Introduction: The stair-climbing test (SCT) is considered a submaximal test of simple implementation and easy access, which assesses the individual’s functional capacity. Although widely used in surgical patients, there is no standardization regarding the height of the stair and verbal stimulus. Objectives: It was determine if verbal stimulus changes the stair-climbing time (SCt) in individuals over 50 years-old. We compared oxygenation, blood pressure, pulse, respiratory rate and Borg scale between SCT performed with and without stimulation. Methods: We evaluated individuals with ages greater than 50 years-old that performed two STC (with and without verbal stimulation), in the stair with 44 steps, achieving 7.04 m in height and recording the time to climb the stair. The systolic blood pressure (SBP) and the diastolic blood pressure (DBP), respiratory rate (RR), pulse, oxygen saturation (SpO2 ) and Borg scale were evaluated before and after the tests. The time in the SCT with and without stimulation were compared using the Student test-t and the other variables were compared using the ANOVA test (p<0.05). Results: The average age was 59.75±6,40 years old in the 21 evaluated individuals. The time in the SCT without stimulus was significantly higher than the one with stimulus. The SBP, pulse, RR and Borg scale were significantly increased when compared to the SCT with and without stimulus. Oxygenation and DBP didn´t have significant differences at any time of the study. Conclusions: The time in the SCT was lower when performed with verbal stimulus. The SBP, pulse, RR and Borg scale changed significantly after the SCT with and without stimulus, remarking that this change was greater in the SCT with stimulus.

The six-minute walking test can be affected by several variables, among them, the respiratory and peripheral muscle strength. The objective of this study was to correlate the respiratory muscle strength (maximum inspiratory pressure – MIP; and maximum expiratory pressure – MEP) and the hand grip test (HGT)with the six minute walking test distance in institutionalized elderly. It was included in this study 9 institutionalized elderly with age over 70 years old, both gender, evaluated by means of manometer, dynamometry and six minute walking test. The data were correlated by using the Pearson’s correlation test and the Spearman correlation test. From the evaluated elderly, it was four men and five women, with mean age: 78.8±7.3 years old, MIP: 75.7±33.6 cmH2O, MEP: 62.4±25.0 cmH2O, HGT: 20.4±6.2 kgf (right member) and 20.7±6.8 kgf (left member) and the distance on six minute walking test: 238.5±99.0 meters. There was correlation between expiratory strength and hand grip of both members with the distance on the six minute walking test. It is possible to conclude that there are correlation between expiratory muscle strength with the hand grip test and the functional capacity in institucionalized elderly.

Oxidative stress is related with physiopathology of diabetes mellitus type II and with its secondary complications, such as diabetic nephropathy. Thus, the purpose of this study was to examine the effects of n-acetylcysteine NAC, an antioxidant from Allium cepa, on oxidative stress, morphometrical and nutritional parameters and basal metabolism and energetic substrate utilization, serum glucose and oral glucose tolerance test (OGTT), and renal function of high-sucrose intake rats. Animals were initially divided into three groups. Rats in the control group (C; n=10) were given free access to a standard chow and water; (S; n=10) group received standard chow and 30% sucrose in its drinking water; (SN; n=5) group received standard chow and 2g/L NAC and 30% sucrose in its drinking water. After 25 days of treatments, rats were submitted to indirect calorimetry in fasted and feed states. After 30 days of treatments, rats from C and S groups were divided into four groups of five rats each. The (CC) and (SS) groups were given an intra-gastric dose 0,6mL saline (0,9%NaCl) and the (CNAC) and (SNAC) groups were treated with a intra-gastric dose of 0,6mL NAC (160g/day), and then everybody received a intra-gastric dose of glucose solution (20%) for the OGTT. Animals of S and SN groups have higher liquid consumption and lower food consumption than C group. Calorimetric analyses confirm that despite of the final body weight had not statistical difference among groups, S group have lower resting metabolic rate when compared to C and SN animals. Besides, S group has higher respiratory quotient, higher carbohydrate oxidation and lower lipid oxidation, both in fasted and feed states, than C and SN groups, evidencing the beneficial effect of NAC. Fasting plasma glucose is increased in SS and SNAC animals when compared to CC and CNAC, however SN group has glicemic level at 30 min before OGTT decreased when ...(Complete abstract click electronic access below)

Hypothyroidism is a common endocrine disorder in dogs caused by insufficient production and secretion of thyroid hormones. Most affected dogs have primary hypothyroidism that results from lymphocytic thyroiditis, idiopathic thyroid atrophy, or more rarely, neoplastic destruction. Secondary hypothyroidism resulting from inadequate secretion of thyrotropin (thyroid-stimulating hormone –TSH) from the pituitary gland is less commonly recognized. Tertiary hypothyroidism resulting from a deficiency of hypothalamic thyrotropin-releasing hormone (TRH) has not been documented in dogs. The diagnosis of hypothyroidism in dogs is made on the basis of clinical findings, results of routine laboratory and thyroid gland function tests and response to thyroid hormone replacement. Unfortunately, these tests have high sensitivity, but low specificity, for use in the diagnosis of hypothyroidism. Thyroid hormone supplementation is indicated for the treatment of confirmed hypothyroidism and for the diagnoses of the disease through clinical response to trial therapy

Canine Pyometra is a uterine disease that occurs in sexually mature bitches, with higher incidence in nulliparous and animals over 4 years and is characterized by presenting an accumulation of pus in the uterine lumen, usually occurring in diestrus. Laboratory tests are important tools for the detection of metabolic abnormalities associated with sepsis and renal function, which are serious consequences of pyometra. In blood the main findings are normochromic non-regenerative anemia, presence of dehydration, and sometimes thrombocytopenia. The WBC count may be normal but most often occurs a neutrophilic leukocytosis with a left shift, monocytosis and the presence of toxic neutrophils. In less than 1 / 3 of the animals the presence of azotemia is present and a density lower than 1035 is detected in the urine of almost 90% of bitches which may be in normal range at the onset of the desease. Urinary protein loss is rare but the protein may be elevated in the reagent strip due to urinary contamination by uterine secretion. The increase of gamma-glutamyltransferase (GGT), alkaline phosphatase (ALP), aspartate aminotransferase (AST) and creatine kinase (CK) may be present, indicating disorders in the liver. Currently, additional laboratory tests are being studied for the diagnosis of pyometra and its prognosis, such as the measurement of C-reactive protein and fibrinogen for monitoring the recovery of the inflammatory process and the urine electrophoresis to characterize the origin of proteinuria in these animals . This review will focus on long-term outcomes after acute kidney injury (AKI). Surviving AKI patients have a higher late mortality compared with those admitted without AKI. Recent studies have claimed that long-term mortality in patients after AKI varied from 15% to 74% and older age, presence of previous co-morbidities, and the incomplete recovery of renal function have been identified as risk factors for reduced survival. AKI is also associated with progression to chronic kidney (CKD) disease and the decline of renal function at hospital discharge and the number and severity of AKI episodes have been associated with progression to CKD. IN the most studies, recovery of renal function is defined as non-dependence on renal replacement therapy which is probably too simplistic and it is expected in 60-70% of survivors by 90 days. Further studies are needed to explore the long-term prognosis of AKI patients.

Model Study: An experimental study Introduction: Chronic Kidney Disease (CKD) refers to a syndromic diagnosis which leads to a progressive and irreversible loss of renal function. A hemodialysis patient may have limitations in functional capacity, pulmonary function and respiratory musclular strength impacting in quality of life. Objective: To evaluate the effects of an exercise program on pulmonary function, functional capacity, quality of life and pain in patients undergoing hemodialysis. Methodology: The study included 28 patients of both genders, women and men aged between 40 and 60 years undergoing dialysis at the Kidney Institute, Santa Casa de Misericordia in Presidente Prudente-SP. Primary outcomes included respiratory muscular strength measurements assessed by manovacuometry. The functional capacity was evaluated by a six minute walking test. A life quality questionnaire was applied to evaluate quality of life (SF36-KDQOL). Lung function was evaluated by spirometry. Pain was assessed by a visual analogue scale. The exercise program consisted of training 3 times a week for 40 minutes on hemodialysis during eight weeks. At the end of the program all patients were reassessed. Results: There was no significant difference in the values of FVC and FEV1 before and after the exercise program as well as the index Tiffenau. The value of post MIP was significantly higher than the value obtained in the pre program. For variable MEP no significant difference was found. Functional capacity evaluations showed that there were no significant differences (p> 0.05). The evaluation of quality of life, about the domains of specific areas of CKD showed statistical significance when comparing the list of symptoms and problems with overloading of renal disease and professional role. Indicators related to pain were significantly reduced after the program (P <0.05). Discussion: A chronic kidney patient faces complex situations of physical, social and financial aspects. Although no statistically significant results were found in all variables, the study corroborates to others found in the literature, which suggests that an exercise program can be positive for this population. Conclusion: Although lung capacity and functional capacity did not submit changes to the end of the study, reduced levels of pain, fatigue and dyspnea suggest improvement in functional performance after exercise programs.

In Brazil, the commercial breeding of wild boars (Sus scrofa scrofa) is rapidly expanding. Given the significance of reference values for biochemical analysis for diagnosis of pathological processes and the limitation of data in the literature on this species, Blood samples were collected for determination of biochemical values in 160 wild boars from 180 to 300 days old, clinically healthy farms. The reference range for creatinine was similar to the normal range for swine (1,0 – 2,7mg/dL). There were differences between genders, only for the determination of urea, where the males were higher, although similar to pigs (10 – 30mg/dL). The females, with values from 13.03 to 44.78 mg / dL, do not have statistically significant.
