RESUMO Objetivos: A presente investigação teve como principais objetivos descrever a qualidade subjetiva do sono e as perturbações do sono e analisar a intensidade dos sintomas depressivos e dos sentimentos de solidão em idosos institucionalizados; comparar estes dados com um grupo de idosos não institucionalizados e analisar a relação entre estas variáveis nos dois grupos. Métodos: Este estudo insere-se no Projeto Trajetórias do Envelhecimento de Idosos em Resposta Social de onde foi retirada uma amostra de cento e quarenta idosos sem défice cognitivo, com 70 institucionalizados e 70 não institucionalizados emparelhados por idade, sexo, escolaridade e estado civil. A média de idades foi de 76,58 (DP = 6,10), incluindo 104 mulheres e 36 homens. Como instrumentos foram utilizados um Questionário Sociodemográfico, o Questionário sobre o Sono na Terceira Idade, a Escala Geriátrica de depressão e a Escala de Solidão da Universidade da Califórnia, Los Angeles. Resultados: Verificou-se que os idosos institucionalizados apresentavam mais sentimentos de solidão do que os não institucionalizados. Contudo, não se verificaram diferenças entre os dois grupos em relação aos sintomas depressivos, qualidade subjetiva do sono ou perturbações do sono, com algumas exceções: os idosos residentes na comunidade mostraram ter a perceção de demorar mais tempo a adormecer, de acordar mais cedo e de ter mais pesadelos. Através de uma análise correlacional verificou-se, na amostra global, que quanto pior a qualidade subjetiva do sono mais sintomas depressivos se observavam e quanto mais sintomas depressivos, mais sentimentos de solidão, não havendo, contudo, relação entre o sono e a solidão. Conclusões: Concluímos que a situação de institucionalização se acompanha de mais sentimentos de solidão, mas não de sintomas depressivos ou de pior qualidade de sono. Por esse motivo, sugere-se que se desenvolvam programas de intervenção dirigidos à solidão em idosos institucionalizados. ABSTRACT Aims: The main objectives of this investigation were to describe the subjective quality of sleep and sleep disorders and analyze the intensity of depressive and loneliness symptoms in institutionalized elderly people; compare these data against a non-institutionalized elderly people subsample and analyze the relationship between these variables in both subsamples. Method: This study is part of Trajectories of Elderly Aging in Social Response Project from which a sample of one hundred and forty elderly people with no cognitive impairment was taken, with 70 institutionalized and 70 non-institutionalized matched by age, sex, education, and marital status. The average age was 76.58 (SD = 6.10), including 104 women and 36 men. The tools used for this analysis were a sociodemographic questionnaire, the Questionnaire About Sleep in the Older Adults, Geriatric Depression Scale, and the Loneliness Scale of the University of California, Los Angeles. Results: The study confirmed that institutionalized elderly people had more feelings of loneliness than those non-institutionalized. However, there were no differences between the two subsamples regarding depressive symptoms and subjective sleep quality or sleep disturbances, with some exceptions: Elderly people living in the community showed to have the perception of taking more time to fall asleep, waking up earlier, and having more nightmares. Through a correlational analysis it was found, in both subsamples, that the worse the subjective sleep quality the more depressive symptoms were observed; and the more depressive symptoms, the more feelings of loneliness, despite of not existing a relation between sleep and loneliness. Conclusions: We concluded that institutionalization is linked to more feelings of loneliness but not to depressive symptoms nor to worse quality of sleep. For this reason, it is suggested that intervention programs are developed with a focus on elderly institutionalized populations.

Several studies have found that fatigue is one of the most commonly reported symptoms after stroke and the most difficult to cope with. The present study aimed to investigate the presence and severity of self-reported fatigue six years after stroke onset and associated factors. The cohort "Life After Stroke Phase I" (n = 349 persons) was invited at six years to report fatigue (Fatigue Severity Scale 7-item version), perceived impact of stroke and global recovery after stroke (Stroke Impact Scale), anxiety and depression (Hospital Anxiety and Depression Scale), life satisfaction (Life Satisfaction Checklist) and participation in everyday social activities (Frenchay Activities Index). At six years 37% of the 102 participants in this cross-sectional study reported fatigue. The results showed that in nearly all SIS domains the odds for post-stroke fatigue were higher in persons with a higher perceived impact. Furthermore, the odds for post-stroke fatigue were higher in those who had experienced a moderate/severe stroke and had signs of depression and anxiety. Fatigue is still present in one-third of persons as long as six years after stroke onset and is perceived to hinder many aspects of functioning in everyday life. There is an urgent need to develop and evaluate interventions to reduce fatigue.

A velhice pode estar associada ao sofrimento, aumento da dependência física, declínio funcional, isolamento social, depressão e improdutividade. No envelhecimento observam-se lentificação no processamento cognitivo, redução da atenção, dificuldades na retenção das informações aprendidas (memória de trabalho) e diminuição na velocidade de pensamento e habilidades visuoespaciais. Por outro lado, as que se mantêm inalteradas são: inteligência verbal, atenção básica, habilidade de cálculo e a maioria das habilidades de linguagem (Moraes, Moraes & Lima, 2010). O objetivo deste estudo é comparar funções executivas com grau de funcionalidade para averiguar em que medida estas variáveis predizem funcionalidade. Trinta idosos de três valências diferentes constituíram a amostra deste estudo. Os instrumentos de avaliação administrados foram os seguintes: Escala de Barthel, MontrealCognitiveAssessment (MoCA), Trail Making Test (TMT), Teste de Aprendizagem Audio-Verbal de Rey (RAVLT), Figura Complexa de Rey, Teste Stroop de Cores e Palavras (TSCP), DigitSpan, Escala Geriátrica de Depressão. Dos resultados obtidos destacam-se a existência de relações estatisticamente significativas entre a saúde mental e a funcionalidade. Quanto melhor é a saúde mental, maior é o grau de funcionalidade e os participantes do “Domicílio” possuem melhor saúde mental, atenção, planeamento e construção visuo-espacial do que os do “Centro de Dia”, e estes melhor do que os do “Lar”. A Organização Mundial de Saúde (OMS) destaca a capacidade funcional e a independência como fatores preponderantes para o diagnóstico de saúde física e mental na população idosa. Alguns autores indicam que a avaliação cognitiva deve ser sempre acompanhada de uma avaliação funcional e vice-versa.
Centro de dia e lar: saúde mental de idosos e capacidade para o trabalho dos seus cuidadores formais

O Envelhecimento da população é uma realidade cada vez mais presente na nossa sociedade. A investigação junto da população idosa e dos seus cuidadores requer que sejam criadas condições para que, estes grupos, possam usufruir de uma boa qualidade de vida. Propõe-se analisar a capacidade de trabalho dos cuidadores formais de idosos em contexto institucional mas com modalidades de trabalho distintas, no Centro de Dia, onde praticam um horário diurno e fixo, em paralelo com o horário por turnos rotativos diurnos/noturnos praticados no Lar. Foi proposto também analisar o estado mental dos idosos dessas mesmas instituições. A amostra deste estudo contou com 90 participantes dos quais 50 idoso e 40 cuidadores formais. Utilizou-se para a recolha de dados com os idosos o MMSE – Mini-Mental State Examination e a GDS-30 – Escala de Depressão Geriátrica, com os cuidadores a Escala de Graffar e o ICT – Índice de Capacidade para o Trabalho. Os resultados demonstraram não existirem diferenças significativas ao nível da demência e da depressão entre os idosos do Lar e do Centro de Dia. Outros resultados refletiram, para os cuidadores formais, uma capacidade para o trabalho excelente, ligeiramente superior aos dados de referência. Não foi conseguida uma relação entre a saúde mental dos idosos e a capacidade de trabalho dos seus cuidadores formais o que pode retratar o sucesso das medidas de apoio e educação desenvolvidas nesta área.

Purpose: Identify predictors and normative data for quality of life (QOL) in a sample of Portuguese adults from general population Methods: A cross-sectional correlational study was undertaken with two hundred and fifty-five (N=255) individuals from Portuguese general population (mean age 43yrs, range 25-84yrs; 148 females, 107 males). Participants completed the European Portuguese version of the World Health Organization Quality of Life short-form instrument (WHOQOL-Bref) and the European Portuguese version of the Center for Epidemiologic Studies Depression Scale (CES-D). Demographic information was also collected. Results: Portuguese adults reported their QOL as good. The physical, psychological and environmental domains predicted 44% of the variance of QOL. The strongest predictor was the physical domain and the weakest was social relationships. Age, educational level, socioeconomic status and emotional status were significantly correlated with QOL and explained 25% of the variance of QOL. The strongest predictor of QOL was emotional status followed by education and age. QOL was significantly different according to: marital status; living place (mainland or islands); type of cohabitants; occupation; health. Conclusions: The sample of adults from general Portuguese population reported high levels of QOL. The life domain that better explained QOL was the physical domain. Among other variables, emotional status best predicted QOL. Further variables influenced overall QOL. These findings inform our understanding on adults from Portuguese general population QOL

Objectives To determine whether a home based exercise programme can improve outcomes in patients with knee pain. Design Pragmatic, factorial randomised controlled trial of two years' duration. Setting Two general practices in Nottingham. Participants 786 men and women aged >45 years with self reported knee pain. Interventions Participants were randomised to four groups to receive exercise therapy, monthly telephone contact, exercise therapy plus telephone contact, or no intervention. Patients in the no intervention and combined exercise and telephone groups were randomised to receive or not receive a placebo health food tablet. Main outcome measures Primary outcome was self reported score for knee pain on the Western Ontario and McMaster universities (WOMAC) osteoarthritis index at two years. Secondary outcomes included knee specific physical function and stiffness (scored on WOMAC index), general physical function (scored on SF36 questionnaire), psychological outlook (scored on hospital anxiety and depression scale), and isometric muscle strength. Results 600 (76.3%) participants completed the study. At 24 months, highly significant reductions in knee pain were apparent for the pooled exercise groups compared with the nonexercise groups (mean difference –0.82, 95% confidence interval –1.3 to –0.3). Similar improvements were observed at 6, 12, and 18 months. Regular telephone contact alone did not reduce pain. The reduction in pain was greater the closer patients adhered to the exercise plan. Conclusions A simple home based exercise programme can significantly reduce knee pain. The lack of improvement in patients who received only telephone contact suggests that improvements are not just due to psychosocial effects because of contact with the therapist.

Background: Over the last 10 years, Australia's spontaneous vaginal birth rate has decreased approximately 1% each year and the caesarean section rate has increased approximately 1% each year. This trend has serious implications for the health of women and babies. As midwives we are the caretakers of normal birth and therefore partly responsible for its decline and the solution to its decline. Although antenatal education is in a potentially powerful position to promote normal birth, a structured review conducted as part of this thesis found that it does not realise that potential. Currently framed in pathogenesis, antenatal education in particular and maternity services in general, are in need of reframing. The theory of salutogenesis may offer a new lens as it focuses on health rather than illness. Sense of coherence is the cornerstone of salutogenesis and is a predictive indicator of health. What is unclear is the role pregnant women's sense of coherence plays in their birthing outcomes. This study explored associations between pregnant women's sense of coherence, their pregnancy choices, their anticipated labour choices, their labour and birthing outcomes as well as factors associated with modification to sense of coherence from the antenatal to postnatal periods. Methods: After a comprehensive review of the literature, questionnaire development and psychometric tool testing and modification, a longitudinal survey was conducted where eligible women completed a questionnaire before the 30th week of pregnancy (Phase One) and approximately 8 weeks after birth (Phase Two). Eligible women were less than 30 weeks pregnant with a single fetus, could read and write in English and lived in the Australian Capital Territory in Australia. Phase One provided information on women's sense of coherence scores, Edinburgh Postnatal Depression Scale (EPDS) scores, Support Behaviour Inventory (SBI) scores, pregnancy choices including care proVider, planned place of birth, planned birth type and anticipated epidural use and demographics. Phase Two provided information on women's sense of coherence scores, EPDS scores and their labour and birthing outcomes. Findings: 1074 women completed Phase One representing a 61.3% response rate. 753 women completed Phase Two representing a 70.1% retention rate between phases. Compared to women with low sense of coherence, women with high sense of coherence were older, reported fewer pregnancy conditions such as diabetes or hypertension, were less likely to have depressive symptoms, were more likely to feel well supported, were less likely to experience a caesarean section and more likely to experience an assisted vaginal birth. Sense of coherence was not associated with women's pregnancy choices. Higher EPDS scores, lower sense of coherence and greater satisfaction with birth were associated with an increase in women's sense of coherence from the antenatal to the postnatal period. Decreased birth satisfaction and experiencing epidural anaesthesia in labour and assisted vaginal birth were associated with a decrease in sense of coherence from the antenatal to the postnatal period. Conclusion: Strong sense of coherence in pregnant women halved the likelihood of experiencing caesarean section compared to women with low sense of coherence. Sense of coherence is a modifiable predictor of women's childbearing health and was found to be raised by birth satisfaction and lowered by birth dissatisfaction and labour interventions. These important findings add to the limited body of knowledge about sense of coherence and childbearing.

Résumé : Le chant choral serait bénéfique à tout âge : cette étude choisit d’en mesurer les impacts auprès de personnes très âgées. Un devis quantitatif quasi-expérimental à trois groupes fut adopté : la Chorale, l’Hebdo-Bistro (ateliers et conférences, groupe de comparaison), et le groupe Témoin. L’étude longitudinale, intergénérationnelle, comporta trois saisons. La cognition (Mattis, 3MS, Trail Making, empan numérique, fluences formelle, catégorielle), l’humeur (bien-être général, dépression (GDS)), l’autoefficacité (GSES) et l’autonomie (QAF) furent mesurées à trois reprises (pré, post, 2e post). En outre, des mesures hebdomadaires furent administrées concernant la santé physique (consultations médicales, médicaments, chutes) et la participation sociale (activités). L’analyse intergroupe ne rapporta aucune différence significative. Les comparaisons intragroupe montrèrent une amélioration significative pour la Chorale (3MS et activités sociales), et une tendance d’amélioration pour la Chorale et l’Hebdo-Bistro (fluence formelle). Bien que le petit échantillon (n=21) exclue toute généralisation, les résultats demeurent inspirants en contexte de vieillissement populationnel.

Pretendemos estudar quais os traços desadaptativos de personalidade que se relacionam com os sintomas depressivos, a afectividade positiva e negativa, numa amostra de 187 estudantes universitários, entre os 18 e os 43 anos. Calculámos as correlações entre as escalas de padrões de personalidade e as escalas de padrões graves de personalidade do Inventário Multiaxial clínico de Millon II (MCMI-II) com os sintomas depressivos avaliados pela Escala de Depressão do Centro de Estudos Epidemiológicos e com a afectividade positiva e negativa avaliadas pela Escala de Afectividade Positiva e Afectividade Negativa. Realizámos uma análise da regressão múltipla utilizando as escalas de padrões de personalidade do MCMI-II como predictores e o resultado na CES-O, como variável dependente. Obtivemos uma correlação significativa entre os padrões de personalidade borderline, masoquista, esquizotípico, negativista e evitante e paranóide e os sintomas depressivos. A afectividade negativa apresenta-se também positivamente correlacionada com os padrões dependente e esquizóide. A afectividade positiva correlacionou-se significativa e positivamente com os padrões histriónico e narcisista e negativamente com os padrões esquizóide, esquizotípico, negativista, borderline, evitante e masoquista. Na análise de regressão, o padrão borderline prevê a presença de sintomas depressivos e os padrões sádico e dependente também, mas num sentido negativo. /ABSTRACT: We intendend to study wich personality traits relate to depressiva symptoms, and positive and negative affectivity, in a sample of 187 university students, between 18 and 43 years. We did the correlation between the personality patterns scales and the personality patterns of severe personality with the depressive symptoms measured by the Center for Epidemiologic Studies Depression Scale and the positive and negative affectivity measured by the Positive Affect and Negative Affect Shedule. We did a multiple regression analysis using the personality patterns scale of Millon Clinical Multiaxial lnventory (MCMI-II) as predictors and the result of CES-D, as dependent variable. We obtained a significant correlation between the borderline, masochist, schizotypal, negativistic, avoidant and paranoid personality patterns and the depressive symptoms. Negative affectivity relates also positively with dependent and schizoid patterns. Positive affectivity correlated significant and positively with histrionic and narcissistic personality patterns and negatively with schizoid, schizotypal, negativistic, borderline, avoidant and masochist personality patterns. ln the regression analysis, the borderline pattern predicts the presence of depressive symptoms as well as the sadistic and dependent, but in the negative sense.

The aim of the present study is to test a theory-based model of suicide in a low-risk nonclinical sample. A community sample of convenience of 200 adults, 102 men and 98 women, responded to the Depressive Experiences Questionnaire, the Center for the Epidemiologic Studies of Depression Scale, the Psychache Scale, the Interpersonal Needs Questionnaire, and the Suicide Behaviors Questionnaire Revised. The hypothesized structural equation model, including trait dimensions of self-criticism and neediness, and state dimensions of depression, psychache, perceived burdensomeness, and thwarted belongingness, fit the observed data well and significantly explained 49% of the variance of suicidality.

Event-specific scales commonly have greater power than generalized scales in prediction of specific disorders and in testing mediator models for predicting such disorders. Therefore, in a preliminary study, a 6-item Alcohol Helplessness Scale was constructed and found to be reliable for a sample of 98 problem drinkers. Hierarchical multiple regression and its derivative path analysis were used to test whether helplessness and self-efficacy moderate or mediate the link between alcohol dependence and depression, A test of a moderation model was not supported, whereas a test of a mediation model was supported. Helplessness and self-efficacy both significantly and independently mediated between alcohol dependence and depression. Nevertheless, a significant direct effect of alcohol dependence on depression also remained, (C) 2001 John Wiley & Sons, Inc.

The 24-item Brief Psychiatric Rating Scale (BPRS, version 4.0) enables the rater to measure psychopathology severity. Still, little is known about the BPRS's reliability and validity outside of the psychosis spectrum. The aim of this study was to examine the factorial structure and sensitivity to change of the BPRS in patients with unipolar depression. Two hundred and forty outpatients with unipolar depression were administered the 24-item BPRS. Assessments were conducted at intake and at post-treatment in a Crisis Intervention Centre. An exploratory factor analysis of the 24-item BPRS produced a six-factor solution labelled "Mood disturbance", "Reality distortion", "Activation", "Apathy", "Disorganization", and "Somatization". The reduction of the total BPRS score and dimensional scores, except for "Activation", indicates that the 24-item BPRS is sensitive to change as shown in patients that appeared to have benefited from crisis treatment. The findings suggest that the 24-item BPRS could be a useful instrument to measure symptom severity and change in symptom status in outpatients presenting with unipolar depression.
