One of the main reasons for the failure in dental implant treatments is the overload, which can cause bone resorption and later, the osseointegration loss in the implant. Therefore, the aim of this study was to analyze the tension generated around dental implants in the rehabilitation of three mandible posterior teeth, varying the connection type, the disposal, and the quantity of implants. The photoelasticity method was used in order to accomplish it. Through photoelasticity, the quantity and localization of the tensions around the implants in the different studied groups were compared (three straight line implants, three offset placement implants, two implants with a mesial cantilever, and two implants with a pontic). The results showed that the tension quantity and disposition around the dental implants of the connection external hexagon and internal hexagon were similar in all groups. In the group where the cantilever was used, an increase of the tension around the implant, adjacent to the cantilever, was observed. From the results it is concluded that the type of connection used in this study did not influence the tension quantity and distribution around the implants; however, the prosthetic configuration with the cantilever use, led to an increase of the tension around the implant, adjacent to the cantilever.

The biological principles of osseointegration caused the rehabilitation treatment with osseointegrated implant become a safe, well alternative accepted by the dental community for the high success rate, allowing the preparation of functional and aesthetic prostheses in edentulous and partial patients. We passed the initial phase of functional vision for an aesthetic approach, too, depending on the demands of the patient and the quest for excellence by professionals. Over these last years, implant treatment has undergone many changes in surgical and prosthetic protocols. The less invasive surgical techniques and the development of restorative materials, especially ceramics allow the prosthetic rehabilitation of high functional quality and aesthetics. The installation of implants in sockets immediately after extraction of teeth involved by fracture, periodontal disease, endodontic lesions, is part of the arsenal of techniques for rehabilitating a number of advantages that we get to the outcome of multidisciplinary treatment. For this, we must consider several criteria in its planning as a fundamental unit, adjacent teeth, multiple losses, system, form and number of implants, type of retention of the prosthesis components, the patient expectation of the outcome perspective, gingival biotype, standard of ridge resorption, the smile line, etc. occlusal analysis. With the three-dimensional placement of the implant can immediately join the biological processes of repair of the socket, implant osseointegration, speeding treatment time. Much has been made by companies in the surface treatment of endosseous implants to be osseointegration shortened and restorative procedures started earlier, bringing benefits to the patient and professional. Titanium has proven its suitability as an implant material in surgery over many years. Excellent biocompatibility and corrosion resistance are outstanding features. Implant surfaces always causes concern and interest in scientific communities, due to its close relationship with the time required for osseointegration. Surface modification can be performed by several methods, being laser irradiation one of them. Titanium implants with two different surfaces were inserted in rabbits: Group I (G-I: machined surface, control group), and group II (G-II: laser irradiated, test group) being processed 30 and 60 days after surgery for histological analysis. Surface characterization was performed with SEM-EDS, contact angle measurement, and mean roughness (Ra) parameters. Surface analysis in the GII group showed a nanomorphology affected by melt and quick solidification zones following laser irradiation (SEM), as well as total wettability and Ra mean values significantly higher than in the G-I group. The laser treatment resulted in a homogenized, porous surface, with increased surface area and volume. Histological analysis of bone-implant contact linear extension (BIC) showed better results in G-II at 30 days (39.26 ± 18.23 and 68.41 ± 13.68 for G-I and G-II groups, respectively). Titanium implants modified by laser irradiation showed important features that may accelerate early osseointegration.

The purpose of this literature review is to present the state-of-the-art on the application of tissue engineering techniques in sinus lifts procedures, reporting the influence of these techniques in increasing bone height and volume in the maxillary sinus, and the osseointegration of the implants installed in the grafted areas. PubMed and Scopus databases were searched using the keywords “scaffold” OR “engineered tissue” AND “sinus augmention” OR “sinus floor elevation”. In the initial search 463 articles were selected, of which 19 were selected abstracts reading, with nine papers selected by the end, for evaluation of the application of tissue engineering techniques in sinus lift procedures. Despite the positive results of tissue engineering procedures in sinus floor elevation reported in the selected articles, further studies are still needed, for a better standardization of experimental models and materials used, leading to definitive conclusions about the effects of the application of tissue engineering procedures on bone formation in maxillary sinus lifting procedures.

This paper presents a case report with the use of short dental implants and immediate prosthesis placement. After 13 months of follow-up, peri-implant probing values are near 1.87 mm and ISQ values around 78.56.

Dental implants represent a great improvement in Dentistry in respect to rehabilitation of edentulous ridges due to the functional and aesthetic reestablishment with long-term predictability and success. However, the effectiveness of this treatment relies on successful osseointegration during the healing period. In this way, the applicability of dental implants in patients with diabetes mellitus (DM) remains controversial, by the fact that the hyperglycemic status presents a negative effect on the osseointegration. It is not clear yet if the therapy with dental implants is an absolute contraindication to these patients. For this reason, the aim of the present study is to review the literature about the treatment with dental implants in patients with diabetes and to propose a protocol to perform dental implants in patients with diabetes. Diabetes has been considered a relative contraindication to dental implants, as the patients with adequate metabolic control can be treated with this kind of therapy, because inadequate metabolic control can lead to the failure of the treatment. Besides, there are risk factors for the diabetic implant patient that may decrease the success rates of dental implants therapy. In this way, the dentist should understand all the relevant implications before considering the indication of dental implants to patients with diabetes. It is important to have more controlled studies to evaluate the effects of diabetes on the implant-tissue interface and further investigations are necessary in order to elucidate the role of insulin and molecular mechanisms that might interfere on the osseointegration in patients with diabetes.

This case report presents details of a new surgical technique for mandibular ridge sagittal osteotomy and expansion associated to immediate dental implants in atrophic ridges. The bone atrophies represents a challenge for the surgeons that intends to modify this situation. In the past, the only viable option was the onlay bone graft. However, the bone graft requests a second surgical site that certainly increases the postoperative morbidity, without mentioning the longer treatment time required. The sagittal osteotomy of the alveolar crest represents a faster option, because it eliminates the time requested for bone graft integration, providing rehabilitation of edentulous areas with thin alveolar crests that otherwise would need additi onal surgical procedures for a satisfactory result. The authors report a clinical case in which this technique was used with the installation of a Bicon dental implant in the same surgical time, showing all the steps for this single-tooth rehabilitation.

The surface of dental implants is an important factor for osseointegration process and different methods of surface treatment have been described. Objective: To investigate the bone apposition in implant surface treated with sandblasting and acid-etching. Material and methods: Ten rabbits were selected and received one implant treated with method I in the left tibia and one implant treated with method II in the right tibia. Then, twenty implants were divided in two groups, according to methods of sandblasting and acid-etching (method I and method II). After 7, 14, 30, 45 and 60 days, tibias were retrieved and submitted to histotechnical procedures. The percentages of bone–implant contact (BIC) and bone area between threads (BABT) were determined throughout histomorphometric analysis and bone apposition was detected in implants of both groups. Results: In BABT measurements, an increase was observed after 45 and 60 days in the method II, compared to method I and no differences were found after 7, 14 and 30 days. In BIC measurements, an increase was detected with method II at 45 days when compared to method I. No differences between groups in BIC values were observed after 7, 14, 30 and 60 days. Conclusion: Our data demonstrated that implants treated with the method II presented increase in the contact between bone and implant after 45 days compared to method I. Moreover, with concern to bone area between threads, it was observed an increased in the method II after 45 and 60 days. However, both groups can be successfully used as a therapeutic strategy to rehabilitation of edentulous patients. Then, further experiments are needed to evaluate, in depth, the putative differential role of each surface treatment.

The purpose of this paper was to evaluate the influence of different implant designs on the micromovements of immediately placed implants. CT-based finite element models comprising an upper central incisor socket and four commercially available internal connection implant designs (SIN SW®, 3i Certain®, Nobel ReplaceTM, and RN synOcta® ITI Standard) of comparable diameter and length were constructed. 50, 100 and 200N magnitude loads were applied over the implant. ANOVA at 95% level of significance was used to evaluate bone to implant relative displacement (micromovements). The implant design (68,80%) greatly influences the micromovement of immediately placed implants. However, the loading magnitude (68,80%) is the most important factor regarding the implant stability in this protocol.
