In view of the lack of researches on otomycoses in Brazil, we have tried to study their incidence, their clinical characteristics and the predisponent factors During one year, 22 suspected cases were seen, 20 of them corresponded to otomycosis infections. The most frequent species were Aspergillus niger (35%) and Candida albicans (20%). The genus Aspergillus represented 75% of the isolates. Itching and hyperaemia (70%), otalgia (65%), hipoacusia (50%) were the commonest signs. Lack of cerumen (70%) chronic otitis (30%) previous antibiotic therapy and eczema (25%) were the most outstanding predisponent factors.

A total of 479 diarrhoeic children and 337 children without diarrhoea (controls) less than 5 years old were investigated in a two-year study in the city of S. Luís (MA), with the purpose to determine the incidence, the age distribution and the seasonality of rotaviruses, as well as to establish the severity of the disease in this region between the North and the Northeast of Brazil. rotavirus incidence was highest in children of the 1st. year of life, showing an average of 25% per year among the diarrhoeic patients attending the two main hospitals and three health units at the periphery of the city. It was shown that rotaviruses are significant enteropathogens in children less than 18 months old. Frequency of rotaviruses droped in diarrhoeic patients 18 to 23 months old to only 4%, the same percentage observed in children of the control group. A typical seasonal distribution of rotaviruses was not seen during the two years of study. There was a peak in the incidence of rotaviruses in 1986, during the rainy season, and two peaks in 1987, one in the rainy season and one in the dry season. It was also shown that severity of diarrhoea in rotavirus positive cases was higher than in the negative cases. Rotavirus diarrhoeic patients had more loose stools per day, and higher frequencies of vomiting and fever, resulting more often (> 2 times) in moderate or severe dehydration. Finally, it is concluded that the introduction of immunoprophylaxis may reduce significantly the high mortality rates in early childhood observed in S. Luís.

The aim of this research was to evaluate the protein polymorphism degree among seventy-five C. albicans strains from healthy children oral cavities of five socioeconomic categories from eight schools (private and public) in Piracicaba city, São Paulo State, in order to identify C. albicans subspecies and their similarities in infantile population groups and to establish their possible dissemination route. Cell cultures were grown in YEPD medium, collected by centrifugation, and washed with cold saline solution. The whole-cell proteins were extracted by cell disruption, using glass beads and submitted to SDS-PAGE technique. After electrophoresis, the protein bands were stained with Coomassie-blue and analyzed by statistics package NTSYS-pc version 1.70 software. Similarity matrix and dendrogram were generated by using the Dice similarity coefficient and UPGMA algorithm, respectively, which made it possible to evaluate the similarity or intra-specific polymorphism degrees, based on whole-cell protein fingerprinting of C. albicans oral isolates. A total of 13 major phenons (clusters) were analyzed, according to their homogeneous (socioeconomic category and/or same school) and heterogeneous (distinct socioeconomic categories and/or schools) characteristics. Regarding to the social epidemiological aspect, the cluster composition showed higher similarities (0.788 < S D < 1.0) among C. albicans strains isolated from healthy children independent of their socioeconomic bases (high, medium, or low). Isolates of high similarity were not found in oral cavities from healthy children of social stratum A and D, B and D, or C and E. This may be explained by an absence of a dissemination route among these children. Geographically, some healthy children among identical and different schools (private and public) also are carriers of similar strains but such similarity was not found among other isolates from children from certain schools. These data may reflect a restricted dissemination route of these microorganisms in some groups of healthy scholars, which may be dependent of either socioeconomic categories or geographic site of each child. In contrast to the higher similarity, the lower similarity or higher polymorphism degree (0.499 < S D < 0.788) of protein profiles was shown in 23 (30.6%) C. albicans oral isolates. Considering the social epidemiological aspect, 42.1%, 41.7%, 26.6%, 23.5%, and 16.7% were isolates from children concerning to socioeconomic categories A, D, C, B, and E, respectively, and geographically, 63.6%, 50%, 33.3%, 33.3%, 30%, 25%, and 14.3% were isolates from children from schools LAE (Liceu Colégio Albert Einstein), MA (E.E.P.S.G. "Prof. Elias de Melo Ayres"), CS (E.E.P.G. "Prof. Carlos Sodero"), AV (Alphaville), HF (E.E.P.S.G. "Honorato Faustino), FMC (E.E.P.G. "Prof. Francisco Mariano da Costa"), and MEP (E.E.P.S.G. "Prof. Manasses Ephraim Pereira), respectively. Such results suggest a higher protein polymorphism degree among some strains isolated from healthy children independent of their socioeconomic strata or geographic sites. Complementary studies, involving healthy students and their families, teachers, servants, hygiene and nutritional habits must be done in order to establish the sources of such colonization patterns in population groups of healthy children. The whole-cell protein profile obtained by SDS-PAGE associated with computer-assisted numerical analysis may provide additional criteria for the taxonomic and epidemiological studies of C. albicans.

Since October 2001, the Adolfo Lutz Institute has been receiving vesicular fluids and scab specimens of patients from Paraíba Valley region in the São Paulo and Minas Gerais States and from São Patricio Valley, in the Goiás State. Epidemiological data suggested that the outbreaks were caused by Cowpox virus or Vaccinia virus. Most of the patients are dairy milkers that had vesiculo-pustular lesions on the hands, arms, forearms, and some of them, on the face. Virus particles with orthopoxvirus morphology were detected by direct electron microscopy (DEM) in samples of 49 (66.21%) patients of a total of 74 analyzed. Viruses were isolated in Vero cell culture and on chorioallantoic membrane (CAM) of embryonated chicken eggs. Among 21 samples submitted to PCR using primers for hemagglutinin (HA) gene, 19 were positive. Restriction digestion with TaqI resulted in four characteristic Vaccinia virus fragments. HA nucleotide sequences showed 99.9% similarity with Cantagalo virus, described as a strain of Vaccinia virus. The only difference observed was the substitution of one nucleotide in the position 616 leading to change in one amino acid of the protein in the position 206. The phylogenetic analysis showed that the isolates clustered together with Cantagalo virus, other Vaccinia strains and Rabbitpox virus.

The protozoan Cryptosporidium sp. has been frequently detected in faeces from children with persistent diarrhoea. This work achieved to investigate an outbreak of cryptosporidiosis, in a day care center, attending children of high socio-economic level, between 0 and six years old. The outbreak was detected through the network of public health, when stool samples, not diarrhoeic, were examined at the Parasitology Service of the Adolfo Lutz Institute. Among the 64 examined children, 13 (20.3%) showed oocysts of Cryptosporidium sp. in the faeces examined by Kinyoun technique: seven children one year old, three, two years old and three, three years old. Among the 23 examined adults, only a 22 years old woman, possibly having an immunocomprometiment, was positive. Clinical and epidemiological aspects were investigated by questionnaires, highlighting the occurrence of the outbreak in a very dry period.

A study was carried out in the area of influence of the Porto Primavera Hydroelectric Power Station, in western São Paulo State, to investigate ecological and epidemiological aspects of malaria in the area and monitor the profile of the anopheline populations following the environmental changes brought about by the construction of the lake. Mosquitoes captured were analyzed by standardized indicator species analysis (ISA) before and during different flooding phases (253 m and 257 m elevations). The local human population was studied by means of parasitological (thin/thick blood smears), molecular (PCR) and serological tests. Serological tests consisted of Enzyme Linked Immunosorbent Assay (ELISA) with synthetic peptides of the circumsporozoite protein (CSP) from classic Plasmodium vivax, P. vivax variants (VK247 and "vivax-like"), P. malariae and P. falciparum and Indirect Immunofluorescence Assay (IFA) with asexual forms of P. vivax, P. malariae and P. falciparum. The results of the entomological survey indicated that, although the Anopheles darlingi population increased after the flooding, the population density remained very low. No malaria, parasite infection or DNA was detected in the inhabitants of the study area. However, there was a low frequency of antibodies against asexual forms and a significant prevalence of antibodies against P. vivax, P. vivax variants, P. falciparum and P. malariae; the presence of these antibodies may result from recent or less recent contact with human or simian Plasmodium (a parallel study in the same area revealed the existence of a sylvatic cycle). Nevertheless, these results suggest that, as in other places where malaria is present and potential vectors circulate, the local epidemiological conditions observed could potentially support the transmission of malaria in Porto Primavera Lake if infected individuals are introduced in sufficient numbers. Further studies are required to elucidate the phenomena described in this paper.

The objective of the present study was to evaluate the serum viral load in chronically infected Hepatitis B virus (HBV) patients and to investigate the distribution of HBV genotypes in São Paulo city. Quantitative HBV-DNA assays and HBV genotyping have gained importance for predicting HBV disease progression, have been employed for assessing infectivity, for treatment monitoring and for detecting the emergence of drug resistance. Twenty-nine Brazilian patients with suspected chronic hepatitis B were studied, using real time PCR for viral load determination and direct DNA sequencing for the genotyping. The serology revealed chronic HBV infection in 22 samples. The HBV-DNA was positive in 68% samples (15/22). The phylogenetic analysis disclosed that eleven patients were infected with HBV genotype A, two with genotype F and two with genotype D. Thus, the genotype A was the most prevalent in our study.

Colombian physician Luis Benigno Patiño Camargo was one of the pioneers in the study of rickettsioses in South America, demonstrating for the first time in Colombia the presence of Rickettsia rickettsii as the etiological agent of a highly deadly exanthematic febrile syndrome in the 1930s. However, Patiño-Camargo performed other investigations from 1917-1943, which represent the first descriptions and scientific evidence of the presence ofR. prowazekii and R. typhi in Colombia. Almost 60 years after the latest research conducted by Dr. Patiño-Camargo, rickettsioses were again a matter of interest and research. In the last decade over 20 research studies have been published, showing new endemic areas forR. rickettsii, as well as the description of new rickettsial species in Colombia.

Os autores apresentam os resultados de levantamentos seccionais sôbre a prevalência e a morbidade da esquistossomose mansônica nas zonas rural e urbana do município de São Lourenço da Mata. A prevalência da esquistossomose e de outras helmintoses foi muito elevada. As formas hepato-esplénicas variaram de 1 a 2 %.

De um total de 545 amostras de sangue, colhidas ao acaso entre a população rural do município de São Lourenço do Sul (RS) e submetidas a reação de fixação de complemento, 21 reagiram positivamente para a Doença de Chagas (3,85%). Não houve diferença significativa com relação ao sexo, cor ou etnia, dos examinados. Os índices de prevalência mostraram-se mais elevados nas zonas periféricas do município, contíguas as áreas endêmicas dos municípios vizinhos. A análise dos eletrocardiogramas revelou 57,4% de alterações nos positivos contra 26,72% nos negativos com diferença estatisticamente significativa a favor dos positivos. A medida de tensão arterial mostrou que entre 442 pessoas acima dos 18 anos, 154 (34,84%) apresentaram valores tensionais aumentados, sem diferença significativa entre os sexos ou os grupos étnicos.

Com o objetiiro de identificar a espécie de Leishmania envolvida nas afecções cutâneas de indivíduos residentes nos municípios de Cosmópolis e Indaiatuba, região de Campinas, Estado de São Paulo, correlacionamos dados clínicos, histopatológicos e testes de bibridização in situ. Os expressivos índices de incidência de leishmaniose tegumentar americana obsewados nesses municípios nos levaram a iniciar esses estudos. Nesse trabalho, apresentamos os dados relativos a seis indivíduos. As características das lesões, ulceradas, de dificil cicatrização e presentes em locais expostos do corpo sugeriram quadros de leishmaniose tegumentar americana. Os testes de intradermoreação de Montenegro, positivos em cinco dos seis pacientes analisados também reforçaram a suspeita de leishmaniose tegumentar americana. Os padrões histopatológicos, como reação exsudativa celular e granulomatosa e a dificuldade de isolamento dos parasitas obtidos de biópsias de lesões são compatíveis com aqueles descritos para Leishmania (Viannia) braziliensis. Em testes de bibridização in situ do DNA do cinetoplasto de amastigotas das lesões dos seis pacientes, obseivamos que quatro deles apresentavam sinais de bibridização com a sonda de L. (Viannia) braziliensis, confirmando as suspeitas de que a Leishmania responsável pelas afecções cutâneas nos pacientes analisados era do subgenêro Viannia e do complexo braziliensis.

Nos países em desenvolvimento, particularmente nas zonas remotas, os exames médicos imagiológicos são quase inexistentes devido ao elevado custo de aquisição e manutenção. A Organização Mundial de Saúde (OMS) considera essencial a difusão da ecografia por ser um exame imagiológico de baixo custo, rápido e de grande fiabilidade, se realizada por um técnico formado. O advento da ecografia portátil diminuiu os custos, aumentando a flexibilidade da ecografia. Assim como o estetoscópio revolucionou a semiologia, é de prever que os ecógrafos de bolso se introduzam como complemento ao exame físico, permitindo a validação do diagnóstico semiológico. Durante um período de 4 meses utilizou-se um ecógrafo de bolso na prática clínica de uma missão humanitária em zona remota no distrito de Angolares em São Tomé e Príncipe. O clínico, através de um formulário de relato de caso (FRC), registou o diagnóstico, confiança no diagnostico (escala de Likert) e gestão terapêutica, baseado na semiologia, sendo esta informação reavaliada após a ecografia de bolso. Foram realizados 84 exames em 76 pacientes (12,8% dos pacientes observados). Os FRC foram revistos por pares, determinando a contribuição com informação relevante em 79,8% dos casos, alteração do diagnóstico em 52,4% e gestão terapêutica em 59,5%. Foi descrito um aumento na confiança do clínico no diagnóstico (p<0,01) e diminuição nos custos globais da gestão terapêutica (p<0,05). Do primeiro período de dois meses para o segundo verificou-se um aumento do risco relativo da ecografia não contribuir com informação relevante [4,31; 95% CI 1,5-12; χ2=9,87, p<0,01] e em simultâneo um decréscimo de exames por paciente observado. Neste contexto, a ecografia de bolso aumentou a qualidade do diagnostico clínico e diminuiu os custos com a gestão terapêutica, reduzindo o tempo até ao diagnostico definitivo e os riscos inerentes a terapêuticas desnecessárias. Esta tecnologia, como complemento do exame físico, embora dependente do observador, aparenta ser benéfica para os pacientes em zonas remotas. O aumento da exactidão de diagnostico semiológico durante o estudo sugere a necessidade de investigação sobre a aprendizagem informal e ecografia de bolso.
