Optically transparent cocatalyst film materials is very desirable for improved photoelectrochemical (PEC)oxygen evolution reaction (OER) over light harvesting photoelectrodes which require the exciting light to irradiate through the cocatalyst side, i.e., front-side illumination. In view of the reaction overpotential at electrode/electrolyte interface, the OER electrocatalysts have been extensively used as cocatalysts for PEC water oxidation on photoanode. In this work, the feasibility of a one-step fabrication of the transparent thin film catalyst for efficient electrochemical OER is investigated. The Ni-Fe bimetal oxide films, 200 nm in thickness, are used for study. Using a reactive magnetron co-sputtering technique, transparent(> 50% in wavelength range 500-2000 nm) Ni-Fe oxide films with high electrocatalytic activities were successfully prepared at room temperature. Upon optimization, the as-prepared bimetal oxide film with atomic ratio of Fe/Ni = 3:7 demonstrates the lowest overpotential for the OER in aqueous KOH solution, as low as 329 mV at current density of 2 mA cm 2, which is 135 and 108 mV lower than that of as-sputtered FeOx and NiOx thin films, respectively. It appears that this fabrication strategy is very promising to deposit optically transparent cocatalyst films on photoabsorbers for efficient PEC water splitting.

Serão avaliadas as dimensões do bem-estar físico e actividades quotidianas, bem-estar psicológico, relações sociais e sintomas. ----- ABSTRACT - Breast Cancer is a worldwide public health concern due to the incidence, mortality and economic costs associated. Although effective, therapies used in its treatment lead to changes in all Quality of Life (QoL) dimensions of a woman suffering from Breast Cancer. QoL is an outcome measure, and the insurance of quality of care provided should be a priority to health organizations. Taking into consideration that in Portugal there is a potential difference in the way women with Breast Cancer are provided with physical therapy, it is important to know whether physical therapy does or does not influence the QoL of women with breast cancer. If it does, it will lead to a health care quality improvement to cancer patients. The goal of the following study is to build an analysis model in order to answer the initial investigation question: “Does Physical Therapy contribute to enhance the Quality of Life of women with breast cancer who underwent surgery and other oncology treatments?” The project was divided in different stages. Initially, a literature revision was elaborated and exploratory interviews were held, which allowed an actual knowledge of the themes that define the variables and the object of study. The next stage included a critical analysis of the theme, which allowed the definition of variables of study, the choice of instrument of measure and the acquisition of some knowledge on how to proceed. After the definition of the general goal (to evaluate the influence of physical therapy on the QoL of women with breast cancer who underwent surgery and other oncology treatments) and specific goals, the choice of a right methodology took place, in order to answer the investigation questions (type of study, variables, unit analysis, methods and techniques on data collection, procedures and data treatment). In the scope of the project, it is decided to put out on the field an effective case-study which assures a real contribution on the choice of te methodology. In this particular work, there was a pilot study, included in the methodology procedures, with the goal of obtaining conclusions on the applicability of the instrument of measure; the length of time to collect data, the socio-demographic and clinical characteristics of the sample; the investigation questions. The pilot study consisted on a one group pretest-postest design, with a sample of 35 individuals who underwent surgery and other oncology treatments. Dimensions such as physical well-being and everyday life activities, psychological well-being, social relationships, symptoms and socio-demographical/clinical characteristics were assessed at the beginning of physical therapy individual treatment and at the moment of release. The instrument of measure used was the EORTC QLQ–30 questionnaire and its complementary questionnaire EORTC QLQ–23. A chart was made in order to collect socio-demographic and clinical data. Statistic significance was accepted for values of p<0,05. To compare between groups and to detect the evolution within each group, the t-student test and the Mann-Whitney test were applied. The outcome analysis of the pilot study allowed to verify that: - The instrument of measure proposed (EORTC QLQ30 and BR23) was easy to apply, and the subjects did not show any difficulty in filling it up. There was also no problem on calculating the scores or interpreting them; - A considerable part of the women with breast cancer will be submitted to protocols that may occur throughout several months after surgery (e.g., QT+RT+HT). This reality leads us to suggest several moments of assessment of the QoL dimensions in various moments of the different protocol treatments. We consider that the ideal number of evaluations would be 4 (3/4 weeks, 3 months, 6 months and 9 months after surgery). We also suggest the use of a larger sample; - Since the pilot study resorted to a one group pretest-postest design (there is an absence of control group and only two moments of assessment), there is no consistency of outcome. However, the results obtained indicate that physical therapy influences the dimensions of QoL on women with breast cancer who underwent surgery and other oncology treatments, which may be an asset to the quality of care provided to cancer patients. The outcome of the pilot study allowed to redefine the methodology given as adequate to answer the initial investigation question. Our suggestion is as follows: quasi-experimental design, with a sample of 120 subjects (2 groups of 60 women) with breast cancer who underwent surgery and other oncology treatments. The experimental group will be submitted to individual treatments of physical therapy. Dimensions such as physical well-being and everyday life activities, psychological well-being, social relationships and symptoms will be assessed. The collection of data will occur at 3 weeks, 3 months, 6 months and 9 months after surgery. The instrument of measure is the EORTC QLQ–30 questionnaire and its complementary questionnaire EORTC QLQ–23, and social-demographic and clinical information will also be collected. The statistic significance will be accepted for values of p<0,05. Parametric and non-parametric tests will be used to compare between groups and to detect the evolution within each group. Carrying out a study that followed the methodology discussed above would allow a better consistency of results, possibly enabling the confirmation that physical therapy influences the QoL of women with breast cancer who underwent surgery and other oncology treatments. The evidence that physical therapy influences the QoL of women with breast cancer, and the fact that QoL is an indicator of quality of care provided to cancer patients, may work as a facilitating agent in the change of human resources management in health organizations associated to oncology, which will lead to a change in oncology physical therapy practice patterns in Portugal, guiding to a health care quality improvement to cancer patients.

The recognition profile of the tissue cysts antigens by IgG antibodies was studied during acute and chronic human toxoplasmic infection. Thus the IgG response against Toxoplasma gondii was investigated by immunoblotting in two patients accidentally infected with the RH strain as well as in group of naturally infected patients at acute and chronic phase. There was an overall coincidence of molecular mass among antigens of tachyzoites and tissue cysts recognized by these sera, however, they appear not to be the same molecules. The response against tissue cysts starts early during acute infection, and the reactivity of antibodies is strong against a wide range of antigens. Six bands (between 82 and 151 kDa) were exclusively recognized by chronic phase sera but only the 132 kDa band was positive in more than 50% of the sera analysed. A mixture of these antigens could be used to discriminate between the two infection phases. The most important antigens recognized by the acute and the chronic phase sera were 4 clusters in the ranges 20-24 kDa, 34-39 kDa, 58-80 kDa and 105-130 kDa as well as two additional antigens of 18 and 29 kDa. Both accidentally infected patients and some of the naturally infected patients showed a weak specific response against tissue cyst antigens.

The use of questionnaires has been recommended for identifying, at a lower cost, individuals at risk for schistosomiasis. In this study, validity of information obtained by questionnaire in the screening for Schistosoma mansoni infection was assessed in four communities in the State of Minas Gerais, Brazil. Explanatory variables were water contact activities, sociodemographic characteristics and previous treatment for schistosomiasis. From 677, 1474, 766 and 3290 individuals eligible for stool examination in the communities, 89 to 97% participated in the study. The estimated probability of individuals to be infected, if they have all characteristics identified as independently associated with S.mansoni infection, varied from 15% in Canabrava, to 42% in Belo Horizonte, 48% in Comercinho and 80% in São José do Acácio. Our results do not support the hypothesis that a same questionnaire on risk factors could be used in screening for S.mansoni infection in different communities.

Proteins secreted to the extracellular environment or to the periphery of the cell envelope, the secretome, play essential roles in foraging, antagonistic and mutualistic interactions. We hypothesize that arms races, genetic conflicts and varying selective pressures should lead to the rapid change of sequences and gene repertoires of the secretome. The analysis of 42 bacterial pan-genomes shows that secreted, and especially extracellular proteins, are predominantly encoded in the accessory genome, i.e. among genes not ubiquitous within the clade. Genes encoding outer membrane proteins might engage more frequently in intra-chromosomal gene conversion because they are more often in multi-genic families. The gene sequences encoding the secretome evolve faster than the rest of the genome and in particular at non-synonymous positions. Cell wall proteins in Firmicutes evolve particularly fast when compared with outer membrane proteins of Proteobacteria. Virulence factors are over-represented in the secretome, notably in outer membrane proteins, but cell localization explains more of the variance in substitution rates and gene repertoires than sequence homology to known virulence factors. Accordingly, the repertoires and sequences of the genes encoding the secretome change fast in the clades of obligatory and facultative pathogens and also in the clades of mutualists and free-living bacteria. Our study shows that cell localization shapes genome evolution. In agreement with our hypothesis, the repertoires and the sequences of genes encoding secreted proteins evolve fast. The particularly rapid change of extracellular proteins suggests that these public goods are key players in bacterial adaptation.

Since the discovery of the first penicillin bacterial resistance to β-lactam antibiotics has spread and evolved promoting new resistances to pathogens. The most common mechanism of resistance is the production of β-lactamases that have spread thorough nature and evolve to complex phenotypes like CMT type enzymes. New antibiotics have been introduced in clinical practice, and therefore it becomes necessary a concise summary about their molecular targets, specific use and other properties. β-lactamases are still a major medical concern and they have been extensively studied and described in the scientific literature. Several authors agree that Glu166 should be the general base and Ser70 should perform the nucleophilic attack to the carbon of the carbonyl group of the β-lactam ring. Nevertheless there still is controversy on their catalytic mechanism. TEMs evolve at incredible pace presenting more complex phenotypes due to their tolerance to mutations. These mutations lead to an increasing need of novel, stronger and more specific and stable antibiotics. The present review summarizes key structural, molecular and functional aspects of ESBL, IRT and CMT TEM β-lactamases properties and up to date diagrams of the TEM variants with defined phenotype. The activity and structural characteristics of several available TEMs in the NCBI-PDB are presented, as well as the relation of the various mutated residues and their specific properties and some previously proposed catalytic mechanisms.

Mini- Mental State Examination. The aging process in institutionalized populations has implications that encompass various issues related to health, including occupational development and the performance of daily activities. It was this study’s objective to identify the occupational needs of a sample of institutionalized elderly and to identify the existence of changes in occupational performance and satisfaction after participation in a program of therapeutic activities. The sample consisted of 20 individuals (mostly female and widowed) from two nursing homes in Lisbon, having various levels of education and an average age of 86,2 years (SD=6,0) as well as the ability of verbal expression without severe cognitive deficit. The instruments used were a structured interview for socio-demographic characterization, the Mini Mental State Examination, the Katz Index and the Canadian Occupational Performance Measure. We found that older respondents claimed that the performance of daily activities constituted a problem. They stated that the activities of self-care and leisure were the most problematic, that they had negative self-perceptions regarding their own performance, and that they were dissatisfied with this performance of their daily activities. After participating in program activities, eight respondents experienced no change in their respective grades of performance and satisfaction, five showed increases in both grades and, for two respondents, the performance remained unchanged while the satisfaction level showed a decline. Three stopped having at least one problem in performance, and one participant even stopped referring feeding problems, having become independent in this area. Regarding the evolution of the Canadian Measure performance and satisfaction scores (before versus after the intervention), we found no significant differences. In conclusion, most participants stated that they were performing badly in what concerned the occupational activities they valued the most. The same applies to low levels of satisfaction regarding that performance. In this small sample, and despite some benefits, the program of therapeutic activities did not prove to be significantly effective.

BACKGROUND: In ST-segment elevation myocardial infarction (STEMI) patients treated with primary angioplasty, neutrophil response and its prognostic significance are not entirely understood. METHODS: We retrospectively studied 305 consecutive and non-selected STEMI patients. They were divided into three groups according to the maximum neutrophil percentage in the first 48 hours. We compared baseline demographic characteristics, coronary disease risk factors, cardiac history, clinical presentation, therapeutics administered and clinical evolution. We then assessed survival in the three groups and determined predictors of 30-day mortality. Group 1 (G1) had a mean age of 57 +/- 14 years and showed mean neutrophilia of 73.3%, Group 2 (G2) 61 +/- 13 years and 79.9%, and Group 3 (G3) 66 +/- 13 years and 84.2%. We compared outcomes and 30-day mortality between the groups. RESULTS: Mean age rose with increased neutrophil response. There were no statistically significant baseline differences between the groups except for more smokers in Groups 1 and 2, and more patients presenting with Killip class > or = 2 and fewer with uncomplicated evolution in Group 3. During 30-day follow-up there were 19 deaths (G1=1, G2=3 and G3=15). In univariate analysis mortality predictors were age > or = 75 years, anterior STEMI, maximum creatinine kinase > or = 2500 UI/L, culprit lesion in proximal anterior descending artery, incomplete revascularization, Killip > or = 2 at presentation, and being in G3. After multivariate regression analysis independent predictors were age > or = 75 years, incomplete revascularization and being in G3. CONCLUSION: In myocardial infarction patients undergoing mechanical revascularization, an intense neutrophil response (routinely, easily and inexpensively assessed) is related to worse short-term prognosis.

A total of 24 patients with syphilis and HIV infection were treated from January 1997 to March 2003 at the Infectious Dermatology Outpatient Clinic of the Evandro Chagas Clinical Research Institute, Oswaldo Cruz Foundation, Rio de Janeiro, Brazil. The caseload consisted of 20 males (83.3%) and four females (16.7%), with a mean age of 38.04 years and mean T CD4+ count of 389.5 cells/mL. Syphilis was diagnosed as secondary in 16 (62.5%) patients, late latent in eight (33.3%), and tertiary in one (4.2%). Manifestations of secondary syphilis were palmar and plantar erythematopapulous cutaneous lesions in nine (37.5%), papulous exanthema in four (16.7%), patchy alopecia in 3 (12.5%) and osteochondritis in one patient (4.2%). Tertiary syphilis was characterized by verrucous lesions. Neurosyphilis was diagnosed in four patients (16.7%), with headache as the only manifestation in two patients. Drugs used in treatment included benzathine penicillin, ceftriaxone, erythromycin, and crystalline penicillin. Cure was achieved in 18 patients (75%). Five patients (20.8%) were retreated, three of whom presented a history of re-exposure. This study confirms the importance of establishing the diagnosis of neurosyphilis in patients with HIV infection, in addition to performing follow-up on treatment for syphilis.

The ability to control human immunodeficiency virus type 1 (HIV-1) infection and progression of the disease is regulated by host and viral factors. This cross-sectional study describes the socio-demographic and epidemiological characteristics associated with HIV-1 infection in 1,061 subjects attended in Londrina and region, south of Brazil: 136 healthy individuals (Group 1), 147 HIV-1-exposed but uninfected individuals (Group 2), 161 HIV-1-infected asymptomatic patients (Group 3), and 617 patients with AIDS (Group 4). Data were obtained by a standardized questionnaire and serological tests. The age of the individuals ranged from 15.1 to 79.5 years, 54.0% and 56.1% of the Groups 3 and 4 patients, respectively, were men. The major features of groups 2, 3, and 4 were a predominance of education level up to secondary school (55.8%, 60.2% and 62.4%, respectively), sexual route of exposure (88.4%, 87.0% and 82.0%, respectively), heterosexual behavior (91.8%, 75.2% and 83.7%, respectively), and previous sexually transmitted diseases (20.4%, 32.5%, and 38.1%, respectively). The patients with AIDS showed the highest rates of seropositivity for syphilis (25.6%), of anti-HCV (22.3%), and anti-HTLV I/II obtained by two serological screening tests (6.2% and 6.8%, respectively). The results documenting the predominant characteristics for HIV-1 infection among residents of Londrina and region, could be useful for the improvement of current HIV-1 prevention, monitoring and therapeutic programs targeted at this population.

BACKGROUND: The pathogenesis of chronic hepatitis C is still a matter of debate. CD4+ and CD8+ T lymphocytes (TL) are typically observed within the portal and periportal spaces of affected livers, but their functional role in hepatitis C progression has not been fully elucidated. METHODS: CD4+ and CD8+ TL were quantified by immunohistochemistry in portal and periportal spaces of 39 liver biopsies from patients with chronic hepatitis C. They were associated to demographic data, histological parameters, laboratory findings of patients and hepatitis C genotypes. RESULTS: There was high numbers of CD4+ and CD8+ TL from which the density of CD4+ T was higher than CD8+ TL in portal and periportal spaces. CD4+ and CD8+ TL were directly correlated to intensity of interface hepatitis. CD8+ TL correlated to serum enzyme levels. CONCLUSION: The high numbers of CD4+ and CD8+ TL in portal and periportal spaces and their correlation to interface hepatitis suggest that hepatitis C evolution depends on the action of intrahepatic T lymphocytes, lending support to the notion of an immune-mediated mechanism in the pathogenesis of chronic hepatitis C.
