BACKGROUND AND STUDY AIMS: Appropriate use of colonoscopy is a key component of quality management in gastrointestinal endoscopy. In an update of a 1998 publication, the 2008 European Panel on the Appropriateness of Gastrointestinal Endoscopy (EPAGE II) defined appropriateness criteria for various colonoscopy indications. This introductory paper therefore deals with methodology, general appropriateness, and a review of colonoscopy complications. METHODS:The RAND/UCLA Appropriateness Method was used to evaluate the appropriateness of various diagnostic colonoscopy indications, with 14 multidisciplinary experts using a scale from 1 (extremely inappropriate) to 9 (extremely appropriate). Evidence reported in a comprehensive updated literature review was used for these decisions. Consolidation of the ratings into three appropriateness categories (appropriate, uncertain, inappropriate) was based on the median and the heterogeneity of the votes. The experts then met to discuss areas of disagreement in the light of existing evidence, followed by a second rating round, with a subsequent third voting round on necessity criteria, using much more stringent criteria (i. e. colonoscopy is deemed mandatory). RESULTS: Overall, 463 indications were rated, with 55 %, 16 % and 29 % of them being judged appropriate, uncertain and inappropriate, respectively. Perforation and hemorrhage rates, as reported in 39 studies, were in general < 0.1 % and < 0.3 %, respectively CONCLUSIONS: The updated EPAGE II criteria constitute an aid to clinical decision-making but should in no way replace individual judgment. Detailed panel results are freely available on the internet (www.epage.ch) and will thus constitute a reference source of information for clinicians.

A doação de órgãos é um processo complexo que desafia os profissionais e gestores do sistema de saúde. Este estudo objetiva apresentar um modelo teórico de organização do cuidado ao paciente em morte encefálica e o processo de doação de órgãos, balizado pelas principais ideias do pensamento Lean que possibilitam a melhoria da produção a partir de ciclos de planejamentos e criação de um ambiente propício para o sucesso da sua implementação. O pensamento Lean pode tornar mais eficaz e eficiente o processo de doação de órgãos e contribuir com a sua melhoria, a partir da sistematização das informações e capacitação dos profissionais para a excelência do cuidado. O modelo apresentado configura-se como um referencial disponível para validação e aplicação pelos profissionais e gestores de saúde e enfermagem na prática da gestão do cuidado ao paciente potencial doador de órgãos em morte encefálica e respectiva demanda por transplante.

AbstractOBJECTIVEPresenting methodology for transferring knowledge to improve maternal outcomes in natural delivery based on scientific evidence.METHOD: An intervention study conducted in the maternity hospital of Itapecerica da Serra, SP, with 50 puerperal women and 102 medical records from July to November 2014. The PACES tool from Joanna Briggs Institute, consisting of pre-clinical audit (phase 1), implementation of best practice (phase 2) and Follow-up Clinical Audit (phase 3) was used. Data were analyzed by comparing results of phases 1 and 3 with Fisher's exact test and a significance level of 5%.RESULTSThe vertical position was adopted by the majority of puerperal women with statistical difference between phases 1 and 3. A significant increase in bathing/showering, walking and massages for pain relief was found from the medical records. No statistical difference was found in other practices and outcomes. Barriers and difficulties in the implementation of evidence-based practices have been identified. Variables were refined, techniques and data collection instruments were verified, and an intervention proposal was made.CONCLUSIONThe study found possibilities for implementing a methodology of practices based on scientific evidence for assistance in natural delivery.

PURPOSE: As the magnetic susceptibility induced frequency shift increases linearly with magnetic field strength, the present work evaluates manganese as a phase imaging contrast agent and investigates the dose dependence of brain enhancement in comparison to T1 -weighted imaging after intravenous administration of MnCl2 . METHODS: Experiments were carried out on 12 Sprague-Dawley rats. MnCl2 was infused intravenously with the following doses: 25, 75, 125 mg/kg (n=4). Phase, T1 -weighted images and T1 maps were acquired before and 24h post MnCl2 administration at 14.1 Tesla. RESULTS: Manganese enhancement was manifested in phase imaging by an increase in frequency shift differences between regions rich in calcium gated channels and other tissues, together with local increase in signal to noise ratio (from the T1 reduction). Such contrast improvement allowed a better visualization of brain cytoarchitecture. The measured T1 decrease observed across different manganese doses and in different brain regions were consistent with the increase in the contrast to noise ratio (CNR) measured by both T1 -weighted and phase imaging, with the strongest variations being observed in the dentate gyrus and olfactory bulb. CONCLUSION: Overall from its high sensitivity to manganese combined with excellent CNR, phase imaging is a promising alternative imaging protocol to assess manganese enhanced MRI at ultra high field. O Lean não é apenas uma prática. É uma revolução nas Tecnologias de Informação (TI) proporcionando uma maior e melhor utilização dos recursos e procurando alcançar custos mais baixos dos que existem atualmente. É muito mais do que uma lista de ferramentas e metodologias e para que seja estabelecido é necessário mudar comportamentos culturais e incentivar todas as organizações a pensarem de forma diferente sobre o poder da informação versus o valor do negócio. Normalmente associa-se o Lean à criação de valor para a organização. Mas o valor é significativo quando é trazido com eficiência e resultando na eliminação de processos que consomem tempo, recursos e espaço desnecessário. Os princípios Lean podem ajudar as organizações na melhoria da qualidade, redução de custos e no alcance da eficiência através de uma melhor produtividade. Existem vários conceitos Lean que podem ser associados a diferentes objetivos de resolução de problemas. Em particular, este trabalho é uma dissertação programada para analisar um novo paradigma sobre o Lean que surgiu recentemente - Lean para Tecnologias de Informação (Lean IT). Esta dissertação apresenta uma abordagem para o Lean IT (enquadramento, objetivos e metodologia) para realizar o trabalho e utiliza um único estudo de caso, com abordagem à técnica 5S/6S (até o terceiro nível de avaliação), numa Pequena, Média Empresa (PME), de forma a demonstrar a agregação de valor e as vantagens na eliminação de resíduos/desperdícios nos seus processos. A técnica também mostra a evolução da avaliação antes e depois da sua aplicação. Este estudo de caso individual avalia um Departamento de TI (com uma equipe de cinco colaboradores e um chefe de Departamento) através da observação direta, documentação e arquivos de registos e os equipamentos analisados são computadores, postos de trabalho e projetos (código desenvolvido, portais e outros serviços de TI). x Como guia, a metodologia inclui a preparação da avaliação em conjunto com o responsável/chefe do Departamento de TI, o desenrolar das operações, a identificação do fluxo de valor para cada atividade, o desenvolvimento de um plano de comunicação e a análise de cada passo da avaliação do fluxo de processamento. Os principais resultados estão refletidos nas novas ferramentas de trabalho (Microsoft SharePoint e Microsoft Project em detrimento do Microsoft Excel) que fornecem comunicação remota e controlo de projetos para todos os stakeholders, tais como, a gestão de topo, parceiros e clientes (algumas organizações incluem o Outsourcing no desenvolvimento de funcionalidades específicas). Os resultados também refletem-se na qualidade do trabalho, no cumprimento de prazos, na segurança física e lógica, na motivação dos colaboradores e na satisfação dos clientes. A técnica 5S/6S ajuda a esclarecer os conceitos e princípios Lean, exequibilidade e aumenta a curiosidade sobre a implementação da técnica noutros departamentos tais como o Financeiro e ou o de Recursos Humanos. Como forma de consolidação do trabalho tornou-se possível organizar a avaliação para que a organização possa candidatar-se a uma certificação na norma ISO/IEC 25010:2011, no modelo de qualidade de software (software é o core business desta PME). Mas tal só será possível se toda a organização atingir a padronização dos seus processos. Este estudo de caso mostra que os conceitos Lean e a aplicação de uma ou mais das suas técnicas (neste caso particular a técnica 5S/6S) ajuda a obter melhores resultados através da gestão e melhoria dos seus principais serviços.

Accomplish high quality of final products in pharmaceutical industry is a challenge that requires the control and supervision of all the manufacturing steps. This request created the necessity of developing fast and accurate analytical methods. Near infrared spectroscopy together with chemometrics, fulfill this growing demand. The high speed providing relevant information and the versatility of its application to different types of samples lead these combined techniques as one of the most appropriated. This study is focused on the development of a calibration model able to determine amounts of API from industrial granulates using NIR, chemometrics and process spectra methodology.

Com a resultat de les politiques i estratègies de col·laboració entre la universitat de Vic i de l’hospital i de la voluntat de realitzar activitats formatives conjuntes , s’estableix un línia de treball orientada a l’estudi i anàlisi de la situació logística interna actual del laboratori d’anàlisis clíniques de l’Hospital General de Vic. El treball es centra en el procés intern del laboratori i l’abast de l’estudi es troba limitat a les àrees especifiques d’hematologia i coagulació i bioquímica. D’aquestes dues àrees el treball realitza un estudi exhaustiu del seu procés intern, identifica les seves activitats i la seva metodologia de treball amb l’objectiu d’elaborar el Value Stream Map de cadascuna de les àrees. Les àrees de Microbiologia, Banc de Sang i Urgències resten fora d’aquest estudi exhaustiu tot i que són presents en el treball per la inevitable interacció que tenen en la globalitat del procés. El treball es centra bàsicament en els processos automatitzats tot i que els processos que es duen a terme en el laboratori són tant automatitzats com manuals. També es limita al sistema productiu intern del laboratori tot i la interacció que té aquest sistema intern amb altres centres productius del sistema com ara són els centres d’atenció primària, els diversos hospitals i centres d’atenció sociosanitària. El laboratori es troba immers en el moment de l’elaboració d’aquest treball en un situació de canvi i millora del seus processos interns que consisteixen principalment en la substitució de part la maquinària actual que obliguen a la definició d’un nou layout i d’una nova distribució de la producció a cada màquina. A nivell extern també s’estan produint millores en el sistema informàtic de gestió que afecten a part del seu procés. L’objectiu del treball és donar visibilitat total al procés de logística interna actual del laboratori, identificant clarament com són i quina seqüència tenen els processos logístics interns i els mètodes de treball actuals, tant de recursos màquina com recursos persona, per poder identificar sota una perspectiva de generació de valor, aquells punts concrets de la logística interna que poden ser millorats en quant a eficiència i productivitat amb l’objectiu que un cop identificats es puguin emprendre accions i/o projectes de millora. El treball finalitza amb un anàlisis final del procés logística interna des d’una òptica Lean. Per fer-ho, identifica aquelles activitats que no aporten valor al procés o MUDA i les classifica en set categories i es realitzen diverses propostes de millora com són la implantació d’un flux continu , anivellat i basat en un concepte pull , identifica activitats que poden ser estandarditzades i/o simplificades i proposa modificacions en les infraestructures físiques per donar major visibilitat al procés. L’aspecte humà del procés es planteja des d’un punt de vist de metodologia, formació, comunicació i aplicació de les 5S.

During the period 1996-2000, forty-three heavy rainfall events have been detected in the Internal Basins of Catalonia (Northeastern of Spain). Most of these events caused floods and serious damage. This high number leads to the need for a methodology to classify them, on the basis of their surface rainfall distribution, their internal organization and their physical features. The aim of this paper is to show a methodology to analyze systematically the convective structures responsible of those heavy rainfall events on the basis of the information supplied by the meteorological radar. The proposed methodology is as follows. Firstly, the rainfall intensity and the surface rainfall pattern are analyzed on the basis of the raingauge data. Secondly, the convective structures at the lowest level are identified and characterized by using a 2-D algorithm, and the convective cells are identified by using a 3-D procedure that looks for the reflectivity cores in every radar volume. Thirdly, the convective cells (3-D) are associated with the 2-D structures (convective rainfall areas). This methodology has been applied to the 43 heavy rainfall events using the meteorological radar located near Barcelona and the SAIH automatic raingauge network.

A statistical methodology for the objective comparison of LDI-MS mass spectra of blue gel pen inks was evaluated. Thirty-three blue gel pen inks previously studied by RAMAN were analyzed directly on the paper using both positive and negative mode. The obtained mass spectra were first compared using relative areas of selected peaks using the Pearson correlation coefficient and the Euclidean distance. Intra-variability among results from one ink and inter-variability between results from different inks were compared in order to choose a differentiation threshold minimizing the rate of false negative (i.e. avoiding false differentiation of the inks). This yielded a discriminating power of up to 77% for analysis made in the negative mode. The whole mass spectra were then compared using the same methodology, allowing for a better DP in the negative mode of 92% using the Pearson correlation on standardized data. The positive mode results generally yielded a lower differential power (DP) than the negative mode due to a higher intra-variability compared to the inter-variability in the mass spectra of the ink samples.

My research in live drawing and new technologies uses a combination of a human figure in live in composition, overlaid with a digital projection of a second human figure. The aim is to explore, to amplify and thoroughly analyse the search for distinctive identities and graphic languages of representation for live and projected models.

Two trends which presently exist in relation to the concept of Paleontology are analyzed, pointing out some of the aspects which negative influence. Various reflections are made based on examples of some of the principal points of paleontological method, such as the influence of a punctual sampling, the meaning of size-frequency distribution and subjectivity in the identification of fossils. Topics which have a marked repercussion in diverse aspects of Paleontology are discussed.

Synthetic root exudates were formulated based on the organic acid composition of root exudates derived from the rhizosphere of aseptically grown corn plants, pH of the rhizosphere, and the background chemical matrices of the soil solutions. The synthetic root exudates, which mimic the chemical conditions of the rhizosphere environment where soil-borne metals are dissolved and absorbed by plants, were used to extract metals from sewage-sludge treated soils 16 successive times. The concentrations of Zn, Cd, Ni, Cr, and Cu of the sludge-treated soil were 71.74, 0.21, 15.90, 58.12, and 37.44 mg kg-1, respectively. The composition of synthetic root exudates consisted of acetic, butyric, glutaric, lactic, maleic, propionic, pyruvic, succinic, tartaric, and valeric acids. The organic acid mixtures had concentrations of 0.05 and 0.1 mol L-1 -COOH. The trace elements removed by successive extractions may be considered representative for the availability of these metals to plants in these soils. The chemical speciation of the metals in the liquid phase was calculated; results showed that metals in sludge-treated soils were dissolved and formed soluble complexes with the different organic acid-based root exudates. The most reactive organic acid ligands were lactate, maleate, tartarate, and acetate. The inorganic ligands of chloride and sulfate played insignificant roles in metal dissolution. Except for Cd, free ions did not represent an important chemical species of the metals in the soil rhizosphere. As different metals formed soluble complexes with different ligands in the rhizosphere, no extractor, based on a single reagent would be able to recover all of the potentially plant-available metals from soils; the root exudate-derived organic acid mixtures tested in this study may be better suited to recover potentially plant-available metals from soils than the conventional extractors.
