We aimed to evaluate the influence of different types of wheelchair seats on paraplegic individuals' postural control using a maximum anterior reaching test. Balance evaluations during 50, 75, and 90% of each individual's maximum reach in the forward direction using two different cushions on seat (one foam and one gel) and a no-cushion condition were carried out on 11 individuals with a spinal cord injury (SCI) and six individuals without SCI. Trunk anterior displacement and the time spent to perform the test were assessed. No differences were found for the three types of seats in terms of trunk anterior displacement and the time spent to perform the test when intragroup comparisons were made in both groups (P > 0.05). The intergroup comparison showed that body displacement was less prominent and the time spent to perform the test was more prolonged for individuals with SCI (P < 0.05), which suggests a postural control deficit. The seat type did not affect the ability of the postural control system to maintain balance during the forward-reaching task.

Rice bran oil was obtained from rice bran by solvent extraction using ethanol. The influence of process variables, solvent hydration (0-24% of water, on mass basis), temperature (60-90 degrees C), solvent-to-rice bran mass ratio (2.5:1 to 4.5:1) and stirrer speed (100-250 rpm) were analysed using the response surface methodology. The extraction yield was highly affected by the solvent water content, and it varied from 8.56 to 20.05 g of oil/100 g of fresh rice bran (or 42.7-99.9% of the total oil available) depending on the experimental conditions. It was observed that oryzanol and tocols behave in different ways during the extraction process. A larger amount of tocols is extracted from the solid matrix in relation to gamma-oryzanol. It was possible to obtain values from 123 to 271 mg of tocols/kg of fresh rice bran and 1527 to 4164 mg of oryzanol/kg of fresh rice bran, indicating that it is feasible to obtain enriched oil when this renewable solvent is used. No differences in the chemical composition of the extracted oils were observed when compared to the data cited in the literature. (C) 2011 The Institution of Chemical Engineers. The microencapsulation of Lippia sidoides extracts in blends of carbohydrates was investigated. The extraction conditions were determined through a 2(2) factorial design. The effects of the plant:solvent ratio (A - 7.5:100 and 15:100 m/m) and the extraction time (B - 30 and 90 min) on thymol content of extractive solutions were evaluated, using a 2:1 (v/v) of ethanol:water at a temperature of 50 degrees C, as a solvent system. The selected extract was subjected to spray drying. Blends of maltodextrin and gum arabic at different proportions (4:1; 3:2; 2:3; 0:1) (m/m) were used as encapsulating material. The protective effects of the maltodextrin and gum arabic blends were evaluated by determination of the thymol retention in the dried product, which ranged from 70.2 to 84.2% (related to the content in the extractive solution). An increase in the gum arabic to maltodextrin (DE10) ratio has positive effect on thymol retention. L. sidoides extracts and spray-dried products showed antifungal activity against tested fungal strains (Candida albicans - ATCC 64548, Candida glabrata - ATCC 90030, Candida krusei - ATCC 6258, and Candida parapsilosis - ATCC 22019), evidencing their potential as a natural antifungal agent for medicinal, food, and cosmeceutical purposes. Microencapsulation can be an alternative to minimize lycopene instability. Thus, the aim of this study was to microencapsulate lycopene by spray drying, using a modified starch (Capsul (R)) as an encapsulating agent, and to assess the functionality of the capsules applying them in cake. The quantity of lycopene was varied at 5, 10 and 15% in a solution containing 30% of solids in order to obtain the microcapsules. These microcapsules were evaluated as to encapsulation efficiency and morphology and then submitted to a stability test and applied in cakes. Encapsulation efficiency values varied between 21 and 29%. The microcapsules had a rounded outer surface with the formation of concavities and they varied in size. The stability test revealed that microencapsulation offered greater protection to lycopene compared to its free form and it was observed that the microcapsules were able to release pigment and color the studied food system in a homogenous manner. Objective: To determine the accuracy of the Timed Up and Go Test (TUGT) for screening the risk of falls among community-dwelling elderly individuals. Method: This is a prospective cohort study with a randomly by lots without reposition sample stratified by proportional partition in relation to gender involving 63 community-dwelling elderly individuals. Elderly individuals who reported having Parkinson's disease, a history of transitory ischemic attack, stroke and with a Mini Mental State Exam lower than the expected for the education level, were on a wheelchair and that reported a single fall in the previous six months were excluded. The TUGT, a mobility test, was the measure of interested and the occurrence of falls was the outcome. The performance of basic activities of daily living (ADL) and instrumental activities of daily living (IADL) was determined through the Older American Resources and Services, and the socio-demographic and clinical data were determined through the use of additional questionnaires. Receiver Operating Characteristic Curves were used to analyze the sensitivity and specificity of the TUGT. Results: Elderly individuals who fell had greater difficulties in ADL and IADL (p<0.01) and a slower performance on the TUGT (p=0.02). No differences were found in socio-demographic and clinical characteristics between fallers and non- fallers. Considering the different sensitivity and specificity, the best predictive value for discriminating elderly individuals who fell was 12.47 seconds [(RR= 3.2) 95% CI: 1.3- 7.7]. Conclusions: The TUGT proved to be an accurate measure for screening the risk of falls among elderly individuals. Although different from that reported in the international literature, the 12.47 second cutoff point seems to be a better predictive value for Brazilian elderly individuals.

L’attività fisica produce effetti benefici a livello mentale (Biddle et al.,2004) e sembra agire sull’emozione modificando attitudini e motivazioni verso la pratica motoria (Digelidis et al., 2003) a partire da sensazioni come il piacere o la noia del fare (Spray et al., 1999). Da questi presupposti il progetto che comprendeva l’analisi di due studi volti a verificare gli effetti dell’attività motoria e sportiva in ambito scolastico, sui comportamenti di adattamento sociale e self efficacy dei bambini. Un terzo studio, di analisi qualitativa, in ambito calcistico, per verificare correlazioni tra orientamento motivazionale genitoriale e comportamenti di adattamento sociale dei figli. Gli strumenti di rilevazione dei livelli di adattamento sociale (Caprara et al, 1992) e self efficacy (Colella, 2008), sono stati somministrati prima e dopo il trattamento delle relative attività motorie mentre, l’orientamento motivazionale dei genitori (Borgogni et al, 2004) è stato rilevato una volta e confrontato con l’adattamento sociale dei figli. Il modulatore del primo studio, l’attività ad alto contenuto emotivo o aggressivo, ha mostrato variazioni significative (p<.05) nei livelli di aggressività fisica-verbale e di comportamento pro sociale tra i 2 gruppi, confermando la letteratura sull’argomento (Pellegrini, in Storch e Roth, 2005; Vaughn, 2005; Tappern e Boulton, 2005). Il modulatore del secondo studio, rappresentato dal giocosport rugby, sempre realizzato nelle ore curricolari di educazione fisica, ha evidenziato differenze significative (p<.05) nell’aumentata self efficacy da parte del gruppo sperimentale, con effetto preponderante sulle femmine rispetto ai maschi. Il terzo studio, descrittivo, ha evidenziato la correlazione tra orientamento motivazionale dei genitori e instabilità emotiva dei figli in risposta a profili genitoriali tendenti alla leadership o al successo nell’ambito lavorativo. I risultati evidenziati mostrano effetti significativi (p<.05), successivi al trattamento, sui comportamenti di adattamento sociale, aggressivo e sulla self efficacy a conferma, della letteratura, sull’importanza di determinate esperienze motorie in età scolare.

BACKGROUND AND PURPOSE: Current knowledge of long-term outcome in patients with acute spinal cord ischemia syndrome (ASCIS) is based on few studies with small sample sizes and <2 years' follow-up. Therefore, we analyzed clinical features and outcome of all types of ASCIS to define predictors of recovery. METHODS: From January 1990 through October 2002, 57 patients with ASCIS were admitted to our center. Follow-up data were available for 54. Neurological syndrome and initial degree of impairment were defined according to American Spinal Injury Association (ASIA)/International Medical Society of Paraplegia criteria. Functional outcome was assessed by walking ability and bladder control. RESULTS: Mean age was 59.4 years; 29 were women; and mean follow-up was 4.5 years. The origin was atherosclerosis in 33.3%, aortic pathology in 15.8%, degenerative spine disease in 15.8%, cardiac embolism in 3.5%, systemic hypotension in 1.8%, epidural anesthesia in 1.8%, and cryptogenic in 28%. The initial motor deficit was severe in 30% (ASIA grades A and B), moderate in 28% (ASIA C), and mild in 42% (ASIA D). At follow-up, 41% had regained full walking ability, 30% were able to walk with aids, 20% were wheelchair bound, and 9% had died. Severe initial impairment (ASIA A and B) and female sex were independent predictors of unfavorable outcome (P=0.012 and P=0.043). CONCLUSIONS: Considering a broad spectrum of clinical presentations and origins, the outcome in our study was more favorable than in previous studies reporting on ASCIS subgroups with more severe initial deficits.

People with disabilities often encounter difficulties while trying to learn something, because teaching material is for example not accessible to blind people or rooms, where courses take place, are not accessible to people using a wheelchair. E-learning provides an opportunity to disabled people. With the new German law on the equalisation of opportunities for people with disabilities for the first time access to information technology was explicitly taken up in German legislation. As a consequence of this law the framework law on universities (Hochschulrahmengesetz) was changed. The law now commit universities not to discriminate disabled students in their studies. Enzyme replacement therapy (ERT) with recombinant human alglucosidase alfa (rhGAA) in late-onset Pompe disease is moderately effective. Little is known about the clinical course after treatment termination and the resumption of ERT. In Switzerland, rhGAA therapy for Pompe disease was temporarily withdrawn after the federal court judged that the treatment costs were greatly out of proportion compared to the benefits. Re-treatment was initiated after the therapy was finally licensed. We retrospectively analysed seven Pompe patients, who underwent cessation and resumption of ERT (median age 43 years). The delay from first symptoms to final diagnosis ranged from 4 to 20 years. The demographics, clinical characteristics, assessments with the 6-min walking test (6-MWT), the predicted forced vital capacity (FVC) and muscle strength were analysed. Before initiation of ERT, all patients suffered from proximal muscle weakness of the lower limbs; one was wheelchair-bound and two patients received night-time non-invasive ventilation. Initial treatment stabilised respiratory function in most patients and improved their walking performance. After treatment cessation, upright FVC declined in most and the 6-MWT declined in all patients. Two patients needed additional non-invasive ventilatory support. Twelve months after resuming ERT, the respiratory and walking capacity improved again in most patients. However, aside for one patient, none of the patients reached the same level of respiratory function or distance walked in 6 min, as at the time of ERT withdrawal. We conclude that cessation of ERT in Pompe disease causes a decline in clinical function and should be avoided. Resuming treatment only partially recovers respiratory function and walking capacity.

Einleitung und Ziel: Das Arbeiten mit Adhäsivsystemen und Kompositmaterialien ist techniksensitiv und oft auch zeitaufwändig. Adhäsivsysteme und Kompositmaterialien sind feuchtigkeitsanfällig, weswegen die Verwendung eines Kofferdams von den Herstellern empfohlen wird - nicht nur, um während des Arbeitens mit Adhäsivsystemen und Kompositmaterialien die Gefahr einer Blut- oder Speichelkontamination zu minimieren sondern auch, um die hohe relative Luftfeuchtigkeit in der Mundhöhle auf die Luftfeuchtigkeit der Umgebung zu reduzieren. Abgesehen davon bestehen viele Adhäsivsysteme aus mehreren Applikationsschritten und diese Applikationsschritte weisen jeweils von den Herstellern genau angegebene Applikationszeiten auf. Je nach Adhäsivsystem erscheinen diese Applikationszeiten relativ lang und im klinischen Alltag kann die Gefahr bestehen, dass es zu einer Reduktion der vom Hersteller angegeben Applikationszeiten kommt. Es ist sehr wahrscheinlich, dass sowohl erhöhte relative Luftfeuchtigkeit als auch reduzierte Applikationszeiten die Dentinhaftkraft von Adhäsivsystemen negativ beeinflussen. Das Ziel dieser Studie war folglich, den Einfluss von relativer Luftfeuchtigkeit und Applikationszeit auf die Dentinhaftkraft von sechs verschiedenen Adhäsivsystemen zu untersuchen. Material und Methoden: Insgesamt 360 extrahierte menschliche Molaren ohne Füllungen und/oder Karies wurden verwendet (n=15 pro Gruppe; 24 Gruppen). Die Molaren wurden gereinigt und von okklusal bis ins mittlere Zahnkronendentin plangeschliffen. Die plangeschliffenen Molaren wurden anschliessend in selbsthärtendem Kunststoff eingebettet. Danach wurden die Oberflächen des Zahnkronendentins mit einem von sechs Adhäsivsystemen behandelt und Komposit (Filtek Z250; 3M ESPE) in Form eines Zylinders appliziert. Die sechs Adhäsivsysteme umfassten: 1) Syntac Classic (Ivoclar Vivadent), 2) OptiBond FL (Kerr), 3) Clearfil SE Bond (Kuraray), 4) AdheSE (Ivoclar Vivadent), 5) Xeno Select (DENTSPLY DeTrey) sowie 6) Scotchbond Universal (3M ESPE). Die Applikation des Adhäsivsystems und des Kompositzylinders wurde unter vier experimentellen Bedingungen durchgeführt. Die vier experimentellen Bedingungen wurden in einer modifizierten Handschuhbox erstellt (Abb. 1; CSL-GB24, Cleaver Scientific, Rugby, Grossbritannien) und umfassten: a) relative Luftfeuchtigkeit der Umgebung (~45%) und Applikationszeit gemäss Herstellerangaben, b) relative Luftfeuchtigkeit der Umgebung (~45%) und reduzierte Applikationszeit, c) erhöhte relative Luftfeuchtigkeit (≥85%) und Applikationszeit gemäss Herstellerangaben sowie d) erhöhte relative Luftfeuchtigkeit (≥85%) und reduzierte Applikationszeit. Nach 24 Stunden Lagerung wurden die auf Dentin applizierten Kompositzylinder in einer Universalprüfmaschine (Abb. 2; zwicki Z1.0 TN, Zwick, Ulm, Deutschland) mittels Scherkrafttest belastet (Traversengeschwindigkeit: 1 mm/min) und so die Haftkraftwerte bestimmt. Die Haftkraftwerte wurden anschliessend mit einer nichtparametrischen Varianzanalyse statistisch analysiert, gefolgt von Kruskal-Wallis- und Mann Whitney U-Tests sowie einer Bonferroni-Holm Korrektur für multiples Testen. Das Signifikanzniveau wurde auf α = 0.05 festgelegt. Resultate: Erhöhte relative Luftfeuchtigkeit sowie reduzierte Applikationszeit hatten keinen signifikanten Einfluss auf die Haftkraft von zwei der sechs untersuchten Adhäsivsysteme (Clearfil SE Bond und Scotchbond Universal; p = 1.00). Die Haftkraft der anderen vier Adhäsivsysteme (Syntac Classic, OptiBond FL, AdheSE und Xeno Select) wurde durch eine reduzierte Applikationszeit nicht signifikant beeinflusst (p ≥ 0.403). Allerdings reduzierte eine erhöhte relative Luftfeuchtigkeit (ungeachtet der Applikationszeit) die Haftkraft von Syntac Classic, OptiBond FL und Xeno Select signifikant (p ≤ 0.003). Eine erhöhte relative Luftfeuchtigkeit reduzierte die Haftkraft von AdheSE nur dann signifikant, wenn das Adhäsivsystem mit Applikationszeit gemäss Herstellerangaben verwendet wurde (p = 0.002). Hinsichtlich Unterschiede in der Haftkraft zwischen den sechs Adhäsivsystemen zeigte sich, dass OptiBond FL und Scotchbond Universal generell (sprich ungeachtet der vier experimentellen Bedingungen) die höchste Haftkraft erzielten. Von den anderen vier Adhäsivsystemen wurde Clearfil SE Bond am wenigsten durch erhöhte relative Luftfeuchtigkeit oder reduzierte Applikationszeit beeinflusst und zeigte zwar niedrigere Haftkraftwerte im Vergleich zu OptiBond FL und Scotchbond Universal dafür aber die konstanteste Haftkraft unter allen vier experimentellen Bedingungen. Schlussfolgerung: Die Dentinhaftkraft von zwei der sechs untersuchten Adhäsivsysteme (Clearfil SE Bond und Scotchbond Universal) wurde weder durch erhöhte relative Luftfeuchtigkeit noch durch reduzierte Applikationszeiten beeinflusst. Die Haftkraft der anderen vier Adhäsivsysteme wurde vor allem durch eine erhöhte relative Luftfeuchtigkeit negativ beeinflusst, grossmehrheitlich aber nicht durch eine reduzierte Applikationszeit. Klinisch ist daher die Kontrolle und Reduktion der relativen Luftfeuchtigkeit durch die Verwendung eines Kofferdams sehr zu empfehlen. Obwohl diese Studie gezeigt hat, dass reduzierte Applikationszeiten einen weniger starken, negativen Einfluss auf die Haftkraft haben, ist es trotzdem angezeigt, die Gebrauchsanleitungen der Adhäsivsysteme zu befolgen und sich an die Herstellerangaben zu halten.

An adjustable art table was designed for the artists at Passion Works Studio to fulfill a need for a wheelchair-friendly art table. The client desired that the table be sturdy, not electronic and can be used by multiple users. In response, a mechanical approach was taken and various raising devices were explored. A mechanical height adjustment would make for a more stable table and would not require any electricity or motor to adjust. The table also was built with a large, smooth tabletop designed specifically for multiple users making art. The highlight feature is the height adjustment which allows the table to adjust between 29 and 42.5 inches. The table requires just one person to raise, and two people to lower. To raise the table, an individual only needs to unlock the legs and then press in a button to activate the gas springs, which raise the table. Once the table is set to the desired height, the table can lock into place securely. To lower the table, one person must activate the gas springs and push down simultaneously with another person pushing down on the other side of the table. There is enough room for three people, depending on the size of the wheelchairs. With no wheelchairs, as many as six people can use the table. Therefore, the specifications were met, since it raises and lowers within the desired range, and it provides a solid surface for multiple users to do art work.

Individuals with disabilities face numerous barriers to participation due to biological and physical characteristics of the disability as well as social and environmental factors. Participation can be impacted on all levels from societal, to activities of daily living, exercise, education, and interpersonal relationships. This study evaluated the impact of pain, mood, depression, quality of life and fatigue on participation for individuals with mobility impairments. This cross sectional study derives from self-report data collected from a wheelchair using sample. Bivariate correlational and multivariate analysis were employed to examine the relationship between pain, quality of life, positive and negative mood, fatigue, and depression with participation while controlling for relevant socio-demographic variables (sex, age, time with disability, race, and education). Results from the 122 respondents with mobility impairments demonstrated that after controlling for socio-demographic characteristics in the full model, 20% of the variance in participation scores were accounted for by pain, quality of life, positive and negative mood, and depression. Notably, quality of life emerged as being the single variable that was significantly related to participation in the full model. Contrary to other studies, pain did not appear to significantly impact participation outcomes for wheelchair users in this sample. Participation is an emerging area of interest among rehabilitation and disability researchers, and results of this study provide compelling evidence that several psychosocial factors are related to participation. This area of inquiry warrants further study, as many of the psychosocial variables identified in this study (mood, depression, quality of life) may be amenable to intervention, which may also positively influence participation.^
