In the present study, a total of 455 enterococcal isolates, recovered from patients living in the city of Porto Alegre, State of Rio Grande do Sul, Brazil, during the period from July 1996 to June 1997, were identified to the species level by conventional biochemical and microbiological tests, and assayed for their susceptibilities to antimicrobial agents. The genetic diversity of antimicrobial resistant strains was evaluated by pulsed-field gel electrophoresis (PFGE) analysis of SmaI restricted chromosomal DNA. The most frequent species was Enterococcus faecalis (92.8%). Other species identified were: E. faecium (2.9%), E. gallinarum (1.5%), E. avium (1.1%), E. hirae (0.7%), E. casseliflavus (0.4%), E. durans (0.4%) and E. raffinosus (0.2%). The overall prevalence of isolates with high-level resistance (HLR) to aminoglycosides was 37.8%. HLR to gentamicin was found in 24.8%. No strains with acquired resistance to vancomycin were found. PFGE analysis showed the predominance of clonal group A, comprising strains isolated from different clinical specimens obtained from patients in three hospitals. These results suggest intra and inter-hospital dissemination of one predominant clonal group of E. faecalis isolates with HLR to gentamicin in the hospitals included in this study.

Estudo dos flebótomos (Diptera, Phlebotominae), vectores de Leishmania sp. no Concelho de Torres Novas, Portugal. Sofia Isabel Martins Branco PALAVRAS-CHAVE: flebótomos, bioecologia, Leishmania, Torres Novas, Portugal. Os flebótomos são insectos vectores de vários agentes patogénicos, dos quais se destacam os protozoários do Género Leishmania. Em Portugal, as leishmanioses, canina e humana, são causadas por L. infantum, sendo o cão o principal reservatório e Phlebotomus perniciosus e P. ariasi os vectores comprovados do parasita. São conhecidos três focos de doença, mas casos de leishmaniose canina têm sido reportados em outras regiões nas quais se desconhecem as espécies flebotomínicas presentes e respectivas taxas de infecção. Neste trabalho, efectuou-se a primeira prospecção flebotomínica no Concelho de Torres Novas, Distrito de Santarém, localizado na região Centro de Portugal. Os principais objectivos foram determinar a fauna flebotomínica do Concelho, os aspectos bioecológicos, as taxas de infecção por Leishmania e os factores de risco para a transmissão vectorial. De Junho a Novembro de 2010, 275 biótopos foram prospectados com armadilhas CDC. As capturas foram realizadas em 91 localidades, nas 17 freguesias do Concelho, e incluíram habitats domésticos, peridomésticos e silváticos. Os exemplares capturados foram identificados morfologicamente, as fêmeas utilizadas para detecção molecular de DNA de Leishmania e identificação das refeições sanguíneas. Análises de regressão simples e múltipla foram utilizadas para avaliação dos factores de risco para a presença das várias espécies flebotomínicas. Testes não paramétricos foram usados para comparar densidades. Dos 1262 flebótomos capturados, quatro espécies foram assinaladas com as seguintes abundâncias relativas: P. perniciosus 73,69%, P. ariasi 8,16%, P. sergenti 6,58% e Sergentomyia minuta 11,57%. Em 82% das localidades prospectadas foi detectada pelo menos uma espécie flebotomínica e em 71,4% destas foi capturada pelo menos uma das espécies comprovadamente vectoras de L. infantum. P. perniciosus foi assinalado em todas as 17 freguesias do Concelho. Os factores de risco identificados foram: temperaturas elevadas e humidades relativas baixas, locais abrigados e ausência de vento forte, presença de pinheiros como vegetação dominante, biótopos peridomésticos, particularmente currais de ovelhas e coelheiras, ou na proximidade de ovelhas, aves de capoeira e ninhos com andorinhas. A taxa de infecção flebotomínica por L. infantum foi de 4% para P. ariasi e de 0,32% para o total de fêmeas capturadas. A maioria das fêmeas para as quais se identificou a origem da refeição sanguínea pertencia a P. perniciosus. Esta espécie apresentou um comportamento oportunista, alimentando-se numa grande variedade de hospedeiros vertebrados. A elevada abundância e distribuição das espécies vectoras, juntamente com a seroprevalência de Leishmania nos cães do Distrito (5-10%), e a captura de uma fêmea grávida de P. ariasi (infectante), sugerem que o Concelho de Torres Novas é um foco de leishmaniose no país. A maior abundância relativa de P. sergenti, comparando com prospecções realizadas noutras áreas da região Centro de Portugal, sugere que este potencial vector esteja a expandir-se para latitudes mais elevadas, aumentando o risco de introdução de L. tropica no território, por contacto com imigrantes ou viajantes infectados de áreas endémicas. A monitorização flebotomínica, e dos hospedeiros vertebrados, deverá ser continuada no Concelho para que medidas eficazes de controlo possam ser definidas e implementadas.

E. faecium was the first reported VRE species, carrying the vanA gene in Brazil. In spite of this, vancomycin-resistant E. faecalis has become the predominant species in Brazilian hospitals. The aim of this study was to evaluate the genetic relatedness of VREs isolated in a Brazilian teaching hospital eight years apart from its first isolation. We analyzed 38 VRE strains obtained from 81 surveillance cultures of patients admitted to the four largest intensive care units in Hospital São Paulo in February, 2006. Presence of the vanA gene was assayed by PCR and PFGE analysis was used for molecular characterization. All VRE strains carried the vanA gene. Two distinct clonal groups were observed among vancomycin-resistant E. faecalis. Vancomycin-resistant E. faecium belonged to five distinct clones were demonstrated by molecular typing. All of these clones were different from the first vancomycin-resistant enterococci clone isolated eight years ago in our hospital.

Extended-spectrum β-lactamases (ESBL) in enterobacteria are recognized worldwide as a great hospital problem. In this study, 127 ESBL-producing Enterobacteriaceae isolated in one year from inpatients and outpatients at a public teaching hospital at São Paulo, Brazil, were submitted to analysis by PCR with specific primers for blaSHV, blaTEM and blaCTX-M genes. From the 127 isolates, 96 (75.6%) Klebsiella pneumoniae, 12 (9.3%) Escherichia coli, 8 (6.2%) Morganella morganii, 3 (2.3%) Proteus mirabilis, 2 (1.6%) Klebsiella oxytoca, 2 (1.6%) Providencia rettgeri, 2 (1.6%) Providencia stuartti, 1 (0.8%) Enterobacter aerogenes and 1 (0.8%) Enterobacter cloacae were identified as ESBL producers. BlaSHV, blaTEM and blaCTX-M were detected in 63%, 17.3% and 33.9% strains, respectively. Pulsed field gel eletrophoresis genotyping of K. pneumoniae revealed four main molecular patterns and 29 unrelated profiles. PCR results showed a high variety of ESBL groups among strains, in nine different species. The results suggest the spread of resistance genes among genetically different strains of ESBL-producing K. pneumoniae in some hospital wards, and also that some strongly related strains were identified in different hospital wards, suggesting clonal spread in the institutional environment.

Antes de mais o tema: as Ciências Sociais no século XXI. É um título que revela o espírito de uma História que não é a História habitualmente descrita como narração e análise de acontecimentos pretéritos, mais ou menos conhecidos ou importantes. Esta idéia de Ciências Sociais no século XXI envolve, em 2002, um espírito prospectivo que vincula a História ao presente e, talvez mais ainda, ao futuro. As ciências sociais no século XXI, neste momento, são ainda as ciências sociais do século XX, não mudámos, nas ciências sociais e nas Humanidades, em nada de substantivo desde 31 de Dezembro de 1999 ou de 2000, como se prefira. Mas justamente as ciências sociais e humanas do século XX são (pelo menos plausivelmente) descritíveis como ciências prospectivas. Foram-no pela associação à actividade política, foram-no na ligação com a crítica social ou especializada, foram-no no domínio da investigação estritamente acadêmica que, cada vez mais, se desenvolve uniformizadamente em todas as ciências, sociais ou não. Essa ligação ao futuro pela prospecção teórica (nem sempre científica, depende da concepção de ciência que alguns ainda negam ao "social") marcou o século XX e, para todos os efeitos, é ainda sob a sua influência que se realiza tão precocemente no século XXI uma reflexão como a que este encontro promove.

This work aimed to assess pathogenic potential and clonal relatedness of Aeromonas sp. and Vibrio cholerae isolates recovered during a diarrhea outbreak in Brazil. Clinical and environmental isolates were investigated for the presence of known pathogenic genes and clonal relatedness was assessed by intergenic spacer region (ISR) 16S-23S amplification. Four Aeromonas genes (lip, exu, gcat, flaA/B) were found at high overall frequency in both clinical and environmental isolates although the lip gene was specifically absent from selected species. A fifth gene, aerA, was rarely found in A. caviae, the most abundant species. The ISR profile revealed high heterogeneity among the Aeromonas isolates and no correlation with species identification. In contrast, in all the V. cholerae isolates the four genes investigated (ctxA, tcpA, zot and ace) were amplified and revealed homogeneous ISR and RAPD profiles. Although Aeromonas isolates were the major enteric pathogen recovered, their ISR profiles are not compatible with a unique cause for the diarrhea events, while the clonal relationship clearly implicates V. cholerae in those cases from which it was isolated. These results reinforce the need for a better definition of the role of aeromonads in diarrhea and whether they benefit from co-infection with V. cholerae.

O presente estudo centra‐se na conceptualização de pormenor do modelo de circulação do aquífero termomineral de Chaves na sua zona de descarga no campo hidromineral e geotérmico de Chaves (Norte de Portugal). Para tal, utilizam‐se as informações resultantes da bibliografia disponível, da construção das captações de água termomineral AC1 e AC2, já existentes, e, principalmente, as informações obtidas a partir dos trabalhos de pesquisa, prospecção e construção da captação CC3, obra desenvolvida no ano de 2014 para reforço de caudal das Caldas de Chaves. Incidindo fundamentalmente na análise de perfis geofísicos, de parâmetros de perfuração, “cuttings” e diagrafias diferidas inerentes à sondagem de pesquisa CC3, e da sua posterior transformação em captação, integrando‐se posteriormente toda a informação daí resultante, o estudo culmina na apresentação de um modelo conceptual de pormenor para a circulação de águas tipo “Chaves” na zona de descarga que permite, com considerável robustez, à luz dos dados conhecidos à presente data, o melhor conhecimento do aquífero para futuros projectos de desenvolvimento dos recursos hidrominerais e geotérmicos de Chaves.

As poroqueratoses resultam de uma hiper-proliferação clonal dos queratinócitos, encontrando-se pelo menos descritas seis formas clínicas, que partilham o achado da lamela cornóide no exame histopatológico. Os autores descrevem o caso de uma poroqueratose de Mibelli numa mulher de 27 anos, raça negra, com início na infância, eficazmente tratada com retinóide tópico, apresentando-se as manifestações típicas de uma dermatose pouco frequente e destacando-se a importância da histopatologia na confirmação do seu diagnóstico. A poroqueratose de Mibelli é uma dermatose crónica e progressiva, raramente com remissão espontânea. A sua evolução para neoplasia maligna, particularmente carcinoma espino-celular, pode ocorrer em cerca de 7% dos doentes, reforçando-se a importância de uma adequada vigilância.

BACKGROUND: This study was designed to investigate, for the first time, the short-term molecular evolution of the HIV-2 C2, V3 and C3 envelope regions and its association with the immune response. Clonal sequences of the env C2V3C3 region were obtained from a cohort of eighteen HIV-2 chronically infected patients followed prospectively during 2-4 years. Genetic diversity, divergence, positive selection and glycosylation in the C2V3C3 region were analysed as a function of the number of CD4+ T cells and the anti-C2V3C3 IgG and IgA antibody reactivity RESULTS: The mean intra-host nucleotide diversity was 2.1% (SD, 1.1%), increasing along the course of infection in most patients. Diversity at the amino acid level was significantly lower for the V3 region and higher for the C2 region. The average divergence rate was 0.014 substitutions/site/year, which is similar to that reported in chronic HIV-1 infection. The number and position of positively selected sites was highly variable, except for codons 267 and 270 in C2 that were under strong and persistent positive selection in most patients. N-glycosylation sites located in C2 and V3 were conserved in all patients along the course of infection. Intra-host variation of C2V3C3-specific IgG response over time was inversely associated with the variation in nucleotide and amino acid diversity of the C2V3C3 region. Variation of the C2V3C3-specific IgA response was inversely associated with variation in the number of N-glycosylation sites. CONCLUSION: The evolutionary dynamics of HIV-2 envelope during chronic aviremic infection is similar to HIV-1 implying that the virus should be actively replicating in cellular compartments. Convergent evolution of N-glycosylation in C2 and V3, and the limited diversification of V3, indicates that there are important functional constraints to the potential diversity of the HIV-2 envelope. C2V3C3-specific IgG antibodies are effective at reducing viral population size limiting the number of virus escape mutants. The C3 region seems to be a target for IgA antibodies and increasing N-linked glycosylation may prevent HIV-2 envelope recognition by these antibodies. Our results provide new insights into the biology of HIV-2 and its relation with the human host and may have important implications for vaccine design.

The term “mastocytosis” denotes a heterogeneous group of disorders characterised by abnormal growth and accumulation of mast cells (MC) in one or more organ systems. Symptoms result from MC chemical mediator’s release, pathologic infiltration of neoplastic MC in tissues or both. Multiple molecular, genetic and chromosomal defects seem to contribute to an autonomous growth, but somatic c-kit D816V mutation is more frequently encountered, especially in systemic disease. We present a literature review of mastocytosis and a rare case report of an 18 month-old-girl with a bullous dermatosis, respiratory distress and anaphylaxis, as clinical manifestations of mastocytosis. The developments of accepted classification systems and novel useful markers allowed a re-evaluation and updating of the classification of mastocytosis. In paediatric age cutaneous forms of disease prevail and may regress spontaneously. SM is more frequently diagnosed in adults and is a persistent(clonal) disease of bone marrow. The clinical course in these patients is variable.Today diagnostic criteria for each disease variant are reasonably well defined. There are, however, peculiarities, namely in paediatric age, that makes the diagnostic approach difficult. Systemic disease may pose differential diagnostic problems resulting from multiple organ systems involvement. Coversly, the “unexplained” appearance of those symptoms with no skin lesions should raise the suspicion of MC disease. This case is reported in order to stress the clinical severity and difficult diagnostic approach that paediatric mastocytosis may assume.

The role of rodents in the epidemiology of toxoplasmosis was investigated inLondrina, Paraná State, Brazil. One hundred and eighty-one Rattus rattus and one Mus musculus were caught in 37 places. Blood and tissues were collected and submitted to the indirect fluorescence antibody test (IFAT) and the bioassay. Serum samples from 61 contacting dogs were also collected. Sixteen rats (8.8%) were positive for Toxoplasma gondii, but just two of them were positive by serology and bioassay test. Antibodies were found in nine (4.9%) rats. Tissues of nine rats bioassayed were positive and four isolates were obtained. Restriction fragment length polymorphism (RFLP) analysis was performed using 12 markers (SAG1, SAG2, SAG2-alt, C22-8, C29-2, L358, PK1, BTUB, GRA6, SAG3, Apico, CS3). Genotyping revealed that the four strains isolated from this study have been isolated before in cats and chickens from Brazil. None of the isolates was identified like clonal archetypal T-types I, II, and III. The rats presented lower serologic Toxoplasma gondii prevalence (8.8%) compared to contacting dogs (70.5%).
