Abstract Aim The purpose of the present study was to assess the dietary fat intake, glucose, insulin, Homeostasis model assessment for insulin resistance HOMA-IR, and endotoxin levels and correlate them with adipokine serum concentrations in obese adolescents who had been admitted to long-term interdisciplinary weight-loss therapy. Design The present study was a longitudinal clinical intervention of interdisciplinary therapy. Adolescents (n = 18, aged 15–19 y) with a body mass index > 95th percentile were admitted and evaluated at baseline and again after 1 year of interdisciplinary therapy. We collected blood samples, and IL-6, adiponectin, and endotoxin concentrations were measured by ELISA. Food intake was measured using 3-day diet records. In addition, we assessed glucose and insulin levels as well as the homeostasis model assessment for insulin resistance (HOMA-IR). Results The most important finding from the present investigation was that the long-term interdisciplinary lifestyle therapy decreased dietary fat intake and endotoxin levels and improved HOMA-IR. We observed positive correlations between dietary fat intake and endotoxin levels, insulin levels, and the HOMA-IR. In addition, endotoxin levels showed positive correlations with IL-6 levels, insulin levels and the HOMA-IR. Interestingly, we observed a negative correlation between serum adiponectin and both dietary fat intake and endotoxin levels. Conclusions The present results indicate an association between dietary fat intake and endotoxin level, which was highly correlated with a decreased pro-inflammatory state and an improvement in HOMA-IR. In addition, this benefits effect may be associated with an increased adiponectin level, which suggests that the interdisciplinary therapy was effective in improving inflammatory pathways.

INTRODUÇÃO: A entorse de tornozelo é uma das lesões mais comuns em atletas. Uma forma de avaliar a frouxidão ligamentar pode ser através da medida da amplitude passiva dos movimentos de inversão e eversão do pé para estimar a resistência passiva das estruturas capsuloligamentares do tornozelo, o qual pode ser chamado de torque de resistência passiva. Existem poucos estudos que utilizam a avaliação do torque passivo do tornozelo para avaliar a resistência da cápsula e dos ligamentos. OBJETIVO: O objetivo deste estudo foi comparar o torque passivo dos movimentos de inversão e eversão do pé em atletas com e sem história de entorse de tornozelo. MÉTODO: Participaram do estudo 32 atletas de basquetebol e voleibol feminino (16,06 ± 0,8 anos, 67,63 ± 8,17kg, 177,8 ± 6,47cm). Seus tornozelos foram divididos em dois grupos: grupo controle (29), composto por tornozelos sem sintomas, e grupo entorse de tornozelo, composto por tornozelos que sofreram lesão (29). O torque dos movimentos passivos do tornozelo foi registrado por um dinamômetro isocinético, e a atividade dos músculos fibular longo e tibial anterior foi medida por um eletromiógrafo. As atletas realizaram duas repetições do movimento de inversão e eversão, nas velocidades de 5, 10 e 20°/s e, em seguida, o mesmo protocolo foi repetido apenas para o movimento de inversão máxima do pé. RESULTADOS: O torque de resistência passiva durante os movimentos de inversão e eversão do pé foi menor no grupo com entorse do tornozelo. Este grupo também mostrou menor torque durante o movimento de inversão máxima do pé. Não foram observadas diferenças entre o movimento de inversão e eversão. CONCLUSÕES: A entorse de tornozelo leva a um menor torque de resistência passiva, indicando redução da resistência dos ligamentos colaterais do tornozelo e uma frouxidão articular mecânica.

AIM: The purpose of the present study was to analyze the relationship between root formation of the first premolars and skeletal maturation stages identified in hand-wrist radiographs. METHODS: A cross-sectional study was carried out involving the panoramic and hand-wrist radiographs obtained on the same date of 232 patients, 123 boys and 109 girls aged 4 years and 5 months to 17 years and 12 months. Root formation stages of the first premolars were related to the ossification stages of the sesamoid bone, epiphyseal stages of the phalanx of the thumb and epiphyseal stages of the radius. RESULTS: The studied variables demonstrated statistically significant correlations. CONCLUSION: Roots of the lower first premolars do not reach 2/3 of their complete length before adolescence.

Objetivo: Dente Duplo (“double tooth”) é uma anomalia de forma que ocorre pela fusão de dois ou mais dentes ou geminação de um único dente, geralmente durante a fase de morfodiferenciação do germe dentário. A incidência de dente duplo varia 0,14-5,0% da população mundial, sem predileção pelo gênero. Apresentação unilateral é mais comum do que a bilateral. No geral, as anomalias dentarias em pacientes com fissura labiopalatina ocorrem próximas à região da fissura. O objetivo deste trabalho é relatar o tratamento endodôntico não cirúrgico de um caso de dentes duplos em um paciente com fissura labiopalatina. Relato de Caso: Paciente de gênero feminino de 10 anos com fissura labiopalatina matriculado no Hospital de Reabilitação de Anomalias Craniofaciais (HRAC-USP), encaminhado ao setor de Endodontia do HRAC-USP para realizar o tratamento endodôntico dos dentes 11 e 12 para ser re-anatomizados. Pela superposição dos dentes 11, 12, e presença de uma cúspide acessória (“talon cusp”) além de um dente supranumerário, as radiografia panorâmica e a radiografia periapical não mostram claramente a anatomia externa do dente e a interna dos condutos radiculares, motivo pelo qual foi solicitada uma tomografia computadorizada Cone-Beam para determinar o comprimento de trabalho para preparo biomecânico e obturação dos condutos radiculares. Serão apresentadas as etapas de diagnóstico e tratamento realizados no caso. Conclusão: Pela morfologia anormal da coroa e complexidade dos canais radiculares, o tratamento endodôntico apresentaria dificuldades, por isso o exame clínico e radiográfico cuidadoso é essencial para o sucesso do tratamento endodôntico. Desta forma concluise que a imagem fornecida pela tomografia computadorizada Cone-Beam foi útil no planejamento e tratamento deste caso.

The utopian communities of Finns are linked to world history and the great ideological foundations behind numerous utopian endeavors. In the paper, Finnish utopian communities will be described, compared, and contrasted by their ideological backgrounds and in a global context. In addition, the reasons for the dissolution of these settlements are analyzed. Even though the Finnish utopian communities are not often mentioned together with More's Utopia, or with Fourier, Owen, Cabet, or Oneida, they have an interesting history reaching back to the 1792 “New Jerusalem” plan in Sierra Leone. While the best-known Finnish utopian ventures are Sointula in Canada (1901-1905) and Colonia Finlandesa (1906-1940) in Argentina there were, however, almost twenty similar Finnish ventures around the world based on nationalism, utopian socialism, cooperative movements, “tropic fevers,” and religious ideas.

Aim Geographical, climatic and soil factors are major drivers of plant beta diversity, but their importance for dryland plant communities is poorly known. The aim of this study was to: (1) characterize patterns of beta diversity in global drylands; (2) detect common environmental drivers of beta diversity; and (3) test for thresholds in environmental conditions driving potential shifts in plant species composition. Location Global. Methods Beta diversity was quantified in 224 dryland plant communities from 22 geographical regions on all continents except Antarctica using four complementary measures: the percentage of singletons (species occurring at only one site); Whittaker's beta diversity, β(W); a directional beta diversity metric based on the correlation in species occurrences among spatially contiguous sites, β(R2); and a multivariate abundance-based metric, β(MV). We used linear modelling to quantify the relationships between these metrics of beta diversity and geographical, climatic and soil variables. Results Soil fertility and variability in temperature and rainfall, and to a lesser extent latitude, were the most important environmental predictors of beta diversity. Metrics related to species identity percentage of singletons and β(W) were most sensitive to soil fertility, whereas those metrics related to environmental gradients and abundance (β(R2) and β(MV) were more associated with climate variability. Interactions among soil variables, climatic factors and plant cover were not important determinants of beta diversity. Sites receiving less than 178 mm of annual rainfall differed sharply in species composition from more mesic sites (> 200 mm). Main conclusions Soil fertility and variability in temperature and rainfall are the most important environmental predictors of variation in plant beta diversity in global drylands. Our results suggest that those sites annually receiving c. 178 mm of rainfall will be especially sensitive to future climate changes. These findings may help to define appropriate conservation strategies for mitigating effects of climate change on dryland vegetation.

Aims Climate and human impacts are changing the nitrogen (N) inputs and losses in terrestrial ecosystems. However, it is largely unknown how these two major drivers of global change will simultaneously influence the N cycle in drylands, the largest terrestrial biome on the planet. We conducted a global observational study to evaluate how aridity and human impacts, together with biotic and abiotic factors, affect key soil variables of the N cycle. Location Two hundred and twenty-four dryland sites from all continents except Antarctica widely differing in their environmental conditions and human influence. Methods Using a standardized field survey, we measured aridity, human impacts (i.e. proxies of land uses and air pollution), key biophysical variables (i.e. soil pH and texture and total plant cover) and six important variables related to N cycling in soils: total N, organic N, ammonium, nitrate, dissolved organic:inorganic N and N mineralization rates. We used structural equation modelling to assess the direct and indirect effects of aridity, human impacts and key biophysical variables on the N cycle. Results Human impacts increased the concentration of total N, while aridity reduced it. The effects of aridity and human impacts on the N cycle were spatially disconnected, which may favour scarcity of N in the most arid areas and promote its accumulation in the least arid areas. Main conclusions We found that increasing aridity and anthropogenic pressure are spatially disconnected in drylands. This implies that while places with low aridity and high human impact accumulate N, most arid sites with the lowest human impacts lose N. Our analyses also provide evidence that both increasing aridity and human impacts may enhance the relative dominance of inorganic N in dryland soils, having a negative impact on key functions and services provided by these ecosystems.

BACKGROUND VEGF and VEGF receptor-2-mediated angiogenesis contribute to hepatocellular carcinoma pathogenesis. Ramucirumab is a recombinant IgG1 monoclonal antibody and VEGF receptor-2 antagonist. We aimed to assess the safety and efficacy of ramucirumab in advanced hepatocellular carcinoma following first-line therapy with sorafenib. METHODS In this randomised, placebo-controlled, double-blind, multicentre, phase 3 trial (REACH), patients were enrolled from 154 centres in 27 countries. Eligible patients were aged 18 years or older, had hepatocellular carcinoma with Barcelona Clinic Liver Cancer stage C disease or stage B disease that was refractory or not amenable to locoregional therapy, had Child-Pugh A liver disease, an Eastern Cooperative Oncology Group performance status of 0 or 1, had previously received sorafenib (stopped because of progression or intolerance), and had adequate haematological and biochemical parameters. Patients were randomly assigned (1:1) to receive intravenous ramucirumab (8 mg/kg) or placebo every 2 weeks, plus best supportive care, until disease progression, unacceptable toxicity, or death. Randomisation was stratified by geographic region and cause of liver disease with a stratified permuted block method. Patients, medical staff, investigators, and the funder were masked to treatment assignment. The primary endpoint was overall survival in the intention-to-treat population. This study is registered with ClinicalTrials.gov, number NCT01140347. FINDINGS Between Nov 4, 2010, and April 18, 2013, 565 patients were enrolled, of whom 283 were assigned to ramucirumab and 282 were assigned to placebo. Median overall survival for the ramucirumab group was 9·2 months (95% CI 8·0-10·6) versus 7·6 months (6·0-9·3) for the placebo group (HR 0·87 [95% CI 0·72-1·05]; p=0·14). Grade 3 or greater adverse events occurring in 5% or more of patients in either treatment group were ascites (13 [5%] of 277 patients treated with ramucirumab vs 11 [4%] of 276 patients treated with placebo), hypertension (34 [12%] vs ten [4%]), asthenia (14 [5%] vs five [2%]), malignant neoplasm progression (18 [6%] vs 11 [4%]), increased aspartate aminotransferase concentration (15 [5%] vs 23 [8%]), thrombocytopenia (13 [5%] vs one [<1%]), hyperbilirubinaemia (three [1%] vs 13 [5%]), and increased blood bilirubin (five [2%] vs 14 [5%]). The most frequently reported (≥1%) treatment-emergent serious adverse event of any grade or grade 3 or more was malignant neoplasm progression. INTERPRETATION Second-line treatment with ramucirumab did not significantly improve survival over placebo in patients with advanced hepatocellular carcinoma. No new safety signals were noted in eligible patients and the safety profile is manageable. FUNDING Eli Lilly and Co.

A menudo la diosa Crítica argentina se empecina en sumergir en el río del olvido a no pocas figuras literarias de nuestro país del siglo XIX. Se podrían ofrecer como excusas, en algunos casos, el despótico paso del tiempo (mientras más nos alejamos del presente más difícil es indagar sobre talo cual autor); en otros, la escasa información biográfica; o la condición paupérrima de ciertas ediciones; o el estado a veces inabarcable de hombresorquesta de estos individuos (son poetas, guerreros, filósofos, etc. y esto sin descuidar ninguno de sus oficios), y así seguiríamos enumerando hasta el hartazgo. Pero así como existe una tendencia a lograr una moderna historiografia argentina de la mano de estudiosos como Tulio Halperin Donghi, revisando los hechos a la luz de sus desencadenantes ideológicos, también se debería proponer un buceo histórico-literario para sacar a la luz varias perlas literarias, engarzadas en sus correspondientes ideologías. Este trabajo se propone ser la ayuda virgiliana que rescate del olvido a Juan Crisóstomo Lafinur, sobre todo su labor poética, ya que de la filosófica todos sabemos de su importante trascendencia. Para esta última sólo tenemos que recordar las palabras de Roberto F. Giusti: "La historia de la evolución mental argentina quedaría incompleta si en ella no hiciésemos un lugar a Lafinur".

La categoría de paisaje ha sido muchas veces utilizada para analizar textos latinoamericanos que, en efecto, representaban las particularidades del espacio latinoamericano a través de técnicas paisajísticas: separación entre el sujeto y el objeto, regulación de límites entre cultura y naturaleza. Sin embargo, algunos textos (como Orgia, de Tulio Carella) proponen una nueva manera de leer el espacio latinoamericano, una nueva imagen de América Latina, y, con ello, un punto de fuga respecto de la violencia que implica el imaginario como aparato de captura de las diferencias

La concordia está fundada, de acuerdo con Cicerón, en la objetividad universal que representa la superación de la multiplicidad relativista. La gloria es en este marco el premio para el buen gobernante que sacrifica su vida por la patria. El pacto, en cambio, cimenta la unidad en un acto voluntario. El gobernante es el táctico que cumple con habilidad lo pactado. La unión de ciudadanos no es el fruto de la concordia sino del poder, sólo consiste en cumplir lo pactado; por consiguiente no es necesario honrar a nadie ni otorgarle gloria.
