Fundamento: La salud mental de las personas cuidadoras se ve perjudicada por las tareas de cuidado. Son necesarios estudios que especifiquen la influencia de diversas variables en la asociación entre cuidado informal y salud mental. El objetivo del trabajo es analizar los efectos del tiempo dedicado al cuidado informal en la salud mental por sexo según el tipo de personas dependientes. Métodos: Los datos que se utilizaron fueron los de la Encuesta Nacional de Salud 2006, cuya muestra estaba compuesta por 29.478 participantes. Variables: salud mental (GHQ-12), tiempo de cuidado, edad, clase social y apoyo funcional (Duke-UNC). Se realizó análisis de regresión logística para determinar la probabilidad de contar con mala salud mental según horas de cuidado, ajustado por edad, clase social y apoyo funcional. Resultados: Las mujeres presentan peor salud mental a partir de 97 horas semanales en el cuidado a menores (OR=1,372), de 25 horas en el cuidado a personas mayores de 74 años (OR=1,602 entre 25 y 48 horas; OR=1,467 49-96h; OR=1,874 97-168h) y cuando dedican alguna hora al cuidado de personas adultas con discapacidad (OR=1,996 0-24h; OR=2,507 25-48h; OR=3,016 49-96h; OR=1,651 97-168h). Los varones muestran deterioro de la salud psicológica cuando dedican un elevado número de horas al cuidado de personas mayores de 74 años (OR=2,810 97168h.) y personas con discapacidad (OR=3,411 97-168h.) y cuando dedican de 0 a 24h al cuidado de menores (OR=1,595). Conclusiones: El efecto del número de horas de cuidado en la salud mental de las personas cuidadoras está influido por el tipo de persona dependiente y por el sexo de la persona cuidadora.

Objetivo: Determinar si la ENS y la EPA de 2006 producen la misma información sobre labores del hogar y doble carga de trabajo en la población de 25 a 64 años, en ambos sexos. Métodos: Comparación entre las ENS y EPA sobre la forma de recoger información de la doble carga de trabajo. Fuente: Preguntas ENS: actividad económica (C.1.2:categorías 1,2,6), dedicación labores del hogar (A.11:categorías 1,2,3). EPA: actividad económica (H.1:categorías 1,5). Descripción por sexo en España y Comunidades Autónomas (CC.AA). Resultados: El 43,4% de las mujeres según la EPA tienen doble carga de trabajo, pero solo un 0,7% según la ENS. En los hombres el 31,5% (EPA) y el 0,02% (ENS). Alternativamente, cruzando a quienes afirman trabajar (C.1.2:categorías 1,2) con quienes realizan labores del hogar (A.11:categorías 1,2,3), la doble carga de ambas encuestas se aproxima (Hombres: ENS:31,7%; EPA:31,5%; Mujeres: ENS:46,3%; EPA:43,4%). Ambas encuestas ordenan de forma similar a las CC.AA según la doble carga de trabajo (ρmujeres:0,770 (p=0,001); ρhombres:0,647 (p=0,003)). Conclusión: La pregunta de actividad económica de la ENS subestima la frecuencia de la doble carga de trabajo. Esta es parecida en ambas encuestas, si se cruzan los datos de quienes afirman trabajar con quienes realizan labores del hogar de la ENS. En este caso, ambas encuestas ordenan de igual forma a las CC.AA. La exclusión del adverbio «principalmente» de la categoría sobre dedicación a las labores del hogar de la ENS 2011 normalizará la pregunta sobre actividad económica respecto a las utilizadas en encuestas de salud internacionales y de CC.AA.

Objectives: To analyse the association between self-perceived discrimination and social determinants (social class, gender, country of origin) in Spain, and further to describe contextual factors which contribute to self-perceived discrimination. Methods: Cross-sectional design using data from the Spanish National Health Survey (2006). The dependent variable was self-perceived discrimination, and independent and stratifying variables were sociodemographic characteristics (e.g. sex, social class, country of origin, educational level). Logistic regression was used. Results: The prevalence of self-perceived discrimination was 4.2% for men and 6.3% for women. The likelihood of self-perceived discrimination was higher in people who originated from low-income countries: men, odds ratio (OR) 5.59 [95% confidence interval (CI) 4.55–6.87]; women, OR 4.06 (95% CI 3.42–4.83). Women were more likely to report self-perceived discrimination by their partner at home than men [OR 8.35 (95% CI 4.70–14.84)]. The likelihood of self-perceived discrimination when seeking work was higher among people who originated from low-income countries than their Spanish counterparts: men, OR 13.65 (95% CI 9.62–19.35); women, OR 10.64 (95% CI 8.31–13.62). In comparison with Spaniards, male white-collar workers who originated from low-income countries [OR 11.93 (95% CI 8.26–17.23)] and female blue-collar workers who originated from low-income countries (OR 1.6 (95% CI 1.08–2.39)] reported higher levels of self-perceived discrimination. Conclusions: Self-perceived discrimination is distributed unevenly in Spain and interacts with social inequalities. This particularly affects women and immigrants.

Introducción: analizar la asociación entre la frecuencia de consumo de alimentos y la presencia de sobrepeso-obesidad a partir de datos de la Encuesta Nacional de Salud Española 2006 (ENS-06). Material y métodos: estudio transversal retrospectivo de fuentes secundarias de la ENS-06. Se seleccionaron 29.321 individuos que respondieron el Cuestionario de Frecuencia de Consumo de Alimentos, compuesto por una lista de 12 grupos, sobre los que se preguntaba su frecuencia de consumo en 5 opciones de respuesta y que se categorizaron en 3 para el análisis (consumo diario, semanal y ocasional). Se realizó un análisis descriptivo del consumo de grupos de alimentos categorizados por frecuencia de consumo, y calculando la diferencia (brecha) entre hombres y mujeres. La asociación entre frecuencia de consumo de alimentos y sobrepeso-obesidad se realizó mediante regresión logística. Resultados: el 40,7% de la población consume a diario verduras-hortalizas y el 65,2% consume frutas a diario. El 69,8%, y el 46,3% de las mujeres cumplen las recomendaciones de consumo de frutas (brecha -7,2), y de verduras-hortalizas (brecha -14,0) respectivamente. Consumir fruta ocasionalmente, disminuye la probabilidad de padecer obesidad (OR=0,72 IC95%: 0,63-0,82), frente a consumirla a diario. Consumir embutidos a diario, disminuye la probabilidad de padecer obesidad (OR=0,75 IC95%: 0,68-0,83), frente a un consumo ocasional. Conclusiones: Los resultados muestran contradicciones científicas por la presencia de sesgos en el instrumento utilizado en la recogida de la información, sugiriendo la necesidad de que la ENS incluya un método de recogida de información dietética-nutricional validado y adoptado, que permita mediciones de mayor calidad y exactitud.

Introduction: Self-image is important in the behaviour and lifestyle of children and adolescents. Analysing the self-image they have and the factors that might influence their distortion, can be used to prevent problems of obesity and anorexia. The main objective of present publication was to analyse the risk factors that may contribute to self-image distortion. Material and Methods: A descriptive survey study was conducted among 659 children and adolescents in two social classes (low and medium-high), measuring height and weight, calculating BMI percentile for age and gender. Body image and self-perception were registered. Results: The percentage of overweight-obesity is higher in scholars (41.8% boys, 28.7% girls) than in adolescents (30.1% and 22.2% respectively), with no difference between socioeconomic classes. The multinomial logistic regression analysis gives a risk of believing thinner higher (p=0.000) among boys OR=2.9(95%CI:1.43-3.37), school (p=0.000) OR=2.42(95%CI:1.56-3.76) and much lower (p=0.000) between normally nourished OR=0.08(95%CI:0.05-0.13), with no differences according to socioeconomic status. The risk of believing fatter is lower (p=0.000) between boys OR=0.28(95%CI:0.14-0.57), school(p=0.072) OR=0.54(95%CI:0.27-1.6), and much higher among underweight (p=0.000) OR=9x108(95% CI:4x108-19x108). Conclusions: Are risk factors of believing thinner: males, being in a group of schoolchildren and overweight-obesity. Conversely, are risk factors of believing fatter: females, teen and above all, be thin.

Enquadramento: Embora o ciclo menstrual seja um fenômeno normal na vida de mulheres em idade fértil, a ele podem se associar síndromes de intensidade variável. Objectivo: Determinar o impacto da síndrome pré-menstrual e da síndrome disfórica pré-menstrual na qualidade de vida de mulheres em idade fértil, comparadas a mulheres sem essas síndromes. Material e Métodos: Procedeu-se a estudo quantitativo, descritivo, correlacional, transversal, com 60 participantes do sexo feminino (16 com síndrome pré-menstrual, 28 com sintomatologia compatível com SDPM e 16 não apresentavam sintomas compatíveis com diagnóstico), A colheita de dados foi no Ambulatório de Ginecologia do Hospital Militar de nível terciário no período de janeiro a fevereiro de 2015, apresentando idade entre 18 e 56 anos, menarca há mais de um ano; ciclos menstruais em ausência de uso de terapia de reposição hormonal, com queixas associadas ao período menstrual. Três instrumentos de recolha de dados foram empregados: um questionário de identificação da paciente e caracterização de vida reprodutiva; o Questionário de Triagem de Sintomas Pré-menstruais e o Short Form Health-Survey com 36 itens. Para organização e análise dos dados, foi empregado o programa Statistical Package for Social Sciences, na versão 21.0. As variáveis foram dicotomizadas, para análise de contingência com o teste exato de Fisher. Para comparação de médias, foram utilizados o teste de diferença de médias de amostras independentes e o teste Kruskal Wallis, todos em nível de significância de 0,05. Resultados: A prevalência de síndrome menstrual igualou-se a 76,3%, sendo 46,6% compatível com síndrome disfórica e 29,7% com síndrome pré-menstrual. Esses grupos diferiram significantemente nos sintomas de ansiedade/tensão, fadiga/perda de energia, dificuldade de concentração, fome excessiva ou anorexia e sentimento de opressão. A síndrome disfórica pré-menstrual associou-se a maior comprometimento de relações sociais, familiares e domésticas, bem como à perda de saúde geral e de saúde mental, nos domínios de vitalidade, funções sociais, limitações devidas a problemas emocionais e saúde mental. Essas alterações não interferiram na percepção da libido e de orgasmo, mas alguns domínios da qualidade de vida estiveram associados a características reprodutivas. Conclusão: As síndromes pré-menstruais comprometem a qualidade de vida das utentes e requerem assistência voltada à melhoria do conhecimento das mulheres sobre seu problema de saúde. En este estudio se examina la relación entre el comportamiento reproductivo y algunas características de la nupcialidad en el Brasil, sobre la base de datos de las Encuestas de Demografía y Salud (Demographic and Health Survey (DHS)) de 1986 y 1996 y de la Encuesta Nacional de Demografía y Salud de la Infancia y la Mujer (PNDS) de 2006. Los resultados indican que el nivel de fecundidad de las mujeres que viven en uniones consensuales es un poco más elevado que el de las que optan por el matrimonio. Sin embargo, también se constata un movimiento de convergencia entre los niveles de fecundidad de ambos grupos. Para establecer el aporte de cada tipo de unión al cálculo de la fecundidad total se aplica la descomposición de la tasa de fecundidad. Además de aumentar la proporción de las uniones consensuales a lo largo del tiempo, crece también la participación de este tipo de unión en la fecundidad total. Se argumenta que el significado de tener hijos ha cambiado y que, en el caso brasileño, las uniones consensuales pueden comprenderse mejor a la luz de la teoría de la institucionalización.

The Center for Epidemiologic Studies Depression Scale (CES-D) is frequently used in epidemiological surveys to screen for depression, especially among older adults. This article addresses the problem of non-completion of a short form of the CES-D (CESD-10) in a mailed survey of 73- to 78-year-old women enrolled in the Australian Longitudinal Study on Women's Health. Completers of the CESD-10 had more education, found it easier to manage on available income and reported better physical and mental health. The Medical Outcomes Study Short Form Health Survey (SF-36) scores for non-completers were intermediate between those for women classified as depressed and not depressed using the CESD-10. Indicators of depression had an inverted U-shaped relationship with the number of missing CESD- 10 items and were most frequent for women with two to seven items missing. Future research should pay particular attention to the level of missing data in depression scales and report its potential impact on estimates of depression.

Appropriate measures of physical activity are essential for determining the population prevalence of physical activity, for tracking trends over time, and for guiding intervention efforts. Physical activity measurement is characterised by the synthesis of information on the type, frequency, intensity, and duration of activity over a specified period. To date, emphasis in physical activity assessment has been on the measurement of leisure time physical activities. However, some domestic and transport related activities entail energy expenditures equivalent to moderate intensity of 3.0–6.0 METS1 considered to be of sufficient intensity to achieve a health benefit are yet to be included in routine population level physical activity surveillance. This leads to population estimates based only on measures of leisure time physical activities.

Accurate monitoring of prevalence and trends in population levels of physical activity (PA) is a fundamental public health need. Test-retest reliability (repeatability) was assessed in population samples for four self-report PA measures: the Active Australia survey (AA, N=356), the short International Physical Activity Questionnaire (IPAQ, N=104), the physical activity items in the Behavioral Risk Factor Surveillance System (BRFSS, N=127) and in the Australian National Health Survey (NHS, N=122). Percent agreement and Kappa statistics were used to assess reliability of classification of activity status as 'active', 'insufficiently active' or 'sedentary'. Intraclass correlations (ICCs) were used to assess agreement on minutes of activity reported for each item of each survey and for total minutes. Percent agreement scores for activity status were very good on all four instruments, ranging from 60% for the NHS to 79% for the IPAQ. Corresponding Kappa statistics ranged from 0.40 (NHS) to 0.52 (AA). For individual items, ICCs were highest for walking (0.45 to 0.78) and vigorous activity (0.22 to 0.64) and lowest for the moderate questions (0.16 to 0.44). All four measures provide acceptable levels of test-retest reliability for assessing both activity status and sedentariness, and moderate reliability for assessing total minutes of activity.

Background: Major depression is the largest single cause of nonfatal disease burden in Australia. Effective drug and psychological treatments exist, yet are underused. Objective: To quantify the burden of disease currently averted in people seeking care for major depression and the amount of disease burden that could be averted in these people under optimal episodic and maintenance treatment strategies. Design: Modeling impact of current and optimal treatment strategies based on secondary analysis of mental health survey data, studies of the natural history of major depression, and meta-analyses of effectiveness data. Monte Carlo simulation of uncertainty in the model. Setting: The cohort of Australian adults experiencing an episode of major depression in 2000 are modeled through "what if" scenarios of no treatment, current treatment, and optimal treatment strategies with cognitive behavioral therapy or antidepressant drug treatment. Main Outcome Measure: Disability-Adjusted Life Year. Results: Current episodic treatment averts 9% (95% uncertainty interval, 6%-12%) of the disease burden of major depression in Australian adults. Optimal episodic treatment with cognitive behavioral therapy could avert 28% (95% uncertainty interval, 19%-39%) of this disease burden, and with drugs 24% (95% uncertainty interval, 19%-30%) could be averted. During the 5 years after an episode of major depression, current episodic treatment patterns would avert 13% (95% uncertainty interval, 10%-17%) of Disability-Adjusted Life Years, whereas maintenance drug treatment could avert 50% (95% uncertainty interval, 40%-60%) and maintenance cognitive behavioral therapy could avert 52% (95% uncertainty interval, 42%-64%), even if adherence of around 60% is taken into account. Conclusions: Longer-term maintenance drug or psychological treatment strategies are required to make significant inroads into the large disease burden associated with major depression in the Australian population.
