Objective: To examine sociodemographic and dental factors for associations with dental sealant placement in children and adolescents aged 6-18 years old. Methods: Secondary data analysis of 2011-2012 NHANES data was conducted. Multiple logistic regression models were used to assess relationships between predictor variables and sealant presence. Results: More than a third (37.1%) of children and adolescents have at least one sealant present; 67.9% of children compared with 40.4% of adolescents. Racial/ethnic differences exist, with Non-Hispanic black youth having the lowest odds of having sealants. Sealant placement odds vary by presence of dental home; the magnitude of the odds varies by age group. Those with untreated decay have lower odds of having sealants than those who do not have untreated decay (child OR: 2.6, 95% CI: 1.83-3.72; adolescent OR: 3.9, 95% CI: 2.59-6.07). Conclusion: Disparities exist in odds of sealant prevalence across racial/ethnic groups, income levels, and dental disease and visit characteristics. Further research is necessary to understand the reasons for these differences and to inform future interventions.

This dissertation comprises three chapters. The first chapter motivates the use of a novel data set combining survey and administrative sources for the study of internal labor migration. By following a sample of individuals from the American Community Survey (ACS) across their employment outcomes over time according to the Longitudinal Employer-Household Dynamics (LEHD) database, I construct a measure of geographic labor mobility that allows me to exploit information about individuals prior to their move. This enables me to explore aspects of the migration decision, such as homeownership and employment status, in ways that have not previously been possible. In the second chapter, I use this data set to test the theory that falling home prices affect a worker’s propensity to take a job in a different metropolitan area from where he is currently located. Employing a within-CBSA and time estimation that compares homeowners to renters in their propensities to relocate for jobs, I find that homeowners who have experienced declines in the nominal value of their homes are approximately 12% less likely than average to take a new job in a location outside of the metropolitan area where they currently reside. This evidence is consistent with the hypothesis that housing lock-in has contributed to the decline in labor mobility of homeowners during the recent housing bust. The third chapter focuses on a sample of unemployed workers in the same data set, in order to compare the unemployment durations of those who find subsequent employment by relocating to a new metropolitan area, versus those who find employment in their original location. Using an instrumental variables strategy to address the endogeneity of the migration decision, I find that out-migrating for a new job significantly reduces the time to re-employment. These results stand in contrast to OLS estimates, which suggest that those who move have longer unemployment durations. This implies that those who migrate for jobs in the data may be particularly disadvantaged in their ability to find employment, and thus have strong short-term incentives to relocate.

Background and Objectives: The present study aimed to evaluate the effect of classical and azithromycin-containing triple therapy eradication regimen against H. pylori in children, and to determine the level of patients’ tolerance. Patients and Methods: This single clinical trial was performed in 2014 on 2 to 15 years old children. All children, in whom H. Pylori infection was confirmed through multiple biopsies of the stomach and required treatment, were enrolled in the study. H. Pylori-positive patients were treated alternately with two different drug regimens; Group OCA received clarithromycin 7.5 mg/kg/day every 12 hours for 10 days, amoxicillin 50 mg/kg/day every 12 hours for 10 days, and omeprazole 1 mg/kg/day every 12 hours for two weeks, and Group OAA received azithromycin 10 mg/kg/day once a day (before meal) for 6 days along with amoxicillin and omeprazole. Four to six weeks after completion of treatment, patients’ stool was tested for H. Pylori through the monoclonal method using the Helicobacter antigen quick kit. Results: There were no significant differences between the two groups regarding gender and age of patients. Based on ITT analysis, the therapeutic response in the OAA and OCA groups were 56.2% and 62.5%, respectively (P = 0.40). Drug adverse effects were 15.6% in the OCA and 3.1% in the OAA group (P = 0.19). Conclusions: The therapeutic response was seen in more than half of the patients treated with triple therapy of H. Pylori eradication regimen including azithromycin or clarithromycin, and there was no significant difference between the two treatment groups. Keywords: Treatment,

Background: Preterm labor, which defines as live-birth delivery before 37 weeks of gestation is a main determinant of neonatal morbidity and mortality around the world. Objective: The aim of this study was to determine the prevalence of preterm labor in Iran by a meta-analysis study, to be as a final measure for policy makers in this field. Materials and Methods: In this meta-analysis, the databases of Thomson database (Web of Knowledge), PubMed/Medline, Science Direct, Scopus, Google Scholar, Iranmedex, Scientific Information Database (SID), Magiran, and Medlib were searched for articles in English and Persian language published between 1995 and 2014. Among the studies with regard to the inclusion and exclusion criteria, 14 studies (out of 1370 publications) were selected. Data were analyzed by using Stata software version 11. The heterogeneity of reported prevalence among studies was evaluated by the Chi-square based Q test and I2 statistics. Results: The results of Chi-square based on Q test and I2 statistics revealed severe heterogeneity (Q=2505.12, p-value < 0.001 and I2= 99.5%) and consequently, the random effect model was used for the meta-analysis. Based on the random effect model, the overall estimated prevalence of preterm in Iran was 9.2% (95% CI: 7.6 – 10.7). Conclusion: Present study summarized the results of previous studies and provided a comprehensive view about the preterm delivery in Iran. In order to achieve a more desirable level and its reduction in the coming years, identifying affecting factor and interventional and preventive actions seem necessary.

BACKGROUND Eradication of bovine tuberculosis (bTB) through the application of test-and-cull programs is a declared goal of developed countries in which the disease is still endemic. Here, longitudinal data from more than 1,700 cattle herds tested during a 12 year-period in the eradication program in the region of Madrid, Spain, were analyzed to quantify the within-herd transmission coefficient (β) depending on the herd-type (beef/dairy/bullfighting). In addition, the probability to recover the officially bTB free (OTF) status in infected herds depending on the type of herd and the diagnostic strategy implemented was assessed using Cox proportional hazard models. RESULTS Overall, dairy herds showed higher β (median 4.7) than beef or bullfighting herds (2.3 and 2.2 respectively). Introduction of interferon-gamma (IFN-γ) as an ancillary test produced an apparent increase in the β coefficient regardless of production type, likely due to an increase in diagnostic sensitivity. Time to recover OTF status was also significantly lower in dairy herds, and length of bTB episodes was significantly reduced when the IFN-γ was implemented to manage the outbreak. CONCLUSIONS Our results suggest that bTB spreads more rapidly in dairy herds compared to other herd types, a likely cause being management and demographic-related factors. However, outbreaks in dairy herds can be controlled more rapidly than in typically extensive herd types. Finally, IFN-γ proved its usefulness to rapidly eradicate bTB at a herd-level.

In July 2012 the Iowa Legislative Council requested the Public Safety Advisory Board (PSAB) provide recommendations to the General Assembly relating to crimes against children. This request came in response to the high profile kidnapping of two girls and subsequent murder of one by Michael Klunder. The PSAB directed the Iowa Department of Human Rights, Division of Criminal and Juvenile Justice Planning (CJJP) to provide an analysis of child kidnapping and review of the effectiveness of Iowa kidnapping law.

L’objectif de cette étude consiste à déterminer si les conventions de l’Organisation internationale du travail (OIT) sont effectives en ce qui concerne l’éradication du travail des enfants en Mauritanie. Cette effectivité est appréciée en mesurant la réception juridique et la réception sociale en Mauritanie de la Convention 29 sur le travail forcé, de la Convention 138 sur l’âge minimum d’admission à l’emploi et de la Convention 182 sur les pires formes de travail des enfants. La réception juridique des conventions est mesurée par un examen de l’intégration de leurs dispositions dans le droit national mauritanien. La réception juridique comprend également l’appréciation du contrôle du respect des conventions en territoire mauritanien. La réception sociale fait référence, quant à elle, aux stratégies de mise en œuvre des conventions de l’OIT par le Gouvernement mauritanien à travers ses programmes et ses politiques. Notre analyse démontre que l’effectivité des Conventions 29, 138 et 182 de l’OIT en ce qui concerne l’éradication du travail des enfants en Mauritanie est, selon nous, partielle. Dans l’ensemble, la situation tend à s’améliorer et le Gouvernement mauritanien tente de respecter l’esprit de ces conventions et de leur faire écho dans le droit national. Toutefois, il n’existe pas beaucoup d’information sur l’impact des programmes mis en place pour éradiquer le travail des enfants.

El objetivo de este trabajo de grado es describir una disputa política por el significado del concepto de explotación en el marco de la redacción del “Protocolo para prevenir, reprimir y sancionar la trata de personas, especialmente mujeres y niños”, en el cual se define aquello que se entiende por trata de personas. La construcción de este concepto excluye e invisibiliza otros tipos de explotación comunes en el sistema de producción capitalista contemporánea que pertenecen al universo de violencia objetiva sistémica a través del énfasis en la explotación sexual que hace parte de la violencia subjetiva.

El objetivo de este documento es evaluar empíricamente el efecto que tiene una mayor o menor exposición del programa Proniño de la Fundación Telefónica en la cantidad de horas trabajadas de los niños, niñas y adolescentes. Tomando información desde el 2010 y hasta el 2012 se evalúa empíricamente el impacto en la duración a la exposición del tratamiento, eliminando el sesgo de selección por duración a través de la metodología de Propensity Score Matching. Los resultados muestran que la exposición al tratamiento sí logra reducir el número de horas trabajadas de los niños, niñas y adolescentes, alcanzando los niveles más altos de reducción cuando el tiempo de exposición al programa es más amplio; en este caso, tres años y particularmente para el grupo de edad comprendido entre los 12 a 14 años. Finalmente se evidencia que el programa es más efectivo en la reducción del número de horas trabajadas a la semana de los niños (hombres) que de las niñas (mujeres).

This chapter explores the spatialities of children's rights through a focus on how children's paid and unpaid work in Sub-Saharan Africa intersects with wider debates about child labor, child domestic work and young caregiving. Several tensions surround the universalist and individualistic nature of the rights discourse in the context of Sub-Saharan Africa and policymakers, practitioners, children and community members have emphasized children's responsibilities to their families and communities, as well as their rights. The limitations of ILO definitions of child labor and child domestic work and UNCRC concerns about 'hazardous' and 'harmful' work are highlighted through examining the situation of children providing unpaid domestic and care support to family members in the private space of their own or a relative's home. Differing perspectives towards young caregiving have been adopted to date by policymakers and practitioners in East Africa, ranging from a child labor/ child protection/ abolitionist approach, to a 'young carers'/ child-centered rights perspective. These differing perspectives influence the level and nature of support and resources that children involved in care work may be able to access. A contextual, multi-sectorial approach to young caregiving is needed that seeks to understand children's, family members' and community members' perceptions of what constitutes inappropriate caring responsibilities within particular cultural contexts and how these should best be alleviated.

The hypothesis that child labor impacts future income generation negatively, for it harms the formal acquisition of education, is widely accepted by the existing literature on this issue. However, some researchers agree that labor might be beneficial to teenagers once they can develop skills, acquire job experience, or even help them to afford their own education acquisition. Thus, the main goal of this study is to assess if there is an age which the negative impact of the early access to the labor market over income and the conclusion of high school, during the adulthood of Brazilian people, becomes positive. To do so, PNADs (Pesquisas Nacionais de Amostra de Domicílios), a National Census of Household Samples, issues 1992 to 2011, were utilized plus the employment of an econometric technique called pseudo-panel. For this analysis, generations of people born between 1982 and 1991 were observed from the ages 10 to 17 (child labor) and from the ages 20 to 29 (conclusion of high-school & income). The results show that starting at the age of 15, the negative effect of an early access to the labor market over income, between ages 20-29, becomes positive. As per high-school, it was observed that accessing the labor market before the age 15 diminishes the probability for an individual to conclude high school before the age 21. From this age on, labor does not have a negative impact anymore. The second goal of this study is to assess how much of the reduction of the child labor occurrence in Brazil for the past years is due to changes of economic and demographic characteristics of children and families. For these analyses PNADs - National Census of Household Samples, issues 2003 to 2011, were employed plus the methodology of decomposition that divides the variation of child labor into 2 components: (a) changing of the probability of children with the same characteristics (intragroup) to start working – access labor market & (b) changing of the distribution of characteristics (intergroups). The results show that the reduction of the child labor occurrence is due, mainly, to changes on the probabilities. In general terms, the occurrence of child labor took place, more significantly, among individuals of the ages 15 to 17 & household heads with less education. Besides the characteristics mentioned, the reduction between non-white individuals was also significant among individuals from 4-member families. The results show that the reduction of child labor took place, mainly, among children and teenagers from non-white and poor families.

This study aims to analyze the main effect of social programs and cash transfers on the labor supply of non-farm family members in poverty in rural areas of the Northeast. Among the specific objectives, we sought to investigate the effects of these programs and individual characteristics on the decision of participation and allocation of working hours of parents and children in non-agricultural activities. It was assumed, as a theoretical basis, the model of neoclassical labor supply as well as the principle that the decision of allocation of working hours, non-agricultural, is subject to the initial choice of the worker devote or not the non-agricultural employment . The hypothesis assumes that access to social programs and income transfer contributes to the dismay of rural workers, in poverty, in its decision to participate and offer hours of work in non-agricultural activities. To achieve this objective, we applied the models of Heckman (1979) and Double Hurdle, of Cragg (1971), consisting of associating the decision to participate in the labor market with the decision on the amount of hours allocated. The database used was the National Survey by Household Sampling (PNAD) of 2006. The results of the heads of households showed that transfers of income, although they may have some effect on labor supply rural nonfarm, the magnitude has to say that there may be some dependence on benefits. The estimates for the joint children of 10 to 15 years showed that the programs have negatively influenced participation in suggesting an increase in school participation, although for the allocation of working hours the results were not significant on the incidence of child labor
Infância na publicidade: um estudo sobre o trabalho de crianças na publicidade na cidade de Natal-RN

In Brazilian scene the child labor is a serious and relevant theme regarding issues that involve infancy, arousing the interest and concerning of public and private institutions, and organized civil society. Thus, the child labor is forbidden by the law, considered that the child worker have a distinguished life experience, especially because it is demanded them the early mature of some skill, generating implications on their physical, emotional, cognitive, social and psychological development. Observing those issues, we proposed think the child labor, over and above those considered the worst forms of one and more found in literature, what lead us to think the child labor in the advertising. In that way, this research aimed to investigate the insertion and development conditions of children who work in advertisements, and the possible effects on their lives in the face of that insertion. Participated of this study: three advertising agencies, six video producers companies, one child model s agent and five children with their respective families. Those children were four boys and two girls, with ages between seven and eleven years old, which one that have accomplished four or more advertisements, in Natal city, Rio Grande do Norte. Based on social-historical perspective, we used the semi-structured interview as methodological instrument, which underlays the comprehension of the data obtained in field research processes. This study revealed that when children are inserted in the advertising activities they are submitted to irregular and adverse work situations, what occurs in others productive activities mentioned in the literature. The data still exposed that the family is the main responsible to the children insertion in the advertisement work; moreover, there is an ambiguousness of wishes in relation of that activity. On the other hand, we realized that the legal content concerning to the child labor forbidding, carries contradictory aspects in face of the ECA s 149 article, that one, under some conditions, allows that a child take part in the activities of advertising context. This highlights the necessity of a rigorous evaluation of the children insertion in the advertising, in order to ensure the whole children and adolescents protection. This study put in evidence the complexity that involves the child labor in this context, mainly, due to the exposition and manipulation of the childish image on the media. Besides, we understand that the discussion on this subject should be stimulated and, therefore, enlarged this on over and above those considered the worst forms of child labor

Apesar das demandas e pressão da sociedade civil e dos próprios trabalhadores no sentido de fazer valer uma Política de Combate ao Trabalho Escravo que, existe desde a década de noventa do século passado no Brasil, todavia, identifica-se, em pleno século XXI, a existência de trabalho escravo, que coloca em evidência o problema da ausência de direitos humanos e sociais, assim como a fragilidade da construção da democracia e da cidadania na sociedade brasileira. Para compreender a política de combate ao trabalho escravo no Brasil, parte-se de um referencial crítico de análise que compreende a política pública como um desdobramento da relação Estado e sociedade na sociedade capitalista e, apoiado, em autores da tradição marxista, faz-se um percurso teórico-metodológico, que abrange desde a constituição do Estado oligárquico brasileiro, na época da escravidão colonial, até o surgimento de novas formas de trabalho escravo contemporâneo, com conteúdos semelhantes às antigas práticas sociais. Desta forma, a pesquisa apresenta uma análise das convergências e divergências, por parte dos executores, na operacionalização da Política de Combate ao Trabalho Escravo, materializada no II Plano Nacional para Erradicação do Trabalho Escravo, criado em 2008. Ressalta-se na operacionalização da política, ambiguidades e ambivalências da própria constituição do Estado capitalista brasileiro, que através das políticas públicas media interesses que são “inconciliáveis”. Essas ambiguidades e ambivalências do Estado se traduzem nas divergências e convergências dos executores da política, tal fato se apresenta como um limite ao funcionamento da política e contribui para a reincidência dos trabalhadores ao trabalho escravo contemporâneo. Ao mesmo tempo, reflete um Estado pactual que ao tomar a sua posição junto aos “donos do poder”, não dispõe de instrumental para que estes direitos sejam cumpridos através da política. Assim sendo, apesar dos avanços, a política de combate ao trabalho escravo no Brasil apresenta-se como uma exigência transnacional para se manter a política da “boa vizinhança” do que a efetivação da promoção da condição de cidadão para os que dela necessitam, não passa de um simulacro de civilidade que visa manter uma imagem de país democrático e disposto a resolver as causas dos direitos humanos quando na verdade mantém uma política interna cruel que reforça as condições de exploração desse tipo de trabalho.
