Oral tongue squamous cell carcinoma (OTSCC) has an aggressive biological behavior, with a high propensity for the development of lymph node metastases. In this context, lymphangiogenesis is considered an important phenomenon for the spread of tumor cells and may be influenced by microenvironmental stimuli. Mast cells have been implicated in tumor progression, although their influence in the formation of lymphatic vessels is not well established. The aim of this study was to analyze, in a case series of OTSCC (n=50), possible correlations between lymphatic vessel density (LVD), mast cell count and clinicopathological features, including tumor-node-metastasis (TNM) stage, histological grade of malignancy (Bryne, 1998), and nodal metastasis. LVD was established as the mean number of lymphatic vessels immunostained by anti-podoplanin (D2-40) antibody, identified in five microscopic fields (200x). For the analysis of mast cells, tryptase-immunoreactive cells were quantified in five fields (400x). Both immunostainings were analyzed in the tumor center and invasion front. Intratumoral lymphatic density (ILD) was higher in cases in advanced clinical stages (III-IV), compared to those in initial stages (I-II), as well as in metastatic cases in respect of non-metastatic (p<0,05). There were no statistically significant differences between low-grade and high-grade malignancy cases with respect to ILD (p>0,05). Peritumoral lymphatic density (PLD) and mast cell counts showed no significant relations with any of the clinicopathological parameters evaluated (p>0,05). Also there were no significant correlations between LVD and mast cell counts, whether in intratumoral (r = -0,004; p=0,977) or peritumoral region (r = -0,154; p=0,285). The results of the present study suggest that intratumoral lymphatic vessels may contribute in part to the progression of OTSCC, although PLD may be insufficient to justify differences in biological behavior. This supports the hypothesis of involvement of other mechanisms in metastatic spread of malignant cells, which could complement the effects of lymphangiogenesis. Although mast cells perform several pro- and antitumoral functions, they do not appear to directly influence aggressiveness of OTSCC. In addition, the quantity of these cells may not be essential for lymphatic vessel formation.

Angiogenesis and lymphangiogenesis are changes that occur due to gingival inflammation caused by microorganisms present in the biofilm, as well as the migration of immune cells and secretion of mediators in the aggressed site. This study aimed to research angiogenesis and lymphangiogenesis in 90 specimens of clinically healthy, with gingivitis and chronic periodontitis gingival tissue biopsies. The histological sections were evaluated by hematoxylin and eosin and the immunohistochemical technique through immunostaining for CD34 and podoplanin. To evaluate the angiogenic and lymphangiogenic indexes we performed a microvessel counting technique. The results showed that there is a correlation between the indexes (p = 0.030), however, we observed that periodontitis showed less lymphatic vessels than clinically healthy gingival tissue (p = 0.016). Podoplanin showed positive staining in the basal layers of the epithelium, and we observed a relationship between immunostaining intensity and the intensity of inflammatory infiltrate, with more intense staining in the presence of severe inflammatory infiltrate (p = 0.033). For this study, we concluded that there are fewer blood vessels in periodontitis compared with clinically healthy gingiva. The signaling present in the inflammatory process and the actual role of gingival blood and lymphatic vasculature are not fully understood, with further studies on angiogenesis and lymphangiogenesis being suggested.

Skeletal muscle consists of muscle fiber types that have different physiological and biochemical characteristics. Basically, the muscle fiber can be classified into type I and type II, presenting, among other features, contraction speed and sensitivity to fatigue different for each type of muscle fiber. These fibers coexist in the skeletal muscles and their relative proportions are modulated according to the muscle functionality and the stimulus that is submitted. To identify the different proportions of fiber types in the muscle composition, many studies use biopsy as standard procedure. As the surface electromyography (EMGs) allows to extract information about the recruitment of different motor units, this study is based on the assumption that it is possible to use the EMG to identify different proportions of fiber types in a muscle. The goal of this study was to identify the characteristics of the EMG signals which are able to distinguish, more precisely, different proportions of fiber types. Also was investigated the combination of characteristics using appropriate mathematical models. To achieve the proposed objective, simulated signals were developed with different proportions of motor units recruited and with different signal-to-noise ratios. Thirteen characteristics in function of time and the frequency were extracted from emulated signals. The results for each extracted feature of the signals were submitted to the clustering algorithm k-means to separate the different proportions of motor units recruited on the emulated signals. Mathematical techniques (confusion matrix and analysis of capability) were implemented to select the characteristics able to identify different proportions of muscle fiber types. As a result, the average frequency and median frequency were selected as able to distinguish, with more precision, the proportions of different muscle fiber types. Posteriorly, the features considered most able were analyzed in an associated way through principal component analysis. Were found two principal components of the signals emulated without noise (CP1 and CP2) and two principal components of the noisy signals (CP1 and CP2 ). The first principal components (CP1 and CP1 ) were identified as being able to distinguish different proportions of muscle fiber types. The selected characteristics (median frequency, mean frequency, CP1 and CP1 ) were used to analyze real EMGs signals, comparing sedentary people with physically active people who practice strength training (weight training). The results obtained with the different groups of volunteers show that the physically active people obtained higher values of mean frequency, median frequency and principal components compared with the sedentary people. Moreover, these values decreased with increasing power level for both groups, however, the decline was more accented for the group of physically active people. Based on these results, it is assumed that the volunteers of the physically active group have higher proportions of type II fibers than sedentary people. Finally, based on these results, we can conclude that the selected characteristics were able to distinguish different proportions of muscle fiber types, both for the emulated signals as to the real signals. These characteristics can be used in several studies, for example, to evaluate the progress of people with myopathy and neuromyopathy due to the physiotherapy, and also to analyze the development of athletes to improve their muscle capacity according to their sport. In both cases, the extraction of these characteristics from the surface electromyography signals provides a feedback to the physiotherapist and the coach physical, who can analyze the increase in the proportion of a given type of fiber, as desired in each case.

A Síndrome de Sjögren (SS) é uma das três patologias autoimunes mais frequentes na população mundial, ainda assim frequentemente infradiagnosticada. Esta síndrome é uma doença autoimune crónica que afeta as glândulas exócrinas, particularmente as glândulas salivares e lacrimais. A SS primária (SSp) afeta especificamente as glândulas exócrinas, enquanto que a SS secundária (SSs) aparece associada a outras patologias autoimunes sistémicas. A SS é caracterizada, histopatologicamente, por um infiltrado inflamatório linfocitário que interfere com a função glandular normal. Afeta 0,5% da população mundial, sendo claramente mais predominante em mulheres (9:1 versus homens), principalmente por volta dos 50 anos (após a menopausa), ainda que também possa aparecer depois da menarca (entre os 20 e os 30 anos). Considera se que a etiologia da SS é multifatorial. Fatores genéticos, ambientais, hormonais e virais estão implicados na sua etiopatogénese. É muito importante o papel dos Médicos Dentistas no diagnóstico da SSp, uma vez que na maior parte dos casos são eles quem detetam os primeiros sintomas, mais propriamente a boca seca. Diversos testes auxiliares são utilizados para o diagnóstico desta doença, tais como: o teste de Schimer, o teste de Rosa Bengala, a Sialometria, a Sialografia, a Biópsia das glândulas salivares, entre outros. A SSp, regra geral, tem um curso não doloroso, sendo a boca seca e a secura ocular as suas duas características clínicas mais salientes. A maior parte das manifestações orais que se apresentam nestes pacientes são resultado de hipofunção das glândulas salivares (da boca seca), tais como: a cárie dentária, a doença periodontal, as infeções fúngicas, entre outras. Esta doença também se pode associar a problemas a nível sistémico, que podem ser subdivididos em não viscerais (pele, artralgia, mialgia) e viscerais (pulmão, coração, rim, sistema gastrointestinal, sistema endócrino e sistema nervoso central e periférico). O tratamento é empírico, sintomático e direcionado a tratar as complicações da doença mais inicial, que consiste em limitar os danos da xerostomia e da queratoconjuntivite. A prevenção dos sintomas a nível oral e ocular é fundamental em pacientes com SSp, para assim terem maior qualidade de vida.

Criança do sexo feminino, caucasiana, 15 anos, com história de hordéolo da pálpebra inferior do olho esquerdo, medicada com antibiótico e corticóide tópico associado a limpeza palpebral, desde há cerca de 4 semanas. Por ausência de melhoria e com crescimento progressivo da lesão, recorreu ao serviço de urgência. Negava outras queixas oftalmológicas ou sistémicas associadas. Ao exame objectivo observou-se tumefação da pálpebra inferior do olho esquerdo, com eritema mas sem dor ou calor à palpação, sem blefarite. Acuidade visual de 10/10 em ambos os olhos, com movimentos oculares mantidos e sem proptose. Reflexo pupilar mantido. Segmento anterior e fundoscopia sem alterações de relevo. As 2 principais hipóteses de diagnósticas foram celulite pré-septal ou rabdomiossarcoma. A TC de órbita revelou lesão infero-medial da órbita esquerda, com componente pré-septal envolvendo a pálpebra inferior e pós-septal, de densidade identifica à dos músculos extra-oculares, sem aparente envolvimento do recto medial e inferior. Foi realizada biópsia excisional e estabeleceu-se o diagnóstico de rabdomiossarcoma do tipo embrionário (T1N0M0). Iniciou tratamento com quimioterapia (ifosfamida, vincristina, doxorubicina) e radioterapia local (40Gy). Após 1 ano, apresenta –se clinicamente estável, sem recidiva local nem complicações oculares, mantendo uma AV de 10/10. Conclusão: O diagnóstico de rabdomiossarcoma deve ser lembrado, pois apesar de raro, é o tumor maligno primário da órbita mais frequente na criança. Na presença de uma massa palpebral de rápido crescimento geralmente indolor e/ou proptose, a hipótese de rabdomiossarcoma orbitário deve ser equacionada. Para se obterem bons resultados terapêuticos do ponto de vista de sobrevida e funcional é essencial um diagnóstico e tratamento precoce.

Female genital tuberculosis remains a major health problem in developing countries and is an important cause of infertility. As symptoms, laboratory data and physical fndings are non-specifc, its diagnosis can be diffcult. We describe a case of a 39-year-old woman suffering from peri-umbilical pain and increased abdominal size for one year, anorexia, asthenia, weight loss, occasionally dysuria and dyspareunia, and four months amenorrhea. Laboratory data revealed cancer antigen 125 (CA-125) level of 132.3 U/mL, erythrocyte sedimentation rate of 42 mm/h, and gamma-globulins of 2.66 g/dL. Computer Tomography scan showed loculated ascites. It was initially suspected a carcinomatous origin, but ascites evaluation was negative for malignant cells. Magnetic Resonance Imaging from another hospital showed endometrial heterogeneity. Therefore, an endometrial biopsy was performed demonstrating an infammatory infltrate with giant cells of type Langhans and bacteriological culture identifed Mycobacterium tuberculosis

O carcinoma do endométrio apresenta uma taxa de incidência em Portugal de cerca de 7.2%, sendo a 5ª neoplasia mais comum na mulher. Apesar de apresentar uma prevalência relativamente elevada, o seu prognóstico global é favorável, uma vez que 75% dos casos são diagnosticados em estádio precoce. O estudo por ressonância magnética é geralmente efectuado após a realização de uma ecografia para avaliação de uma hemorragia uterina anormal e após o diagnóstico histológico por histeroscopia ou ressecção. Contudo, a ressonância magnética pode apresentar um papel determinante no diagnóstico em casos de impossibilidade de biópsia e nos quais a biópsia é inconclusiva. Além do mais, apesar de esta técnica não ser contemplada na classificação para o estadiamento do carcinoma do endométrio da International Federation of Gynecology and Obstetrics de 2009, apresenta uma função fundamental no estadiamento pré-operatório destas doentes, sendo crucial para definir a abordagem cirúrgica e terapêutica. No presente artigo, as autoras descrevem o estado da arte da ressonância magnética funcional no diagnóstico e no estadiamento do carcinoma do endométrio, chamando a atenção para o papel do estudo dinâmico após administração de contraste endovenoso e do estudo ponderado em difusão nestes cenários através da revisão da literatura mais recente sobre este tópico.
