The dissertation accomplishes two aims: 1) to diagnose what prevents true beliefs from being knowledge; 2) to give an positive account of knowledge. Concerning the first aim, it offers an account of the notion of luck. It defends the view that luck is a form of risk and distinguishes two types of luck. Then, it applies the account to the problem of epistemic luck and distinguishes, accordingly, two types of epistemic luck. It is argued that these two types of epistemic luck explain the whole range of cases of not-known true belief. Concerning the second aim, the dissertation advances an account of knowledge in terms of the notion of cognitive control that deals with the two forms of epistemic luck distinguished.

BACKGROUND Job satisfaction of nurses is a determinant factor in the quality and organizational adaptation of clinical management models in the current socio-economic context. The aim of this study was to construct and validate a questionnaire to measure job satisfaction of nurses in the Clinical Management Units in the Andalusian Public Health System. METHODS Clinimetric and cross-sectional study with a sample of 314 nurses of two university hospitals from Seville. Nurses were surveyed in 2011, from March to June. We used the Font Roja questionnaire adapted to our study variables. We performed analyses of correlations, reliability and construct validity, using exploratory factor analysis (EFA) and confirmatory factor analysis (CFA) to test the a priori model. RESULTS The end questionnaire consists of 10 items, whose internal consistency was 0.75, with a percentage of variance explaining of 63.67%. CFA confirmed 4 dimensions (work environment, work relationships, motivation, and recognition): significant χ2 (p < .001); χ2/gl = 2.013; GFI= 0.958, RMR = 0.055 y RMSEA = 0.057; AGFI = 0.927, NFI = 0.878, TLI = 0.902, CFI =0.933 e IFI = 0.935; AIC = 132.486 y ECVI = 0.423. CONCLUSION This new questionnaire (G_Clinic) improves clinimetric values of the Font Roja questionnaire, because it reduces the number of items, improves the reliability of the dimensions, increases the value of variance explained, and allows knowing job satisfaction of nurses in clinical managementt.

BACKGROUND & AIMS Hy's Law, which states that hepatocellular drug-induced liver injury (DILI) with jaundice indicates a serious reaction, is used widely to determine risk for acute liver failure (ALF). We aimed to optimize the definition of Hy's Law and to develop a model for predicting ALF in patients with DILI. METHODS We collected data from 771 patients with DILI (805 episodes) from the Spanish DILI registry, from April 1994 through August 2012. We analyzed data collected at DILI recognition and at the time of peak levels of alanine aminotransferase (ALT) and total bilirubin (TBL). RESULTS Of the 771 patients with DILI, 32 developed ALF. Hepatocellular injury, female sex, high levels of TBL, and a high ratio of aspartate aminotransferase (AST):ALT were independent risk factors for ALF. We compared 3 ways to use Hy's Law to predict which patients would develop ALF; all included TBL greater than 2-fold the upper limit of normal (×ULN) and either ALT level greater than 3 × ULN, a ratio (R) value (ALT × ULN/alkaline phosphatase × ULN) of 5 or greater, or a new ratio (nR) value (ALT or AST, whichever produced the highest ×ULN/ alkaline phosphatase × ULN value) of 5 or greater. At recognition of DILI, the R- and nR-based models identified patients who developed ALF with 67% and 63% specificity, respectively, whereas use of only ALT level identified them with 44% specificity. However, the level of ALT and the nR model each identified patients who developed ALF with 90% sensitivity, whereas the R criteria identified them with 83% sensitivity. An equal number of patients who did and did not develop ALF had alkaline phosphatase levels greater than 2 × ULN. An algorithm based on AST level greater than 17.3 × ULN, TBL greater than 6.6 × ULN, and AST:ALT greater than 1.5 identified patients who developed ALF with 82% specificity and 80% sensitivity. CONCLUSIONS When applied at DILI recognition, the nR criteria for Hy's Law provides the best balance of sensitivity and specificity whereas our new composite algorithm provides additional specificity in predicting the ultimate development of ALF.

El WP té com a objectiu examinar per què els països del nord d'Àfrica estan augmentant les seves importacions de Xina i per què la Xina està augmentant les seves inversions estrangeres directes a la zona. la avantatges per als països del nord d'Àfrica es troben en els actuals acords de lliure comerç, el absència de barreres aranzelàries i la seva ubicació geogràfica summament beneficiosa com Hub de la Mediterrània. El paper de la Unió per la Mediterrània és clau per identificar aspectes de la política regional, tenint en compte la participació xinesa a la zona. com conseqüència, concloem que els actors polítics han de trobar un equilibri entre sostenible desenvolupament i els interessos econòmics de la regió.

BACKGROUND The objective of this research was to evaluate data from a randomized clinical trial that tested injectable diacetylmorphine (DAM) and oral methadone (MMT) for substitution treatment, using a multi-domain dichotomous index, with a Bayesian approach. METHODS Sixty two long-term, socially-excluded heroin injectors, not benefiting from available treatments were randomized to receive either DAM or MMT for 9 months in Granada, Spain. Completers were 44 and data at the end of the study period was obtained for 50. Participants were determined to be responders or non responders using a multi-domain outcome index accounting for their physical and mental health and psychosocial integration, used in a previous trial. Data was analyzed with Bayesian methods, using information from a similar study conducted in The Netherlands to select a priori distributions. On adding the data from the present study to update the a priori information, the distribution of the difference in response rates were obtained and used to build credibility intervals and relevant probability computations. RESULTS In the experimental group (n = 27), the rate of responders to treatment was 70.4% (95% CI 53.287.6), and in the control group (n = 23), it was 34.8% (95% CI 15.354.3). The probability of success in the experimental group using the a posteriori distributions was higher after a proper sensitivity analysis. Almost the whole distribution of the rates difference (the one for diacetylmorphine minus methadone) was located to the right of the zero, indicating the superiority of the experimental treatment. CONCLUSION The present analysis suggests a clinical superiority of injectable diacetylmorphine compared to oral methadone in the treatment of severely affected heroin injectors not benefiting sufficiently from the available treatments. BACKGROUND Health sector reforms taking place in Colombia during the Nineties included policies to promote social participation in the health system, which is considered essential to its functioning. The aim of this article is to analyse the meaning and the significance of participation in health for the different social actors involved in implementing policies in Colombia. METHODS A qualitative, descriptive, exploratory study was carried out using focal groups (FG) and semi-structured individual interviews (I) of the different social actors: 210 users (FG), 40 community leaders (FG), 3 policy makers (E) and 36 healthcare professionals (E). A carried out analysis was content up of the contents. The study area corresponded to the municipalities of Tulua and Palmira in Colombia. RESULTS The concept of participation was interpreted differently depending on the actor studied: for users and leaders the concept referred to contributing ideas, presence in social spaces, solidarity and frequently, and use of the health services. Healthcare professionals considered the activities carried out by institutions together with the community as social participation, the use of services and affiliation to the health system. Policy markers considered participation to concern evaluation and control of the health services by the community, to improve its quality. CONCLUSIONS The different concepts of participation reveal dif ferences between the content of the policy and how it is understood and interpreted by the different social actors in their interaction with the health services. These different perspectives must be taken into account to develop a link between society and the health services.

Se examinan los comportamientos anticonceptivos de 225 jóvenes universitarios (22'7% hombres y 77'3% mujeres) con el objetivo de conocer su eficacia para evitar embarazos no deseados, SIDA y otras enfermedades de transmisión sexual (ETS). Se recogió información sobre los diferentes métodos anticonceptivos empleados por los sujetos con su última pareja, sin establecer ningún límite temporal. La mitad de los estudiantes no habían tenido nunca relaciones sexuales completas. La mitad de las conductas heterosexuales de los jóvenes sexualmente activos fueron adecuadas para protegerse frente al SIDA, otras ETS y embarazos no deseados (usar siempre preservativo); un tercio de las conductas únicamente eran útiles para evitar embarazos (píldora anticonceptiva) y las restantes (usar ocasionalmente el preservativo o el coitus interruptus ) les pusieron en riesgo frente a los tres problemas. Además, conforme se eleva la edad o el número de parejas de los estudiantes, es más frecuente el uso de píldoras anticonceptivas y menos probable el empleo de preservativo. Estos resultados sugieren que los estudiantes sexualmente activos están más interesados en la prevención de los embarazos no deseados que en evitar infectarse con el SIDA u otras ETS, por lo que seria conveniente promocionar entre ellos el uso del preservativo como método anticonceptivo, destacando sus ventajas frente a la píldora

En una muestra de 119 estudiantes de cuarto de Educación Secundaria Obligatoria (ESO) y primerode Bachillerato (52,6% mujeres) se analizan los conocimientos sobre la prevención de latransmisión sexual del virus del sida, las expectativas de resultados y de autoeficacia respecto a losmétodos preventivos y el tipo de prevención utilizada durante la última relación sexual. Para evitarlas respuestas inducidas respecto a los comportamientos preventivos se emplea un formato de preguntaabierta. Los resultados muestran que sólo el 23,5% de los estudiantes han dado dos respuestastotalmente correctas sobre estrategias consideradas eficaces en la prevención sexual del VIH:uso del preservativo y abstinencia (por este orden). El 70,5% valoran totalmente o muy eficaz elpreservativo para evitar la transmisión sexual del VIH y el 95% de los que dan la segunda respuestajuzgan totalmente eficaz la práctica de la abstinencia con la misma finalidad. En el caso del preservativose sienten totalmente o muy capaces de usarlo el 64,3%, mientras que cuando se trata de laabstinencia sólo se perciben con esa competencia el 20%. Por lo que se refiere al uso autoinformadode métodos preventivos en la última relación, por parte de los 29 estudiantes que tuvieronactividad sexual durante el mes anterior, se observa que 21 de ellos emplearon el preservativo, dosla píldora anticonceptiva, otros dos no precisan el tipo de precaución y el resto no tomó ninguna.Tanto el reducido nivel de conocimientos sobre prevención, como la baja percepción de autoeficaciapara mantenerse abstinentes, nos alertan sobre la necesidad de hacer un mayor esfuerzo de informaciónpara eliminar creencias equivocadas, como por ejemplo: sobre la pretendida eficacia protectorade tener relaciones sexuales con una pareja estable o conocida. Así mismo, conviene insistiren el uso del preservativo como anticonceptivo de elección entre los adolescentes

Se revisan diferentes formas en que la influencia social puede incidir sobre los comportamientosheterosexuales de prevención de la transmisión del VIH de los jóvenes y se presentanlos resultados de algunos trabajos de las autoras, así como de otros investigadores, en que seanalizan dichas relaciones. Se concluye resaltando: 1) la utilidad clínica de la evaluación de lasexpectativas de autoeficacia para poder intervenir específicamente en aquellas áreas en que losjóvenes se perciban con menores capacidades para ser preventivos, 2) la relación observadaentre el uso de preservativo autoinformado y la creencia en su aceptación por parte de los referentessociales más cercanos y 3) la conveniencia de que los jóvenes posean suficientes habilidadesde comunicación que les permitan negociar con éxito el uso del preservativo y les ayudena compensar posibles influencias sociales en contra de su empleo

This paper presents a first approach of Evaluation Engine Architecture (EEA) as proposal to support adaptive integral assessment, in the context of a virtual learning environment. The goal of our research is design an evaluation engine tool to assist in the whole assessment process within the A2UN@ project, linking that tool with the other key elements of a learning design (learning task, learning resources and learning support). The teachers would define the relation between knowledge, competencies, activities, resources and type of assessment. Providing this relation is possible obtain more accurate estimations of student's knowledge for adaptive evaluations and future recommendations. The process is supported by usage of educational standards and specifications and for an integral user modelling

Usability is critical to consider an interactive software system successful. Usability testing and evaluation during product development have gained wide acceptance as a strategy to improve product quality. Early introduction of usability perspectives in a product is very important in order to provide a clear visibility of the quality aspects not only for the developers, but also for the testing users as well. However, usability evaluation and testing are not commonly taken into consideration as an essential element of the software development process. Then, this paper exposes a proposal to introduce usability evaluation and testing within a software development through reuse of software artifacts. Additionally, it suggests the introduction of an auditor within the classification of actors for usability tests. It also proposes an improvement of checklists used for heuristics evaluation, adding quantitative and qualitative aspects to them
