A reação sistêmica aos traumatismos e infecções graves, reação de fase aguda, (RFA), pode determinar imunossupressão e reativação de infecções latentes. O objetivo do trabalho foi verificar, em 71 chagásicos crônicos com ousem RFÁ, a freqüência de parasitismo pelo T. cruzi na veia central da supra-renal (VCSR). Os critérios para RFA (+) foram observados em 30 chagásicos: l)morteporsepsis e/outrauma após evolução maior que umasemana e 2)presença de úlceras de stress sangrantes, ou 3) hiperplasia reacional do baço ou 4) esteatose hepática. Registrou-se peso e altura e calculou-se o índice de massa corporal (IMC). Chagásicos com RFA (+) apresentaram maior comprometimento nutricional que os RFA (-): peso = 49,0 vs 54,5 kg; IMC = 17,5 vs 20,6kg/m2 (mediana p< Q,05). O parasitismo na VCSR não diferiu entre os grupos: 43,3% e 43,9%, respectivamente. Concluímos que os chagásicos com RFA (+) são mais subnutridos que os RFA (-) e que o desenvolvimento pré-óbito de RFA não afeta a frequência de parasitismo na VCSR.

The effect of an 8 hour-period of water deprivation on fluid and electrolyte renal excretion was investigated in male Wistar rats infected with the strain São Felipe (12SF) of Trypanosoma cruzi, in comparison with age and sex matched non-infected controls. The median percent reductions in the urinary flow (-40% v -63%) and excretion ofsodium (-57% v-79%) were smaller in chagasic than in control rats, respectively. So, chagasic rats excreted more than controls. On the other hand, the median percent decrement in the clearance of creatinine was higher in chagasic (-51%) than in controls (-39%). Thus, chagasic rats showed some disturbed renal hydroelectrolytic responses to water deprivation, expressed by smaller conservation, or higher excretion of water and sodium in association with smaller glomerularfiltration rate. This fact denoted an elevation in the fractional excretion of sodium and water.

Verificou-se a ocorrência de alterações em alguns parâmetros da função renal de indivíduos infectados pelo virus da imunodeficiência humana (HIV), assintomáticos. Foram estudados 47 indivíduos, realizando-se provas funcionais renais: depuração de creatinina endógena, depuração de água livre, depuração osmolar, reabsorção tubular proximal e distal de sódio, excreção fracional de sódio e potássio e pH urinário. Os resultados revelaram diferenças significantes (p < 0,05) no pH urinário, maior no grupo HIV (6,36 ± 0,41), do que nos controles (6,02 ± 0,41); na depuração de água livre, que indicou reabsorção de água maior no grupo HIV (1,00 ± 0,64ml/min), do que nos controles (0,53 ± 0,48ml/min) e na depuração osmolar, que foi 2,00 ± 0,83ml/min no grupo HIV e 1,57 ± 0,48ml/min nos controles. O restante dos indicadores de função renal estudados não se mostraram diferentes estatisticamente entre um e outro grupo. Concluiu-se que essas diferenças são significativas, apesar de os valores absolutos estarem dentro da normalidade, pois podem estar associadas a alterações evolutivas tardias da doença, tais como o aumento na frequência de infecções do trato urinário e a hiponatremia dilucional. Mais estudos são necessários para se confirmarem essas hipóteses.

Na síndrome da imunodeficiência adquirida (AIDS) pode-se verificar o acometimento da supra-renal por efeito citopático direto pelo HIV, por infecções oportunistas ou neoplasias. Estes achados poderiam variar de acordo com a procedência do paciente, devido às doenças peculiares à região. Neste trabalho avaliou-se o comprometimento da supra-renal em quatorze pacientes que morreram de AIDS no Hospital Escola, em Uberaba. Treze eram do sexo masculino e treze brancos. A idade foi de 29,9 ± 7,8 anos e o índice de massa corporal foi de 19 ± 4,1kg/m2. Os fragmentos de supra-renal obtidos nas necropsias foram analisados em microscópio de luz. Encontramos inflamação em 100% dos casos, identificando-se o agente etiológico em oito (58,1%) casos. O Citomegalovírus foi identificado em sete casos, o Cryptococcus sp e o Herpes simplex em dois e o Histoplasma sp em um caso, estes achados são semelhantes aos da literatura. Em um caso, encontramos calcificação do parênquima e em outro, flebite da veia central. Em alguns casos que apresentavam lesão não foi possível identificar o agente etiológico, talvez em decorrência do efeito citopático direto pelo HIV ou devido a toxicidade das drogas utilizadas no tratamento da AIDS e das infecções oportunistas.

The frequency of infection by Cryptosporidium parvum was determined in two groups of renal patients submitted to immunosuppression. One group consisted of 23 renal transplanted individuals, and the other consisted of 32 patients with chronic renal insufficiency, periodically submitted to hemodialysis. A third group of 27 patients with systemic arterial hypertension, not immunosuppressed, was used as control. During a period of 18 months all the patients were submitted to faecal examination to detect C. parvum oocysts, for a total of 1 to 6 tests per patient. The results showed frequencies of C. parvum infection of 34.8%, 25% and 17.4%, respectively, for the renal transplanted group, the patients submitted to hemodialysis and the control group. Statistical analysis showed no significant differences among the three groups even though the frequency of C. parvum infection was higher in the transplanted group. However, when the number of fecal samples containing C. parvum oocysts was taken in account, a significantly higher frequency was found in the renal transplanted group.

Fluids in which Mycobacterium tuberculosis are seldom found, such as pleural and cerebrospinal liquids, are good candidates to be studied using PCR techniques. We detail our experience with a PCR assay applied to pleural and cerebrospinal fluids using the primer MPB64. Seventy three specimens were analyzed: 30 pleural fluids (PF), 26 pleural biopsies (PB) and 17 cerebrospinal fluids (CSF). The gold standard for the diagnosis of tuberculous meningitis was the positive culture for M. tuberculosis in CSF. Tuberculous pleural effusion was diagnosed when cultures of PF and/or PB were positive for M. tuberculosis, or the PB histology showed granulomas. Our results, compared to the gold standards employed, showed a sensitivity of 70%, specificity of 88%, positive predictive value of 82% and negative predictive value of 80%. The high specificity of the MPB64 fragment while still retaining a good sensitivity makes it very well suited for pleural and cerebrospinal tuberculosis diagnosis.

This study aimed to compare the radiographic characteristics of patients with pulmonary tuberculosis (TB) and human immunodeficiency virus (HIV) infection with those of HIV-negative patients. In all, 275 TB patients attending the outpatients clinics at the University Hospital/UFPE, were studied from January 1997 to March 1999. Thirty nine (14.2%) of them were HIV+, with a higher frequency of males in this group (p=0.044). Seventy-five percent of the HIV+ patients and 19% of the HIV- had a negative tuberculin test (PPD) (p < 0.001). The proportion of positive sputum smears in the two groups was similar. The radiological finding most strongly associated with co-infection was absence of cavitation (p < 0.001). It may therefore be concluded that the lack of cavitation in patients with pulmonary TB may be considered a useful indicator of the need to investigate HIV infection. This approach could contribute to increasing the effectiveness of local health services, by offering appropriate treatment to co-infected patients.
Paradoxical reaction to the treatment of tuberculosis uncovering previously silent meningeal disease

The development of paradoxical clinical worsening following initiation of tuberculosis treatment may complicate the clinical course of both HIV-infected and uninfected patients. We report a severe manifestation of the so called paradoxical reaction to the treatment of tuberculosis that unmasked previously silent meningeal disease in a 34-year-old HIV-infected male patient.

We report a significantly higher prevalence of intestinal nematodes in patients with pulmonary tuberculosis (TB) compared to a matched control group: 33/57 (57.8%) in patients with TB and 18/86 (20.9%) in the control group; OR=5.19; 95% CI= 2.33-11.69; p=0.000). When TB patients eosinophilia was also significantly higher among those with intestinal parasites (69.8%) compared to those without this condition (45.6%). We hypothesized that the immune modulation induced by nematodes is a factor that enhances TB infection/progression and that eosinophilia seen in TB patients is a consequence of helminth infection.
