Individualmente nenhuma modalidade imagiológica oferece em simultâneo uma elevada resolução espacial e temporal, aliado a uma boa resolução de contraste e sensibilidade. No entanto, combinações de diferentes modalidades podem aproximar-se deste ideal, permitindo a integração de diferentes sistemas num sistema de imagem única. Um dos marcos históricos da medicina nuclear ocorreu com o aparecimento dos sistemas de Tomografia por Emissão de Positrões/Tomografia Computorizada (PET/CT) que permitem obter, em simultâneo, informações funcionais e anatómicas sobre as alterações metabólicas e estruturais que ocorrem em situações de doença.

The purpose of our study was to evaluate the accuracy of dynamic incremental bolus-enhanced conventional CT (DICT) with intravenous contrast administration, early phase, in the diagnosis of malignancy of focal liver lesions. A total of 122 lesions were selected in 74 patients considering the following criteria: lesion diameter 10 mm or more, number of lesions less than six per study, except in multiple angiomatosis and the existence of a valid criteria of definitive diagnosis. Lesions were categorized into seven levels of diagnostic confidence of malignancy compared with the definitive diagnosis for acquisition of a receiver-operator-characteristic (ROC) curve analysis and to determine the sensitivity and specificity of the technique. Forty-six and 70 lesions were correctly diagnosed as malignant and benign, respectively; there were 2 false-positive and 4 false-negative diagnoses of malignancy and the sensitivity and specificity obtained were 92 and 97%. The DICT early phase was confirmed as a highly accurate method in the characterization and diagnosis of malignancy of focal liver lesions, requiring an optimal technical performance and judicious analysis of existing semiological data.

OBJETIVO: Testar o desempenho dos parâmetros diretos do duplex scan no diagnóstico da estenose da artéria renal (EAR) e verificar se os pontos de corte recomendados pela literatura são os mais adequados para se discriminar a gravidade da lesão. MÉTODOS: Estudo prospectivo, incluindo 62 pacientes portadores de EAR, submetidos ao duplex scan, seguido da arteriografia seletiva. O pico de velocidade sistólico (PVS) e a relação renal-aorta (RRA) foram mensurados. A análise estatística incluiu a curva ROC (receiver operating characteristic curve), t test student não pareado a sensibilidade, especificidade, os valores preditivos positivo e negativo, e a acurácia. RESULTADOS: A arteriografia revelou EAR 0-59% em 31 artérias (24%); EAR 60-99% em 91 artérias (72%) e 5 oclusões (4%). A análise de ROC mostrou que o PVS e a RRA apresentaram desempenho semelhante na detecção da lesão, cujas áreas sob as curvas foram 0,96 e 0,95, respectivamente. Considerando os pontos de corte recomendados pela literatura, o PVS de 180 cm/s apresentou sensibilidade de 100% e especificidade de 81%, enquanto que a RRA de 3,5 apresentou sensibilidade de somente 79%, com 93% de especificidade. Estes parâmetros foram analisados de forma conjugada (critério direto), revelando 79% de sensibilidade e 97% de especificidade. Os pontos de corte otimizados foram: PVS de 189 cm/s e RRA de 2,6, demonstrando 100%, 87%, 96% e 87% de sensibilidade e especificidade para o PVS e para a RRA, respectivamente. CONCLUSÃO: O uso isolado do PVS otimizado apresentou o melhor desempenho na detecção e na graduação da EAR.

AbstractIntroduction:Coronary computed tomography angiography (CCTA) allows for non-invasive coronary artery disease (CAD) phenotyping. There are still some uncertainties regarding the impact this knowledge has on the clinical care of patients.Objective:To determine whether CAD phenotyping by CCTA influences clinical decision making by the prescription of cardiovascular drugs and their impact on non-LDL cholesterol (NLDLC) levels.Methods:We analysed consecutive patients from 2008 to 2011 submitted to CCTA without previous diagnosis of CAD that had two serial measures of NLDLC, one up to 3 months before CCTA and the second from 3 to 6 months after.Results:A total of 97 patients were included, of which 69% were men, mean age 64 ± 12 years. CCTA revealed that 18 (18%) patients had no CAD, 38 (39%) had non-obstructive (< 50%) lesions and 41 (42%) had at least one obstructive ≥ 50% lesion. NLDLC was similar at baseline between the grups (138 ± 52 mg/dL vs. 135 ± 42 mg/dL vs. 131 ± 44 mg/dL, respectively, p = 0.32). We found significative reduction in NLDLC among patients with obstrctive lesions (-18%, p = 0.001). We also found a positive relationship between clinical treatment intensification with aspirin and cholesterol reducing drugs and the severity of CAD.Conclusion:Our data suggest that CCTA results were used for cardiovascular clinical treatment titration, with especial intensification seen in patients with obstructive ≥50% CAD.

Die Mesostruktur von Werkstoffen wie Faserverbundwerkstoffen und Schaumstoffen beeinflusst deren mechanisches Verhalten wesentlich. Die inneren Strukturen des Werkstoffes müssen in die Analyse des Verformungs- und Versagensverhaltens einbezogen werden. Die Röntgenanalyse, insbesondere die Röntgen-Computertomographie (Röntgen-CT) ermöglicht eine zerstörungsfreie 3-dimensionale (3-d) Visualisierungund Charakterisierung innerer Strukturen. Für die Untersuchung des Einflusses der Mesostruktur eines Polymethacrymlid-Hartschaumstoffes auf das mechanische Verhalten speziell unter zugbelastung sollte eine speziell vom Fraunhofer-Institut für Werkstoffmechanik in Halle entwickelte In-situ-Verformungseinrichtung erprobt werden. Die Anlage wurde speziell für den Zugversuch optimiert. Es folgten mechanische Experimente, die mit den 3d-Bilddaten der Zellstruktur des Schaumstoffes korrelieren. Die ermittelten Bilddaten werden mit Hilfe einer Bildanalysesoftware ausgewertet und Rückschlüsse auf Verformungs- und Versagensverhalten grtroffen.

Purpose: The aim of this educational poster is to introduce the technical principles of cerebral perfusion CT and to provide examples of its clinical applications and potential limitations in the everyday emergency practice. Methods and materials: Cerebral perfusion CT is a well established investigatory tool for many vascular and parenchymal brain dysfunctions. CT perfusion maps allow a semiquantitative assessment of cerebral perfusion. Results: Currently, cerebral perfusion CT has a pivotal role in differentiating reversible from irreversible ischemic parenchymal insult besides its integral role in grading vasospasm after subarachnoid hemorrhage. Furthermore, cerebral perfusion CT can be coupled to acetazolamide administration in order to assess the cerebrovascular reserve capacity before performing extra-/intra-cranial bypass surgery in patients with cerebral vascular insufficiency. Cerebral perfusion CT can also identify diffuse abnormalities of cerebral perfusion in children with traumatic brain injury showing a low initial GCS in order to predict the final outcome regarding the late occurrence of irreversible parenchymal damage. Cerebral Perfusion CT is also able to detect focal parenchymal perfusion abnormalities in acute epileptic seizures. Conclusion: Cerebral perfusion CT can be integrated in the management of many vascular, traumatic and functional disorders of the brain.
