BACKGROUND: Several studies have shown that adherence to the Mediterranean Diet measured by using the Mediterranean diet score (MDS) is associated with lower obesity risk. The newly proposed Nordic Diet could hold similar beneficial effects. Because of the increasing focus on the interaction between diet and genetic predisposition to adiposity, studies should consider both diet and genetics. OBJECTIVE: We investigated whether FTO rs9939609 and TCF7L2 rs7903146 modified the association between the MDS and Nordic diet score (NDS) and changes in weight (Δweight), waist circumference (ΔWC), and waist circumference adjusted for body mass index (BMI) (ΔWCBMI). DESIGN: We conducted a case-cohort study with a median follow-up of 6.8 y that included 11,048 participants from 5 European countries; 5552 of these subjects were cases defined as individuals with the greatest degree of unexplained weight gain during follow-up. A randomly selected subcohort included 6548 participants, including 5496 noncases. Cases and noncases were compared in analyses by using logistic regression. Continuous traits (ie, Δweight, ΔWC, and ΔWCBMI) were analyzed by using linear regression models in the random subcohort. Interactions were tested by including interaction terms in models. RESULTS: A higher MDS was significantly inversely associated with case status (OR: 0.98; 95% CI: 0.96, 1.00), ΔWC (β = -0.010 cm/y; 95% CI: -0.020, -0.001 cm/y), and ΔWCBMI (β = -0.008; 95% CI:-0.015, -0.001) per 1-point increment but not Δweight (P = 0.53). The NDS was not significantly associated with any outcome. There was a borderline significant interaction between the MDS and TCF7L2 rs7903146 on weight gain (P = 0.05), which suggested a beneficial effect of the MDS only in subjects who carried 1 or 2 risk alleles. FTO did not modify observed associations. CONCLUSIONS: A high MDS is associated with a lower ΔWC and ΔWCBMI, regardless of FTO and TCF7L2 risk alleles. For Δweight, findings were less clear, but the effect may depend on the TCF7L2 rs7903146 variant. The NDS was not associated with anthropometric changes during follow-up.

This systematic review discusses data on the dietary intake of preschool children living in the Mediterranean countries of the European Union, including the comparison with a Mediterranean-like diet and the association with nutritional status. Specifically, data from the multinational European Identification and Prevention on Dietary and life style induced health effects in children and infants (IDEFICS) study and national studies, such as the Estudo do Padrão Alimentar e de Crescimento Infantil (EPACI) study and Geração XXI cohort in Portugal, ALimentando la SAlud del MAñana (ALSALMA) study in Spain, Étude des Déterminants pré-et postnatals précoces du développement et de la santé de l'ENfant (EDEN) cohort in France, Nutrintake 636 study in Italy, and Growth, Exercise and Nutrition Epidemiological Study in preSchoolers (GENESIS) cohort in Greece, were analyzed. In the majority of countries, young children consumed fruit and vegetables quite frequently, but also consumed sugared beverages and snacks. High energy and high protein intakes mainly from dairy products were found in the majority of countries. The majority of children also consumed excessive sodium intake. Early high prevalence of overweight and obesity was found, and both early consumption of energy-dense foods and overweight seemed to track across toddler and preschool ages. Most children living in the analyzed countries showed low adherence to a Mediterranean-like diet, which in turn was associated with being overweight/obese. Unhealthier diets were associated with lower maternal educational level and parental unemployment. Programs promoting adherence of young children to the traditional Mediterranean diet should be part of a multi-intervention strategy for the prevention and treatment of pediatric overweight and obesity.

Dietary patterns have been related to health outcomes and morbi-mortality. Mediterranean diet indexes are correlated With adequate nutrient intake. The objective of the present study was to analyse the adequacy of nutrient intake of a posteriori defined Mediterranean (MDP) and Western (WDP) diet patterns in the Seguimiento Universidad de Navarra (SUN) cohort. A sample of 17 197 subjects participated in the study. Participants completed I 136-item validated semi-quantitative FFQ. Principal component analysis was used to define dietary patterns. Individuals were classified according to quintiles of adherence based on dietary pattern scores. Non-dietary variables, such as smoking and physical activity habits, were also taken into account. The probability approach was used to assess nutrient intake adequacy of certain vitamins (vitamins B(12), B(6), B(3), B(2), B(1), A, C, D and E) and minerals (Na, Zn, iodine, Se, folic acid, P, Mg, K, Fe and Ca). Logistic regression analysis was used to assess the adequacy of nutrient intake according to adherence to dietary patterns. WDP and MDP were defined. A higher quintile of adherence to an MDP was associated to I lower prevalence of inadequacy for the intake of Zn, iodine, vitamin E, Mg, Fe, vitamin B I, vitamin A, Se, vitamin C and folic acid. The adjusted OR for not reaching at least six (or at leas( ten) nutrient recommendations were 0.09 (95% Cl: 0.07, 0.11) (and 0.02 (95% Cl: 0.00, 0.16)) for the upper quintile of MDP and 4.4 (95% Cl: 3.6, 5.5) and 2.5 (95 % Cl: 1.1, 5.4) for the WDP. The MDP was associated to a better profile of nutrient intake.

Our project aims at analyzing the relevance of economic factors (mainly income and other socioeconomic characteristics of Spanish households and market prices) on the prevalence of obesity in Spain and to what extent market intervention prices are effective to reduce obesity and improve the quality of the diet, and under what circumstances. In relation to the existing literature worldwide, this project is the first attempt in Spain trying to get an overall picture on the effectiveness of public policies on both food consumption and the quality of diet, on one hand, and on the prevalence of obesity on the other. The project consists of four main parts. The first part represents a critical review of the literature on the economic approach of dealing with the obesity prevalence problems, diet quality and public intervention policies. Although another important body of obesity literature is dealing with physical exercise but in this paper we will limit our attention to those studies related to food consumption respecting the scope of our study and as there are many published literature review dealing with the literature related to the physical exercise and its effect on obesity prevalence. The second part consists of a Parametric and Non-Parametric Analysis of the Role of Economic Factors on Obesity Prevalence in Spain. The third part is trying to overcome the shortcomings of many diet quality indices that have been developed during last decades, such as the Healthy Eating Index, the Diet Quality Index, the Healthy Diet Indicator, and the Mediterranean Diet Score, through the development of a new obesity specific diet quality index. While the last part of our project concentrates on the assessment of the effectiveness of market intervention policies to improve the healthiness of the Spanish Diet Using the new Exact Affine Stone Index (EASI) Demand System.

BACKGROUND/AIMS: The determinants of a healthy diet have not been studied in Switzerland. This study aimed at assessing the individual and behavioural factors associated with a healthy diet in a Swiss city. METHODS: Cross-sectional, population-based study conducted between 2009 and 2013 (n = 4,439, 2,383 women, mean age 57.5 ± 10.3 years) in Lausanne. Food consumption was assessed using a validated food frequency questionnaire. Two Mediterranean diet scores (classic score and specific for Switzerland) and the Harvard School of Public Health alternate healthy eating index were computed. RESULTS: For all three dietary scores considered, living in couple or having a high education were associated with a healthier diet. An unhealthy lifestyle (smoking, sedentary behaviour) or a high body mass index were associated with an unhealthier diet. Participants born in Italy, Portugal and Spain had healthier diets than participants born in France or Switzerland. Women and elderly participants had healthier diets than men and young participants according to 2 scores, while no differences were found for the Swiss-specific Mediterranean score. CONCLUSIONS: In Switzerland, healthy eating is associated with high education, a healthy lifestyle, marital status and country of origin. The associations with gender and age depend on the dietary score considered.

Sofrito is a key component of the Mediterranean diet, a diet that is strongly associated with a reduced risk of cardiovascular events. In this study, different Mediterranean sofritos were analysed for their content of polyphenols and carotenoids after a suitable work-up extraction procedure using liquid chromatography/electrospray ionisation-linear ion trap quadrupole-Orbitrap-mass spectrometry (LC/ESI-LTQ-Orbitrap- MS) and liquid chromatography/electrospray ionisation tandem triple quadrupole mass spectrometry (LC/ESI-MS-MS). In this way, 40 polyphenols (simple phenolic and hydroxycinnamoylquinic acids, and flavone, flavonol and dihydrochalcone derivatives) were identified with very good mass accuracy (<2 mDa), and confirmed by accurate mass measurements in MS and MS2 modes. The high-resolution MS analyses revealed the presence of polyphenols never previously reported in Mediterranean sofrito. The quantification levels of phenolic and carotenoid compounds led to the distinction of features among different Mediterranean sofritos according to the type of vegetables (garlic and onions) or olive oil added for their production. © 2013 Elsevier Ltd. Introdução – A nível mundial, o cancro da mama é o tipo de cancro mais frequente nas mulheres. Estudos têm sugerido que os fatores de estilo de vida (alimentação, consumo de álcool, atividade física) e a adiposidade corporal estão implicados na sua etiologia, sobretudo nos tumores diagnosticados após a menopausa. O objetivo deste artigo foi o de identificar quais os fatores nutricionais e de composição corporal implicados na etiologia da doença e qual a sua relação. Metodologia – Foi realizada uma revisão da literatura referente a estudos de revisão, de coorte e experimentais, de acordo com a temática em estudo. Resultados – As mulheres obesas têm maior risco de desenvolver cancro da mama após a menopausa e a adiposidade corporal, localizada na região abdominal, é também fator de risco. A obesidade, além de fator promotor, relaciona-se inversamente com o prognóstico da doença. Relativamente ao padrão alimentar, a ingestão de ácidos gordos saturados confere risco, ainda que com uma associação fraca e, com uma associação modesta, a dieta mediterrânica parece estar associada a um menor risco de cancro da mama pós-menopausa. Conclusão – A adoção de um estilo de vida saudável (alimentação e atividade física) e a obtenção/manutenção de um peso corporal saudável devem ser encorajados para prevenir o cancro da mama. Mais estudos devem ser realizados no futuro para consolidar a associação com outras variáveis dietéticas. ABSTRACT - Background – Breast cancer is worldwide the most common cancer in women. Studies have suggested that lifestyle such as nutrition, alcohol intake and physical activity are enrolled in aetiology, especially in postmenopausal tumours. This review aimed to investigate the relationship between nutritional factors and body composition with the disease. Methods – A selective review of the literature for recent studies and meta-analyses on this topic. Results – Obese women have higher risk to develop breast cancer after menopause. Abdominal adiposity is also a risk factor. Obesity, beside of promote the disease, is also a negative factor of prognoses. In respect to dietary pattern, the intake of saturated fatty acids seems to attribute risk, even if with a weak association. Although with a modest association, Mediterranean diet may be associated with lower breast cancer risk among postmenopausal women. Conclusion – Adopt a healthy lifestyle (nutrition and physical activity) and maintain a healthy body weight should be encouraged to prevent breast cancer. Future research should focus other dietary variables already investigated but still with no clear evidence.

Title: «Mediterranean diet in Portuguese proverbs». Mediterranean diet has been considered Immaterial World Patrimony since December 2013. It is characteristic of basin Mediterranean people (Portugal, Spain, Morocco, Italy, Greece, Cyprus and Croatia) and includes their gastronomy with local products, their traditions, their culture and their healthy lifestyle. Mediterranean diet is passed on from one generation to another mostly orally and it is likewise present in phraseological units, especially in proverbs. This paper analyses Portuguese proverbs aiming to show their importance when transmitting cookingrelated knowledge and information, such as, local ingredients, cooking techniques and the social function, both daily or on holidays. Since the practice of physical exercise is so important for the Mediterranean diet, our study includes physical exercise and relaxation-related proverbs as well.

Durant les darreres dècades, i degut, principalment, a un canvi en els hàbits alimentaris, hi ha hagut un augment a nivell mundial de malalties cròniques (l’obesitat, malalties cardiovasculars, etc.). En els països mediterranis hi ha menys incidència d’aquestes malalties i sembla ser que això es deu a l’anomenada dieta mediterrània. La dieta mediterrània es caracteritza per una combinació d’oli d’oliva com a grassa principal, verdures, hortalisses i fruites en abundància, lleguminoses, fruits secs, formatges i iogurt, peix, pa, pasta, cereals i els seus derivats i un consum moderat de vi i carns. Aquest model alimentari, ric en tocoferols, fitosterols i fitoestanols que ajuden a reduir el contingut de colesterol en sang, fa que en les poblacions mediterrànies hi hagi menys incidència de malalties cardiovasculars. Aquests compostos inhibeixen el deteriorament oxidatiu dels olis, actuen com agent antipolimerització per olis de fregir. Tenen capacitat de reduir els nivells de colesterol, evitant la incidència de malalties cardiovasculars. Els fitoesterols y fitoestanols es poden trobar en forma lliure o esterificada amb àcids grassos, àcids fenòlics i glucosa. Els objectius d’ aquest treball han estat, primer en el desenvolupament de mètodes d'anàlisi ràpids, fiables i robusts dels tocoferols, fitoesterols i fitoestanols i la seva aplicació en fruits sec, oli de segó, oli de pinyol de raïm i productes que els continguin. El primer mètode va estar basat en la cromatografía líquida (HPLC-DAD) amb extracció en fase sòlida (SPE) com tècnica alternativa a la saponificació para la determinació de fitoesterols lliures. Aquest mètode va estar aplicada a mostres de bombons que contenia fitoesterols. El segon mètode va estar basat en la cromatografia de gasos (GCFID) amb aponificació i SPE per quantificar fitoesterols i fitoestanols lliures, esterificats i totals. En els documents annexos es descriuen a profunditat els mètodes desenvolupats.

Introducció: Entre els comportaments alimentaris que poden contribuir a una ingesta dietètica inadequada hi ha el consum d’aliments de conveniència, aliments que es caracteritzen per ser d’elevat contingut calòric, riquesa en greixos saturats i sucres. Les màquines de vending són un dels principals exemples de disponibilitat d’aliments de conveniència. Objectius: Avaluar entre els estudiants de la Universitat de Vic (UVic) els hàbits de consum de productes de les màquines de vending i estudiar les característiques generals dels seus hàbits alimentaris. Materials i mètodes: Enquesta realitzada a 253 estudiants de la UVic durant el curs 07/08. Resultats: Un 72.7% dels enquestats consumeixen productes de les Màquines Expenedores d’Aliments i Begudes (MEAB), fonamentalment com a complement dels àpats i un 63.8% dels enquestats refereix que els agradaria rebre informació nutricional sobre els productes ofertats. Els hàbits alimentaris dels estudiants són bastant adequats ja que un 81.4% realitza almenys 4 àpats diaris. La qualitat de l’esmorzar és bona en el 12.3% dels casos i entre aquells que prenen algun aliment a mig matí la qualitat total de l’esmorzar millora (55.7% bona qualitat). Segons el Kidmed un 39.9% realitzen una dieta mediterrània i s’observen diferències segons la qualitat total de l’esmorzar (p&0.001). Conclusions: El consum de productes de les MEAB en l’entorn universitari és molt important, per la qual cosa la millora del perfil nutricional i la implantació d’estratègies informatives sobre els productes, permetrà que els consumidors estiguin més informats i puguin fer una elecció alimentària més saludable. Els hàbits alimentaris de la població d’estudi reflexen que la qualitat de la dieta dels universitaris és bastant bona, tot i que diferents factors podrien millorar-se. Introducció: La dieta mediterrània és considerada un dels patrons alimentaris més saludables ja que diversos estudis epidemiològics mostren que protegeix front diverses malalties cròniques com ara les malalties cardiovasculars, la diabetis i alguns càncers. Malauradament al nostre país els patrons alimentaris estan canviant a conseqüència de la modernització de la societat i estan portant a l’allunyament del patró de dieta mediterrània. Això és especialment crític entre la població infantil i juvenil, fet que predisposa a que en un futur augmenti la prevalença de malalties cròniques entre la població adulta. Objectiu: avaluar els hàbits alimentaris i l’estat nutricional d’escolars de 8 a 12 anys de la comarca d’Osona. Mètodes: estudi de disseny observacional i transversal. La mostra estava constituïda per 191 escolars de 4rt i 5è d’ensenyança primària d’escoles de la comarca d’Osona. La informació sobre la dieta es va recollir a través d’una enquesta alimentària, un recordatori de 24 hores, un qüestionari de freqüència de consum alimentari i el test KIDMED. Les determinacions antropomètriques que es van realitzar van ser talla, pes, IMC (Índex de Massa Corporal), plec tricipital, perímetre braquial i perímetre abdominal. L’anàlisi estadística de totes les dades es va portar a terme mitjançant el programa estadístic SPSS per Windows versió 12.0. Resultats: quan es valora la qualitat de l’esmorzar, s’observa que només en el 16.8% dels casos la qualitat era bona, en el 68.6% dels casos la qualitat havia de millorar i en el 12.6% era de qualitat insuficient i en el 2.1% era de mala qualitat. També s’observa que la dieta que segueixen és desequilibrada quantitativament, concretament és baixa en hidrats de carboni, rica en greixos i lleugerament elevada en proteïnes. L’anàlisi qualitativa mostra que hi ha un molt baix consum de verdures i hortalisses i de fruita, mentre que s’arriba a les racions recomanades de carnis i d’olis i greixos. El test KIDMED mostra una puntuació mitjana de 7.21±1.96 punts, el que indica que la dieta de la població és de qualitat millorable. Pel que fa a l’estat nutricional s’observa que un 24,6% presenta valors de baix pes i un 17,8% sobrepès o obesitat, el que indica que un 42,4% de la població estudiada presenta un estat nutricional incorrecte. Conclusions: La dieta del col·lectiu estudiat s’allunya de les recomanacions de la SENC (Sociedad Española de Nutrición Comunitaria). Els resultats del test KIDMED indiquen que només un 44% de la població segueix un patró òptim de Dieta mediterrània i l’anàlisi de l’estat nutricional mostra que un 42,4% presenta un estat nutricional incorrecte, sigui per excés o per defecte de pes. Per tant, es fa necessari dissenyar estratègies d’educació alimentària adequades per millorar els hàbits alimentaris dels escolars i aconseguir en un futur un estat òptim de salut.

Within only two decades olive oil developed from a niche product which could hardly be found in food stores outside the producing regions towards an integrated component in the diets of industrial countries. This paper discusses the impacts of the promotion of the “healthy Mediterranean diet” on land use and agro-ecosystems in the producing countries. It examines the dynamics of olive oil production, trade and consumption in the EU15 in the period 1972 to 2003 and the links between dietary patterns, trade and land use. It analyses the underlying socio-economic driving forces behind the increasing spatial disconnect between production and consumption of olive oil in the EU15 and in particular in Spain, the world largest producer during the last three decades. In the observed period olive oil consumption increased 16 fold in the non-producing EU15 countries. In the geographically limited producing regions like Spain, the 5 fold increase in export production was associated with the rapid industrialization of olive production, the conversion of vast Mediterranean landscapes to olive monocultures and a range of environmental pressures. High amounts of subsidies of the European Common Agricultural Policy and feedback loops within production and consumption systems were driving the transformation of the olive oil system. Our analysis indicates the process of change was not immediately driven by increases in demand for olive oil in non-producing countries, but rather by the institutional setting of the European Union and by concerted political interventions.
