As tecnologias da informação e comunicação (TIC) estão presentes nas mais diversas áreas e atividades cotidianas, mas, em que pesem as ações de governos e instituições privadas, a informatização da saúde ainda é um desafio em aberto no Brasil. A situação atual leva a um questionamento sobre as dificuldades associadas à informatização das práticas em saúde, assim como, quais efeitos tais dificuldades têm causado à sociedade Brasileira. Com objetivo de discutir as questões acima citadas, esta tese apresenta quatro artigos sobre processo de informação da saúde no Brasil. O primeiro artigo revisa a literatura sobre TIC em saúde e baseado em duas perspectivas teóricas – estudos Europeus acerca dos Sistemas de Informação em Saúde (SIS) nos Países em Desenvolvimento e estudos sobre Informação e Informática em Saúde, no âmbito do Movimento da Reforma Sanitária –, formula um modelo integrado que combina dimensões de análise e fatores contextuais para a compreensão dos SIS no Brasil. Já o segundo artigo apresenta os conceitos e teóricos e metodológicos da Teoria Ator-Rede (ANT), uma abordagem para o estudo de controvérsias associadas às descobertas científicas e inovações tecnológicas, por meio das redes de atores envolvidos em tais ações. Tal abordagem tem embasado estudos de SI desde 1990 e inspirou as análises dois artigos empíricos desta tese. Os dois últimos artigos foram redigidos a partir da análise da implantação de um SIS em um hospital público no Brasil ocorrida entre os anos de 2010 e 2012. Para a análise do caso, seguiram-se os atores envolvidos nas controvérsias que surgiram durante a implantação do SIS. O terceiro artigo se debruçou sobre as atividades dos analistas de sistema e usuários envolvidos na implantação do SIS. As mudanças observadas durante a implantação do sistema revelam que o sucesso do SIS não foi alcançado pela estrita e técnica execução das atividades incialmente planejadas. Pelo contrário, o sucesso foi construído coletivamente, por meio da negociação entre os atores e de dispositivos de interessamento introduzidos durante o projeto. O quarto artigo, baseado no conceito das Infraestruturas de Informação, discutiu como o sistema CATMAT foi incorporado ao E-Hosp. A análise revelou como a base instalada do CATMAT foi uma condição relevante para a sua escolha durante a implantação do E-Hosp. Além disso, descrevem-se negociações e operações heterogêneas que aconteceram durante a incorporação do CATMAT no sistema E-Hosp. Assim, esta tese argumenta que a implantação de um SIS é um empreendimento de construção coletiva, envolvendo analistas de sistema, profissionais de saúde, políticos e artefatos técnicos. Ademais, evidenciou-se como os SIS inscrevem definições e acordos, influenciando as preferências dos atores na área de saúde.

A análise do financiamento do Sistema Único de Saúde (SUS) é extremamente complexa. Enquanto grande parte dos estudos desta temática concentra-se na análise das transferências constitucionais e legais (automáticas) da União e Estados para os governos subnacionais, poucos são os trabalhos dedicados à análise das transferências voluntárias (TVs). De certa forma, pode-se dizer que tal tema ou é ignorado pela comunidade epistêmica ou é tratado de forma apenas marginal nos estudos acadêmicos. Com o intuito de complementar essa parte pouco explorada da literatura da saúde, o presente trabalho realiza uma análise exploratória sobre a realização de transferências discricionárias efetivadas entre as diferentes esferas de governo e busca compreender a relevância deste tipo de repasse para o financiamento da saúde. Embasado em conceitos advindos da teoria federalista, o trabalho procura entender se o caso das transferências voluntárias na saúde pode ser considerado como uma expressão do federalismo coercivo. Além da atualização sobre as regras de financiamento do SUS, realizou-se como uma das etapas de análise entrevistas com gestores de saúde, além da análise de dados secundários. Como conclusões, verificou-se que na etapa de análise documental, as TVs podem ser configuradas como instrumentos de centralização de poder decisório (Misoczky, 2003), uma vez que impõem aos municípios os objetivos desenhados por governos superiores e demonstram uma relação coerciva de federalismo (Watts, 2006; Arretche, 2004). No entanto, a análise de dados do município de São Paulo permitiu relativizar esta situação em termos econômicos, dada a baixa representatividade das transferências voluntárias frente aos gastos totais.

O presente estudo teve como objectivos caracterizar do ponto de vista da saúde mental a população idosa da Região Autónoma da Madeira (RAM); determinar as prevalências das situações de saúde mental positiva e negativa e avaliar a influência positiva (protectora) ou negativa (de risco) de certos factores pessoais e do meio na saúde mental. Foi um estudo de natureza psicossocial,transversal, probabilístico, com uma componente descritiva e outra inferencial. A amostra (N=342) representativa das pessoas com 65 e mais anos, residentes na comunidade, foi estratificada por concelhos, por géneros e por classes etárias. A selecção foi feita da base de dados do Cartão de Utente do Serviço Regional de Saúde Empresa Pública Empresarial. As pessoas idosas foram entrevistadas pelas enfermeiras dos Centros de Saúde, que utilizaram para tal um questionário estruturado. Na avaliação das variáveis utilizaram-se diversos instrumentos alguns dos quais amplamente usados em outros estudos com população idosa. A saúde mental foi avaliada utilizando-se o Mental Health Inventory - MHI (Ware & Veit, 1983; Ribeiro, 2000), que contempla uma dimensão mais positiva o bem-estar psicológico e outra mais negativa o distress psicológico. Nas variáveis independentes (pessoais e do meio ambiente) utilizaram-se: para a classe social a Classificação Social de Graffar (Graffar, 1956); para a rede social a Lubben Social Network Scale - LSNS (Lubben, 1988); para a autonomia nas actividades instrumentais da vida diária a IADL (Lawton & Brody, 1969; Botelho, 2000); para a capacidade funcional (ABVD) o Índice de Katz (Katz et al., 1963; Cantera, 2000). As restantes variáveis, nomeadamente as de caracterização demográfica bem como as auto-percepções relativas ao rendimento, à habitação, de controlo, a ocupação do tempo, os acontecimentos de vida significativos, a autonomia física, a percepção relacionada com a saúde, as queixas de saúde ou doenças, os apoios de saúde e sociais, foram avaliadas através de questões formuladas para o efeito. No tratamento de dados, procedeu-se à análise descritiva das diferentes variáveis obtendo-se uma primeira caracterização da saúde mental das pessoas inquiridas. A fim de determinar as prevalências das situações de saúde mental mais positiva e mais negativa, recorreu-se à análise com três clusters. Para determinar a associação entre as variáveis pessoais e do meio e a saúde mental, usaram-se dois modelos de regressão logística (MRL). Num 1º MRL o enfoque colocou-se na relação das capacidades físicas e na percepção de saúde detidas pelas pessoas idosas, na disponibilidade de apoios específicos e a saúde mental. O 2º MRL focalizou-se na interacção entre a percepção de controlo detida pelas pessoas idosas, as condições sócio-económicas e a saúde mental. Resumem-se os resultados: na amostra identificou-se uma percentagem superior de mulheres (66,4%) face aos homens. A classe etária dos 65–74 anos incluiu maior número de idosos (64,9%). A maioria de (55,6%) residiam fora do Funchal. Os reformados eram prevalentes (78,1%) bem como os que detinham 1 a 11 anos de escolaridade (58,2%). As mulheres (65,2%) eram mais analfabetas do que os homens (48,7%). Dos idosos 44,4% pertenciam à classe social V (muito baixa) sendo a maioria mulheres (53,3%). A idade mínima da amostra foi 65 anos e a máxima 89 anos. A idade X =72,6 anos com umS =5,77. Foram encontrados níveis mais positivos nas diferentes dimensões da saúde mental. Prevalências: saúde mental positiva 67,0%, bem-estar psicológico elevado 24,3%. Apenas 3,2% apresentaram distress psicológico mais elevado. Com depressão maior identificaram-se 0,3% dos idosos. Num 1º MRL com as possíveis variáveis explicativas ajustadas, verificou-se que a probabilidade da saúde mental ser mais positiva era cerca de 0,3 vezes inferior nas mulheres, nos idosos com redes sociais muito limitadas e nos que percepcionavam a saúde própria como razoável ou pior. Era menor 0,5 vezes quando não sabiam ou percepcionavam a saúde como pior comparativamente aos pares, e 0,3 vezes quando referiram o mesmo, comparando-a com há um ano atrás. Era ainda 0,1 vez inferior quando possuíam limitações físicas para satisfazer as necessidades próprias. A probabilidade de ser mais positiva era 2,5 vezes superior quando as pessoas possuíam 1 a 11 anos de escolaridade. A variância no nível de saúde mental, explicada com base no 1º modelo foi 44,2%, valor estimado através do Nagelkerke R Square. Os resultados do 2º MRL com variáveis ajustadas, permitem afirmar que a probabilidade da saúde mental ser mais positiva era 0,3 vezes menor nas mulheres, 0,1 vez inferior nos idosos que percepcionavam o rendimento auferido como razoável ou fraco e 0,4 vezes menor quando tinham uma rede social muito limitada. Ter limitações físicas deslocando-se na rua apenas com apoio diminuía 0,3 vezes a probabilidade de saúde mental mais positiva, verificando-se o mesmo nos que auferiam apoio dos serviços sociais. Uma probabilidade 2,4 vezes superior da saúde mental ser mais positiva foi encontrada nos idosos com 1 a 11 anos de escolaridade quando comparada com os analfabetos. O Nagelkerke R Square = 37,3%, foi menor do que o obtido no modelo prévio, pelo que a variação ao nível da saúde mental explicada por este modelo é inferior. A evidência de que as pessoas idosas possuíam maioritariamente um nível superior de saúde mental, comprovou que a velhice não é sinónimo de doença. Foi também superior a percentagem daqueles que possuíam redes sociais menos limitadas. O nível mais elevado de distress psicológico surgiu com uma prevalência de 3,2% e apenas 0,3% das pessoas idosas estavam mais deprimidas o que evidenciou a necessidade de serem providenciadas respostas na comunidade para o seu tratamento. Dos inquiridos 8,8% apresentavam um nível médio de depressão, sugerindo a pertinência de serem efectuados às pessoas nessa situação, diagnósticos clínicos mais precisos. As limitações na capacidade física para a satisfação de necessidades diárias e a percepção de saúde mais negativa emergiram como factores significativos para a pior saúde mental confirmando resultados de pesquisas prévias. No 2º MRL a percepção pelos idosos de que o rendimento mensal auferido era fraco aumentou também a probabilidade da saúde mental ser pior. Nos dois modelos verificaram-se influências positivas quando os idosos possuíam 1 a 11 anos de escolaridade comparativamente aos analfabetos, o que pode ser considerado um factor protector para a saúde mental. Sublinhamos como principais conclusões deste estudo: O protocolo e os instrumentos de avaliação foram adequados para atingir os objectivos. Da avaliação à saúde mental concluiu-se que as pessoas idosas possuíam situações mais positivas e favoráveis. Dos três factores utilizadas na estratificação da amostra apenas o género feminino estava associado significativamente à pior saúde mental. Sugere-se a replicação deste estudo para acompanhar a evolução da saúde mental da população idosa da RAM. Os resultados deverão ser divulgados à comunidade científica e técnica bem como aos decisores políticos e aos gestores dos serviços de saúde, sociais, educativos e com acção directa sobre a vida dos idosos a fim de serem extraídas ilações, favoráveis à adopção de políticas e programas promotores da saúde mental que passem pelo aumento da escolaridade e por medidas/acções que reduzam a maior susceptibilidade de saúde mental negativa associada ao género feminino, promovam o reforço das redes sociais das pessoas idosas, a autonomia física necessária à satisfação das necessidades próprias bem como as auto - percepções positivas relacionadas com a saúde e com os rendimentos auferidos. Deverão serlhes facultadas também oportunidades de participação activa na comunidade a que pertencem.

The study aimed to analyze the influence of chronic health conditions (CHC) on quality of life (QOL) of UFRN servers assaulted by CHC. It is a descriptive and cross-sectional study with prospective data and quantitative approach, accomplished in the ambulatory clinic of the Department of Server Assistance (DSA) of the Pro-Rectory of Human Resources, during three months. The sample was composed by accessibility, totaling 215 people, being 153 active and 62 inactive servers, in chronic health condition. The data were collected through the application of the sociodemographic characterization, health, environmental and laboral form, the Medical Outcome Study 36-Item Short Form (SF-36). The study was evaluated by the HUOL Ethics Committee (CAAE no. 0046.0.294.000.10), obtaining assent. The results were analyzed in the SPSS 15.0 program through the descriptive and inferential statistics. It was identified servants predominantly male (59,1%), under 60 years old, married or in stable union, Catholics, brown color, living in the capital and residents in own home. Regarding labor issues, there was a predominance of active servers technical-administrative with intermediate and medium level positions and small proportion of docents. Among the CHC, the non-communicable diseases - NCDs (95.8%) had a higher frequency, followed by persistent mental disorders - PMDs (18.6%) and, finally, the continuous and structural physical deficiency - CSPD (16.9 %). The QOL of servers was considered good, with a mean score of 72.5 points in the total score, with the most affected domains: physical (59.1), general health (66.2), bodily pain (66.3) and functional aspects (72.0). The mental health dimension (76.5) had a better average than the physical dimension (68.0 points). It was found that the decrease in QOL scores is significant statistically related to higher number of CHC (ρ <0.001), with no statistical significance regarding the functional situation (p = 0.259). The administrative technicians of elementary, primary, secondary levels and docents had the worst QOL scores. After the correlation analysis of CHC with the domains and dimensions of the SF-36, there was statistically significant, negative and weak correlation of the domains: functional aspect (ρ = 0.002, r = -0.207), physical aspects (ρ = 0.007; r = -0.183), vitality (ρ = 0.002, r = -0.213), social function (ρ = 0.000, r = -0.313), emotional aspects (ρ = 0.000, r = -0.293), mental health (ρ = 0.000 , r = -0.238), physical health dimension (ρ = 0.002, r = -0.210) and mental health dimension (ρ = 0.000, r = -0.298). The presence of PMD isolated or together, contributed to a lower SF-36 scores, being the domains variation of mean significant, except for bodily pain, general health and physical aspects. By correlating the categories of CHC and QOL, there was a weak correlation (r ≤ -0.376) and significant (ρ ≤ 0.011), mainly related to the NCD, PMDs and NCD + PMD, affecting the mental health, social function, emotional aspects, vitality and functional aspect domains. Front of the results, it was concludes that the servers quality of life is influenced by the CHC. Thus, it was inferred that the presence of CHC causes a negative effect on quality of life, leading the active and inactive servers to exposure their overall life activities and work over the years, due to the morbidity affected, mainly related to NCDs and PMDs. This issue analises the unevenness in the brazilian system of public health care as an extension of socials inequities. It is a theoretical study based in a historical method, using empirical camp from academic, corporation and institution researchs, along the period 2002-2006. Equality and effectiveness in health systems are analitical basic cathegories grap in the root of the doctrine, principles and organization of the Unique Health System, in which sectorial actions are inserted. Discuss the estructural prodution and determined those inequalities through some social determiners of health system: income, land, food securitiy, nutritional situation, basic sanitation, epidemiological inequities and public management policy. Carry out a thematic review over health social production, it formlation and the goals of social policies, as well as the insertion of the equality principle in the assistance system, in the frame of the running public health regulations. It uses reflections that enlighted the correlation between the process of political-institutional actions and equity on health assistance. Analized the pertinency of sectorial reorganizational strategies on basic attendance, confronting the hipothesis that those strategies reinforce social inequities in health system, because it organize diferential assistance levels over not equal baselines. The results show up that social inequalities, even remaining, have had a small decrease; that the selectiviness of actual public policies and the duplication of the health system, increases the differences within and between the social classes and configures the assistance as inequal. The basic care system has great shortages that also appeares in middle and complex assistance levels. As conclusion, it remarks that the health assintance system, even with it integrality has limits; structural problems on material conditions of living and health system could not be reversed only with institutional legal arragements; by the contrary, in border conditions, these strategies produce policies that reinforce inequities, neglecting the equity principle of the system in which frame, they work. It is a fact that the fundamental rights of citizens are being recognized and guaranteed by the state over time, regardless of the belief that if these rights has always been part of the heritage of subjective individuals, or whether they will be aggregated during the course of human history. In that, emerged the rights of freedom of men and, subsequently, the rights to create a situation of equality between the humans, the so-called social rights. In turn, as these rights known as social, to be implemented, need a positive action by the state, more precisely by the state power whose function is to manage public money and create policies for implementation of fundamental rights. Given this, pay attention to the right to health, was created the Programa de Medicamentos de Dispensação Excepcional, which aims to provide high-cost medicines to citizens Brazilian carriers of serious diseases, such as Alzheimer's and Mal Hepatitis C. Also on the program, it provides a way which will be mandatory that the drugs will be offered in such situations, and does not include a means of updating the list predicted able to monitor the progress of medicine that have been in the interest of the program. Given that, at present it is necessary to mention the recognition of another fundamental right: the right to development, which is the right of access to positive actions being implemented by the State, which are nothing more than public policy, gender which the Programa de Medicamentos de Dispensação Excepcional is kind. Thus, through the search in legislation and doctrine in relation to the theme, this work has the aim to examine the extent of the state to provide exceptional dispensing of medicines. Specifically, if the State in attention to the right to development and the implementation of the right to health, can really list exhaustively the drugs to be provided by the State, and what are the elements guiding this choice and how to control the same

This study is an analysis of opportunities and challenges of health assistance migration from hospitals to home care from the approach of the Domiciliary Internment Program (PID) in Natal / RN. The research aims to identify the ways that the multidisciplinary team act and know the stories of these professionals about the situation experienced in the transition between the instituting and instituted on home care modalities. PID has as a prior focus the elderly person in stable medical conditions, not to replace the hospital care, but to offer a therapeutic support turned to the exercise of their autonomy and coexistence with the situation of diseases. The home in their internal coexistence rules preserves own customs. As the hospital care migrates to the home care, it happens in the confrontation and rationality negotiation and becomes something new, that is going to be directed by an instituting dimension. In the view of New History, that suggests an interdisciplinary approach and interprets the problems on its time and from the technique of thematic oral history, it can be seen that working in interdisciplinary team is able to incorporate new values in the way of healthcare assistance, it longs for maintaining the maximum functional capacity of patients, it presents results as the prevention of diseases, costs reduction in connection with the Hospital Service, empowers and expands the possibilities for the patient recovery by aligning with the daily life and the opportunity of the patient being assisted by a multiprofessional team, interacting on the concrete reality. Therefore, PID is in line with the contemporary demands and as an instrument to be considered in the review of a wider concept of the health-disease process

This study aims to analyze social representations of elders to their fragile situation at home, with the presence of one or more characteristics, as defined by the Brazilian Ministry of Health. It is a descriptive and qualitative study, based on methodological -principles of the Theory of Social Representations. Setting was the homes of elderly residents in the area ascribed to a Family Health Unit (FHU) in the city of Natal. A total of 10 elderly subjects, whose choice was intentional and according to the need for USF home visit in a period of time, considering the saturation process of the information. As collection procedures were used the semi-structured interview and participant observation in accordance with the ethical rules of Resolution No. 196/96, with the assent of the Ethics and Research UFRN. To analyze the results, it was used the thematic content analysis in the aspect of preparation of representations, focusing on the totality of the discourse of the subjects. The results indicate that most study participants felt difficult to give meaning to the terms weakness and to be weak, although many present one or more aspects of the syndrome of frailty. From the content analysis of participants speeches in this study, we achieved the following categories: fragility as illness and disease as aging, aging and frailty as causes of changes and difficulties in daily life, the presence of family life in the fragile elderly, fragility as weakness and the risk for falls, the perception of being weak like a different person in addition to the absence of fragility in elderly life. Thus, through the processes of anchoring and objectification, the "fragile being" became familiar and concrete, showing that the meaning of weakness, besides the scientific definition found in the reified universes, can be reinterpreted and built within the consensus universes. About the care received by the staff of Family Health, from the viewpoint of older people there seems to be an understanding about the role of professional nurses; on the other hand, older people often mention the role of the Community Health Agent

The research aimed to understand the general perception of men about their health care in a health unit família. This is an exploratory and descriptive qualitative approach, developed at the Center for Health Dr Vulpiano Cavalcante in the City of Parnamirim / RN. Participated in the investigation 12 men enrolled in the Estratégia de Saúde da Família ESF (Family Health Strategy), in the age-group 20 to 59 years lived in the catchment area of one of the ESF teams unit above. Data were collected from July to August 2009, through structured interviews, which, after transcription have undergone a process of identifying meaning units, coded and categorized according to the precepts of content analysis according to Bardin. Following in the footsteps of this method, originated the following themes: "Revealing what motivates men to seek assistance in the Estratégia de Saúde da Família," "Expressing knowledge of the Estratégia de Saúde da Família ", "an opinion about the care health and relationship with the ESF. The analysis was processed according to the principles of symbolic interactionism as Blumer. To support the discussions were used literary themes about man in the context of public health policies and the man in the family and the influences of gender. These, when they had their properties and dimensions analyzed, raised the central category Man on the Estratégia de Saúde da Família . The results show that respondents have limited knowledge about the ESF, your actions turn to use the service when no obligation arising from discomforts and accuracy of care. Moreover, delivering the opinion of the care with your health, even need, but recognizing that do not perform self-care. Given this reality, we can conclude that the perception of the man about his health in the ESF permeates gender issues that influence their behavior toward disease prevention and health promotion. This situation requires that the professionals and managers of health initiatives for the inclusion of man in the actions of the ESF assistance starting from the understanding of their conceptions of health care.

This study is an exploratory descriptive study with a quantitative approach. The objective was to identify the actions for the early detection of breast cancer conducted by the health professionals of the Family Health Strategy in the Trairi region of the State of Rio Grande do Norte, Brazil. The research was conducted in nine municipalities of the region. Data were collected by means of a questionnaire with 52 Family Health Strategy professionals, 30 nurses and 22 physicians, that work in the region. Analysis was conducted using descriptive statistics. The results were organized and discussed in three areas: Knowledge about the early detection of breast cancer; Actions for early detection detection of breast cancer, and Difficulties experienced in the screening actions for breast cancer. The results indicate that these professionals (100%) have knowledge of the signs and symptoms of breast cancer and that the majority (96,2%) conduct screening actions in accordance with the recommendations of the Ministry of Health. However, a considerable number (55,8%) of these professionals encounters difficulties while conducting the screening procedures in his work setting. The difficulties varied from those of a personal nature to those of access to the procedure, such as the unavailability of sufficient quotas of screening exams. We conclude that the majority of health professionals execute the screening actions for breast cancer in their work settings according to the recommendations of the Ministry of Health, even though they encounter difficulties in the conclusion of the mammography and ultrasound exams, essential procedures in the early detection of breast cancer. We understand that these professionals demonstrate knowledge of secondary prevention even though they do not execute all the actions necessary for early detection primarily because they are impeded by the blockage of access to exam quotas. We conclude that, in spite of the difficulties experienced, the procedures for the early detection of breast cancer are being executed by the majority of the Family Health Strategy professionals in the Trairí region, specifically the clinical examination of the breast, orientation of breast self examination, requests for mammography and ultra sound examinations. Measures are needed that can mediate the difficulties, that will permit the realization of secondary prevention procedures with the population at risk in the region. We suggest training and actualization courses on the complete screening process that includes a wide discussion of the new legislation that provides the mammography exam for women over 40 years. We believe that the acquisition of such a work perspective for the early detection of breast cancer, along with knowledge of health vigilance and of breast cancer, will enhance integral health care of women that constitutes an aim of the nurse and the family health team

The ongoing transformations in brazilian society, arising from technical and organizational changes in the working world, are making, with much emphasis, heated debates resurge related to themes and issues that refer to the relationship between work, skills and education. Thus, this study is inserted in the link between education and work, pointing to the work as an educational principle according to Antonio Gramsci. This paper aims to discuss the interfaces between education and work in the everyday health care teams and learn about the activities developed by health teams related to the learning processes in and with the work to analyze the opportunities and challenges of transforming spaces of health work in an environment of lifelong learning. This is a descriptive, exploratory with a qualitative approach case study developed from semi-structured interviews with the health staff professionals of the Unidade de Terapia Intensiva da Casa de Saúde Dix-Sept Rosado in Mossoró / RN , who answered open questions about the relationship between education and work. The interviews were conducted during the month of January 2010, the same being recorded, transcribed and analyzed, culminating in the production of new knowledge on the subject. It is understood that work and education activities are eminently human, therefore only the human being works and educates. Given the statements of participants, it is noticed that all work processes in health are learning moments. This happens through new demands imposed by the everyday of the services, by interaction with a multidisciplinary team, participation in educational activities and individual study. It was noticed that the institution in this case does not promote study courses related to Intensive Care and that there are obstacles to the realization of educational activities on and with the work, such as: excessive workload, inability to release staff to participate in events, low pay, which leads the worker to have more than one employment, rejection of new knowledge by some workers and lack of physical infrastructure and incentives for the activities. The daily situations must be transformed in learning, selfanalyzing the problems of practice and valuing the work process itself in its intrinsic context. We conclude that dealing with the web of relationships between educational processes and production processes of health services, unraveling the intricacies of the world of work and education requirements in this sector are increasingly on the agenda of Sistema Único de Saúde workers and managers. The continuing consideration of this issue becomes an essential condition for the proper discharge of their responsibilities. We consider that bringing the education to everyday life is the result of recognition of the educational potential of the work situation

Brazilian health public assistance is going through two Reforms, Sanitary and Psychiatric, and through these the assistance is guaranteed in the three levels: primary, secondary and tertiary. Thus, mental health assistance should be offered since preventive cares until the ones that demand larger technological apparatus. Programs like Health Community Agent's Program (HCAP) and Family Health Strategy (FHS), besides increasing the services coverage, have been making possible the system reorientation in the meaning of integrality, universalization and equity. Thus, united intervention of mental health team and FHS can offer several benefits to the population, providing assistance and follow-up to patients with mental disorder. It was aimed to assess health community agents facing the user of Family Health Strategy in depressive state. This quanti-qualitative study took place in the municipal district of Abaiara-CE. Semi-structured interview was applied with health community agents and Beck Depression Inventory with the users registered in Family Health Strategy. It was verified that among the 64 users interviewed, 12.5% didn't present symptoms of depression, 10.9% presented symptoms of light depression, 14.1% symptoms of moderate depression and 62.5% symptoms of serious depression. For the 22 health community agents interviewed, they all reported the existence of people with symptoms of depression in their personal micro-areas, being difficult to work with them, once the FHS team is not qualified to work with mental health problems. It was verified that the Municipal district doesn't have specialized professionals, making difficult the routing and treatment. Based on these results, it was concluded that in spite of the articulation of mental health with FHS is necessary and benefactor to the population, it still doesn't exist, worsening the situation, mainly in small Municipal districts, once they don't have mental health services. Thus, the population is exposed and without follow-up, which allows the identification of installed diseases and with gravity, like depression, because there are no prevention and control activities. It is recommended, due the extreme need, the elaboration and implantation of a mental health program in these municipal districts, articulated with FHS

The objective of analyze the shift of the working process of the ESF team in care of children with disabilities, from awareness-raising actions. It is a qualitative study, with the action-research method. Thirteen health professionals were involved from two teams of ESF unit area of the Unidade de Saúde da Família Dr. Chico Porto (UBSFCP) in Mossoró, from March to August 2011. Data were analyzed following the direction of freirean s thematic analysis. In the situational diagnosis of the current reality of CwD assistance in that UBSFCP, through participant observation and application of semi-structured interviews with professionals, we realize that despite these actions carry some assistance to the CwD, in practice few are used for inclusion and accessibility. The monitoring of the CwD is done through individual consultations by each team professional, home visits when possible, both ruled on the complaints and problems, with little solving in the used actions. Since the need for a change in the treatment model and training requirements as pointed out by professionals in the interview, then we decided to build the proposed of training suggested by the multidisciplinary team and put together collectively the achievement of this moment in all its phases. In the step of implementation (action), aspects related to the current situation in Brazil and Mossoró (Laws, policies and health care) for the CwD and CwD Assistance and their family in the ESF in the first two moments of the first training (action) were contemplate. On the second day we discussed the specialized care to CwD, contribution of the Handicapped Parents and Friends Association of Mossoró and in a second moment a workshop was held in which awareness for inclusion of CwD and actions of ESF were discussed. All these moments were discussed and collectively constructed. In the evaluation, we found that implementation (action) allowed to the professional the comprehension of new understandings about people with disabilities, on ways to include, guiding, caring, watching, and mainly to have a new vision on health assistance of the CwD, expanding assistance beyond clinical aspects and recognizing the educational aspects of the rights and duties of citizens and the inclusion of these children in the social spaces area. As difficulties, we face the need for some professionals to be absent to attend another job, solve personal problems, and little or no participation. Thus, during this action-research, the subjects were able to realize the importance of carrying out their practice to the quality of life for him and to the one they care

The joint enters the teams of the Strategy Health of Family (ESF) and the Municipal Center of Infantile education (CMEI) blunts as a form to assure the monitoring and promotion to the health of the children of 2 the 5 years when entering the day-care center environment/daily pay-school. It was traced as objective: To analyze the actions developed for the team of the Strategy Health of the Family in the promotion the health of the child, taken care of in a CMEI. Description-exploratory is to a study, qualitative nature, the type research-action. Developed in a CMEI and the USF of the quarter of New City, Natal-RN. The population was constituted by the professionals of the team of the ESF and the CMEI and parents. During the stages of the research-action diverse techniques had been used as the individually interview and in group, focal group, comment participant, and daily of field. The analysis of the data occurred by means of the content analysis, in the thematic modality, proposal for Bardin (1977) and description of the stages of the research-action. In the stage of situational diagnosis that it investigates the reality lived deeply for the citizens of ESF and CMEI how much to the health of the child seven categories had emerged that they enclosed: the context of the attention child in the CMEI identifying the actions that already came being developed for the ESF in the CMEI; the functioning of the CMEI and its routine of activities; the paper of the CMEI in the care the child; the daily one of the ESF, how much to the care to the health of the child of 2 the 5 years involving the diverse difficulties faced for the ESF; difficulties faced in daily of the CMEI for the care the child of 2 the 5 years; paper of joint ESF and CMEI for the confrontation of the difficulties; e action of health to be developed that they had subsidized the stage of planning of the research-action. During the stages of planning and implementation of the actions the actions of education in health with professionals of the CMEI and parents had been materialize and the actions of direct attention the health of the child. In the stage of evaluation of the actions for the involved citizens one searched to ahead understand the perception of the actions developed and perspective of continuity of the actions, through 4 boarded subjects for the citizens. For all the passage of the research-action it can be inferred that joint ESF and CMEI is a necessary initiative ahead of the current situation of the services of health for the promotion of an integral attention the health of the child, but that the teams of the ESF not yet make use of material conditions and staff enough to develop actions that exceed the limits of the USF, being necessary for this the reinforcement of the joints mainly with the Federal University of the Rio Grande of the North.

We aim to understand the social representations of man's aggressive behavior from the perspective of women in situations of domestic violence. This is a descriptive, exploratory and representational study, whose methodological approach falls into the qualitative category. We chose as a scenario for research, by the Reference Center for Citizen Women (CRMC), Natal / RN. The criteria for selection of participants were women who lived/live in situations of domestic violence, with affective or relationship bonding with the assailant, in psychological and emotional positions appropriated to the reality; that are being protected or assisted by the service listed above; whose aggressor is male. We adopted as data collection instruments: questionnaire, Drawing-Story (DE) and a field diary. For analysis of textual data, we decided to use the ALCESTE software conjugated to editing analyze and initial reading. Were investigated 20 women victims of domestic violence, whose author of the attacks was the husband/partner. We identified, from the respondents, that 70% (n = 14) of men with aggressive behavior also had a family history of violence and fragile family relationships. About the physical and emotional condition of the assailant at the time of violence, 50% (n = 10) of these men, regardless the use of alcohol, had often quarrelsome and/or nervous behavior, impatient and unpredictable humor facing a setback, worry or annoyance. Regarding the nature of violence, we observed that women were victims of all types of violence, however, the psychological prevailed in 100% of cases. The corpus "Men" has three classes, whose focuses are, respectively: resignation, denounce and violence/aggression, being possible categorizing them as well: Category 1: The imprisonment of women; Category 2: Violence and its meanings; Category 3: Breaking the violent cycle. We show that the social representations of man s aggressive behavior, from the women in situations of violence, are anchored in the social roles of men in family and society, becoming a dominant model of masculinity. It is aimed, on one hand, from the reproduction of what is already known and/or experienced by male aggressors in the family, as repetitions of behavior. And on the other, present themselves as a state of illness, addiction or psychopathy
