Despite numerous government projects aimed at reorganizing and qualifying obstetric and neonatal care in Brazil, it remains problematic, with repercussions for maternal and newborn mortality and humanized care of both the mother and child. The objective of this study was to analyze the care provided to women during the pregnancy-puerperium cycle, based on reports of public health service users regarding their pregnancy and delivery experiences, using comprehensiveness and humanization as reference. The study applied a qualitative approach and the methodological strategy consisted of listening to the women, in order to identify, based on the meanings of their discourse concerning their experiences with health services, continuities and discontinuities of care during the pregnancy-puerperium cycle. Study participants were women who gave birth at a municipal public maternity, residents of Natal, Brazil, who at the time of the interviews, were between 10 and 42 days postpartum. Seven women reported their pregnancy and delivery experiences at public services. As interviews and observation took place, the material produced was also analyzed, in order to achieve simultaneous production and data analysis. Using systematization, a dialogue was established between the women’s discourses and production in the field of Collective Health, with respect to concepts and discussion about obstetric and neonatal care as well as the Comprehensiveness and Humanization of such care. Participant discourses underscored aspects related to prenatal care starting at pregnancy and its repercussions as well as prenatal monitoring by health services; aspects associated with care during labor and delivery, as well as those involved in postpartum in the maternity, both with respect to newborn and maternal careç and lastly, puerperium care after discharge from the maternity. Analysis of results sought to identify lines of continuity and discontinuity in the comprehensiveness and humanization of care. Based on these lines and as final contributions of the study, the following paths were proposed to achieve comprehensive and humanized production of health care for women during the pregnancy-puerperium cycle: Path 1- Reassess care in the maternal and newborn health network, aimed at comprehensiveness in terms of guaranteeing access to the various services and technological resources available to enhance health and life. Path 2- Reorganize work processes in order to attain comprehensive and humanized care for women in the pregnancy-puerperium cycle. Path 3 – Qualify the professional-user relationship in care management during the pregnancy-puerperium cycle. Path 4 – Invest in the qualification of communication processes in the different dimensions of care during the pregnancy-puerperium cycle.

This research was an exploratory pilot study on the use and need of solution-focused practices in supervision according to child welfare workers and child welfare supervisors in Nova Scotia, Canada. Child welfare workers and supervisors were invited to participate in an online questionnaire exploring the use and need of solution-focused practices in child welfare supervision. The collected data indicated that these practices were being implemented and that workers and supervisors believe there is a need for them. Further research and training on solution-focused practices in the supervision of child welfare workers could be beneficial to help create the most productive supervision experience that positively impacts everyone involved.

Commonly used paradigms for studying child psychopathology emphasize individual-level factors and often neglect the role of context in shaping risk and protective factors among children, families, and communities. To address this gap, we evaluated influences of ecocultural contextual factors on definitions, development of, and responses to child behavior problems and examined how contextual knowledge can inform culturally responsive interventions. We drew on Super and Harkness' "developmental niche" framework to evaluate the influences of physical and social settings, childcare customs and practices, and parental ethnotheories on the definitions, development of, and responses to child behavior problems in a community in rural Nepal. Data were collected between February and October 2014 through in-depth interviews with a purposive sampling strategy targeting parents (N = 10), teachers (N = 6), and community leaders (N = 8) familiar with child-rearing. Results were supplemented by focus group discussions with children (N = 9) and teachers (N = 8), pile-sort interviews with mothers (N = 8) of school-aged children, and direct observations in homes, schools, and community spaces. Behavior problems were largely defined in light of parents' socialization goals and role expectations for children. Certain physical settings and times were seen to carry greater risk for problematic behavior when children were unsupervised. Parents and other adults attempted to mitigate behavior problems by supervising them and their social interactions, providing for their physical needs, educating them, and through a shared verbal reminding strategy (samjhaune). The findings of our study illustrate the transactional nature of behavior problem development that involves context-specific goals, roles, and concerns that are likely to affect adults' interpretations and responses to children's behavior. Ultimately, employing a developmental niche framework will elucidate setting-specific risk and protective factors for culturally compelling intervention strategies.

Care has come to dominate much feminist research on globalized migrations and the transfer of labor from the South to the North, while the older concept of reproduction had been pushed into the background but is now becoming the subject of debates on the commodification of care in the household and changes in welfare state policies. This article argues that we could achieve a better understanding of the different modalities and trajectories of care in the reproduction of individuals, families, and communities, both of migrant and nonmigrant populations by articulating the diverse circuits of migration, in particular that of labor and the family. In doing this, I go back to the earlier North American writing on racialized minorities and migrants and stratified social reproduction. I also explore insights from current Asian studies of gendered circuits of migration connecting labor and marriage migrations as well as the notion of global householding that highlights the gender politics of social reproduction operating within and beyond households in institutional and welfare architectures. In contrast to Asia, there has relatively been little exploration in European studies of the articulation of labor and family migrations through the lens of social reproduction. However, connecting the different types of migration enables us to achieve a more complex understanding of care trajectories and their contribution to social reproduction.

This article draws attention to the importance of routinely collected administrative data as an important source for understanding the characteristics of the Northern Ireland child welfare system as it has developed since the Children (Northern Ireland) Order 1995 became its legislative base. The article argues that the availability of such data is a strength of the Northern Ireland child welfare system and urges local politicians, lobbyists, researchers, policy-makers, operational managers, practitioners and service user groups to make more use of them. The main sources of administrative data are identified. Illustration of how these can be used to understand and to ask questions about the system is provided by considering some of the trends since the Children Order was enacted. The “protection” principle of the Children Order provides the focus for the illustration. The statistical trends considered relate to child protection referrals, investigations and registrations and to children and young people looked after under a range of court orders available to ensure their protection and well-being.

Anxiety disorders are the most commonly diagnosed group of mental disorders in children (Kessler et al., 2012). Resiliency, defined as a child’s ability to successfully overcome an adverse event (Newland, 2014) is believed to be comprised of protective factors such as self-esteem and positive coping strategies (Rutter, 1987). These protective factors are related to child anxiety in that their presence or absence may augment or hinder a child’s resiliency towards anxiety-provoking events and situations (Lo Casico, Guzzo, & Pace, 2013; Thorne, Andrews, & Nordstokke, 2013). The FRIENDS for Life (FFL) program is a school-based anxiety prevention program which aims to decrease anxiety and increase resiliency in 8- to 11-year-old children (Barrett & Sonderegger, 2003). Previous studies have shown FFL to be an effective tool in decreasing anxiety and increasing resiliency; however, not all previous studies have utilized control or comparison groups (Brownlee et al., 2013; Neil & Christensen 2007; Stopa, Barrett, & Golingi, 2011). Moreover, existing FRIENDS literature has not previously considered the potential role of parent anxiety in child outcomes. The present study aimed to evaluate child anxiety, resiliency, and parent anxiety in relation to the FFL program while including a no-treatment control group. It was hypothesized that child anxiety would decrease and child resiliency would increase following FFL. Results obtained from a non-identified school-based sample were not entirely consistent with predictions, such that decreases in anxiety and increases in resiliency were observed in both the experimental and control groups.

Enquadramento: A amamentação nutre, protege e favorece o desenvolvimento cognitivo e a criação do vínculo afetivo entre mãe e filho. No entanto, há vários fatores que dificultam a manutenção da amamentação como os psicológicos, socioculturais, profissionais, nível de educação, ação dos profissionais de saúde entre outros. Objetivos: Determinar o perfil sociodemográfico das puérperas; identificar as dificuldades mais frequentes associadas à amamentação ao 7º e 30º dia após o nascimento; analisar a influência das variáveis sociodemográficas associadas às dificuldades na amamentação nos dois momentos de avaliação; avaliar a influência das atitudes maternas e a da vinculação pós-natal nas dificuldades associadas à amamentação nos dois momentos de avaliação. Métodos: Estudo quantitativo, longitudinal em painel de curta duração, descritivo-correlacional, numa amostra não probabilística por conveniência de 255 participantes (média de idades de 30,78 anos). Utilizou-se um questionário para caracterizar sociodemograficamente e os contextos de amamentação da amostra, a escala de atitudes maternas face à amamentação (Pereira, 2004) e a escala de vinculação pós-natal (Airosa, Silva & Bachu, 2012), versão portuguesa de Gomez e Leal (2007). Este foi aplicado às participantes em dois momentos (7º e 30º dias após o nascimento do bebé). Resultados: As participantes são maioritariamente casadas, com o ensino superior, empregadas em tempo completo e residentes na cidade. Quanto às dificuldades mais frequentes associadas à amamentação nos dois momentos de avaliação do estudo (7º e 30º dia de vida do bebé) verificamos que as dificuldades sentidas ao 7º dia se mantêm no 30º, predominando as fissuras (7.º dia 41.7%; 39.3% 30.º dia), as dificuldades na pega (7.º dia 28.8%; 31.9% 30.º dia), o ingurgitamento mamário (7.º dia 16.7%; 14.8% 30.º dia). Em relação à influência das variáveis sociodemográficas nas dificuldades associadas à amamentação ao 7º e 30º dia após o nascimento, apurou-se que as diferenças são estatisticamente significativas em relação à escolaridade (p=0,000), situação profissional (p=0,034) e local de residência (0,042), sugerindo que estas variáveis têm poder explicativo sobre as dificuldades sentidas na amamentação pelas mães nos dois momentos de avaliação. Nenhuma das dimensões relativas às atitudes maternas face à amamentação e à vinculação pós-natal se assumiu preditora das dificuldades associadas à amamentação. Conclusão: Os resultados sugerem que os enfermeiros, especialmente no âmbito da saúde materna, obstétrica e ginecológica, estejam mais despertos para as dificuldades que as mães possam ter na amamentação sobretudo no primeiro mês, considerado o mais crítico na adaptação ao aleitamento materno, de modo a conseguirem que estas ultrapassem essas complicações, evitando assim, o abandono precoce da amamentação. The aim of this text is to consider the notions of life and art, revealed in Camus’s texts, mainly The Myth of Sisyphus (published in French in 1942) and The Rebel (1951), and extrapolate them to the present state of contemporary art by analysing a few artworks by Damien Hirst, the richest living artist, a modern Sisyphus in the self-awareness he manifests towards the incongruity of his work. Always looking on the absurd side of life dwells on the inevitability of life and art, accepting the fact, according to Camus’s words, that: the impossible remains impossible. Instead of denying the meaningless and finding some form of redemption, both French writer and British artist have embraced this potential and have used the absurd as a form of conveying meaning to their art. Hirst’s artworks that will be referred in this text include the series: The Physical Impossibility of Death in the Mind of Someone Living (1991); Mother and Child Divided (1993); For the Love of God (2007) and For Heaven’s Sake (2008).

Micronutrient deficiencies affect individuals mainly in developing countries, where vitamin A deficiency is a public health problem worldwide more worrying, especially in groups with increased physiological needs such as children and women of reproductive age. Vitamin A is supplied to the body through diet and has an important role in the visual process, cell differentiation, maintenance of epithelial tissue, reproductive and resistance to infection. The literature has demonstrated the relationship between vitamin A and diabetes, including gestational, leading to a risk to both mother and child. Gestational diabetes is any decrease in glucose tolerance of variable magnitude diagnosed each the first time during pregnancy, and may or may not persist after delivery. Insulin resistance during pregnancy is associated with placental hormones, as well as excess fat. Studies have shown that retinol transport protein produced in adipose tissue in high concentrations, this would be associated with resistance by interfering with insulin signaling. Therefore, this study aimed to evaluate the concentration of retinol in serum and colostrum from healthy and diabetic mothers in the immediate postpartum period. One hundred and nine parturient women were recruited, representing seventy-three healthy and thirty-six diabetic. Retinol was extracted and subsequently analyzed by High Performance Liquid Chromatography. Among the results highlights the mothers with gestational diabetes were older than mothers healthy, had more children and a higher prevalence of cases of cesarean section. Fetal macrosomia was present in 1.4% of healthy parturient women and in 22.2% of diabetic mothers. The maternal serum retinol showed an average of 39.7 ± 12.5 mg/dL for healthy parturients 35.12 ± 15 mg/dL for diabetic and showed no statistical difference. It was observed that in the group of diabetic had 17% vitamin A deficiency, whereas in the healthy group, only 4% of the women were deficentes. Colostrum, the concentration of retinol in healthy was 131.3 ± 56.2 mg/dL and 125.3 ± 41.9 mg/dL in diabetic did not differ statistically. This concentration of retinol found in colostrum provides approximately 656.5 mg/day for infants born to healthy mothers and 626.5 mg/day for infants of diabetic mothers, based on a daily consumption of 500 mL of breast milk and need Vitamin A 400 mg/day, thus reaching the requirement of the infant. The diabetic mothers showed significant risk factors and complications related to gestational diabetes. Although no 11 difference was found in serum retinol concentration and colostrum among women with and without gestational diabetes, the individual analysis shows that parturients women with diabetes are 4.9 times more likely to develop vitamin A deficiency than healthy parturients. However, the supply of vitamin A to the newborn was not committed in the presence of gestational diabetes

Cette recherche vise à étudier l’impact d’interventions réalisées par les parents dans l’unité néonatale de soins intensifs. Plus spécifiquement, le premier objectif est de documenter les effets différentiels de la Méthode Mère Kangourou « MMK » accompagnée ou non du Massage en incubateur «MI » ou du Massage en Position Kangourou « MPK » et des Soins Traditionnels «ST » accompagnés ou non du massage dans l’incubateur sur la croissance physique mesurée par le poids, la taille et le périmètre crânien pendant une période de 5 et 15 jours dans l’unité néonatale et l’impact à 40 semaines d’âge gestationnel. Le second objectif est de comparer, chez des enfants qui bénéficient de la « MMK » la valeur ajoutée du « MPK » ou du «MI » sur le neuro-développement à 6 et 12 mois d’âge corrigé de l’enfant. Un échantillon total de 198 enfants et leur famille a été recruté de la façon suivante dans trois hôpitaux de Bogota. Dans chaque hôpital, 66 sujets ont été répartis aléatoirement à deux conditions. Ces hôpitaux ont été choisis afin de tester les effets de diverses conditions expérimentales et de diminuer les bais de sélection. Dans chaque hôpital, deux techniques ont été assignées aléatoirement. Il s’agit, dans le premier, de la « MMK & MPK » vs « MMK & MI ». Dans le second, « MMK sans massage » vs « MMK & MI ». Dans le troisième, « MI » a été comparé aux « ST » ce qui implique une absence de contact physique continu des bébés avec leurs parents. Les résultats rapportés dans le premier article sont à l’effet que, dans le premier hôpital, il y a un effet compensatoire de l’intervention « MMK & MPK » sur la perte physiologique du poids de l’enfant prématuré dans les 15 premiers jours de vie avec un impact sur le poids à 40 semaines d’âge gestationnel, sur la durée du portage kangourou et sur la durée d’hospitalisation totale. Aucun effet sur le périmètre crânien ou la taille n’est apparu. Dans le deuxième hôpital, aucune différence significative n’est rapportée pour le poids sauf quand l’intervention est commencée après le 10ième jours de vie alors que l’enfant « MPK» semble grossir mieux que le «MMK avec MI». Finalement, dans le troisième hôpital il n’y a aucun effet du massage sur les variables anthropométriques, le groupe avec MI grossissant moins vite avec un léger impact sur le poids à 40 semaines. Cela pourrait être dû à la perte de chaleur due à l’ouverture de l’incubateur quand l’enfant est très immature. Dans le second article, les 66 enfants de l’hôpital sont répartis aléatoirement dans le groupe « MMK & MPK» vs le groupe « MMK & MI», ont complété, à 6 et 12 mois d’âge corrigé, un test de neuro-développement, le Griffiths. Les résultats à 6 mois ne montrent aucune différence entre les 2 interventions, mais a 12 mois le IQ semble dépendant du nombre de jours d’hospitalisation de l’enfant, cette durée d’hospitalisation correspond au temps que met l’enfant à se stabiliser physiquement et correspond également au temps que mettent la mère et l’enfant à s’adapter à la méthode kangourou. Une fois, l’adaptation kangourou réussie, la dyade mère enfant sort avec l’enfant toujours en position kangourou. Le temps d’hospitalisation correspond au temps que met l’enfant à être éligible à l’apprentissage de la MMK par la mère. À 12 mois les deux groupes montrent des résultats équivalents, mais des différences positives sont apparues pour le groupe « MMK & MPK» dans les sous échelle Coordination Oculo Manuelle et Audition et Langage du test Griffiths. Dans l’ensemble, les résultats suggèrent que la pratique des deux interventions non traditionnelles peut contribuer à une meilleure croissance physique dans nos cohortes. Le gain de poids du bébé, notamment, est affecté par l’intervention MPK (Hôpital 1) ou sans l’ajout du Massage (Hôpital 2). Par ailleurs, le massage en incubateur n’a pas de différence significative en comparaison aux soins traditionnels, ces interventions ont toutefois un impact mineur (tendances) sur le neuro développement à 6 et 12 mois d’âge corrigé dans cette étude.

São muitas as vantagens do aleitamento materno para a mãe e recém-nascido, havendo consenso de que a sua prática exclusiva deve ser até aos seis meses de vida. A intervenção teve como objetivo empoderar a puérpera para o sucesso no aleitamento materno. Foram aplicados três questionários a duas populações-alvo: um aos enfermeiros do Serviço de Obstetrícia do Centro Hospitalar Barreiro-Montijo, Entidade Pública Empresarial e outros dois às puérperas, que permitiram a recolha de informações sobre os conhecimentos dos enfermeiros acerca das práticas no aleitamento materno e da perceção das puérperas, quanto aos cuidados recebidos durante o internamento. Os resultados apontaram para a necessidade de formação dos enfermeiros sobre o aleitamento materno e a necessidade de empoderamento/acompanhamento no pós-parto. Desenvolveram-se atividades formativas e educacionais, quer para os profissionais, quer para as puérperas. A intervenção profissional revelou-se benéfica para a melhoria contínua dos cuidados de enfermagem na promoção do aleitamento materno exclusivo; EMPOWERMENT OF POSTPARTUM WOMEN FOR SUCCESSFUL BREASTFEEDING ABSTRACT: Breastfeeding presents several advantages to the mother and child, being generally agreed that its practice should be exclusive until six months of age. The goal of this intervention was to empower the puerpera towards breastfeeding success. Three questionnaires were applied to two populations: one to the nurses from the Obstetric Department of the Centro Hospitalar Barreiro-Montijo, Entidade Pública Empresarial and other two to puerperas, which allowed collection of information on the nurses’ knowledge about breastfeeding practices and the puerperas’ perception on received care during hospitalization. The results demonstrated the nurses’ need of training on breastfeeding and the need of empowerment/follow-up during the post-partum period. Educational and training activities, directed to professionals and puerperas, were developed. The professional intervention demonstrated benefits to the improvement of the nursing care in the promotion of exclusive breastfeeding.
