Aims: To evaluate the reliability of fine needle aspirate cell blocks in the assessment of oestrogen receptor (ER), progesterone receptor (PR) and HER-2/neu proteins by immunohistochemistry in comparison with surgical specimens. Materials and methods: This is a retrospective study of 62 cases of breast carcinoma diagnosed by fine needle aspiration cytology (FNAC) and confirmed using the surgical specimen. Immunohistochemical tests were performed to assess the presence of oestrogen receptor (ER), progesterone receptor (PR) and HER-2/neu proteins in cell blocks and the corresponding surgical specimens. The cell block method used alcohol prior to formalin fixation. Cases with 10% or more stained cells were considered positive for ER and PR. Positivity for HER-2/neu was assessed on a scale of 0-3+. The criterion for positivity was a score of 3+. Results: Sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV) and accuracy of the cell blocks in the investigation of ER, PR and HER-2/neu protein (3+) were (%): ER, 92.7, 85.7, 92.7, 85.7 and 90.3; PR, 92.7, 94.7, 97.4, 87.0 and 93.5; HER-2/neu, 70.0, 100.0, 100.0, 94.5 and 95.2. Discrepancies were seen in cell blocks in the 1+ and 2+ HER-2/neu staining scores: two of 12 cases scoring 2+ and one case of 26 scoring 1+ on cell blocks scored 3+ on surgical specimens. The correlation index between cell block and corresponding surgical specimen varied from 90% to 94%. Conclusion: Cell blocks provide a useful method of assessing ER, PR and HER-2/neu, mainly for inoperable and recurrent cases, but consideration should be given to carrying out FISH analysis on 1+ as well as 2+ HER-2/neu results. © 2012 Blackwell Publishing Ltd.

CONTEXTO:A melhor dose para o início do tratamento anticoagulante com varfarina vem sendo debatida nos últimos dez anos. Em nosso meio, não observamos nenhum estudo comparativo quanto a estas características.OBJETIVO:Comparar segurança e eficácia de dois esquemas de dosagem inicial de varfarina para tratamento anticoagulante.MÉTODOS:Foram estudados prospectivamente 110 pacientes de ambos os sexos, consecutivos, com indicação de anticoagulação por tromboembolismo venoso ou arterial. Durante os três primeiros dias de tratamento, estes pacientes receberam doses adequadas de heparina (RT - razão dos tempos - alvo entre 1,5 e 2,5) e 5 mg de varfarina, cuja dose foi reajustada a partir do quarto dia pelo Razão Normatizada Internacional - RNI (alvo entre 2 e 3). Esse grupo foi comparado com série histórica de 110 pacientes que receberam 10 mg nos dois primeiros dias, 5 mg a partir do terceiro dia, com ajuste posterior de dose baseado no RNI. Os desfechos foram: recorrência do tromboembolismo, sangramentos e tempo para alcançar níveis terapêuticos.RESULTADOS:A eficácia, a segurança e o tempo de internação foram similares entre os grupos. O grupo que recebeu 10 mg atingiu níveis terapêuticos mais precocemente (média de 4,5 dias × 5,8 dias), sendo as doses na alta menores e os níveis terapêuticos mais adequados na primeira visita de retorno.CONCLUSÃO:O esquema de dosagem de 10 mg proporcionou menor tempo para alcançar nível terapêutico, com menores doses de varfarina na alta e RNI mais adequado no retorno.

Background: Exposure to fine fractions of particulate matter (PM2.5) is associated with increased hospital admissions and mortality for respiratory and cardiovascular disease in children and the elderly. This study aims to estimate the toxicological risk of PM2.5 from biomass burning in children and adolescents between the age of 6 and 14 in Tangara da Serra, a municipality of Subequatorial Brazilian Amazon. Methods: Risk assessment methodology was applied to estimate the risk quotient in two scenarios of exposure according to local seasonality. The potential dose of PM2.5 was estimated using the Monte Carlo simulation, stratifying the population by age, gender, asthma and Body Mass Index (BMI). Results: Male asthmatic children under the age of 8 at normal body rate had the highest risk quotient among the subgroups. The general potential average dose of PM2.5 was 1.95 mu g/kg.day (95% CI: 1.62 - 2.27) during the dry scenario and 0.32 mu g/kg. day (95% CI: 0.29 - 0.34) in the rainy scenario. During the dry season, children and adolescents showed a toxicological risk to PM2.5 of 2.07 mu g/kg. day (95% CI: 1.85 - 2.30). Conclusions: Children and adolescents living in the Subequatorial Brazilian Amazon region were exposed to high levels of PM2.5 resulting in toxicological risk for this multi-pollutant. The toxicological risk quotients of children in this region were comparable or higher to children living in metropolitan regions with PM2.5 air pollution above the recommended limits to human health.

The progression of carcinogenesis entails the detachment of cells, invasion and migration of neoplastic cells. Alterations in epithelial adhesion and basement membrane proteins might mediate the early stages of carcinogenesis. This study investigated the expression of adhesion molecules and the basement membrane protein laminin-5 in actinic cheilitis (AC) and incipient squamous cell carcinoma of the lower lip to understand early photocarcinogenesis. Ln-5 gamma 2 chain as well as alpha 3, beta 1 subunits of alpha 3 beta 1 heterodimer and beta 4 subunit of integrin alpha 6 beta 4 were evaluated by immunohistochemistry in 16 cases of AC and 16 cases of superficially invasive squamous cell carcinoma (SISCC). Most AC cases showed reduced expression of beta 1, beta 4 and alpha 3 integrins, and SISCCs lacked beta 1, beta 4 and alpha 3 integrins in the invasive front. AC cases were negative for the Ln-5 gamma 2 chain. Five cases of SISCC (31%) showed heterogeneous Ln-5 gamma 2 chain expression in the invasive front of the tumor. Integrin beta 1, beta 4 and alpha 3 expression is lost during the early stages of lip carcinogenesis. Expression of Ln-5 gamma 2 in the invasive front in cases and its correlation with tumor progression suggest that it mediates the acquisition of the migrating and invading epithelial cell phenotype. Abstract Background Forearm blood flow responses during mental stress are greater in individuals homozygous for the Glu27 allele. A high-fat meal is associated with impaired endothelium-dependent dilatation. We investigated the impact of high-fat ingestion on the muscle vasodilatory responses during mental stress in individuals with the Glu27 allele and those with the Gln27 allele of the β2-adrenoceptor gene. Methods A total of 162 preselected individuals were genotyped for the Glu27Gln β2-adrenoceptor polymorphism. Twenty-four individuals participated in the study. Fourteen were homozygous for the Gln27 allele (Gln27Gln, 40 ± 2 years; 64 ± 2 kg), and 10 were homozygous for the Glu27 allele (Glu27Glu, 40 ± 3 years; 65 ± 3 kg). Forearm blood flow was evaluated by venous occlusion plethysmography before and after ingestion of 62 g of fat. Results The high-fat meal caused no changes in baseline forearm vascular conductance (FVC, 2.2 ± 0.1 vs. 2.4 ± 0.2; P = 0.27, respectively), but reduced FVC responses to mental stress (1.5 ± 0.2 vs. 0.8 ± 0.2 units; P = 0.04). When volunteers were divided according to their genotypes, baseline FVC was not different between groups (Glu27Glu = 2.4 ± 0.1 vs. Gln27Gln = 2.1 ± 0.1 units; P = 0.08), but it was significantly greater in Glu27Glu individuals during mental stress (1.9 ± 0.4 vs. 1.0 ± 0.3 units; P = 0.04). High-fat intake eliminated the difference in FVC responses between Glu27Glu and Gln27Gln individuals (FVC, 1.3 ± 0.4 vs. 1.2 ± 0.4; P = 0.66, respectively). Conclusion These findings demonstrate that a high-fat meal impairs muscle vasodilatation responses to mental stress in humans. However, this reduction can be attributed to the presence of the homozygous Glu27 allele of the β2-adrenoceptor gene.

Beständig werden Spurenstoffe in die Atmosphäre emittiert, die ihren Ursprung in biogenen oder anthropogenen Quellen haben. Daß es dennoch im allgemeinen nicht zu einer Anreicherung bis hin zu toxischen Konzentrationen kommt, liegt an dem Vermögen der Atmosphäre sich durch Oxidationsprozesse selbst zu reinigen. Eine wichtige Aufgabe kommt dabei dem Hydroxylradikal OH zu, welches tagsüber die Oxidationskapazität der Atmosphäre bestimmt. Hierbei spielen die tropischen Regionen mit einer der höchsten OH-Produktionsraten eine zentrale Rolle. Gleichzeitig sind die tropischen Regenwälder eine bedeutende globale Quelle für Kohlenwasserstoffe, die durch Reaktion mit OH-Radikalen dessen Konzentration und damit die Oxidationskapazität der Atmosphäre herabsetzen. Während der GABRIEL-Meßkampagne 2005 im äquatorialen Südamerika wurde der Einfluß der Regenwaldemissionen auf das HOx-Budget (HOx = OH+HO2) untersucht. Zu diesem Zweck wurde das Radikalmeßinstrument HORUS entwickelt. Im Rahmen dieser Arbeit wurden unterschiedliche Komponenten des Gerätes optimiert, der Meßaufbau ins Flugzeug integriert und Methoden zur Kalibrierung entwickelt. Bei der internationalen Vergleichskampagne HOxComp2005 zeigte HORUS seine Eignung zur Messung von troposphärischen OH- und HO2-Radikalen.rnrnDie durchgeführten HOx-Messungen während der GABRIEL-Meßkampagne sind die ersten ihrer Art, die über einem tropischen Regenwald stattgefunden haben. Im Gegensatz zu den Vorhersagen globaler Modelle wurden unerwartet hohe OH- und HO2-Konzentrationen in der planetaren Grenzschicht des tropischen Regenwalds beobachtet. Der Vergleich der berechneten OH-Produktions- und Verlustraten, die aus dem umfangreichen Datensatz von GABRIEL ermittelt wurden, zeigte, daß hierbei eine wichtige OH-Quelle unberücksichtigt blieb. Mit Hilfe des Boxmodells MECCA, in welchem die gemessenen Daten als Randbedingungen in die Simulationen eingingen, wurden die modellierten OH- und HO2- Konzentrationen im Gleichgewichtszustand den beobachteten Konzentrationen gegenübergestellt. Luftmassen der freien Troposphäre und der maritimen Grenzschicht zeigten eine gute Übereinstimmung zwischen Messung und Modell. Über dem tropischen Regenwald jedoch wurden die beobachteten HOx-Konzentrationen in der planetaren Grenzschicht durch das Modell, vor allem am Nachmittag, signifikant unterschätzt. Dabei lag die Diskrepanz zwischen den beobachteten und simulierten Konzentrationen bei einem mittleren Wert von OHobs/OHmod = 12.2 ± 3.5 und HO2obs/HO2mod = 4.1 ± 1.4. Die Abweichung zwischen Messung und Modell korrelieren hierbei mit der Isoprenkonzentration. Während für niedrige Isoprenmischungsverhältnisse, wie sie über dem Ozean oder in Höhen > 3 km vorherrschten, die Beobachtungen mit den Simulationen innerhalb eines Faktors 1.6±0.7 übereinstimmten, nahm die Unterschätzung durch das Modell für steigende Isoprenmischungsverhältnisse > 200 pptV über dem tropischen Regenwald zu.rnrnDer kondensierte chemische Mechanismus von MECCA wurde mit der ausführlichen Isoprenchemie des ”Master Chemical Mechanism“ überprüft, welches vergleichbare HOx-Konzentrationen lieferte. OH-Simulationen, durchgeführt mit der gemessenen HO2-Konzentration als zusätzliche Randbedingung, zeigten, daß die Konversion zwischen HO2 und OH innerhalb des Modells nicht ausreichend ist. Durch Vernachlässigung der gesamten Isoprenchemie konnte dagegen eine Übereinstimmung zwischen Modell und Messung erreicht werden. Eine OH-Quelle in der gleichen Größenordnung wie die OH-Senke durch Isopren, ist somit zur Beschreibung der beobachteten OH-Konzentration notwendig. Reaktionsmechanismen, die innerhalb der Isoprenchemie die gleiche Anzahl an OH-Radikalen erzeugen wie sie verbrauchen, könnten eine mögliche Ursache sein. Unterschiedliche zusätzliche Reaktionen wurden in die Isoprenabbaumechanismen des Modells implementiert, die zur Erhöhung der OH-Quellstärke führen sollten. Diese bewirkten eine Zunahme der simulierten HO2-Konzentrationen um einen maximalen Faktor von 5 für OH und 2 für HO2. Es wird eine OH-Zyklierungswahrscheinlichkeit r von bis zu 94% gefordert, wie sie für die GABRIEL-Messungen erreicht wurde. Die geringe OH-Zyklierungswahrscheinlichkeit von 38% des Modells zeigte, daß wichtige Zyklierungsvorgänge im chemischen Mechanismus bislang nicht berücksichtigt werden. Zusätzliche Zyklierungsreaktionen innerhalb des Isoprenmechanismus, die auch unter niedrigen NO-Konzentrationen zur Rückbildung von OHRadikalen führen, könnten eine Erklärung für die über dem Regenwald beobachteten hohen OH-Konzentration liefern.rn

This work deals with the oxidation of 5-hydroxymethylfurfural (HMF) to 2,5-furandicarboxylic acid (FDCA) using metal supported catalysts. Catalysts were prepared from the immobilisation of preformed monometallic (Au, Pd) and bimetallic (AuCu, AuPd) nanoparticles on commercial oxides (TiO2, CeO2). Au-TiO2 catalyst was found to be very active for HMF oxidation; however, this system deactivated very fast. For this reason, we prepared bimetallic gold-copper nanoparticles and an increase in the catalytic activity was observed together with an increase in catalyst stability. In order to optimise the interaction of the metal active phase with the support, Au and AuCu nanoparticles were supported onto CeO2. Au-CeO2 catalyst was found to be more active than the bimetallic one, leading to the conclusion that in this case the most important feature is the interaction between gold and the support. Catalyst pre-treatments (calcination and washing) were carried out to maximise the contact between the metal and the oxide and an increase in the FDCA production could be observed. The presence of ceria defective sites was crucial for FDCA formation. Mesoporous cerium oxide was synthesised with the hard template method and was used as support for Au nanoparticles to promote the catalytic activity. In order to study the role of active phase in HMF oxidation, PdAu nanoparticles were supported onto TiO2. Au and Pd monometallic catalysts were very active in the formation of HMFCA (5-hydroxymethyl-2-furan carboxylic acid), but Pd was not able to convert it, leading to a low FDCA yield. The calcination of PdAu catalysts led to Pd segregation on the particles surface, which changed the reaction pathway and included an important contribution of the Cannizzaro reaction. PVP protected PdAu nanoparticles, synthesised with different morphologies (core-shell and alloyed structure), confirmed the presence of a different reaction mechanism when the metal surface composition changes.

Le biomasse sono attualmente una promettente alternativa ai combustibili fossili per la produzione di sostanze chimiche e fuels. Nella trasformazione delle biomasse in prodotti chimici un ruolo importante è giocato dai derivati del furfurale; il 5-idrossimetil-2-furfurale (HMF), per esempio, è un precursore chiave per la sintesi di prodotti con applicazioni nell'industria dei polimeri e in campo farmaceutico. Può essere ossidato per ottenere l’acido 2,5-furandicarbossilico (FDCA), un monomero per la sintesi di una nuova classe di polimeri, alternativi a quelli ottenuti da acido tereftalico. Per la preparazione di FDCA da HMF sono stati utilizzati vari catalizzatori e differenti condizioni di reazione; il principale svantaggio è la necessità di sali metallici e solventi organici, che rendono il processo costoso e ad elevato impatto ambientale. Recentemente sono stati trovati catalizzatori di Au supportati, attivi nell’ossidazione del HMF a FDCA; tuttavia la stabilità del catalizzatore e la produttività del processo rimangono basse. Lo scopo del lavoro è stato lo studio di sistemi attivi e stabili nella reazione di ossidazione del HMF a FDCA. In particolare è stato approfondito l’effetto della morfologia del supporto di CeO2, utilizzato per la preparazione di catalizzatori a base di Au e l’influenza della fase attiva sul meccanismo di reazione in sistemi misti Pd/Au. Il lavoro svolto ha avuto come obiettivi: -La preparazione di catalizzatori Au-CeO2 mesoporoso, ottenuto mediante “hard template” e la loro caratterizzazione mediante analisi XRD, HRTEM, BET, XRF, TPR e porosimetriche. Confrontando questi sistemi con catalizzatori di Au-CeO2 commerciale è stato possibile osservare le differenze in termini di attività catalitica e di struttura. -La sintesi di nanoparticelle Pd/Au in lega o core-shell e la loro caratterizzazione mediante analisi DLS, XRF, XRD e HRTEM per comprendere come il tipo di fase attiva formata influisce sull’attività e sul meccanismo di reazione nell’ossidazione di HMF a FDCA.

The chemotherapeutic drug 5-fluorouracil (5-FU) is widely used for treating solid tumors. Response to 5-FU treatment is variable with 10-30% of patients experiencing serious toxicity partly explained by reduced activity of dihydropyrimidine dehydrogenase (DPD). DPD converts endogenous uracil (U) into 5,6-dihydrouracil (UH(2) ), and analogously, 5-FU into 5-fluoro-5,6-dihydrouracil (5-FUH(2) ). Combined quantification of U and UH(2) with 5-FU and 5-FUH(2) may provide a pre-therapeutic assessment of DPD activity and further guide drug dosing during therapy. Here, we report the development of a liquid chromatography-tandem mass spectrometry assay for simultaneous quantification of U, UH(2) , 5-FU and 5-FUH(2) in human plasma. Samples were prepared by liquid-liquid extraction with 10:1 ethyl acetate-2-propanol (v/v). The evaporated samples were reconstituted in 0.1% formic acid and 10 μL aliquots were injected into the HPLC system. Analyte separation was achieved on an Atlantis dC(18) column with a mobile phase consisting of 1.0 mm ammonium acetate, 0.5 mm formic acid and 3.3% methanol. Positively ionized analytes were detected by multiple reaction monitoring. The analytical response was linear in the range 0.01-10 μm for U, 0.1-10 μm for UH(2) , 0.1-75 μm for 5-FU and 0.75-75 μm for 5-FUH(2) , covering the expected concentration ranges in plasma. The method was validated following the FDA guidelines and applied to clinical samples obtained from ten 5-FU-treated colorectal cancer patients. The present method merges the analysis of 5-FU pharmacokinetics and DPD activity into a single assay representing a valuable tool to improve the efficacy and safety of 5-FU-based chemotherapy.
