Notre thèse de doctorat a pour but d’évaluer les contraintes psychosociales au travail et les symptômes dépressifs majeurs chez les femmes enceintes. Plus spécifiquement, il est question d’identifier les facteurs associés aux symptômes dépressifs majeurs, à une tension psychologique au travail ou travail "tendu" ("high-strain" job), à un travail "tendu" avec un faible soutien social au travail ("Iso-strain"), et enfin d’évaluer l’association entre ces contraintes psychosociales au travail et les symptômes dépressifs majeurs chez les femmes enceintes au travail. Les données analysées sont issues de l’Étude Montréalaise sur la Prématurité, une étude de cohorte prospective menée entre mai 1999 et avril 2004, auprès de 5 337 femmes enceintes interviewées à 24-26 semaines de grossesse dans quatre hôpitaux de l’île de Montréal (Québec, Canada). L’échelle CES-D (Center for Epidemiological Studies Depression Scale) a été utilisée pour mesurer les symptômes dépressifs majeurs (score CES-D ≥23). L’échelle abrégée de Karasek a été utilisée pour mesurer les contraintes psychosociales au travail. La présente étude a conduit à la rédaction de quatre articles scientifiques qui seront soumis à des revues avec comité de pairs. Le premier article a permis de comparer la prévalence des symptômes dépressifs majeurs dans différents sous-groupes de femmes enceintes : femmes au foyer, femmes au travail, femmes en arrêt de travail, femmes aux études et de rechercher les facteurs de risque associés aux symptômes dépressifs majeurs pendant la grossesse. À 24-26 semaines de grossesse, la prévalence des symptômes dépressifs majeurs était de 11,9% (11,0-12,8%) pour l’ensemble des femmes enceintes à l’étude (N=5 337). Les femmes enceintes au travail avaient une proportion de symptômes dépressifs moins élevée [7,6% (6,6-8,7%); n=2 514] par rapport aux femmes enceintes au foyer qui avaient les prévalences les plus élevées [19,1% (16,5-21,8%); n=893], suivi des femmes enceintes en arrêt de travail [14,4% (12,7-16,1%); n=1 665] et des femmes enceintes aux études [14,3% (10,3-19,1%); n=265]. Les caractéristiques personnelles (non professionnelles) associées aux symptômes dépressifs majeurs étaient, après ajustement pour toutes les variables, le statut d’emploi, un faible niveau d’éducation, un faible soutien social en dehors du travail, le fait d’avoir vécu des événements stressants aigus, d’avoir manqué d’argent pour les besoins essentiels, les difficultés relationnelles avec son partenaire, les problèmes de santé chronique, le pays de naissance et le tabagisme. Le deuxième article avait pour objectif de décrire l’exposition aux contraintes psychosociales au travail et d’identifier les facteurs qui y sont associés chez les femmes enceintes de la région de Montréal, au Québec (N=3 765). Au total, 24,4% des travailleuses enceintes se trouvaient dans la catégorie travail "tendu" ("high-strain" job) et 69,1% d’entre elles avaient eu un faible soutien social au travail ("Iso-strain"). Les facteurs de risque associés à un travail "tendu" étaient : un faible soutien social au travail, certains secteurs d’activité et niveaux de compétences, le fait de travailler plus de 35 heures par semaine, les horaires irréguliers, la posture de travail, le port de charges lourdes, le jeune âge des mères, une immigration ≥ 5 ans, un bas niveau d’éducation, la monoparentalité et un revenu annuel du ménage <50 000$. Le troisième article a évalué l’association entre les contraintes psychosociales au travail et les symptômes dépressifs majeurs chez les femmes enceintes au travail (N=3 765). Dans les analyses bivariées et multivariées, les femmes enceintes qui avaient un "high-strain job" ou un "Iso-strain" présentaient davantage de symptômes dépressifs majeurs que les autres sous-groupes. Les contraintes psychosociales au travail étaient associées aux symptômes dépressifs majeurs lorsqu’on prenait en compte les autres facteurs organisationnels et les facteurs personnels auxquels elles étaient confrontées à l’extérieur de leur milieu de travail. Notre étude confirme les évidences accumulées en référence aux modèles théoriques "demande-contrôle" et "demande-contrôle-soutien" de Karasek et Theorell. L’impact de ce dernier et le rôle crucial du soutien social au travail ont été mis en évidence chez les femmes enceintes au travail. Cependant, l’effet "buffer" du modèle "demande-contrôle-soutien" n’a pas été mis en évidence. Le quatrième article a permis d’évaluer l’exposition aux contraintes psychosociales au travail chez les femmes enceintes au travail et en arrêt de travail pour retrait préventif et de mesurer l’association entre les contraintes psychosociales au travail et les symptômes dépressifs majeurs en fonction du moment du retrait préventif (N=3 043). À 24-26 semaines de grossesse, les femmes enceintes en retrait préventif du travail (31,4%) avaient été plus exposées à un "high-strain job" (31,0% vs 21,1%) et à un "Iso-strain" (21,0% vs 14,2%) que celles qui continuaient de travailler (p<0,0001); et elles avaient des proportions plus élevées de symptômes dépressifs majeurs. Après ajustement pour les facteurs de risque personnels et professionnels, "l’Iso-strain" restait significativement associé aux symptômes dépressifs majeurs chez les femmes qui continuaient de travailler tout comme chez celles qui ont cessé de travailler, et cela quel que soit leur durée d’activité avant le retrait préventif du travail (4 à 12 semaines/ 13 à 20 semaines/ ≥ 21 semaines). Les contraintes psychosociales au travail représentent un important facteur de risque pour la santé mentale des travailleuses enceintes. Malgré l’application du programme "pour une maternité sans danger" il s’avère nécessaire de mettre en place dans les milieux de travail, des mesures de prévention, de dépistage et d’intervention afin de réduire la prévalence des symptômes dépressifs prénataux et l’exposition aux contraintes psychosociales au travail pour prévenir les complications maternelles et néonatales. D’autant plus que, la dépression prénatale est le principal facteur de risque de dépression postpartum, de même que les enfants nés de mères souffrant de dépression sont plus à risque de prématurité et de petit poids de naissance.

Urolithiasis is identified to be a major urological disorder affecting people all over the world irrespective of their age, sex and race. Urinary stone samples resected from the urinary bladders of two patients belonging to tropical region, Kollam District of Kerala State, India are investigated by using XRD,SEM, EDAX, TGA, DSC and FTIR to understand its chemical structure. Uric acid shows exothermic peak around 432°C is due to the decomposition with the evolution of CO and cracking of the remaining products. Results of analytical studies reveal that samples under investigation consist mainly in uric acid and hydrated uric acid. Hydrogen bonding exists in hydrated uric acid samples

Generalidades. Las encefalopatías espongiformes transmisibles son enfermedades neurodegenerativas ocasionadas por la acumulación anormal de una variante mal plegada de la proteína priónica, lo cual induce la formación de conglomerados proteicos resistentes a la degradación. Además, son responsables de la disfunción sináptica, daño neuronal y de la sintomatología clásica acompañante. Esta proteína de membrana es codificada por el exón 2 del gen PRNP, ubicado en el brazo corto del cromosoma 20 y parece estar involucrada en la trasmisión sináptica, la transducción de señales, la actividad antioxidante de la superoxidodismutasa, neuroplasticidad y sobrevida celular. Un polimorfismo en el codón 129 se asocia con una susceptibilidad diferencial a la enfermedad Creutzfeldt-Jakob esporádica. Objetivo. Estudio clínico, patológico y molecular de un caso de una mujer de 58 años con diagnóstico de enfermedad de Creutzfeldt- Jakob esporádica. Métodos y resultados. Se presenta el caso de una mujer en quien aparece un trastorno depresivo del afecto con demencia progresiva y sintomatología general. Al final de la enfermedad, el cuadro progresó a un déficit neurológico focalizado en el área visual. La RMN mostró hiperintensidades inespecíficas córtico-subcorticales en el núcleo estriado; en el EEG se encontró pérdida de ritmos de fondo, patrón de descargas periódicas generalizadas y complejos trifásicos; en la biopsia cerebral postmorten se evidenció pérdida severa de la población neuronal en todas las capas, vacuolas en el neuropil, en el soma neuronal y en la glía. El análisis de secuencia del gen PRNP, a partir de extracción de DNA de sangre periférica, identificó homocigosis para metionina en el codón 129. La paciente fallece a los 3 meses del inicio de la sintomatología. Conclusión. Por epidemiología, curso clínico y exámenes paraclínicos se confirma el diagnóstico de enfermedad de Creutzfeldt- Jakob esporádica.La determinación del genotipo para los polimorfismos de riesgo se convierte en una herramienta útil para complementar por medios moleculares el diagnóstico y para profundizar la comprensión de la fisiopatología de la enfermedad de Creutzfeldt-Jakob, tanto para formas esporádicas como para la nueva variante.

La prevalencia de la Pérdida de Cabello de Patrón Femenino (FPHL) aumenta con la edad y no ha sido documentada en la población latinoamericana. Este estudio describe la prevalencia y variables asociadas a la FPHL en una consulta Dermatológica. Métodos: Estudio descriptivo en 99 mujeres mayores de 18 años asistentes a consulta dermatológica con diagnóstico de FPHL. Se realizó una prueba piloto en 22 pacientes, y posteriormente se analizaron los datos de las 99 pacientes acorde a la clasificación de Ludwig, edad, comorbilidades y alteraciones del estado del ánimo. Resultados: La prevalencia estimada de FPHL fue de 20.84%. 43,4% de las pacientes estudiadas pertenecen a edades entre 31 a 49 años; el 50,5% fueron diagnosticadas con FPHL grado II. El 62,6% tienen algún antecedente médico de importancia. El 54,4% manifestaron alguna alteración del estado del ánimo. La totalidad de las pacientes con Grado III tuvieron algún antecedente médico de importancia (ORP= 1,14 IC95% (1; 1,26) (p < 0,05)). De las pacientes con Grado II, el 61,1% presentaron alguna alteración secundaria del estado del ánimo (ORP=2,58 IC95% (1,14; 5,84) (p<0,05)). Conclusiones: Debido al aumento de la FPHL en la población colombiana la cual conlleva a múltiples gastos del sistema salud, se hacen necesarios más estudios con el fin de caracterizar la enfermedad. Se recomienda un estudio con un mayor marco muestral con el fin de caracterizar la prevalencia y las variables relacionadas con la FPHL en toda la población Colombiana.

Background: Maternal postnatal depression (PND) has been associated with adverse outcomes in young children, but an association with longer-term psychiatric disorder has not been demonstrated. We present the preliminary findings of a 13-year longitudinal study. Methods: In the course of a prospective longitudinal study, we examined DSM-IV Axis I disorders in 13-year-old adolescents who had (n=53) or had not (n=41) been exposed to maternal PND. We also detailed the occurrence of depression in mothers throughout the 13-year follow-up period. Results: Maternal PND was associated with higher rates of affective disorders in adolescent offspring. However, mothers who developed PND were also substantially more likely than those who did not to experience depression subsequently, a fact that contributed to the development of depressive disorder in offspring. Maternal PND was associated with increased risk for depression in adolescent offspring only if there had also been later episodes of maternal depression. In contrast, anxiety disorders in offspring were elevated in the maternal PND group regardless of the occurrence of subsequent maternal depression. Limitations: Due to the modest sample size and consequently limited power, findings must be regarded as preliminary. Conclusions: The particular association between early maternal depression and anxiety disorders in offspring was consistent with theories that emphasise the primacy of early environmental exposures. This position was not supported with respect to offspring depressive disorder, where overall duration of maternal depression was a significant factor. PND was associated with recurrent episodes of depression in the majority of cases, underlining the need for monitoring of this population beyond the postnatal period. (c) 2006 Elsevier B.V. Objective To assess the efficacy of an intervention designed to improve the mother-infant relationship and security of infant attachment in a South African peri-urban settlement with marked adverse socioeconomic circumstances. Design Randomised controlled trial. Setting Khayelitsha, a peri-urban settlement in South Africa. Participants 449 pregnant women. Interventions The intervention was delivered from late pregnancy and for six months postpartum. Women were visited in their homes by previously untrained lay community workers who provided support and guidance in parenting. The purpose of the intervention was to promote sensitive and responsive parenting and secure infant attachment to the mother. Women in the control group received no therapeutic input from the research team. Main outcome measures Primary outcomes: quality of mother-infant interactions at six and 12 months postpartum; infant attachment security at 18 months. Secondary outcome: maternal depression at six and 12 months. Results The intervention was associated with significant benefit to the mother-infant relationship. At both six and 12 months, compared with control mothers, mothers in the intervention group were significantly more sensitive (6 months: mean difference=0.77 (SD 0.37), t=2.10, P<0.05, d=0.24; 12 months: mean difference=0.42 (0.18), t=-2.04, P<0.05, d=0.26) and less intrusive (6 months: mean difference=0.68 (0.36), t=2.28, P<0.05, d=0.26; 12 months: mean difference=-1.76 (0.86), t=2.28, P<0.05, d=0.24) in their interactions with their infants. The intervention was also associated with a higher rate of secure infant attachments at 18 months (116/156 (74%) v 102/162 (63%); Wald=4.74, odds ratio=1.70, P<0.05). Although the prevalence of maternal depressive disorder was not significantly reduced, the intervention had a benefit in terms of maternal depressed mood at six months (z=2.05, P=0.04) on the Edinburgh postnatal depression scale). Conclusions The intervention, delivered by local lay women, had a significant positive impact on the quality of the mother-infant relationship and on security of infant attachment, factors known to predict favourable child development. If these effects persist, and if they are replicated, this intervention holds considerable promise for use in the developing world. Trial registration Current Controlled Trials ISRCTN25664149.

The present research aimed to comprehensively explore psychopathology in Williams syndrome (WS) across the lifespan and evaluate the relationship between psychopathology and age category (child or adult), gender and cognitive ability. The parents of 50 participants with WS, aged 6-50 years, were interviewed using the Schedule for Affective Disorders and Schizophrenia for School-Age Children (K-SADS-PL). The prevalence of a wide range of Axis I DSM-IV disorders was assessed. In addition to high rates of anxiety and Attention Deficit Hyperactivity Disorder (ADHD) (38% and 20% respectively), 14% of our sample met criteria for a depressive disorder and 42% of participants were not experiencing any significant psychopathological difficulties. There was some evidence for different patterns of psychopathology between children and adults with WS and between males and females. These relationships were largely in keeping with those found in the typically developing population, thus supporting the validity of applying theory and treatment approaches for psychopathology in the typically developing population to WS.

Background Major depressive disorders (MDD) are a debilitating and pervasive group of mental illnesses afflicting many millions of people resulting in the loss of 110 million working days and more than 2,500 suicides per annum. Adolescent MDD patients attending NHS clinics show high rates of recurrence into adult life. A meta-analysis of recent research shows that psychological treatments are not as efficacious as previously thought. Modest treatment outcomes of approximately 65% of cases responding suggest that aetiological and clinical heterogeneity may hamper the better use of existing therapies and discovery of more effective treatments. Information with respect to optimal treatment choice for individuals is lacking, with no validated biomarkers to aid therapeutic decision-making. Methods/Design Magnetic resonance-Improving Mood with Psychoanalytic and Cognitive Therapies, the MR-IMPACT study, plans to identify brain regions implicated in the pathophysiology of depressions and examine whether there are specific behavioural or neural markers predicting remission and/or subsequent relapse in a subsample of depressed adolescents recruited to the IMPACT randomised controlled trial (Registration # ISRCTN83033550). Discussion MR-IMPACT is an investigative biomarker component of the IMPACT pragmatic effectiveness trial. The aim of this investigation is to identify neural markers and regional indicators of the pathophysiology of and treatment response for MDD in adolescents. We anticipate that these data may enable more targeted treatment delivery by identifying those patients who may be optimal candidates for therapeutic response.

To evaluate sleep disorder complaints in outpatients with depressive disorder from a general hospital. Methods: An observational, cross-sectional study was carried out with a study sample composed of 70 patients (44 women and 26 men) with diagnosis of depressive disorder, according to the DSM-IV criteria. The patients were interviewed and evaluated by the Identification Questionnaire, the Sleep Habits Questionnaire and the Beck Depression Inventory (BDI). Results: In this study, 50 (71.3%) patients had recurrence of sleep disorder complaints. Mean BDI score was 35.83+8.85, with significant differences between patients with (38.50+8.70) and without (29.60+7.80) recurrence (p<0.05) and among patients with 1, 2, 3 and >3 episodes (p<0.05). In this study, 49 (70%) patients had insomnia and 21 (30%) had subjective excessive sleepiness. Significant differences were observed between the mean duration in months of the sleep disorders (7.16+2.10) and the depressive disorder (6.12+1.90) (p<0.05). Discussion: In the study sample, recurrence of sleep disorder complaints was high and significantly associated with severe depression. Insomnia was prevalent and the mean duration of sleep disorders was higher in relation to depressive disorder

Iron (Fe) is essential for chlorophyll formation and plant growth. Irondeficiency chlorosis is a major nutritional disorder in several fruit trees cultivated in calcareous and alkaline soils, reducing fruit yield and quality and causing heavy economic losses. Since chelated Fe, the most widespread fertilizers used for preventing or curing Fe deficiency, pose risks of environmental pollution, the development of sustainable agronomic alternatives represents a priority for the fruit industry. In this work, we investigated the effectiveness of a bovine blood-derived product (BB; 0,125% Fe) for preventing Fe-deficiency in grapevine plants. During the vegetative season 2011 potted plants of five graft combinations: Sangiovese/S4O, Cabernet Sauvignon/S4O and Cabernet Sauvignon/140 Ruggeri, 140 Ruggeri/Cabernet Sauvignon, Vitis riparia/Cabernet Sauvignon were grown on calcareous soil. Soil treatments included: 1) Control; 2) Fe-EDDHA (Fe 6%); 3) Bovine-Blood (5 g/L); 4) Bovine-Blood (20 g/L). With the exception of Cabernet Sauvignon/S4O plants, Fe-EDDHA increased SPAD units (leaf chlorophyll content). Bovine-blood at low concentrations had similar or higher SPAD units than Fe-EDDHA. Increasing concentration resulted in further increases in SPAD units only in some graft combinations. Data highlight the efficiency of Fe blood-compound in the prevention of grapevine Fe-deficiency over one growing season.

Fármacos antidepressivos são largamente utilizados no tratamento sintomático do transtorno depressivo. Inúmeras pesquisas atuais sobre a depressão vêm contribuindo para o avanço da terapia farmacológica e para o surgimento de novos fármacos antidepressivos. Diretrizes atuais para testes de genotoxicidade de novos medicamentos sugerem a importante utilidade de ensaios que detectem danos ao DNA, ou seja, testes para avaliar a indução de quebras no DNA. Entretanto, o número escasso de dados sobre a genotoxicidade de fármacos faz com que seja reduzido o número de fármacos que realmente podem ser usados em segurança. Portanto, é de extrema importância estudos sobre a avaliação genotoxicológica de fármacos, principalmente, drogas utilizadas por um longo período de tempo como é o caso dos antidepressivos. A duloxetina é um antidepressivo novo, pertencente à classe dos inibidores seletivos da recaptação de serotonina e noradrenalina (ISRSN), utilizada no tratamento sintomático da depressão. Apesar da existência de trabalhos demonstrando que alguns fármacos antidepressivos são genotóxicos, não existe até hoje nenhum estudo sobre a possível genotoxicidade da duloxetina em células de origem humana. Assim, o presente estudo tem como objetivo explorar o possível potencial genotóxico in vitro da duloxetina em culturas primárias de linfócitos humanos através das técnicas de detecção de aberrações cromossômicas e micronúcleos. Culturas primárias de linfócitos sanguíneos de voluntários sadios foram expostas a diferentes concentrações de duloxetina (10-150 ng/ml) e ciclofosfamida (6 μg/ml) como controle positivo. Aberrações cromossômicas estruturais, índice mitótico, índice de divisão nuclear, índice de binucleação, número de células com um, dois, três e quatro micronúcleos e o número de células com pontes nucleoplasmáticas foram avaliadas. Todos os índices das culturas incubadas com duloxetina foram significativamente menores que aqueles dos grupos controles, indicando um certo grau de citotoxicidade da droga. Entretanto, só as concentrações de 100 e 150 ng/ml provocaram o aumento significativo da presença de aberrações cromossômicas e micronúcleos. Considerando que essas concentrações ficam perto do limite superior da faixa terapêutica da droga usada em humanos, nossos resultados alertam já sobre a necessidade de aprofundar no conhecimento da genotoxicidade humana da duloxetina.

Purpose of reviewTo critically discuss the neuropsychiatric symptoms in the prodromal stages of dementia in order to improve the early clinical diagnosis of cognitive and functional deterioration.Recent findingsCurrent criteria for cognitive syndrome, including Alzheimer's disease, comprise the neuropsychiatric symptoms in addition to cognitive and functional decline. Although there is growing evidence that neuropsychiatric symptoms may precede the prodromal stages of dementia, these manifestations have received less attention than traditional clinical hallmarks such as cognitive and functional deterioration. Depression, anxiety, apathy, irritability, agitation, sleep disorders, among other symptoms, have been hypothesized to represent a prodromal stage of dementia or, at least, they increase the risk for conversion from minor neurocognitive disorder to major neurocognitive disorder. Longitudinal investigations have provided increased evidence of progression to dementia in individuals with minor neurocognitive disorder when neuropsychiatric symptoms also were present.SummaryAlthough neuropsychiatric symptoms are strongly associated with a higher risk of cognitive and functional deterioration, frequently the clinician does not acknowledge these conditions as increasing the risk of dementia. When the clinician accurately diagnoses neuropsychiatric symptoms in the prodromal stage of dementia, he could early establish appropriate treatment and, may be, delay the beginning of clinical and functional deterioration.

Objectives: Analyze the association between DC and DP on institutionalized elders (IDI) and non-institutionalized elders (IDNI) and verify the protective function (regarding DP and DC) of RC activities. Design and Method: For the analysis were selected 48 elders, from both genders, from where 24 were institutionalized and 24 were not. The subjects were distributed in four subgroups: 1) IDI participating RC (n=12); 2) non participant IDI on RC (n=12); 3) IDNI participating UNATI (“Open College for the Elderly”) and attendees of Memory Workshop (MW) (n=12) and; 4) IDNI participating UNATI, not attending MW (n=12). The data were gathered from the instruments: Mini Mental State Exam (MEEM) to determine the degree of DC and Beck Depression Inventory (BDI) to verify the symptomatology of DP. Results: From the general sample, 8,3% of the subjects presented indicatives of DC and 52,1% traces of DP. Lesser educated elders showed more incidence of DC and DP. There was no meaningful related to the DC prevalence and DP related to gender. Among the IDI there were more incidence of DC and DP than the IDNI. There were no meaningful differences in terms of scores acquired by MEEM and BDI for the subjects participating and not participating RC activities or MW. Conclusion: Therefore, it is necessary the development and application of curative and preventive strategies for depressive disorder. Special attention must be given for INI, more vulnerable to DP and DC.

The intervertebral disc disease is considered a major neurological disorder involving the spinal cord, being a disease commonly seen in dogs in clinical practice of small animals. The pathophysiology of most discopathies have not been fully elucidated, but it is known that the symptoms observed will depend on the severity of spinal cord compression and location of spinal injury, ranging from localized pain to complete paralysis. Through a detailed clinical examination and laboratory tests, we choose the most appropriate treatment for each patient, which can be conservative, through the use of anti-inflammatories, analgesics and acupuncture or surgical, through decompression techniques. Acupuncture, therapeutic method of ancient Chinese medicine, uses the stimulation of specific points on the body in order to balance the body's physiological. The action mechanism of acupuncture in discopathies has not been fully elucidated, but its analgesic effects, anti-inflammatory and restoration of sensory and motor function are known, showing results as good as surgical therapy
