Introduction: la biopsie du ganglion sentinelle (GS) est une procédure reconnue et fiable pour établir le stade ganglionnaire du mélanome cutané. Le GS est le facteur pronostique le plus puissant pour la survie des patients atteints d'un mélanome à risque intermédiaire, cliniquement localisé. Celui-ci est métastatique dans environ 15-30% des cas. Lorsque le GS est positif, un curage de l'aire ganglionnaire concernée est généralement entrepris. Néanmoins, seuls 20-25% de ces patients présentent des ganglions non-sentinelles (GNS) métastatiques. Ces données suggèrent que le curage, et les risques opératoires qui y sont associés, n'est peut-être pas nécessaire chez le trois-quarts de ces patients. Un autre aspect est que l'impact sur la survie des curages basé sur le résultat du GS n'est pas clairement démontré. La nécessité de ce curage d'emblé est actuellement en cours d'évaluation par un protocole international (Multicenter Selective Lymphadenectomy Trial II : MSLT II). Plusieurs auteurs ont essayé de classifier la charge tumorale du GS afin d'évaluer s'il était possible d'épargner le curage à certains patients et de mieux affiner ce facteur pronostic sans succès. En 2009, le Groupe Mélanome de l'EORTC (European Organisation for Research and Treatment of Cancer) a recommandé un protocole d'évaluation anatomopathologique du GS-positif en trois items: (1) la localisation micro-anatomique des métastases à l'intérieur du ganglion selon Dewar (A = sous-capsulaire, B = combinée sous-capsulaire and parenchymateuse, C = parenchymateuse, D = multifocale, and'E = extensive) ; (2) la mesure de la taille tumorale dans le ganglion selon les critères de Rotterdam pour le diamètre maximal. Le diamètre de la plus grande métastase est exprimé en nombre absolu et (3) la taille tumorale stratifiée par catégories : <0.1mm, 0.1-1.0mm et >1.0 mm. Le but de cette étude rétrospective d'une cohorte de patients, était d'investiguer les résultats des GS-positifs et d'analyser les facteurs pronostiques de la survie à la lumière des recommandations de l'EORTC. Ainsi que de comparer les sous-groupes du GS-positif avec une invasion minimale (taille tumorale <0.1mm et/ou atteinte sous-capsulaire) avec le GS-négatif. Les facteurs pouvant prédire la présence de GNS- positif ont également été analysés. Matériel et méthode : une étude des dossiers a été réalisée pour les 499 patients consécutifs entre 1997 et 2008 qui ont eu une biopsie du GS dans notre institution. Le dégrée d'envahissement du GS-positif a été entièrement revue par l'équipe référente de l'Institut de Pathologie (Dresse E. Saiji et Dresse H. Bouzourène) selon les recommandations de l'EORTC. Des analyses univariées et multivariées des potentiels facteuis pronostics ont été réalisées. Des analyses de survie ont également été effectuées avec des courbes d'estimation de Kaplan-Meier combinées à une régression de Cox. Le protocole a été accepté par la Commission d'Ethique. Résultats: un GS-positif a été trouvé chez 123 (25%) patients panni les 499 qui ont bénéficié d'une biopsie. Avec un suivi médian de 52 mois, la survie à 5 ans sans récidive (SSR), spécifique à la maladie (SS) et globale (SG) étaient de 88%, 94%, et 90% respectivement pour les patients avec GS-négatif. Concernant les GS avec invasion minimale, 21 patients étaient dans le sous-groupe <0.1 mm selon les critères de Rotterdam et 52 patients dans le sous-groupe sous-capsulaire selon Dewar. La survie dans ces deux sous-groupes était de 80% et 57% pour la SSR, 87% et 70% pour la SS, 87% et 68% pour la SG, respectivement. L'analyse multivariée des GS-positifs a montré que les facteuis suivants influençaient significativement la survie (SSR, SS et SG): l'épaisseur selon Breslow de la tumeur primaire (p=0.002, 0.006, 0.004), la taille tumorale du GS-positif >0.1 mm (p= 0.01, 0.04, 0.03), le genre masculin (p=0.06, 0.005, 0.002) et l'ulcération de la tumeur primaire (p=0.05, 0.03, 0.007). L'analyse des sous-groupes avec invasion minimale n'a pas permis d'établir de facteur pour prédire la négativité des GNSs. Conclusion: La classification du GS-positif par la taille tumorale selon les critères de Rotterdam est un facteur pronostique simple et utile pour évaluer la survie des patients atteints de mélanome. Nous avons observé une tendance (non statistiquement significative) d'une survie diminuée pour le sous-groupe des patients avec GS-positif et une taille de la métastase <0.1 mm comparée à celle des patients avec GS-négatif. Ceci nous incite à conclure que ce sous-groupe de patients ne devrait pas être assimilé et traité comme ceux qui ont un GS-négatif. D'autre part nos résultats montrent que la localisation micro-anatomique selon Dewar n'est pas un outil pronostique utile pour évaluer la survie, ni pour prédire le status des GNSs.

This paper proposes an automatic hand detection system that combines the Fourier-Mellin Transform along with other computer vision techniques to achieve hand detection in cluttered scene color images. The proposed system uses the Fourier-Mellin Transform as an invariant feature extractor to perform RST invariant hand detection. In a first stage of the system a simple non-adaptive skin color-based image segmentation and an interest point detector based on corners are used in order to identify regions of interest that contains possible matches. A sliding window algorithm is then used to scan the image at different scales performing the FMT calculations only in the previously detected regions of interest and comparing the extracted FM descriptor of the windows with a hand descriptors database obtained from a train image set. The results of the performed experiments suggest the use of Fourier-Mellin invariant features as a promising approach for automatic hand detection.

BACKGROUND: We report a patient with a highly unusual presentation of a mitochondrial disorder. HISTORY AND SIGNS: An 8-year old girl presented with muscular cramps as well as height and weight deceleration. Investigations revealed lactic acidosis, electrolytic imbalance and urinary loss of glucose and electrolytes secondary to proximal renal tubulopathy consistent with Fanconi syndrome (FS). Ophthalmic examination revealed asymptomatic retinitis pigmentosa (RP) with no other ocular manifestations. A mitochondriopathy was suspected and genetic analysis performed. THERAPY AND OUTCOME: Southern blotting documented a heteroplasmic mutation of mtDNA with deletion/duplication. Three discrete mitochondrial genomes were detected: normal; deletion of 6.7 kb and a deletion/duplication consisting of 1 normal and 1 deleted genome. The relative proportions varied considerably between tissues. CONCLUSIONS: The association of FS and RP combines features of Kearns-Sayre syndrome and Pearson marrow-pancreas syndrome, without being typical of either. This highly unusual clinical presentation emphasises the need for systemic investigation of patients with FS and further underlines the importance of mtDNA analysis in patients with unexpected associations of affected tissues.

The collaboration led to the establishment of the Training and Employment Outcomes System (TEOS) which combines wage data with education records for community colleges. This report summarizes results on four areas: Descriptive wage summaries; the returns to education; transitions for program majors to the workforce; and the five-years in-state retention rate of community college graduates.

3 Summary 3. 1 English The pharmaceutical industry has been facing several challenges during the last years, and the optimization of their drug discovery pipeline is believed to be the only viable solution. High-throughput techniques do participate actively to this optimization, especially when complemented by computational approaches aiming at rationalizing the enormous amount of information that they can produce. In siiico techniques, such as virtual screening or rational drug design, are now routinely used to guide drug discovery. Both heavily rely on the prediction of the molecular interaction (docking) occurring between drug-like molecules and a therapeutically relevant target. Several softwares are available to this end, but despite the very promising picture drawn in most benchmarks, they still hold several hidden weaknesses. As pointed out in several recent reviews, the docking problem is far from being solved, and there is now a need for methods able to identify binding modes with a high accuracy, which is essential to reliably compute the binding free energy of the ligand. This quantity is directly linked to its affinity and can be related to its biological activity. Accurate docking algorithms are thus critical for both the discovery and the rational optimization of new drugs. In this thesis, a new docking software aiming at this goal is presented, EADock. It uses a hybrid evolutionary algorithm with two fitness functions, in combination with a sophisticated management of the diversity. EADock is interfaced with .the CHARMM package for energy calculations and coordinate handling. A validation was carried out on 37 crystallized protein-ligand complexes featuring 11 different proteins. The search space was defined as a sphere of 15 R around the center of mass of the ligand position in the crystal structure, and conversely to other benchmarks, our algorithms was fed with optimized ligand positions up to 10 A root mean square deviation 2MSD) from the crystal structure. This validation illustrates the efficiency of our sampling heuristic, as correct binding modes, defined by a RMSD to the crystal structure lower than 2 A, were identified and ranked first for 68% of the complexes. The success rate increases to 78% when considering the five best-ranked clusters, and 92% when all clusters present in the last generation are taken into account. Most failures in this benchmark could be explained by the presence of crystal contacts in the experimental structure. EADock has been used to understand molecular interactions involved in the regulation of the Na,K ATPase, and in the activation of the nuclear hormone peroxisome proliferatoractivated receptors a (PPARa). It also helped to understand the action of common pollutants (phthalates) on PPARy, and the impact of biotransformations of the anticancer drug Imatinib (Gleevec®) on its binding mode to the Bcr-Abl tyrosine kinase. Finally, a fragment-based rational drug design approach using EADock was developed, and led to the successful design of new peptidic ligands for the a5ß1 integrin, and for the human PPARa. In both cases, the designed peptides presented activities comparable to that of well-established ligands such as the anticancer drug Cilengitide and Wy14,643, respectively. 3.2 French Les récentes difficultés de l'industrie pharmaceutique ne semblent pouvoir se résoudre que par l'optimisation de leur processus de développement de médicaments. Cette dernière implique de plus en plus. de techniques dites "haut-débit", particulièrement efficaces lorsqu'elles sont couplées aux outils informatiques permettant de gérer la masse de données produite. Désormais, les approches in silico telles que le criblage virtuel ou la conception rationnelle de nouvelles molécules sont utilisées couramment. Toutes deux reposent sur la capacité à prédire les détails de l'interaction moléculaire entre une molécule ressemblant à un principe actif (PA) et une protéine cible ayant un intérêt thérapeutique. Les comparatifs de logiciels s'attaquant à cette prédiction sont flatteurs, mais plusieurs problèmes subsistent. La littérature récente tend à remettre en cause leur fiabilité, affirmant l'émergence .d'un besoin pour des approches plus précises du mode d'interaction. Cette précision est essentielle au calcul de l'énergie libre de liaison, qui est directement liée à l'affinité du PA potentiel pour la protéine cible, et indirectement liée à son activité biologique. Une prédiction précise est d'une importance toute particulière pour la découverte et l'optimisation de nouvelles molécules actives. Cette thèse présente un nouveau logiciel, EADock, mettant en avant une telle précision. Cet algorithme évolutionnaire hybride utilise deux pressions de sélections, combinées à une gestion de la diversité sophistiquée. EADock repose sur CHARMM pour les calculs d'énergie et la gestion des coordonnées atomiques. Sa validation a été effectuée sur 37 complexes protéine-ligand cristallisés, incluant 11 protéines différentes. L'espace de recherche a été étendu à une sphère de 151 de rayon autour du centre de masse du ligand cristallisé, et contrairement aux comparatifs habituels, l'algorithme est parti de solutions optimisées présentant un RMSD jusqu'à 10 R par rapport à la structure cristalline. Cette validation a permis de mettre en évidence l'efficacité de notre heuristique de recherche car des modes d'interactions présentant un RMSD inférieur à 2 R par rapport à la structure cristalline ont été classés premier pour 68% des complexes. Lorsque les cinq meilleures solutions sont prises en compte, le taux de succès grimpe à 78%, et 92% lorsque la totalité de la dernière génération est prise en compte. La plupart des erreurs de prédiction sont imputables à la présence de contacts cristallins. Depuis, EADock a été utilisé pour comprendre les mécanismes moléculaires impliqués dans la régulation de la Na,K ATPase et dans l'activation du peroxisome proliferatoractivated receptor a (PPARa). Il a également permis de décrire l'interaction de polluants couramment rencontrés sur PPARy, ainsi que l'influence de la métabolisation de l'Imatinib (PA anticancéreux) sur la fixation à la kinase Bcr-Abl. Une approche basée sur la prédiction des interactions de fragments moléculaires avec protéine cible est également proposée. Elle a permis la découverte de nouveaux ligands peptidiques de PPARa et de l'intégrine a5ß1. Dans les deux cas, l'activité de ces nouveaux peptides est comparable à celles de ligands bien établis, comme le Wy14,643 pour le premier, et le Cilengitide (PA anticancéreux) pour la seconde.

Background: Single agent DTIC is the standard therapy for metastatic melanoma (MM) with response rates of 5−20%. Temozolomide (Tem) as an oral drug has shown equal efficacy in phase III trials. Preclinical models have shown an inhibitory effect for bevacizumab (Bev) on the proliferation of melanoma cells as well as on sprouting endothelial cells. Therefore, a therapeutic approach that combines angiogenesis inhibitors with cytotoxic agents may provide clinical benefit in MM. Methods: Design: Multicenter phase II trial. Primary endpoint: Clinical benefit (CR, PR and SD) at 12 weeks; secondary endpoints: best overall response by RECIST, response duration, progression free survival, adverse events, survival after 6 months and overall survival. Sample size was calculated according to Simon's two stage optimal design (5% significance level and 80% power) with an overall sample size of 62 patients (pts) to test H0: 20% versus H1: 35% rate of clinical benefit. Response assessment was done every 6 weeks (3 cycles). Eligibility: Stage IV MM, ECOG PS 0−2, no prior treatment for metastatic disease. Treatment regimen: One cycle consisted of Tem at 150 mg/m2 days 1−7 po and Bev at 10 mg/kg day 1 over 30 min iv and was repeated every 2 weeks until progression or unacceptable toxicity. Results: Between January 2008 and April 2009, 62 pts (40 male/22 female) at a median age of 61 years (range 30−86) with stage IV (M1a:4, M1b:12, M1c:46) melanoma were enrolled in 9 centers. The first 50 pts, who received 415 cycles are included in this interim report. The overall response rate was 26% (CR: 1 pt, PR: 12 pts; PR not confirmed yet in 3 pts), and 44% (22 pts) had stable disease over 1.5−7.5 months (median: 3). Only 30% (15 pts) had disease progression at the first evaluation at week 6. The hematological grade 3/4 toxicities according to NCI CTAE 3.0 were thrombocytopenia 10% (5 pts), neutropenia 8% (4 pts), lymphopenia and leucocytopenia each 2% (1 pt). Cumulative non-hematological toxicities grade 3/4 were nausea and fatigue each 6% (3 pts), hypertension, vomiting and hemorrhage, each 4% (2 pts), thrombosis/embolism, infection, constipation, anorexia, elevation of alkaline phosphatase, bilirubin, GGT, ALT and AST each 2% (1 pt). Conclusion: In metastatic melanoma the combination of Tem/Bev is a safe regimen with a promising efficacy and few grade 3/4 toxicities. Updated results of all 62 pts will be presented.

Wars are often associated with a rhetoric of renewal or new beginnings. This essay explores this claim through the lens of civil religion and a recent book by Carolyn Marvin and David Ingle, Blood Sacrifice and the Nation, which combines Emile Durkheim with Réné Girard in proposing that modern national cohesion depends on blood sacrifice. I unpack some of the paradoxes raised by this theory of national renewal in the context of 9/11, with a special focus on the sacred status of the flag and the special attention given to uniformed serviceman in the American body politic.

Correlative fluorescence and electron microscopy has become an indispensible tool for research in cell biology. The integrated Laser and Electron Microscope (iLEM) combines a Fluorescence Microscope (FM) and a Transmission Electron Microscope (TEM) within one set-up. This unique imaging tool allows for rapid identification of a region of interest with the FM, and subsequent high resolution TEM imaging of this area. Sample preparation is one of the major challenges in correlative microscopy of a single specimen; it needs to be apt for both FM and TEM imaging. For iLEM, the performance of the fluorescent probe should not be impaired by the vacuum of the TEM. In this technical note, we have compared the fluorescence intensity of six fluorescent probes in a dry, oxygen free environment relative to their performance in water. We demonstrate that the intensity of some fluorophores is strongly influenced by its surroundings, which should be taken into account in the design of the experiment. Furthermore, a freeze-substitution and Lowicryl resin embedding protocol is described that yields excellent membrane contrast in the TEM but prevents quenching of the fluorescent immuno-labeling. The embedding protocol results in a single specimen preparation procedure that performs well in both FM and TEM. Such procedures are not only essential for the iLEM, but also of great value to other correlative microscopy approaches.

A major problem with holographic optical tweezers (HOTs) is their incompatibility with laser-based position detection methods, such as back-focal-plane interferometry (BFPI). The alternatives generally used with HOTs, like high-speed video tracking, do not offer the same spatial and temporal bandwidths. This has limited the use of this technique in precise quantitative experiments. In this paper, we present an optical trap design that combines digital holography and back-focal-plane displacement detection. We show that, with a particularly simple setup, it is possible to generate a set of multiple holographic traps and an additional static non-holographic trap with orthogonal polarizations and that they can be, therefore, easily separated for measuring positions and forces with the high positional and temporal resolutions of laser-based detection. We prove that measurements from both polarizations contain less than 1% crosstalk and that traps in our setup are harmonic within the typical range. We further tested the instrument in a DNA stretching experiment and we discuss an interesting property of this configuration: the small drift of the differential signal between traps.

OBJECTIVES: In this study, we investigated the structural plasticity of the contralesional motor network in ischemic stroke patients using diffusion magnetic resonance imaging (MRI) and explored a model that combines a MRI-based metric of contralesional network integrity and clinical data to predict functional outcome at 6 months after stroke. METHODS: MRI and clinical examinations were performed in 12 patients in the acute phase, at 1 and 6 months after stroke. Twelve age- and gender-matched controls underwent 2 MRIs 1 month apart. Structural remodeling after stroke was assessed using diffusion MRI with an automated measurement of generalized fractional anisotropy (GFA), which was calculated along connections between contralesional cortical motor areas. The predictive model of poststroke functional outcome was computed using a linear regression of acute GFA measures and the clinical assessment. RESULTS: GFA changes in the contralesional motor tracts were found in all patients and differed significantly from controls (0.001 ≤ p < 0.05). GFA changes in intrahemispheric and interhemispheric motor tracts correlated with age (p ≤ 0.01); those in intrahemispheric motor tracts correlated strongly with clinical scores and stroke sizes (p ≤ 0.001). GFA measured in the acute phase together with a routine motor score and age were a strong predictor of motor outcome at 6 months (r(2) = 0.96, p = 0.0002). CONCLUSION: These findings represent a proof of principle that contralesional diffusion MRI measures may provide reliable information for personalized rehabilitation planning after ischemic motor stroke. Cette thèse explore le rôle de la normalisation technique dans les délocalisations de services en Inde et au Maroc. La recherche appréhende le phénomène en opposant explicitement ou implicitement deux modèles. Un modèle serviciel dans lequel les activités de services sont difficilement délocalisables parce que non-mesurables ; un modèle industriel qui articule des activités de services mesurables par les normes techniques et susceptibles d'être délocalisés dans les pays du Sud à bas salaires. Notre thèse s'interroge sur la manière dont les relations Nord-Sud peuvent s'appréhender au sein de cette dichotomie et propose une réflexion épistémologique sur les représentations culturelles induites au sein de ce cadrage. A partir d'une perspective qui combine les apports de l'économie politique culturelle, la sociologie économique et les études postcoloniales au sein de l'économie politique internationale, elle mobilise trois catégories (la normalisation technique, l'imaginaire économique, la qualité). Ces catégories nous permettent de suggérer la centralité des enjeux de pouvoirs dans la définition de ce que sont les activités de services. L'analyse empirique suggère que les délocalisations de services au Maroc et en Inde expriment des réalités plus poreuses et plus dynamiques que la dichotomie entre modèle serviciel et modèle industriel laisse entendre. Elle met en évidence la capacité d'agir des acteurs des pays du Sud et suggère que les normes techniques ont une fonction politique à travers leurs fonctions de mesure. Abstract This thesis explores the role of technical standards in offshore outsourcing in India and Morocco. Current research captures the phenomenon while opposing explicitly or implicitly two models of production. A service-based model in which service activities are difficult to relocate because they are non-measurable; an industrial model that articulates service activities measured with technical standards and that may be outsourced to developing countries with low wages. Our thesis questions how North-South relations can be grasped within this dichotomy and offers an epistemologica! reflection on cultural representations induced within this framework. From a perspective that combines the contributions of cultural political economy, economic sociology and postcolonial studies in international political economy, it mobilizes three categories (technical standardization, the economic imaginary and quality). These categories allow us to suggest the centrality of power issues in the definition of service activities. The empirical analysis suggests that offshoring of services in Morocco and India express more porous and dynamic realities than the dichotomy suggested between a service model and an industrial model. It highlights the ability of the actors to act in the South and suggests that technical standards have a political function through their measurement functions.

Structures built by animals are a widespread and ecologically important 'extended phenotype'. While its taxonomic diversity has been well described, factors affecting short-term evolution of building behavior within a species have received little experimental attention. Here we describe how, given the opportunity, wandering Drosophila melanogaster larvae often build long tunnels in agar substrates and embed their pupae within them. These embedded larvae are characterized by a longer egg-to-pupariation developmental time than larvae that pupate on the surface. Assuming that such building behaviors are likely to be energetically costly and/or time consuming, we hypothesized that they should evolve to be less pronounced under resource or time limitation. In accord with this prediction, larvae from populations evolved for 160 generations under a regime that combines larval malnutrition with limited developmental time dug shorter tunnels than larvae from control unselected populations. However, the proportion of larvae that embedded before pupation did not differ between the malnutrition-adapted and control populations, suggesting that tunnel length and likelihood of embedding before pupation are controlled by different genetic loci. The behaviors exhibited by wandering larvae of Drosophila melanogaster prior to pupation offer a model system to study evolution of animal building behaviors because the tunneling and embedding phenotypes are simple, facultative and highly variable.

This article provides an in-depth study of long-term female unemployment in Catalonia.Long-term unemployment statistics reveal which social groups are most likely to experience difficulty re-entering the labour market. In this case, we found that women are mainly affected by this type of labour exclusion, in particular poorly qualified, working-class women who are aged over 45 and with family responsibilities.The article aims to explore how the overlapping of factors such as gender, age, social class, origin and the division of work based on gender are related to long-term female unemployment. Moreover, we were able to detect which conceptual tools provide us with the production/reproduction paradigm so as to be able to analyse the future of female unemployment. The methodology we used combines quantitative and qualitative approaches. On the one hand, the analysis of secondary statistical data focusing on Catalonia is useful in understanding the situation from a macro-social perspective. On the other hand, an exploratory discussion group enables us to investigate social imaginary practises among unemployed working class women aged over 45. This discussion group was held in Igualada -capital of the Anoia region - an area of Catalonia deeply affected by unemployment in the current economic crisis.

L'Espai Europeu d'Educació Superior comporta un canvi de paradigma molt important, com ésl'adopció d'un procés d'aprenentatge basat en l'adquisició i desenvolupament de competències mitjançant activitats, relegant els continguts a un segon terme. El propi concepte d'activitat inclou i combina elements molt diversos que involucren recursos docents de tota mena, tot abastant un ventall més ampli del que s'entén estrictament per contingut en el sentit tradicional. És necessari, doncs, plantejar-se una gestió dels recursos docents orientada a facilitar la creació d'itineraris formatius que permetin als estudiants assolir les competències desitjades, així com complementar la seva formació. En lloc de proporcionar els continguts directament als estudiants, l'objectiu és aconseguir un procés d'aprenentatge centrat en l'usuari mitjançant la formació que els permeti ser autònoms en la localització, l'ús i fins i tot la generació de continguts necessaris en cada activitat.
