BACKGROUND: Hemicrania continua is a strictly unilateral, continuous headache, typically mild to moderate in severity, with severe exacerbations commonly accompanied by cranial autonomic features and migrainous symptoms. It is exquisitely responsive to Indomethacin. However, some patients cannot tolerate treatment, often due to gastrointestinal side effects. Therapeutic alternatives are limited and controlled evidence lacking. METHODS: We present our experience of nine patients treated with OnabotulinumtoxinA for hemicrania continua. All patients were injected using the PREEMPT (Phase 3 REsearch Evaluating Migraine Prophylaxis Therapy) protocol for migraine. RESULTS: Five of nine patients demonstrated a 50% or more reduction in moderate to severe headache days with OnabotulinumtoxinA with a median reduction in moderate to severe headache days of 80%. Patient estimate of response was 80% or more in five subjects. The median and mean duration of response in the five responders was 11 and 12 weeks (range 6-20 weeks). Improvements were also seen in headache-associated disability CONCLUSIONS: OnabotulinumtoxinA adds a potential option to the limited therapeutic alternatives available in hemicrania continua.

Este trabalho apresenta a síntese e caracterização de cinco ligantes e quatro complexos derivados de oximas e tiossemicarbazonas. Entre essas, discutem-se as estruturas cristalinas/moleculares determinadas por difração de raios-X em monocristais: do ligante 4-feniltiossemicarbazida-isatina (Ligante 5), do complexo piridina-salicilaldeído-4- feniltiossemicarbazona de niquel (II) (Complexo 1), e do bis-4-feniltiossemicarbazonaisatina de chumbo(II) (Complexo 2). A estrutura do Ligante 5 cristaliza no sistema monoclínico, grupo espacial P21/c, com parâmetros de cela a = 6,3227(2) Å, b = 15,7973(7) Å, c = 14,4572(6) Å, β = 93,9330(10)°, V = 1440,61(10) Å3 , Z = 4. O refinamento da estrutura convergiu aos índices de discordância finais R1 = 0,0520, wR2 = 0,1471. Observa-se ainda a ocorrência de interações intermoleculares do tipo ligações de hidrogênio clássicas [N18−H3---O1′ 2,907(2)Å], com a formação de estruturas dímeras inter-relacionadas por simetria dentro da cela cristalina. Para a estrutura cristalina do Complexo 1, observa-se NC=4, e geometria de coordenação quadrada plana, onde o ligante saliciladeído-4-feniltiossemicarbazida comporta-se como quelante tridentado, e completando a esfera de coordenação do centro metálico temos uma molécula de piridina. A estrutura cristaliza no sistema monoclínico, grupo espacial P21/m, parâmetros de cela a = 12,8211(2) Å, b = 5,73370(10) Å, c = 23,9950(4) Å, β = 101,0910(10)°, V = 1730,98(5) Å3 , índices de discordância finais R1= 0,0320, wR2 = 0,0888, Z=3. O Complexo 1 apresenta ainda interações intermoleculares do tipo [N(3)-H(3)---S(1) = 3,5838(17)º, N(3)–H(3A)---S(1) = 160,91(19)º], formando estruturas dímeras e ligação de hidrogênio intramolecular não-clássica do tipo [C(10)-H(10)---N(2) = 2,838(2)º e C(10) – H(10)---N(2) = 122º]. A estrutura cristalina do complexo 2, apresenta duas formas independentes (uma com centro representado por Pb1 e outra por Pb2). Para a unidade com Pb1 temos o complexo composto por duas unidades do Ligante 5, que comportam-se como quelantes tridentados, e a esfera de coordenação é completada por interações intermoleculares do tipo η 2 areno π e através da ligação polarizada com o O1 da moléculas vizinha, o que confere ao íon Pb1 NC=9. A unidade Pb2 apresenta apenas as duas unidades do Ligante 5 coordenadas conferindo-lhe NC=6. A estrutura cristaliza no sistema monoclínico, grupo espacial C2/c, parâmetros de cela a = 37,9747(6) Å, b= 9,51280(10) Å, c = 31,4378(5) Å, β = 125,951(2)°, V= 9193,5(2) Å3 , Z = 4, índices de discordância finais= R1 = 0,0643, wR2 = 0,1227.

Neste trabalho são apresentadas e discutidas as estruturas cristalinas e moleculares do ligante (1), isatina-3-(toluilsulfono-hidrazona), dos complexos [bis(2-acetilpiridina-N4 - benziltiossemicarbazona-N,N,S)Cd(II)], (1), [bis (isatina-3-N4 -benziltiossemicarbazonaN,S)Hg(II)].Etanol, (2) e [bis (isatina-3-N4 -benziltiossemicarbazona-N,S,O)Zn(II)].DMF, (3). Cristais amarelos vítreos do ligante (1) foram obtidos a partir da evaporação lenta de etanol do ensaio de cristalização. Seus dados cristalográficos indicam que duas moléculas interagem através de ligações de hidrogênio do tipo N1-H···O1, formando unidades dímeras. A reação entre 2-acetilpiridina-N4 -benziltiossemicarbazona e Cd(CH3COO)2.2H2O, em presença de etanol, KOH, e após evaporação lenta da mistura de acetona e DMF(2:1), resultou em cristais amarelos do complexo (1). As interações do tipo C(10)-H(10)···S(1)···H(1)-N(4), e N(8)- H(29)⋅⋅⋅S(2) permitem a dimerização do complexo, e a formação de uma cadeia unidimensional. Os cristais laranja do complexo (2) foram obtidos da reação entre o ligante isatina-3-N4 -benziltiossemicarbazona e Hg(NO3)2.H2O, na presença de metanol, KOH, e após evaporação lenta de uma mistura de tolueno e acetona (2:1). As moléculas do complexo (2) estão associadas por ligações de hidrogênio do tipo N(63)-H(4)···O(21), essas interações centrossimétricas conduzem a formação de dímeros. A reação entre o ligante isatina-3-N4 - benziltiossemicarbazona e Zn(CH3COO)2.2H2O, em presença de etanol e KOH resultou em cristais de coloração laranja do complexo (3). A estrutura do complexo apresenta múltiplas ligações de hidrogênio, com formação de dímeros através das interações N1-H1···O1 e C3- H3···N(7). Os dímeros associam-se por interações N4-H4···O2 numa cadeia unidimensional ao longo da direção cristalográfica [100]. A polimerização bidimensional é observada 7 considerando-se as interações do tipo C20-H20···S1, N8-H8···S2 ao longo da direção cristalográfica [010], bem como das interações, N5-H5···O3DMF e C31-H31B···Car, que ocorrem através da molécula de solvente DMF.

O bruxismo caracteriza-se por um ato involuntário de apertar e/ou ranger os dentes sendo este hábito um fator de risco que pode comprometer a função do sistema estomatognático com várias sequelas associadas (dor dentária e muscular, desgaste dentário, etc.). Esta pesquisa bibliográfica pretende rever as possibilidades terapêuticas que se podem apresentar como soluções no controlo desta parafunção. A bibliografia ainda é inconclusiva sobre qual o melhor tratamento a seguir sugerindo uma abordagem multidisciplinar, tendo em conta a etiologia e a sintomatologia para a definição de um plano de tratamento mais completo e adequado. Têm surgido cada vez mais alternativas terapêuticas e cada uma delas, ao longo do tempo, tem desenvolvido novas metodologias mais eficazes no tratamento do bruxismo.

A 2(3-1) factorial experimental design was used to evaluate the performance of a perforated rotating disc contactor to extract alpha-toxin from the fermented broth of Clostridium perfringens Type A by aqueous two-phase system of polyethylene glycol-phosphate salts. The influence of three independent variables, specifically the dispersed phase flowrate, the continuous phase flowrate and the disc rotational speed, was investigated on the hold up, the mass transfer coefficient, the separation efficiency and the purification factor, taken as the response variables. The optimum dispersed phase flowrate was 3.0 mL/min for all these responses. Besides, maximum values of hold up (0.80), separation efficiency (0. 10) and purification factor (2.4) were obtained at this flowrate using the lowest disc rotational speed (35 rpm), while the optimum mass transfer coefficient (0. 165 h(-1)) was achieved at the highest agitation level (140 rpm). The results of this study demonstrated that the dispersed phase flowrate strongly influenced the performance of PRDC, in that both the mass transfer coefficient and hold up increased with this parameter. BACKGROUND: Purification of a-toxin produced by Clostridium perfringens type A in aqueous two-phase systems (ATPS) was studied with a full two-level factorial design on two factors (concentrations of 8000 g mol(-1) PEG and phosphate salt at pH 8.0), to estimate the influence of these factors on the purification results. RESULTS: The partition coefficient (K), purification factor (PF) and activity yield (Y) were strongly influenced by the PEG and phosphate concentrations. Raising the levels of the two factors increased these responses. The highest purification factor (5.7) was obtained with PEG and phosphate concentrations of 17.5% and 15%, respectively. CONCLUSION: These results support the proposal that polymer excluded volume and hydrophobic interactions are the factors that drive the alpha-toxin in PEG/phosphate aqueous two-phase systems. The C-type natriuretic peptide from the platypus venom (OvCNP) exists in two forms, OvCNPa and OvCNPb, whose amino acid sequences are identical. Through the use of nuclear magnetic resonance, mass spectrometry, and peptidase digestion studies, we discovered that OvCNPb incorporates a D-amino acid at position 2 in the primary structure. Peptides containing a D-amino acid have been found in lower forms of organism, but this report is the first for a D-amino acid in a biologically active peptide from a mammal. The result implies the existence of a specific isomerase in the platypus that converts an L-amino acid residue in the protein to the D-configuration. In cattle, a neurological lesion similar to that produced in sheep and goats by Clostridium perfringens type D enterotoxaemia has been reported. However, no causal relationship has been established between this disease and the lesion in cattle. The effects of single and multiple intravenous injections of epsilon toxin in three calves aged 6 months were studied. A further calf was inoculated intravenously with saline solution and used as a control. Epsilon toxin invariably produced neurological signs within 2-60 min of the end of the injection process. Clinical signs consisted of loss of consciousness, recumbency, convulsions, paddling, opisthotonus, hyperaesthesia and dyspnoea. Gross changes consisted of severe acute pulmonary oedema, which was particularly marked in the interlobular septa. The histological lesions consisted of intra-alveolar and interstitial oedema of the lung and variable degrees of perivascular proteinaceous oedema in the internal capsule, thalamus and cerebellar white matter. No clinical or post-mortem changes were observed in the control calf. These results show that calves are susceptible to the intravenous injection of epsilon toxin, and that they can show at least some of the histological lesions produced in sheep and goats by this toxin. Needle-free procedures are very attractive ways to deliver vaccines because they diminish the risk of contamination and may reduce local reactions, pain or pain fear especially in young children with a consequence of increasing the vaccination coverage for the whole population. For this purpose, the possible development of a mucosal malaria vaccine was investigated. Intranasal immunization was performed in BALB/c mice using a well-studied Plasmodium berghei model antigen derived from the circumsporozoite protein with the modified heat-labile toxin of Escherichia coli (LTK63), which is devoid of any enzymatic activity compared to the wild type form. Here, we show that intranasal administration of the two compounds activates the T and B cell immune response locally and systemically. In addition, a total protection of mice is obtained upon a challenge with live sporozoites.

Snake venom (sv) C-type lectins encompass a group of hemorrhagic toxins that are capable of interfering with blood stasis. A very well-studied svC-type lectin is the heterodimeric toxin, convulxin (CVX), from the venom of South American rattlesnake Crotalus durissus terrificus. CVX is able to activate platelets and induce their aggregation by acting via p62/GPVI collagen receptor. By using polymerase chain reaction homology screening, we have cloned several cDNA precursors of CVX subunit homologs. One of them, named crotacetin (CTC) beta-subunit, predicts a polypeptide with a topology very similar to the tridimensional conformations of other subunits of CVX-like snake toxins, as determined by computational analysis. Using gel permeation and reverse-phase high-performance liquid chromatography, CTC was purified from C. durissus venoms. CTC can be isolated from the venom of several C. durissus subspecies, but its quantitative predominance is in the venom of C. durissus cascavella. Functional analysis indicates that CTC induces platelet aggregation, and, importantly, exhibits an antimicrobial activity against Gram-positive and -negative bacteria, comparable with CVX.

The phylogenetic interrelationships of members of the Clostridium botulinum complex of species was investigated by direct sequencing of their 16S rRNA genes. Comparative analysis of the 16S rRNA sequences demonstrated the presence of four phylogenetically distinct lineages corresponding to: i) proteolytic C. botulinum types Al B, and F, and C. sporogenes, ii) saccharolytic types B, E and F, iii) types C and D and C. novyi type A, and iv) type G and C. subterminale. The phylogenetic groupings obtained from the 16S rRNA were in complete agreement with the four divisions recognised within the 'species complex' on the basis of phenotypic criteria.

The inhibitor cystine-knot motif identified in the structure of CSTX-1 from Cupiennius salei venom suggests that this toxin may act as a blocker of ion channels. Whole-cell patch-clamp experiments performed on cockroach neurons revealed that CSTX-1 produced a slow voltage-independent block of both mid/low- (M-LVA) and high-voltage-activated (HVA) insect Ca(v) channels. Since C. salei venom affects both insect as well as rodent species, we investigated whether Ca(v) channel currents of rat neurons are also inhibited by CSTX-1. CSTX-1 blocked rat neuronal L-type, but no other types of HVA Ca(v) channels, and failed to modulate LVA Ca(v) channel currents. Using neuroendocrine GH3 and GH4 cells, CSTX-1 produced a rapid voltage-independent block of L-type Ca(v) channel currents. The concentration-response curve was biphasic in GH4 neurons and the subnanomolar IC(50) values were at least 1000-fold lower than in GH3 cells. L-type Ca(v) channel currents of skeletal muscle myoballs and other voltage-gated ion currents of rat neurons, such as I(Na(v)) or I(K(v)) were not affected by CSTX-1. The high potency and selectivity of CSTX-1 for a subset of L-type channels in mammalian neurons may enable the toxin to be used as a molecular tool for the investigation of this family of Ca(v) channels.

Collagen XVIII can generate two fragments, NC11-728 containing a frizzled motif which possibly acts in Wnt signaling and Endostatin, which is cleaved from the NC1 and is a potent inhibitor of angiogenesis. Collagen XVIII and Wnt signaling have recently been associated with adipogenic differentiation and obesity in some animal models, but not in humans. In the present report, we have shown that COL18A1 expression increases during human adipogenic differentiation. We also tested if polymorphisms in the Frizzled (c.1136C>T; Thr379Met) and Endostatin (c.4349G>A; Asp1437Asn) regions contribute towards susceptibility to obesity in patients with type 2 diabetes (113 obese, BMI =30; 232 non-obese, BMI < 30) of European ancestry. No evidence of association was observed between the allele c.4349G>A and obesity, but we observed a significantly higher frequency of homozygotes c.1136TT in obese (19.5%) than in non-obese individuals (10.9%) [P = 0.02; OR = 2.0 (95%CI: 1.07-3.73)], suggesting that the allele c.1136T is associated to obesity in a recessive model. This genotype, after controlling for cholesterol, LDL cholesterol, and triglycerides, was independently associated with obesity (P = 0.048), and increases the chance of obesity in 2.8 times. Therefore, our data suggest the involvement of collagen XVIII in human adipogenesis and susceptibility to obesity.
