In Northern Ireland over 127,000 children bring their own food to school so it is important to ensure that food is both appetising and is good for health. Recent surveys looking at Childrens lunchboxes revealed that they are often too high in fat, salt and sugar, with just under half of the lunchboxes containing no fruit. To help parents and carers make lunchboxes healthier, the PHA has produced a leaflet, Are you packing a healthy lunch? in collaboration with safefood, the Department of Health, Social Services and Public Safety and the Department of Education.It will be distributed to every child in primary school and contains practical tips for parents, carers and children on how to pack an appetising, healthy lunchbox.

Today the Public Health Agency is launching a new resource pack designed to assist nursery schools and playgroups deliver a healthy breaks scheme.All nursery schools and playgroups in Northern Ireland will receive the pack - 'Healthy breaks for pre-school children' - which includes a poster and information leaflets for parents explaining why a healthy break is so important for pre-school children and some tips and ideas for healthy nutritious breaks.Judith Hanvey, Regional Food in Schools Co-ordinator, appointed jointly by the PHA and the Department of Education, said: "Break-time snacking schemes currently operate in many pre-school settings across Northern Ireland, however they are implemented in different ways. The PHA has developed this new guidance so that any existing or future schemes have the information that they need to deliver a healthy break programme."Judith explained why a healthy break is so important for pre-school pupils."Young children have small stomachs but high requirements for energy (calories) and nutrients. This means that healthy snacks between meals are very important to make sure that they get all the nourishment they need."Childhood is also an important time to establish good eating and drinking habits for later life."Healthy eating schemes give children the knowledge and opportunity to make healthier choices."The Department of Education and Department of Health, Social Services and Public Safety recently launched 'Healthy Food for Healthy Outcomes: Food in Schools Policy', which builds on the nutritional standards which were introduced in 2007 by the Department of Education.The policy encourages a 'whole school approach' to all food and drink provided and consumed in schools. This ensures that children have the opportunity to benefit from a healthy balanced diet, which can aid learning, leading to improved educational outcomes. It also requires that every school should have their own whole-school food policy in place by September 2014. Nursery schools that receive funding from the Department of Education must follow this policy.This is also in line with the position taken by the Department of Health, Social Services and Public Safety's 10 year obesity prevention strategy, A Fitter Future for All, which aims to take a consistent approach to healthy food within early years settings.

The Northern Ireland Abdominal Aortic Aneurysm (AAA) Screening Programme began in June 2012 and offers AAA screening to all men aged 65 and over in Northern Ireland. All men in their 65th year receive an invite in the post. All men aged over 65 who have not been screened can request an appointment. The aim of the AAA screening programme is to reduce AAA-related mortality by providing systematic, population-based screening.� This poster was sent out to all GPs, GP practice managers and pharmacies as part of a professional information pack in the run-up to the launch of the programme. The poster is also distributed to relevant voluntary organisations and charities as a means of raising awareness among the target population.

Purpose: Several studies have shown that HIV patients are at higher risk of lung cancer. Our aim is to analyse the prevalence and features of lung cancer in HIV-infected patients. Methods: The clinical charts of 4,721 HIV-infected patients seen in three hospitals of southeast Spain (study period 1992-2012) were reviewed, and all patients with a lung cancer were analysed. Results: There were 61 lung cancers, giving a prevalence of 1.2%. There was a predominance of men (82.0%), and smokers (96.6%; mean pack-years 35.2), with a median age of 48.0 (41.7-52.9) years, and their distribution according to risk group for HIV was: intravenous drug use 58.3%, homosexual 20.0%, and heterosexual 16.7%. Thirty-four (56.7%) patients were Aids cases, and 29 (47.5%) had prior pulmonar events: tuberculosis 16, bacterial pneumonia 9, and P. jiroveci pneumonia 4. The median nadir CD4 count was 149/mm3 (42-232), the median CD4 count at the time of diagnosis of the lung cancer was 237/mm3 (85-397), and 66.1% <350/mm3. 66.7% were on ART, and 70% of them had undetectable HIV viral load. The most common histological types of lung cancer were adenocarcinoma and epidermoid, with 24 (40.0%) and 23 (38.3%) cases, respectively. There were 49 (80.3%) cases with advanced stages (III and IV) at diagnosis. The distribution of treatments was: only palliative 23 (39.7%), chemotherapy 14 (24.1%), surgery and chemotherapy 8 (13.8%), radiotherapy 7 (12.1%), surgery 4 (6.9%), and other combined treatments 2 (3.4%). Forty-six (76.7%) patients died, with a median survival time of 3 months. The Kaplan-Meier survival rate at 6 months was 42.7% (at 12 months 28.5%). Conclusions: The prevalence of lung cancer in this cohort of HIV-patients is high. People affected are mainly men, smokers, with transmission of HIV by intravenous drug use, and around half of them with prior opportunistic pulmonary events. Most patients had low nadir CD4 count, and were immunosuppressed at the time of diagnosis. Adenocarcinoma is the most frequent histological type. The diagnosis is usually made at advanced stages of the neoplasm, and mortality is high.

During the second half of the nineteenth century a major mineral water bottling industry appeared in Catalonia which vigorously lasted until the first third of the 20th century. The fact that the industry appeared in Catalonia and in other parts of Europe and the United States almost at the same time and had not existed before can be explained by a series of factors which coincided in time. This situation encouraged producers to pack, transport and sell bottled water from their respective sources. Among these factors there is the rise of hygienism, very influential in Catalonia, the declining water quality due to industrialization, the increase in population density, the improvement in transport, the emergence of thermal tourism or the invention of better containers used to store water. This project aims to explain thoroughly all the mentioned factors, and to give some light to why, when and how the Catalan bottled water industry appeared.

BACKGROUND: Nicotine dependence is the major obstacle for smokers who want to quit. Guidelines have identified five effective first-line therapies, four nicotine replacement therapies (NRTs)--gum, patch, nasal spray and inhaler--and bupropion. Studying the extent to which these various treatments are cost-effective requires additional research. OBJECTIVES: To determine cost-effectiveness (CE) ratios of pharmacotherapies for nicotine dependence provided by general practitioners (GPs) during routine visits as an adjunct to cessation counselling. METHODS: We used a Markov model to generate two cohorts of one-pack-a-day smokers: (1) the reference cohort received only cessation counselling from a GP during routine office visits; (2) the second cohort received the same counselling plus an offer to use a pharmacological treatment to help them quit smoking. The effectiveness of adjunctive therapy was expressed in terms of the resultant differential in mortality rate between the two cohorts. Data on the effectiveness of therapies came from meta-analyses, and we used odds ratio for quitting as the measure of effectiveness. The costs of pharmacotherapies were based on the cost of the additional time spent by GPs offering, prescribing and following-up treatment, and on the retail prices of the therapies. We used the third-party-payer perspective. Results are expressed as the incremental cost per life-year saved. RESULTS: The cost per life-year saved for only counselling ranged from Euro 385 to Euro 622 for men and from Euro 468 to Euro 796 for women. The CE ratios for the five pharmacological treatments varied from Euro 1768 to Euro 6879 for men, and from Euro 2146 to Euro 8799 for women. Significant variations in CE ratios among the five treatments were primarily due to differences in retail prices. The most cost-effective treatments were bupropion and the patch, and, then, in descending order, the spray, the inhaler and, lastly, gum. Differences in CE between men and women across treatments were due to the shape of their respective mortality curve. The lowest CE ratio in men was for the 45- to 49-year-old group and for women in the 50- to 54-year-old group. Sensitivity analysis showed that changes in treatment efficacy produced effects only for less-well proven treatments (spray, inhaler, and bupropion) and revealed a strong influence of the discount rate and natural quit rate on the CE of pharmacological treatments. CONCLUSION: The CE of first-line treatments for nicotine dependence varied widely with age and sex and was sensitive to the assumption for the natural quit rate. Bupropion and the nicotine patch were the two most cost-effective treatments.

INTRODUCTION: Several studies have shown an increased risk of type 2 diabetes among smokers. Therefore, the aim of this analysis was to assess the relationship between smoking, cumulative smoking exposure and nicotine dependence with pre-diabetes. METHODS: We performed a cross-sectional analysis of healthy adults aged 25-41 in the Principality of Liechtenstein. Individuals with known diabetes, Body Mass Index (BMI) >35 kg/m² and prevalent cardiovascular disease were excluded. Smoking behaviour was assessed by self-report. Pre-diabetes was defined as glycosylated haemoglobin between 5.7% and 6.4%. Multivariable logistic regression models were done. RESULTS: Of the 2142 participants (median age 37 years), 499 (23.3%) had pre-diabetes. There were 1,168 (55%) never smokers, 503 (23%) past smokers and 471 (22%) current smokers, with a prevalence of pre-diabetes of 21.2%, 20.9% and 31.2%, respectively (p <0.0001). In multivariable regression models, current smokers had an odds ratio (OR) of pre-diabetes of 1.82 (95% confidential interval (CI) 1.39; 2.38, p <0.0001). Individuals with a smoking exposure of <5, 5-10 and >10 pack-years had an OR (95% CI) for pre-diabetes of 1.34 (0.90; 2.00), 1.80 (1.07; 3.01) and 2.51 (1.80; 3.59) (p linear trend <0.0001) compared with never smokers. A Fagerström score of 2, 3-5 and >5 among current smokers was associated with an OR (95% CI) for pre-diabetes of 1.27 (0.89; 1.82), 2.15 (1.48; 3.13) and 3.35 (1.73; 6.48) (p linear trend <0.0001). DISCUSSION: Smoking is strongly associated with pre-diabetes in young adults with a low burden of smoking exposure. Nicotine dependence could be a potential mechanism of this relationship.

Although cigarette smoking and alcohol consumption increase risk for head and neck cancers, there have been few attempts to model risks quantitatively and to formally evaluate cancer site-specific risks. The authors pooled data from 15 case-control studies and modeled the excess odds ratio (EOR) to assess risk by total exposure (pack-years and drink-years) and its modification by exposure rate (cigarettes/day and drinks/day). The smoking analysis included 1,761 laryngeal, 2,453 pharyngeal, and 1,990 oral cavity cancers, and the alcohol analysis included 2,551 laryngeal, 3,693 pharyngeal, and 3,116 oval cavity cancers, with over 8,000 controls. Above 15 cigarettes/day, the EOR/pack-year decreased with increasing cigarettes/day, suggesting that greater cigarettes/day for a shorter duration was less deleterious than fewer cigarettes/day for a longer duration. Estimates of EOR/pack-year were homogeneous across sites, while the effects of cigarettes/day varied, indicating that the greater laryngeal cancer risk derived from differential cigarettes/day effects and not pack-years. EOR/drink-year estimates increased through 10 drinks/day, suggesting that greater drinks/day for a shorter duration was more deleterious than fewer drinks/day for a longer duration. Above 10 drinks/day, data were limited. EOR/drink-year estimates varied by site, while drinks/day effects were homogeneous, indicating that the greater pharyngeal/oral cavity cancer risk with alcohol consumption derived from the differential effects of drink-years and not drinks/day.

Os recursos hídricos na Bacia hidrográfica da Ribeira Seca não têm sido explorados numa base auto-sustentável. Como consequência, verifica-se a deterioração da qualidade da água dos poços e dos furos como resultado da intrusão salina que chega a avançar à volta dos quatro quilómetros para o interior da bacia hidrográfica. Na tentativa de mitigar os efeitos de uma exploração desequilibrada dos recursos em água, muitos projectos de conservação do solo e água foram levados a cabo, destacando-se o Projecto de Desenvolvimento de Bacias Hidrográficas, “Watershed Development Project” – financiado no quadro da USAID na década de 80, (mais concretamente em 1984) e que contemplava todas as bacias hidrográficas da ilha de Santiago. As principais actividades levadas a cabo no âmbito do “Watershed Development Project” foram essencialmente as seguintes: 1. Infra-estruturas hidráulicas de captação, armazenamento, adução e distribuição de água para a rega e consumo doméstico; 2. Infra-estruturas de correcção do leito da ribeira, de espalhamento e recarga das águas de escoamento superficial; 3. Instalação de bombas eólicas para bombagem da água dos poços; 4. Adaptação ao regadio de terrenos de cultura e construção de terraços; 5. Arborização das encostas em dispositivos anti-erosivos (banquetas, muretes, caldeiras, etc.) 6. Sistemas de “water harvesting/runoff farming” (incluindo construção de grandes reservatórios, diques de captação de águas de escoamento superficial); 7. Plantação de feijão Congo (Cajanus cajan) em muretes e banquetas nas encostas. No quadro do projecto de Desenvolvimento de Bacias Hidrográficas, “Watershed Development Project”, dois projectos de luta contra a intrusão salina foram elaborados. Um dos projectos contemplou a Ribeira de Saltos e outro na Ribeira Seca. Só o da Ribeira de Saltos foi executado sob a orientação do coordenador técnico do projecto e os resultados, embora evidentes, estão sendo quantificados no âmbito de um trabalho por nós conduzido. O projecto de luta contra a intrusão salina na Ribeira Seca não foi implementado e bem assim, os projectos considerados no PDH-Ribeira Seca financiados no quadro da Cooperação Austríaca.Pretende-se com este trabalho de fim de curso não só analisar os efeitos da degradação ecológica dos terrenos de cultura localizados a jusante e submetidos à intrusão salina, como também, quantificar os impactos das infra-estruturas hidráulicas executadas a jusante da Ribeira Seca, mais concretamente, o efeito da construção da Barragem de Poilão na melhoria das condições hidrológicas do troço da bacia hidrográfica a jusante.

Um incremento do nível geral de preços pode ter impacto marcante no bem-estar económico e social, já que há perda de valor da moeda e consequente deterioração do poder de compra da população, conduzindo assim ao agravamento das condições e custo de vida. Contudo, torna-se necessário investigar as causas (fatores determinantes) da inflação, pois é uma das etapas fundamentais para o controlo do processo inflacionário. Neste sentido, o presente estudo tem como objetivo principal averiguar quais os determinantes da inflação em Cabo Verde. Para tal foi utilizado o método de co-integração, recentemente proposto por Pesaran et al. (2001) – Bound Test. Os resultados obtidos evidenciam que as componentes sazonais, cambiais, monetárias, orçamentais e externas explicam grande parte da Inflação em Cabo Verde.

This paper analyses the effect of tobacco prices on the propensity tostart and quit smoking using a pool of the 1993, 1995 and 1997 editionsof the Spanish National Health Surveys. The estimates for severalparametric models of the hazard rate for starting and quitting suggestthat i) The public health measures applied as of 1992 have had asignificative effect on both reducing the hazard of starting andincreasing the hazard of quitting, ii) Prices have a very weak effect onthe hazard of starting in the male population and no significant effectin the female population, iii) The price floor of cigarrettes, proxiedby the average price of a pack of black cigarrettes, has a significanteffect on the quitting hazard which is robust across specifications andapplies to both men and women. The implied price elasticity of the timeup to quitting is situated around -1.4.

No mundo globalizado em que vivemos, o betão é um material utilizado em larga escala no mundo inteiro. Observa-se que nas diversas estruturas que fazem parte do quotidiano da população, sejam nos grandes centros urbanos, no meio rural, indústrias, estruturas públicas, vias de comunicação, portos e aeroportos; deparamos com a grande maioria das construções executadas em betão armado em condições degradantes. Os consumidores da construção civil, públicos e privados têm sofrido com a ausência de durabilidade das estruturas de betão armado. Edifícios comerciais, industriais e residenciais, de pequeno e grande porte, obras hidráulicas, no país e em todo mundo, com alguns anos de vida, podem apresentar manifestações patológicas relacionadas com uma ou mais formas de deterioração do betão armado sendo a corrosão das armaduras a mais predominante e preocupante. Salienta-se que a maioria das empresas não possui um plano de inspecção e manutenção preventiva e correctiva, as atenções são voltadas para as estruturas em betão somente quando se encontram falhas graves, e que colocam vidas humanas em risco.
