Introduction: Alcohol use by men and women is very much influenced by social habits and customs. Cultural peculiarities and biological differences between the sexes require more focused and standardized studies. The objective was to systematize information on patterns of alcohol use between the sexes.Method: A literary review (1972-2004) identified 96 publications (Lilacs, Scielo, Medline) and some related books.Results and conclusions: Men drank more and presented more problems (legal, family, social, clinical, traumas and mortality) associated with alcohol use; the consequences of alcohol use in developing countries with low death rates is even higher. Women can face more discrimination by using alcohol as well as worse health problems when they abuse drinking (liver, pancreas, and central and peripheral nervous system problems, psychiatric comorbidity, etc.); sexual abuse is more commonly associated with women than discussing the different responses to treatment. As for social roles/responsibilities exercised by women, there are indications that marriage, employment, and children have a good influence, discouraging alcohol use, while divorce, unemployment, and no children contribute to higher consumption. For both sexes, religion was a protective factor for alcohol use; acculturation was a strong influence in the pattern of alcohol use, and alcohol worsened the evolution of existing psychiatric disorders. O objetivo deste trabalho consistiu em utilizar estratégias para obter e analisar relatos sobre sexualidade de cinco jovens, ambos os sexos, com deficiência mental, por diferentes procedimentos metodológicos: 1) Desenho da Figura Humana; 2) Apresentação de bonecos da família sexuada; 3) Apresentação das pranchas dos temas: namoro, casamento, masturbação, jogos sexuais, menstruação, relação sexual, gravidez, parto, amamentação e abuso sexual. Observou-se que os jovens: a) têm noção de identidade e papéis sexuais; b) diferenciam e nomeiam órgãos sexuais humanos, especialmente o órgão sexual masculino adulto; c) apesar de saberem nomeá-los, nem todos sabem sua função; d) com frases curtas e objetivas os jovens mostraram os conceitos sobre os diferentes temas apresentados. As estratégias utilizadas foram eficientes para incentivar o relato de jovens com limitação intelectual em temas complexos como a sexualidade.

Based on criminal processes regarding child sexual abuse from the early 1940s to the late 1970s, this article intents to analyze both the conception of innocence and childhood held at that time, and the influence of such on the local judiciary.

Objective: In this study, we compared the frequency and intensity of childhood traumas in alcohol- or other drug-dependent patients, in patients with depression, and in a control group without psychiatric diagnoses. Methods: The study had a retrospective design of a clinical sample of men and women from the groups listed above. They were evaluated by the same standardized instrument: the Childhood Trauma Questionnaire.. Results: A higher frequency and intensity of emotional, physical, and sexual abuse were found in alcohol- and other drug-dependent patients than in patients with depression, who, in turn, presented significantly higher proportions than the control group. In all of the cases, the frequency was higher among women than men. Conclusion: Because of the high frequency and intensity of childhood traumas among alcohol- or other drug-dependent patients and depressed patients, the assessment of problems due to childhood traumas among these patients is essential to a better understanding of the etiology of those disorders and to their treatment. © 2010 Elsevier Ltd. O presente trabalho teve como objetivo investigar os processos de subjetivação de mulheres sem parceiro fixo à exposição ao vírus HIV/ Aids para identificar fatores sobredeterminantes de vulnerabilidade. Utilizamos como método o estudo de caso, visando uma análise em profundidade, que permitisse identificar um maior número de determinantes subjetivos relacionados com a problemática considerada. O estudo apresenta fragmentos de casos clínicos de mulheres vivendo com Aids, internadas nas enfermarias do Hospital Universitário João de Barros Barreto (HUJBB), no Estado do Pará, Brasil. A partir da análise da transferência e da contratransferência, apontamos como resultado o que cada caso em sua singularidade, desvela a partir do encontro terapêutico: No caso Clínico I, encontramos que a paciente, a qual chamamos Dinah, apresentava um modo de subjetivação psicopatológico masoquista feminino, que faz com que ela demonstre certa satisfação quando se expõe ao sofrimento, se posicionando como vítima em seus relacionamentos afetivos e sexuais, sobre determinados pela identificação imaginária com ideais culturais sobre o ser mulher, concebendo imagens de homens e mulheres, e, portanto, suas e de seu parceiro, como pares antitéticos de força/fraqueza, atividade/passividade, poder/submissão. Esse ideal de eu compósito de mulher virgem e de um homem só, levou Dinah a negar seus temores de contaminação, aceitar passivamente relações desprotegidas, atribuindo à iniciativa sexual a seu parceiro e, tornando-se vulnerável a infecção pelo HIV. No caso Clínico II, Alice, submetida a um modo de subjetivação melancólico, auto-destrutivo, se posicionava nas relações afetivas e sexuais procurando incessantemente sua auto-destruição pela própria vulnerabilidade inconsciente à contaminação pelo HIV. Tendo contraído o vírus e contaminado seu marido e, demais parceiros, mesmo após saber de seu diagnóstico, Alice permanecia aprisionada em um silêncio mortífero, impedindo-se de cuidar de sua saúde e procurar atendimento médico contínuo, tornando-se vulnerável à reinfecção. O Caso III, Ana Laura, é de uma mulher que sofreu inúmeras violências desde a infância, como abuso sexual infantil, exploração do trabalho doméstico e, abandono pelos pais. Após ter tido seu primeiro filho, este lhe foi retirado sem seu consentimento, pela tia materna que o deu a terceiros, razão alegada por Ana Laura, para prostituir-se no cais do porto da cidade de Belém, onde trabalhou até bem pouco tempo antes de sua internação. Lá onde a negociação por sexo mais caro sem preservativo era prática comum, Ana Laura negociou sua vida, vendendo sexo sem preservativo, assim se infectando. O desamparo e as violências sofridas por esta paciente são, portanto, sobredeterminantes de sua vulnerabilidade à infecção pelo HIV. Como conclusões, destacamos que as mulheres atendidas sem parceiro fixo, não apresentaram maior facilidade para se protegerem, estando em desacordo com os estudos que apontam que estas mulheres negociam o preservativo com maior liberdade e estão menos vulneráveis, demonstrando a importância de estudos que abordem os aspectos psíquicos, sociais, políticos e culturais, de maneira a desvelar os modos de produção de subjetividade dos sujeitos em sua singularidade, para além da mensuração de dados, a fim de estabelecer estratégias de prevenção em saúde mais eficazes.

The present study aimed to identify the conceptions of students of a Pedagogy course about infantile sexuality and gender relations, checking the needs that they appointed concerning studies about sexuality and sex education in their graduation. Using this information, the authors implemented an interventional course. The instruments used for data collection were two questionnaires. 342 students participated in the first phase of the study, corresponding to the completion of the questionnaire. 38 people participated on the second step – applying the course. The issues they indicated as necessary in their course were: sexuality, sex education, child sexuality, sexual abuse, adolescence and sexuality and disability. Finally, the results of the course were very satisfactory, however, reveals how much we still need to do for training professionals to implement the work of sex education in the school context.

Violence against children and adolescents, ranging from negligence to sexual abuse, is an imperative public health problem. The ill treatments are considered as nonaccidental traumas, actions or omissions against children, who suffer physical or emotional violence. The main perpetrators are fathers, mothers, guardians, family, friends or the children‘s primary caretaker. It is very important to identify abuse cases, because this allows proper medical and legal intervention. In Brazil, the law n. 8.069, of July 13, 1990, known as the ECA -Statute for Children and Adolescents (SCA), stipulates the obligation to report suspected or confirmed cases of ill treatment against a child or adolescent to the Guardianship Council of the respective locality. The health professionals play a crucial role with regard to identifying cases of violence, collecting information, making early diagnosis of suspect cases and reporting such cases to the authorities. The dentists can contribute significantly, as most injuries occur in the orofacial region. Bite marks, burns, bruising, among others, are easily identified during a dental consultation. The aim of this work was to verify the awareness and attitudes of Brazilian dentists concerning violence against children and adolescents. This research was approved by the Research Ethics Committee of the Faculty of Dentistry of Araraquara – São Paulo State University (FOAr – UNESP). Sixty-three dentists answered an open and closed questionnaire concerning their formation, knowledge, experience and attitudes towards ill-treated children and adolescents. Among other results, thirty-nine dentists (61.9%) affirmed to have the means to identify illtreatment cases, 13 (20.6%) reported having some experience on this matter, but only 8 reported the cases to the Council of Guardianship, as determined by Brazilian law. Twenty percent of the reasons presented for not notifying the Council were fear of reprisal, and 60.0% were uncertain concerning the ethical and legal implications. Physical violence was the most reported form of identified violence (76.9%), followed by negligence (38.5%). Among the 13 professionals that had experience with violence cases, in 10 cases the perpetrators were identified – in 70.0% of the cases, the parents were the originators of such violence. It was concluded that further formation and orientation are necessary, in order to prepare dentists to act correctly when, during their professional activities, they encounter cases of violence against children and adolescents.

Condyloma acuminata caused by human papilloma viruses, (HPV) is a sexually transmitted disease (STD) appearing most frequently as soft, pink cauliflower like growths in moist areas, such as the genitalia, mouth and other places. The disease is highly contagious, can appear singly or in groups, small or large. In children, the isolation of a sexually transmitted organism may be the first indication that an abuse has occurred. Although the presence of a sexually transmissible agent from a child beyond the neonatal period is suggestive of sexual abuse, exceptions do exist. The authors report the clinical case of a five-year-old Caucasian male with lesions located in the dorsal surfaces of the posterior tongue and palate. Both lesions had a firm consistency, reddish appearance and presence of whitish areas and regions of ulceration. During the interview, the mother reported that the boy had been sexually abused. Sexually transmitted disease may occur during sexual abuse. Dentists as well as pediatricians have a role to play in identifying and treating these children. The diagnosis is essentially clinical (anamnesis and physical examination), but also the use of cytology eventually resorts to biopsy of the suspicious lesions for histological examination. The therapeutic option was the excision of the lesions.

Child maltreatment has been linked to a myriad of long-term difficulties, including trauma symptomatology. However, not all victims experience long-term distress. Thus, a burgeoning area of research focuses on factors that may impede or facilitate resiliency to the psychological correlates of child maltreatment. Specifically, the severity of the abusive acts may be associated with greater long-term difficulties. To date, however, with the exception of child sexual abuse, few studies have examined the severity of maltreatment as a risk factor in the development of trauma symptoms. In contrast, social support has been theorized to contribute to resiliency following abuse. However, to date, the majority of studies examining positive social support as a protective factor have relied on self-report measures of perceived social support, rather than observational measures of received social support. Moreover, no study to date has examined the role that negative social support (i.e, blaming, criticizing) may play in potentiating trauma symptoms among victims of child maltreatment. Because child maltreatment involves serious boundary violations by a trusted person, a marital relationship is an important domain in which to examine these constructs. That is, it may serve as an arena for the manifestation of psychological disturbances related to maltreatment. Thus, the present study examined whether observationally measured positive and negative spousal social support moderated the relationship between child maltreatment severity (i.e., sexual, physical, psychological abuse; neglect) and trauma symptomatology in women and men. Results indicated that the severity of each type of child maltreatment significantly predicted increased adult trauma symptomatology. Contrary to hypothesized outcomes, positive spousal social support did not predict decreased trauma symptomatology. However, negative spousal social support generally did predict increased trauma symptomatology. There were no consistent patterns of interactions between child maltreatment severity and either type of social support. Future directions for research will be discussed and clinical implications with regard to the intrapersonal and interpersonal functioning of child maltreatment victims will be highlighted.

This study’s purpose was to determine if efficient measures could be created to assess multiple problematic behaviors identified in youth who were sexually abused and in treatment. Because of the lack of easily administered brief instruments that assess multiple domains of interest in this population, complementary parent and child assessment measures were developed. The Weekly Problems Scale–Child Version (WPSC) and the Weekly Problems Scale–Parent Version (WPS-P) were created to monitor the weekly progress of the child and family in treatment and focus specifically on common areas of difficulties in this population. Exploratory factor analysis was conducted to assist in identifying the number of underlying dimensions in the scales. Results indicate that the WPS-C and WPS-P demonstrate adequate internal consistency, temporal stability, and construct validity. The WPS-C and WPSP display significant promise as research and clinical assessment tools for use with youth who are sexually abused and their nonoffending parents in treatment.

Objective: Neuroimaging studies have highlighted important issues related to structural and functional brain changes found in sufferers of psychological trauma that may influence their ability to synthesize, categorize, and integrate traumatic memories. Methods: Literature review and critical analysis and synthesis. Results: Traumatic memories are diagnostic symptoms of post-traumatic stress disorder (PTSD), and the dual representation theory posits separate memory systems subserving vivid re-experiencing (non-hippocampally dependent) versus declarative autobiographical memories of trauma (hippocampally dependent). But the psychopathological signs of trauma are not static over time, nor is the expression of traumatic memories. Multiple memory systems are activated simultaneously and in parallel on various occasions. Neural circuitry interaction is a crucial aspect in the development of a psychotherapeutic approach that may favour an integrative translation of the sensory fragments of the traumatic memory into a declarative memory system. Conclusion: The relationship between neuroimaging findings and psychological approaches is discussed for greater efficacy in the treatment of psychologically traumatized patients.
