques thalamiques sont traditionnellement classés en quatre territoires : antérieur (polaire ou tubérothalamique), paramédian (ou thalamo-perforant), inférolatéral (ou thalamo-genouillé) et postérieur. Le but de cette thèse est de déterminer, à l'aide des techniques d'imagerie actuelles, si cette classification est appropriée ou si il existe d'autres territoires à la jonction entre les précédents territoires. Méthode: Nous avons étudié les 3712 patients hospitalisés pour un premier AVC dans le service de neurologie du CHUV à Lausanne et inclus dans le « Lausanne Stroke Registry » entre 1990 et 2002. Parmi les 71 patients avec un infarctus thalamique confirmé par IRM, nous avons sélectionné tous les patients présentant un AVC hors des quatre territoires classiques en étudiant leur tableau clinique, étiologique et radiologique. Résultats: 21 patients (30% des patients avec AVC thalamiques) avaient un AVC hors des quatre territoires classiques, permettant de délimiter trois nouveaux territoires. 1) territoire anteromédian (9 patients (13%)), atteignant la partie postérieure du territoire antérieur et la partie antérieure du territoire paramédian, avec en premier lieu des troubles cognitifs (principalement troubles dysexecutifs, amnésie antérograde ainsi qu'une aphasie dans les lésions gauches). L'étiologie principale était cardio-embolique. 2) territoire central (4 patients (6%)), atteignant la partie centrale du thalamus provoquant différents signes neurologiques et neuropsychologiques, reflétant l'atteinte de différentes structures. La cause la plus fréquente était microangiopathique. 3) territoire posterolateral (8 patients (8%)), atteignant la partie postérieure du territoire inférolatéral et la partie antérieure du territoire postérieur provoquant en premier lieu une hémihypesthesie mais aussi une hémiataxie ainsi que des troubles dysexécutifs et une aphasie dans les lésions gauches. Les étiologies les plus fréquentes étaient artério-artérielle et microangiopathique. Conclusions: Nous décrivons trois nouvelles variantes topographiques d'AVC thalamiques avec des tableaux cliniques et étiologiques distincts. Nous postulons que ces variantes sont le résultat de variations de la vascularisation thalamique ou reflètent une atteinte ischémique jonctionnelle. Abstract Background and Purpose -Thalamic infarcts have traditionally been classified into 4 territories: anterior, paramedian, inferolateral, and posterior. The purpose of this study was to review this classical versus variant distribution in patients with thalamic stroke. Methods - We reviewed all patients with a first clinical stroke included in the Lausanne Stroke Registry between 1990 and 2002. Among 71 patients with an acute stroke isolated to the thalamus confirmed by MRI, we selected all patients with lesions outside the classical territories and studied their clinical, etiological, and radiological features. Results - A total of 21 patients (30% of all thalamic stroke patients) showed infarction outside the classical territories, allowing us to delineate 3 variant distributions: (1) Anteromedian territory (9 patients [13%]) involving anterior and paramedian territories, with predominantly cognitive impairment, including executive dysfunction, anterograde amnesia, and aphasia in left-sided or bilateral lesions. The most frequent stroke mechanism was cardiac embolism. (2) Central territory (4 patients [6%]), with lesions on the central part of the thalamus, resulting in a variety of neurological and neuropsychological signs, reflecting the involvement of several adjacent structures. Microangiopathy was the most frequent etiology. (3) Posterolateral territory (8 patients [11%]), involving inferolateral and posterior territories, with hemihypesthesia as the most frequent manifestation, followed by hemiataxia, executive dysfunction, and aphasia in left-sided lesions. Artery-to-artery embolism and microangiopathy were the main stroke mechanisms. Conclusions - We describe 3 variant topographic patterns of thalamic infarction with distinct manifestations and etiologies. We postulate that these infarcts are the result of a variation in thalamic arterial supply or reflect borderzone ischemia.

The applause sign was originally described as a quick bedside test to discriminate progressive supranuclear palsy (PSP) (positive applause sign, PAS) from Parkinson's disease (PD) and frontotemporal dementia (FTD) (negative applause sign). However, recent research demonstrated that the test is positive not only in a subset of patients with PD and FTD, but also in other neurodegenerative diseases. We tested 22 patients with amyotrophic lateral sclerosis (ALS) together with 22 healthy sex- and age-matched controls for the occurrence of PAS. Furthermore, we performed neuropsychological testing with the EXIT-25 battery to correlate PAS with neuropsychological deficits, especially frontal lobe dysfunction. Five ALS patients (23%) and none of the controls displayed PAS (p≤0.05). The occurrence of PAS in ALS patients was not correlated with pathologic EXIT-25 scores or subtests for aberrant motor behaviour. We describe for the first time the occurrence of the applause sign in ALS and provide additional evidence that PAS is not specific for Parkinsonian disorders. Although its occurrence has been related to aberrant motor behaviour due to frontal involvement, in our study PAS did not correlate with executive dysfunction as tested by the EXIT-25 test battery, or with subtests of aberrant motor behaviour.

Une étude récente rapporte que des participants souffrant de cauchemars idiopathiques fréquents (CIF) produisaient plus d’erreurs de persévération sur une tâche de fluence verbale (TFV) que des participants contrôle (CTL) (Simor et al., 2012). Cela suggère une dysfonction exécutive chez les participants CIF, soutenant le modèle des cauchemars Affective Network Dysfunction (AND). Notre objectif était de reproduire cette trouvaille auprès d’une cohorte francophone. Des TFV lexicale et sémantique ont été administrées à 23 participants avec CIF et à 16 CTL. Contrairement aux attentes, le groupe CIF n’affichait pas plus de persévération (p=.87). L’absence de reproduction pourrait infirmer la suggestion d’une dysfonction exécutive liée au CIF, toutefois des différences méthodologiques entre les études et les complexités de la TFV peuvent jouer un rôle dans le décalage entre les études. Nous suggérons que l’étude future des déficits exécutifs liés aux CIF emploie des tâches neuropsychologiques non linguistiques empiriquement associées au sommeil paradoxal.

We argue that hyper-systemizing predisposes individuals to show talent, and review evidence that hyper-systemizing is part of the cognitive style of people with autism spectrum conditions (ASC). We then clarify the hyper-systemizing theory, contrasting it to the weak central coherence (WCC) and executive dysfunction (ED) theories. The ED theory has difficulty explaining the existence of talent in ASC. While both hyper-systemizing and WCC theories postulate excellent attention to detail, by itself excellent attention to detail will not produce talent. By contrast, the hyper-systemizing theory argues that the excellent attention to detail is directed towards detecting 'if p, then q' rules (or [input-operation-output] reasoning). Such law-based pattern recognition systems can produce talent in systemizable domains. Finally, we argue that the excellent attention to detail in ASC is itself a consequence of sensory hypersensitivity. We review an experiment from our laboratory demonstrating sensory hypersensitivity detection thresholds in vision. We conclude that the origins of the association between autism and talent begin at the sensory level, include excellent attention to detail and end with hyper-systemizing.

BECTS represents the vast majority of childhood focal epilepsy. Owing to the age peculiarity of children who suffer from this disease, i.e., school-going age of between 6 and 9 years, the condition is often referred to as a school disorder by parents and teachers. Objective: The aim of this study was to evaluate the academic performance of children with BED'S, according to the clinical and electroencephalographic ILAE criteria, and compare the results of neuropsychological tests of language and attention to the frequency of epileptic discharges. Methods: The performances of 40 school children with BED'S were evaluated by applying a school performance test (SBT), neuropsychological tests (WISC and Trail-Making), and language tests (Illinois Test Psycholinguistic Abilities - ITPA - and Staggered Spondaic Word - SSW). The same tests were applied in the control group. Results: Children with BED'S, when compared to those in the control group, showed lower scores in academic performance (SPT), digits and similarities subtests of WISC, auditory processing subtest of SSW, and ITPA - representational and automatic level. The study showed that epileptic discharges did not influence the results. Conclusion: Children with BED'S scored significantly lower scores in tests on academic performance, when compared with those in the control group probably due to executive dysfunction. Parkinson disease (PD) is a movement disorder affecting over one million Americans, and 1% of our population over 60 years of age. Currently, PD has an unknown cause, no predictive biomarker, and no cure, yet there are effective treatments (medicine and surgery) to chronically manage the motor symptoms. But, PD patients also develop cognitive symptoms (e.g., distractibility, executive dysfunction) that remain untreated or may decline as a result of treating the motor symptoms. To address this important issue, I measured covert orienting of attention and overt eye movements in PD patients to assess the patients' ability to automatically detect stimuli in their visual field, to predict and attend to where the stimuli would appear, and to volitionally look somewhere else. ^ PD patients completed the cognitive tasks under multiple treatment conditions, and their performance was compared to healthy adults. PD patients first completed the tasks after they had withdrawn from medication. Their unmedicated performance revealed exaggerated automatic orienting, poor predictability, and weak volitional orienting. PD patients then repeated the tasks while medication was giving its peak benefit. The medication returned automatic covert orienting toward normal but did not improve volitional covert orienting. Several PD patients completed the tasks a third time after receiving surgery (specifically, implantation of stimulating electrodes in a subcortical brain region to alleviate motor symptoms). The stimulation (without medication) returned automatic orienting toward normal, did not change predictability, and further impaired volitional orienting. Taken together, treatments prescribed to alleviate the motor symptoms (a patient's primary concern) only improve some cognitive functions. Future studies may establish criteria to predict which patients are more likely to have cognitive benefit from medication over surgery, or vice versa. ^ I have also hypothesized an anatomical model relating orienting circuitry to abnormal PD circuitry and the therapeutic targets. My results suggest medication is more effective restoring the orienting circuitry than stimulation. Further, automatic and volitional orienting abilities seem to be modulated independently, which differs from an earlier model proposing a dependent, inverse relationship. My results are further discussed in terms of response inhibition, response selection, and the location of the selection. ^

O Transtorno Bipolar (TB) tipo I é uma doença caracterizada por episódios de mania e depressão recorrentes com importante prejuízo do funcionamento global e comprometimento das funções cognitivas. Além disso, sabe-se que o número de episódios de humor patológico ao longo da vida pode também influenciar o funcionamento cognitivo destes sujeitos. Neste cenário, ocorreu a necessidade de se investigar marcadores genéticos para disfunção cognitiva no TB com o objetivo de estudar este fenômeno. Dentre os potenciais genes responsáveis por influenciar a cognição destacam-se os polimorfismos funcionais do fator neurotrófico derivado do cérebro (BDNF), da catecol-O-metiltransferase (COMT), da apolipoproteína-E (APOE) e do canal de cálcio de baixa voltagem subunidade 1-C (CACNA1C). Sabe-se, também, que no TB os marcadores de estresse oxidativo estão aumentados durante todas as fases da doença, entretanto, não é claro qual impacto destes na disfunção cognitiva de indivíduos com TB. O objetivo dessa tese foi avaliar o desempenho cognitivo de pacientes jovens com bipolaridade tipo I e sua associação com o genótipo de BDNF, COMT, APOE e CACNA1C e também com os níveis plasmáticos de oxidação da guanosina (8-OHdG) e citosina (5-Mec) durante os episódios de humor, eutimia e em controles. Para investigar essa associação foram incluídos 116 pacientes (79 em episódio de humor patológico e 37 eutímicos) com diagnóstico de TB tipo I (DSMIV-TR); 97 controles saudáveis foram submetidos à avaliação neuropsicológica e coleta de sangue para extração de DNA visando genotipagem para BDNF (rs6265), COMT (rs4680; rs165599), APOE (rs429358 e rs7412), CACNA1C (rs1006737), 8-OhdG e 5-Mec. A análise dos dados obtidos revelou que pacientes portadores do genótipo Met/Met rs4680/rs165599 do COMT apresentam comprometimento cognitivo mais grave (função executiva, fluência verbal, memória e inteligência) comparado ao genótipo Val/Met ou Val/Val durante episódios maníacos ou mistos. Na mesma direção destes resultados, verificou-se que pacientes portadores do alelo Met rs4680 do COMT apresentam comprometimento do reconhecimento de emoções faciais em episódios de mania e depressão. Nenhum efeito do COMT foi observado em controles. O alelo de risco Met do CACNA1C se associou a um pior comprometimento executivo independente dos sintomas maníacos ou depressivos no TB, porém nenhum efeito se observou nos controles. O alelo Met do BDNF rs6265 ou a presença do alelo 4 da APOE não representa um fator que identifique um grupo com desempenho cognitivo diferenciado durante as fases do TB ou em controles. Sujeitos com TB apresentaram níveis mais elevados de 8-OHdG e tais níveis eram diretamente proporcionais ao número de episódios maníacos ao longo da vida, sugerindo um papel dos episódios hiperdopaminérgicos na oxidação das bases de DNA. Concluiu-se que a genotipagem para COMT e CACNA1C em pacientes com TB pode identificar um grupo de pacientes associados a pior disfunção cognitiva durante as fases maníacas e mistas do TB. Tal dado pode ser um indicador do envolvimento do sistema dopaminérgico e dos canais de cálcio de baixa voltagem na fisiopatologia da disfunção cognitiva no TB e deve ser explorado em outros estudos

A 77-year-old man with 8 year progressive language deterioration in the face of grossly intact memory was followed. No acute or chronic physiological or psychological event was associated with symptom onset. CT revealed small left basal ganglia infarct. Mild atrophy, no lacunar infarcts, mild diffuse periventricular changes registered on MRI. Gait normal but slow. Speech hesitant and sparse. Affect euthymic; neurobehavioral disturbance absent. MMSE 26/30; clock incorrect, concrete. Neuropsychological testing revealed simple attention intact; complex attention, processing speed impaired. Visuospatial copying and delayed recall of copy average with some perseveration. Apraxia absent. Recall mildly impaired. Mild deficits in planning, organization apparent. Patient severely aphasic, dysarthric without paraphasias. Repetition of automatic speech, recitation moderately impaired; prosody intact. Understanding of written language, nonverbal communication abilities, intact. Frontal release signs developed over last 12 months. Repeated cognitive testing revealed mild deterioration across all domains with significant further decrease in expressive, receptive language. Neurobehavioral changes remain absent to date; he remains interested, engaged and independent in basic ADLs. Speech completely deteriorated; gait and movements appreciably slowed. Although signs of frontal/executive dysfunction present, lack of behavioral abnormalities, psychiatric disturbance, personality change argue against focal or progressive frontal impairment or dementia. Relative intactness of memory and comprehension argue against Alzheimer’s disease. Lack of findings on neuroimaging argue against CVA or tumor. It is possible that the small basal ganglia infarct has resulted in a mild lateral prefrontal syndrome. However, the absence of depression as well as the relatively circumscribed language problem suggests otherwise. The progressive, severe nature of language impairments, with relatively minor impairments in attention and memory, argues for a possible diagnosis of primary progressive aphasia.

Frith has argued that people with autism show “weak central coherence,” an unusual bias toward piecemeal rather than configurational processing and a reduction in the normal tendency to process information in context. However, the precise cognitive and neurological mechanisms underlying weak central coherence are still unknown. We propose the hypothesis that the features of autism associated with weak central coherence result from a reduction in the integration of specialized local neural networks in the brain caused by a deficit in temporal binding. The visuoperceptual anomalies associated with weak central coherence may be attributed to a reduction in synchronization of high-frequency gamma activity between local networks processing local features. The failure to utilize context in language processing in autism can be explained in similar terms. Temporal binding deficits could also contribute to executive dysfunction in autism and to some of the deficits in socialization and communication.

Despite the large body of research regarding the role of memory in OCD, the results are described as mixed at best (Hermans et al., 2008). For example, inconsistent findings have been reported with respect to basic capacity, intact verbal, and generally affected visuospatial memory. We suggest that this is due to the traditional pursuit of OCD memory impairment as one of the general capacity and/or domain specificity (visuospatial vs. verbal). In contrast, we conclude from our experiments (i.e., Harkin & Kessler, 2009, 2011; Harkin, Rutherford, & Kessler, 2011) and recent literature (e.g., Greisberg & McKay, 2003) that OCD memory impairment is secondary to executive dysfunction, and more specifically we identify three common factors (EBL: Executive-functioning efficiency, Binding complexity, and memory Load) that we generalize to 58 experimental findings from 46 OCD memory studies. As a result we explain otherwise inconsistent research – e.g., intact vs. deficient verbal memory – that are difficult to reconcile within a capacity or domain specific perspective. We conclude by discussing the relationship between our account and others', which in most cases is complementary rather than contradictory.

Memory deficits and executive dysfunction are highly prevalent among HIV-infected adults. These conditions can affect their quality of life, antiretroviral adherence, and HIV risk behaviors. Several factors have been suggested including the role of genetics in relation to HIV disease progression. This dissertation aimed to determine whether genetic differences in HIV-infected individuals were correlated with impaired memory, cognitive flexibility and executive function and whether cognitive decline moderated alcohol use and sexual transmission risk behaviors among HIV-infected alcohol abusers participating in an NIH-funded clinical trial comparing the efficacy of the adapted Holistic Health Recovery Program (HHRP-A) intervention to a Health Promotion Control (HPC) condition in reducing risk behaviors. ^ A total of 267 individuals were genotyped for polymorphisms in the dopamine and serotonin gene systems. Results yielded significant associations for TPH2, GALM, DRD2 and DRD4 genetic variants with impaired executive function, cognitive flexibility and memory. SNPs TPH2 rs4570625 and DRD2 rs6277 showed a risk association with executive function (odds ratio = 2.5, p = .02; 3.6, p = .001). GALM rs6741892 was associated with impaired memory (odds ratio = 1.9, p = .006). At the six-month follow-up, HHRP-A participants were less likely to report trading sex for food, drugs and money (20.0%) and unprotected insertive or receptive oral (11.6%) or vaginal and/or anal sex (3.2%) than HPC participants (49.4%, p^

Dans les dernières années, les études sur les maladies neurodégénératives telles que la maladie d’Alzheimer (MA) et la maladie de Parkinson sans démence (MP) et avec démence (MPD) ont été nombreuses, mais la différenciation de ces patients sur la base de leur profil cognitif doit être encore améliorée. Effectivement, l’évaluation clinique peut s’avérer difficile en raison du peu de spécificité dans la présentation de leurs déficits neuropsychologiques. Ceci s’explique par la variabilité et le chevauchement des processus cliniques et pathologiques affectant essentiellement les mêmes régions/fonctions, soit celles liées aux lobes temporaux médians (LTM)/Mémoire (fonction LTM/Mémoire) et aux lobes frontaux (LF)/Fonctions exécutives (fonction LF/Exécutive). Toutefois, il existerait une distinction critique au niveau de l’intégrité relative de ces fonctions dans ces maladies neurodégénératives, ce qui permettrait d’identifier des déficits cognitifs spécifiques à la MA, la MP et la MPD. La présente thèse s’inscrit dans cette volonté de caractériser les profils cognitifs propres à la MA, la MP et la MPD, plus précisément par l’étude novatrice de la mémoire de source et des faux souvenirs. Les quatre chapitres qui composent cette thèse servent donc à documenter la nature de ces mécanismes mnésiques, leurs patrons de performance spécifiques dans la MA, la MP et la MPD, et leur sensibilité aux atteintes des fonctions LTM/Mémoire et LF/Exécutive. Ainsi, le Chapitre I démontre la pertinence d’étudier la mémoire de source et les faux souvenirs dans la MA, la MP et la MPD, en décrivant leurs interactions avec les fonctions LTM/Mémoire et LF/Exécutive, toutes les deux atteintes dans ces maladies. Le Chapitre II, présenté sous forme d’article, révèle des déficits en mémoire de source chez des patients MP, mais seulement dans l’une des tâches employées. Également, malgré des atteintes des fonctions LF/Exécutive et LTM/Mémoire, il est démontré que seule la fonction LTM/Mémoire est liée à l’altération de la mémoire de source chez les patients MP. Le Chapitre III, également sous forme d’article, illustre un taux anormal de faux souvenirs chez des patients MA, tandis que chez des patients MP et MPD, il est démontré qu’ils ont un taux de faux souvenirs comparable à celui des participants contrôles. Il est également rapporté que malgré l’atteinte de la fonction LF/Exécutive chez les patients MA, MP et MPD, elle est seulement liée à l’augmentation des faux souvenirs chez les patients MA. Finalement, dans le dernier Chapitre (IV), les résultats obtenus sont discutés dans leur ensemble à la lumière des prédictions et connaissances actuelles, tout en identifiant les limites afin d’orienter les perspectives de recherche.

The Frontal Assessment Batery / A Bateria de Avaliação Frontal (FAB) é um teste neuropsicológico, constituído por seis subtestes, cujo objetivo é avaliar a disfunção executiva global, nomeadamente as funções relacionadas com o lobo frontal, tais como a concetualização, flexibilidade mental, programação motora, sensibilidade à interferência, controlo inibitório e autonomia ambiental frontal. De forma a contribuir para o avanço dos estudos normativos em Portugal, esta dissertação tem como objetivo avaliar as propriedades psicométricas da FAB, numa amostra de adultos da população portuguesa. O protocolo abrangeu a seguinte bateria de testes neuropsicológicos: Bateria de Avaliação Frontal, Figura Complexa de Rey, Matrizes Progressivas de Raven e Teste do Desenho do Relógio. A amostra deste estudo incluiu 376 indivíduos, 155 do sexo masculino e 221 do sexo feminino. Os resultados desta investigação sugerem que a pontuação da FAB é influenciada por algumas variáveis sociodemográficas, designadamente a idade, escolaridade, profissões e região. A análise correlacional mostrou que há apenas uma correlação positiva moderada entre a FAB e as Matrizes Progressivas de Raven. Apesar da consistência interna da FAB ser baixa, existe uma estabilidade temporal moderada. Ao finalizar, consideramos que a FAB reúne os requisitos para se apresentar como uma bateria útil e eficaz, demonstrando um grau razoável de estabilidade temporal, mas fraca consistência interna, sugerindo que a FAB não é indicada para amostra não clínica. / The Frontal Assessment Baterry (FAB) is a neuropsychological test, composed of six subtests, whose aim is to assess the overall executive dysfunction, namely functions related to the frontal lobe, such as conceptualization, mental flexibility, motor programming, sensitivity to interference, inhibitory control and environmental autonomy. In order to contribute to the advancement of normative studies in Portugal, this dissertation aim to evaluate the psychometric properties of the FAB, in an adult sample of the portuguese population. The protocol included the following battery of neuropsychological tests: Frontal Assessment Battery, Complex Figure of Rey, Raven's Progressive Matrices and Clock Drawing Test. The sample this study included 376 individuals, 155 male and 221 female. The results of this investigation suggest that FAB is influenced by some sociodemographic variables, namely age, education, profession and region. The correlational analysis showed that there is only a moderate positive correlation between the FAB and the Raven Progressive Matrices. However, also they found low positive correlations between the FAB and the Complex Figure of Rey, and Clock Drawing Test. Although the FAB has a low internal consistency, there is a moderate temporal stability. Finally, we consider that the FAB gathers the requirements to present itself as a useful and effective battery, demonstrating a reasonable degree of temporal stability, but weaker internal consistency, suggesting that the FAB is not indicate for non-clinical sample.
