The influence of complex plaque morphology on the extent of demand-induced ischemia in unselected patients is not well defined. We sought to investigate the functional significance of lesion morphology in patients who underwent coronary angiography and dobutamine stress echocardiography (DSE).,Angiography and DSE were performed within a 6-month period (mean 1 +/- 1 month) in 196 patients. Angiographic assessments involved quantification of stenosis severity, assessment of the extent of jeopardized myocardium, and categorization of plaque morphology according to the Ambrose classification. DSE was interpreted by separate investigators with respect to wall motion score index (WMSI) and number of coronary territories involved. A general linear model was constructed to assess,the independent contribution of patient characteristics and angiographic and DSE results with respect to extent of ischemic myocardium. Complex lesion morphology was seen in 62 patients (32%). Patients with complex lesions were more likely to have had prior myocardial infarction (p < 0.001) and be current smokers (p = 0.03). During angiography, they exhibited a trend toward a greater number of diseased vessels, had a greater coronary jeopardy score (p < 0.001) and more frequent collateral flow (p = 0.03). During echocardiography, patients had a higher stress WMSI (p < 0.001) and were more likely to show ischemia in all 3 arterial territories (p < 0.01). On multivariate regression, the coronary artery jeopardy score and the presence of complex plaque morphology were independent predictors of the extent of ischemic myocardium (R 2 = 34%, p < 0.001). Thus, patients with complex plaque morphology are older, more likely to smoke, and more likely to have had prior myocardial. infarction. They exhibit more extensive disease with higher coronary jeopardy scores and a higher resting and peak stress WMSI. Background: The surgical cure rate for primary hyperparathyroidism is greater than 95%. For those who have recurrent or persistent disease, preoperative localization improves reoperation success rates. Selective parathyroid venous sampling (SPVS) for intact parathyroid hormone is particularly useful when non-invasive localization techniques are negative or inconclusive. Methods: We present all known cases (n = 13) between 1994 and 2002 who had venous sampling for localization at our institution prior to reoperation for recurrent or persistent primary hyperparathyroidism. Comparison was made with non-invasive localization procedures. Results of invasive and non-invasive localization were correlated with surgical findings. Results: Of the nine reoperated cases, eight had positive correlations between SPVS and operative findings and histopathology. SPVS did not reveal the parathyroid hormone source in one case with negative non-invasive localization procedures. Comparisons between SPVS, computerized tomography (CT), and parathyroid scintigraphy (MIBI) as expressed in terms of true positive (TP), false positive (FP) and false negative (FN) were: SPVS - TP 88.8%, FP 0%, FN 11.1%; CT - TP 22.2%, FP 22.2%, FN 55.5%; and MIBI - TP 33.3%, FP 0%, FN 66.6%. At least seven of the nine operated cases have been cured; another remained normocalcaemic 2 weeks after subtotal parathyroidectomy. Conclusion: In our institution SPVS has proven to be a valuable tool in cases with recurrent or persistent primary hyperparathyroidism and negative non-invasive localization procedures.

Background Latent left ventricular (LV) dysfunction in patients with valvular or myocardial disease may be identified by loss of contractile reserve (CR) at exercise echocardiography. Contraction in the LV longitudinal axis may be more sensitive than radial contraction to minor disturbances of LV function. We sought to determine whether tissue Doppler measurement of longitudinal function could be used to identify CR. Methods Exercise echocardiography was performed in 86 patients (20 women, age 53 +/- 18 years), 72 with asymptomatic or minimally symptomatic mitral regurgitation, and 14 normal controls. Pulsed-wave tissue Doppler imaging (DTI) was used to measure maximum annular systolic velocity at rest and stress. Inducible ischemia was excluded by analysis of wall motion by an experienced observer. CR was defined by greater than or equal to5% improvement of stress compared with rest ejection fraction (EF). Exercise capacity was assessed from expired gas analysis. The work presented herein follows an ongoing research that aims to analyze methodological practices to be applied in Design Education. A reflection about methodological strategies in Design Education and the function of drawing in Design represents the beginning of this study. Then, we developed an interdisciplinary pedagogical experience with the Graphic Design 1st grade students from our institution (IPCA). In the current academic year, 2013/2014, we continue to evolve this project, introducing changes in the initial proposal. Major alterations focused on the aspects that could be strengthened in terms of interdisciplinarity. In this article, the authors describe those changes and discuss the outcomes of the novel proposal. As we have already reported, this investigation follows a reflection about working methods to be adopted in Design Education. This is in accordance with other previously published works that purpose the enlargement of Design into new knowledge fields such as Experience or Service Design, changing not only the role of the graphic designer, but also the skills required to be a professional designer (Alain Findelli, 2001), (Brian Lawson, 2006), (Ciampa-Brewer, 2010). Furthermore, concepts such as cooperation or multidisciplinary design, amongst others, have been frequently debated as design teaching strategies (Heller and Talarico, 2011, pp. 82-85). These educational approaches also have an impact on our research. The analysis of all these authors’ contributions together with a reflection on our teaching practice allowed us to propose an improved interdisciplinary intervention.

Introdução: O síndrome patelo-femural é uma das disfunções músculo-esqueléticas mais comuns ao nível do joelho. É de etiologia multifatorial, sendo a rotação lateral da tíbia um dos fatores contribuintes, sendo que pode potenciar alterações da biomecânica da articulação patelo-femural por aumentar as forças de reação sobre a articulação. Brian Mulligan sugere que a técnica para a correção da rotação lateral da tíbia pode ser benéfica no alívio da dor e no aumento da amplitude de flexão do joelho, em pacientes com síndrome patelo-femural, apesar da evidência acerca da efetividade desta técnica ser ainda escassa. Objetivo: Avaliar os efeitos da técnica de mobilização com movimento de rotação medial da tíbio-femural com flexão do joelho, ao nível da intensidade da dor e da amplitude de movimento de flexão do joelho, durante o agachamento, em indivíduos com síndrome patelo-femural. Métodos: Estudo experimental, com uma amostra constituída por 20 estudantes universitários, do género feminino, com síndrome patelo-femural e dor ao agachamento bilateral. Estes foram distribuídos aleatoriamente por dois grupos: experimental (intervenção com técnica de mobilização com movimento) e placebo (intervenção placebo). Foram avaliadas a amplitude de flexão do joelho com um goniómetro eletrónico (Biometrics®) e a intensidade de dor com a Escala Visual Analógica, durante o agachamento bilateral, antes e imediatamente após as respetivas intervenções. O nível de significância foi de 0,05. Resultados: A realização da Análise da Covariância revelou que, relativamente à intensidade da dor, foi possível constatar que existiram diferenças significativas entre os dois grupos (p<0,001). Entre a avaliação inicial e a final, o grupo experimental diminuiu mais 2,1cm na Escala Visual Analógica do que o grupo placebo. Em relação à avaliação da amplitude articular, foi possível constatar que, existiram diferenças significativas, entre os dois grupos (p=0,004). Entre a avaliação inicial e a final, o grupo experimental teve mais 8,6º de aumento na amplitude articular do que o grupo placebo. Conclusão: Para indivíduos com síndrome patelo-femural, a técnica de mobilização com movimento para correção da rotação lateral da tíbia, parece ser benéfica no alívio da dor e no ganho de amplitude de flexão do joelho, analisando o movimento de agachamento bilateral.

This paper suggests an analysis of "Inanimate Alice" by Kate Pullinger, Chris Joseph and Ian Harper as an example of a transmedia narrative that triggers a new reading experience whilst proposing a literary alterity between reading and performance. Narrative experiences that elect the visual plasticity, interchanging games and tactility as drivers of the creative process are not new. Yet, narrative experiences, which have been created in the gap between reality and fiction, have found on the digital realm the perfect environment to multiple hybrid experiences. Bearing in mind Walter Benjamin’s concept of Erlebnis and Erfahrung, a critical analysis of this digital fiction tries to illustrate how literary art finds its space and time in a metamorphosed continuum only activated by the “patient reader”. All the multimedia hybrids, which this digital literary work may have, challenge readers to interpret different signals and poetic structures that most of readers might not be accustomed to; however even among a cognitive dissonance, meaning is found and reading happens only if time, space and attention are available. All possible transmedia literacies can only respond to this experience of online reading, if they are able to focus and draw attention not to a simple new behaviour or a single new practice, but to a demanding state of affairs that assemble different objective and subjective value forms.

[RuCl(arene)(-Cl)](2) dimers were treated in a 1:2 molar ratio with sodium or thallium salts of bis- and tris(pyrazolyl)borate ligands [Na(BpBr3)], [Tl(TpBr3)], and [Tl(Tp(iPr,4Br))]. Mononuclear neutral complexes [RuCl(arene)((2)-BpBr3)] (1: arene=p-cymene (cym); 2: arene=hexamethylbenzene (hmb); 3: arene=benzene (bz)), [RuCl(arene)((2)-TpBr3)] (4: arene=cym; 6: arene=bz), and [RuCl(arene)((2)-Tp(iPr,4Br))] (7: arene=cym, 8: arene=hmb, 9: arene=bz) have been always obtained with the exception of the ionic [Ru-2(hmb)(2)(-Cl)(3)][TpBr3] (5), which formed independently of the ratio of reactants and reaction conditions employed. The ionic [Ru(CH3OH)(cym)((2)-BpBr3)][X] (10: X=PF6, 12: X=O3SCF3) and the neutral [Ru(O2CCF3)(cym)((2)-BpBr3)] (11) have been obtained by a metathesis reaction with corresponding silver salts. All complexes 1-12 have been characterized by analytical and spectroscopic data (IR, ESI-MS, H-1 and (CNMR)-C-13 spectroscopy). The structures of the thallium and calcium derivatives of ligand TpBr3, [Tl(TpBr3)] and [Ca(dmso)(6)][TpBr3](2)2DMSO, of the complexes 1, 4, 5, 6, 11, and of the decomposition product [RuCl(cym)(Hpz(iPr,4Br))(2)][Cl] (7) have been confirmed by using single-crystal X-ray diffraction. Electrochemical studies showed that 1-9 and 11 undergo a single-electron (RuRuIII)-Ru-II oxidation at a potential, measured by cyclic voltammetry, which allows comparison of the electron-donor characters of the bis- and tris(pyrazol-1-yl)borate and arene ligands, and to estimate, for the first time, the values of the Lever E-L ligand parameter for BpBr3, TpBr3, and Tp(iPr,4Br). Theoretical calculations at the DFT level indicated that both oxidation and reduction of the Ru complexes under study are mostly metal-centered with some involvement of the chloride ligand in the former case, and also demonstrated that the experimental isolation of the (3)-binuclear complex 5 (instead of the mononuclear 5) is accounted for by the low thermodynamic stability of the latter species due to steric reasons.

A population-based case-control design was used to investigate the association between migration, urbanisation and schistosomiasis in the Metropolitan Region of Recife, Northeast of Brazil. 1022 cases and 994 controls, aged 10 to 25, were selected. The natives and the migrants who come from endemic areas have a similar risk of infection. On the other hand, the risk of infection of migrants from nonendemic areas seems to be related with the time elapsed since their arrival in São Lourenço da Mata; those who have been living in that urban area for 5 or more years have a risk of infection similar to that of the natives. Those arriving in the metropolitan region of Recife mostly emigrate from "zona da mata" and "zona do agreste" in the state of Pernambuco. Due to the changes in the sugar agro-industry and to the increase in the area used for cattle grazing these workers were driven to villages and cities. The pattern of urbanisation created the conditions for the establishment of foci of transmission in São Lourenço da Mata.

RESUMO: A cefaleia cervicogénica é uma forma comum de dor de cabeça, que tem sido associada à existência de uma disfunção das estruturas da coluna cervical superior. Estudos recentes mostram uma grande incidência dessa disfunção a nível de C1-C2, avaliada pelo teste de flexão-rotação. Vários terapeutas manuais, como Brian Mulligan e Mariano Rocabado, têm sido sugerido técnicas de tratamento para este tipo de disfunção. Contudo, a evidência acerca da efectividade dessas técnicas é escassa. Desenho do estudo: Foi efectuado um ensaio clínico aleatório, duplamente cego, composto por três fases: pré-intervenção, intervenção e pós-intervenção. Objectivos: Avaliar e comparar os efeitos imediatos de duas técnicas de Terapia Manual Ortopédica (SNAG C1/2 de Mulligan e técnica de desrotação do atlas de Rocabado), na amplitude de movimento de rotação do segmento vertebral C1-C2, em indivíduos com história de cefaleia cervicogénica e com limitação no teste de flexão-rotação. As técnicas de tratamento foram usadas de forma isolada, em comparação a um grupo placebo. Métodos: Uma amostra de 60 indivíduos, com cefaleia cervicogénica e limitação do teste de flexão-rotação, foram aleatoriamente distribuídos por três grupos: SNAG C1/2 de Mulligan, técnica de desrotação do atlas de Rocabado e grupo placebo. O outcome primário foi a amplitude de movimento obtida no teste de flexão-rotação, que foi medido antes e imediatamente após a intervenção. Resultados: Imediatamente após a intervenção, a amplitude verificada no teste de flexão-rotação aumentou 21.8º (DP, 4.68) no grupo submetido ao SNAG C1/2 de Mulligan, 15º (DP, 5.07) no grupo em que foi aplicada a técnica de desrotação do atlas de Rocabado e 0.65º (DP, 0.67) no grupo placebo. Uma ANOVA modelo misto, 2 por 3, revelou efeito principal significativo do tempo (p<.001) e grupo (p<.001), assim como uma interacção significativa entre grupo e tempo (p<.001), relativamente à variável amplitude do teste de flexão-rotação. Estes resultados indicam que as diferenças verificadas entre os grupos eram dependentes do momento de avaliação. Uma comparação múltipla post hoc revelou que quer as técnicas de Mulligan, quer de Rocabado, produziram efeitos significativamente maiores que a intervenção placebo na amplitude de movimento do teste de flexão-rotação (p<.001 e p=.001, respectivamente). No entanto,não se verificou uma diferença significativa no que diz respeito à efectividade de ambas as técnicas de Terapia Manual Ortopédica aplicadas (p=.42). Conclusão: Esta investigação sugere que as duas técnicas de Terapia Manual Ortopédica avaliadas produziram efeito clínica e estatisticamente significativo na amplitude do teste de flexão-rotação. No entanto, não se verificaram diferenças entre as duas técnicas, no que diz respeito ao seu efeito no ganho de amplitude de movimento. Os resultados obtidos fornecem evidência preliminar sobre a efectividade de ambas as intervenções no tratamento da redução de amplitude de movimento em indivíduos com história de cefaleia cervicogénica.-------------------------------ABSTRACT:Background: Cervicogenic headache is a common form of headache arising from dysfunction in structures of the upper cervical spine. Recent studies have shown a high incidence of C1/2 dysfunction, evaluated by the flexion-rotation test (FRT). Several manual therapists have suggested different approaches to manage that dysfunction, such as Brian Mulligan and Mariano Rocabado. However, the evidence of the effectiveness of those manual techniques is anedoctal. Design: Randomized double blinded controlled trial with three phases: pre-intervention, intervention and post-intervention. Objectives: To determine and compare the immediate effects of two manual therapy techniques (Mulligan’s SNAG C1/2 and Rocabado’s atlas’ derotation technique) in the range of motion of C1-C2 vertebral segments, in cervicogenic headache patients and with limitation on the flexion-rotatoin test. The treatment techniques were used as single treatments against a placebo group. Methods: A sample of 60 subjects with cervicogenic headache and FRT limitation were randomly allocated into one of three groups: Mulligan’s C1/2 SNAG, Rocabado’s atlas derotation technique or placebo group. The primary outcome was the flexion rotation test range, which was measured before and immediately after the intervention. Results: Immediately after the application of the interventions, FRT range increased by 21.8º (SD, 4.68) for the Mulligan’s C1-2 SNAG group, 15º (SD, 5.07) for the Rocabado’s atlas derotation technique and 0.65º (SD, 0.67) for the placebo group. A 2-by-3 mixedmodel ANOVA a significant main effect of time (p<.001) and group (p<.001), as well as a significant interaction between group and time (p<.001) for the variable FRT range. These results indicate that group differences were dependent on time. A pairwise post hoc comparison revelad that both the Mulligan and Rocabado techniques produced significantly more effect on FRT range of motion than the placebo intervention (p<.001 and p=.001, respectively). However, there was not a significant difference between the effectiveness of the two manual therapy techniques (p=.42).Conclusion: This investigation’s findings suggest that both Mulligan’s C1/2 SNAG and Rocabado’s atlas derotation techniques produced a clinically and statistically significant effect on FRT range, but there were no changes between the two techniques in their effectiveness. These results provide preliminary evidence for the efficacy of both manual therapy techniques in the management of individuals with cervicogenic headache and FRT limitation.

A serosurvey was conducted in wild animals captured close to two areas where hantavirus cardiopulmonary syndrome (HCPS) occurred in São Paulo State, Brazil. Serum samples from a total of 43 mammals were tested for antibodies reactive with Sin Nombre (SN) hantavirus using a strip immunoblot assay. RNAs from the blood clots of the positive samples were submitted to reverse transcriptase-polymerase chain reaction (RT-PCR). Two rodents of the genus Oligoryzomys were positive for hantavirus antibodies. These animals were captured in the Iguape region and represented 16.7% (2/12) of the sera from rodents and 100.0% (2/2) of the Oligoryzomys captured in that area. RT-PCR failed to amplify any viral cDNA. These results are in agreement with other data that suggest that members of this genus are important reservoirs of hantaviruses in Brazil.
