Staphylococcus aureus is a major human pathogen, first recognized as a leading cause of hospital-acquired infections. Community-associated S. aureus (CA-SA) pose a greater threat due to increase in severity of infection and disease among children and healthy adults. CA-SA strains in India are genetically diverse, among which is the sequence type (ST) 772, which has now spread to Australia, Europe and Japan. Towards understanding the genetic characteristics of ST772, we obtained draft genome sequences of five relevant clinical isolates and studied the properties of their PVL-carrying prophages, whose presence is a defining hallmark of CA-SA. We show that this is a novel prophage, which carries the structural genes of the hlb-carrying prophage and includes the sea enterotoxin. This architecture probably emerged early within the ST772 lineage, at least in India. The sea gene, unique to ST772 PVL, despite having promoter sequence characteristics typical of low expression, appears to be highly expressed during early phase of growth in laboratory conditions. We speculate that this might be a consequence of its novel sequence context. The crippled nature of the hlb-converting prophage in ST772. suggests that widespread mobility of the sea enterotoxin might be a selective force behind its `transfer' to the PVL prophage. Wild type ST772 strains induced strong proliferative responses as well as high cytotoxic activity against neutrophils, likely mediated by superantigen SEA and the PVL toxin respectively. Both proliferation and cytotoxicity were markedly reduced in a cured ST772 strain indicating the impact of the phage on virulence. The presence of SEA alongside he genes for the immune system-modulating PVL toxin may contribute to the success and virulence of ST772.

A infecção pulmonar de etiologia bacteriana é um dos principais problemas que levam a morbi-mortalidade na fibrose cística (FC). Staphylococcus aureus se destaca como um dos micro-organismos mais frequentes e com um agravante para a terapêutica quando se apresentam resistentes à oxacilina (MRSA). Amostras MRSA podem ser classificadas tanto genotipicamente quanto fenotipicamente em MRSA adquiridas na comunidade (CA-MRSA) ou adquiridas no hospital (HA-MRSA). Fenotipicamente, essa classificação é muito controversa, podendo se basear em critérios epidemiológicos ou ainda pelo perfil de susceptibilidade aos antimicrobianos. Por outro lado, a classificação genotípica consiste na determinação dos cassetes cromossômicos (SCCmec), local de inserção do gene mecA (que confere resistência a meticilina). Atualmente são reconhecidos 11 tipos de SCCmec, sendo os de tipo I ao III e VIII relacionados ao genótipo HA-MRSA e IV ao XI ao genótipo CA-MRSA. Classicamente CA-MRSA é capaz de produzir a toxina Panton-Valentine leukocidin (PVL), codificada pelos genes luk-S e luk-F que está associada à pneumonia necrotizante e infecções de tecidos moles em pacientes com FC com quadros de exacerbação pulmonar. No Brasil, raros são os trabalhos envolvendo caracterização de SCCmec em amostras de pacientes com FC. Diante disso, este estudo teve como objetivo principal a caracterização dos tipos de SCCmec e ainda a determinação do perfil de susceptibilidade a antimicrobianos em uma população de MRSA recuperada de pacientes com FC assistidos em dois centros de tratamento no Rio de Janeiro, Hospital Universitário Pedro Ernesto (HUPE) e Instituto Fernandes Figueira (IFF). Foram estudadas 108 amostras de MRSA isoladas do período de 2008 a 2010, sendo 94 oriundas de 28 pacientes adultos atendidos no IFF e 14 de 2 pacientes adultos atendidos no HUPE. Foram encontradas altas taxas de resistência para os antimicrobianos oxacilina, cefoxitina e eritromicina. Todas as amostras foram sensíveis à vancomicina e a linezolida quando determinada as Concentrações Inibitórias Mínimas (CIM). Através da técnica de PCR foi possível a tipificação dos SCCmec em 82,4% das amostras, sendo 64% destas compatíveis ao genótipo CA-MRSA. Não houve diferença estatística nas taxas de susceptibilidade aos antimicrobianos entre as amostras CA-MRSA e HA-MRSA. Foram encontrados os SCCmec dos tipos I, III, IV e V, sendo os tipos I e IV os mais frequentes. O gene que codifica a toxina PVL foi encontrado em 34,2% das amostras e foi observado em amostras CA-MRSA e HA-MRSA. Nosso estudo se destaca por apresentar um alto percentual de amostras CA-MRSA e ainda por ser o primeiro do país a detectar a presença do gene que codifica a toxina PVL em pacientes com FC. Além disso, de forma inédita na literatura, encontramos o gene luk-S, em amostras classificadas como HA-MRSA em pacientes com FC. Os poucos estudos nacionais, bem como as diferenças encontradas entre trabalhos, refletem a necessidade de conhecimento mais aprimorado do MRSA envolvido nas infecções pulmonares dos pacientes com FC.

Staphylococcus aureus resistente à meticilina (MRSA) é um dos principais microrganismos envolvidos nas Infecções relacionadas à Assistência à Saúde (IrAS). Porém, um clone de MRSA, o CA-MRSA, emergiu na comunidade e atualmente vem sendo agente de IrAS. O objetivo desta dissertação é avaliar fenotípica e genotipicamente 111 amostras de Staphylococcus aureus resistentes à meticilina e sensíveis a antibióticos não ß-lactâmicos de pacientes atendidos em cinco hospitais no município do Rio de Janeiro. Utilizando os critérios padronizados pelo CLSI 2012, foram determinadas as susceptibilidades a 11 antimicrobianos pelo método de disco difusão em ágar e concentração inibitória mínima para vancomicina e oxacilina pelo método da microdiluição em caldo. A multirresistência (resistência a 3 ou mais antimicrobianos não ß-lactâmicos) foi observada em 31,5% das amostras, sendo que 53,2% apresentaram resistência ao antimicrobiano clindamicina, uma das opções para o tratamento empírico das infecções de pele/tecidos moles. 86,4% apresentaram concentração inibitória mínima (CIM) para vancomicina ≥ 1,0 g/mL ou seja, elevado percentual de amostras associadas ao fenômeno MIC creep, o qual está associado ao insucesso na terapia antimicrobiana anti-MRSA. Não foi observado até o momento nenhuma amostra com CIM ≥ 4cg/mL para vancomicina, entretanto, já há resistência à linezolida em quatro hospitais do estudo. A tipificação do SCCmec nos permitiu classificar 4,5% das amostras em HA-MRSA e 86,5% em CA-MRSA, nas quais a resistência heterogênea típica à oxacilina foi observada em 57,2%. A toxina de Panton-Valentine (PVL) foi identificada pela metodologia de PCR em 28% das amostras com genótipo CA-MRSA. Os fatores de riscos clássicos, da literatura, relacionados à infecção por HA-MRSA foram também observados nos pacientes com infecção por CA-MRSA portadoras de SCCmec IV e V. No intuito de verificar a existência de similaridades genéticas ou a presença de clone predominante entre as amostras dos cinco hospitais, foi realizada a técnica de eletroforese em gel sob campo pulsado (PFGE) e observou-se diversidade genética assim como a presença de amostras com padrões similares aos clones OSPC (18,5%) e USA400. Não foram encontradas amostras com padrões de eletroforese similares aos clones USA300, USA800 e CEB. É essencial a vigilância da resistência aos antimicrobianos não ß-lactâmicos no CA-MRSA, em especial à vancomicina. A mudança na epidemiologia deste microrganismo vem impactando os padrões característicos dos genótipos limitando os critérios de diferenciação entre eles. Neste contexto, as técnicas moleculares atuam como excelentes ferramentas de caracterização. O conhecimento do patógeno auxilia na elaboração e implementação de medidas preventivas, contribuindo para o controle da doença tanto no ambiente hospitalar quanto na comunidade.

Staphylococcus aureus resistente à meticilina (MRSA) é um importante patógeno pulmonar em pacientes com fibrose cística (FC). Caracteriza-se pela resistência a todos os β-lactâmicos, devido a presença do elemento genético móvel SCCmec o qual abriga o gene mecA. Além disso, é reconhecido por vários fatores de virulência o qual destacamos a toxina Panton-Valentine Leukocidin (PVL), uma citolisina formadora de poros na célula hospedeira, e por apresentar diversos clones epidêmicos envolvidos em surtos hospitalares. O objetivo desse estudo foi caracterizar a epidemiologia de MRSA, isolados de pacientes com FC referente a dois centros de referência no Rio de Janeiro a partir da aplicação de técnicas fenotípicas e genotípicas. Um total de 57 amostras de MRSA foi submetido ao teste de difusão em ágar para 11 antimicrobianos a fim de avaliar perfil de resistência, com aplicação da técnica da PCR foi tipificado o SCCmec e investigado a presença do gene LukS-PV responsável pela codificação da toxina PVL com intuito de estabelecer uma melhor caracterização epidemiológica dos clones identificados pela técnica do MLST (Multilocus Sequence Typing). Os antimicrobianos não β-lactâmicos apresentaram um percentual de resistência abaixo de 50%, em que destacamos a eritromicina com o maior percentual 45,6% e quanto ao perfil de resistência 24,6% foram multirresistentes. Com exceção do SCCmec II, os outros tipos foram encontrados (I, III, IV e V) com os respectivos percentuais de 22,8% (n=13), 7,1% (n=4), 61,4% (n=35) e 3,5% (n=2) e apenas 5,3% (n=3) das amostras não foram caracterizadas, não há dados da prevalência do SCCmec IV. Vinte (35,1%) amostras apresentaram produtos de amplificação compatível com a presença do gene lukS, aproximadamente metade dessas amostras (55%) estava correlacionada ao SCCmec IV. Com a análise do MLST, obtivemos os STs 1 (n=1, 1,7%), 5 (n=28, 49,1%), 30 (n=11, 19,3%), 72 (n=1, 1,7%), 398 (n=1, 1,7%), 1635 (n=7, 12,3%), 1661 (n=2, 3,5%), 239 (n=5, 8,8%), e ainda identificamos um novo ST (2732) presente em 1 amostra. A partir de uma análise associativa entre o MLST e o SCCmec foi possível observar a presença de linhagens características de clones epidêmicos, como o UK-EMRSA-3 (ST5, SCCmec I), USA 800/pediátrico (ST5, SCCmec IV), Oceania Southwest Pacific Clone - OSPC (ST30, SCCmec IV) e Brazilian Epidemic Clone - BEC (ST239, SCCmec III). Em conclusão este estudo é o primeiro a caracterizar linhagens epidêmicas de MRSA nos centros de atendimento a pacientes com FC no Rio de Janeiro, sendo necessário um monitoramento constante a fim de evitar a disseminação desses clones.

Recent studies in mammals have revealed that the cyanobacterial toxin MC-LR suppresses immune functions. Nevertheless, immunotoxic effects of microcystins have been little studied in fish. In this paper, we present the profiles of the immune modulation of MC-LR in grass carp, and quantitative real-time PCR methodology was developed for the measurement of relative transcription changes of six immune-related genes in the spleen and head kidney of the grass carp Ctenopharyngodon idella, which were intraperitoneally injected with 50 mu g MC-LR center dot kg(-1) body weight in a three-week period. This study was focused exclusively on gene transcription level changes at different time points after MC-LR exposure, so, only one dose was given. The investigated genes were interleukin-1 beta (IL-1 beta), tumor necrosis factor-alpha (TNF-alpha), type I interferon (Type I IFN), peptidoglycan recognition protein-L (PGRP-L), immunoglobulin M (IgM) and major histocompatibility complex class I (MHC-I) genes. The results demonstrated that the transcription levels of the TNF-alpha, type I IFN, and PGRP-L genes in the spleen and head kidney were significantly low at all time points, and those of IL-1 beta were significantly low in the head kidney at different time points. In addition, IgM and MHC-I transcription levels were only significantly low in the spleen and head kidney at 21 d postinjection. The changes in the transcription levels of immune-related genes induced by MC-LR confirmed its effect on inhibiting immune function at the transcription level.

A novel aflatoxin B(1) bioassay was created by introducing a Lipomyces kononenkoae alpha-amylase gene into a strain of S. cerevisiae capable of expressing the human cytochrome P450 3A4 (CYP3A4), and the cognate human CYP450 reductase. This strain and a dextranase-expressing strain were used in the development of a microtitre plate mycotoxin bioassay, which employed methanol as the solvent and polymyxin B nonapeptide as a permeation enhancer. Stable co-expression of the CYP3A4 gene system and of the dextranase and amylase genes in the two bioassay strains was demonstrated. The bioassay signalled toxicity as inhibition of secreted carbohydrase activity, using sensitive fluorimetric assays. The amylase-expressing strain could detect aflatoxin B(1) at 2 ng/ml, and was more sensitive than the dextranase-expressing strain. Aflatoxin G(1) could be detected at 2 microg/ml, and the trichothecene mycotoxin T-2 toxin was detectable at 100 ng/ml.

The venoms of buthid scorpions are known to contain basic, single-chain protein toxins (alpha toxins) consisting of 60–70 amino acid residues that are tightly folded by four disulfide bridges. Here we describe isolation and sequencing of three novel putative alpha toxins (AamH1-3) from the venom of the North African scorpion, Androctonus amoreuxi, and subsequent cloning of their precursor cDNAs from the same sample of venom. This experimental approach can expedite functional genomic analyses of the protein toxins from this group of venomous animals and does not require specimen sacrifice for cloning of protein toxin precursor cDNAs.

Many zeranol immunoassay test kits cross-react with toxins formed by naturally occurring Fusarium spp. fungi, leading to false-positive screening results. This paper describes the evaluation and application of recently published, dry reagent time-resolved fluoroimmunoassays (TR-FIA) for zeranol and the toxin alpha-zearalenol. A ring test of bovine urine fortified with zeranol and/or alpha-zearalenol in four European Union National Reference Laboratories demonstrated that the TR-FIA tests were accurate and robust. The alpha-zearalenol TR-FIA satisfactorily quantified alpha-zearalenol in urine fortified at 10-30 ng ml(-1) . The specificity-enhanced zeranol TR-FIA accurately quantified zeranol in the range 2-5 ng ml(-1) and gave no false-positive results in blank urine, even in the presence of 30 ng ml(-1) alpha-zearalenol. Zeranol TR-FIA specificity was demonstrated further by analysing incurred zeranol-free urine samples containing natural Fusarium spp. toxins. The TR-FIA yielded no false-positive results in the presence of up to 22 ng ml(-1) toxins. The performance of four commercially available zeranol immunoassay test kits was more variable. Three kits produced many false-positive results. One kit produced only one potential false-positive using a protocol that was longer than that of the TR-FIA. These TR-FIAs will be valuable tools to develop inspection criteria to distinguish illegal zeranol abuse from contamination arising from in vivo metabolism of Fusarium spp. toxins.

Introduction: Human alpha defensins are a family of neutrophil-derived antimicrobial peptides also known as human neutrophil peptides (HNPs). The defensin family of peptides are characterised by six invariant cysteine residues forming three disulphide bridges. The formation of the correct disulphide pairs complicates the synthesis of full length human alpha defensin and limits its therapeutic potential as an antimicrobial peptide. Objectives: The aim of this study was to determine whether truncated alpha defensins displayed antimicrobial activity against a range of micro-organisms including oral pathogens. Methods: Engineered peptides were synthesised by solid-phase methods using standard Fmoc chemistry. Antibacterial assays were performed using a previously described ultra sensitive radial diffusion method. A total of five engineered defensin peptides and full length alpha defensin were tested for their sensitivity against eight micro-organisms, including Gram negative bacteria, Gram positive bacteria and fungal pathogens Results: Antimicrobial activity was identified as clear zones around peptide-containing wells. Zone diameters were used to calculate minimum inhibitory concentrations (MICs) for each peptide. There was considerable variability in the susceptibility of the micro-organisms to the truncated analogues. Bacillus subtilis and Enterococcus faecalis were sensitive to the majority of the engineered peptides whereas Staphylococcus aureus, Escherichia coli and Candida albicans displayed resistance (defined as an MIC of greater than 250 ug/ml) to the truncated defensins. Of the five engineered peptides synthesised, the 2-aminobenzoic acid (Abz)-containing analogues based on the C-terminal sequence of alpha defensin displayed MIC values closest to that of the full length defensin in 5 out of 8 micro-organisms studied. Conclusion: This study demonstrates that truncated alpha defensins display variable antimicrobial activity against a range of micro-organisms, including oral pathogens. The generation of truncated defensins without disulphide bridges simplifies their synthesis and increases their therapeutic potential.

OBJECTIVES: In patients with septic shock, circulating monocytes become refractory to stimulation with microbial products. Whether this hyporesponsive state is induced by infection or is related to shock is unknown. To address this question, we measured TNF alpha production by monocytes or by whole blood obtained from healthy volunteers (controls), from patients with septic shock, from patients with severe infection (bacterial pneumonia) without shock, and from patients with cardiogenic shock without infection. MEASUREMENTS: The numbers of circulating monocytes, of CD14+ monocytes, and the expression of monocyte CD14 and the LPS receptor, were assessed by flow cytometry. Monocytes or whole blood were stimulated with lipopolysaccharide endotoxin (LPS), heat-killed Escherichia coli or Staphylococcus aureus, and TNF alpha production was measured by bioassay. RESULTS: The number of circulating monocytes, of CD14+ monocytes, and the monocyte CD14 expression were significantly lower in patients with septic shock than in controls, in patients with bacterial pneumonia or in those with cardiogenic shock (p < 0.001). Monocytes or whole blood of patients with septic shock exhibited a profound deficiency of TNF alpha production in response to all stimuli (p < 0.05 compared to controls). Whole blood of patients with cardiogenic shock also exhibited this defect (p < 0.05 compared to controls), although to a lesser extent, despite normal monocyte counts and normal CD14 expression. CONCLUSIONS: Unlike patients with bacterial pneumonia, patients with septic or cardiogenic shock display profoundly defective TNF alpha production in response to a broad range of infectious stimuli. Thus, down-regulation of cytokine production appears to occur in patients with systemic, but not localised, albeit severe, infections and also in patients with non-infectious circulatory failure. Whilst depletion of monocytes and reduced monocyte CD14 expression are likely to be critical components of the hyporesponsiveness observed in patients with septic shock, other as yet unidentified factors are at work in this group and in patients with cardiogenic shock.

L’entérotoxine B staphylococcique (SEB) est une toxine entérique hautement résistante à la chaleur et est responsable de plus de 50 % des cas d’intoxication d’origine alimentaire par une entérotoxine. L’objectif principal de ce projet de maîtrise est de développer et valider une méthode basée sur des nouvelles stratégies analytiques permettant la détection et la quantification de SEB dans les matrices alimentaires. Une carte de peptides tryptiques a été produite et 3 peptides tryptiques spécifiques ont été sélectionnés pour servir de peptides témoins à partir des 9 fragments protéolytiques identifiés (couverture de 35 % de la séquence). L’anhydride acétique et la forme deutérée furent utilisés afin de synthétiser des peptides standards marqués avec un isotope léger et lourd. La combinaison de mélanges des deux isotopes à des concentrations molaires différentes fut utilisée afin d’établir la linéarité et les résultats ont démontré que les mesures faites par dilution isotopique combinée au CL-SM/SM respectaient les critères généralement reconnus d’épreuves biologiques avec des valeurs de pente près de 1, des valeurs de R2 supérieure à 0,98 et des coefficients de variation (CV%) inférieurs à 8 %. La précision et l’exactitude de la méthode ont été évaluées à l’aide d’échantillons d’homogénat de viande de poulet dans lesquels SEB a été introduite. SEB a été enrichie à 0,2, 1 et 2 pmol/g. Les résultats analytiques révèlent que la méthode procure une plage d’exactitude de 84,9 à 91,1 %. Dans l’ensemble, les résultats présentés dans ce mémoire démontrent que les méthodes protéomiques peuvent être utilisées efficacement pour détecter et quantifier SEB dans les matrices alimentaires. Les cellules myéloïdes incluant les monocytes, les macrophages et les cellules dendritiques (DCs, dendritic cells) contribuent à la pathogénèse de l’infection à VIH-1 en participant à la dissémination du virus, mais également en représentant des réservoirs viraux potentiels. Leurs fonctions sont exploitées par le VIH-1 afin d’assurer sa propagation à travers l’organisme. Notamment, une infection à VIH-1 est associée à une altération de la présentation antigénique et la perte de lymphocytes T CD4+ spécifiques à des antigènes. L’autophagie est un processus catabolique universel impliqué dans la régulation de la présentation antigénique subséquente à la neutralisation/destruction du pathogène. Des études récentes suggèrent que le VIH-1 altère le mécanisme d’autophagie afin d’assurer sa survie. Le premier volet de ce projet de maîtrise a visé la caractérisation des effets du VIH-1 sur l’autophagie dans les DCs dérivées de monocytes circulants (MDDC, monocyte-derived dendritic cells) et l’identification des stratégies thérapeutiques pour contrecarrer ces effets. Les objectifs spécifiques ont été de : (i) caractériser l’expression de marqueurs de maturation sur des MDDC exposées au VIH-1 in vitro, (ii) quantifier l’expression des protéines liées à la régulation positive (i.e., ATG5, LC3, p62) et négative (i.e., mTOR) de l’autophagie dans les MDDC exposées au VIH, (iii) déterminer le rôle de l’autophagie dans la trans infection du VIH-1 aux lymphocytes T CD4+ et (iv) explorer l’impact de l’autophagie sur la présentation antigénique par les MDDC infectées à VIH-1 in vitro. Nos résultats démontrent qu’une exposition des MDDC au VIH-1 in vitro altère dramatiquement leur maturation et leur habileté à induire la prolifération des cellules T autologues en réponse à Staphylococcus aureus et Cytomegalovirus (CMV) mais pas la réponse induite par Staphylococcal enterotoxin B (SEB). Nous démontrons que l’exposition des MDDC au VIH s’associe à une augmentation de l’expression de la protéine mTOR totale et de sa forme phosphorylée (phospho-mTOR) et de la protéine p62. Le traitement des MDDC à la rapamycine diminue l’expression de mTOR et réduit la capacité de trans infection du VIH-1 par les MDDC, sans toutefois restaurer leur potentiel immunogène. En effet, nous observons que la rapamycine réduit l’expression de CD83 par les MDDC et augmente l’expression de CCR7, indiquant ainsi que l’effet immunosuppresseur documenté de la rapamycine est associé à une défaillance de maturation des MDDC. Le second volet de ce projet de recherche s’est intéressé à la contribution des cellules myéloïdes à la persistance virale chez les sujets infectés par le VIH-1 sous thérapie antirétrovirale. Les objectifs spécifiques ont été : (i) d’évaluer la présence de différentes formes d’ADN viral dans les monocytes circulants de patients infectés par le VIH-1 et (ii) de mesurer l’intégration et la réplication virale dans des macrophages dérivés de monocytes (MDM) de patients infectés sous ART. Nos résultats indiquent que les monocytes portent des formes précoces de transcription virale inverse (ADN du VIH RU5) et que, malgré une charge virale plasmatique indétectable sous ART, les MDM supportent la réplication virale. Ces données très préliminaires apportent des évidences en faveur de la contribution des cellules myéloïdes à la persistance virale sous ART et représentent une ouverture pour un projet de recherche plus complexe dans le futur. En somme, nos résultats démontrent que le VIH-1 altère le potentiel immunogène des MDDC et que la rapamycine peut être employée pour limiter la trans infection des lymphocytes T CD4+ par les MDDC. Néanmoins, l’incapacité de la rapamycine à rétablir le potentiel immunogène des MDDC incite à identifier de nouvelles stratégies manipulant l’autophagie pour une restauration optimale de la compétence immunitaire chez les sujets infectés à VIH-1. Les cellules myéloïdes jouent un rôle primordial dans la dissémination et la persistance virale et doivent donc être ciblées par les stratégies actuelles d’éradication des réservoirs du VIH sous ART.

A detailed study was made on the microbial quality, with special reference to food safety, of the fish and fishery products in the retail trade in Cochin and around. Also, farmed molluscan shellfishes like mussels and oysters were investigated for the microbial quality including the presence of pathogenic bacteria. Special stress has been given to monitor the incidence of coagulase positive as well as coagulase negative Staphylococcus in these products and their relative incidence have been recorded.In the next part, the investigation was centered mainly on toxigenic S.aureus. This is because among the Gram positive toxigenic bacteria, the Saureus with potential to produce thermostable enterotoxins are more relavent in food safety conceming seafoods in comparison with the Gram-negative pathogens like Salmonella and V.cholerae.The incidence, toxigenic potential and conditions of toxin production by S.aureus have been investigated in detail. An attempt has also been made to relate the toxigenisis with the presence of the concerned toxigenic genes in the genomes of S. aureus strains.

Die vorliegende Arbeit liefert erstmals einen umfassenden Überblick über die molekulare Epidemiologie von Methicillin resistenten Staphylococcus aureus (MRSA) eines nordhessischen Krankenhauses inklusive seines Umfeldes und deren Entwicklung in einem Zeitraum von fünf Jahren. Von besonderer Bedeutung ist, dass die MRSA-Stämme hierfür nicht nur anhand ihrer SCCmec-Region (staphylococcal cassette chromosome) typisiert wurden, sondern eine weitergehende Charakterisierung auf Grund der Bestimmung des Vorkommens von Antibiotikaresistenz- und Toxingenen, sowie Plasmiden erfolgte. Dabei wurde ein neuer SCCmec-Typ entdeckt und charakterisiert und weitere noch unbekannte SCCmec-Elemente beschrieben. Bei der Charakterisierung der MRSA-Kollektive konnten bzgl. aller untersuchten Eigenschaften im Laufe der Zeit signifikante Veränderungen beobachtet werden. Am deutlichsten waren diese Unterschiede zwischen dem ältesten Kollektiv aus 1999 und allen nachfolgenden Kollektiven. Die Kollektive aus 2001, 2002, 2003 und 2004 zeigten untereinander größere Ähnlichkeiten, aber dennoch gleichzeitig eine tendenziell divergente Entwicklung einzelner Eigenschaften. Besonders auffallend war das dominante Auftreten von SCCmecIV mit 63-87% der Isolate eines Kollektivs ab 2001, gegenüber 16% in 1999. Weiterhin erfolgte eine markante Veränderung im Vorkommen einzelner Antibiotikaresistenzgene von 1999 bis 2004. So waren aacA-aphD und ermA bei MRSA aus 1999 mit 84% bzw. 90% deutlich häufiger als in allen Kollektiven der folgenden Jahre (aacA-aphD: max. 32%, ermA: max. 40%). Wohingegen ermC ein stets zunehmendes Vorkommen von 3% auf 67% über den Untersuchungszeitraum zeigte. Unkontinuierliches aber statistisch relevant vermehrtes Auftreten von tetM konnte bei Isolaten aus 1999 (40%) und 2004 (74%) nachgewiesen werden. Auch bei Toxingenen zeigten sich deutliche Unterschiede in der zeitlichen Verteilung. Ab 2001 zeigten alle Isolate wesentlich höhere Anteile an sec, seg und sei verglichen mit den MRSA aus 1999. So konnte sec im Kollektiv aus 1999 gar nicht nachgewiesen werden, in denen der Folgejahre mit 54-77%. Die Werte für seg und sei stiegen von 48% bzw. 41% in 1999 kontinuierlich auf über 90% in 2004. Die Häufigkeit von MRSA sowohl mit mehreren Resistenzgenen als auch die mit mehreren Toxingenen nahm im Laufe der Zeit zu und korrelierte mit dem Vorkommen von Plasmiden. Bezüglich seiner Korrelation mit den vorkommenden Plasmiden zeigte SCCmecIV im Erhebungszeitraum besonders deutlich eine Veränderung. So nahm über den Zeitraum der Beobachtung die Anzahl der Stämme die zusätzlich zu einem großen Plasmid ein weiteres kleines Plasmid besaßen signifikant zu. Auch beim Vergleich der SCCmec-Typen der MRSA-Isolate konnten Unterschiede bzgl. aller weiteren untersuchten Eigenschaften dargestellt werden. So zeigten z.B. alle SCCmecIIIA das sea-Gen, während dies bei allen anderen in der vorliegenden Arbeit untersuchten SCCmec-Typen nur vereinzelt vorkam. SCCmecII-Stämme wiesen sowohl die meisten Antibiotikaresistenz- als auch Toxingene auf. Es wurde ferner gezeigt, dass Stämme mit vielen Resistenzgenen auch eine hohe Anzahl Toxingene besaßen und dies im Zusammenhang mit einem erhöhten Plasmidgehalt stehen könnte. Aus den MRSA-Kollektiven isolierte Plasmide konnten aufgrund von Restriktionsanalysen als verwandt zu β-Laktamase-Plasmiden des Grundtyps pI524 und pI258 beschrieben werden. Der in vorliegender Arbeit gezeigte Zusammenhang zwischen der Anzahl von direct repeat units (dru) in der Hypervariablen Region (HVR) und dem SCCmec-Typ half den Unterschied zwischen SCCmecIV und SCCmecIVA, sowie die Sonderstellung des in vorliegender Arbeit erstmalig beschriebenen SCCmecIA/II darzustellen. Nicht alle Isolate konnten einem bekannten SCCmec-Typ zugeordnet werden, es handelt sich bei diesen Ausnahmen um weitere noch unbekannte und hier erstmalig beschriebene SCCmec-Typen. Aufgrund der vorliegenden Arbeit konnte ein neuer SCCmec-Typ definiert werden, namentlich der Typ SCCmecIA/II, der seit 1999 in der Region gehäuft vorkommt Die vorliegenden Untersuchungen zeigten somit, dass die Epidemiologie von MRSA der Region Nordhessen trotz bestehender Gemeinsamkeiten zur MRSA-Situation in ganz Deutschland auch Besonderheiten aufweist. Diese nun zu kennen kann einen Beitrag zur gezielten Verbesserung bisheriger Maßnahmen zur Ausbreitungskontrolle von MRSA in der nordhessischen Region leisten.
