Desde a criação do instituto de Berlim, em 1919, primeiro serviço ambulatorial em que psicanalistas prestavam assistência aos neuróticos de guerra, a psicanálise não mais se limita às quatro paredes de um consultório particular. A aplicação da psicanálise se dá nas mais diversas instituições e o hospital geral é uma delas. Portanto, esse trabalho é resultado de um estudo realizado durante o estágio que fazia parte do programa da especialização em Psicologia Hospitalar do Instituto Central do Hospital das Clínicas de São Paulo. Além disso, encontra-se aqui também experiências decorrentes de nossa experiência enquanto pesquisador auxiliar do mesmo instituto. Assim, atendimentos à beira dos leitos do pronto socorro, de unidades de tratamento intensivo, de enfermaria, além de atendimentos ambulatoriais, discussão de casos e o trabalho de consultoria e interconsultoria com equipes multiprofissionais, são relatados nesta dissertação, numa tentativa de seguir a indicação freudiana de que, em Psicanálise, clinica e pesquisa coincidem. O objetivo principal deste trabalho foi dissertar sobre as possibilidades e limites de atuação do analista em um hospital geral, partindo da indicação lacaniana de que todo discurso traz em sua estrutura a marca do real, ou seja, o impossível de coincidir seu produto com a verdade, que é sempre não toda. Para isso, coloca-se em relevo a necessidade do retorno de cada um psicanalista aos fundamentos da teoria freudiana e da técnica da psicanálise para que possa não só entrar, mas se inserir em uma equipe multiprofissional. Além disso, partimos da teoria dos quatro discursos de Jacques Lacan para diferenciar a ética da psicanálise da ética aristotélica e a ética dos bens. Isso se faz importante para evidenciar que os limites de atuação de um analista no contexto hospitalar são mais éticos do que burocráticos. Posteriormente, considerando a avaliação como um dos principais efeitos do discurso capitalista no hospital, discutimos o sentido do significante eficácia em psicanálise, ao se diferenciar eficácia simbólica de eficácia analítica. Finalmente, delimitamos que as especificidades de atuação do analista no hospital refere-se ao método e não à teoria e, com isso, propomos a localização subjetiva como o que geralmente se faz possível no hospital em termos de entrevistas preliminares

This thesis is a retrospective qualitative study based on psychotherapy sessions with children presenting autistic features who use language in atypical ways. The aim was to understand, through the transferential relationship, what psychological context in terms of anxieties and defences prevents the child from using language efficiently. Hypotheses concerning children‘s use of language in the context of their emotional oscillation and evolution during the course of treatment were noted and checked against subsequent developments. They were also triangulated with the outcome of a grounded theory analysis. The grounded theory analysis led to the emergence of higher-level themes that were compared within and across cases and allowed factors surrounding the children‘s use of language to be conceptualized. The grounded theory method was used in parallel with the usual process of evaluation of the dynamics of each session and patient used by psychoanalysts a posteriori and which is part of the researcher‘s background. This procedure was enhanced by the use of Bion‘s Grid, here in a version adapted to the aims of the research. This approach is discussed in detail in the Methodology chapter. The psychoanalytic theoretical background that supported the research was mainly based on the tradition of Object-Relations Theory, particularly the evolution of Kleinian thinking represented by Bion‘s works, and as far as autism is concerned, by Frances Tustin and Donald Meltzer‘s formulations. Concerning the subject of language development, Meltzer‘s explorations on the necessary conditions for its development and the philosopher Wittgenstein‘s investigations on the social function of language were the main influences of this work. The evolution of the children‘s use of language in parallel with their emotional development in the context of their psychotherapies was analyzed and some hypotheses about the oscillations in their emotional and mental functioning were made. The oscillation in the children‘s emotional state, language use and thinking processes was also studied in terms of a general fluctuation between different mental states that was considered to be present in different degrees and quality in mental life and more strongly when there are limitations in communication skills and social interaction. A few excerpts from notes on adolescent and adult cases with autistic features were included in the Discussion Chapters to briefly illustrate this aspect.

Les professionnels ont par définition un public bénéficiaire de leurs services : le patient pour le médecin, le psychologue ou l’infirmière ; le client pour l’avocat ; le consommateur pour le gestionnaire d’une entreprise commerciale ; etc. L’usager ou l’habitant du cadre bâti constitue un des destinataires des services professionnels du designer d’intérieur. De quelle manière peut-on apprendre aux étudiants/futurs professionnels du design d’intérieur à se mettre à la place de l’usager/habitant des espaces qu’ils conçoivent ? Le concept de l’empathie, communément décrit comme la capacité de se mettre à la place de l’autre, de le comprendre et de ressentir ses sentiments et ses émotions, est bien adapté pour explorer cette attitude. Cet article est composé de deux parties. La première présente une expérience pédagogique en design d’intérieur où des étudiants apprennent à se représenter les usagers de leur projet d’aménagement en utilisant un outil méthodologique appelé « boussole éthique ». Cette boussole est constituée essentiellement de trois pôles qui renvoient aux trois rapports fondamentaux de l’être humain tels qu’étudiés dans la tradition stoïcienne : rapport à soi-même, à autrui et à la nature. Dans la seconde partie, l’article met en relief plusieurs éléments théoriques qui permettent de comprendre, de consolider et, éventuellement, de faire évoluer les bases conceptuelles qui sous-tendent cette démarche. Il s’agira notamment des théories éthiques et de certaines approches spécifiques au concept de l’empathie.

La recherche vise à élaborer la condition tragique qui émane de la figure de l'enfant au cinéma – à travers ses traumas, ses blessures, voire sa destruction et sa perte – devant laquelle le spectateur adulte éprouve un certain malaise. Nous pouvons voir au cinéma des enfants aux prises avec des conditions de vie traumatisantes, voire inhumaines. Que ce soit les enfants abandonnés ou orphelins, les enfants violés, les enfants maltraités, les enfants victimes de guerre – incluant la dualité paradoxale qu’instaure l'enfant-soldat, de Ivan (« L’enfance d’Ivan », Tarkovski, 1962) à Komona (« Rebelle », Kim Nguyen, 2012) – ou les enfants face à la mort, diverses situations peuvent contribuer à inscrire l'enfance dans le registre du tragique, par la destruction ou la perte de l'enfance. L'enfance détruite porte en elle une blessure (trauma) – qu'elle soit physique ou psychique – qui renvoie au « manque », ou à la fragilité humaine que traduit l'image de l'enfant dit « sans secours ». Quand l’enfant doit faire face à une situation tragique où il est sans défense et confronté à son propre anéantissement, sa figure devient pour nous traumatique. Nous suggérons que le tragique place l’enfant au cinéma dans un après-coup qui renvoie à un traumatisme premier fondamental, prototype de toute situation traumatique. Pour le psychanalyste Sigmund Freud, ce serait un traumatisme lié à la détresse infantile – le « Hilflosigkeit » – qui correspond à la première angoisse vécue par le nourrisson dû à son impuissance (« Inhibition, symptôme et angoisse », [1926] 1951, Paris : PUF). Dans cette lignée, certains psychanalystes (comme Otto Rank ou Jean-Marie Delassus) préciseraient que le traumatisme s'installe dès la naissance, qui marque de manière universelle le premier sentiment de perte et d'angoisse suite à l'éjection dans un monde étranger et d'abord impossible, ce qui correspond à une dislocation première (suivant le sens qu’en propose Benoit Goetz dans « La dislocation » (2002, Paris : Éditions de la Passion)). L'enfant au cinéma, qui traverse des épreuves qui nous sont difficiles à concevoir, nous regarde et pointe en nous nos propres faiblesses, et cela interagit avec nos propres traumatismes. Quelques exemples filmiques viendront soutenir la recherche, avec principalement l’analyse d’un film exemplaire, « Les tortues volent aussi » de Bahman Ghobadi (2004).

Ce texte comprend quatre parties. Dans la première, nous insistons sur deux aspects : la présentation de l’analyse de 31 cas traités par Freud mettant en évidence la non efficacité de son approche et l’aveu de Freud quant à sa vocation littéraire. Au cours de la deuxième partie, nous montrons que le caractère dogmatique des débuts de la psychanalyse sévit encore aujourd’hui dans certains milieux. Nous verrons d’abord que le dogmatisme a mis fin à la carrière de Piaget à titre de psychanalyste, puis nous insisterons sur le fait que la soumission à l’autorité, une caractéristique centrale du dogmatisme, imprègne la psychanalyse et que le cas fondateur de celle-ci, Anna O., repose sur le dogme et le mensonge. Dans la troisième partie, à l’aide d’exemples tirés des écrits de Lacan, Dolto et Bettelheim ainsi que de la pratique actuelle, nous répondrons à ceux qui prétendent que la psychanalyse a évolué, que tel n’est pas nécessairement le cas. Enfin, nous mettons en évidence que la tentative des psychanalystes de récupérer le courant de la neuropsychologie à leur profit ne tient pas la route.

Autistische Phänomene haben seit ihrer Entdeckung (Kanner 1943, Asperger 1944) Wissenschaftler verschiedener Disziplinen immer wieder beschäftigt: Psychiater, Neurowissenschaftler, kognitive Psychologen, Säuglingsforscher, Evolutionspsychologen und Psychoanalytiker haben sich sowohl mit der Beschreibung des Krankheitsbildes wie auch mit den psychischen Prozessen, die bei autistischen Phänomenen vorhanden sind, befasst. Wenn man von einem globalen interdisziplinären Ergebnis der Autismus-Forschung sprechen wollte, könnte man behaupten, dass diese sich als eine Möglichkeit anbot, ein umfassenderes Verständnis für psychische Vorgänge im Allgemeinen zu entwickeln. Die Psychoanalyse als eine „Wissenschaft der Subjektivität“ (Meissner 1983) hat eine lange Tradition in der Entwicklung von Theorie- und Behandlungsansätzen entwickelt. Die kognitiv-psychologischen Untersuchungen haben sich ebenfalls ausführlich mit autistischen Phänomenen befasst. Diese Arbeit stellt einen Versuch dar, eine Fragestellung zu entwickeln, die das psychoanalytische Verständnis autistischer Phänomene mit Auffassungen kognitions-psychologischer Autoren zu verbinden und zu vergleichen sucht. Aus der Sichtweise der Psychoanalyse ist das ein Versuch, die interne bzw. narrative Kohärenz (Leuzinger-Bohleber 1995) psychoanalytischen Verständnisses durch die externe Kohärenz des interdisziplinären Dialoges mit einer anderen Wissenschaft her zu stellen. Zugrunde liegt der Versuch einer Vernetzung verschiedener Erkenntnisse im Sinne verschiedener Modelle, die sich gegenseitig bereichern können. Dem Begriff der Theory of Mind, deren Beeinträchtigung von Kognitionswissenschaftlern (Baron-Cohen 1993, 1995; Baron-Cohen et.al. 1985; Hobson 1993, 2007; Frith 1989, 2003) in der Autismusforschung als grundlegendes Merkmal betrachtet wird, werde ich die psychodynamische Betrachtung (Tustin 1972, 1995; 1981, 1989; 1991; Meltzer 1975; Bion 1962, 1992) gegenüber stellen, die eher von einer Dysfunktion projektiv-identifikatorischer Prozesse (-PI) ausgeht, die sich in einem scheinbaren Mangel an diesen Prozessen äußert. Den von Baron-Cohen entwickelten Parametern, die eine phänomenologische Betrachtung autistischer Phänomene ermöglichen, werde ich die intrapsychische und objektbezogene psychoanalytische Betrachtungsweise gegenüberstellen, die postuliert, dass die Projektive Identifizierung als psychisches Phänomen, das der unbewussten averbalen Kommunikation zugrunde liegt, in autistischen Manifestationen beeinträchtig zu sein scheint. Da die Fähigkeit, psychische Phänomene in sich selbst und in anderen wahrzunehmen, der Psyche immanenteste Eigenschaft und gerade in autistischen Phänomenen beeinträchtigt ist, kann die psychoanalytische Konzeptbildung, die sich mit der Struktur des Psychischen und deren prozesshaftem Charakter befasst, den verschiedenen Disziplinen und auch der Autismus Forschung wichtige Anregungen geben. Anhand einer Einzelfalldarstellung, aus der sich psychodynamische Hypothesen entwickelten, die zu einem psychoanalytischen Verständnis führten, versuche ich, eine gewisse Korrespondenz mit Hypothesen aus der „embodied“ kognitiven Psychologie, wie z.B. die Rolle der Projektiven Identifizierung in der Bildung einer „Theory of Mind“ und ihre Beeinträchtigung (-PI) bei autistischen Phänomenen (Mindblindness, Baron-Cohen) herzustellen.

El presente trabajo de grado es una revisión de la literatura que permite hacer un recorrido desde el psicoanálisis acerca de la compleja problemática de las adicciones. Para ello, se revisaron las bases de datos PepWeb, Ebsco, y Jstor, revisando las posiciones del psicoanálisis freudiano, lacaniano y otras corrientes psicoanalíticas respondiendo a las siguientes preguntas de investigación: (a) ¿Cómo se comprenden las adicciones desde un marco de referencia psicoanalítico? (b) ¿Cómo –desde la teoría freudiana y la teoría lacaniana - se aborda la comprensión de la adicción? (c) ¿Qué dice el psicoanálisis contemporáneo sobre ésta problemática? Se abordan temas como la concepción de una adicción desde la perspectiva de Freud y Lacan hasta psicoanalistas contemporáneos, el rol que juega el goce en la adicción y, finalmente, el complejo debate incipiente sobre la legalidad del tóxico. Se encontró que es un campo de constante publicación y es necesario que los clínicos y los psicoanalistas aborden este campo de estudio clínico permanentemente y sigan produciendo investigaciones sobre el fenómeno.

This article will explore the issues of otherness in the IPA and also consider the local institute level using the example of the British Psychoanalytic Society. According to the IPA it is 'The world's primary psychoanalytic accrediting and regulatory body, with 10,500 members in over forty-five countries. The IPA works in partnership with its Component Organizations to train, support and network psychoanalysts, developing clinical, educational and research programmes'. It is well-known that the history of psychoanalysis is replete with organizational schisms and primary issues of 'inclusion-exclusion'. Who is a real psychoanalyst? "That are the necessary acceptable professional 'standards'? 'While standards' should be derived from knowledge, they are often derived from power and are often really a mask for power-plays. The others' have included those from both outside and within the psychoanalytic movement. Outsiders include medicine and the universities, psychotherapists, other psychoanalysts and other disciplines. Deviationists ,.vithin the IPA include other schools vvithin the'broad church' of psychoanalysis or even just those who don't fit with the proclivities of those in power sometimes rationalized into issues of standards

There are only 5,000 patients in psychoanalysis with members of the American Psychoanalytic Association (APsaA), and these analysts are often viewed as arrogant and insular. As a laboratory for psychoanalytic institutions the APsaA provides crucial lessons for the future across the field. What ingredients are needed for psychoanalysis to be a vibrant discipline? What factors have prevailed where psychoanalysis is successful? The author explores the cases of Argentina and France, where psychoanalysis is relatively popular, and then returns to the U.S. situation. Insular mind-sets led to many missed opportunities for cultural and academic engagement in the United States. As an example, the author explores responses to the making of John Huston's film Freud: The Secret Passion. To become revitalized, psychoanalysis needs to be a cultural asset. Psychoanalysts need to build bridges, engage in partnerships, and emphasize the exciting method of philosophical probing of the human mind and the nature of human nature.

In an age of managed care and new biological therapies for mental illness, psychoanalysis is generally seen as a 'profession on the ropes' whose hour is up. What went wrong? While external factors have played their part in the fall of psychoanalysis, psychoanalysts have generally disregarded their own crucial role in creating this decline. This thesis examines this role as played out through their own institutions, the freestanding psychoanalytic institutes. Freud was an explorer but he also codified his ideas. His work has been taken as an inspiration to explore without presuppositions but also as Holy Writ. Psychoanalysis deals with emotions and excites passions. Like religion, psychoanalysis asks big questions, and, like religion, is easily influenced and seduced by dogmatic answers to these difficult questions. Psychoanalytic institutes have been notable as closed shops. Their solid walls have kept them sealed off and mysterious to the outside world, including the mental health professions and the academy. Authoritarian cliques, power struggles and intrigues have predominated inside the institutes. Institute life has been secret, the subject of rumour rather than knowledge. Insiders often know little about of other institutes (unless they are involved in site visits to particular institutes). Sometimes, insiders have a limited view of their own institutions because they see them through the vantage points of their own experience and that of some close colleagues. I have interviewed central participants of the dramas of the histories of some key psychoanalytic institutes in the US. For the first time, this thesis recounts the intricate inside history of these organizations. The thesis reveals the detailed inner political histories of arguably the four most important and varied psychoanalytic institutes affiliated with the APsaA. The New York Psychoanalytic Institute was the first and for decades the prestigious institute which set the model for many others. It became pre-eminent on a world scale with the immigration of leading European analysts fleeing the Nazis. The Boston Psychoanalytic Society and Institute, the Chicago Psychoanalytic Institute and the Los Angeles Psychoanalytic Society and Institute are quite varied in their organization and histories. The cultures are often quite different yet many of the problems will be found to be similar at base. I first examine the detailed political history of the New York Psychoanalytic Institute which provides a quintessential example of analytic anointment in practice, together with its pitfalls. I then examine a split that occurred in the Boston Psychoanalytic Society and Institute, which demonstrates some of the tensions and ambiguities that seem inherent in psychoanalytic organizations, especially where society and institute are part of the same institution. I move on to investigate a very different history in the Chicago Psychoanalytic Institute, which is quite differently organized: in Chicago, the institute with a lay Board of Trustees is quite separate from the society, and has for most of its history been headed by a powerful director. Then I look at the very complex history of the Los Angeles Psychoanalytic Society and Institute which in the 1970s came very close to being closed down by the APsaA. The Los Angeles Institute history is especially colourful and informative, given the introduction of Kleinian and object relations ideas into the institute and the reactions to them. These histories provide dramatic insights into what psychoanalysts and their institutions have contributed to what has gone wrong with psychoanalysis from the basis of a critique. A major aspect of the problem, in my view, is that a basically humanistic discipline has conceived and touted itself as a positivist science while organizing itself institutionally as a religion. I argue that psychoanalysts approach psychoanalysis with an inappropriate paradigm, 'as if it were a science. Their systemic misconception of their own discipline, and the resultant, widespread creation of what Christopher Bollas calls a 'false expertise' contributes to their present-day decline. I argue that qualification from an institute assumes the transmission of a body of knowledge which has not really been established as knowledge. This presumed knowledge is then transmitted by means of anointment reminiscent of the Bible. There is no unified body of knowledge within the psychoanalytic field nor is there a unified practice that can be readily empirically tested. Therefore, by default, psychoanalytic education has become a process of anointment, transmission through a subjective process akin to consecration. The large gap between the small knowledge base of psychoanalysis and the high level of 'pretend' knowledge which is inculcated during training and upon which qualification is based entrench conditions which themselves make real knowledge in this complex field more and more difficult to attain. This argument has implications not only for psychoanalysis but for many other professions where knowledge and qualification have unrealistic and inappropriate bases.

Essa dissertação foi escrita com o objetivo de promover a discussão sobre o sofrimento psíquico no trabalho. Tema atual, presente na vida de todos os trabalhadores, sejam quais forem suas nacionalidades, sexo, raça, faixa etária, nível de escolaridade, enfim, sujeitos possuidores de subjetividade e de desejo. Contudo, muitas vezes o sofrimento é negado pelas organizações produtivas públicas ou privadas. O sofrimento no trabalho é vivido no quotidiano das organizações como uma carga psíquica responsável por doenças físicas e psicológicas, desajustes emocionais, conflitos interpessoais dentro e fora do ambiente de trabalho, vícios, violência familiar e urbana. O polimorfismo do sofrimento aliado a sua negação pelas organizações e sindicatos confunde e envergonha o trabalhador, que passa a vivê-lo de forma individualizada e solitária. A investigação do tema teve como substrato teórico os estudos sobre psicopatologia do trabalho desenvolvidos por Christophe Dejours e seus seguidores. Além da literatura específica, buscou-se recorrer aos estudos da psicanálise e da teoria geral da administração com o objetivo de proporcionar uma ampla análise do fenômeno abordado. Finalmente, foi realizado um estudo exploratório em uma organização pública, o Hospital Getúlio Vargas Filho, com o objetivo de ilustrar o tema, abordando as questões mais significativas a ele relacionadas.

This article describes the different moments that characterized the diffusion of Kleinian thought in psychoanalytic societies in Brazil. The article results from qualitative historical research based on interviews with thirteen psychoanalysts who participated in this process of diffusion. The first influences appeared in 1950, v pioneers trained either in Britain or in Argentina. The areas in which the Kleinians were pioneers -psychoanalysis of children and of psychotics - were the first aspects dealt with by this group in Brazil. Between 1950 and 1970 very dogmatic approaches were taken toward Kleinian ideas. As of 1980, with the publication of new, translations of Melanie Klein work and with the introduction of contemporary Kleinian thought, a more balanced use of this theoretical-technical model could been seen.

This dissertation aims to answer the question: What are the specifics of psychoanalytical clinic with children in neurosis and psychosis and its consequences for the treatment direction? It constitutes a theoretical study based on Freud, Lacan and the current productions of Lacanian psychoanalysts about the clinic with children. It presents some clinical vignettes. To answer this question, were constructed four chapters. The chapter The subject constitution treats the psychoanalysis subjectivity, based on a structure from the relationship with the Other. Key concepts of Lacanian psychoanalysis are shown, necessary to understand what becomes present in clinic with children. The second chapter, The clinic of neurosis, reveals the structure of the subject in its oedipal mooring held by the Name-of the-Father, that separates the mother-child dual relationship. The child neurosis is the effect of psyche constitution and the symptoms are an interpretation of what child picks up from parents and helps him/her on the passage through the Oedipus. The analyst is there to help him/her through this path. The next chapter is entitled The clinic of psychosis. In psychosis the non-occurrence of the Name-of-the-Father is concerned. The subject is stuck in duality with the mother, and becomes what fills the Other s gap. To protect themselves, they have to be in incessant work. The analyst will be a child s partner in daily work already carried out by him/her. The last chapter, The consequences for the treatment direction, shows that the standard analytic treatment works well to the clinic of neurosis. To psychosis it s not true. Psychoanalysts thought about a different way of psychotic children treatment: the practice held in a multiprofessional team work. The practice shared by many has been a team strategy applied to the institutional practice that aims to attenuate the invasive character of the Other, facilitating the partnership between the analyst and the child in treatment and the Other contention
