"Quaternary prevention is more than a Philosophy , it is a concrete answer and a solution to many complex problems that affect Medicine. In 1981, Illich recognized iatrogenic complications, as clinical, social and cultural. The first, gained such a dimension that has become the third leading cause of death in the U.S. , in 2000 as referred by Starfield. When you live more, much more, you want quality of life and not just life subject of increasing comorbidities, with all the parade of treatments, medications that we already know are not without riscs, far from it, we know that over five drugs per day represent the 50% probability of drug-drug interactions and seven drugs 100 %. In elderly attending the reduction of functional reserve, there is a greater risk of iatrogenic problems, adverse reactions and drug interactions, truly unpredictable , these highly medicalized societies in which are accounted diagnoses instead of investigating patterns of disease. So quaternary prevention reminds us that both preventive and curative care in excess behave as risk factors for health , reaching in the case of using preventive medications, millions of individuals who are confident in Medicine. The idea of extending the use of drugs to healthy populations, as preventive actions, expands the pharmaceutical market, which accounts simultaneously increasing profits and losses in economies around the world. Quaternary prevention itineraries in health and disease, may contribute to successful aging about witch many important authors have spoken on."

Esta investigação tem como objetivo estudar as representações de doença em pacientes com o diagnóstico de esquizofrenia e a forma como estas são varáveis preditoras da adesão ao tratamento. Este estudo teve como base o modelo de auto­regulação de Leventhal, que tem sido largamente aplicado à doença física e começa a dar os primeiros passos em trabalhos a nível da saúde mental. A investigação empírica deste trabalho foi assim conduzida tendo em conta o papel ativo que os sujeitos possuem na elaboração das suas próprias doenças. O estudo consistiu na aplicação de questionários de perceção de doença (IPQS), Crenças sobre os medicamentos (BMQ) adesão ao tratamento (RAM) e espiritualidade. Os resultados demonstram que algumas representações de doença são preditoras significativas da adesão ao tratamento. Também as crenças acerca dos medicamentos possuem um efeito preditor ainda que de uma forma menos acentuada. A interação entre representações de doença e crenças espirituais também possuem efeitos preditores na adesão ao tratamento. /ABSTRACT: This study has the objective of make an approach about the illness perceptions of patients with the diagnosis of schizophrenia and the way by witch this variable are predictors of treatment adherence. Using the model of self-regulation of Leventhal, that has been largely applied to physical disease and starts now to develop investigations in the area of mental health problems. The empirical investigation has been conducted having in mind the active role that subjects have in elaborating their own diseases. The present study was made by means of application of the disease illness perception questionnaire (IPQS), beliefs about medicines (BMQ), treatment adherence (RAM) and spirituality. Our results show that some illness cognitions are significative predictors of treatment adherence, as well as the beliefs about medicines have a predictive effect but in a less strong way. Also the interactions between illness representations and spiritual beliefs have predicted the treatment adherence.

This study evaluates the influence of depth and environmental parameters on the development of Gracilaria birdiae Plastino & Oliveira (Gracilariaceae Rhodophyta) in an organic shrimp pound (Litopenaeus vannamei) under euthrophical conditions. PVC structures (module) witch four ropes laden with 150 g of macroalgae each, were kept during 35 days at three different depths (surface, 10 and 20 cm depth). Wet biomass weighing and environmental parameters (temperature, salinity, turbidity, pH, transparence, precipitation, evaporation, insolation, accumulated solar radiation, nitrite, nitrate, ammonium and orthophosphate) were measured weekly. At all three proposed depths, the macroalgae displayed a higher biomass at the end of experiment than at the initial inoculations. The module kept at a 10 cm depth presented the greatest average biomass (186,3), followed by that kept at 20 cm (180,4 g) and the surface module (169,9 g). Biomass variations showed algae to suffer the direct effects of depths. Biomass loss was associated with the factors that influence light penetration, such as sediment deposits above the thallus, rate of evaporation and precipitation. The smallest loses occurred in the algae kept on surface (0,16%), followed by the algae kept at 20 cm (0,20%) and 10 cm (0,22%). The specific growth rate (SGR) of G. birdiae showed no significant difference between the three depths nor the sample periods. Nevertheless, the modules kept at 10 and 20 cm depths presented similar growth evolution, both growing 0,38%·per day-1, while the module kept on surface had an average SGR of 0,36%·day-1. The models related to growth rate demonstrated temperature, salinity, pH, orthophosphate, ammonium, precipitation and turbidity as the principal environmental parameters influencing the development of G. birdiae

Context - It is well recognized that celiac disease is an immune-mediated systemic disorder highly prevalent among relatives of celiac patients. Objectives - The aim of this study is to determine the prevalence of celiac disease in a group of first degree relatives of celiac children, and to access the frequency of human leukocyte antigen HLA-DQ2 and DQ8 in celiac disease patients and their affected relatives. Methods - A survey was conducted of 39 children with celiac disease with follow-up in the Pediatric outpatient’s clinic of Dr. Nélio Mendonça Hospital, in Madeira Island, Portugal. Were invited 110 first degree relatives to undergo serological screen for celiac disease with IgA antibody to human recombinant tissue transglutaminase (IgA-TGG) quantification. In all seropositive relatives, small intestinal biopsy and HLA typing was recommended. Results - HLA- typing was performed in 38 celiac patients, 28/74% DQ2 positive, 1/2% DQ8 positive and 9/24% incomplete DQ2. Positive IgA-TGG was found in five out of the 95 relatives, and CD was diagnosed in three of them. Three relatives had the presence of HLA-DQ2, two were DQ2 incomplete (DQB1*02). Conclusion - The prevalence of celiac disease among first degree celiac patients´ relatives was 3.1%, 4.5 times higher than the general Portuguese population (0,7%) witch reinforces the need of extensive diagnostic screening in this specific group. HLA-DQ2 typing may be a tool in the diagnostic approach.
