Background: Allergy is a form of hypersensitivity to normally innocuous substances, such as dust, pollen, foods or drugs. Allergens are small antigens that commonly provoke an IgE antibody response. There are two types of bioinformatics-based allergen prediction. The first approach follows FAO/WHO Codex alimentarius guidelines and searches for sequence similarity. The second approach is based on identifying conserved allergenicity-related linear motifs. Both approaches assume that allergenicity is a linearly coded property. In the present study, we applied ACC pre-processing to sets of known allergens, developing alignment-independent models for allergen recognition based on the main chemical properties of amino acid sequences.Results: A set of 684 food, 1,156 inhalant and 555 toxin allergens was collected from several databases. A set of non-allergens from the same species were selected to mirror the allergen set. The amino acids in the protein sequences were described by three z-descriptors (z1, z2 and z3) and by auto- and cross-covariance (ACC) transformation were converted into uniform vectors. Each protein was presented as a vector of 45 variables. Five machine learning methods for classification were applied in the study to derive models for allergen prediction. The methods were: discriminant analysis by partial least squares (DA-PLS), logistic regression (LR), decision tree (DT), naïve Bayes (NB) and k nearest neighbours (kNN). The best performing model was derived by kNN at k = 3. It was optimized, cross-validated and implemented in a server named AllerTOP, freely accessible at http://www.pharmfac.net/allertop. AllerTOP also predicts the most probable route of exposure. In comparison to other servers for allergen prediction, AllerTOP outperforms them with 94% sensitivity.Conclusions: AllerTOP is the first alignment-free server for in silico prediction of allergens based on the main physicochemical properties of proteins. Significantly, as well allergenicity AllerTOP is able to predict the route of allergen exposure: food, inhalant or toxin. © 2013 Dimitrov et al.; licensee BioMed Central Ltd.

The appearance of testicular oocytes (TO) in wild fish populations has received considerable attention in the scientific literature and public media. Current methods to quantify TO are lethal; instead, a non-lethal alternative was examined. Laparoscopic insertion into the genital pore allowed internal visualization of the gonad and detection of TO by collecting five testis biopsies in smallmouth bass Micropterus dolomieu and largemouth bass Micropterus salmoides. Overall, biopsies quantified similar levels of TO detection and severity to conventional transverse sectioning with less than 10% mortality. Suitability of surgical anesthetics, tricaine methanesulfonate and electronarcosis were examined in laboratory and field applications. Electronarcosis had the added benefit of rapid sex identification and immediate release of female fish with minimal trauma, representing significant benefits when sampling small or compromised populations. Laparoscopy may be useful for monitoring the prevalence and severity of TO in these fish species when lethal sampling is not a desired outcome.

In this work, we report a 20-ns constant pressure molecular dynamics simulation of the uncharged form of two amino-amide local anesthetics (LA). etidocaine and prilocaine, present at 1:3 LA:lipid, molar ratio inside the membrane, in the hydrated liquid crystal bilayer phase of 1-palmitoyl-2-oleoyl-sn-glycero-3-phosphatidylcholine (POPC). Both LAs induced lateral expansion and a concomitant contraction in the bilayer thickness. A decrease in the acyl chain segment order parameter, -S(CD), compared to neat bilayers, was also observed. Besides, both LA molecules got preferentially located in the hydrophobic acyl chains region, with a maximum probability at similar to 12 and similar to 10 angstrom from the center of the bilayer for prilocaine and etidocaine, respectively. We have examined the effect of the uncharged species of lidocaine (LDC) and etidocaine (EDC) on the acyl chain moiety of egg phosphatidylcholine liposomes. Changes in membrane organization caused by both anesthetics were detected through the use of EPR spin labels (5, 7 and 12 doxyl stearic acid methyl ester) or fluorescence probes (4, 6, 10, 16 pyrene-fatty acids). The disturbance caused by the LA was greater when the probes were inserted in more external positions of the acyl chain and decreased towards the hydrophobic core of the membrane. The results indicate a preferential insertion of LDC at the polar interface of the bilayer and in the first half of the acyl chain, for EDC. Additionally, 2 H NMR spectra of multilamellar liposomes composed by acyl chain-perdeutero DMPC and EPC (1:4 mol%) allowed the determination of the segmental order (S-mol) and dynamics (T-1) of the acyl chain region. In accordance to the fluorescence and EPR results, changes in molecular orientation and dynamics are more prominent if the LA preferential location is more superficial, as for LDC while EDC seems to organize the acyl chain region between carbons 2-8, which is indicative of its positioning. We propose that the preferential location of LDC and EDC inside the bilayers creates a "transient site", which is related to the anesthetic potency since it could modulate the access of these molecules to their binding site(s) in the voltage-gated sodium channel. Las atopias constituyen un problema de salud que puede afectar a adolescentes y adultos, llegando incluso a ser mortal en ciertos casos. La investigación en este tema es escasa en nuestro medio, es por ello que este estudio tiene como objetivo determinar la prevalencia de atopias y los alérgenos inhalantes más comunes entre los adolescentes de octavo y noveno de básica de las áreas urbana y rural del cantón Cuenca, mediante la aplicación de un “skin prick test”. Para ello fueron evaluados los estudiantes de octavo y noveno de básica de los colegios del cantón Cuenca, incuyendo una muestra aleatoria representativa de 991 alumnos. Los resultados demostraron que más de la mitad de los participantes fueron atópicos para al menos uno de los alérgenos probados, situación que refleja un problema de salud pública que debe ser considerado. La prevalencia de atopia no fue diferente de acuerdo al sexo de los participantes; de los 20 alérgenos probados, los alérgenos intra-domiciliarios y las gramíneas estuvieron entre los más prevalentes, los cinco más comunes fueron los ácaros D. pteronyssinus y blomia tropicalis, epitelio de cucaracha, mezcla de pólenes de gramíneas y ecynodon. No se encontró una diferencia significativa en la prevalencia global de atopia según la ubicación geográfica de los colegios. Sin embrago, el análisis por alérgeno reveló que la atopia al epitelio de cucaracha fue mayor entre los alumnos que asisten a colegios rurales y la atopia al salix fue mayor entre los adolescentes de los colegios urbanos

Several anesthetics are used in aquaculture to reduce the stress caused by management. In the field, the most common anesthetic is benzocaine, but several authors are researching about eugenol. The objective of this study were to compare the action of these drugs on the anesthetics stages and measure the effects of successive administration of eugenol on the anesthetics stages in Oreochromis niloticus. In the first test, fish (47.73 +/- 8.73 g and 14.23 +/- 0.81 cm) were subjected to five concentrations of eugenol (40, 60, 80, 100 and 120 mg L-1) and one of benzocaine (100 mg L-1). All the anesthetic concentrations tested were able to induce fish to all stages of anesthesia, and revealed an inverse relationship between the induction times and increased concentrations. The minimum concentration of eugenol to induce anesthesia was 100 mg L-1. In the second test, the juveniles (38.07 +/- 5.00 g and 12.70 +/- 0.54 cm) were exposed to concentrations of 60, 80, 100 mg L-1 of eugenol, as determined in the first test. Evaluating the differences between successive days, there was no significant difference (P > 0.05) between anesthetics stages in the concentrations of eugenol tested, suggesting that the residue of eugenol was removed from the fish in less than 24 hours.

Purpose: To investigate the effect of propofol on brain development in neonatal mice and long-term neurocognitive impact in adult mice. Method: The offspring of female C57Bl/6 and male CD-1 mice were administered propofol at concentrations of 2.5 and 5.0 mg/kg (treatment group) or normal saline (control) on postnatal day 7. Thereafter, histological and immunohistochemical examinations were performed on the mice brain. Apoptotic assay, neuronal nuclei antigen immunohistochemistry (to assess neuron density), and behavioral and neurocognitive tests were conducted on the adult mice. Results: Propofol induced cellular degeneration and apoptosis in the brains of neonatal mice. It also modulated physiological parameters (pH, PO2, glucose and lactate), among which decreased blood glucose might be associated with cellular degeneration in the brain. Propofol also caused long-term neuronal deficits in adults, which showed impaired neurocognitive functions. Upon reaching adulthood, propofol-treated mice showed slow learning response and poor memory compared to controls. Conclusion: Propofol causes neurodegeneration in neonatal mice and has long-term neurocognitive consequences in adults, indicating that the use of propofol anesthetics in neonates requires careful consideration.

Introdução: Em Endodontia, a anestesia local é o método de controlo de dor mais utilizado, no entanto vários estudos revelaram que as técnicas anestésicas convencionais apresentam uma eficácia reduzida em casos sintomáticos. Existem várias alternativas às técnicas e anestésicos convencionais, assim como anestesias suplementares que podem ser utilizadas para aumentar a profundidade da anestesia pulpar, e que devem fazer parte do arsenal clínico de modo a possibilitar um tratamento indolor ao paciente. Objetivo: O presente trabalho visou reunir e analisar bibliografia sobre anestesia local em Endodontia e fatores que podem influenciar a sua administração. Foram abordadas técnicas e anestésicos utilizados atualmente, assim como outros métodos estudados recentemente, sendo destacada a eficácia destes na anestesia de pacientes diagnosticados com pulpite irreversível. Materiais e métodos: Foi realizada uma pesquisa bibliográfica no motor de busca Pubmed, tendo sido utilizadas as seguintes palavras chave: “Anesthesia”, “Local anesthesia”, “Anesthesia Technique”, “Anesthetic efficacy”, “Endodontics”, “Lidocaine”, “Articaine”, “Pulpitis”. Estabeleceu-se uma limitação temporal de 2005 a 2016, tendo sido incluídos 54 artigos com ênfase em estudos do tipo meta-análise, revisões bibliográficas e estudos clínicos controlados e randomizados. Conclusão: Em pacientes sintomáticos, de modo a controlar a dor pré-operatória, torna-se muitas vezes necessária a utilização de anestésicos de maior potência e de técnicas suplementares. Aconselham-se, por isso, técnicas como a injeção intraligamentar, intraóssea e infiltrações suplementares para assegurar a anestesia pulpar após técnicas primárias falhadas. Deve-se, ainda, ter em consideração a sensibilidade que alguns pacientes apresentam a determinados componentes presentes nos anestésicos locais, exigindo-se um especial cuidado na seleção e administração destes agentes.

Background. Clinical significance of multifocal pulmonary neuroendocrine proliferations (MNEP), including tumorlets and pulmonary neuroendocrine cell hyperplasia, in association with Typical Carcinoid (TC), is still debated. Methods. A large retrospective series of TC with long-term follow-up data prospectively collected from two institutions was evaluated. Recurrence or new TC development was followed-up. Patients with TC alone and MNEP+TC were compared. Results. 234 TC patients undergone surgery were included: 41 MNEP+TC (17.5%) and 193 TC alone (82.5%). In the MNEP+TC group older age (p<0.001), peripheral tumors (p=0.0032), smaller tumor size (p=0.011) and lymph-nodal spread (p=0.02) were observed in comparison with TC group. Relapses occurred in 8 patients (19.5%) in the MNEP+TC group and in 7 (3.6%) of the TC group. The 10-years progression-free survival were 96.1% in TC and 83.8% in MNEP+TC (p<0.001). After matching, in 36 pairs of patients a significantly higher 5-years progression-free survival was calculated for TC group (p<0.01). Furthermore the odds of belonging to MNEP+TC group was higher with work-related exposure to inhalant agents (p=0.008), asthma/bronchitis (p=0.002), emphysema, fibrosis and inflammatory status (p=0.032), micronodules on the chest CT scan and respiratory insufficiency (p=0.036). Conclusions. The identification of MNEP requires careful pathological examination and postoperative follow-up. MNEP seems to be an adverse prognostic factor in patients with synchronous TC. Therefore, suspicion of MNEP during the pre-operative assessment should not be underestimated, enabling changes in the surgical strategy.
