The aim of this research was to estimate the skeletal growth and development by observingradiographically the morphological changes of the first, second and third cervical vertebrae, according to the method proposed by Hassel &Farman32 (1995) in patients who were going through the outbreak of pubertal growth. The radiographic inspection of the cervical vertebrae was performed by means of cephalometric radiographs and the outbreak of pubertal growth was identified through the events of ossification of the hand and wrist. For that purpose, we selected thecephalometric radiographs and hand•wrist radiographs of 110 Brazilianleukoderms, male and female,with chronological age ranging from 8 to 14,6 years old for the girls and from 9,5 to 15,4 years old for the boys.The results showed that there was a statistically significant correlation between the maturation indicators of the cervical vertebrae and the patients who were going through the outbreak of pubertal growth. The conclusion was that the radiographic evalution of the changes of the cervical vertebrae, in the cephalometric radiographs, is an alternative parameter, which is reliable and practical in the skeletal evalution, and the it completes the range of information that must be obtained from the patient under orthodontic treatment and that it can, circumstantially, replace other methods of evalution

Skeletal tissues of 49 humpback whales Megaptera novaeangliae that stranded between 2002 and 2011 along the Abrolhos Bank seashore and its adjacent waters in Brazil were studied. Twelve (24.5%) animals presented pathological changes in one or more bones. Degenerative changes and developmental malformations were most frequent (10.2% each), followed by inflammatory/infectious and traumatic lesions (8.2% each). Infectious diseases led to severe lesions of the caudal vertebrae of 2 whales. In one of these individuals, the lesions involved 6 caudal vertebrae, leading to ankylosis of 3 vertebrae. Degenerative changes were observed in the vertebral columns of 3 animals, involving the joints of 13 ribs of 1 individual, and in the humerus of 1 whale. Traumatic lesions, such as osseous callus in the ribs, were observed in 4 animals. In 1 whale, the rib showed severe osteomyelitis, possibly resulting from the infection of multiple fractures. Developmental abnormalities such as spina bifida on 3 cervical vertebrae of 1 whale, fusion of spinal processes on thoracic vertebrae of 1 individual and fusion of the first 2 ribs unilaterally or bilaterally in 4 animals were found. Chronic infectious conditions found in the axial skeleton may have restrained spinal mobility and had detrimental effects on the general health of the animals, contributing to stranding and death. To our knowledge, this is the first systematic study on skeletal lesions in stranded humpback whales.

The authors present a prospective study on the coexistence of spinal injury (SI) and severe traumatic brain injury (TBI) in patients who were involved in traffic accidents and arrived at the Emergency Department of Hospital das Clinicas of the University of Sao Paulo between September 1, 2003 and December 31, 2009. A whole-body computed tomography was the diagnostic method employed in all cases. Both lesions were observed simultaneously in 69 cases (19.4%), predominantly in males (57 individuals, 82.6%). Cranial injuries included epidural hematoma, acute subdural hematoma, brain contusion, ventricular hemorrhage and traumatic subarachnoid hemorrhage. The transverse processes were the most fragile portion of the vertebrae and were more susceptible to fractures. The seventh cervical vertebra was the most commonly affected segment, with 24 cases (34.78%). The distribution of fractures was similar among the other cervical vertebrae, the first four thoracic vertebrae and the lumbar spine. Neurological deficit secondary to SI was detected in eight individuals (11.59%) and two individuals (2.89%) died. Traumatic subarachnoid hemorrhage was the most common intracranial finding (82.6%). Spinal surgery was necessary in 24 patients (34.78%) and brain surgery in 18 (26%). Four patients (5.79%) underwent cranial and spinal surgeries. The authors conclude that it is necessary a judicious assessment of the entire spine of individuals who presented in coma after suffering a brain injury associated to multisystemic trauma and whole-body CT scan may play a major role in this scenario.

Hemangioblastomas of the central nervous system (CNS) are low-grade highly vascularized tumors that may be sporadic or associated with Von Hippel-Lindau disease. Extradural hemangioblastomas are uncommon and those located extra and intradurally are even rarer. This study uses an illustrative case and literature review to discuss the difficulties to consider the correct diagnosis and to select the best surgical approach. A 57 years-old white male patient presented with myelopathy and right C5 radiculopathy. The images showed a lobulated, hourglass shaped, highly enhanced extra/intradural lesion that occupied the spinal canal and widened the C4-C5 right intervertebral foramen. Total resection of the intradural lesion was achieved through a posterior approach, but the extradural part could only be partially removed. Complete improvement was observed after four months of follow-up and the residual tumor has been followed up clinically and radiologically. Even though the preoperative impression was of a spinal schwannoma, the histopathological examination revealed grade I hemangioblastoma as per WHO. Despite their rarity, current complementary exams allow considering the diagnosis of hemangioblastoma preoperatively. That is essential to a better surgical planning in view of the particular surgical features of this lesion.

Cerebrovascular accidents are responsible for killing or disabling more than half a million Americans every year. They are the third leading cause of death in this country. In Germany, the annual stroke incidence reaches 182 cases per 100,000 inhabitants. Stroke there is the fourth leading cause of death. There is a need of finding cost-effective means of decreasing stroke mortality and morbidity. Instruments for early diagnosis are of great humanitarian and economic importance. All possible clinical findings should be taken into account. It is not the demand of this study to present the panoramic radiograph as a screening test method for early diagnosis of atherosclerosis. The aim is to show the potential of this radiograph used in everyday clinical dental practice by the prevalence of radiopaque findings in the carotid region. This study included panoramic dental radiographs of 2,557 patients older than 30 years of age. Fifty-nine percent of the patients were women and 41% were men. The radiographs were adjudged for signs compatible with carotid arterial calcifications appearing as a radiopaque nodular mass adjacent to the cervical vertebrae at or below the intervertebral space C3-4. Of all these radiographs, 4.8% showed radiopaque findings compatible with atherosclerotic lesions. The proportion of women reached 64.8% and that of men reached 35.2%. In accordance to recent literature, the results of this study show that about 5% of the patients show radiological findings compatible with carotid arterial calcifications. Some of these patients at risk for a cerebrovascular accident may be identified in the dentist's office by appropriate review of the panoramic dental radiograph. The suspicion of carotid artery calcifications demands an impetuous referral to an appropriate practitioner who can assist in the control of risk factors and if necessary arrange surgical removal of the carotid arterial plaque. So, the dentist should be aware of this problem and able to make a contribution to stroke prevention.

A two-year-old female Lucerne Hound was presented with a one-week history of signs of progressive neck pain, inappetence, apathy, and an elevated rectal temperature. Findings of magnetic resonance imaging (MRI) were consistent with a foreign body abscess in the epidural space at the level of the first and second cervical vertebrae. A left-sided dorso-lateral atlantoaxial approach was performed, revealing an epidural abscess containing a grass awn. The clinical signs resolved within three days of surgery and the dog made a full recovery. This case report shows that grass awns can migrate to the atlantoaxial region in dogs and MRI findings lead to a suspicion of caudo-cranial migration within the spinal canal.

The Hox gene products are transcription factors involved in specifying regional identity along the anteroposterior body axis. In Drosophila, where these genes are known as HOM-C (Homeotic-complex) genes and where they have been most extensively studied, they are expressed in restricted domains along the anteroposterior axis with different anterior limits. Genetic analysis of a large number of gain- and loss-of-function alleles of these genes has revealed that these genes are important in specifying segmental identity at their anterior limits of expression. Furthermore, there is a functional dominance of posterior genes over anterior genes, such that posterior genes can dominantly specify their developmental programs in spite of the expression of more anterior genes in the same segment. In the mouse, there are four clusters of HOM-C genes, called Hox genes. Thus, there may be up to four genes, called paralogs, that are more highly homologous to each other and to their Drosophila homolog than they are to the other mouse Hox genes. The single mutants for two paralogous genes, hoxa-4 and hoxd-4, presented in this dissertation, are similar to several other mouse Hox mutants in that they show partial, incompletely penetrant homeotic transformations of vertebrae at their anterior limit of expression. These mutants were then bred with hoxb-4 mutants (Ramirez-Solis, et al. 1993) to generate the three possible double mutant combinations as well as the triple mutant. The skeletal phenotypes of these group 4 Hox compound mutants displayed clear alterations in regional identity, such that a nearly complete transformation towards the morphology of the first cervical vertebra occurs. These results suggest a certain degree of functional redundancy among paralogous genes in specifying regional identity. Furthermore, there was a remarkable dose-dependent increase in the number of vertebrae transformed to a first cervical vertebra identity, including the second through the fifth cervical vertebrae in the triple mutant. Thus, these genes are required in a larger anteroposterior domain than is revealed by the single mutant phenotypes alone, such that multiple mutations in these genes result in transformations of vertebrae that are not at their anterior limit of expression. ^

OBJECTIVE: To compare the biomechanical properties of a ventral transarticular lag screw fixation technique, a new dorsal atlantoaxial instability (AAI) clamp, and a new ventral AAI hook plate under sagittal shear loading after transection of the ligaments of the atlantoaxial joint. STUDY DESIGN: Cadaveric biomechanical study. ANIMALS: Canine cadavers (n = 10). MATERIALS AND METHODS: The occipitoatlantoaxial region of Beagles euthanatized for reasons unrelated to the study was prepared leaving only ligamentous structures and the joint capsules between the first 2 cervical vertebrae (C1 and C2). The atlanto-occipital joints were stabilized with 2 transarticular diverging positive threaded K-wires. The occipital bone and the caudal end of C2 were embedded in polymethylmethacrylate and loaded in shear to a force of 50 Newtons. The range of motion (ROM) and neutral zone (NZ) of the atlantoaxial joint were determined after 3 loading cycles with atlantoaxial ligaments intact, after ligament transection, and after fixation with each implant. The testing order of implants was randomly assigned. The implants tested last were subjected to failure testing. RESULTS: All stabilization procedures decreased the ROM and NZ of the atlantoaxial joint compared to transected ligament specimens. Only stabilization with transarticular lag screws and ventral plates produced a significant reduction of ROM compare to intact specimens. CONCLUSION: Fixation with transarticular lag screws and a ventral hook plate was biomechanically similar and provided more rigidity compared to dorsal clamp fixation. Further load cycling to failure tests and clinical studies are required before making clinical recommendations.

Este estudo foi realizado com o propósito de testar a reprodutibilidade, a confiabilidade e a concordância existente entre os métodos de Martins e Sakima (1977) para a radiografia de mão e punho, e Hassel e Farman (1995) e Baccetti et al. (2002) para as vértebras cervicais, quando comparados 2 a 2, e entre todos, conjuntamente. A amostra constou de 72 radiografias, sendo 36 telerradiografias em norma lateral da cabeça e 36 radiografias de mão e punho do lado esquerdo, de 36 indivíduos com Síndrome de Down (SD), sendo 13 do sexo feminino e 23 do masculino na faixa etária entre oito anos e seis meses até 18 anos e sete meses, com média de 13 anos e dez meses. De acordo com os resultados obtidos concluímos que, os índices de maturação avaliados por meio das vértebras cervicais e os centros de ossificação observados nas radiografias de mão e punho foram estatisticamente significativos, obtendo um excelente grau de concordância entre eles, considerados reprodutíveis e confiáveis. Quando comparados onjuntamente, todos os métodos se mostraram estatisticamente significantes com grau de concordância de razoável a boa, sendo considerados confiáveis na aplicação clínica.

Este trabalho teve como finalidade avaliar a fase de crescimento em uma amostra de 49 pacientes, com idades variando de 9 a 15 anos, observando as vértebras cervicais em radiografias laterais, tomadas na posição natural de cabeça (PNC) e com erros de posicionamento, com inclinações de cabeça de 15o para cima (PNC-Alta) e de 15 o para baixo (PNC-Baixa), visando definir o quão importante isto é na precisão da avaliação da fase de crescimento. O teste kappa ponderado mostrou um grau de concordância substancial entre PNC e PNC-Alta e entre PNC e PNC-Baixa. Já o teste não paramétrico de wilcoxon (p<0,05) mostrou diferença estatisticamente significante entre essas comparações. Desta forma, a PNC demonstrou ser mais confiável na avaliação do Índice de Maturação das Vértebras Cervicais (IMVC), porém as telerradiografias com erro de posicionamento avaliadas não tiveram um grau de discordância que indicasse sua não utilização na clínica para predição de crescimento.(AU)

Este estudo teve como objetivo avaliar cefalometricamente as alterações dentoesqueléticas decorrentes do tratamento da maloclusão de Classe II, divisão 1, com o aparelho Forsus®, por meio de Tomografia Computadorizada de Feixe Cônico (TCFC). O grupo avaliado foi composto por 10 pacientes, sendo 7 do sexo masculino e 3 do sexo feminino, com idade média de 16,1 anos, maloclusão com severidade mínima de 1/2 Classe II, trespasse horizontal mínimo de 5 mm, padrão facial meso ou braquifacial. Estes jovens se encontravam no estágio IV ou V de maturação óssea, verificada pelas vértebras cervicais. O tempo de uso do aparelho Forsus foi de 7,16 meses (média), período de avaliação compreendido entre a aquisição da primeira teleradiografia gerada através da TCFC (T1 - pré-Forsus) e da segunda teleradiografia (T2 - pós-Forsus). Para análise estatística foi utilizado o teste-t pareado. Os resultados mostraram um pequeno crescimento mandibular que, juntamente com uma diminuição do SNA levaram a uma melhora da relação maxilomandibular. Houve uma rotação no sentido anti-horário da mandíbula e do plano oclusal no sentido horário. Os incisivos superiores foram retruídos, verticalizados e extruídos e os molares superiores distalizaram por inclinação. Houve vestibularização, protrusão e intrusão dos incisivos inferiores, além de mesialização e extrusão dos molares inferiores. Desta maneira, conclui-se que o aparelho Forsus foi efetivo na correção da maloclusão de Classe II, propiciando maiores alterações dentoalveolares do que esqueléticas.(AU)

Objetivo: El objetivo del presente estudio descriptivo, fue evaluar la posición del hueso hioides en los diferentes patrones esqueletales de Clase I, II y III mediante el trazado cefalométrico del triángulo hioideo propuesto por Bibby y Preston, estableciendo diferencias entre cada clase esqueletal. Materiales y métodos: La muestra consistió en 161 radiografías cefálicas laterales digitales, correspondientes a individuos de ambos sexos (75 hombres y 86 mujeres), entre edades de 9 y 18 años, las mismas que fueron divididas en tres subgrupos (Clase I, clase II y clase III) de acuerdo a los ángulos ANB y APDI. Se determinó la posición anteroposterior, vertical y angular del hueso hioides mediante el trazado cefalométrico del triángulo hioideo siendo el mentón, la tercera vértebra cervical y el hueso hioides las estructuras anatómicas utilizadas para el trazado del mismo. Se obtuvieron medidas estándar para cada clase esqueletal. Resultados: Se observaron diferencias estadísticamente significativas en la medida de H-Rgn entre clase I y II y entre clase II y III (p<0,005). El valor del ángulo del plano hioidal presentó diferencias estadísticamente significativas entre clase I y III y entre clase II y III (p<0,005). Se evidenciaron diferencias estadísticamente significativas entre hombres y mujeres con clase I esqueletal en la medida H-Rgn (p<0,005). Conclusiones: La posición del hueso hioides varía en los diferentes patrones esqueletales. Sin embargo, su posición en relación a la columna cervical presenta menos variabilidad que su relación con la mandíbula

Descreveu-se a anatomia da região cervical de equinos adultos, com base em imagens obtidas no exame de tomografia computadorizada helicoidal realizado em peças anatômicas de equinos adultos. A tomografia computadorizada foi o método de imagem diagnóstica de escolha e possibilita as reconstruções de imagens tridimensionais e em outros planos anatômicos, como sagital e coronal. Todas as imagens foram adquiridas e avaliadas em filtro e janela para tecido ósseo. Observaram-se diferenças anatômicas e as particularidades normais das vértebras, principalmente da região occipitoatlantoaxial, a qual apresenta maior incidência de alterações.
