This study deals with the ethical and human aspects present in the teaching-learning process within the dentists' formation. It arises from the growing need for professionals involved with the quality of the services they provide for the population in health care centers. In this research a qualitative approach was used and data was obtained by means of focal groups, interviews and participative observation. The sample consisted of 28 dentistry students and 33 patients attended at the dentistry course. According to the results, it was shown that the main problems are the excess of authority in the teacher-student-patient relationship and the dissociation of the body-mind-spirit as seen in the biomedical model health practice. These findings show the future professionals' insufficient abilities for developing a satisfactory relationship with their patients and the need of considering these aspects during their formation.

This research aimed to analyse the effect of different territorial divisions in the random fluctuation of socio-economic indicators related to social determinants of health. This is an ecological study resulting from a combination of statistical methods including individuated and aggregate data analysis, using five databases derived from the database of the Brazilian demographic census 2010: overall results of the sample by weighting area. These data were grouped into the following levels: households; weighting areas; cities; Immediate Urban Associated Regions and Intermediate Urban Associated Regions. A theoretical model related to social determinants of health was used, with the dependent variable Household with death and as independent variables: Black race; Income; Childcare and school no attendance; Illiteracy; and Low schooling. The data was analysed in a model related to social determinants of health, using Poisson regression in individual basis, multilevel Poisson regression and multiple linear regression in light of the theoretical framework of the area. It was identified a greater proportion of households with deaths among those with at least one black resident, lower-income, illiterate, who do not attend or attended school or day-care and less educated. The analysis of the adjusted model showed that most adjusted prevalence ratio was related to Income, where there is a risk value of 1.33 for households with at least one resident with lower average personal income to R$ 655,00 (Brazilian current). The multilevel analysis demonstrated that there was a context effect when the variables were subjected to the effects of areas, insofar as the random effects were significant for all models and with different prevalence rates being higher in the areas with smaller dimensions - Weighting areas with coefficient of 0.035 and Cities with coefficient of 0.024. The ecological analyses have shown that the variable Income and Low schooling presented explanatory potential for the outcome on all models, having income greater power to determine the household deaths, especially in models related to Immediate Urban Associated Regions with a standardized coefficient of -0.616 and regions intermediate urban associated regions with a standardized coefficient of -0.618. It was concluded that there was a context effect on the random fluctuation of the socioeconomic indicators related to social determinants of health. This effect was explained by the characteristics of territorial divisions and individuals who live or work there. Context effects were better identified in the areas with smaller dimensions, which are more favourable to explain phenomena related to social determinants of health, especially in studies of societies marked by social inequalities. The composition effects were better identified in the Regions of Urban Articulation, shaped through mechanisms similar to the phenomenon under study.

Clostridium difficile causes a broad range of diseases in humans, from mild colitis to pseudomembranous colitis and disease refractory to treatment, fulminant and fatal. It is an infection whose frequency, seriousness and related morbidity and mortality have increased in recent years [1-4]. Nowadays it is regarded as an emerging public health problem, and prevention and monitoring are particularly recommended. In recent years, different authors have described a change in its epidemiology, which affects not only the populations traditionally involved, but also children and patients from the community [2, 5]. Moreover, the Spanish situation has proven to be different, in terms of the ribotypes present, to other countries in Europe, Canada and the USA. Thus, the performance of an in-depth study in this type of patients in Spain, as well as the source of the acquisition of Clostridium difficile infection (CDI), is of major relevance. The main predisposing factor to acquiring CDI is the use of antibiotics in the previous 8 weeks (90% cases in some series), even with a single prophylactic dose. Other risk factors are a previous stay in health-care centers, particularly hospitals, being old and immunodepression, including transplantations and HIV [6]. Background: Because most developing countries lack sufficient resources and infrastructure to conduct population-based studies on childhood blindness, it can be difficult to obtain epidemiologically reliable data available for planning public health strategies to effectively address the major determinants of childhood blindness. The major etiologies of blindness can differ regionally and intra-regionally. The objective of this retrospective study was to determine (1) the major causes of childhood blindness (BL) and severe visual impairment (SVI) in students who attend Wa Methodist School for the Blind in Upper West Region, North Ghana, and (2) any potential temporal trends in the causes of blindness for this region.

Methods: In this retrospective study, demographic data and clinical information from an eye screening at Wa Methodist School for the Blind were coded according to the World Health Organization/Prevention of Blindness standardized reporting methodology. Causes of BL and SVI were categorized anatomically and etiologically. We determined the major causes of BL/SVI over time using information provided about the age at onset of visual loss for each student.

Results: The major anatomical causes of BL/SVI among the 190 students screened were corneal opacity and phthisis bulbi (n=28, 15%), optic atrophy (n=23, 13%), glaucoma (n=18, 9%), microphthalmos (n=18, 9%), and cataract (n=18, 9%). Within the first year of life, students became blind mainly due to whole globe causes (n=23, 26%), cataract (n=15, 17%), and optic atrophy (n=11, 13%). Those who became blind after age one year had whole globe causes (n=26, 26%), corneal opacity (n=24, 24%), and optic atrophy (n=13, 13%).

Conclusion: At the Wa Methodist School for the Blind, the major anatomical causes of BL/SVI were corneal opacity and phthisis bulbi. About half of all students became blind within the first year of life, and were disproportionately affected by cataract and retinal causes in comparison to the other students who became blind after age one year. While research in blind schools has a number of implicit disadvantages and limitations, considering the temporal trends and other epidemiological factors of blindness may increase the usefulness and/or implications of the data that come from blind school studies in order to improve screening methods for newborns in hospitals and primary care centers, and to help tailor preventative and treatment programs to reduce avoidable childhood blindness in neonates and schoolchildren.

Conscientes de la importancia de la implicación de la familia en la realidad de los centros, esta herramienta se ha diseñado para ser un canal seguro, ágil y eficiente. La familia de cada residente recibe en su smartphone notificaciones en tiempo real de los eventos y las anotaciones que tienen lugar en el centro. El equipo de cuidadores dispondrá de una app para tabletas Android (también compatible con teléfonos) con la que podrá tomar los registros que alimentan al sistema y notificar a los familiares.

Este estudo integra-se no coorte I de um Projecto de Investigação baseado na população, denominado “ Trajectórias do Envelhecimento de Idosos em Resposta Social: Estudos dos factores Preditivos do Envelhecimento Saudável e da Demência. O principal objectivo deste estudo é verificar e descrever se existe relação entre a satisfação com a vida e os recursos sociais reais e os percepcionados pelos idosos institucionalizados do concelho de Coimbra. Como objectivos específicos propusemonos avaliar: a relação entre os recursos sociais reais e os percepcionados pelos idosos; a relação entre os recursos sociais reais e os percepcionados pelos informantes; conhecer a avaliação dos recursos sociais dos idosos avaliados pelo OARS; conhecer a satisfação com a vida dos idosos avaliados pelo SWLS; e finalmente avaliar a correlação entre o SWLS e o OARS. Foram avaliados 45 idosos com idades compreendidas entre os 65 e os 95 anos que se encontram em resposta social (lar e centro de dia). Na colheita de dados utilizámos o SWLS, (Satisfaction with Life Scale) e o OARS (Olders Americans Research and Services). A população idosa do nosso estudo revela na sua maioria estar insatisfeita com a vida e com limitações ao nível da saúde física, mental e no desempenho das actividades diárias. Verificámos no entanto, que não existe relação entre a satisfação com a vida e os recursos sociais, económicos, mentais e físicos, sendo que estes se correlacionam entre si. Este estudo possibilitou que se fizesse uma análise crítica acerca da institucionalização dos idosos, com vista a uma maior humanização neste grupo etário. / This study is part of the cohort I of a research project based on the population, called "Trajectories of Aging Social Response in the Elderly: Study of the Predictive Factors of Healthy Aging and Dementia. The main objective is to study and describe whether a relationship exists between life satisfaction and social resources are real and perceived by the institutionalized elderly in the municipality of Coimbra. Specific objectives we set out to assess: the relationship between social resources and the actual perceived by the elderly, the relationship between social resources and the actual perceived by informants, to know the assessment of social resources of the elderly evaluated by OARS; know the satisfaction life of the elderly evaluated by the SWLS, and finally evaluate the correlation between the SWLS and the OARS we evaluated 45 elderly aged 65 and 95 years who are in social response (home and day care). In data collection used in the SWLS (Satisfaction with Life Scale) and OARS (Olders Americans Research and Services). The elderly population of our study reveals mostly being dissatisfied with life and with limitations in terms of physical, mental and performance of daily activities. We found however, that there is no relationship between life satisfaction and social resources, economic, mental and physical and these correlate with each other. This study made it possible to do a critical analysis of the institutionalization of the elderly with a view to further humanize this age group.

Abstract The developmental changes during adolescence may affect subsequent risk for diseases and health-related behaviors. Traditionally, professionals assume that knowledge is sufficient for behavioral changes; however adolescents continue to engage in unhealthy behaviors despite clearly knowing what they should do and how to change. "What is lacking is the motivation to apply that knowledge". Motivational Interview (MI) may be taken as an essential tool in the provision of nursing care to adolescents, being itself a workspace with possible therapeutic effects. Objectives The objective of this scoping is to examine and map the use of MI by nurses in their clinical practice with adolescents to promote health behaviors. The review will focus on knowing what is the current extent of the use of nurse-led MI; which adolescent populations were included; in which contexts nurses use MI; which MI techniques/strategies have been used and what outcomes on health behaviors promotion have been reported. Methodology This scoping review will be informed by JBI methodology. The population of this study is adolescents aged 10 to 19 years participating in nurse-led MI. The concept of MI include MI done by personal or telephone call, with any number of sessions, brief interventions and other motivational interventions grounded but not limited to the principles described by Miller & Rollnick (2008). All geographical and all clinical practice contexts where nurses' undertake MI with adolescents such as hospitals, primary health care, health care centers, community or schools will be contemplated. English, Spanish and Portuguese published studies will be considered for inclusion. Results An initial limited search of MEDLINE and CINAHL was undertaken followed by analysis of the text words contained in the title and abstract, and of the index terms used to describe the concepts, synonyms (with truncations), MeSH Terms and Cinhal headings of this study. It was identified 5 synonyms for "Adolescents", 7 for "MI" and 2 for "nurse". A first search using the all 14 identified keywords and index terms was made at Medline (Title/Abstract) and brought up 125 articles. Other 16 databases referenced at the protocol will be searched to identify additional studies. Articles identified from the final search will be assessed for relevance to the review, based on information provided in the title and abstract. The full article will be retrieved for all studies that meet the inclusion criteria of the review. It is expected that findings from this Scoping Review provide needed information to nurses related to the use of MI to promote health behaviors in adolescents. Conclusions There is little knowledge of what works for whom (which adolescent subpopulation) under what circumstances (in which setting, for what problem) in relation to nurse-led MI. There is a need for scoping or mapping the nurse-led MI with adolescents to identify evidence gaps and to inform opportunities for future development in nursing practice. Moreover, information regarding implemented and evaluated interventions, techniques used, contexts of application and adolescents groups is dispersed in the literature which impedes the formulation of questions about the outcomes and effectiveness of those interventions. The practical implication of this mapping will be clarifying all these aspects.

Background: The transport of children in ground ambulances is a rarely studied topic worldwide. The ambulance vehicle is a unique and complex environment with particular challenges for the safe, correct and effective transportation of patients. Unlike the well developed and readily available guidelines on the safe transportation of a child in motor vehicles, there is a lack on consistent specifications for transporting children in ambulances. Nurses are called daily to transfer children to hospitals or other care centers, so safe transport practices should be a major concern. Purpose: to know which are the safety precautions and specific measures used in the transport of children in ground ambulances by nurses and firefighters and to identify what knowledge these professionals had about safe modes of children transportation in ground ambulances. Methods: In this context, an exploratory - descriptive study and quantitative analysis was conducted. A questionnaire was completed by 135 nurses and firefighters / ambulance crew based on 4 possible children transport scenarios proposed by the NHTSA (National Highway Traffic Safety Administration) and covered 5 different children´s age groups (new born children, 1 to 12 months; 1 to 3 years old; 4 to 7 years old and 8 to 12 years old). Results: The main results showed a variety of safety measures used by the professionals and a significant difference between their actual mode of transportation and the mode they consider to be the ideal considering security goals. In addition, findings showed that achieved scores related to what ambulance crews do in the considered scenarios reflect mostly satisfactory levels of transportation rather than optimum levels of safety, according to NHTSA recommendations. Variables as gender, educational qualifications, occupational group and local where professionals work seem to influence the transport options. Female professionals and nurses from pediatric units appear to do a safer transportation of children in ground ambulances than other professionals. Conclusion: Several professionals refereed unawareness of the safest transportation options for children in ambulances and did not to know the existence of specific recommendations for this type of transportation. The dispersion of the results suggests the need for investment in professional training and further regulation for this type of transportation.

O presente Relatório Final de Estágio foi desenvolvido no âmbito da unidade curricular de Prática de Ensino Supervisionada III (PES III) do 2.º Ciclo de Estudos, do Mestrado em Educação Pré-Escolar e Ensino do 1.º Ciclo do Ensino Básico e inclui duas partes distintas. A primeira parte diz respeito às práticas de estágio desenvolvidas no decorrer da unidade curricular Prática de Ensino Supervisionada (PES). Aí, proceder-se-á a uma reflexão crítica acerca do trabalho desenvolvido nos referidos contextos e das experiências de aprendizagem que daí resultaram. A segunda parte do relatório final de estágio está relacionada com a execução de um trabalho de investigação centrado nos seguintes objetivos: compreender a importância da frequência da creche no desenvolvimento e aprendizagem das crianças, identificar a opinião dos profissionais sobre a creche e as suas atividades e conhecer a relação pais/Jardim-de-Infância/Creche. O estudo em causa segue uma metodologia qualitativa, mais concretamente o estudo de caso e envolve duas educadoras de creche, três educadoras do jardim-de-infância, nove pais das crianças da creche e sessenta e uma crianças (9 responderam às entrevistas acerca da frequência da creche e 52 executaram o desenho alusivo à visita à sala de creche). Com a realização deste trabalho de investigação pretende-se acima de tudo obter resposta à seguinte questão: “Qual a importância da frequência da Creche na transição para o Jardim-de-Infância?” Tendo em conta o estudo realizado foi possível obter determinadas conclusões de acordo com as perpetivas dos profissionais (educadoras de creche e educadoras do jardim-de-infância), pais e crianças do jardim-de-infância. No parecer dos profissionais a creche é importante no sentido de autonomia (gerir a separação dos pais), em termos de regras, altruísmo e partilha de brinquedos, bem como o desenvolvimento global das crianças. Os pais consideram que a creche proporciona o desenvolvimento pessoal e aprendizagem das crianças. Para além disto, a mesma permite educar os seus filhos e é a primeira etapa da educação escolar. Porém, existem pais que referem que o facto de deixarem os seus filhos na referida instituição está relacionado com a inexistência de tempo da sua parte.De acordo com as entrevistas realizadas às crianças do jardim-de-infância podemos verificar que a frequência da creche foi fundamental para o crescimento saudável das crianças, sendo que 89% das crianças têm lembranças positivas sobre o primeiro dia de creche, 57% das crianças menciona como atividades que mais gostavam brincar com as bolas, 100% mencionaram que preferem brincar com outras crianças, 78% revelaram a existência de uma interação positiva entre as mesmas e a educadora, bem como 100% afirmou existir reações positivas à participação dos pais nas suas atividades.

Kinder werden gemeinhin als Adressaten, aber weniger als Akteure von institutionellen Angeboten der Bildung und Betreuung in früher Kindheit wahrgenommen – und das obwohl sie ein komplexes Leben zwischen Familie, Kindertagesbetreuung und Vorschule führen. Das Forschungsprojekt CHILD - Children in the Luxembourgian Day Care System hat das Feld der frühen Bildung und Betreuung von der Position der Kinder aus betrachtet und in kindheitstheoretischer und praxisanalytischer Perspektive nach der Vielfalt betreuter Kindheiten gefragt. Vom Standpunkt der Kinder aus differenziert sich das Luxemburger Feld von Bildung und Betreuung in früher Kindheit in vielfältige Bildungs- und Betreuungsarrangements aus. Sie bestimmen nicht nur die Erfahrungen, die Kinder mit nicht-familialer Bildung und Betreuung machen – und man muss hinzufügen: die nur Kinder machen –, sondern sie bedingen auch die strukturelle Position der Kinder als Mitgestalter von Bildungs- und Betreuungslandschaften früher Kindheit. Bildungs- und Betreuungsarrangements sind eine Domäne der Kinder, obwohl sie sich in einem Zusammenspiel vieler aufeinander bezogener Orte, Kontexte und Akteure realisieren. In diesem Forschungsbericht werden acht ethnographische Fallstudien präsentiert, welche die Multilokalität, Multikontextualität und Multiperspektivität der Bildungs- und Betreuungsarrangements zwei- bis vierjähriger Kinder im Lichte der Alltagspraxis der Kinder analysieren. Die acht Fallstudien zeigen daher nicht nur, dass und wie Kinder zur täglichen Herstellung des Feldes früher Bildung und Betreuung beitragen, sie machen auch die Vielfalt betreuter Kindheiten in Luxemburg sichtbar. Few studies have assessed the quality of life (QOL) related to chronic respiratory diseases in the elderly. In the framework of the geriatric study on the health effects of air quality in elderly care centers (GERIA) study, a questionnaire was completed by elderly subjects from 53 selected nursing homes. It included various sections in order to assess respiratory complaints, QOL (World Health Organization QOL (WHOQOL)-BREF), and the cognitive and depression status. The outcome variables were the presence of a score lower than 50 (<50) in each of the WHOQOL-BREF domains (physical health, psychological health, social relationships, and environmental health). Chronic bronchitis, frequent cough, current wheezing, asthma, and allergic rhinitis were considered as potential risk factors. The surveyed sample was (n = 887) 79% female, with a mean age of 84 years (SD: 7 years). In the multivariable analysis, a score of <50 in the physical domain was associated with wheezing in the previous 12 months (odds ratio (OR): 2.03, confidence interval (CI): 1.25-3.31) and asthma (OR: 1.95, CI: 1.12-3.38). The psychological domain was related with a frequent cough (OR: 1.43, CI: 0.95-2.91). A score of <50 in the environmental domain was associated with chronic bronchitis (OR: 2.89, CI: 1.34-6.23) and emphysema (OR: 3.89, CI: 1.27-11.88). In view of these findings, the presence of respiratory diseases seems to be an important risk factor for a low QOL among elderly nursing home residents.
