Background: The increased prevalence of foot and ankle pathologies in Rheumatic and Musculoskeletal diseases (RMDs) is well documented1, however the provision of foot & ankle (F&A) healthcare services for people with RMDs in Europe has not been evaluated. Objectives: To assess the current healthcare systems for providing foot & ankle healthcare services for people with RMDs in Europe. Methods: A survey was undertaken to evaluate current provision of F&A health care services for people with RMDs across Europe. A questionnaire was distributed to all 22 country presidents representing HP associations within EULAR. The questionnaire used was developed and piloted (in 7 countries) by the EULAR F&A Study Group, and structured to capture the provision and type of F&A services for people with RMDs. When the HP presidents felt unable to answer specific questions they were encouraged to consult a colleague who may be better placed to provide the answers. Results: Sixteen questionnaires were completed (Norway, Ireland, Sweden, Hungary, Netherlands, UK, Denmark, Portugal, Italy, Switzerland, Austria, France, Czech Republic, Spain, Belgium, Malta). Of the 16, 13 respondents indicated provision of F&A health care services in their country, but only three countries had services specialising in RMD-related F&A problems (Netherlands, UK, Malta). The professions providing the care for patients with RMD-related F&A problems were different depending on the pathology and the country (Table1). Podiatrists provided care for F&A pain and deformity problems in 11 countries, but provided F&A ulcer care in only 8 countriesConclusions: Only 3 countries have F&A health care services specialised to the needs of people with RMDs. The professions providing the care varied between countries, and also depended on the F&A pathology. Interestingly, F&A healthcare services were provided by professions that do not solely specialised in F&A care. Further research is needed to assess the variation of F&A healthcare services between and within European countries and the impact on healthcare of various F&A healthcare service designs. Mental stress is known to disrupt the execution of motor performance and can lead to decrements in the quality of performance, however, individuals have shown significant differences regarding how fast and well they can perform a skilled task according to how well they can manage stress and emotion. The purpose of this study was to advance our understanding of how the brain modulates emotional reactivity under different motivational states to achieve differential performance in a target shooting task that requires precision visuomotor coordination. In order to study the interactions in emotion regulatory brain areas (i.e. the ventral striatum, amygdala, prefrontal cortex) and the autonomic nervous system, reward and punishment interventions were employed and the resulting behavioral and physiological responses contrasted to observe the changes in shooting performance (i.e. shooting accuracy and stability of aim) and neuro-cognitive processes (i.e. cognitive load and reserve) during the shooting task. Thirty-five participants, aged 18 to 38 years, from the Reserve Officers’ Training Corp (ROTC) at the University of Maryland were recruited to take 30 shots at a bullseye target in three different experimental conditions. In the reward condition, $1 was added to their total balance for every 10-point shot. In the punishment condition, $1 was deducted from their total balance if they did not hit the 10-point area. In the neutral condition, no money was added or deducted from their total balance. When in the reward condition, which was reportedly most enjoyable and least stressful of the conditions, heart rate variability was found to be positively related to shooting scores, inversely related to variability in shooting performance and positively related to alpha power (i.e. less activation) in the left temporal region. In the punishment (and most stressful) condition, an increase in sympathetic response (i.e. increased LF/HF ratio) was positively related to jerking movements as well as variability of placement (on the target) in the shots taken. This, coupled with error monitoring activity in the anterior cingulate cortex, suggests evaluation of self-efficacy might be driving arousal regulation, thus affecting shooting performance. Better performers showed variable, increasing high-alpha power in the temporal region during the aiming period towards taking the shot which could indicate an adaptive strategy of engagement. They also showed lower coherence during hit shots than missed shots which was coupled with reduced jerking movements and better precision and accuracy. Frontal asymmetry measures revealed possible influence of the prefrontal lobe in driving this effect in reward and neutral conditions. The possible interactions, reasons behind these findings and implications are discussed.

Brain is one of the safe sanctuaries for HIV and, in turn, continuously supplies active viruses to the periphery. Additionally, HIV infection in brain results in several mild-to-severe neuro-immunological complications termed neuroAIDS. One-tenth of HIV-infected population is addicted to recreational drugs such as opiates, alcohol, nicotine, marijuana, etc. which share common target-areas in the brain with HIV. Interestingly, intensity of neuropathogenesis is remarkably enhanced due to exposure of recreational drugs during HIV infection. Current treatments to alleviate either the individual or synergistic effects of abusive drugs and HIV on neuronal modulations are less effective at CNS level, basically due to impermeability of therapeutic molecules across blood-brain barrier (BBB). Despite exciting advancement of nanotechnology in drug delivery, existing nanovehicles such as dendrimers, polymers, micelles, etc. suffer from the lack of adequate BBB penetrability before the drugs are engulfed by the reticuloendothelial system cells as well as the uncertainty that if and when the nanocarrier reaches the brain. Therefore, in order to develop a fast, target-specific, safe, and effective approach for brain delivery of anti-addiction, anti-viral and neuroprotective drugs, we exploited the potential of magnetic nanoparticles (MNPs) which, in recent years, has attracted significant importance in biomedical applications. We hypothesize that under the influence of external (non-invasive) magnetic force, MNPs can deliver these drugs across BBB in most effective manner. Accordingly, in this dissertation, I delineated the pharmacokinetics and dynamics of MNPs bound anti-opioid, anti-HIV and neuroprotective drugs for delivery in brain. I have developed a liposome-based novel magnetized nanovehicle which, under the influence of external magnetic forces, can transmigrate and effectively deliver drugs across BBB without compromising its integrity. It is expected that the developed nanoformulations may be of high therapeutic significance for neuroAIDS and for drug addiction as well.

Tornar acessível a oferta cultural de um museu é premissa essencial numa sociedade que se quer global e integradora de todos os públicos. No que concerne ao público sénior, com necessidades especiais, nomeadamente com perturbações neuro cognitivas (PNC), a preocupação de tornar acessível a abordagem da obra de arte passa pelo conhecimento prévio das caraterísticas específicas dessas doenças degenerativas. Uma das competências a registar comprometimento mais ou menos acentuado é a comunicação verbal. Assim, a comunicação aumentativa revela-se de especial interesse como mecanismo /instrumento mediador entre a pessoa com perturbação neuro cognitiva e a obra de arte cuja abordagem exerce efeitos benéficos de estimulação cognitiva. O presente estudo foi desenvolvido no Museu Nacional de Machado de Castro e no âmbito do projeto EU no musEU, que visa a estimulação cognitiva e o bem-estar global deste público específico. Foram criadas três estórias, depois adaptadas em SPC e aplicadas por quatro participantes oriundos do EU no musEU. Teve por objetivo avaliar as vantagens destes materiais como facilitadores das competências de comunicação da pessoa com PNC. E perceber das mais-valias das narrativas de obras de arte em SPC no apoio a uma visita auto gerida, e inclusiva, destas pessoas e seus cuidadores, em contexto museológico. Humour production and showing mirth (i.e., smiling and laughing) confer prosocial advantages. However, there is a paucity of literature evaluating how humour manifests in psychopathology. Humour and mirth may be especially relevant in depression, wherein profound impairments are evident in emotional and social functioning. Chapters 2 and 3 present correlational and predictive relations of depressive, social anxiety, and social anhedonia symptoms with humour styles, and consider the role of motivational systems and expressivity of positive affect as they relate to humour. As expected, symptoms and avoidance-based motivation were positively related to maladaptive humour styles and negatively related to adaptive humour styles. Interestingly, the pattern of relations shifted when considered among individuals in a depressive episode; acutely depressed individuals generally shy away from any humour style rather than gravitating toward specific styles. In a mediation model, the inverse relation between depressive symptoms and affiliative humour was fully mediated by approach-based motivation and expressivity of positive emotions. Chapters 4 and 5 examined subjective and observed mirth responses (facial affect and laughter) demonstrated by depressed and healthy comparison groups. Relative to non-depressed individuals, depressed persons reported less enjoyment, lower ratings of funniness, and fewer instances and shorter durations of positive facial affect and laughter when viewing humourous videos. There was no significant change in retrospective ratings of enjoyment and funniness at a one-week follow-up. The pattern of responsivity by depressed persons shifted when they viewed humourous videos while hearing others laughing. Both groups demonstrated more mirth when hearing others laugh; there were no differences between groups on mirthful behaviours. The one exception was that the total duration of laugher produced by depressed individuals was shorter than that produced by individuals in the healthy comparison group. This research project demonstrates that facets of depressive symptomatology are differentially associated with humour use and depressed individuals show blunted emotional responsivity to humourous stimuli. However, the pattern of reduced affective responsivity is context specific in that it fluctuates in response to hearing others’ laughter. These findings have important implications for the conceptualization of depression and the subsequent avenues for the treatment of individuals with depression.

Multiple Sclerosis (MS), an idiopathic progressive immune-mediated neurological disorder of the central nervous system (CNS), is characterized by recurrent episodes of inflammatory demyelination and consequent axonal deterioration. It accounts for functional deterioration and lasting disability among young adults. A body of literature demonstrates that physical activity counteracts fatigue and depression and may improve overall quality of life in MS patients. Furthermore, much data indicates that exercise ameliorates chronic neuroinflammation and its related pathologies by tipping cytokine profiles toward an anti-inflammatory signature. Recent data has focused on the direct impact of exercise training on the innate immune system by targeting toll-like receptors (TLRs), signaling pattern recognition receptors that govern the innate immune response, shedding light on the physiological role of TLRs in health and disease. Indeed, TLRs continue to emerge as players in the neuroinflammatory processes underpinning MS. This review will highlight evidence that physical activity and exercise are potential immunomodulatory therapies, targeting innate signaling mechanism(s) to modulate MS symptom development and progression.

RESUMEN El dolor dorso lumbar es un síntoma que afecta a gran cantidad de personas y población trabajadora en el mundo y comprende un importante problema de salud. La prevalencia de dolor de espalda baja en población trabajadora se estima en un 60% a un 85%, mientras que la prevalencia anual en la población general varía entre un 15% al 45%. La incidencia anual de dolor de espalda en la población general se estima entre 10% y 15%. Objetivo Determinar la prevalencia de síntomas musculoesqueléticos de la región dorso lumbar y los hábitos de vida de trabajadores en una empresa de construcción en la ciudad de Bogotá. Metodología Se realizó un estudio de corte transversal. Se analizaron datos secundarios procedentes de una base de datos de una empresa de construcción ubicada en la ciudad de Bogotá, con 282 trabajadores. Se incluyeron variables sociodemográficas, laborales, hábitos de vida y síntomas musculoesqueléticos. Se hizo un análisis descriptivo considerando las medidas de tendencia central para las variables cuantitativas y distribución de frecuencias para variables cualitativas. Resultados Se incluyeron a 282 trabajadores con un promedio de edad de 33.4 años (DE=10.2). El 93% correspondió al sexo masculino. La prevalencia de síntomas musculoesqueléticos en región dorsal fue de 26% y región lumbar 31.2%. La mayoría de trabajadores se encuentra en el área operativa con un 87,9% en la tercera década de la vida y con un tiempo en la empresa de 11 a 18 años (42.8%). Otros factores que se asociaron con la presencia de síntomas en la región dorsolumbar fueron: realizar oficios domésticos, el sedentarismo y dormir menos de 6 horas. Conclusiones Los síntomas musculoesqueléticos derivados de la actividad laboral en la construcción constituyen un problema significativo influenciado por los factores de riesgo relacionados con el trabajo, las características sociodemográficas y hábitos de vida que pueden jugar un papel fundamental para la aparición de sintomatología. Por lo tanto, se requiere que los trabajadores sean incluidos en programas de vigilancia epidemiológica, con el fin de establecer acciones de prevención que permitan disminuir la aparición de dichos síntomas.

L’apparato muscolo scheletrico è composto da strutture muscolari, articolari e ossee. Tali tessuti sono molto diversi tra loro e hanno proprietà meccaniche estremamente variabili, pertanto presentano una transizione graduale in corrispondenza della loro giunzione, onde evitare l’insorgere di concentrazioni di tensione. L’evoluzione ha portato alla formazione di particolari interfacce che permettono la corretta trasmissione dei carichi distribuendo le tensioni su una superficie più ampia in corrispondenza della giunzione. Le interfacce che vanno a inserirsi nell’osso vengono definite entesi e in particolare, in questa review, analizzeremo il caso di quelle tra tendini/legamenti e osso. In questo lavoro ci siamo anche concentrati sulla giunzione miotendinea, ovvero tra muscolo e tendine. Sono numerose le lesioni che riguardano muscoli, ossa, tendini o legamenti e molto spesso l’infortunio avviene a livello della giunzione. Quando ciò accade vi sono diverse strade, ciascuna con i suoi vantaggi e svantaggi: sutura, autograft, allograft o xenograft. Oltre a queste soluzioni si è fatta gradualmente più spazio la possibilità di realizzare degli scaffold che vadano temporaneamente a sostituire la parte danneggiata e a promuovere la sua rigenerazione, degradandosi man mano. L’elettrofilatura (Elettrospinning) è un processo produttivo che negli ultimi decenni si è affermato come tecnica per la fabbricazione di questi scaffold, fino a diventare uno tra i principali processi utilizzati dai ricercatori in questo campo. Questa tecnica infatti permette di realizzare scaffold di nanofibre porose utilizzando polimeri biodegradabili e soprattutto biocompatibili. Lo scopo della review è proprio quello di scoprire tutti i lavori e gli studi che utilizzano l’elettrofilatura per realizzare degli scaffold per interfacce, delineando così lo stato dell’arte sui progressi fatti e sulle varie tecniche utilizzate.

Lo studio di modelli teorici e matematici della memoria semantica ricopre un ruolo importante nelle neuroscienze cognitive. I modelli neuro-computazionali sviluppati negli ultimi decenni vengono impiegati per spiegare e simulare come le informazioni recepite dall’esterno vengono memorizzate e successivamente utilizzate. In questo elaborato si sviluppa un modello di rete semantica per il riconoscimento di concetti, definiti come insieme di caratteristiche. Fondamentale è il ruolo assunto dalle diverse proprietà, che sono state suddivise fra salienti e marginali, distintive e condivise. I concetti presi in considerazione con le rispettive feature, fanno parte di un ampio data set fornito dalla Dott.ssa Catricalà. La rete sviluppata rientra tra i modelli di massa neuronale che considera quattro popolazioni di neuroni: piramidali, inter-neuroni eccitatori, inter-neuroni inibitori lenti e inter-neuroni inibitori veloci. Il modello sviluppato si basa sullo studio del professor Ursino et al. e utilizza oscillatori in banda gamma. Tramite sincronizzazione di queste oscillazioni è possibile memorizzare concetti e successivamente recuperarli, mantenendoli in memoria simultaneamente. Il richiamo di più concetti contemporaneamente avviene tramite desincronizzazione delle oscillazioni ad opera di un inibitore globale, modellato tramite funzione a gradino. Per modellare l’apprendimento della rete è stata utilizzata la regola di Hebb, sfruttando soglie pre e post-sinaptiche differenti così da costruire sinapsi asimmetriche che permettono una differenziazione delle feature.

La realtà aumentata (AR) è una nuova tecnologia adottata in chirurgia prostatica con l'obiettivo di migliorare la conservazione dei fasci neurovascolari (NVB) ed evitare i margini chirurgici positivi (PSM). Abbiamo arruolato prospetticamente pazienti con diagnosi di cancro alla prostata (PCa) sul base di biopsia di fusione mirata con mpMRI positiva. Prima dell'intervento, i pazienti arruolati sono stati indirizzati a sottoporsi a ricostruzione del modello virtuale 3D basato su mpMRI preoperatoria immagini. Infine, il chirurgo ha eseguito la RARP con l'ausilio del modello 3D proiettato in AR all'interno della console robotica (RARP guidata AR-3D). I pazienti sottoposti a AR RARP sono stati confrontati con quelli sottoposti a "RARP standard" nello stesso periodo. Nel complesso, i tassi di PSM erano comparabili tra i due gruppi; I PSM a livello della lesione indice erano significativamente più bassi nei pazienti riferiti al gruppo AR-3D (5%) rispetto a quelli nel gruppo di controllo (20%; p = 0,01). La nuova tecnica di guida AR-3D per l'analisi IFS può consentono di ridurre i PSM a livello della lesione dell'indice

Aims and methods: 1) characterization of patients with Dominant Optic Atrophy (DOA) associated with mutations in AFG3L2 and ACO2 genes in comparison with classical OPA1-DOA; 2) characterization of patients with mtDNA mutations causing MELAS and MERRF syndromes and correlation with heteroplasmy; 3) longitudinal evaluation of subacute m.11778G>A/MTND4 Leber’s Hereditary Optic Neuropathy (LHON) patients co-treated with rAAV2/2-ND4 gene therapy and idebenone. We performed a comprehensive neuro-ophthalmological assessment coupled with electrophysiological examination. Results: 1) We described and compared 23 ACO2 and 13 AFG3L2 patients with 72 OPA1 patients. All patients presented temporally predominant optic atrophy, with ACO2 showing higher RNFL and GCL thicknesses at OCT, while AFG3L2 was virtually-indistinguishable from OPA1. 2) Retinopathy was the most common manifestation in 17/33 MELAS patients, conversely, optic atrophy was the most common finding in 7/8 MERRF patients. Correlation of heteroplasmy with neuro-ophthalmological parameters failed to disclose any significance in MELAS, while it negatively correlated with OCT parameters in MERRF. 3) We compared modifications in visual acuity, OCT and electrophysiological parameters at 3 timepoints in 9 LHON patients. We observed significant decrease of RNFL thickness and reduction of PhNR amplitude. Visual acuity improved of about -0.37 LogMAR, correlating significantly with time from onset and from injection, but not with idebenone therapy duration. Discussion: 1) ACO2 seems associated to better preservation of retinal ganglion cells, depending on a different pathogenic mechanism involving mtDNA maintenance, as opposed to AFG3L2 which is involved in OPA1 processing. 2) MELAS and MERRF patients presented with a clearly distinct ocular phenotype, possibly reflecting a selective susceptibility of different retinal cell types to global energy defect or oxidative stress. 3) Follow up of LHON patients treated with gene therapy confirmed the deterioration in OCT and electrophysiological parameters, while the amount of visual improvement was similar to the one observed in recent clinical trials.
