Background: Numerous studies have shown a negative association between birth weight (BW) and blood pressure (BP) later in life. To estimate the direct effect of BW on BP, it is conventional to condition on current weight (CW). However, such conditioning can induce collider stratification bias in the estimate of the direct effect. Objective: To bound the potential bias due to U, an unmeasured common cause of CW and BP, on the estimate of the (controlled) direct effect of BW on BP. Methods: Data from a school based study in Switzerland were used (N = 4,005; 2,010 B/1,995 G; mean age: 12.3 yr [range: 10.1-14.9]). Measured common causes of BW-BP (SES, smoking, body weight, and hypertension status of the mother) and CW-BP (breastfeeding and child's physical activity and diet) were identified with DAGs. Linear regression models were fitted to estimate the association between BW and BP. Sensitivity analyses were conducted to assess the potential effect of U on the association between BW and BP. U was assumed 1) to be a binary variable that affected BP by the same magnitude in low BWand in normal BW children and 2) to have a different prevalence in low BW children and in normal BW children for a given CW. Results: A small negative association was observed between BW and BP [beta: -0.3 mmHg/kg (95% CI: -0.9 to 0.3)]. The association was strengthened upon conditioning for CW [beta: -1.5 mmHg/kg (95% CI: -2.1 to -0.9)]. Upon further conditioning on common causes of BW-BP and CW-BP, the association did not change substantially [beta: -1.4 mmHg/kg (95% CI: -2.0 to -0.8)]. The negative association could be explained by U only if U was strongly associated with BP and if there was a large difference in the prevalence of U between low BWand normal BW children. Conclusion: The observed negative association between BW and BP upon adjustment for CW was not easily explained by an unmeasured common cause of CWand BP.

BACKGROUND: The effect of the increasing prevalence of obesity on blood pressure (BP) secular trends is unclear. We analyzed BP and body mass index secular trends between 1998 and 2006 in children and adolescents of the Seychelles, a rapidly developing island state in the African region. METHODS AND RESULTS: School-based surveys were conducted annually between 1998 and 2006 among all students in 4 school grades (kindergarten and 4th, 7th, and 10th years of compulsory school). We used the Centers for Disease Control and Prevention criteria to define obesity and elevated BP. The same methods and instruments were used in all surveys. Some 25 586 children and adolescents 4 to 18 years of age contributed 43 867 observations. Although the prevalence of obesity in boys and girls increased from 5.1% and 6.0%, respectively, in 1998 to 2000 to 8.0% and 8.7% in 2004 to 2006, the prevalence of elevated BP decreased from 8.4% and 9.8% to 6.9% and 7.8%. During the interval, mean age-adjusted body mass index increased by 0.57 kg/m(2) in boys and 0.58 kg/m(2) in girls. Mean age- and height-adjusted systolic BP decreased by -3.0 mm Hg in boys and -2.8 mm Hg in girls, whereas mean diastolic BP did not change substantially in boys (-0.2 mm Hg) and increased slightly in girls (0.4 mm Hg). CONCLUSIONS: At a population level, the marked increase in the prevalence of obesity in children and adolescents in the Seychelles was not associated with a commensurate secular rise in mean BP.

BACKGROUND: We examined the associations between substance use (cigarette smoking, alcohol drinking, and cannabis use) and psychosocial characteristics at the individual and family levels among adolescents of the Seychelles, a rapidly developing small island state in the African region. METHODS: A school survey was conducted in a representative sample of 1432 students aged 11-17 years from all secondary schools. Data came from a self-administered anonymous questionnaire conducted along a standard methodology (Global School-based Health Survey, GSHS). Risk behaviors and psychosocial characteristics were dichotomized. Association analyses were adjusted for a possible classroom effect. RESULTS: The prevalence of cigarette smoking, alcohol drinking and cannabis use was higher in boys than in girls and increased with age. Age-adjusted and multivariate analyses showed that several individual level characteristics (e.g. suicidal ideation and truancy) and family level characteristics (e.g. poor parental monitoring) were associated with substance use among students. CONCLUSIONS: Our results suggest that health promotion programs should simultaneously address multiple risk behaviors and take into account a wide range of psychosocial characteristics of the students at the individual and family levels.

To estimate the possible direct effect of birth weight on blood pressure, it is conventional to condition on the mediator, current weight. Such conditioning can induce bias. Our aim was to assess the potential biasing effect of U, an unmeasured common cause of current weight and blood pressure, on the estimate of the controlled direct effect of birth weight on blood pressure, with the help of sensitivity analyses. We used data from a school-based study conducted in Switzerland in 2005-2006 (n = 3,762; mean age = 12.3 years). A small negative association was observed between birth weight and systolic blood pressure (linear regression coefficient βbw = -0.3 mmHg/kg, 95% confidence interval: -0.9, 0.3). The association was strengthened upon adjustment for current weight (βbw|C = -1.5 mmHg/kg, 95% confidence interval: -2.1, -0.9). Sensitivity analyses revealed that the negative conditional association was explained by U only if U was relatively strongly associated with blood pressure and if there was a large difference in the prevalence of U between low-birth weight and normal-birth weight children. This weakens the hypothesis that the negative relationship between birth weight and blood pressure arises only from collider-stratification bias induced by conditioning on current weight.

Background: Little is known on the relative importance of growth at different periods between birth and adolescence on blood pressure (BP). Objective: To assess the association between birth weight, change in body weight (growth) and BP across the entire span of childhood and adolescence. Methods: School-based surveys were conducted annually between 1998 and 2006 among all children in four school grades (kindergarten, 4th, 7th, and 10th year of compulsory school) in the Seychelles, Indian Ocean. Height and weight and BP were measured. Three cohorts of children examined twice were analyzed: 1606 children surveyed at age 5.5 and 9.1, 2557 at age 9.2 and 12.5, and 2065 at age 12.5 and 15.5, respectively. Weights at birth and at one year were extracted from medical files. Weights were expressed as Z-scores and growth was defined as a change in weight Z-scores (corresponding to weight centile crossing). The association between BP (at age 5.5, 9.2, 12.5, and 15.5) and weight at different times was assessed by linear regression. Using results of regression models of BP on all successive weights, life course plots were drawn by plotting regression coefficients against age at which weight was measured. The figure shows a life course plot of systolic BP in boys aged 15.5. Results: Without adjustment for current weight (at the time of BP measurement), birth weight was not associated with current BP, irrespective of age, excepted for girls at age 15.5 for whom a modest positive association was found. When adjusted for current weight, birth weight was negatively and modestly associated with current BP. BP was strongly associated with current weight, irrespective of age. Life course plots showed that BP was strongly associated with growth during the few preceding years but not with growth during earlier years, except for growth during the first year of life which tended to be associated with systolic BP. Conclusions: Our findings suggest that BP during childhood and adolescence is mainly determined by current body weight and recent growth.

Tutkimukseni käsittelee suomen kielen sanaston kehitystä 1800-luvulla eli aikana, jolloin suomen kielestä kehittyi monialainen sivistyskieli. Esimerkkiaineistona on yhden erikoisalan, maantieteen sanasto. Suomen kirjakieli syntyi 1500-luvulla, mutta aluksi kirjoitettua kieltä tarvittiin pääasiassa uskonnollisissa yhteyksissä. 1800-luvun aikana kielen käyttöalat monipuolistuivat ja uutta sanastoa tarvittiin monien erikoisalojen tarpeisiin. Ryhdyttiin tietoisesti kääntämään tietokirjallisuutta ja kirjoittamaan eri aiheista. Tutkimukseni selvittää maantieteen sanaston kehittymistä sadassa vuodessa erityisesti maantieteen oppikirjoissa. Tutkimus kuvaa sanaston kehitystä teoreettisesti uudenlaisista lähtökohdista tarkastelemalla leksikaalista variaatiota. Variaatiota on kuvattu tarkasti sekä yksittäisten käsitteiden nimitysten kehityksenä että ilmiönä yleisesti. Tutkimus hyödyntää myös kognitiivista lähestymistapaa, etenkin sosiokognitiivisen terminologian teoriaa. Aineiston analyysin pohjalta syntyy kuva sanaston kehityksestä ja vakiintumisesta. Tutkimus kuvaa myös tapoja, joilla uusia käsitteitä nimettiin. Se pohtii eri nimeämistapojen suhdetta sekä kirjoittajien ja aikalaisten roolia sanaston vakiintumisessa. 1800-luvun maantieteen sanastossa on runsaasti variaatiota; vain harvojen käsitteiden nimitykset ovat vakiintuneita tai vakiintuvat nopeasti. Tämän variaation kuvaaminen leksikaalisena variaationa osoittautui tutkimuksessa hyväksi metodiksi. Koska kirjakieli oli vakiintumatonta, nimityksissä esiintyy paljon kontekstuaalista variaatiota esimerkiksi sanojen kirjoitusasuissa. Kirjoittajat myös pohtivat havainnollista tapaa nimetä käsitteitä, ja tästä aiheutuu onomasiologista variaatiota. Semasiologinen variaatio taas kertoo käsitejärjestelmän vakiintumattomuudesta. Aineiston sanaston lähtökohdat ovat vanhan kirjasuomessa, mutta tältä pohjalta luodaan valtava määrä uutta sanastoa tai otetaan aiemmin kirjakielessä käytettyjä nimityksiä uuteen merkitykseen. Tärkeä rooli on sekä nimitysten muodostamisella kotoisista aineksista että kääntämisellä, jossa malli saadaan toisesta kielestä mutta nimitysten ainekset ovat omaperäisiä.

Tutkimuksessa arvioidaan millaisia kyvykkyyksiä toimijoiltavaaditaan, jotta voidaan edistää verkostoja palvelevan innovaatiopolitiikan toteutumista ja toteuttaa käytäntölähtöistä innovaatiotoimintaa. Empiirinen osa tarkastelee Päijät-Hämeen toimijoiden asenneympäristöä ja toimimisen valmiuksia käytäntölähtöisen innovaatiotoiminnan tarpeisiin sopivaksi. Osaaminen kerääntyy yliopistopaikkakunnille ulkoisten suurtuotannon etujen mukaisesti. Ne alueet, joilla ei ole yliopistoa joutuvat luomaan muunlaista innovaatiokyvykkyyttä saavuttaakseen kilpailuetua. Siksi Päijät-Hämeen visiona on tulla johtavaksi käytäntölähtöisen innovaatiotoiminnan alueeksi hyvien toimintamallien ja tehokkaiden tiedonsiirtomekanismien avulla. Tämä vaatii alueen toimijoilta mm. korkeaa absorptiivista kapasiteettia ja heikkoja linkkejä alueen ulkopuolelle. Työn empiirinen osa koostuu 12:sta puolistrukturoidusta haastattelusta sekä kyselytutkimuksesta. Tiedonluonti ja -siirto alueelle nähtiin pääasiassa tutkimusmaailman tehtävänä, mutta varianssianalyysin perusteella tutkimusmaailma ei itse nähnyt olevansa siinä asemassa. Yhteisen kielen puuttuminen tutkimus- ja käytännön työelämän väliltä nähtiin puutteena.
