BACKGROUND: Secondary prevention programs for patients experiencing an acute coronary syndrome have been shown to be effective in the outpatient setting. The efficacy of in-hospital prevention interventions administered soon after acute cardiac events is unclear. We performed a systematic review and meta-analysis to determine whether in-hospital, patient-level interventions targeting multiple cardiovascular risk factors reduce all-cause mortality after an acute coronary syndrome. METHODS AND RESULTS: Using a prespecified search strategy, we included controlled clinical trials and before-after studies of secondary prevention interventions with at least a patient-level component (ie, education, counseling, or patient-specific order sets) initiated in hospital with outcomes of mortality, readmission, or reinfarction rates in acute coronary syndrome patients. We classified the interventions as patient-level interventions with or without associated healthcare provider-level interventions and/or system-level interventions. Twenty-six studies met our inclusion criteria. The summary estimate of 14 studies revealed a relative risk of all-cause mortality of 0.79 (95% CI, 0.69 to 0.92; n=37,585) at 1 year. However, the apparent benefit depended on study design and level of intervention. The before-after studies suggested reduced mortality (relative risk [RR], 0.77; 95% CI, 0.66 to 0.90; n=3680 deaths), whereas the RR was 0.96 (95% CI, 0.64 to 1.44; n=99 deaths) among the controlled clinical trials. Only interventions including a provider- or system-level intervention suggested reduced mortality compared with patient-level-only interventions. CONCLUSIONS: The evidence for in-hospital, patient-level interventions for secondary prevention is promising but not definitive because only before-after studies suggest a significant reduction in mortality. Future research should formally test which components of interventions provide the greatest benefit.

Diplomityön tavoitteena oli antaa kuva vastapainevoimalaitoksen automaation toiminnallisesta suunnittelusta ja soveltaa teoriaa suunnittelemalla toiminnallisuus vesihöyrypiirin tärkeimmille osuuksille. Työssä on esitelty tyypillinen EPCM-voimalaitosprojekti, joka toteutetaan ulkopuolisen avustajan kanssa tehdyllä yhteistyöllä. Projektin koostuu laitoksen suunnittelu-, rakennus-, asennus- sekä käyttöönottovaiheista. Huomioitavia asioita ovat mm. projektin budjetti sekä aikataulu. Valvonnalla on suuri merkitys projektin onnistumiseen. Lisäksi työssä esitellään vastapainevoimalaitoksen vesihöyrypiirin prosessi, pääsäädöt sekä automaatiojärjestelmä. Vesihöyrypiirillä tarkoitetaan syöttövesi-, höyry- ja kaukolämpöjärjestelmää. Pääsäädöillä pyritään saamaan tuotanto vastaamaan kulutusta. Voimalaitoksen painopiste on kaukolämmön tuottaminen. Automaatiojärjestelmän toiminnoilla tarkoitetaan järjestelmän suorittamia ohjauksia, säätöjä sekä hälytyksiä. Toiminnallisessa suunnittelussa tehdään toimilaitteille niin yksittäisohjaukset, säädöt kuin hälytysluettelot. Työssä tehty toiminnallinen suunnittelu keskittyy erityisesti toimilaitteiden säätöpiireihin. Säätöpiirit koostuvat tärkeimmistä prosessiin liittyvistä komponenteista ja säätömerkeistä. Toiminnallisen suunnittelun dokumentaatioita käytetään automaatiojärjestelmän sovellusohjelmoinnin pohjana.

Sähkömarkkinoiden vapautumisen jälkeen energia-alalle on muodostunut entistä suurempi kysyntä kehittyneille energiatiedon hallintaaan erikoistuneille tietojärjestelmille. Uudet lakisäädökset sekä tulevaisuuden kokonaisvaltaiset tiedonkeruujärjestelmät, kuten älykkäät mittarit ja älykkäät sähköverkot, tuovat mukanaan entistä suuremman prosessoitavan tietovirran. Nykyaikaisen energiatietojärjestelmän on kyettävä vastaamaan haasteeseen ja palveltava asiakkaan vaatimuksia tehokkaasti prosessien suorituskyvyn kärsimättä. Tietojärjestelmän prosessien on oltava myös skaalautuvia, jotta tulevaisuuden lisääntyneet prosessointitarpeet ovat hallittavissa. Tässä työssä kuvataan nykyaikaisen energiatietojärjestelmän keskeiset energiatiedon hallintaan ja varastointiin liittyvät komponentit. Työssä esitellään myös älykkäiden mittareiden perusperiaate ja niiden tuomat edut energia-alalla. Lisäksi työssä kuvataan visioita tulevaisuuden älykkäiden sähköverkkojen toteutusmahdollisuuksista. Diplomityössä esitellään keskeisiä suorituskykyyn liittyviä kokonaisuuksia. Lisäksi työssä kuvataan keskeiset suorituskyvyn mittarit sekä suorituskykyvaatimukset. Järjestelmän suorituskyvyn arvioinnin toteuttamiseen on erilaisia menetelmiä, joista tässä työssä kuvataan yksi sen keskeisine periaatteineen. Suorituskyvyn analysointiin käytetään erilaisia tekniikoita, joista tässä diplomityössä esitellään tarkemmin järjestelmän mittaus. Työssä toteutetaan myös case-tutkimus, jossa analysoidaan mittaustiedon sisääntuontiin käytettävän prosessin kahta eri kehitysversiota ja näiden suorituskykyominaisuuksia. Kehitysversioiden vertailussa havaitaan, että uusi versio on selkeästi edellistä versiota nopeampi. Case-tutkimuksessa määritetään myös suorituskyvyn kannalta optimaalinen rinnakkaisprosessien määrä ja tutkitaan prosessin skaalautuvuutta. Tutkimuksessa todetaan, että uusi kehitysversio skaalautuu lineaarisesti.

Il est très commun d’entendre dans les discussions sur l’urbanisme que la participation du public est nécessaire, mais la manière de faire participer reste encore sujette à énormément d’expérimentation et d’incertitudes. Cette étude s’intéresse au design institutionnel des processus de participation en urbanisme, sous l’angle de leurs effets de démocratisation. L’étude cherchera à identifier les aspects déterminants des processus de participation en urbanisme afin d’offrir, à ceux qui les conçoivent, un éclairage sur les implications des choix qu’ils effectuent. L’analyse se construira à partir de l’étude du fonctionnement concret d’une opération de planification concertée à Porto Alegre, dans le secteur Lomba do Pinheiro. Elle s’appuiera sur un découpage des modalités du processus élaboré à partir d’une revue des ouvrages techniques sur la participation en aménagement. Après avoir décrit la culture de la participation et de planification propre à Porto Alegre et comment celle-ci s’est construite, l’analyse décrira, à partir d’observations sur l’évolution du processus et de ses résultats, les effets du processus en termes d’impact sur l’espace bâti, de transformation des relations sociales et de possibilité de faire apparaître des conflits. Les conclusions feront voir l’importance des modalités plus informelles du travail des organisateurs, de même que la nécessité de mettre le projet au centre du processus afin d’établir un terrain commun. Le rôle crucial de la sélection des participants sera souligné. De fortes réserves seront exprimées quant à la pertinence de la transformation des relations sociales comme objectif pour les processus de planification concertée.

Highly heterogeneous mountain snow distributions strongly affect soil moisture patterns; local ecology; and, ultimately, the timing, magnitude, and chemistry of stream runoff. Capturing these vital heterogeneities in a physically based distributed snow model requires appropriately scaled model structures. This work looks at how model scale—particularly the resolutions at which the forcing processes are represented—affects simulated snow distributions and melt. The research area is in the Reynolds Creek Experimental Watershed in southwestern Idaho. In this region, where there is a negative correlation between snow accumulation and melt rates, overall scale degradation pushed simulated melt to earlier in the season. The processes mainly responsible for snow distribution heterogeneity in this region—wind speed, wind-affected snow accumulations, thermal radiation, and solar radiation—were also independently rescaled to test process-specific spatiotemporal sensitivities. It was found that in order to accurately simulate snowmelt in this catchment, the snow cover needed to be resolved to 100 m. Wind and wind-affected precipitation—the primary influence on snow distribution—required similar resolution. Thermal radiation scaled with the vegetation structure (~100 m), while solar radiation was adequately modeled with 100–250-m resolution. Spatiotemporal sensitivities to model scale were found that allowed for further reductions in computational costs through the winter months with limited losses in accuracy. It was also shown that these modeling-based scale breaks could be associated with physiographic and vegetation structures to aid a priori modeling decisions.

Bioaccessibility studies have been widely used as a research tool to determine the potential human exposure to ingested contaminants. More recently they have been practically applied for soil borne toxic elements. This paper reviews the application of bioaccessibility tests across a range of organic pollutants and contaminated matrices. Important factors are reported to be: the physiological relevance of the test, the components in the gut media, the size fraction chosen for the test and whether it contains a sorptive sink. The bioaccessibility is also a function of the composition of the matrix (e.g. organic carbon content of soils) and the physico-chemical characteristics of the pollutant under test. Despite the widespread use of these tests, there are a large number of formats used and very few validation studies with animal models. We propose a unified format for a bioaccessibility test for organic pollutants. The robustness of this test should first be confirmed through inter laboratory comparison, then tested in-vivo.

Today there are many system development projects that break both budget and time plan. Often this depends on defects in the information systems that could have been prevented. The cost of test can in some cases be as high as 50 % of the projects total cost and it's at the same time an important part of development. Test as such has moved its focus from the software it self and its faults to a wider perspective on whole infrastructures of information systems where assure a good quality is important. Sogeti in the Netherlands have developed a test method called TMap (Test Management approach) that can be used for structured testing of information systems. TMap haven't been used as much as desired in the office in Borlänge. Because Microsoft is releasing a new version of their platform Visual Studio Team System (VSTS 2010) some colleges at Sogeti in the Netherlands are about to develop a template that can support the use of TMap in VSTS 2010. When we write this the template is still in development. The goal for Sogeti was to find out the differences between the test functionality in VSTS 2008 and 2010. By using the purpose with this essay, which was to analyze the test process in VSTS 2008 with TMap against the test process in VSTS 2010 together with the template we got much help to achieve the goal. The analysis was done with four different aspects: The TPI and TMMi models, problem and strength analyses and a few question formulations. The TPI and TMMi models where used to analyses and evaluate the test process. The analysis showed that there were differences between the both test processes. VSTS 2010 together with the template gave a better support to use TMap and perform test. In VSTS 2010 the test tool Camano is connected to TFS and the tool is also to make the execution and logging of tests easier. This leads to a test process that is easier to handle and has a better support for TMap.

Tuberculosis (TB) is one of the most important health problems being faced worldwide. In Brazil, the responsibility for the actions of to diagnosis and control of this disease was transferred to the municipalities within the Primary Health Care (PHC), aiming at improvement in epidemiological indicators, requiring reorientation of the practice of family health teams and requiring methodologies to analyze the extent to which components of the PHC are being achieved. Thus, this study aims to analyze the performance of primary care services in the city of Natal-RN for the diagnosis and control of TB, from the perspective of health professionals (doctors and nurses). The study is descriptive, cross-sectional and quantitative. Data collection was conducted from March to July 2011 and involved 121 health professionals working in 52 health units (family health unit, basic health unit and mixed units). The instrument is structured based on the Primary Care Assessment Tool (PCAT), validated and adapted to assess attention to TB in Brazil, and includes questions regarding the Structure and Process components of health services. For quantitative analysis, it was constructed indicators, whose response patterns are followed according to the Likert scale between one and five, which meant the degree of preference relation (or agreement) of the claims. Values between 1 and 3 were considered unsatisfactory for the indicator, between 3 and less than 4, regular, and between 4 and 5, satisfactory. With regard to inputs and equipment, the units had satisfactory condition for form ( = 4.26), consultation ( = 4.02) and basic basket ( = 4.24); regular condition to pot ( = 3.56) and unsatisfactory conditions for transportation tickets ( = 1.50) and sputum smear microscopy ( = 2.42) and X-rays ( = 1.07). In relation to actions, there was satisfactory development for those focused on the individual patient. Actions aimed at the collective level, as the search for respiratory symptoms (RS), monitoring of contacts and guidelines for the community ranged from regular to unsatisfactory ( = 3.16 - = 1.34). With regard to training, 94,2% received training to identify RS. As regards the time for diagnosis, the median time elapsed between the identification of RS and the beginning of treatment it was 22 days. In relation to the difficulties faced by professionals in the diagnosis of TB, 56,2% reported that they are related only to health services, especially for the failure in the rearguard laboratory and in the specialized services reference, the lack of human and material resources and low performing an active search. The professionals perceive the performance of diagnosis and control of TB, permeated with limitations and barriers to organizational and operational character of various sizes, emerging the need for effective coordination of various sectors and key stakeholders of TB care, to adoption of a new intersectoral strategies that aim to increase the responsiveness of the PHC, providing the best performance in service delivery to the user, family and community, and ensuring effective action and resolving the needs of this population group.

Millon describes the normal personality by means of adaptation styles that are effective in normal environments and personality disorders such as unadapted operating styles. To operacionalize his theoretical model, Millon has built several instruments, including the Millon Clinical Multiaxial Inventory III (MCMI-III), wich consists of a self report inventory composed by 175 true or false response items, containing four verification scales, and others scales wich evaluates 14 personality patterns and 10 clinical syndromes. The Substance Dependence scale (T) is placed along with Clinical Syndromes scales. This research is justified by the lack of a Brazilian instrument to assess personality psychopathological aspects, and aims to translate and semantically adapt the MCMI-III to the Brazilian context, checking validity elements of the Substance Dependence scale, and developing a computer application for assisting the evaluation of assessment results. To this intent, 2.588 individuals data was collected, male and female, aged between 18 and 85 years, characterized as belonging to a clinical or non-clinical group, who took part in the survey via the internet or in person. Respondents completed the MCMI-III, a socio-demographic questionnaire and a subgroup also answered to the Goldberg General Health Questionnaire (GHQ). Besides descriptive statistics, we performed the analysis using the Student t test, principal components analysis and internal consistency. Despite difficulties related to translating very specific English terms, the assessment by judges, experts on Millon´s theory, and the back translation, attested the adequacy of the Brazilian version. Factorial analysis indicated the grouping of translated T scale items into three factors (social activities prejudice, lack of impulse control, and oppositional behavior), by presenting a single item on a fourth factor (apparently related to seeking pleasurable stimuli). The Cronbach alpha for this set of items was 0,82, indicating an acceptable scale reliability. The data analysis resulted in distinction of scores between clinical and non-clinical groups and between men and women; the relationship between high scores on the scale T and the other scales; scores of drug users according to the declared used substance; and the relationship between high scores on T and the verification of disorder or risk on GHQ mental health factor, indicating the instrument´s adequate sensistivity in identifying psychopathologies and the relationship between the different disorders or psychopathological personality patterns. Although further studies are necessary to develop the scores transformation factors, the computerized correction tool was adequate.

Some programs may have their entry data specified by formalized context-free grammars. This formalization facilitates the use of tools in the systematization and the rise of the quality of their test process. This category of programs, compilers have been the first to use this kind of tool for the automation of their tests. In this work we present an approach for definition of tests from the formal description of the entries of the program. The generation of the sentences is performed by taking into account syntactic aspects defined by the specification of the entries, the grammar. For optimization, their coverage criteria are used to limit the quantity of tests without diminishing their quality. Our approach uses these criteria to drive generation to produce sentences that satisfy a specific coverage criterion. The approach presented is based on the use of Lua language, relying heavily on its resources of coroutines and dynamic construction of functions. With these resources, we propose a simple and compact implementation that can be optimized and controlled in different ways, in order to seek satisfaction the different implemented coverage criteria. To make the use of our tool simpler, the EBNF notation for the specification of the entries was adopted. Its parser was specified in the tool Meta-Environment for rapid prototyping

Automation has become increasingly necessary during the software test process due to the high cost and time associated with such activity. Some tools have been proposed to automate the execution of Acceptance Tests in Web applications. However, many of them have important limitations such as the strong dependence on the structure of the HTML pages and the need of manual valuing of the test cases. In this work, we present a language for specifying acceptance test scenarios for Web applications called IFL4TCG and a tool that allows the generation of test cases from these scenarios. The proposed language supports the criterion of Equivalence Classes Partition and the tool allows the generation of test cases that meet different combination strategies (i.e., Each-Choice, Base-Choice and All Combinations). In order to evaluate the effectiveness of the proposed solution, we used the language and the associated tool for designing and executing Acceptance Tests on a module of Sistema Unificado de Administração Pública (SUAP) of Instituto Federal Rio Grande do Norte (IFRN). Four Systems Analysts and one Computer Technician, which work as developers of the that system, participated in the evaluation. Preliminary results showed that IFL4TCG can actually help to detect defects in Web applications
