A leishmaniose tegumentar tem mostrado caráter endêmico no Norte-Noroeste do Paraná, em áreas que vêm sendo ocupadas desde os anos 40, onde persistem matas residuais com modificações bastante acentuadas. A infecção tem atingido sobretudo adultos do sexo masculino, mas também vem sendo constatada em mulheres e crianças, e por vezes em diversos componentes de uma mesma família. Investigou-se 513 casos de leishmaniose tegumentar notificados pela SUC AM, com informações de terem tido apenas diagnóstico clínico, de janeiro de 1987 a agosto de 1989, os quais iniciaram o tratamento com Glucantime®, em Centros de Saúde do Estado ou na própria SUCAM. De 513 pacientes tratados 260 (50,67%) tiveram alta por cura clínica e os 253 (49,33%) restantes não retornaram para verificação da cura clínica ou abandonaram o tratamento. A quantidade de ampolas de Glucantime® fornecidas para os pacientes oscilou de 10 a mais de 200, com ou sem intervalo durante o tratamento, sendo que 217 (85,46%) dos 260 casos receberam de 10 a 75 ampolas. A ocorrência de leishmaniose em 163 (31,75%) mulheres e em 34 (6,62%) crianças, de ambos os sexos, até dez anos de idade, sugere a transmissão domiciliar. Não se sabe se os 253 (49,33%) indivíduos que não retornaram ou abandonaram o tratamento seguiram a prescrição terapêutica e se restabeleceram. O elevado número de indivíduos que não retornou ou abandonou o tratamento mostra a necessidade de maior conscientização das conseqüências que podem advir do tratamento incompleto. Accumulation of microcystin-LR (MC-LR) in edible aquatic organisms, particularly in bivalves, is widely documented. In this study, the effects of food storage and processing conditions on the free MC-LR concentration in clams (Corbicula fluminea) fed MC-LR-producing Microcystisaeruginosa (1 × 105 cell/mL) for four days, and the bioaccessibility of MC-LR after in vitro proteolytic digestion were investigated. The concentration of free MC-LR in clams decreased sequentially over the time with unrefrigerated and refrigerated storage and increased with freezing storage. Overall, cooking for short periods of time resulted in a significantly higher concentration (P < 0.05) of free MC-LR in clams, specifically microwave (MW) radiation treatment for 0.5 (57.5%) and 1 min (59%) and boiling treatment for 5 (163.4%) and 15 min (213.4%). The bioaccessibility of MC-LR after proteolytic digestion was reduced to 83%, potentially because of MC-LR degradation by pancreatic enzymes. Our results suggest that risk assessment based on direct comparison between MC-LR concentrations determined in raw food products and the tolerable daily intake (TDI) value set for the MC-LR might not be representative of true human exposure.

OBJECTIVES: To find the existing clinical evidence on interventions for leptospirosis. The objective is to evaluate the effectiveness and safety of any intervention on leptospirosis through systematic reviews of randomized controlled trials (RCTs). DATA SOURCE: The sources of studies used (where there were no limitations concerning language, date, or other restrictions) were: EMBASE, LILACS, MEDLINE, the Cochrane Controlled Clinical Trials Database, and the Cochrane Hepato-Biliary Group Randomized Trials register. SELECTION OF STUDIES: Type of Study: All systematic reviews of randomized controlled trials. Participants: patients with clinical and/or laboratorial diagnosis of leptospirosis, and subjects potencially exposed to leptospirosis as defined by the authors Interventions: any intervention for leptospirosis (as antibiotics or vaccines for prevention or treatment). DATA COLLECTION: The assessment will be independently made by the reviewers and cross-checked. The external validity was assessed by analysis of: studies, interventions, and outcomes. DATA SYNTHESIS: Located 163 studies using the search strategy described above, at the electronic databases above. Only 2 hits were selected, which are protocols of systematic reviews of Cochrane Collaboration, and not full reviews. One of the protocols evaluates antibiotics for treatment, and the other evaluates antibiotics for prevention of leptospirosis. CONCLUSIONS: There were not complete systematic reviews on interventions for leptospirosis. Any interventions for leptospirosis, such as prevention and treatment remains unclear for guidelines and practice.

Ticks were obtained from dogs from February to September of 1999 at weekly intervals, in the County of Piraí, State of Rio de Janeiro. Four hundred seventy four ixodids were taxonomically identified, 103 Amblyomma cajennense, seven Amblyomma ovale, 209 Rhipicephalus sanguineus, and 155 Amblyomma sp. An hemolymph test associated with Giemsa's stain revealed two specimens in 163 ticks tested (R. sanguineus and Amblyomma sp), containing rickettsia-like organisms. Direct immunofluorescence verified the presence of spotted fever group rickettsia in one specimen of R. sanguineus. Considering the limited information on rickettsiosis in Brazil, principally in relation to the vectors involved in perpetuating it in foci, these preliminary results give us an idea on the importance of infection in ticks, allowing to expand our knowledge on this zoonosis.

Cuidar de um doente crónico do foro neurocirúrgico é uma arte que requer da parte do enfermeiro o desenvolvimento de competências específicas, dado que é um processo longo e complexo. Esta complexidade, está relacionada não só com a dependência que o doente apresenta, mas também com a forte carga emocional que se reflecte na família/prestador de cuidados. A elaboração de um procedimento orientador de preparação para a alta emergiu como uma necessidade sentida pela equipa de enfermagem. Este procedimento sugere as etapas do ensino a efectuar, permitindo sinalizar o caminho percorrido, inventariar as actividades mais difíceis para o cuidador informal e ser ainda um instrumento de continuidade, para a equipa hospitalar e de comunidade. Esperamos que o desenvolvimento de competências na família, possa diminuir a ansiedade e o stress promovendo estratégias de coping no cuidado ao doente em estado vegetativo.

To describe the epidemiologic profile and prevalence of cardiopathy in 163 Trypanosoma cruzi serum positive blood donor candidates, a descriptive study was carried out between August, 1996 and November, 1997 at the Londrina State University Chagas Disease Outpatient Clinic. The profile found was: young, average age 42.95 ± 8.62 years; male (65%); Caucasian (84%); low level of schooling; low family income; agricultural worker (26%); born in the state of Paraná (67%); from rural areas (85%); migrated to the city (85%); and the vector as the main mechanism of transmission. During the clinical characterization a chronic cardiac form was found in 38% of the patients and classified as cardiac suggestive form in 21% and little suggestive of Chagas disease in 17%. No significant difference was found among age group distribution, sex and the presence of cardiac symptoms in patients with or without cardiopathy. This study emphasizes the importance of expanding medical services to areas with a greater prevalence of infected individuals, in a hierarchical manner and aiming at decentralization.

Este relatório foi realizado no âmbito da unidade curricular de DIPRE (Dissertação/Projecto/Estágio) lecionada no Mestrado em Engenharia Civil – Infraestruturas, no Instituto Superior de Engenharia do Porto. O estágio foi realizado na Divisão Municipal de Obras e Iluminação Pública, na Câmara Municipal do Porto. Neste relatório procurou-se descrever e caracterizar todos os tipos de pavimentos, desenvolver e estudar novas técnicas de orçamentação e planeamento, e ainda analisar vários casos para mostrar a validade do que se realizou. Este relatório inicia-se com uma primeira parte de âmbito teórico, em que se faz referência aos diferentes tipos de pavimentos, analisando-se o seu comportamento, execução, patologias e métodos de dimensionamento. Para além disso faz-se a interpretação do Decreto-Lei nº 163/2006 e das questões de mobilidade urbana. Para o estágio foram necessárias diversas ferramentas de trabalho, não só fornecidas pela Divisão Municipal de Obras e Iluminação Pública, mas também propostas e exploradas pelo aluno. Com estas ferramentas conseguiu-se desenvolver um novo método de orçamentação, estudando os Rendimentos dos operários para um maior rigor nas estimativas de custo efetuadas. As soluções que se apresentam para mostrar o trabalho desenvolvido foram escolhidas de acordo com a sua importância e abrangência para demonstrar tudo o que foi acompanhado e realizado durante o estágio. Começando pela Rua do Dr. Magalhães Lemos, que foi selecionada porque houve a oportunidade de acompanhar e fiscalizar uma obra que contempla a execução de dois pavimentos distintos, o pavimento flexível e o rígido, em Betão Armado Contínuo. Optou-se também por selecionar dois casos de melhoria da acessibilidade no centro da cidade, porque foram dois projetos desenvolvidos pelo aluno em que se conseguiu explorar as diferentes decisões que teve de se tomar. Por fim, apresenta-se o estudo do dimensionamento da Rua de Santo Ildefonso de acordo com as diretrizes da Câmara Municipal do Porto no percurso académico.

The authors analyzed 704 transthoracic echocardiographic (TTE) examinations, performed routinely to all admitted patients to a general 16-bed Intensive Care Unit (ICU) during an 18-month period. Data acquisition and prevalence of abnormalities of cardiac structures and function were assessed, as well as the new, previously unknown severe diagnoses. A TTE was performed within the first 24 h of admission on 704 consecutive patients, with a mean age of 61.5+/-17.5 years, ICU stay of 10.6+/-17.1 days, APACHE II 22.6+/-8.9, and SAPS II 52.7+/-20.4. In four patients, TTE could not be performed. Left ventricular (LV) dimensions were quantified in 689 (97.8%) patients, and LV function in 670 (95.2%) patients. Cardiac output (CO) was determined in 610 (86.7%), and mitral E/A in 399 (85.9% of patients in sinus rhythm). Echocardiographic abnormalities were detected in 234 (33%) patients, the most common being left atrial (LA) enlargement (n=163), and LV dysfunction (n=132). Patients with these alterations were older (66+/-16.5 vs 58.1+/-17.4, p<0.001), presented a higher APACHE II score (24.4+/-8.7 vs 21.1+/-8.9, p<0.001), and had a higher mortality rate (40.1% vs 25.4%, p<0.001). Severe, previously unknown echocardiographic diagnoses were detected in 53 (7.5%) patients; the most frequent condition was severe LV dysfunction. Through a multivariate logistic regression analysis, it was determined that mortality was affected by tricuspid regurgitation (p=0.016, CI 1.007-1.016) and ICU stay (p<0.001, CI 1-1.019). We conclude that TTE can detect most cardiac structures in a general ICU. One-third of the patients studied presented cardiac structural or functional alterations and 7.5% severe previously unknown diagnoses.

Introduction. IgA nephropathy is the dominant primary glomerular disease found throughout the majority of the world’s developed countries. Accurately identifying patients who are at risk of progressive disease is challenging. We aimed to characterise clinical and histological features that predict poor prognosis in adults. Patients and Methods. We performed a single-centre retrospective observational study of biopsy-proven IgA nephropathy. The primary outcome was renal survival and death from any cause, and the secondary outcome was proteinuria remission. Results. Data from 49 cases were available for analysis with a median follow-up of 4 years. There were no deaths. Univariable analyses identified acute renal failure, low estimated glomerular filtration rate for ≥3 months (low eGFR), arterial hypertension, baseline proteinuria, glomerular sclerosis >50% and interstitial fibrosis >50% as poor prognostic markers. Low eGFR persisted significant by multivariable model that used only clinical parameters. Multivariable models with histopathologic parameters observed that tubular atrophy/interstitial fibrosis >50% was independently associated with the primary outcome. Proteinuria remission throughout follow-up had no prognostic value in our revision. Conclusions. Two independent predictors of poor renal survival at time of biopsy were found: low eGFR and tubular atrophy/interstitial fibrosis >50%.

Existem raríssimos casos citados de granulomas da medula óssea por iatrogenia ao alopurinol. Os granulomas são um achado raro nas biópsias da medula óssea e podem ter várias etiologias, nomeadamente infeciosas, neoplásicas, autoimunes, iatrogénicas ou sarcoidose. Na ausência de características patognomónicas, recorre-se à integração de dados clínicos, laboratoriais, imagiológicos e histológicos para estabelecer o diagnóstico. Descrevemos o caso de um caucasiano, 59 anos, referenciado à consulta de Medicina por gota úrica, tendo sido medicado com alopurinol e colchicina. Ao longo do seguimento surgiram anemia e trombocitopenia. Do estudo por mielograma e biópsia osteomedular salientavam-se “numerosos granulomas de tipo epitelióide, sem células multinucleadas, sem identifica- ção de microrganismos PAS pós diástase ou Ziehl-Neelsen, sem neoplasia, compatível com mielite granulomatosa”. A etiologia estabelecida foi a de reação de hipersensibilidade ao alopurinol pelo que foi descontinuado. Repetido o exame um ano depois, não se observaram granulomas, facto que reforçou a hipótese diagnóstica.

Overview and Aims: Several behavioral and biological factors can make adolescents particularly vulnerable to unwanted pregnancies and sexually transmitted diseases. The aim of this study was to evaluate sexual behavior and contraceptive use patterns of a population of adolescents. Study Design: Retrospective study. Population: 163 female adolescents attending an Adolescence Unit for the first time, during 2010. Methods: Analysis of clinical charts and assessment of demographic data, smoking and drinking habits, drug use, gynecologic and obstetric history, sexual behavior and contraceptive use. Results: The mean age was 16.04 years (±1.32). 71.7% were students (of these, 70% had failed one or more years and were behind in their studies), 2.5% were working and 23.9% were neither studying or working. 95.1% had already had sexual intercourse and the mean age of first coitus was 14.53 years (±1.24). There was a history of at least one previous pregnancy in 77.3% of the cases. Before the first appointment at the AU, the contraceptive methods used were: the pill (33.2%, but 41.3% of these reported inconsistent use), and the condom (23.9%, with inconsistent use in 28.3% of these cases). 19.6% did not use any contraceptive method.. After counseling at the AU, 54% of the teenagers chose the contraceptive implant and 35% preferred the pill. Adolescents who had already been pregnant preferred a long acting method (namely, the contraceptive implant)in 61.9% of cases; those who had never been pregnant decided to use an oral contraceptive in 67.6% of cases (p<0.001). Conclusions: After counseling the number of teenagers using contraception increased. In this population there were a high number of adolescents with a previous pregnancy. This factor seems to have influenced the choice of the contraceptive method, with most of these adolescents choosing a long-acting method.
