Introduction: Malgré des taux d’efficacité comparable du traitement antiviral de l’hépatite C (VHC) entre utilisateurs de drogues par injection (UDIs) et non-UDIs, il y a encore d’importantes barrières à l’accessibilité au traitement pour cette population vulnérable. La méfiance des UDIs à l’égard des autorités médicales, ainsi que leur mode de vie souvent désorganisé ont un impact sur l’initiation du traitement. L’objectif de cette étude est d’examiner les liens entre l’initiation du traitement du VHC et l’utilisation des services de santé chez les UDIs actifs. Methode: 758 UDIs actifs et séropositifs aux anticorps anti-VHC ont été interrogés durant la période de novembre 2004 à mars 2011, dans la région de Montréal. Des questionnaires administrés par des intervieweurs ont fourni des informations sur les caractéristiques socio-économiques, ainsi que sur les variables relatives à l’usage de drogues et à l’utilisation des services de santé. Des échantillons sanguins ont été prélevés et testés pour les anticorps anti-VHC. Une régression logistique multivariée a permis de générer des associations entre les facteurs relatifs aux services de santé et l’initiation du traitement contre le VHC. Resultats: Parmi les 758 sujets, 55 (7,3%) avaient initié un traitement du VHC avant leur inclusion dans l’étude. Selon les analyses multivariées, les variables significativement associées à l’initiation du traitement sont les suivantes: avoir vu un médecin de famille dans les derniers 6 mois (Ratio de Cote ajusté (RCa): 1,96; Intervalle de Confiance à 95% (IC): 1,04-3,69); plus de 2 ans sous traitement de la dépendance à vie, sans usage actuel de méthadone (RCa: 2,25; IC: 1,12-4,51); plus de 2 ans sous traitement de la dépendance à vie, avec usage actuel de méthadone (RCa: 3,78; IC: 1,85-7,71); et avoir déjà séjourné en prison (RCa: 0,44; IC: 0,22-0,87). Conclusion: L’exposition à des services d’aide à la dépendance et aux services médicaux est associée à l’initiation du traitement du VHC. Ces résultats suggèrent que ces services jouent leur rôle de point d’entrée au traitement. Alternativement, les UDIs ayant initié un traitement du VHC, auraient possiblement adopté une attitude proactive quant à l'amélioration de leur santé globale. D’autre part, l’incarcération ressort comme un obstacle à la gestion de l’infection au VHC.

Cette recherche tente de comprendre le rôle de la culture des nouveaux médias (Jenkins, 2006a) dans le processus d'affirmation qui mène au dévoilement de l'orientation sexuelle (aussi nommé « sortie du placard ») des jeunes gais, population souvent perçue en tant que vulnérable. Pour ce faire, une approche interprétative a été choisie ainsi qu'une méthode d'entrevues rétrospectives semi-structurées et axées sur la participation des jeunes gais en ligne, telle qu'abordée par Ito et al. (2010). Suite aux deux entrevues avec chacun des cinq participants, il semble que leur engagement en ligne soit influencé par leurs contextes hors ligne. Les résultats montrent le rôle des nouveaux médias dans la construction identitaire et la sortie du placard des participants, notamment via l'exploration des relations affectives, la création et la gestion de profils et leurs réflexions sur la culture néomédiatique et sur « être gai ». Toutefois, le cœur de la discussion est centré sur le processus de ce que j'appelle l'accueil de la différence. J'aborde et je discute alors des concepts tels que la différence (Hall, 1997)et la normativité sociale (Butler, 1990; Duggan, 2002).

Le stress chronique en milieu de travail est lié à une variété d’effets négatifs tant physiologiques que psychologiques tels l’anxiété, la dépression et l’épuisement professionnel (Taylor et al., 1997). Les professionnels de la santé et les travailleurs sociaux étant les plus touchés (Felton, 1998), les intervenants des centres de réadaptation de jeunes tels le Centre-Jeunesse de Montréal-Institut Universitaire (CJM-IU) sont donc une population particulièrement vulnérable. L’objectif principal de la présente étude était de tester auprès de 70 intervenants du CJM-IU l’effet d’un programme web de gestion de stress chronique auto-administré sur le stress psychologique et physiologique puisque nous le savons, le stress implique la sécrétion d’hormones et l’activation du système nerveux. Nous avions pour hypothèse que cet effet si présent, aura un impact subséquent sur la dépression, l’anxiété et l’épuisement professionnel. Le programme testé, Stress et Compagnie, développé par le Centre d’Études sur le Stress Humain de l’Institut Universitaire en Santé Mentale de Montréal est basé sur des récentes découvertes en psychoneuroendocrinologie. L’évaluation a été faite par un devis expérimental avec groupe expérimental et témoin avec un pré-test et deux post-test dont un directement après le programme et un autre un mois plus tard. Les résultats sans être significatifs tendent à suggérer que le programme permet de réduire le stress chronique psychologique chez un sous groupe d’intervenants, notamment ceux n’ayant pas été exposés à un évènement potentiellement traumatisant lors de la dernière année. Les effets sur le stress physiologique sont plus instables.

In Sri Lanka policy responses have direct impacts on rural dwellers. Over 80% of Sri Lanka’s population live in rural areas and 90% of them represent low income dwellers. Their production system may be hampered by fragmented landholding, poor economics of scale, low investment levels resulting from poor financial services as well as inappropriate or limited technology. They are vulnerable to price hikes of basic foods and food security issues due to fragmented landholding and poor financial services. Policy measures to reduce the transmission of higher international prices in domestic markets exist to protect the food security of the vulnerable population. This paper will discuss the food policy and strategies implemented by the government and outside to the above facts this paper also describes the effectiveness of the policies forwarded by the government. The objective of this study is to analyse the impact of policy responses to the food price crisis and rural food security in Sri Lanka. Outside of the above facts this study also treats the impact of policies and decisions on the nutritional condition of rural dwellers. Furthermore this study is to analyse the fluctuation of buying power with the price hikes and the relation of above facts with issues like malnutrition. This paper discusses why policy makers should pay greater attention to rural dwellers and describes the multiple pathways through which food price increases have on rural people. It also provides evidence of the impact of this crisis in particular, through hidden hunger, and discusses how current policy responses should adjust and improve to protect the rural dwellers in the short and long term.

La crisis de refugiados como consecuencia de la inestabilidad política en Costa de Marfil, ha desencadenado en una amenaza regional. La población vulnerable presente en la frontera porosa que caracteriza a Costa de Marfil y Liberia, es objeto de actividades ilícitas como el narcotráfico, tráfico de diamantes y tráfico de armas. Es entonces, como la crisis de refugiados marfileños resulta ser un elemento desestabilizador de las relaciones bilaterales entre Costa de Marfil y Liberia, en tanto se facilita el tráfico de armas entre combatientes liberianos y los refugiados marfileños. Este panorama, sin duda, intensifica el conflicto interno que hoy en día tiene lugar en Costa de Marfil.

Esta investigación tiene el propósito de analizar los aspectos jurídicos de los desastres naturales y del reasentamiento poblacional. Se presentan las distintas tipologías del reasentamiento poblacional desde la óptica estatal para señalar su importancia en las actividades de las autoridades administrativas. En el trabajo se muestra la vulnerabilidad del Estado Colombiano frente a la ocurrencia de desastres naturales, por la falta de planeación efectiva y la poca importancia de la reducción del riesgo en la agenda pública. Desarrolla igualmente, el fundamento de la responsabilidad de la administración frente a desastres de la naturaleza y su papel ante la protección de los derechos humanos. Además, desarrolla el reasentamiento poblacional como mecanismo de atención y prevención de desastres naturales, y la normatividad nacional y de la región andina en la cual está contenido. Establece estrategias de acción estatal para la atención práctica de los desastres naturales, el planteamiento del reasentamiento poblacional como política pública y herramientas de protección de la población vulnerable. Por último, plantea la importancia de la reducción de los riesgos de la población ante desastres de la naturaleza y la necesidad de protección del Estado de las poblaciones vulnerables para mejorar sus condiciones de vida.

This article aims to present an approach to the issue of farm or rural zone workers, including a labour law study of agrarian legal decisions, so as to demonstrate their importance in respect to social, economic and cultural rights in Colombia. The study will serve to illustrate through the history, the applicable law and the jurisprudence, the different ways in which farmers have been treated from the time of the origin until the arrival of modern systems of industrialization. It calls into question the effectiveness of existing laws and the role of the courts, in spite of globalization, to maintain the minimum rights and guarantees of farm workers who are considered to be a vulnerable population. In conclusion, this study seeks to illustrate the current role of the Labor law and the National Health Service in the area of demonstrating of the existence or absence of mechanisms to protect workers in rural areas and the need to create some mechanisms that involve social justice given its prime importance in the Constitution of 1991.

O presente trabalho busca analisar os motivos pelos quais o Estado realiza parceria com organizações da sociedade civil na prestação de serviços públicos. Para tanto, tomamos como exemplo a política pública nacional de combate ao HIV/Aids, capitaneada pelo Programa Nacional de DST/Aids do Ministério da Saúde, tido como referência na realização de parcerias com estas organizações. A análise é conduzida tentando responder três questões básicas: os motivos da parceria, as áreas ou as modalidades de serviço onde esta parceria ocorre e como se dão os mecanismos de contratualização. O trabalho mostra a construção de uma política de parcerias a partir da constatação das dificuldades do Estado em prestar diversos serviços dentro desta política, serviços estes que já eram executados com sucesso por muitas organizações não-governamentais. A partir disso se estabelece uma espécie de divisão de trabalho, na qual as ONGs desempenharão diversas atividades no campo da prevenção, assistência e direitos humanos, notadamente com as chamadas populações mais vulneráveis à epidemia. Após vários anos centralizada no governo federal, esta política passa por um processo de descentralização, inserindo de forma mais definitiva os níveis estadual e municipal de governo. No bojo deste processo, percebe-se um conflito ainda não resolvido a respeito da ¿titularidade¿ destes serviços: se serão incorporados pelo Estado ou se continuarão sendo executados pelas ONGs. Apesar disso, a política de descentralização foi pactuada de forma a se garantir que um percentual mínimo de recursos destinadas a cada unidade da federação seja destinado ao apoio de projetos destas organizações, representando uma institucionalização desta política de parcerias.

Iron deficiency is the commonest nutritional deficiency in the world. Although it affects adults, particularly women of reproductive age and during gestation, the most vulnerable group is children under two years of age. It affects mainly people living in developing countries, who have less access to balanced diets and health services and are exposed to precarious sanitary conditions. Iron deficiency has an impact on the immunity, adult work capacity and the cognitive development of children. Combating and preventing iron deficiency is one of the priorities in promoting public health. The strategies to achieve this end include the evaluation and correction of iron deficiency in pregnant women, the encouragement of breast feeding, oral iron supplementation in premature and newborn babies of low birth-weight, food enrichment and mobilization of the community.

Preface This study was prepared for the Government of Jamaica following the significant physical damage and economic losses that the country sustained as a result of flood rains associated with the development of Hurricane Michelle. The Planning Institute of Jamaica (PIOJ) submitted a request for assistance in undertaking a social, environmental and economic impact assessment to the Economic Commission for Latin America and the Caribbean (ECLAC) on 14 November 2001. ECLAC responded with haste and modified its work plan to accommodate the request. A request for training in the use of the ECLAC Methodology to be delivered to personnel in Jamaica was deferred until the first quarter of 2002, as it was impossible to mount such an initiative at such short notice. This appraisal considers the consequences of the three instances of heavy rainfall that brought on the severe flooding and loss of property and livelihoods. The study was prepared by three members of the ECLAC Natural Disaster Damage Assessment Team over a period of one week in order to comply with the request that it be presented to the Prime Minister on 3 December 2001. The team has endeavoured to complete a workload that would take two weeks with a team of 15 members working assiduously with data already prepared in preliminary form by the national emergency stakeholders. There is need for training in disaster assessment as evidenced by the data collected by the Jamaican officials engaged in the exercise. Their efforts in the future will be more focused and productive after they have received training in the use of the ECLAC Methodology. This study undertakes a sectoral analysis leading to an overall assessment of the damage. It appraises the macroeconomic and social effects and proposes some guidelines for action including mitigating actions subsequent to the devastation caused by the weather system. The team is grateful for the efforts of the Office of Disaster Preparedness and Emergency Management (ODPEM), the associated government ministries and agencies, the Statistical Institute of Jamaica (STATIN), the Planning Institute of Jamaica and the Inter American Development Bank (IDB) for assistance rendered to the team. Indeed, it is the recommendation of the team that STATIN is poised to play a pivotal role in any disaster damage assessment and should be taken on board in that regard. The direct and indirect damages have been assessed in accordance with the methodology developed by ECLAC (1). The results presented are based on the mission's estimates. The study incorporates the information made available to the team and evidence collected in interviews and visits to affected locations. It is estimated that the magnitude of the losses exceeds the country's capacity to address reparations and mitigation without serious dislocation of its development trajectory. The government may wish to approach the international community for assistance in this regard. This appraisal is therefore designed to provide the government and the international community with guidelines for setting national and regional priorities in rehabilitation and reconstruction or resettlement programmes. A purely economic conception of the problem would be limited. A more integrated approach would have a human face and consider the alleviation of human suffering in the affected areas while attending to the economic and fiscal fallout of the disaster. Questions of improved physical planning, watershed management, early warning, emergency response and structural preparedness for evacuation and sheltering the vulnerable population are seen as important considerations for the post disaster phase. Special attention and priority should be placed on including sustainability and increased governance criteria in making social and productive investments, and on allocating resources to the reinforcing and retrofitting of vulnerable infrastructure, basic lifelines and services as part of the reconstruction and rehabilitation strategy. The Jamaican society and government face the opportunity of undertaking action with the benefit of revised paradigms, embarking on institutional, legal and structural reforms to reduce economic, social and environmental vulnerability. The history of flood devastation in the very areas of Portland and St. Mary shows a recurrence of flooding. Accounts of flooding from the earliest recorded accounts pertaining to 1837 are available. Recurrences in 1937, 1940, 1943 and 2001 indicate an ever-present probability of recurrence of similar events. The Government may wish to consider the probable consequences of a part of its population living in flood plains and address its position vis-à­¶is land use and the probability of yet another recurrence of flood rains. This issue of the Bulletin deals with road safety, which has become an urgent worldwide problem. Given the fact that road accidents are increasing, that they affect the planet's most vulnerable population (namely the lowest income groups in developing countries) and that this is becoming a genuine public health crisis, the United Nations has decided that it is urgent to address the matter. The World Health Organization (WHO) has therefore dedicated the World Health Day 2004 to road safety.Given the urgent need for action, the Chiefs of Transport of the five Regional Economic Commissions of the United Nations held a meeting in Geneva (September 2004), where they agreed to reinforce the studies and projects carried out in this area.Below is a summary of various information sources and initiatives adopted to assess and tackle this modern epidemic and offers a pessimistic outlook for 2020, when road traffic crashes will constitute the third cause of death unless serious action is taken from today.
