Background - Medical image perception research relies on visual data to study the diagnostic relationship between observers and medical images. A consistent method to assess visual function for participants in medical imaging research has not been developed and represents a significant gap in existing research. Methods - Three visual assessment factors appropriate to observer studies were identified: visual acuity, contrast sensitivity, and stereopsis. A test was designed for each, and 30 radiography observers (mean age 31.6 years) participated in each test. Results - Mean binocular visual acuity for distance was 20/14 for all observers. The difference between observers who did and did not use corrective lenses was not statistically significant (P = .12). All subjects had a normal value for near visual acuity and stereoacuity. Contrast sensitivity was better than population norms. Conclusion - All observers had normal visual function and could participate in medical imaging visual analysis studies. Protocols of evaluation and populations norms are provided. Further studies are necessary to understand fully the relationship between visual performance on tests and diagnostic accuracy in practice.

Nine cases of tuberculosis (TB) were diagnosed among 800 uremic patients, followed-up during 11 years, a prevalence of 1125%, 2.5 times higher than that in the general population. Six patients (66.7%) had lymph node involvement (4 cervical and 2 mediastinal). Three patients (33.3%) had pulmonary involvement (2 pleuro-pulmonary and 1 bilateral apical pulmonary). Eight patients were undergoing dialysis and 1 was pre-dialytic. The duration of dialysis ranged from 1 to 60 months. Three patients had previously received immunosuppressive drugs for unsuccessful renal transplantation. Daily fever was present in all but one patient; he was asymptomatic and TB was suspected after routine chest radiography. Biopsy was the diagnostic procedure in 7 patients (77.8%), four by direct cervical lymph node biopsy, 2 by mediastinal, performed by mediastinoscopy and 1 by pleural biopsy. In 2 other patients TB was confirmed by the presence of tubercle bacilli; in sputum (1 patient) and in a bronchial flushing specimen (the other patient). Triple therapy was used in all patients (isoniazid and ethambutol in all), plus rifampicin in 8 and streptomycin in 1. One patient had jaundice and another had optical neuritis. Five patients were cured. The other four died during treatment of causes unrelated to TB or its treatment.

Purpose: This study aims to investigate the influence of tube potential (kVp) variation in relation to perceptual image quality and effective dose for pelvis using automatic exposure control (AEC) and non-AEC in a computed radiography (CR) system. Methods and Materials: To determine the effects of using AEC and non-AEC by applying the 10 kVp rule in two experiments using an anthropomorphic pelvis phantom. Images were acquired using 10 kVp increments (60-120 kVp) for both experiments. The first experiment, based on seven AEC combinations, produced 49 images. The mean mAs from each kVp increment were used as a baseline for the second experiment producing 35 images. A total of 84 images were produced and a panel of 5 experienced observers participated for the image scoring using the 2 AFC visual grading software. PCXMC software was used to estimate the effective dose. Results: A decrease in perceptual image quality as the kVp increases was observed both in non-AEC and AEC experiments, however no significant statistical differences (p> 0.05) were found. Image quality scores from all observers at 10 kVp increments for all mAs values using non-AEC mode demonstrates a better score up to 90 kVp. Effective dose results show a statistical significant decrease (p=0.000) on the 75th quartile from 0.3 mSv at 60 kVp to 0.1 mSv at 120 kVp when applying the 10 kVp rule in non-AEC mode. Conclusion: No significant reduction in perceptual image quality is observed when increasing kVp whilst a marked and significant effective dose reduction is observed.

RESUMO: Contexto: Indicadores fidedignos da composição corporal são importantes na orientação das estratégias nutricionais de recém-nascidos e pequenos lactentes submetidos a cuidados intensivos. O braço é uma região acessível para avaliar a composição corporal regional, pela medida dos seus compartimentos. A antropometria e a ultrassonografia (US) são métodos não invasivos, relativamente económicos, que podem ser usados à cabeceira do paciente na medição desses compartimentos, embora esses métodos não tenham ainda sido validados neste subgrupo etário. A ressonância magnética (RM) pode ser usada como método de referência na validação da medição dos compartimentos do braço. Objectivo: Validar em lactentes pré-termo, as medidas do braço por antropometria e por US. Métodos: Foi estudada uma coorte de recém-nascidos admitidos consecutivamente na unidade de cuidados intensivos neonatais, com 33 semanas de idade de gestação e peso adequado para a mesma, sem anomalias congénitas major e não submetidas a diuréticos ou oxigenoterapia no momento da avaliação. Nas vésperas da alta, foram efectuadas medições do braço, com ocultação, pelos métodos antropométrico, ultrassonográfico e RM. As medidas antropométricas directas foram: peso (P), comprimento (C), perímetro cefálico (PC), perímetro braquial (PB) e prega cutânea tricipital (PT). As área braquial total, área muscular (AM) e área adiposa foram calculadas pelos métodos de Jeliffee & Jeliffee e de Rolland-Cachera. Utilizando uma sonda PSH-7DLT de 7 Hz no ecógrafo Toshiba SSH 140A foram medidos os perímetros braquial e muscular e calculadas automaticamente as áreas braquial e muscular, sendo a área adiposa obtida por subtracção. Como método de referência foi utilizada a RM – Philips Gyroscan ACS-NT, Power-Track 1000 ®, 1.5 Tesla com uma antena de quadratura do joelho. Na análise estatística foram utilizados os métodos paramétricos e não paramétricos, conforme adequado. Resultados: Foram incluídas 30 crianças, nascidas com ( ±DP) 30.7 ±1.9 semanas de gestação, pesando 1380 ±325g, as quais foram avaliadas às 35.4 ±1.1 semanas de idade corrigida, quando pesavam 1786 ±93g. Nenhuma das medidas antropométricas, individualmente, constitui um indicador aceitável (r2 <0.5) das medições por RM. A melhor e mais simples equação alternativa encontrada é a que estima a AM (r2 = 0.56), derivada dos resultados da análise de regressão múltipla: AMRM = (P x 0.17) + (PB x 5.2) – (C x 6) – 150, sendo o P expresso em g, o C e o PB em cm. Nenhuma das medidas ultrassonográficas constitui um indicador aceitável (r2 <0.4) das medições por RM. Conclusões: A antropometria e as medidas ultrassonográficas do braço não são indicadores fidedignos da composição corporal regional em lactentes pré-termo, adequados para a idade de gestação.----------ABSTRACT: Background: Accurate predictors for body composition are valuable tools guiding nutritional strategies in infants needing intensive care. The upper-arm is a part of the body that is easily accessible and convenient for assessing the regional body composition, throughout the assessment of their compartments. Anthropometry and by ultrasonography (US) are noninvasive and relatively nonexpensive methods for bedside assessment of the upper-arm compartments. However, these methods have not yet been validated in infants. Magnetic resonance imaging (MRI) may be used as gold standard to validate the measurements of the upper-arm compartments. Objective: To validate the upper-arm measurements by anthropometry and by US in preterm infants. Methods: A cohort of neonates consecutively admitted at the neonatal intensive care unit, appropriate for gestational age, with 33 weeks, without major congenital abnormalities and not subjected to diuretics or oxygen therapy, was assessed. Before the discharge, the upper-arm was blindly measured by anthropometry, US and MRI. The direct anthropometric parameters measured were: weight (W), length (L), head circumference (HC), mid-arm circumference (MAC), and tricipital skinfold thickness. The arm area (AA), arm muscle area (AMA) and arm fat area were calculated applying the methods proposed by Jeliffee & Jeliffee and by Rolland-Cachera. Using the sonolayer Toshiba SSH 140A and the probe PSH-7DLT 7Hz, the arm and muscle perimeters were measured by US, the arm and muscle areas included were automatically calculated, and the fat area was calculated by subtraction. The MR images were acquired on a 1.5-T Philips Gyroscan ACS-NT, Power-Track 1000 scanner, and a knee coil was chosen for the upper-arm measurements. For statistical analysis parametric and nonparametric methods were used as appropriate. Results: Thirty infants born with ( ±SD) 30.7 ±1.9 weeks of gestational age and weighing 1380 ±325g were included in the study; they were assessed at 35.4 ±1.1 weeks of corrected age, weighing 1786 ±93g. None of the anthropometric measurements are individually acceptable (r2 <0.5) for prediction of the measurements obtained by MRI. The best and simple alternative equation found is the equation for prediction of the AMA (r2 = 0.56), derived from the results of multiple regression analysis: AMARM = (W x 0.17) + (MAC x 5.2) – (L x 6) – 150, being the W expressed in g, and L and MAC in cm. None of the ultrasonographic measurements are acceptable (r2 <0.5) predictors for the measurements obtained by MRI. Conclusions: The measurements of the upper-arm by anthropometry and by US are not accurate predictors for the regional body composition in preterm appropriate for gestational age infants.

BACKGROUND: Atherosclerotic carotid disease represents approximately 20% of the causes of ischemic stroke. Effective treatment options, such as endovascular or surgical revascularization procedures, are available. Doppler Ultrasound (DUS) is a non-invasive, inexpensive, routine exam used to evaluate the presence of internal carotid artery (ICA) stenosis. We retrospectively analysed the prevalence of severe atherosclerotic carotid disease in a population of patients with acute ischemic stroke/transitory ischemic attacks (TIAs), and the role of DUS in the detection of ICA stenosis and treatment decisions in these patients. METHODS: A total of 318 patients with ischemic stroke or TIAs was admitted to our stroke unit, and 260 patients were studied by DUS. ICA stenosis was evaluated by DUS according to peak systolic velocity. All DUS exams were performed by the same operator. ICA stenosis was further assessed in 43 patients by digital subtraction angiography (DSA) using NASCET criteria. RESULTS: Of the total 318 patients, 260 (82%) had DUS evaluation. Of the total 520 ICAs studied by DUS, degrees of ICA stenosis were: 0-29% n= 438 (84%); 30-49% n= 8 (2%); 50-69% n= 27 (5%); 70-89% n= 15 (3%); 90-99% n= 20 (4%); oclusão n= 14 (2%). Of the total 260 patients studied, 43 (16.5%) underwent DSA. Sensibility and specificity of DUS in the diagnosis of carotid stenosis over 70% were, respectively, 91% e 84%. Of the total 31 patients with significant carotid stenosis (70-99%), 23 (74%) underwent subsequent carotid revascularization procedures. DISCUSSION: DUS is an important screening test in our stroke unit, justifying its use as a routine exam for all patients with ischemic stroke/TIAs. Moreover, our results show the relevance of severe carotid disease in a population with acute ischemic stroke/TIAs (16.5%), with a total of 9% of patients being submitted to carotid revascularization procedures.

Ollier Disease and Maffucci Syndrome are two rare diseases that can cause tumors in several organs, having a special predilection for the hand. However, there have been very few reports in the literature focusing on hand manifestations of these diseases. We report the cases of three female patients: one with Ollier Disease, and two other with Maffucci Syndrome. All patients had hand involvement as their initial primary complaint. The Ollier Disease patient developed chondrosarcomas of two digits and had to have these fingers amputated. One of the Maffucci patients died one year after presentation from a brain glioblastoma. These cases emphasize the importance of early diagnosis of Ollier Disease and Maffucci Syndrome, as these two conditions are associated not only to crippling hand deformity, but also to a significant risk of chondrosarcoma, and other malignant tumors.

Paracoccidioidomycosis is a common disease in Latin America but it is rare in organ transplant recipient patients. We report on a case of such mycosis in a renal transplant recipient. The patient presented with a large lung cavity on the left lower lobe, a rare radiological presentation of paracoccidioidomycosis. Unusual clinical and radiological manifestations of Paracoccidioides brasiliensis infection can occur in immunocompromised patients.

Extranodal involvement is common in lymphomas associated with human immunodeficiency virus infection (HIV) and acquired immunodeficiency syndrome (AIDS). However, primary pulmonary AIDS-related non-Hodgkin's lymphoma is very rare and only few reports were published in the medical literature. Clinical presentation is nonspecific, with "B" and respiratory symptoms. Also, patients were with advanced immunodeficiency at the time of diagnosis. Generally, chest radiography showed peripheral nodules or cavitary masses. Primary pulmonary lymphoma associated with AIDS is generally a high-grade B-cell non-Hodgkin lymphoma and Epstein-Barr virus is strongly associated with the pathogenesis of these tumors. We report a patient with AIDS and primary pulmonary lymphoma which clinical presentation was a total atelectasis of the left lung.

The role of cerebral angiography in the diagnosis of cerebrovascular disease is currently being questioned due to both the increasing availability of carotid sonography and the recent introduction of Magnetic Resonance Angiography (MRA). After a technical foreword about the different modalities available today in Cerebral Angiography, we discuss its present indications (Conventional or Digital subtraction by intra-arterial route), in patients with extra and intra cranial atherosclerotic cerebro vascular disease, subarachnoid hemorrhage and arterial aneurysms, in vascular malformations, particularly arterio-venous malformations (AVM's), in occlusive non-atherosclerotic non hypertensive arteriopathies and in occlusive venous pathology. Although it is possible that the future will show us the progressive replacement of the invasive technologies by MRA, at the present stage of Magnetic Resonance development there is still an important role, if not crucial, for catheter angiography in the diagnosis of most of the diseases producing stroke syndromes.

The use of solid wastes and natural fibres to the production of innovative composites is now a matter of environmental need. A set of hybrid composites constituted of waste rubber particles and sugarcane bagasse fibres into a thermoset composite material are drilled to determine the effect of rubber particle addition and size, sugarcane fibre addition and length and fibre chemical treatment on damage extension and related mechanical properties. Damage extension is determined by enhanced radiography for further damage measurement – diameter and areas – and correlated with mechanical test results — bearing test. The results demonstrated significant effect of the rubber and fibre additions on mechanical properties of the composites.

PURPOSE: To describe the anatomy and imaging findings of the prostatic arteries (PAs) on multirow-detector pelvic computed tomographic (CT) angiography and digital subtraction angiography (DSA) before embolization for symptomatic benign prostatic hyperplasia (BPH). MATERIALS AND METHODS: In a retrospective study from May 2010 to June 2011, 75 men (150 pelvic sides) underwent pelvic CT angiography and selective pelvic DSA before PA embolization for BPH. Each pelvic side was evaluated regarding the number of independent PAs and their origin, trajectory, termination, and anastomoses with adjacent arteries. RESULTS: A total of 57% of pelvic sides (n = 86) had only one PA, and 43% (n = 64) had two independent PAs identified (mean PA diameter, 1.6 mm ± 0.3). PAs originated from the internal pudendal artery in 34.1% of pelvic sides (n = 73), from a common trunk with the superior vesical artery in 20.1% (n = 43), from the anterior common gluteal-pudendal trunk in 17.8% (n = 38), from the obturator artery in 12.6% (n = 27), and from a common trunk with rectal branches in 8.4% (n = 18). In 57% of pelvic sides (n = 86), anastomoses to adjacent arteries were documented. There were 30 pelvic sides (20%) with accessory pudendal arteries in close relationship with the PAs. No correlations were found between PA diameter and patient age, prostate volume, or prostate-specific antigen values on multivariate analysis with logistic regression. CONCLUSIONS: PAs have highly variable origins between the left and right sides and between patients, and most frequently arise from the internal pudendal artery.

Objectivo: Avaliar a acuidade da angiografia de subtracção digital (ASD) no diagnóstico morfológico da conexão venosa pulmonar anómala (CVPA) em crianças. Concepção do estudo: Estudo prospectivo de doentes consecutivos entre Janeiro de 1989 e Julho de 1992. Tipo de Atendimento: Serviço de Cardiologia Pediátrica de um Hospital Central. População: Vinte e quatro doentes com CVPA. Métodos: Todos os doentes fizeram avaliação clínica e ecocardiográfica completa (modo M, bidimensional e Doppler) antes da realização do exame hemodinâmico. Em todos os casos se fizeram, de modo sistemático, injecções selectivas de contraste de baixa osmolaridade (0,5-1 ml/kg; dose total <6 mi/kg) no tronco e ramos da artéria pulmonar com registo em angiografia com subtracção digital (ASD). As imagens colhidas foram trabalhadas, selecciona das e armazenadas em video-cassetes e películas fotográficas (câmara multiformato). Resultados: Dezasseis doentes tinham CVPA total (CVPAT): onze à veia cava superior (VCS), dois ao seio coronário e três infradiafragmáticos (dois à veia cava inferior (VCI) e um à veia porta). Oito crianças tinham CVPA parcial (CVPAP): três à VCS, uma à aurícula direita (AD), três à VCI (síndroma da cimitarra) e num caso a CVPA era mista. Em oito doentes (seis com CVPAT e dois com CVPAP), a ASD contribuiu significativamente para o diagnóstico final tendo completado ou corrigido a informação obtida por ecocardiografia. Nos dezoito doentes submetidos a cirurgia cardíaca foi confirmado o diagnóstico obtido por ASD. Conclusões: A ASD é um método muito útil para o diagnóstico anatómico de doentes com CVPA. Na nossa experiência foi particularmente informativa a análise de registos em «video». A ASD está indicada nos casos em que os achados clínicos e ecocardiográficos não sejam típicos.

Nos últimos anos, a utilização dos materiais compósitos tem vindo a tornar-se cada vez mais comum em várias indústrias, onde se verifica uma ascensão na procura pelos mesmos. Características como o baixo peso aliado à sua alta resistência e rigidez permitem que estes materiais possuam diversas aplicações em variadas áreas, desde a medicina, aeronáutica, indústria automóvel e aeroespacial, até à indústria eletrónica. Hoje em dia, o uso de desperdícios sólidos de borracha e fibras naturais na produção de materiais compósitos é, mais que uma opção, uma necessidade ambiental. De forma a reduzir as enormes quantidades de desperdícios, foi criado um material compósito constituído por uma resina termoendurecível reforçada com esses dois tipos de desperdícios. Parâmetros de fabrico como a percentagem de borracha, o tamanho das partículas de borracha, a percentagem de fibras de cana-de-açúcar e o comprimento dessas fibras foram variados, com o objetivo de estudar a influência destes dois materiais nas propriedades mecânicas do compósito. Apesar da maior parte dos compósitos serem fabricados na forma de uma peça funcional quase pronta a ser utilizada, por vezes é necessário recorrer à maquinação de furos. Apesar das muitas técnicas de furação existentes, os defeitos resultantes deste processo aplicado aos materiais compósitos são ainda muito comuns. Desses defeitos o que mais se destaca é sem dúvida a delaminação. Trinta e seis provetes de epóxido reforçado com borracha e fibra de cana-de-açúcar foram fabricados e furados, de modo a possibilitar o estudo das propriedades mecânicas do material compósito, assim como a análise da zona danificada durante a furação. Diferentes condições de furação, como tipos de broca e velocidades de avanço diferentes, foram impostas aos provetes de forma a variar o mais possível a zona de dano de uns furos para os outros. Parâmetros como a área de dano ou ainda o fator de delaminação provam ser muito úteis na caracterização e quantificação do dano na zona periférica de um furo. Recorrendo a técnicas de processamento de imagem foi possível obter esses parâmetros. O processamento e análise de imagem pode ser feito através de vários métodos. O método utilizado neste trabalho foi o software MATLAB® associado a ferramentas de processamento de imagem. Depois de feita a análise dos furos foram realizados ensaios de esmagamento a todos os provetes. Este passo permitiu assim avaliar de que forma os parâmetros de furação influenciam a resistência mecânica do material, e se a avaliação realizada aos furos é um método viável para a avaliação da extensão de dano nesses furos.
