Arthritis of the knee is the most common type of joint inflammatory disorder and it is associated with pain and inflammation of the joint capsule. Few studies address the effects of the 810-nm laser in such conditions. Here we investigated the effects of low-level laser therapy (LLLT; infrared, 810-nm) in experimentally induced rat knee inflammation. Thirty male Wistar rats (230-250 g) were anesthetized and injected with carrageenan by an intra-articular route. After 6 and 12 h, all animals were killed by CO(2) inhalation and the articular cavity was washed for cellular and biochemical analysis. Articular tissue was carefully removed for real-time PCR analysis in order to evaluate COX-1 and COX-2 expression. LLLT was able to significantly inhibit the total number of leukocytes, as well as the myeloperoxidase activity with 1, 3, and 6 J (Joules) of energy. This result was corroborated by cell counting showing the reduction of polymorphonuclear cells at the inflammatory site. Vascular extravasation was significantly inhibited at the higher dose of energy of 10 J. Both COX-1 and 2 gene expression were significantly enhanced by laser irradiation while PGE(2) production was inhibited. Low-level laser therapy operating at 810 nm markedly reduced inflammatory signs of inflammation but increased COX-1 and 2 gene expression. Further studies are necessary to investigate the possible production of antiinflammatory mediators by COX enzymes induced by laser irradiation in knee inflammation.

Chemical investigation of the n-hexane and EtOAc fractions of the ethanolic extract from Styrax pohlii (Styracaceae) aerial parts resulted in the isolation of the benzofuran nor-neolignan derivatives egonol (1), homoegonol (2), homoegonol gentiobioside (3), homoegonol glucoside (4) and egonol gentiobioside (5). This is the first report of compounds 1-5 in S. pohlii. Compounds 1-5, the acetyl derivatives 1a and 2a, the ethanolic extract (EE), the n-hexane fraction (HF) and EtOAc fraction (EF) were tested for their inhibitory activities against COX-1 and COX-2. The results showed that EE, HF, EF and compounds 1-5 and 1 a-2 a shown weak to moderate inhibition of COX-1 and COX-2. Among the assayed nor-neolignans, 4 gave a COX-1 inhibition of 35.7% at 30 mu M. Compound 5 displayed a COX-2 inhibition of 19.7% at 30 mu M.

Celecoxib (Cx) shows high efficacy in the treatment of osteoarthritis and rheumatoid arthritis as a result of its high specificity for COX-2, without gastrolesivity or interference with platelet function at therapeutic concentrations. Besides of anti-inflammatory effects, Cx also has a potential role for oral cancer chemoprevention. For these conditions, oral administration in long-term treatment is a concern due to its systemic side effects. However, local application at the site of injury (e.g., buccal inflammation conditions or chemoprevention of oral cancer) is a promising way to reduce its toxicity. In this study, the in vitro characterization of mucoadhesive chitosan (CHT) gels associated to AzoneA (R) was assessed to explore the potential buccal mucosal administration of Cx in this tissue. Rheological properties of gels were analyzed by a rheometer with cone-plate geometry. In vitro Cx release and permeability studies used artificial membranes and pig cheek mucosa, respectively. Mucoadhesion were measured with a universal test machine. CHT gels (3.0%) containing 2.0% or 3.0% Az showed more appropriate characteristics compared to the others: pH values, rheology, higher amount of Cx retained in the mucosa, and minimal permeation through mucosa, besides the highest mucoadhesion values, ideal for buccal application. Moreover, the flux (J) and amounts of drug released decreased with increased CHT and Az concentrations. CHT gels (3.0%) associated with 2.0% or 3.0% Az may be considered potential delivery systems for buccal administration of Cx.

The Th1/Th2 balance represents an important factor in the pathogenesis of renal ischemia-reperfusion injury (IRI). In addition, IRI causes a systemic inflammation that can affect other tissues, such as the lungs. To investigate the ability of renal IRI to modulate pulmonary function in a specific model of allergic inflammation, C57Bl/6 mice were immunized with ovalbumin/albumen on days 0 and 7 and challenged with an ovalbumin (OA) aerosol on days 14 and 21. After 24 h of the second antigen challenge, the animals were subjected to 45 minutes of ischemia. After 24 h of reperfusion, the bronchoalveolar lavage (BAL) fluid, blood and lung tissue were collected for analysis. Serum creatinine levels increased in both allergic and non-immunized animals subjected to IRI. However, BAL analysis showed a reduction in the total cells (46%) and neutrophils (58%) compared with control allergic animals not submitted to IRI. In addition, OA challenge induced the phosphorylation of ERK and Akt and the expression of inducible nitric oxide synthase (iNOS) and cyclooxygenase-2 (COX-2) in lung homogenates. After renal IRI, the phosphorylation of ERK and expression of COX-2 and iNOS were markedly reduced; however, there was no difference in the phosphorylation of Akt between sham and ischemic OA-challenged animals. Mucus production was also reduced in allergic mice after renal IRI. IL-4, IL-5 and IL-13 were markedly down-regulated in immunized/challenged mice subjected to IRI. These results suggest that renal IRI can modulate lung allergic inflammation, probably by altering the Th1/Th2 balance and, at least in part, by changing cellular signal transduction factors. A new diruthenium(II,III) complex, of formula [Ru2Cl(ket)(4)], Ruket, containing the non-steroidal anti-inflammatory drug ketoprofen was synthesized and mainly characterized by electrospray ionization mass spectrometry (ESI-MS), UV-Vis-IR electronic spectroscopy and FTIR and Raman vibrational spectroscopies. The four drug-carboxylato bridging ligands stabilize a Ru-2(II,III) mixed valent core in a paddlewheel type structure as confirmed by ESI mass spectra, electronic and vibrational spectroscopies and magnetic measurements. Ruket and the analogous compounds containing ibuprofen, Ruibp, and naproxen, Runpx, were tested for the biological effects in the human colon carcinoma cells HT-29 and Caco-2 expressing high and low levels of COX-2 respectively. All compounds only weakly affected the proliferation of the colorectal cancer cells HT-29 and Caco-2, and similarly only partially inhibited the production/activity of MMP-2 and MMP-9 by HT-29 cells, suggesting that COX-2 inhibition by these drugs can only partially be involved in the pharmacological effects of these derivatives. Background and Objective Muscle regeneration is a complex phenomenon, involving coordinated activation of several cellular responses. During this process, oxidative stress and consequent tissue damage occur with a severity that may depend on the intensity and duration of the inflammatory response. Among the therapeutic approaches to attenuate inflammation and increase tissue repair, low-level laser therapy (LLLT) may be a safe and effective clinical procedure. The aim of this study was to evaluate the effects of LLLT on oxidative/nitrative stress and inflammatory mediators produced during a cryolesion of the tibialis anterior (TA) muscle in rats. Material and Methods Sixty Wistar rats were randomly divided into three groups (n?=?20): control (BC), injured TA muscle without LLLT (IC), injured TA muscle submitted to LLLT (IRI). The injured region was irradiated daily for 4 consecutive days, starting immediately after the lesion using a AlGaAs laser (continuous wave, 808?nm, tip area of 0.00785?cm2, power 30?mW, application time 47?seconds, fluence 180?J/cm2; 3.8?mW/cm2; and total energy 1.4?J). The animals were sacrificed on the fourth day after injury. Results LLLT reduced oxidative and nitrative stress in injured muscle, decreased lipid peroxidation, nitrotyrosine formation and NO production, probably due to reduction in iNOS protein expression. Moreover, LLLT increased SOD gene expression, and decreased the inflammatory response as measured by gene expression of NF-k beta and COX-2 and by TNF-a and IL-1 beta concentration. Conclusion These results suggest that LLLT could be an effective therapeutic approach to modulate oxidative and nitrative stress and to reduce inflammation in injured muscle. Lasers Surg. Abstract Background Phagocytosis of apoptotic cells by macrophages induces a suppressor phenotype. Previous data from our group suggested that this occurs via Platelet-activating factor receptor (PAF-R)-mediated pathways. In the present study, we investigated the impact of apoptotic cell inoculation or induction by a chemotherapeutic agent (dacarbazine, DTIC) on tumour growth, microenvironmental parameters and survival, and the effect of treatment with a PAF-R antagonist (WEB2170). These studies were performed in murine tumours: Ehrlich Ascitis Tumour (EAT) and B16F10 melanoma. Methods Tumour growth was assessed by direct counting of EAT cells in the ascitis or by measuring the volume of the solid tumour. Parameters of the tumour microenvironment, such as the frequency of cells expressing cyclo-oxygenase-2 (COX-2), caspase-3 and galectin-3, and microvascular density, were determined by immunohistochemistry. Levels of vascular endothelium growth factor (VEGF) and prostaglandin E2 (PGE2) were determined by ELISA, and levels of nitric oxide (NO) by Griess reaction. PAF-R expression was analysed by immunohistochemistry and flow cytometry. Results Inoculation of apoptotic cells before EAT implantation stimulated tumour growth. This effect was reversed by in vivo pre-treatment with WEB2170. This treatment also reduced tumour growth and modified the microenvironment by reducing PGE2, VEGF and NO production. In B16F10 melanoma, WEB2170 alone or in association with DTIC significantly reduced tumour volume. Survival of the tumour-bearing mice was not affected by WEB2170 treatment but was significantly improved by the combination of DTIC with WEB2170. Tumour microenvironment elements were among the targets of the combination therapy since the relative frequency of COX-2 and galectin-3 positive cells and the microvascular density within the tumour mass were significantly reduced by treatment with WEB2170 or DTIC alone or in combination. Antibodies to PAF-R stained the cells from inside the tumour, but not the tumour cells grown in vitro. At the tissue level, a few cells (probably macrophages) stained positively with antibodies to PAF-R. Conclusions We suggest that PAF-R-dependent pathways are activated during experimental tumour growth, modifying the microenvironment and the phenotype of the tumour macrophages in such a way as to favour tumour growth. Combination therapy with a PAF-R antagonist and a chemotherapeutic drug may represent a new and promising strategy for the treatment of some tumours.

Abstract Background Caspase-1 is a cysteine protease responsible for the processing and secretion of IL-1β and IL-18, which are closely related to the induction of inflammation. However, limited evidence addresses the participation of caspase-1 in inflammatory pain. Here, we investigated the role of caspase-1 in inflammatory hypernociception (a decrease in the nociceptive threshold) using caspase-1 deficient mice (casp1-/-). Results Mechanical inflammatory hypernociception was evaluated using an electronic version of the von Frey test. The production of cytokines, PGE2 and neutrophil migration were evaluated by ELISA, radioimmunoassay and myeloperoxidase activity, respectively. The interleukin (IL)-1β and cyclooxygenase (COX)-2 protein expression were evaluated by western blotting. The mechanical hypernociception induced by intraplantar injection of carrageenin, tumour necrosis factor (TNF)α and CXCL1/KC was reduced in casp1-/- mice compared with WT mice. However, the hypernociception induced by IL-1β and PGE2 did not differ in WT and casp1-/- mice. Carrageenin-induced TNF-α and CXCL1/KC production and neutrophil recruitment in the paws of WT mice were not different from casp1-/- mice, while the maturation of IL-1β was reduced in casp1-/- mice. Furthermore, carrageenin induced an increase in the expression of COX-2 and PGE2 production in the paw of WT mice, but was reduced in casp1-/- mice. Conclusion These results suggest that caspase-1 plays a critical role in the cascade of events involved in the genesis of inflammatory hypernociception by promoting IL-1β maturation. Because caspase-1 is involved in the induction of COX-2 expression and PGE2 production, our data support the assertion that caspase-1 is a key target to control inflammatory pain.

Ocular enucleation induces profound morphological alterations in central visual areas. However, little is known about the response of glial cells and possible inflammatory processes in visual brain areas resulting from eye enucleation. In this study, immunoblotting and immunostaining assays revealed increased expression of astrocyte and microglia markers in the rat superior colliculus (SC) between 1 and 15 days after contralateral enucleation. A transient increase of neuronal COX-2 protein expression was also found in the SC. To evaluate the role of an anti-inflammatory drug in attenuating both COX-2 and glial cell activation, the synthetic glucocorticoid dexamethasone (DEX) was administered (1mg/kg i.p., for 3 days) to enucleated rats. Immunoblotting data revealed that DEX treatment significantly inhibited COX-2 protein expression. Postlesion immunostaining for astrocyte and microglia markers was also significantly reduced by DEX treatment. These findings suggest that the removal of retinal ganglion cell input generates inflammatory responses in central retinorecipient structures

Die AMPK ist ein ubiquitär exprimiertes, heterotrimeres Enzym, das bei Energiemangel das Überleben der Zelle sichert. Um diese Funktion ausüben zu können fungiert die AMPK als sogenannter „Energie-Sensor“, der durch steigende AMP Mengen aktiviert wird. In diesem Zustand werden ATP verbrauchende Reaktionen inhibiert und gleichzeitig ATP generierende Vorgänge induziert. Im vaskulären System konnte gezeigt werden, dass die endotheliale NOSynthase durch die AMPK aktiviert, die Angiogenese stimuliert, die Endothelzellapoptose und das Wachstum von Gefäßmuskelzellen inhibiert wird. All diese Prozesse sind fundamental in der Entwicklung von kardiovaskulären Krankheiten, was auf eine protektive Funktion der AMPK im vaskulären System hindeutet. In der vorliegenden Arbeit sollten die Effekte der in vivo Modulation der AMPK Aktivität auf Endothelfunktion, oxidativen Stress und Inflammation untersucht werden. Dazu wurden zwei unterschiedliche Mausmodelle genutzt: Einerseits wurde die AMPK Aktivität durch den pharmakologischen AMPK-Aktivator AICAR stimuliert und andererseits die vaskulär vorherrschende AMPK-Isoform durch knock out ausgeschaltet. Zur Induktion von oxidativem Stress wurde ein bereits charakterisiertes Angiotensin II-Modell angewandt. Zur Untersuchung gehörten neben den Superoxid-Messungen auch die Bestimmung der Stickstoffmonoxid-Mengen in Serum und Aortengewebe, die Relaxationsmessungen in isometrischen Tonusstudien sowie HPLC-basierte Assays. Es konnte gezeigt werden, dass durch die Aktivierung der AMPK mittels AICAR die Angiotensin II induzierte Endotheldysfunktion, der oxidative Stress und auch die vaskuläre Inflammation verbessert werden konnte. Weiterhin zeigte sich dass der knock out der vaskulären Isoform (α1) im Angiotensin II Modell eine signifikant verstärkte Endotheldysfunktion, oxidativen Stress und Inflammation nach sich zog. Anhand der erhobenen Daten konnte die NADPH-Oxidase als Hauptquelle des Angiotensin II induzierten oxidativen Stresses identifiziert werden, wobei sich diese Quelle als AMPK sensitiv erwies. Durch die Aktivierung konnte die Aktivität der NADPH-Oxidase verringert und durch die α1AMPK Defizienz signifikant erhöht werden. Auch die mitochondriale Superoxidproduktion konnte durch die Modulation der AMPK Aktivität beeinflusst werden. Die vaskuläre Inflammation, die anhand der Surrogaten VCAM-1, COX-2 und iNOS untersucht wurde, konnte durch Aktivierung der AMPK verringert werden, der knock out der α1AMPK führte so einer sehr starken Expressionssteigerung der induzierbaren NO-Synthase, was in einem starken Anstieg der NO-Produktion und somit der Peroxynitritbildung resultierte.Die dargestellten Daten deuten stark auf eine protektive Funktion der AMPK im vaskulären System hin und sollte als therapeutisches Ziel, nicht nur in Bezug auf diabetische Patienten, in Betracht gezogen werden.

Mechanismen der zentralen und der peripheren Toleranz schützen den Körper vor Immunreaktionen gegen körpereigenes Gewebe oder gegen harmlose Umweltantigene. An der Aufrechterhaltung der peripheren Toleranz sind tolerogene Dendritische Zellen (DC) beteiligt. Tolerogene DC können in vitro u.a. mit Hilfe von immunsuppressiven und antiinflammatorischen Substanzen, aber auch durch virale Transduktionen, die zur Denovo- oder Überexpression toleranzassoziierter Moleküle führen, generiert werden. rnDa die Wirkung einiger immunmodulatorischer Substanzen über den intrazellulären sekundären Botenstoff cAMP vermittelt wird, sollte getestet werden, welchen Einfluss eine direkte Erhöhung des intrazellulären cAMP-Niveaus mittels Dibutyryl-cyclo-Adenosin-3´,5´-Mono-Phoshat (db-cAMP) auf die phänotypischen und funktionellen Eigenschaften von BM-DC („bone marrow derived dendritic cells“) hat.rnIm Vergleich zu unbehandelten BM-DC wiesen db-cAMP-DC ein vermindertes T-Zell-Stimulierungs-potenzial auf. Dieses verminderte T-Zell-Stimulierungspotenzial wird teilweise über die Proteinkinase A, nicht aber über Cyclooxygenase-2 (Cox-2) vermittelt. rnAnhand der FACS-Analyse mit DC- und MDSC- („myeloid derived suppressor cells“) spezifischen Markern konnte gezeigt werden, dass es sich bei den db-cAMP-DC um CD11c-positive DC mit einer vergleichsweise niedrigen Expression von MHCII und kostimulatorischen Oberflächenmolekülen handelt. Des Weiteren zeigte sich, dass sie verglichen mit BM-DC eine vermehrte mRNA-Expression der koinhibitorischen Moleküle B7-H1 und LIGHT und der toleranzassoziierten Moleküle FcγRIIB, HO-1 und Cox-2 aufweisen. Mittels ELISA konnte eine gesteigerte Expression der HO-1- und eine moderat gesteigerte PGE2-Synthese beobachtet werden. PGE2 wird mit Hilfe der Cox-2 aus Arachidonsäure gebildet.rnIm Gegensatz zu BM-DC wiesen db-cAMP-DC in beiden Reifungsstadien ein verändertes Zytokinprofil auf: Auf mRNA-Ebene zeigte sich, dass db-cAMP-DC verglichen mit BM-DC vermehrt IL-1RA und IL-10 exprimieren. Dieser Unterschied konnte für IL-10 auch mittels ELISA bestätigt werden. In den Kulturüberständen der stimulierten db-cAMP-DC konnte, im Gegensatz zu denen stimulierter BM-DC, kaum bioaktives IL-12 nachgewiesen werden. rnDb-cAMP-DC induzierten des Weiteren in kokultivierten allogenen T-Zellen ein differenzielles Zytokinprofil: Sie förderten die INFγ- und IL-17-Sezernierung durch T-Zellen, während die IL-5-Sezernierung geringer war, wenn T-Zellen mit stimulierten db-cAMP-DC kokultiviert wurden. Db-cAMP-DC hatten hingegen keinen Einfluss auf die IL-10-Produktion. Außerdem führte eine Kokultur der db-cAMP-DC mit allogenen T-Zellen nicht zu einer gesteigerten Induktion von FoxP3+ Treg. rnIn einem zweiten Ansatz sollte getestet werden ob es möglich ist die murine DC-Linie SP37A3 lentiviral mit dem toleranzassoziierten Oberflächenprotein B7-H3 zu transduzieren. Dies ist von Interesse, da die SP37A3-Zellen einige Vorteile gegenüber BM-DC aufweisen, wie z.B. ihren homogeneren Phänotyp und die Möglichkeit sie in einer Expansionskultur zu halten.rnEs konnte gezeigt werden, dass SP37A3-Zellen als Modell für myeloide DC für die Transduktion mit lentiviralen Partikeln geeignet sind. Hierbei zeigte es sich aber, dass darauf geachtet werden muss, mit konzentriertem Virus zu arbeiten und dass die Reportergen-Expression der Zielzellen über mehr als 3 Tage (mindestens 7 Tage) untersucht werden muss. Nur so kann eine eventuell auftretende Pseudotransduktion erkannt und verhindert werden. Ab einer MOI („multiplicity of infection“) von 50 konnte in SP37A3-Zellen eine Transgen-Expression nachgewiesen werden.rn

Die antioxidative Aktivität des Enzyms Glutathionperoxidase-1 (GPx-1) schützt vor Atherosklerose und ihren Folgeerkrankungen. In einer Vorstudie konnten wir zeigen, dass der Mangel an GPx-1 die Atheroskleroseentwicklung in Apolipoprotein E defizienten (ApoE-/-) Mäusen beschleunigt und modifiziert. Allerdings sind die Verteilung der GPx-1 in atherosklerotischen Läsionen und die Mechanismen für den erhöhten Makrophagengehalt in der Läsion noch nicht geklärt. Deshalb haben wir (1) die in-situ Expression der GPx-Isoformen in atherosklerotischen Läsionen von GPx-1-/-ApoE-/- und ApoE-/- Mäusen und (2) den Einfluss der GPx-1 Defizienz auf die Schaumzellbildung und Proliferation der Peritonealmakrophagen in ApoE-/- Mäusen untersucht. Die GPx-1-/-ApoE-/- und ApoE-/- Weibchen wurden für 6 und 12 Wochen auf einer atherogenen „Western-type“ Diät gehalten. Die in situ-Hybridisierung zeigte, dass die verschiedenen Isoformen der GPx (GPx-1, GPx-3, GPx-4) vorwiegend in Makrophagen, nicht jedoch in glatten Muskelzellen der atherosklerotischen Läsionen von ApoE-/- Mäusen exprimiert wurden. Für die in vitro Untersuchungen wurden 5 Monate alte, GPx-1 defiziente und Wildtyp-Mäuse, gehalten auf Normaldiät, verwendet. Die Öl-Rot-O Färbung zeigte, dass die GPx-1 Defizienz die OxLDL (oxidiertes LDL) - und E-LDL (enzymatisch modifiziertes LDL) - induzierte Schaumzellbildung förderte. Darüber hinaus war die OxLDL-induzierte Cholesterinakkumulation (zellulärer Cholesterinester/ Cholesterin-Gehalt) in GPx-1 defizienten Makrophagen verstärkt, sodass ein Mangel an GPx-1 die Aufnahme von OxLDL durch Monozyten und damit die Umwandlung in Schaumzellen beschleunigt. Hinsichtlich der Proliferation zeigte sich, dass MCSF (Macrophage Colony-Stimulating Facotr) ein stärkerer Stimulus als OxLDL ist. Ein Mangel an GPx-1 fördert die Proliferation zusätzlich. Daran ist die ERK1/2 (extracellular-signal regulated kinase 1/2) - Kaskade beteiligt, denn es wurde eine schnelle Phosphorylierung der ERK1/2-Kaskade durch MCSF und/oder OxLDL nachgewiesen. Entsprechend reduzieren ERK1/2-Inhibitoren die proliferative Aktivität der Makrophagen. Die Hemmung der p38-MAPK (p38 mitogen-activated protein kinase) führt zur vermehrten Proliferation und bei gleichzeitig verringerter Caspase-3/7 Aktivität der Makrophagen unabhängig von der Expression der GPx-1. Ein Mangel an GPx-1 hat auch keinen Einfluss auf die MCSF-vermittelte Aktivierung der p38-MAPK und JNK (c-Jun N-terminal kinase). Zusammenfassend läßt sich feststellen, dass die GPx-1-Defizienz einen signifikanten Einfluss auf die Schaumzellbildung und Proliferation von Makrophagen hat, was zur Beschleunigung der Atherosklerose und zu vermehrter Zellularität der entstehenden atherosklerotischen Läsionen führt. Die Proliferation wird über den ERK1/2 Signal-transduktionsweg positiv und über den p38-MAPK Weg negativ reguliert, wobei die ERK1/2-Kaskade empfindlich gegenüber oxidativem Stress bei GPx-1-Defizienz ist.

Lo studio è stato condotto su pazienti affetti da carcinoma nasale trattati con radioterapia presso il Centro Oncologico Veterinario (Sasso Marconi, BO). Lo studio, prospettico, randomizzato e in doppio cieco, ha valutato l’efficacia del trattamento radioterapico in combinazione o meno con firocoxib, un inibitore selettivo dell’enzima ciclossigenasi 2 (COX-2). Sono stati inclusi pazienti con diagnosi istologica di carcinoma nasale sottoposti a stadiazione completa. I pazienti sono stati successivamente suddivisi in due gruppi in base alla tipologia di trattamento: radioterapia associata a firocoxib (Gruppo 1) o solo radioterapia (Gruppo 2). Dopo il trattamento, i pazienti sono stati monitorati a intervalli di 3 mesi sia clinicamente che mediante esami collaterali, al fine di valutare condizioni generali del paziente, un’eventuale tossicità dovuta alla somministrazione di firocoxib e la risposta oggettiva al trattamento. Per valutare la qualità di vita dei pazienti durante la terapia, è stato richiesto ai proprietari la compilazione mensile di un questionario. La mediana del tempo libero da progressione (PFI) è stata di 228 giorni (range 73-525) nel gruppo dei pazienti trattati con radioterapia e firocoxib e di 234 giorni (range 50-475) nei pazienti trattati solo con radioterapia. La sopravvivenza mediana (OS) nel Gruppo 1 è stata di 335 giorni (range 74-620) e di 244 giorni (range 85-505) nel Gruppo 2. Non si sono riscontrate differenze significative di PFI e OS tra i due gruppi. La presenza di metastasi ai linfonodi regionali condizionava negativamente PFI e sopravvivenza (P = 0.004). I pazienti trattati con firocoxib hanno mostrato un significativo beneficio in termini di qualità di vita rispetto ai pazienti trattati con sola radioterapia (P=0.008). La radioterapia può essere considerata un’efficace opzione terapeutica per i cani affetti da neoplasie nasali. Firocoxib non sembra migliorare significativamente i tempi di sopravvivenza, ma risulta utile al fine di garantire una migliore qualità di vita.

The objectives of this study were to establish dose-response and blood concentration-response relationships for robenacoxib, a novel nonsteroidal anti-inflammatory drug with selectivity for inhibition of the cyclooxygenase (COX)-2 isoenzyme, in a canine model of synovitis. Acute synovitis of the stifle joint was induced by intra-articular injection of sodium urate crystals. Robenacoxib (0.25, 0.5, 1.0, 2.0 and 4.0 mg/kg), placebo and meloxicam (0.2 mg/kg) were administered subcutaneously (s.c.) 3 h after the urate crystals. Pharmacodynamic endpoints included data from forceplate analyses, clinical orthopaedic examinations and time course of inhibition of COX-1 and COX-2 in ex vivo whole blood assays. Blood was collected for pharmacokinetics. Robenacoxib produced dose-related improvement in weight-bearing, pain and swelling as assessed objectively by forceplate analysis (estimated ED(50) was 1.23 mg/kg for z peak force) and subjectively by clinical orthopaedic assessments. The analgesic and anti-inflammatory effects of robenacoxib were significantly superior to placebo (0.25-4 mg/kg robenacoxib) and were non-inferior to meloxicam (0.5-4 mg/kg robenacoxib). All dosages of robenacoxib produced significant dose-related inhibition of COX-2 (estimated ED(50) was 0.52 mg/kg) but no inhibition of COX-1. At a dosage of 1-2 mg/kg administered s.c., robenacoxib should be at least as effective as 0.2 mg/kg of meloxicam in suppressing acute joint pain and inflammation in dogs.

BACKGROUND ; AIMS: Selective cyclooxygenase-2 inhibitors were developed to reduce the gastrointestinal risk associated with nonsteroidal anti-inflammatory drugs (NSAIDs). The Therapeutic Arthritis Research and Gastrointestinal Event Trial was the largest study to evaluate primarily the gastrointestinal safety outcomes of selective cyclooxygenase-2 inhibitors. Data from the Therapeutic Arthritis Research and Gastrointestinal Event Trial were used to identify risk factors and investigate the safety of lumiracoxib in subgroups. METHODS: Patients with osteoarthritis (age, >or=50 y) were randomized to receive lumiracoxib 400 mg once daily, naproxen 500 mg twice daily, or ibuprofen 800 mg 3 times daily for 12 months. Events were categorized by a blinded adjudication committee. The primary end point was all definite or probable ulcer complications. RESULTS: For patients taking NSAIDs, factors associated with an increased risk of ulcer complications were age 65 years or older (hazard ratio [HR], 2.30; 95% confidence interval [CI], 1.48-3.59), previous history of gastrointestinal bleed or ulcer (HR, 3.61; 95% CI, 1.86-7.00), non-Caucasian racial origin (HR, 2.10; 95% CI, 1.35-3.27), and male sex (HR, 1.70; 95% CI, 1.08-2.68). With lumiracoxib, significant risk factors were age 65 years or older (HR, 3.18; 95% CI, 1.40-7.20), male sex (HR, 2.60; 95% CI, 1.25-5.40), non-Caucasian racial origin (HR, 2.16; 95% CI, 1.02-4.59), and concomitant aspirin use (HR, 2.89; 95% CI, 1.40-5.97). Increased risks in patients age 65 years and older were increased further if other risk factors were present. Lumiracoxib maintained an advantage over NSAIDs across all subgroups except aspirin use. CONCLUSIONS: Lumiracoxib was associated with a reduced risk of ulcer complications compared with NSAIDs in all significant subgroups except aspirin users.
