Le remodelage cardiaque est le processus par lequel la structure ou la fonction cardiaque change en réponse à un déséquilibre pathophysiologique tel qu'une maladie cardiaque, un contexte d'arythmie prolongée ou une modification de l'équilibre hormonal. Le système rénine-angiotensine (SRA) est un système hormonal largement étudié et il est impliqué dans de nombreuses activités associées au remodelage cardiovasculaire. L’existence d'un système circulatoire couplé à un système de tissus locaux est une représentation classique, cependant de nouvelles données suggèrent un SRA indépendant et fonctionnellement actif à l'échelle cellulaire. La compréhension de l'activité intracellulaire du SRA pourrait mener à de nouvelles pistes thérapeutiques qui pourraient prévenir un remodelage cardiovasculaire défavorable. L'objectif de cette thèse était d'élucider le rôle du SRA intracellulaire dans les cellules cardiaques. Récemment, les récepteurs couplés aux protéines G (RCPG), les protéines G et leurs effecteurs ont été détectés sur des membranes intracellulaires, y compris sur la membrane nucléaire, et les concepts de RCPG intracellulaires fonctionnels sont en voie d'être acceptés comme une réalité. Nous avons dès lors fait l'hypothèse que la signalisation du SRA délimitant le noyau était impliquée dans le contrôle de l'expression des gènes cardiaques. Nous avons démontré la présence de récepteurs d'angiotensine de type-1 (AT1R) et de type-2 (AT2R) nucléaires dans les cardiomyocytes ventriculaires adultes et dans une fraction nucléaire purifiée de tissu cardiaque. Des quantités d'Ang II ont été détectées dans du lysat de cardiomyocytes et des microinjections d'Ang-II-FITC ont donné lieu à des liaisons préférentielles aux sites nucléaires. L'analyse transcriptionnelle prouve que la synthèse d'ARN de novo dans des noyaux isolés stimulés à l'Ang-II, et l'expression des ARNm de NF-κB étaient beaucoup plus importants lorsque les noyaux étaient exposés à de l'Ang II par rapport aux cardiomyocytes intacts. La stimulation des AT1R nucléaires a engendré une mobilisation de Ca2+ via les récepteurs de l'inositol trisphosphate (IP3R), et le blocage des IP3R a diminué la réponse transcriptionnelle. Les méthodes disponibles actuellement pour l'étude de la signalisation intracrine sont limitées aux méthodes indirectes. L'un des objectifs de cette thèse était de synthétiser et caractériser des analogues d'Ang-II cellule-perméants afin d’étudier spécifiquement dans les cellules intactes l'activité intracellulaire du SRA. Nous avons synthétisé et caractérisé pharmacologiquement des analogues photosensibles Ang-II encapsulée en incorporant un groupement 4,5-diméthoxy-2-nitrobenzyl (DMNB) photoclivable sur les sites actifs identifiés du peptide. Chacun des trois analogues d'Ang II encapsulée synthétisés et purifiés: [Tyr(DMNB)4]Ang-II, Ang-II-ODMNB et [Tyr(DMNB)4]Ang-II-ODMNB a montré une réduction par un facteur deux ou trois de l'affinité de liaison envers AT1R et AT2R dans les dosages par liaison compétitive et une activité réduite dans la contraction de l'aorte thoracique. La photostimulation de [Tyr(DMNB)4]Ang-II dans des cellules HEK a augmenté la phosphorylation d'ERK1/2 (via AT1R) et la production de cGMP (via AT2R) alors que dans les cardiomyocytes isolés elle générait une augmentation de Ca2+ nucléoplasmique et initiait la synthèse d'ARNr 18S et d'ARNm du NF-κB. Les fibroblastes sont les principaux générateurs de remodelage cardiaque structurel, et les fibroblastes auriculaires sont plus réactifs aux stimuli profibrotiques que les fibroblastes ventriculaires. Nous avons émis l'hypothèse que l’Ang-II intracellulaire et l'activation des AT1R et AT2R nucléaires associés contrôlaient les profils d'expression des gènes des fibroblastes via des systèmes de signalisation distincts et de ce fait jouaient un rôle majeur dans le développement de la fibrose cardiaque. Nous avons remarqué que les fibroblastes auriculaires expriment l’AT1R et l’AT2R nucléaire et l'Ang-II au niveau intracellulaire. L’expression d'AT1R nucléaire a été régulés positivement dans les cas d’insuffisance cardiaque (IC), tandis que l'AT2R nucléaire a été glycosylé post-traductionnellement. La machinerie protéique des protéines G, y compris Gαq/11, Gαi/3, et Gβ, a été observée dans des noyaux isolés de fibroblastes. AT1R et AT2R régulent l'initiation de la transcription du fibroblaste via les voies de transduction de signal d'IP3R et du NO. La photostimulation de [Tyr(DMNB)4]Ang-II dans une culture de fibroblastes auriculaire déclenche la libération de Ca2+ nucléoplasmique, la prolifération, et la synthèse et sécrétion de collagène qui ne sont pas inhibées par les bloqueurs d'AT1R et/ou AT2R extracellulaires.

Les effets des lésions de la moelle épinière sur la locomotion sont souvent évalués sur un tapis roulant avec une surface plane, ce qui demande peu d’implication active des structures supraspinales. L’objectif du présent travail est d’évaluer si un type d’entraînement nécessitant une plus grande part de contrôle volontaire (c.-à-d. supraspinal) pourrait améliorer la récupération de la marche chez le chat après une hémilésion unilatérale spinale au niveau thoracique (T10). Pour ce faire, pendant 6 semaines les chats ont été entrainés sur un tapis roulant conventionnel ou sur un tapis-échelle roulante, tâche requérant un placement des pattes plus précis. Les paramètres de la marche ont été évalués par cinématique et électromyographie (EMG) avant et une fois par semaine pendant 6 semaines après lésion. Nos résultats comparant la marche sur tapis conventionnel à celle sur échelle roulante montrent des différences dans les excursions angulaires et les couplages entre les membres. On observe aussi des différences dans l’amplitude des EMG notamment une augmentation de la deuxième bouffée du muscle Semitendineux (St) sur l’échelle roulante. Après l’hémilésion spinale cette bouffée disparait du côté de la lésion tandis qu’elle est maintenue du côté intact. Après l’entrainement sur échelle roulante, on observe des changements de trajectoire de la patte et une disparition du pied tombant (foot drag) qui suggèrent une amélioration du contrôle de la musculature distale. Nos résultats montrent que le patron locomoteur observé sur tapis conventionnel est influencé par le type d’entraînement procuré. De plus, certains paramètres de la locomotion suggèrent que l’entraînement sur échelle roulante, qui requiert plus de contrôle supraspinal, favorise une meilleure récupération de la marche après lésion spinale.

Adolescent idiopathic scoliosis (AIS) is a deformity of the spine manifested by asymmetry and deformities of the external surface of the trunk. Classification of scoliosis deformities according to curve type is used to plan management of scoliosis patients. Currently, scoliosis curve type is determined based on X-ray exam. However, cumulative exposure to X-rays radiation significantly increases the risk for certain cancer. In this paper, we propose a robust system that can classify the scoliosis curve type from non invasive acquisition of 3D trunk surface of the patients. The 3D image of the trunk is divided into patches and local geometric descriptors characterizing the surface of the back are computed from each patch and forming the features. We perform the reduction of the dimensionality by using Principal Component Analysis and 53 components were retained. In this work a multi-class classifier is built with Least-squares support vector machine (LS-SVM) which is a kernel classifier. For this study, a new kernel was designed in order to achieve a robust classifier in comparison with polynomial and Gaussian kernel. The proposed system was validated using data of 103 patients with different scoliosis curve types diagnosed and classified by an orthopedic surgeon from the X-ray images. The average rate of successful classification was 93.3% with a better rate of prediction for the major thoracic and lumbar/thoracolumbar types.

Objective To determine scoliosis curve types using non invasive surface acquisition, without prior knowledge from X-ray data. Methods Classification of scoliosis deformities according to curve type is used in the clinical management of scoliotic patients. In this work, we propose a robust system that can determine the scoliosis curve type from non invasive acquisition of the 3D back surface of the patients. The 3D image of the surface of the trunk is divided into patches and local geometric descriptors characterizing the back surface are computed from each patch and constitute the features. We reduce the dimensionality by using principal component analysis and retain 53 components using an overlap criterion combined with the total variance in the observed variables. In this work, a multi-class classifier is built with least-squares support vector machines (LS-SVM). The original LS-SVM formulation was modified by weighting the positive and negative samples differently and a new kernel was designed in order to achieve a robust classifier. The proposed system is validated using data from 165 patients with different scoliosis curve types. The results of our non invasive classification were compared with those obtained by an expert using X-ray images. Results The average rate of successful classification was computed using a leave-one-out cross-validation procedure. The overall accuracy of the system was 95%. As for the correct classification rates per class, we obtained 96%, 84% and 97% for the thoracic, double major and lumbar/thoracolumbar curve types, respectively. Conclusion This study shows that it is possible to find a relationship between the internal deformity and the back surface deformity in scoliosis with machine learning methods. The proposed system uses non invasive surface acquisition, which is safe for the patient as it involves no radiation. Also, the design of a specific kernel improved classification performance.

The value of the lateral bending test is important in the assessment of spinal curve mobility and prediction of surgical outcome in the treatment of adolescent idiopathic scoliosis (AIS). However, radiographic bending tests are unable to assess the reducibility of trunk asymmetry. This study aims to exploit surface topography measurement in order to evaluate the changes in shape of the trunk (a) between bending and neutral standing positions, and (b) between standing pre- and post-operative visits, in a cohort of adolescents with AIS having undergone surgical correction; and to correlate the differences measured in cases (a) and (b). Our cohort includes 13 patients with right thoracic AIS. Each patient had their 3D trunk surface digitized with a multi-head InSpeck system in standing posture (at the pre-op and post-op visits) and in maximum voluntary right and left bending (at the pre-op visit). We developed a novel trunk shape analysis method which produces a set of inclined trunk cross-sections allowing comparison between different postures. Two asymmetry indices, trunk rotation (TR) and back surface rotation (BSR), were computed in all cases and a statistical analysis was performed. Our correlation study (Pearson test) showed fair correlations in most cases between the changes in side-bending and those post-surgery, with the strongest relationship (p-value < 0.01) when combining the TR measurements from both bendings. These results provide evidence that the bending test can be used to assess trunk asymmetry reducibility. The proposed approach could provide a non-invasive trunk asymmetry reducibility test for routine clinical use in AIS surgery planning.

STUDY DESIGN: Concurrent validity between postural indices obtained from digital photographs (two-dimensional [2D]), surface topography imaging (three-dimensional [3D]), and radiographs. OBJECTIVE: To assess the validity of a quantitative clinical postural assessment tool of the trunk based on photographs (2D) as compared to a surface topography system (3D) as well as indices calculated from radiographs. SUMMARY OF BACKGROUND DATA: To monitor progression of scoliosis or change in posture over time in young persons with idiopathic scoliosis (IS), noninvasive and nonionizing methods are recommended. In a clinical setting, posture can be quite easily assessed by calculating key postural indices from photographs. METHODS: Quantitative postural indices of 70 subjects aged 10 to 20 years old with IS (Cobb angle, 15 degrees -60 degrees) were measured from photographs and from 3D trunk surface images taken in the standing position. Shoulder, scapula, trunk list, pelvis, scoliosis, and waist angles indices were calculated with specially designed software. Frontal and sagittal Cobb angles and trunk list were also calculated on radiographs. The Pearson correlation coefficients (r) was used to estimate concurrent validity of the 2D clinical postural tool of the trunk with indices extracted from the 3D system and with those obtained from radiographs. RESULTS: The correlation between 2D and 3D indices was good to excellent for shoulder, pelvis, trunk list, and thoracic scoliosis (0.81>r<0.97; P<0.01) but fair to moderate for thoracic kyphosis, lumbar lordosis, and thoracolumbar or lumbar scoliosis (0.30>r<0.56; P<0.05). The correlation between 2D and radiograph spinal indices was fair to good (-0.33 to -0.80 with Cobb angles and 0.76 for trunk list; P<0.05). CONCLUSION: This tool will facilitate clinical practice by monitoring trunk posture among persons with IS. Further, it may contribute to a reduction in the use of radiographs to monitor scoliosis progression.

Study Design Cross-sectional descriptive study. Objectives To characterize breast asymmetry (BA), as defined by breast volume difference, in girls with significant adolescent idiopathic scoliosis (AIS), using magnetic resonance imaging (MRI). Summary and Background BA is a frequent concern among girls with AIS. It is commonly believed that this results from chest wall deformity. Although many women exhibit physiological BA, the prevalence is not known in adolescents and it remains unclear if it is more frequent in AIS. Breasts vary in shape and size and many ways of measuring them have been explored. MRI shows the highest precision at defining breast tissue. Methods Thirty patients were enrolled on the basis of their thoracic curvature, skeletal and breast maturity, without regard to their perception on their BA. MRI acquisitions were performed in prone with a 1.5-Tesla system using a 16-channel breast coil. Segmentation was achieved using the ITK-SNAP 2.4.0 software and subsequently manually refined. Results The mean left breast volume (528.32 ± 205.96 cc) was greater compared with the mean right breast volume (495.18 ± 170.16 cc) with a significant difference between them. The mean BA was found to be 8.32% ± 6.43% (p < .0001). A weak positive correlation was observed between BA and thoracic Cobb angle (0.177, p = .349) as well as thoracic gibbosity angle (0.289, p = .122). The left breast was consistently larger in 65.5% of the patients. Twenty patients (66.7%) displayed BA ≥5%. Conclusions We have described BA in patients with significant AIS using MRI. This method is feasible, objective, and very precise. The majority of patients had a larger left breast, which could compound the apparent BA secondary to trunk rotation. In many cases, BA is present independently of thoracic deformity. This knowledge will assist in counseling AIS patients in regards to their concerns with BA.

Among the external manifestations of scoliosis, the rib hump, which is associated with the ribs' deformities and rotations, constitutes the most disturbing aspect of the scoliotic deformity for patients. A personalized 3-D model of the rib cage is important for a better evaluation of the deformity, and hence, a better treatment planning. A novel method for the 3-D reconstruction of the rib cage, based only on two standard radiographs, is proposed in this paper. For each rib, two points are extrapolated from the reconstructed spine, and three points are reconstructed by stereo radiography. The reconstruction is then refined using a surface approximation. The method was evaluated using clinical data of 13 patients with scoliosis. A comparison was conducted between the reconstructions obtained with the proposed method and those obtained by using a previous reconstruction method based on two frontal radiographs. A first comparison criterion was the distances between the reconstructed ribs and the surface topography of the trunk, considered as the reference modality. The correlation between ribs axial rotation and back surface rotation was also evaluated. The proposed method successfully reconstructed the ribs of the 6th-12th thoracic levels. The evaluation results showed that the 3-D configuration of the new rib reconstructions is more consistent with the surface topography and provides more accurate measurements of ribs axial rotation.

This thesis entitled “Histological ,Histochemical and biochemical characterisation of male morphotypes of macrobrachium rosenbergii(De Man). The giant fresh water prawn Macrobrachium rosenbergii (de Man) is emerging as a prime candidate species in fresh water aquaculture as a global basis and therefore, receiving much attention in recent years.the present work was aimed at to study the histological variations, if any, in the reproductive system viz. testes, vas deferens including androgenic gland, hepatopancreas and the neurosecretory system viz. eye stalk, brain and thoracic ganglion among the male morphotypes and their transitional stages of M.rosenbergii from growouts. This study was also aimed at to bring out the histochemical variations, if any, in the reproductive system comprising of testes, vas deferens including androgenic gland and the hepatopancreas among the male morphotypes and their transitional stages collected from growouts. Biochemical characterisation of various male morphotyes and their transitional stages have also been attempted in order to find out biochemical evidence, if any, in the morphotypic transformation.Histological study of the testes of three male morphotypes viz., SM, SOC and SBC and their 'transitional stages viz., WOC, tSOC, WBC and OBC have been carried out with a view to unravel the structural and functional differences of the testes, if any, of these morphotypes. Studies on the lipid components viz., cholesterol, phospholipid and triglyceride In the muscle tissue and hepatopancreas have been carried out.

Pedicle screw insertion technique has made revolution in the surgical treatment of spinal fractures and spinal disorders. Although X- ray fluoroscopy based navigation is popular, there is risk of prolonged exposure to X- ray radiation. Systems that have lower radiation risk are generally quite expensive. The position and orientation of the drill is clinically very important in pedicle screw fixation. In this paper, the position and orientation of the marker on the drill is determined using pattern recognition based methods, using geometric features, obtained from the input video sequence taken from CCD camera. A search is then performed on the video frames after preprocessing, to obtain the exact position and orientation of the drill. An animated graphics, showing the instantaneous position and orientation of the drill is then overlaid on the processed video for real time drill control and navigation

In this paper the effectiveness of a novel method of computer assisted pedicle screw insertion was studied using testing of hypothesis procedure with a sample size of 48. Pattern recognition based on geometric features of markers on the drill has been performed on real time optical video obtained from orthogonally placed CCD cameras. The study reveals the exactness of the calculated position of the drill using navigation based on CT image of the vertebra and real time optical video of the drill. The significance value is 0.424 at 95% confidence level which indicates good precision with a standard mean error of only 0.00724. The virtual vision method is less hazardous to both patient and the surgeon

The diffusion of highly productive breeds across developing countries goes along with a neglect of indigenous breeds, which are well suited to their environment but often show low yields. Thus, in Niger, the flock of Koundoum sheep are rapidly decreasing. The Koundoum is one of the few wool sheep breeds of Africa and shows important adaptive feature to its native environment, i.e. the humid pastures on the banks of the Niger River. To characterise the breed and to understand its production context, a survey has been conducted in 104 herds in four communes along the Niger River (Kollo, Tillabery, Say and Tera). Nine body measurements, including live weight, were taken on 180 adult sheep (101 females and 79 males). The herds varied from 2 to 60 heads, with a median size of eight animals and two thirds of the herds having less than 10 animals. Mainly fed on natural pastures, 85.6% of the herds received crop residues. Only natural mating was practiced. Veterinary care was restricted to anti-helminthic and some indigenous treatments. The frequent affiliation of breeders to professional unions appeared as favourable to the implementation of a collective conservation program. The Koundoum sheep were white or black coated, with the black colour being most frequent (75.6%). Wattles were present in both sexes at similar frequencies of around 14%. All biometric variables were significantly and positively correlated between them. The thoracic perimeter showed the best correlation with live weight in both males and females. Three variables were selected for live weight prediction: thoracic perimeter, height at withers and rump length. From the present study, it is expected that the in situ conservation of the Koundoum sheep will be highly problematic, due to lack of market opportunities for wool and the willingness of smallholders to get involved in pure Koundoum rearing.

Objetivos : evaluar las características operativas del examen físico en el diagnóstico de neumonía y evaluar su acuerdo inter-observador. Marco de referencia : los estudios que avaluaron al examen físico como prueba diagnóstica en neumonía son metodológicamente deficientes. Diseño : estudio ciego de corte transversal para evaluación de prueba diagnóstica. Pacientes : adultos quienes consultan al servicio de urgencias y hospitalización de la FCI por síntomas respiratorios agudos o exacerbación de los mismos. Mediciones : examen físico por dos observadores independientes, toma de radiografía de tórax y lectura por radiólogo experto. Se tomaron los datos que permitieron calcular el índice de severidad de neumonía (PSI). Resultados : de 198 pacientes, 85(42%) tenían neumonía radiográficamente. Las características operativas del examinador1 fueron: Sensibilidad:63.2%, Especificidad:54,1%, LR(+)=1,36, LR(-)=0,68; para el examinador2: Sensibilidad:34,3%, Especificidad:71,7%, LR(+)=1,17, LR(-)=0,92. La correlación entre diagnóstico clínico para derrame pleural fue k=-0,052, no significativa (p=0,445); y para neumonía k=0.25 significativa (p=0.022). Al medirse la severidad de neumonía por PSI, la sensibilidad aumento estratificada a severidad (II:Sensibilidad:40%; III:Sensibilidad: 57%; IV:Sensibilidad;75%; V:Sensibilidad:80%). Conclusiones : el examen físico no es sensible ni especifico en el diagnóstico de neumonía. Existe un índice de acuerdo débil en el examen físico de tórax para el diagnóstico de derrame pleural y neumonía Es más probable el diagnóstico clínico de neumonía al aumentar la severidad por PSI.

El reparo endovascular de la aorta torácica (REVAT), se ha incrementado en el mundo en los últimos años con relación al abierto. Los resultados de la intervención son controversiales. La información en Colombia es insuficiente. Objetivo: Comparar los resultados clínicos del REVAT frente al reparo abierto en la Fundación Cardio Infantil (Bogotá, Col) entre 2002 y 2011. Metodología: Análisis retrospectivo de la cohorte de pacientes sometidos a reparo abierto (Grupo 1) en comparación de REVAT (grupo 2). En cada uno de los grupos se evaluó el tiempo quirúrgico, tasa de morbilidad (infección, hemo-rragia, isquemia medular, evento cerebrovascular [ECV] y falla renal postoperatoria), mortalidad, reintervención y estancia hospitalaria. Para las comparaciones se realizó un análisis univariado considerando un valor p<0.05 estadísticamente significativo. Resultados: Fueron incluidos 57 pacientes en el análisis (26% reparo abierto; 74% REVAT). Se reintervinieron 2 pacientes por endofugas tipo 1 en el Grupo 2 y un caso por sangrado en el Grupo 1. La mortalidad operatoria fue (Grupo1: 20%; Grupo 2: 2.3%). El tiempo quirúrgico fue de 398 ± 180 min Grupo 1 versus 85.5 ± 35 min en el Grupo 2 (p:0.0001) y el tiempo de estancia hospitalaria promedio fue de 9.8 días:Grupo 1 y 5,3 días Grupo 2 [p:0.01]). El tiempo promedio de seguimiento fue 4,8± 3,1 años. Conclusiones: El REVAT parece ofrecer menor morbilidad, mortalidad, tiempo quirúrgico y estancia hospitalaria respecto al reparo abierto, aunque las pobla-ciones de pacientes incluidos no fueron estrictamente comparables. Se requieren nuevos análisis en un diseño prospectivo, idealmente aleatorizado para documentar los beneficios a largo plazo de este tipo de reparo.
