A presente pesquisa propôs-se investigar a efetividade dos acordos de pesca na Amazônia enquanto instrumento de gestão participativa através das populações residentes nas comunidades do Pixuna e Jarí do Socorro no município de Santarém-Pa. Para tanto, analisou-se como as referidas comunidades veem os acordos de pesca vigente, já que as mesmas são consideradas agroextrativistas e que possui na atividade pesqueira sua fonte de renda, proteína e ao longo dos anos vem praticando o mecanismo do acordo de pesca como forma de gestão desta atividade. Também buscou-se entrevistar representantes de ONGs como o IPAM enquanto organização responsável em divulgar de uma maneira mais simples para os pescadores o que vem a ser na forma da Lei e na prática a Instrução Normativa do IBAMA Nº 29/2002 que trata da criação dos acordos de pesca; a Colônia de Pescadores Z-20 representante legal e responsável em defender os interesses e direitos dos trabalhadores do setor artesanal da pesca local perante os órgãos públicos e a sociedade e o IBAMA representante oficial do poder publico, gestor e fiscalizador de quaisquer ações de pesca predatória e de degradação do meio ambiente evidenciados nos acordos implantados nas comunidades. A pesquisa descreve o ambiente de várzea como cenário para essas relações que se estabelecem a partir do uso de um recurso de acesso livre a todos como é o caso do pescado. No que diz respeito à metodologia as técnicas utilizadas se constituíram em: observação direta, aplicação de entrevistas semi-estruturadas e questionários. Foram apresentadas as características da pesquisa, os seus sujeitos e as informações relativas à coleta e à análise dos dados. Assim, o estudo caracterizou-se como qualitativo. Os dados foram analisados através de procedimentos estatísticos básicos com a utilização do software SPSS a partir de frequências e estimativas de parâmetros descritivos, com representação de tendências através de tabelas e gráficos, que tem como finalidade uma melhor visualização destas análises e observações para o contexto da pesquisa. Por fim, foram feitas as considerações finais do estudo, onde foi identificado que a falta de articulação entre os comunitários residentes nas localidades, e das comunidades com o poder publico podem, ser apontados como importantes fatores na decadência observada nos acordos de pesca vigente; outro ponto interessante e de importância considerável é a ausência do órgão responsável pela fiscalização das regras encontradas nos acordos de pesca, o IBAMA. Diante disso, foram sugeridas algumas recomendações aos membros das organizações analisadas.

O artigo busca discutir algumas das principais políticas de urbanização desenvolvidas ao longo da década de 1990 por governos locais de capitais brasileiras com participação social e parceria público-privado, que definiram a agenda de gestão urbana no Brasil. Com base nos casos de Recife e Salvador, pretende-se entender os fatores políticos que tornaram diferentes os processos de implementação de políticas semelhantes de urbanização nas duas capitais.

Discussions about the new meaning of citizenship, valuing differences and respecting knowledge diversity bring the question of how public policies can be locally rooted to ensure access to diversity. This is evident when we deal with health policies, where the interaction between implementers and beneficiaries is essential to understand the results of the policy. The Family Health Program (FHP) has tried to change the relationship between state and society, bringing health professionals with the daily experienced by the users where there is (re) production of the components that lead to insecurity, poor health and disease. To analyse this kind of policy we must take into account the interaction processes and the practices of the actors involved in the FHP implementation. This article aims to analyze the role of Health Communitarian Agents (HCA) as FHP implementers. Through ethnographic research followed the practices of ACS in different cities, we seek to understand how they deal with their different knowledge, enable and disable referrals and adapt action to enact the program. We want to understand how they use mediations and interactions in their practices and built policies locally rooted, constructing alternative ways to implement these policies.

A complexidade do desenvolvimento sustentável configura um paradigma em que são necessárias ações efetivas de gestão ambiental que, em última instância, têm relação com a formulação e a implementação de políticas públicas. Para que haja uma efetiva gestão ambiental no âmbito municipal é necessária a participação da sociedade nos processos de gestão. Logo, os conselhos municipais do meio ambiente são peças fundamentais na promoção da gestão ambiental local com vistas a uma melhor qualidade de vida. A literatura enfatiza que a efetividade dessas instâncias está ligada, principalmente, à sua estrutura e funcionamento. Assim sendo, essa pesquisa objetivou a identificação de oportunidades de melhoria na estrutura e no funcionamento do Conselho Municipal de Gestão e Saneamento Ambiental de Santo André (COMUGESAN) como uma contribuição para sua atuação na gestão ambiental. Para tanto, utilizou-se a pesquisa bibliográfica, documental e qualitativa, no ano de 2008, caracterizando-se como estudo de caso exploratório no Município de Santo André, SP. Observou-se que a atuação de Conselhos Municipais do Meio Ambiente na gestão ambiental transcende as ações de tomada de decisão, formulação e implementação das políticas locais, constituindo-se, sobretudo, como espaço educativo e de construção da cidadania. Como oportunidades de melhoria, identificaram-se seis aspectos primordiais a serem fortalecidos em sua atuação: estrutura; composição e representatividade; capacitação; educação em meio ambiente; informação; avaliação constante. Com os resultados obtidos, sugere-se que o COMUGESAN adote atividades de capacitação para aperfeiçoar a representatividade, instituindo cursos de formação de conselheiros e atividades em educação continuada, bem como a realização de diagnósticos internos.

This doctoral work gains deeper insight into the dynamics of knowledge flows within and across clusters, unfolding their features, directions and strategic implications. Alliances, networks and personnel mobility are acknowledged as the three main channels of inter-firm knowledge flows, thus offering three heterogeneous measures to analyze the phenomenon. The interplay between the three channels and the richness of available research methods, has allowed for the elaboration of three different papers and perspectives. The common empirical setting is the IT cluster in Bangalore, for its distinguished features as a high-tech cluster and for its steady yearly two-digit growth around the service-based business model. The first paper deploys both a firm-level and a tie-level analysis, exploring the cases of 4 domestic companies and of 2 MNCs active the cluster, according to a cluster-based perspective. The distinction between business-domain knowledge and technical knowledge emerges from the qualitative evidence, further confirmed by quantitative analyses at tie-level. At firm-level, the specialization degree seems to be influencing the kind of knowledge shared, while at tie-level both the frequency of interaction and the governance mode prove to determine differences in the distribution of knowledge flows. The second paper zooms out and considers the inter-firm networks; particularly focusing on the role of cluster boundary, internal and external networks are analyzed, in their size, long-term orientation and exploration degree. The research method is purely qualitative and allows for the observation of the evolving strategic role of internal network: from exploitation-based to exploration-based. Moreover, a causal pattern is emphasized, linking the evolution and features of the external network to the evolution and features of internal network. The final paper addresses the softer and more micro-level side of knowledge flows: personnel mobility. A social capital perspective is here developed, which considers both employees’ acquisition and employees’ loss as building inter-firm ties, thus enhancing company’s overall social capital. Negative binomial regression analyses at dyad-level test the significant impact of cluster affiliation (cluster firms vs non-cluster firms), industry affiliation (IT firms vs non-IT fims) and foreign affiliation (MNCs vs domestic firms) in shaping the uneven distribution of personnel mobility, and thus of knowledge flows, among companies.

INTRODUCTION: Urogenital prolapse is a very common condition in women with a prevalence of 30%. If conservative therapy fails or is not desired by the patient, prolapse repair is usually performed under general or regional anaesthetic. The aim of the study was to evaluate feasibility, efficacy and functional outcome after fascial prolapse repairs under local anaesthetic (LA). PATIENTS AND METHODS: Between November 1999 and December 2000, 130 consecutive patients presenting with anterior or posterior prolapse or both were invited to have their procedure performed under LA. All patients with a symptomatic minimum stage II prolapse were included. Prior to surgery all women completed a standardized questionnaire examining the specific and non-specific symptoms of prolapse and their situation was classified using the ICS Pelvic Organ Prolapse (POP-Q) system. Follow up was 30 months. Objective success was defined as a stage 1 or less and no symptoms of bulge, subjective success was defined as lack of specific or non-specific symptoms of prolapse. RESULTS: There were 128 patients who agreed to have their operations performed under LA: 68 in the anterior group, 52 in the posterior group and 8 with a combined anterior and posterior repair. Objective cure rate was 88% for posterior repair, 87% for anterior repair and 63% for combined repair. Success rates were no different in primary from recurrent cases. There were no intraoperative complications and operating time was 21 min (anterior repair) or 23 min (posterior repair). There was no de novo postoperative urinary or stool incontinence and all patients but two would have the operation performed again under the same circumstances. The two remaining refused due to embarrassment but for no other reason. CONCLUSION: Local anaesthetic prolapse repair is feasible and effective in middle term results. It is well accepted by the patients who benefit from less side effects and short hospital stay.

Autogenous iliac crest has long served as the gold standard for anterior lumbar arthrodesis although added morbidity results from the bone graft harvest. Therefore, femoral ring allograft, or cages, have been used to decrease the morbidity of iliac crest bone harvesting. More recently, an experimental study in the animal showed that harvesting local bone from the anterior vertebral body and replacing the void by a radio-opaque beta-tricalcium phosphate plug was a valid concept. However, such a concept precludes theoretically the use of posterior pedicle screw fixation. At one institution a consecutive series of 21 patients underwent single- or multiple-level circumferential lumbar fusion with anterior cages and posterior pedicle screws. All cages were filled with cancellous bone harvested from the adjacent vertebral body, and the vertebral body defect was filled with a beta-tricalcium phosphate plug. The indications for surgery were failed conservative treatment of a lumbar degenerative disc disease or spondylolisthesis. The purpose of this study, therefore, was to report on the surgical technique, operative feasibility, safety, benefits, and drawbacks of this technique with our primary clinical experience. An independent researcher reviewed all data that had been collected prospectively from the onset of the study. The average age of the patients was 39.9 (26-57) years. Bone grafts were successfully harvested from 28 vertebral bodies in all but one patient whose anterior procedure was aborted due to difficulty in freeing the left common iliac vein. This case was converted to a transforaminal interbody fusion (TLIF). There was no major vascular injury. Blood loss of the anterior procedure averaged 250 ml (50-350 ml). One tricalcium phosphate bone plug was broken during its insertion, and one endplate was broken because of wrong surgical technique, which did not affect the final outcome. One patient had a right lumbar plexopathy that was not related to this special technique. There was no retrograde ejaculation, infection or pseudoarthrosis. One patient experienced a deep venous thrombosis. At the last follow up (mean 28 months) all patients had a solid lumbar spine fusion. At the 6-month follow up, the pain as assessed on the visual analog scale (VAS) decreased from 6.9 to 4.5 (33% decrease), and the Oswestry disability index (ODI) reduced from 48.0 to 31.7 with a 34% reduction. However, at 2 years follow up there was a trend for increase in the ODI (35) and VAS (5). The data in this study suggest that harvesting a cylinder of autograft from the adjacent vertebral body is safe and efficient. Filling of the void defect with a beta-tricalcium phosphate plug does not preclude the use of posterior pedicle screw stabilization.
