Modern thinking about human nature is notoriously divided between two contradictory notions: The Hobbesian tradition portrays men as driven by selfish desires, while the Rousseauian tradition recognizes altruistic proclivities as truemotivations to cooperate. We tested preschoolers' predictions about the prosocial or antisocial manner in which people would behave toward each other. Four stories were presented to 3- and 4-year-old children. In each story, the protagonists could either cooperate, act in terms of their own interests, or adopt a behavior unrelated to the ongoing scenario. Children as young as 3 years of age expected the protagonists to behave prosocially - and even more so if the protagonists were female. The results suggest that, even at an early age, children are inclined to adopt a "Rousseau-like" stance rather than a "suspicious" or "pessimistic" Hobbesian stance.

Background: Intravenous thrombolysis with alteplase for ischemic stroke is fixed at a maximal dose of 90 mg for safety reasons. Little is known about the clinical outcomes of stroke patients weighing >100 kg, who may benefit less from thrombolysis due to this dose limitation. Methods: Prospective data on 1,479 consecutive stroke patients treated with intravenous alteplase in six Swiss stroke units were analyzed. Presenting characteristics and the frequency of favorable outcomes, defined as a modified Rankin scale (mRS) score of 0 or 1, a good outcome (mRS score 0-2), mortality and symptomatic intracranial hemorrhage (SICH) were compared between patients weighing >100 kg and those weighing ≤100 kg. Results: Compared to their counterparts (n = 1,384, mean body weight 73 kg), patients weighing >100 kg (n = 95, mean body weight 108 kg) were younger (61 vs. 67 years, p < 0.001), were more frequently males (83 vs. 60%, p < 0.001) and more frequently suffered from diabetes mellitus (30 vs. 13%, p < 0.001). As compared with patients weighing ≤100 kg, patients weighing >100 kg had similar rates of favorable outcomes (45 vs. 48%, p = 0.656), good outcomes (58 vs. 64%, p = 0.270) and mortality (17 vs. 12%, p = 0.196), and SICH risk (1 vs. 5%, p = 0.182). After multivariable adjustment, body weight >100 kg was strongly associated with mortality (p = 0.007) and poor outcome (p = 0.007). Conclusion: Our data do not suggest a reduced likehood of favorable outcomes in patients weighing >100 kg treated with the current dose regimen. The association of body weight >100 kg with mortality and poor outcome, however, demands further large-scale studies to replicate our findings and to explore the underlying mechanisms.

Depuis une cinquantaine d'années, nombreuses sont les études qui s'intéressent aux interactions entre les milieux de vie et leur impact sur la santé. Deux axes principaux se dégagent : l'un focalisé sur le conflit travail-famille en termes de notions liées au stress, et l'autre faisant la promotion du bien-être via l'équilibre travail-vie privée. Cependant, peu de travaux portent sur la dimension contextualisée de ce phénomène, dans son rapport à l'activité quotidienne concrète. Cette étude est consacrée à l'articulation travail - vie privée et son influence sur le bien-être subjectif (corporo-socio-psychologique) chez des femmes cadres en Suisse romande. Cette population est un terrain privilégié pour comprendre le sens donné à l'activité à partir des contraintes, responsabilités, et sollicitations perçues dans des contextes différents. Abordée selon une perspective qualitative en psychologie de la santé, notre approche permet de définir l'expérience plurielle de ces femmes par rapport aux divers groupes d'appartenance et par rapport à leur propre corporéité. Ces résultats ouvrent de nouvelles perspectives de recherche et d'intervention, notamment dans le domaine de la santé au travail.

BACKGROUND: In this study, we aimed at assessing Inflammatory Bowel Disease patients' needs and current nursing practice to investigate to what extent consensus statements (European Crohn's and Colitis Organization) on the nursing roles in caring for patients with IBD concur with local practice. METHODS: We used a mixed-method convergent design to combine quantitative data prospectively collected in the Swiss IBD cohort study and qualitative data from structured interviews with IBD healthcare experts. Symptoms, quality of life, and anxiety and depression scores were retrieved from physician charts and patient self-reported questionnaires. Descriptive analyses were performed based on quantitative and qualitative data. RESULTS: 230 patients of a single center were included, 60% of patients were males, and median age was 40 (range 18-85). The prevalence of abdominal pain was 42%. Self-reported data were obtained from 75 out of 230 patients. General health was perceived significantly lower compared with the general population (p < 0.001). Prevalence of tiredness was 73%; sleep problems, 78%; issues related to work, 20%; sexual constraints, 35%; diarrhea, 67%; being afraid of not finding a bathroom, 42%; depression, 11%; and anxiety symptoms, 23%. According to experts' interviews, the consensus statements are found mostly relevant with many recommendations that are not yet realized in clinical practice. CONCLUSION: Identified prevalence may help clinicians in detecting patients at risk and improve patient management. © 2015 S. Karger AG, Basel.

OBJECTIVE: To assess the iodine status of Swiss population groups and to evaluate the influence of iodized salt as a vector for iodine fortification. DESIGN: The relationship between 24 h urinary iodine and Na excretions was assessed in the general population after correcting for confounders. Single-day intakes were estimated assuming that 92 % of dietary iodine was excreted in 24 h urine. Usual intake distributions were derived for male and female population groups after adjustment for within-subject variability. The estimated average requirement (EAR) cut-point method was applied as guidance to assess the inadequacy of the iodine supply. SETTING: Public health strategies to reduce the dietary salt intake in the general population may affect its iodine supply. SUBJECTS: The study population (1481 volunteers, aged ≥15 years) was randomly selected from three different linguistic regions of Switzerland. RESULTS: The 24 h urine samples from 1420 participants were determined to be properly collected. Mean iodine intakes obtained for men (n 705) and women (n 715) were 179 (sd 68.1) µg/d and 138 (sd 57.8) µg/d, respectively. Urinary Na and Ca, and BMI were significantly and positively associated with higher iodine intake, as were men and non-smokers. Fifty-four per cent of the total iodine intake originated from iodized salt. The prevalence of inadequate iodine intake as estimated by the EAR cut-point method was 2 % for men and 14 % for women. CONCLUSIONS: The estimated prevalence of inadequate iodine intake was within the optimal target range of 2-3 % for men, but not for women.

Työssä tutkittiin typen oksidien (NOx) päästöjen muodostumista ja NOx-päästöjen poistomenetelmiä leijukatalyyttisessa krakkausyksikössä (FCC). Koeajossa Porvoon öljynjalostamon FCC-yksikössä tutkittiin NOx-päästöjen hallintaa CO-promoottorin (CO:n palamista edistävä lisäaine) annostelun avulla. Kirjallisuusosassa käsiteltiin NOx-päästöjen muodostumista FCC-yksikön regeneraattorissa. Katalyyttisten lisäaineiden vaikutusta NOx-päästöjen muodostumiseen tarkasteltiin laajasti. Katalyyttisten lisäaineiden ohella käsiteltiin muitakin NOx-päästöjen poistomenetelmiä. Koeajojen tarkoituksena oli optimoida FCC-yksikön katalyyttikiertoon annosteltavan CO-promoottorin määrä. CO-promoottorin sisältämä platina lisää NOx-päästöjen muodostumista regeneraattorissa, mutta vähentää jälkipalamista. Jälkipalamisen vuoksi regeneraattorin ylimenolinjan lämpötila nousee. Regeneraattorin ylimenolinjan lämpötila oli CO-promoottorin vähentämistä rajoittava tekijä, NOx-päästöjä pyrittiin vähentämään ilman merkittävää jälkipalamisen kasvua. Tuorekatalyytin platinapitoisuuden pienentäminen vähentää merkittävästi NOx-päästöjä.

A produtividade em bananeira é um caráter complexo que resulta da associação de diferentes fatores, influenciados pelo ambiente. Este trabalho teve por objetivo estudar os efeitos diretos e indiretos de características vegetativas sobre a produção de bananeira em três ciclos. O trabalho foi conduzido no campo experimental da Embrapa Acre, em Rio Branco, Acre, entre os anos de 2003 e 2006. Foram avaliados 13 genótipos de bananeira (Preciosa, Japira, Pacovan-Ken, Pacovan, BRS Platina, Prata-Anã, ST12-31, Nanicão, Grande Naine, Calipso, Ambrosia, Bucaneiro e FHIA-02), no espaçamento de 3 m x 2 m, no esquema de blocos ao acaso, com cinco repetições. Determinaram-se a altura de plantas, diâmetro do pseudocaule, número de folhas na floração e na colheita, peso do cacho e número de pencas no cacho. Realizou-se a análise de variância para cada ciclo e estimaram-se os coeficientes de correlação fenotípicos e os coeficientes de trilha. Adotou-se como variável básica o peso do cacho, e as demais, como variáveis explicativas. O estudo dos efeitos diretos e indiretos evidenciou que o caráter produção é bastante influenciado pelo ambiente no primeiro ciclo, e, nos ciclos seguintes, a produção é muito influenciada pelas características vegetativas, principalmente número de folhas na colheita.

Existe grande número de cultivares de bananeira no Brasil, porém quando se consideram aspectos como preferência dos consumidores, produtividade, tolerância às pragas e doenças, porte adequado e resistência à seca e ao frio, restam poucas com potencial agronômico para serem usadas comercialmente. Objetivando avaliar o desenvolvimento vegetativo de genótipos de bananeiras nas condições edafoclimáticas do Vale do Ribeira (Brasil), foram testados os seguintes materiais, separados em dois grupos genômicos: AAAA (Bucaneiro, FHIA 02 e FHIA 17); e AAAB, sendo este subdividido de acordo com a genitora utilizada no melhoramento: Prata (BRS Garantida, FHIA 18, BRS FHIA Maravilha, BRS Platina e PA94-01); Pacovan (BRS Japira, BRS Pacovan Ken, PV79-34, PV94-01 e BRS Vitória); e Yangambi n.2 (BRS Tropical, BRS Princesa e YB42-03). As cultivares Grande Naine (AAA), Pacovan (AAB), Prata-anã (AAB) e Yangambi ou Caipira (AAA) apresentam desenvolvimento e produção semelhantes aos genótipos avaliados, portanto foram utilizadas como padrão comparativo. Durante dois ciclos, foram avaliadas as seguintes características de desenvolvimento: altura, diâmetro do pseudocaule, número de folhas ativas (florescimento e colheita), intervalo entre plantio e florescimento, e entre plantio e colheita (dias). Calcularam-se os intervalos de confiança (média ± erro-padrão) nos diferentes grupos e tipos de banana. Diante dos resultados obtidos conclui-se que os genótipos do grupo AAAB e genitora 'Pacovan' não são adequados para o cultivo na região devido ao porte alto das plantas, e os genótipos de bananeiras que possuem potencial de cultivo na região do Vale do Ribeira são: grupo AAAA (FHIA 02 e FHIA 17); grupo AAAB: genitora 'Prata' (FHIA 18, BRS Garantida e PA94-01) e genitora 'Yangambi n.2' (BRS Tropical, BRS Princesa e Yangambi).

RESUMOO norte de Minas Gerais é grande produtor de banana ‘Prata-Anã’ irrigada, cultura altamente suscetível ao Mal-do-Panamá. O uso de genótipos resistentes é uma alternativa, mas os frutos devem apresentar características pós-colheita o mais próximo possível da ‘Prata-Anã’, para melhor aceitação pelos consumidores. O objetivo do trabalho foi caracterizar frutos em pós-colheita, identificar a preferência e a intenção de compra de diferentes genótipos de bananeira tipo Prata. Os genótipos Prata-Anã, BRS Platina e Fhia-18. foram caracterizados no ponto de colheita (verdes) e maduros (estádio seis de maturação), por avaliações químicas, físicas e sensoriais. Quando verde, ‘BRS Platina’ apresentou maior massa fresca e tamanho que ‘Fhia-18’ e ‘Prata-Anã’. ‘Fhia-18.’ teve a tonalidade verde da casca mais intensa que a dos demais genótipos. Madura, ‘BRS Platina’ foi mais firme, mas com a mesma resistência ao despencamento que ‘Fhia-18’ e superior à ‘Prata-Anã’. ‘Fhia-18’ apresentou cor da casca com amarelo mais clara e tão brilhante quanto da ‘Prata-Anã’, mas ‘BRS Platina’ teve a tonalidade de amarelo mais intensa. Bananas ‘Fhia-18’ foram mais ácidas, ‘BRS Platina’, com menor acidez titulável, e ‘Prata-Anã’, o maior teor de sólidos solúveis. Os genótipos Prata-Anã e BRS Platina tiveram maior preferência e intenção de compra pelos consumidores, sendo as bananas ‘Prata-Anã’ em dedos e ‘BRS Platina’ e ‘Fhia-18’ em dedos, buquê e penca, as mais preferidas. Entretanto, a maioria compraria bananas ‘Prata-Anã’ em buquê e ‘BRS Platina’ e ‘Fhia-18’ em penca. Enquanto verdes, os genótipos foram semelhantes à ‘Prata-Anã’, e maiores diferenças químicas e físicas ocorreram quando maduros.

NlmCategory="UNASSIGNED">Crohn's disease (CD) evolution is characterized by increasing proportions of patients developing complications such as strictures, abscesses and fistulas that require surgical management. After resection of a diseased intestinal segment, CD recurrence concerns up to 60% of patients within a year post surgery. The mucosa just above the site of the intestinal anastomosis is at particularly high risk of relapse. Prophylactic medical therapy to prevent recurrence has been shown to be effective with a variety of medications, but the recurrence rate remains high, demanding that a better risk stratification of patients be achieved. Recognized risk factors for postsurgical CD recurrence include young age at diagnosis and at surgery, smoking, need for repeated surgeries and penetrating disease. These patients require full dose immunosuppressive or anti-tumor necrosis factor (anti-TNF) therapy, which should be initiated in the immediate postoperative period, to prevent the onset of an inflammatory activity in the bowel. Systematic follow-up by endoscopy to monitor treatment benefit should also be part of the management, as endoscopic recurrence heralds clinical relapse in these patients. The role of noninvasive markers of mucosal inflammation, such as stool calprotectin levels, show promise to complete this monitoring. Although the efficacy of mesalazine and imidazole antibiotics has been long recognized, more aggressive approaches, such as thiopurines and anti-TNF antibodies, have shown higher efficacies in direct comparison trials. The potential place of anti-homing agents is not yet defined, but these agents should in principle be of interest for this prophylactic indication due to their mode of action and interesting side-effect profile. The current recommendations are based on a step-up approach that includes immunosuppressors and/or imidazole antibiotics, followed by an anti-TNF agent, such as infliximab and adalimumab, both already tested in randomized trials in this indication. When endoscopic recurrence is identified during follow-up, upscaling to anti-TNF or dose escalation is advocated.

OBJECTIVE: Several smaller single-center studies have reported a prognostic role for Ki-67 labeling index in prostate cancer. Our aim was to test whether Ki-67 is an independent prognostic marker of biochemical recurrence (BCR) in a large international cohort of patients treated with radical prostatectomy (RP). METHODS: Ki-67 immunohistochemical staining on prostatectomy specimens from 3,123 patients who underwent RP for prostate cancer was retrospectively performed. Univariable and multivariable Cox regression models were used to assess the association of Ki-67 status with BCR. RESULTS: Ki-67 positive status was observed in 762 (24.4 %) patients and was associated with lymph node involvement (LNI) (p = 0.039). Six hundred and twenty-one (19.9 %) patients experienced BCR. The estimated 3-year biochemical-free survivals were 85 % for patients with negative Ki-67 status and 82.1 % for patients with positive Ki-67 status (log-rank test, p = 0.014). In multivariable analysis that adjusted for the effects of age, preoperative PSA, RP Gleason sum, seminal vesicle invasion, extracapsular extension, positive surgical margins, lymphovascular invasion, and LNI, Ki-67 was significantly associated with BCR (HR = 1.19; p = 0.019). Subgroup analysis revealed that Ki-67 is associated with BCR in patients without LNI (p = 0.004), those with RP Gleason sum 7 (p = 0.015), and those with negative surgical margins (p = 0.047). CONCLUSION: We confirmed Ki-67 as an independent predictor of BCR after RP. Ki-67 could be particularly informative in patients with favorable pathologic characteristics to help in the clinical decision-making regarding adjuvant therapy and optimized follow-up scheduling.

BACKGROUND/AIMS: The determinants of a healthy diet have not been studied in Switzerland. This study aimed at assessing the individual and behavioural factors associated with a healthy diet in a Swiss city. METHODS: Cross-sectional, population-based study conducted between 2009 and 2013 (n = 4,439, 2,383 women, mean age 57.5 ± 10.3 years) in Lausanne. Food consumption was assessed using a validated food frequency questionnaire. Two Mediterranean diet scores (classic score and specific for Switzerland) and the Harvard School of Public Health alternate healthy eating index were computed. RESULTS: For all three dietary scores considered, living in couple or having a high education were associated with a healthier diet. An unhealthy lifestyle (smoking, sedentary behaviour) or a high body mass index were associated with an unhealthier diet. Participants born in Italy, Portugal and Spain had healthier diets than participants born in France or Switzerland. Women and elderly participants had healthier diets than men and young participants according to 2 scores, while no differences were found for the Swiss-specific Mediterranean score. CONCLUSIONS: In Switzerland, healthy eating is associated with high education, a healthy lifestyle, marital status and country of origin. The associations with gender and age depend on the dietary score considered.
