Background: Obstetric fistula is the development of a necrosis between the bladder and the vagina and/or the bladder and the rectum as a result of prolonged obstructed labor, resulting in urinary or fecal incontinence. In Tanzania surgical repair for obstetric fistula is provided freely by the government but it is estimated that there are over 25,000 women living with an untreated fistula. These women experience high degrees of psycho-social stresses exacerbated by the stigma surrounding their condition. There is a dire need to explore stigma within this population in order to better understand its impact, as stigma affects both treatment seeking behavior as well as long term recovery of those who access surgical repair.
Study Aims: This study aims to understand the experiences of stigma among women with obstetric fistulas by examining both internalized and enacted stigma, and by identifying pertinent correlates of internalized stigma.
Methods: This mixed-methods study utilized both quantitative and qualitative data collected in two related studies at a single hospital in Moshi, Tanzania. All study participants were women receiving surgical repair for an obstetric fistula. In the quantitative portion, cross-sectional survey data were collected from 52 patients. The primary outcome was fistula-related stigma, measured using an adaptation of the HASI-P stigma scale, which included constructs of both internalized and enacted stigma. In the qualitative portion, 45 patients participated in a semi-structured in-depth interview, which explored topics such as stressors caused by the fistula, coping mechanisms, and available support. The transcripts were analyzed using analytic memos and an iterative process of thematic coding using the framework of content analysis.
Results: Expressions of internalized stigma were common in the sample, with a median score of 2.1 on a scale of 0 – 3. Internalized was significantly correlated with negative religious coping, social participation, impact of incontinence and enacted stigma. Qualitative analysis was consistent and demonstrated widespread themes of shame and embarrassment. Experiences of enacted stigma were not as common (median score of 0), although some items, like those pertaining to mockery and blame, were endorsed by up to 25% of the study sample. Themes of anticipated stigma (isolation and non-disclosure due to the possibility of stigmatization) were also evident in the qualitative sample and may explain the low enacted stigma scores observed.
Conclusion: In this sample of women receiving surgical repair for an obstetric fistula, stigma was evident, with internalized stigma resulting in psychological impacts for patients. Experiences of both anticipated and enacted stigma were also observed. There is a need to explore interventions that would decrease stigma while also increasing support for these women, as stigma may be a barrier towards accessing surgical repair and reintegration following surgery.

Background/aims: Objective of the current thesis is to investigate the potential impact of birth by Caesarean section (CS) on child psychological development, including autism spectrum disorder (ASD), attention-deficit/hyperactivity disorder (ADHD), behavioural difficulties and school performance. Structure/methods: Published literature to date on birth by CS, ASD and ADHD was reviewed (Chapter 2). Data from the UK Millennium Cohort Study (MCS) were analysed to determine the association between CS and ASD, ADHD and parent-reported behavioural difficulties (Chapter 3). The Swedish National Registers were used to further assess the association with ASD, ADHD and school performance (Chapters 4-6). Results: In the review, children born by CS were 23% more likely to be diagnosed with ASD after controlling for potential confounders. Only two studies reported adjusted estimates on the association between birth by CS and ADHD, results were conflicting and limited. CS was not associated with ASD, ADHD or behavioural difficulties in the UK MCS. In the Swedish National Registers, children born by CS were more likely to be diagnosed with ASD or ADHD. The association with elective CS did not persist when compared amongst siblings. There was little evidence of an association between birth by elective CS and poor school performance. Children born by elective CS had slight reduction in school performance. Conclusions: The lack of association with the elective CS in the sibling design studies indicates that the association in the population is most probably due to confounding. A small but significant association was found between birth by CS and school performance. However, the effect may have been due to residual confounding or confounding by indication and should be interpreted with caution. The overall conclusion is that birth by CS does not appear to have a causal relationship with the aspects of child psychological development investigated.

Background: Accurate estimates of the burden of diabetes are essential for future planning and evaluation of services. In Ireland, there is no diabetes register and prevalence estimates vary. The aim of this review was to systematically identify and review studies reporting the prevalence of diabetes and complications among adults in Ireland between 1998 and 2015 and to examine trends in prevalence over time. Methods: A systematic literature search was carried out using PubMed and Embase. Diabetes prevalence estimates were pooled by random-effects meta-analysis. Poisson regression was carried out using data from four nationally representative studies to calculate prevalence rates of doctor diagnosed diabetes between 1998 and 2015 and was also used to assess whether the rate of doctor diagnosed diabetes changed over time. Results: Fifteen studies (eight diabetes prevalence and seven complication prevalence) were eligible for inclusion. In adults aged 18 years and over, the national prevalence of doctor diagnosed diabetes significantly increased from 2.2 % in 1998 to 5.2 % in 2015 (p trend ≤ 0.001). The prevalence of diabetes complications ranged widely depending on study population and methodology used (6.5–25.2 % retinopathy; 3.2–32.0 % neuropathy; 2.5-5.2 % nephropathy). Conclusions: Between 1998 and 2015, there was a significant increase in the prevalence of doctor diagnosed diabetes among adults in Ireland. Trends in microvascular and macrovascular complications prevalence could not be examined due to heterogeneity between studies and the limited availability of data. Reliable baseline data are needed to monitor improvements in care over time at a national level. A comprehensive national diabetes register is urgently needed in Ireland.

Objective. To explore risk factors for macro- and microvascular complications in a nationally representative sample of adults aged 50 years and over with type 2 diabetes in Ireland. Methods. Data from the first wave of The Irish Longitudinal Study on Ageing (TILDA) (2009–2011) was used in cross-sectional analysis. The presence of doctor diagnosis of diabetes, risk factors, and macro and microvascular complications were determined by self-report. Gender-specific differences in risk factor prevalence were assessed with the chi-squared test. Binomial regression analysis was conducted to explore independent associations between established risk factors and diabetes-related complications. Results. Among 8175 respondents, 655 were classified as having type 2 diabetes. Older age, being male, a history of smoking, a lower level of physical activity, and a diagnosis of high cholesterol were independent predictors of macrovascular complications. Diabetes diagnosis of 10 or more years, a history of smoking, and a diagnosis of hypertension were associated with an increased risk of microvascular complications. Diabetes is the leading cause of end stage renal disease. Despite evidence for a substantial heritability of diabetic kidney disease, efforts to identify genetic susceptibility variants have had limited success. We extended previous efforts in three dimensions, examining a more comprehensive set of genetic variants in larger numbers of subjects with type 1 diabetes characterized for a wider range of cross-sectional diabetic kidney disease phenotypes. In 2,843 subjects, we estimated that the heritability of diabetic kidney disease was 35% ( p=6x10-3 ). Genome-wide association analysis and replication in 12,540 individuals identified no single variants reaching stringent levels of significance and, despite excellent power, provided little independent confirmation of previously published associated variants. Whole exome sequencing in 997 subjects failed to identify any large-effect coding alleles of lower frequency influencing the risk of diabetic kidney disease. However, sets of alleles increasing body mass index ( p=2.2×10-5) and the risk of type 2 diabetes (p=6.1x10-4 ) were associated with the risk of diabetic kidney disease. We also found genome-wide genetic correlation between diabetic kidney disease and failure at smoking cessation ( p=1.1×10-4 ). Pathway analysis implicated ascorbate and aldarate metabolism ( p=9×10-6), and pentose and glucuronate interconversions ( p=3×10-6) in pathogenesis of diabetic kidney disease. These data provide further evidence for the role of genetic factors influencing diabetic kidney disease in those with type 1 diabetes and highlight some key pathways that may be responsible. Altogether these results reveal important biology behind the major cause of kidney disease.

BACKGROUND: Reconstruction of the distal femur after resection for malignant bone tumors in skeletally immature children is challenging. The use of megaprostheses has become increasingly popular in this patient group since the introduction of custom-made, expandable devices that do not require surgery for lengthening, such as the Repiphysis(®) Limb Salvage System. Early reports on the device were positive but more recently, a high complication rate and associated bone loss have been reported. QUESTIONS/PURPOSES: We asked: (1) what are the clinical outcomes using the Musculoskeletal Tumor Society (MSTS) scoring system after 5-year minimum followup in patients treated with this prosthesis at one center; (2) what are the problems and complications associated with the lengthening procedures of this implant; and (3) what are the specific concerns associated with revision of this implant? METHODS: At our institute, between 2002 and 2007, the Repiphysis(®) expandable prosthesis was implanted in 15 children (mean age, 8 years; range, 6-11 years) after distal femoral resection for malignant bone tumors. During this time, the general indication for use of this implant was resection of the distal femur for localized malignant bone tumors in pediatric patients. Alternative techniques used for this indication were modular prosthetic reconstruction, massive (osteoarticular or intercalary) allograft reconstruction, or rotationplasty. Age and tumor extension were the main factors to decide on the surgical indication. Of the 15 patients who had this prosthesis implanted during reconstruction surgery, five died with the implant in situ or underwent amputation before 5 years followup and the remaining 10 were evaluated at a minimum of 5 years (mean, 104 months; range, 78-140 months). No patients were lost to followup. These 10 patients were long-term survivors and underwent the lengthening program. They were included in our study analysis. The first seven lengthening procedures were attempted in an outpatient setting; however, owing to pain and burning sensations experienced by the patients, the procedures failed to achieve the desired lengthening. Therefore, other procedures were performed with the patients under general anesthesia. We reviewed clinical data at index surgery for all 15 patients. We further analyzed the lengthening procedures, implant survival, radiographic and functional results, for the 10 long-term survivors. Functional results were assessed according to the MSTS scoring system. Complications were classified according to the International Society of Limb Salvage (ISOLS) classification system. RESULTS: Nine of the 10 survivors underwent revision of the implant for mechanical failure. They had a mean MSTS score of 64% (range, 47%-87%) before revision surgery. At final followup the 10 long-term surviving patients had an average MSTS score of 81% (range, 53%-97%). In total, we obtained an average lengthening of 39 mm per patient (range, 17-67 mm). Exact expansion of the implant was unpredictable and difficult to control. Nine of 10 of the long-term surviving patients underwent revision surgery of the prosthesis-eight for implant breakage and one for stem loosening. At revision surgery, six patients had another type of expandable prosthesis implanted and three had an adult-type megaprosthesis implanted. In five cases, segmental bone grafts were used during revision surgery to compensate for loss of bone stock. CONCLUSIONS: We could not comfortably expand the Repiphysis(®) prosthesis in an outpatient setting because of pain experienced by the patients during the lengthening procedures. Furthermore, use of the prosthesis was associated with frequent failures related to implant breakage and stem loosening. Revisions of these procedures were complex and difficult. We no longer use this prosthesis and caution others against the use of this particular prosthesis design. LEVEL OF EVIDENCE: Level IV, therapeutic study.

L’obésité est de nos jours un problème croissant à travers le monde. La morbidité qui y est associée est surtout reliée au développement des différentes composantes du syndrome métabolique (SMet), une constellation de facteurs de risque regroupant l’hypertension, la dyslipidémie (concentration faible de cholestérol des lipoprotéines à haute densité (C-HDL) et élevée de triglycérides (TG)), l’hyperglycémie et l’obésité. Cependant, certains sujets obèses demeurent métaboliquement sains. Les facteurs génétiques joueraient donc un rôle important dans le développement de l’obésité et de ses complications. Les facteurs épigénétiques semblent également y avoir des effets. L’analyse de tissu adipeux viscéral (TAV) a donc été réalisée pour mener à la découverte de plusieurs gènes différentiellement exprimés et méthylés entre les obèses atteints et non atteints par le SMet. Les deux gènes candidats NMT1 et DGKZ font partie de ce groupe et leurs associations avec les composantes du SMet ont été testées. Leurs niveaux de méthylation et d’expression génique ont aussi été analysés.

The number of Laparoscopic Roux-en-Y Gastric Bypass (LRYGB) procedures for morbid obesity and type 2 diabetes mellitus will increase worldwide, and therefore, an increase in perioperative morbidity can be anticipated. The authors present three cases based on different complications after LRYGB to demonstrate the diagnostic challenge that clinicians face in this particular group of patients. Also, a review of the literature covering the value of different imaging in these particular cases is provided by the authors. The role of imaging in the diagnostic process is discussed.

O presente estudo teve como objetivo geral elaborar uma proposta de processo de enfermagem contemplando históricos de enfermagem, diagnósticos de enfermagem, prescrição de cuidados de enfermagem para gestantes e puérperas em Unidade de Internação Obstétrica à luz de Horta, fundamentada em NANDA e, como objetivos específicos: identificar os dados clínico-obstétricos mais frequentes das gestantes em Unidade de Internação Obstétrica; identificar os dados clínico-obstétricos mais frequentes das puérperas em Unidade de Internação Obstétrica; identificar os diagnósticos de enfermagem a partir dos dados clínico-obstétricos mais frequentes das gestantes em Unidade de Internação Obstétrica; identificar os diagnósticos de enfermagem a partir dos dados clínico-obstétricos mais frequentes das puérperas em Unidade de Internação Obstétrica; elaborar modelos de histórico de enfermagem e de prescrição de cuidados de enfermagem para os diagnósticos de enfermagem identificados; e obter o consenso dos enfermeiros assistenciais da Unidade de Internação Obstétrica sobre a proposta construída. Trata-se de pesquisa qualitativa, descritiva e exploratória, realizada em uma Unidade de Internação Obstétrica de um hospital universitário do Sul do Brasil. Realizou-se uma consulta documental em 148 prontuários, com vistas à identificação dos diagnósticos e cuidados de enfermagem, baseados, respectivamente, na taxonomia II da North American Diagnosis Association 2009-2011 e da Nursing Intervention Classification. Desenvolveu-se um modelo de histórico de enfermagem para gestante/puérpera e outro para o recém-nascido. No total, identificaram-se 26 diagnósticos de enfermagem, assim distribuídos: nove direcionados às gestantes; nove, às puérperas; dois, ao binômio mãe/filho; seis, ao recém-nascido. A análise foi realizada por meio da apresentação da proposta construída e discussão com base em autores da área. Com isso, almeja-se contribuir para direcionar o cuidado de enfermagem às necessidades das gestantes, puérperas, binômios mãe/filho e recém-nascidos nesse microcenário, favorecendo a educação em saúde, a identificação precoce e a prevenção de complicações. Permanecem lacunas quanto às necessidades psicoespirituais, visto que elas não foram abordadas nos resultados. Assim, essa proposta de Processo de Enfermagem em Unidade Obstétrica contemplando históricos, diagnósticos e cuidados de enfermagem mantém em aberto o aperfeiçoamento do modelo, visando à inclusão de outros diagnósticos de enfermagem, de forma a aproximar-se de um cuidado mais holístico.

Enquadramento: A amamentação nutre, protege e favorece o desenvolvimento cognitivo e a criação do vínculo afetivo entre mãe e filho. No entanto, há vários fatores que dificultam a manutenção da amamentação como os psicológicos, socioculturais, profissionais, nível de educação, ação dos profissionais de saúde entre outros. Objetivos: Determinar o perfil sociodemográfico das puérperas; identificar as dificuldades mais frequentes associadas à amamentação ao 7º e 30º dia após o nascimento; analisar a influência das variáveis sociodemográficas associadas às dificuldades na amamentação nos dois momentos de avaliação; avaliar a influência das atitudes maternas e a da vinculação pós-natal nas dificuldades associadas à amamentação nos dois momentos de avaliação. Métodos: Estudo quantitativo, longitudinal em painel de curta duração, descritivo-correlacional, numa amostra não probabilística por conveniência de 255 participantes (média de idades de 30,78 anos). Utilizou-se um questionário para caracterizar sociodemograficamente e os contextos de amamentação da amostra, a escala de atitudes maternas face à amamentação (Pereira, 2004) e a escala de vinculação pós-natal (Airosa, Silva & Bachu, 2012), versão portuguesa de Gomez e Leal (2007). Este foi aplicado às participantes em dois momentos (7º e 30º dias após o nascimento do bebé). Resultados: As participantes são maioritariamente casadas, com o ensino superior, empregadas em tempo completo e residentes na cidade. Quanto às dificuldades mais frequentes associadas à amamentação nos dois momentos de avaliação do estudo (7º e 30º dia de vida do bebé) verificamos que as dificuldades sentidas ao 7º dia se mantêm no 30º, predominando as fissuras (7.º dia 41.7%; 39.3% 30.º dia), as dificuldades na pega (7.º dia 28.8%; 31.9% 30.º dia), o ingurgitamento mamário (7.º dia 16.7%; 14.8% 30.º dia). Em relação à influência das variáveis sociodemográficas nas dificuldades associadas à amamentação ao 7º e 30º dia após o nascimento, apurou-se que as diferenças são estatisticamente significativas em relação à escolaridade (p=0,000), situação profissional (p=0,034) e local de residência (0,042), sugerindo que estas variáveis têm poder explicativo sobre as dificuldades sentidas na amamentação pelas mães nos dois momentos de avaliação. Nenhuma das dimensões relativas às atitudes maternas face à amamentação e à vinculação pós-natal se assumiu preditora das dificuldades associadas à amamentação. Conclusão: Os resultados sugerem que os enfermeiros, especialmente no âmbito da saúde materna, obstétrica e ginecológica, estejam mais despertos para as dificuldades que as mães possam ter na amamentação sobretudo no primeiro mês, considerado o mais crítico na adaptação ao aleitamento materno, de modo a conseguirem que estas ultrapassem essas complicações, evitando assim, o abandono precoce da amamentação. Objetivo. Determinar el grado de satisfacción de las pacientes con la atención médica y de enfermería recibida en el Área de Hospitalización del Departamento de Gineco - Obstetricia del Hospital José Carrasco Arteaga. Metodología. Con un diseño descriptivo de corte transversal se recopiló información de 628 pacientes ingresadas a los servicios de Ginecología y Obstetricia entre marzo y mayo del 2014. Se analizó edad, instrucción, estado civil, residencia, diagnóstico al ingreso, al egreso y procedimiento realizado en los dos servicios y grado de satisfacción de las pacientes con el personal médico y el personal de enfermería. Resultados. Las pacientes del servicio de Ginecología representaron el 26.11 %, con un promedio de edad de 41.46 ± 11.41 años y las de Obstetricia el 73.89 % con una edad media de 28.62 ± 6.12 años. La instrucción más frecuente fue la secundaria, en la mayoría casadas y residen en el área urbana. El diagnóstico ginecológico más frecuente al ingreso y al egreso fue la hiperplasia endometrial con el 26.1 % en ambos casos. En el área Obstétrica, el embarazo a término sin complicaciones representó más del 50% de los diagnósticos tanto al ingreso como al egreso. La histerectomía fue el procedimiento ginecológico realizado con mayor frecuencia (25 %). La cesárea es el procedimiento obstétrico más frecuente (45.26 %). Conclusiones. Se obtienen valoraciones altas en la satisfacción de la atención médica en más del 65 % de las pacientes encuestadas tanto del personal médico como del personal de enfermería.
