Überinfusion von Verbrennungsopfern: häufig und schädlich [Over infusion in burn victims: frequent and injurious]

Major burns are characterized by an initial capillary leak, which requires fluid resuscitation for hemodynamic stabilization. While under resuscitation was the major cause of death until the 1980s, over resuscitation has become an important source of complications, including abdominal compartment syndrome, escharosis, impaired gas exchange with prolonged mechanical ventilation and hospital stay. Fluid over infusion started in the 1990s with an increasing proportion of the fluid delivered within the first 24 h being well above the 4 ml/kg/% burn surface area (BSA) according to the Parkland formula. The first alerts were published in the form of case reports of increased mortality due to abdominal compartment syndrome and respiratory failure. This paper analyses the causes of this fluid over infusion and the ways to prevent it, which include rationing prehospital fluid delivery, avoiding early administration of colloids and prevention by permissive hypovolemia.

Ideas previas de una población universitaria sobre el desarrollo infantil

El presente estudio analiza las ideas sobre el calendario evolutivo de 1.460 estudiantes que acaban de entrar en la Universidad. Las respuestas dadas a un cuestionario en el que se plantean diversas preguntas sobre la edad en que aparecen determinadas conductas y sobre la edad en que es oportuno interactuar con los niños, son, en términos generales, pesimistas (los estudiantes predicen edades tardías) e inexactas (porcentajes pequeños de aciertos). Se analiza también como las predicciones varían en funcion de determinadas variables (sexo, paternidad, edad, etc.) y en función de los conocimientos adquiridos por los estudiantes tras un año de enseñanza en psicologia evolutiva.

Bewertung des Risikos für Bias in kontrollierten Studien [Assessment of risk of bias in controlled studies].

BACKGROUND: Practicing physicians are faced with many medical decisions daily. These are mainly influenced by personal experience but should also consider patient preferences and the scientific evidence reflected by a constantly increasing number of medical publications and guidelines. With the objective of optimal medical treatment, the concept of evidence-based medicine is founded on these three aspects. It should be considered that there is a high risk of misinterpreting evidence, leading to medical errors and adverse effects without knowledge of the methodological background. OBJECTIVES: This article explains the concept of systematic error (bias) and its importance. Causes and effects as well as methods to minimize bias are discussed. This information should impart a deeper understanding, leading to a better assessment of studies and implementation of its recommendations in daily medical practice. CONCLUSION: Developed by the Cochrane Collaboration, the risk of bias (RoB) tool is an assessment instrument for the potential of bias in controlled trials. Good handling, short processing time, high transparency of judgements and a graphical presentation of findings that is easily comprehensible are among its strengths. Attached to this article the German translation of the RoB tool is published. This should facilitate the applicability for non-experts and moreover, support evidence-based medical decision-making.

Esto se dio de modomanifiesto en América Latina en torno a las décadas de 1960 y 1970, donde sedesarrollaron, paralelamente, una corriente filosófica con pretensiones de liberación queera fruto de la preocupación por la autenticidad latinoamericana y un movimientogeneralizado de lucha armada protagonizado por diversas organizaciones guerrilleras.
