Introduction: Bioaerosols such as grain dust, via biologically active agents, elicit local inflammation and direct immunological reactions within the human respiratory system. Workplace-dependent exposure to grain dust (GD) may thus induce asthma, chronic bronchitis, and hypersensitivity pneumonitis. The aim of this study is to assess the clinical impact of occupational exposure to GD and to determine quantitative biological markers of bioaerosol exposure in grain workers. Methods: This longitudinal study has been conducted from summer 2012, to summer 2013, comprising 6 groups of 30 active workers with different GD exposure patterns (4 groups of grain workers, 2 control groups). After obtaining informed consent, two evaluations at high- and low-exposing seasons take place, during which an occupational history and a detailed medical history are questionnaire-assessed, lung function is evaluated by spirometry, airway inflammation is measured by exhaled nitric oxide (eNO), and specific blood IgG and IgE are titrated. The preliminary results presented hereafter are those of two of the four exposed groups, namely harvesters and mill workers, compared to the control groups, at first assessment (n=100). Results: Mean age is 38.4 [years]; 98% are male. Exposed groups differ from controls (p<0.05) in daily contact with animals (57% vs. 40%) and active smoking (39% vs. 11%). Grain workers have more respiratory (50%), nasal (57%), ocular (45%), dermatologic (36%) and systemic (20%) occupational symptoms than controls (6.4%, 19%, 16%, 6.4%, 1.6% respectively, p<0.05). Lower mean peak-expiratory-flow (PEF) values (96.1 ± 18.9 vs. 108.2 ± 17.4 [% of predicted], p<0.05) and eNO values (13.9 ± 9.6 vs. 20.5 ± 14.7 [ppm], p<0.05) are observed in the exposed groups. Conclusion: Preliminary results show a higher prevalence of clinical symptoms and a lower mean PEF value in the exposed groups. Detailed supplementary analyses are pending.

L'objectif de cette recherche est d'évaluer les prescriptions de méthylphénidate faites en 2005 aux enfants domiciliés dans le canton de Vaud et de commenter l'évolution depuis 2002. L'analyse comporte 3 volets. Un premier volet cherche à établir si les prescriptions semblent adaptées aux normes et aux recommandations émises quant à l'âge du patient et au traitement à suivre. Il porte sur les doses, la posologie, la durée de traitement et ses formes. Un deuxième volet s'intéresse aux tranches d'âge concernées par les traitements au méthylphénidate et observe s'il existe des variations selon le sexe des patients. Une répartition géographique des cas traités en comparant la répartition des prescripteurs et des patients traités a été faite à l'échelle des districts du canton. Le troisième volet s'intéresse aux médecins prescripteurs de méthylphénidate : est-ce que des différences existent selon la spécialisation médicale ? Est-ce que les médecins, qui, du fait de leur spécialisation (psychiatres ou pédiatres) sembleraient à première vue plus en mesure de décider de la nécessité d'un tel traitement pour un patient, sont plus nombreux à prescrire du méthylphénidate ? L'objectif de ce mémoire de master est de créer un système d'indicateurs de résultats pour évaluer l'impact des soins à domicile sur la qualité de vie des personnes âgées fragiles et dépendantes vivant dans le canton de Vaud. Ainsi, le premier chapitre présente le système des soins à domicile dans le canton de Vaud dans son ensemble et, en particulier, le panel d'acteurs qui y participe, soit les prestataires, les co-acteurs et les bénéficiaires. Afin de connaître l'état de la recherche sur le sujet, une revue de la littérature sur l'évaluation des soins à domicile tant dans le canton de Vaud qu'au niveau international est développée au chapitre 2. Le concept de qualité de vie est défini au chapitre 3. Les différentes mesures de ce concept y sont également détaillées ainsi que les dimensions les plus citées dans la littérature, tant objectives que subjectives. Le chapitre 4 expose la recherche « terrain » effectuée auprès d'une dizaine de client-e-s de CMS par le biais d'interviews. Une fois les différentes étapes (hypothèse de départ, échantillonnage, grille d'entretien) exposées, les rencontres avec les client-e-s de CMS les plus marquantes sont décrites. Enfin, au chapitre 5, sont présentés les onze indicateurs de résultats en lien avec les différentes dimensions de la qualité de vie : la santé, les relations sociales, le statut socio-économique, le domicile, les capacités fonctionnelles et le bien-être.

Background and aims: Because of the changing epidemiology of Inflammatory Bowel Diseases 0131)), we set out to characterize the population-based prevalence of Crohn's Disease (CD) and Ulcerative Colitis (UC) in a defined population of Switzerland. Methods: Adult IBD patients were identified by a cross-matched review of histological, hospital and gastroenterologist files throughout a geographical defined population (Canton of Vaud). Demographic factors statistically significantly associated with prevalence were evaluated using a stepwise Poisson regression analysis. Results were compared to IBD prevalence rates in other population-based studies and time trends were performed, based on a systematic literature review. Results: Age and sex-adjusted prevalence rates were 205.7 IBD (100.7 CD and 105.0 UC) cases per 105 inhabitants. Among 1016 IBD patients (519 CD and 497 UC), females outnumbered mates in CD (p < 0.001), but mates were more represented in elderly UC patients (p = 0.008). Thus, being a mate was statistically associated with UC (Relative Risk (RR) 1.25; p = 0.013), whereas being a female was associated with CD (RR 1.27; p = 0.007). Living in an urban zone was associated with both CD and UC (RR 1.49; p < 0.001, 1.63; p < 0.001, respectively). From 1960 to 2005, increases in UC and CD prevalences of 2.4% (95%CI, 2.1%-2.8%; p < 0.001) and 3.6% (95%CI, 3.1%-4.1%; p < 0.001) per annum were found in industrialised countries.

For several years now, substantial efforts have been devoted to the development and the implementation of a screening program for breast cancer in the Canton of Vaud. A four-year pilot phase is now starting, involving two regional hospitals with their catchment areas; women over 50 and under 70 years old will be invited to participate in the program. A double view mammography will be made, with a double reading made by the hospital radiologists; a third reading will be made in case of discrepancy between the two first radiologists. Patients classified as positive for screening (e.g., with a suspect radiological image) will be referred to their practitioner for further diagnosis and treatment. The medical and public health background of this program is discussed, more specifically the reasons for developing a screening program, the choice of mammography rather than other tools, and the need to implement screening as an organized program.

It is common knowledge that patients' satisfaction influences their compliance with the proposed treatment and their future use of health services, and that the therapeutic alliance represents a recognised factor in the success of psychotherapeutic management. In May and June 2011 we conducted an enquiry in all the paedopsychiatric outpatient units of the Canton of Vaud in Switzerland, from which we gathered 1261 questionnaires from consulting children and their parents. The questions covered inter alia overall satisfaction concerning the quality of the care on off er, perception of the therapeutie alliance and the helpfulness of the varions types of treatment. The instruments used are recognised and validated internationally (HAQII and CSQS). The results reve al a high degree of satisfaction and a good perception of the therapeutic alliance's quality: an average of 4.9 for children and 5.9 for parents concerning the therapeutic alliance, on a scale from 1 (totally disagree) to 6 (totally agree), and concerning patient satisfaction an average of 3.2 for children and 3.4 for parents on a scale of 1 to 4. Regarding perception of the benefit derived from different forms of treatment, individual and family interviews were overwhel~ningly approved (3.2 out of 4), while compliance with drug treatment was less recognised as helpful to children (2.3 out of 4).

The project "Quantification and qualification of ambulatory health care", financed by the Swiss National Science Foundation and covering the Cantons of Vaud and Fribourg, has two main goals: --a structural study of the elements of the ambulatory care sector. This is done through inventories of the professions concerned (physicians, public health nurses, physiotherapists, pharmacists, medical laboratories), allowing to better characterize the "offer". This inventory work includes the collect and analysis of existing statistical data as well as surveys, by questionnaires sent (from September 1980) to the different professions and by interviews. --a functional study, inspired from the US National Ambulatory Medical Care Survey and from similar studies elsewhere, in order to investigate the modes of practice of various providers, with particular regard to interprofessional collaboration (through studying referrals from the ones to the others). The first months of the project have been used for a methodological research in this regard, centered on the use of systems analysis, and for the elaboration of adequate instruments.
