The thesis is first and foremost the examination of the notion and consequences of ‘state failure’ in international law. The disputes surrounding criteria for creation and recognition of states pertain to efforts to analyse legal and factual issues unravelling throughout the continuing existence of states, as best evidenced by the ‘state failure’ phenomenon. It is argued that although the ‘statehood’ of failed states remains uncontested, their sovereignty is increasingly considered to be dependent on the existence of effective governments. The second part of this thesis focuses on the examinations of the legal consequences of the continuing existence of failed states in the context of jus ad bellum. Since the creation of the United Nations the ability of states to resort to armed force without violating what might be considered as the single most important norm of international law, has been considerably limited. State failure and increasing importance of non-state actors has become a greatly topical issue within recent years in both scholarship and the popular imagination. There have been important legal developments within international law, which have provoked much academic, and in particular, legal commentary. On one level, the thesis contributes to this commentary. Despite the fact that the international community continues to perpetuate a notion of ‘statehood’ which allows the state-centric system of international law to exist, when dealing with practical and political realities of state failure, international law may no longer consider external sovereignty of states as an undeniable entitlement to statehood. Accordingly, the main research question of this thesis is whether the implicit and explicit invocation of the state failure provides sufficient legal basis for the intervention in self-defence against non-state actors in located in failed states. It has been argued that state failure has a profound impact, the extent of which is yet to be fully explored, on the modern landscape of peace and security.

Stormwater management has long been a critical societal and environmental challenge for communities. An increasing number of municipalities are turning to novel approaches such as green infrastructure to develop more sustainable stormwater management systems. However, there is a need to better understand the technological decision-making processes that lead to specific outcomes within urban stormwater governance systems. We used the social-ecological system (SES) framework to build a classification system for identifying significant variables that influence urban stormwater governance decisions related to green infrastructure adoption. To adapt the framework, we relied on findings from observations at national stormwater meetings in combination with a systematic literature review on influential factors related to green infrastructure adoption. We discuss our revisions to the framework that helped us understand the decision by municipal governments to adopt green infrastructure. Remaining research needs and challenges are discussed regarding the development of an urban stormwater SES framework as a classification tool for knowledge accumulation and synthesis.

A adesão à terapêutica medicamentosa por idosos é um fenômeno dependente de fatores diversificados. Defende-se como tese: A adesão aos medicamentos prescritos em idosos em atendimento ambulatorial apresenta-se relacionada aos fatores referentes às características: demográficas, socioeconômicas; dos serviços de saúde, dos profissionais de saúde; das condições de saúde; da terapêutica medicamentosa; comportamentais. Esses fatores estão relacionados aos principais motivos referidos pelos idosos para aderirem ou não aos medicamentos prescritos. Foram objetivos: identificar na literatura brasileira e estrangeira a prevalência de adesão à terapêutica medicamentosa e os fatores relacionados em idosos; caracterizar os idosos em atendimento ambulatorial em um hospital universitário no Rio Grande/RS, Brasil, quanto às características demográficas, socioeconômicas, condições de saúde e uso de medicamentos; identificar os motivos referidos por estes idosos que levavam à adesão/não adesão à terapêutica medicamentosa; verificar a prevalência de adesão à terapêutica medicamentosa nestes idosos; verificar se há associação entre adesão à terapêutica medicamentosa e fatores demográficos, socioeconômicos, condições de saúde, terapêutica medicamentosa e fatores comportamentais destes idosos. Pesquisa realizada por meio de uma revisão integrativa da literatura e de um estudo quantitativo. Na revisão integrativa selecionaram-se 49 artigos e os fatores identificados foram organizados nas categorias: demográficas e socioeconômicas; sistema e profissionais de saúde; condições de saúde; terapêutica medicamentosa; comportamentais. O estudo quantitativo foi exploratório, descritivo, transversal, realizado em um serviço ambulatorial de um hospital universitário no Rio Grande/RS, Brasil. Participaram 107 idosos que responderam ao instrumento para caracterização do idoso e dos fatores relacionados à adesão à terapêutica medicamentosa; ao Miniexame do Estado Mental; à Escala de Medida de Adesão aos Tratamentos. A coleta de dados foi realizada em novembro de 2013. Realizou-se análise estatística descritiva e inferencial. Verificaram-se mais idosos do sexo feminino, na faixa etária entre 60-69 anos. A doença mais prevalente foi a Hipertensão Arterial e a média de uso de medicamentos por dia foi de 4,8. Querer sentir-se bem/manter a saúde e querer controlar a doença e os sintomas foram os motivos para aderir à terapêutica medicamentosa prescrita. A ocorrência de reação adversa e falta de condições financeiras foram os motivos para não aderir. A prevalência de adesão à terapêutica medicamentosa foi de 86,9%. Houve associação entre a adesão e receber orientações do médico sobre como tomar os medicamentos, ter reação adversa, acreditar que os medicamentos são importantes para manutenção da saúde e ter vontade de não tomar os medicamentos. A tese é confirmada em parte: a adesão aos medicamentos prescritos em idosos em atendimento ambulatorial apresenta-se relacionada aos fatores referentes às características: dos profissionais de saúde; das condições de saúde; da terapêutica medicamentosa; comportamentais. Esses fatores estiveram relacionados aos principais motivos referidos pelos idosos para aderirem ou não aos medicamentos prescritos. Por outro lado, a adesão não apresentou relação com os fatores demográficos e socioeconômicos, embora as condições financeiras tenham sido referidas pelos idosos como um motivo que leva à não adesão.

O presente Relatório está contextualizado no plano curricular de estágio do 1º Mestrado em Enfermagem da Escola Superior de Saúde de Portalegre - Instituto Politécnico de Portalegre, na Área de Especialização em Enfermagem Comunitária, que decorreu entre 14 de Fevereiro a 30 de Junho de 2011. O estágio foi dividido em duas áreas de intervenção comunitárias distintas: a primeira na área da educação sexual na adolescência que se realizou na Escola Secundária Mouzinho da Silveira e na Escola Secundária de São Lourenço, surgindo como resposta a um projeto trabalho conjunto entre esta escola e a Escola Superior de Saúde de Portalegre, onde foi realizado por nós um diagnóstico de situação. A segunda intervenção surge na área da promoção da imagem da Escola Superior de Saúde de Portalegre, que visou facilitar as escolhas da vida profissional através da promoção da imagem da escola e do conhecimento da oferta formativa nela existente junto dos alunos do 9º e 12º anos do concelho de Portalegre e na informação para o desenvolvimento e promoção de hábitos de vida saudáveis por parte dos jovens e ainda a demonstração da execução do suporte básico de vida. A população alvo de intervenção foram os alunos do 8º,9º 10º, entre os 13 e os 22 anos, num total de 693 jovens. Os objetivos do estágio foram: adquirir competências tendo por base a metodologia do planeamento em saúde, na avaliação do estado de saúde desta comunidade; bem como: Contribuir para o processo de capacitação desta comunidade. Conhecer os comportamentos e os estilos de vida dos adolescentes, integrados no sistema educativo. Em relação ao relatório de estagio o principal objetivo foi: encontrar respostas honestas e proceder a uma auto-avaliação construtiva e de aprendizagem pessoal (Soares et al 1997: 36)

Marine protection has been emphasized through global and European conventions which highlighted the need for the establishment of special areas of conservation. Classification and habitat mapping have been developed to enhance the assessment of marine environment and improve spatial and strategic planning of human activities and to help on the implementation of ecosystem based management. European Nature information System (EUNIS) is a comprehensive habitat classification system to facilitate the harmonised description and collection of habitat and biotopes that has been developed by the European Environment Agency (EEA) in collaboration with experts from institutions throughout Europe.

Physiological signals, which are controlled by the autonomic nervous system (ANS), could be used to detect the affective state of computer users and therefore find applications in medicine and engineering. The Pupil Diameter (PD) seems to provide a strong indication of the affective state, as found by previous research, but it has not been investigated fully yet. In this study, new approaches based on monitoring and processing the PD signal for off-line and on-line affective assessment (“relaxation” vs. “stress”) are proposed. Wavelet denoising and Kalman filtering methods are first used to remove abrupt changes in the raw Pupil Diameter (PD) signal. Then three features (PDmean, PDmax and PDWalsh) are extracted from the preprocessed PD signal for the affective state classification. In order to select more relevant and reliable physiological data for further analysis, two types of data selection methods are applied, which are based on the paired t-test and subject self-evaluation, respectively. In addition, five different kinds of the classifiers are implemented on the selected data, which achieve average accuracies up to 86.43% and 87.20%, respectively. Finally, the receiver operating characteristic (ROC) curve is utilized to investigate the discriminating potential of each individual feature by evaluation of the area under the ROC curve, which reaches values above 0.90. For the on-line affective assessment, a hard threshold is implemented first in order to remove the eye blinks from the PD signal and then a moving average window is utilized to obtain the representative value PDr for every one-second time interval of PD. There are three main steps for the on-line affective assessment algorithm, which are preparation, feature-based decision voting and affective determination. The final results show that the accuracies are 72.30% and 73.55% for the data subsets, which were respectively chosen using two types of data selection methods (paired t-test and subject self-evaluation). In order to further analyze the efficiency of affective recognition through the PD signal, the Galvanic Skin Response (GSR) was also monitored and processed. The highest affective assessment classification rate obtained from GSR processing is only 63.57% (based on the off-line processing algorithm). The overall results confirm that the PD signal should be considered as one of the most powerful physiological signals to involve in future automated real-time affective recognition systems, especially for detecting the “relaxation” vs. “stress” states.

La tuberculosis TB es una de las principales causas de muerte en el mundo en individuos con infección por VIH. En Colombia esta coinfección soporta una carga importante en la población general convirtiéndose en un problema de salud pública. En estos pacientes las pruebas diagnósticas tienen sensibilidad inferior y la enfermedad evoluciona con mayor frecuencia hacia formas diseminadas y rápidamente progresivas y su diagnóstico oportuno representa un reto en Salud. El objetivo de este proyecto es evaluar el desempeño de las pruebas diagnósticas convencionales y moleculares, para la detección de TB latente y activa pacientes con VIH, en dos hospitales públicos de Bogotá. Para TB latente se evaluó la concordancia entre las pruebas QuantiFERON-TB (QTF) y Tuberculina (PPD), sugiriendo superioridad del QTF sobre la PPD. Se evaluaron tres pruebas diagnósticas por su sensibilidad y especificidad, baciloscopia (BK), GenoType®MTBDR plus (Genotype) y PCR IS6110 teniendo como estándar de oro el cultivo. Los resultados de sensibilidad (S) y especificidad (E) de cada prueba con una prevalencia del 19,4 % de TB pulmonar y extrapulmonar en los pacientes que participaron del estudio fue: BK S: 64% E: 99,1%; Genotype S: 77,8% E: 94,5%; PCRIS6110 S: 73% E: 95,5%, de la misma forma se determinaron los valores predictivos positivos y negativos (VPP y VPN) BK: 88,9% y 94,8%, Genotype S: 77,8% E: 94,5%; PCRIS6110 S: 90% y 95,7%. Se concluyó bajo análisis de curva ROC que las pruebas muestran un rendimiento diagnóstico similar por separado en el diagnóstico de TB en pacientes con VIH, aumentando su rendimiento diagnostico cuando se combinan

Se realizó un estudio observacional retrospectivo longitudinal en una Institución prestadora de Servicios de Salud de la ciudad de Bogotá, con el objetivo de evaluar la efectividad en el manejo del dolor de la terapia con acupuntura en el tratamiento de lumbalgia. Se tomaron 150 historias clínicas de pacientes con lumbalgia atendidos de enero de 2014 a mayo de 2016, las cuales fueron sometidas a los criterios de inclusión definidos por los autores, arrojando 48 historias sometidas a la prueba de Friedman con el fin de identificar el impacto sobre el dolor del tratamiento con acupuntura en los pacientes seleccionados. Adicionalmente, bajo un muestreo aleatorio simple de distribución normal sobre las 48 historias clínicas evaluadas, se seleccionaron 25 casos a los cuales se les aplicó una encuesta no estructurada, con el fin de obtener información sobre el estado de la patología después de finalizar el tratamiento e identificar las posibles causas de deserción. Con este estudio se concluye que la terapia con acupuntura es efectiva en el manejo del dolor de pacientes con lumbalgia, y que es necesario realizar más estudios que puedan sustentar la inclusión de la terapéutica en el manejo de esta patología.

Abstract: The State Rio Grande do Sul is the main producer of Brazilian fine wines, with four viticultural regions. The objective is the characterization of the viticultural climatic potential of the State (total surface of 281.749 km2). The methodology use the Géoviticulture Multicriteria Climatic Classification System (Géoviticulture MCC System), based on three climatic indices ? Dryness Index (DI), Heliotermal Index (HI) and Cool Night Index (CI). Based on latitude, longitude, altitude and distance from Atlantic Ocean, the 3 viticultural climatic indices were modeled and the algorithms applied to a DTM using GIS. The results show that Rio Grande do Sul has the following classes of viticultural climate: according to DI ? Moderately Dry, Sub-humid, Humid; according to HI ? Cool, Temperate, Temperate warm, Warm and Very Warm; according to CI ? Cool nights, Temperate nights, Warm nights. Based on the total surface, the most representatives viticultural climates are: « Humid x Temperate » (3,1%), « Humid x Temperate warm » (14,4%), « Humid x Warm » (52,6%), « Sub-humid x Warm » (20,0%) and « Sub-humid x Very warm » (5,8%). According to CI, the viticultural climates have a range of variation as a function of the interaction between « earlyness of the varieties x heliothermal availability ». Hippocampal sclerosis (HS) is considered the most frequent neuropathological finding in patients with mesial temporal lobe epilepsy (MTLE). Hippocampal specimens of pharmacoresistant MTLE patients that underwent epilepsy surgery for seizure control reveal the characteristic pattern of segmental neuronal cell loss and concomitant astrogliosis. However, classification issues of hippocampal lesion patterns have been a matter of intense debate. International consensus classification has only recently provided significant progress for comparisons of neurosurgical and clinic-pathological series between different centers. The respective four-tiered classification system of the International League Against Epilepsy subdivides HS into three types and includes a term of gliosis only, no-HS. Future studies will be necessary to investigate whether each of these subtypes of HS may be related to different etiological factors or with postoperative memory and seizure outcome. Molecular studies have provided potential deeper insights into the pathogenesis of HS and MTLE on the basis of epilepsy-surgical hippocampal specimens and corresponding animal models. These include channelopathies, activation of NMDA receptors, and other conditions related to Ca(2+) influx into neurons, the imbalance of Ca(2+)-binding proteins, acquired channelopathies that increase neuronal excitability, paraneoplastic and non-paraneoplastic inflammatory events, and epigenetic regulation promoting or facilitating hippocampal epileptogenesis. Genetic predisposition for HS is clearly suggested by the high incidence of family history in patients with HS, and by familial MTLE with HS. So far, it is clear that HS is multifactorial and there is no individual pathogenic factor either necessary or sufficient to generate this intriguing histopathological condition. The obvious variety of pathogenetic combinations underlying HS may explain the multitude of clinical presentations, different responses to clinical and surgical treatment. We believe that the stratification of neuropathological patterns can help to characterize specific clinic-pathological entities and predict the postsurgical seizure control in an improved fashion.

Although cartilaginous tumors have low microvascular density, vessels are important for the provision of nutrition so that the tumor can grow and generate metastasis. The aim of this study was to assess the value of the vascular pattern classification as a prognostic tool in chondrosarcomas (CSs) and its relation with vascular endothelial growth factor (VEGF) expression. This was a retrospective study of 21 enchondromas and 57 conventional CSs. Clinical data and outcome were retrieved from medical files. CSs histologic grades (on a scale of 1 to 3) were determined according to the World Health Organization classification. The vascular pattern (on a scale of A to C) was assessed through CD34, according to Kalinski. CD105 and VEGF were also evaluated. Poor outcome was significantly associated with vascular pattern groups B and C. Higher vascular pattern were 6.5 times more frequent in moderate-grade and high-grade CSs than in grade 1 CS. On multivariate analysis, a clear correlation was found between VEGF overexpression and B/C vascular patterns. Only 18 (benign and malignant) tumors stained for CD105. The results point to the use of the vascular pattern classification as a prognostic tool in CSs and to differentiate low-grade from moderate-grade/high-grade CSs. Vascular pattern might be also used to complement histologic grade, VEGF immunostaining, and microvascular density, for indicating a patient's prognosis. Low-grade CSs develop under low neoangiogenesis, which conforms to the slow growth rate of these tumors.

Herpesvirus reactivation is common after liver transplantation. Analyze the presence of cytomegalovirus (HCMV) and human herpesvirus-6 (HHV-6) DNA in liver donor biopsies, seeking to better understand issues involving human donor leukocyte antigens (HLA)-A, B and DR, as well as correlations with acute cellular rejection. Fifty-nine liver transplantation patients were investigated for the presence of HCMV and HHV-6 DNA in liver donor biopsies, using the Nested-PCR technique. The clinical donor information and HLA matches were obtained from the São Paulo State Transplant System. The recipients' records regarding acute cellular rejection were studied. Seven (11.8%) biopsies were positive for HCMV DNA and 29 (49%) were positive for HHV-6 DNA. In 14 donors with HLA-DR 15 nine had HHV-6 DNA positive liver biopsy with a tendency for significant association (p=0.09), 22 recipients developed acute cellular rejection and 9/22 were positive for HLA-DR 15 (p=0.03; χ(2)=4.51), which was statistically significant in univariate analysis and showed a tendency after multivariate analysis (p=0.08). HHV-6 DNA was prevalent in liver donors studied as well as HLA-DR 15. These findings suggest that patients with HLA-DR 15 in liver donor biopsies develop more rejection after liver transplantation.
