A retrospective study of etiology, antimicrobial susceptibility profile and multiple drug resistance, and major epidemiological aspects were investigated in 616 cases of canine otitis. Staphylococcus beta hemolitic (26.27%), Malassezia pachydermatis (12.35%), and Pseudomonas aeruginosa (8.8%) were the most common microorganisms identified. The isolates were susceptible mainly to norfloxacin (89.62%), gentamicin (83.25%), and ofloxacin (80.16%). High occurrence of resistance of isolates was observed to neomicin (30.84%) and cephalexin (27.63%). Multiple drug resistance to three or more and five or more of antimicrobials tested was observed in 34.9% and 15.5% of isolates, respectively. The cases of canine otitis occurred predominantly in first years of age, in mixed breeds animals, at autumn season. The presence of itch, bad smell, and secretion in ear conduct were the major signs observed at clinical examination.

Staphylococcus aureus is the main agent of infections during peritoneal dialysis (PD). The presence of S. aureus in the nasal cavity has been extensively studied and suggested as a risk factor of dialysis-related infections, whereas coagulase-negative Staphylococcus (CNS) species are frequently considered part of the normal human microbiota. The aim of this study was to identify Staphylococcus in the nasal cavity, pericatheter skin and peritoneal effluent from PD patients, as well as to evaluate the antimicrobial activity evolution in vitro. Thirty-two chronic PD patients were observed during 12 months and had nasal and pericatheter skin samples collected for culture. When peritonitis was detected, samples were also collected from the peritoneal effluent for culture. The activity of several antimicrobial drugs (penicillin G, oxacillin, cephalothin, ofloxacin, netilmicin and vancomycin) against different Staphylococcus species was measured by using the agar drug diffusion assay (Kirby-Bauer method). Staphylococcus was separated into S. aureus, S. epidermidis and other CNS species in order to determine the in vitro resistance level. S. epidermidis resistance to oxacillin progressively increased during the study period (p < 0.05). Resistance to ofloxacin was inexpressive, whereas resistance to netilmicin and vancomycin was not detected. of the oxacillin-resistant species (n = 74), 83% were S. epidermidis, 13% other CNS and 4% S. aureus (p < 0.05). Regarding multidrug resistant strains (n = 45), 82% were S. epidermidis, 13% other CNS, and 5% S. aureus (p < 0.05). This study shows the relevance of resistance to oxacillin and CNS multi-drug resistance, particularly concerning S. epidermidis, in PD patients.

In vitro antimicrobial susceptibility of strains of Pseudomonas aeruginosa by standard diffusion disk test and a modified method, by the addition Tris-EDTA, was evaluated. Increase in sensitivity of agent using modified method was observed mainly in aminoglycosides (amikacin, gentamicin, tobramycin), quinolones (ofloxacin and norfloxacin) and cephalosporins (cefoperazone and ceftazidime) groups. by standard diffusion disk test and a modified method, by the addition Tris-EDTA, was evaluated. Increase in sensitivity of agent using modified method was observed mainly in aminoglycosides (amikacin, gentamicin, tobramycin), quinolones (ofloxacin and norfloxacin) and cephalosporins (cefoperazone and ceftazidime) groups.

A revision was accomplished in the literature on the quinolones, antibacterial class that presents wide action spectrum, focusing, mainly, the sparfloxacin, third generation fluorquinolone which has potente activity against Gram positive organisms, including Streptococcus pneumoniae and methicillin-resistant Staphylococcus aureus (MRSA), Gram negative organisms, Legionelia spp, Mycoplasma spp, Chlamydia spp and Mycobacterium spp, including multidrug resistant organisms.

Staphylococcus aureus is a very important hospital and community pathogen. This species is related to supurative disease, systemic and widespread metastatic lesions. The ability of S. aureus to develop resistance to antibiotics, more recently to methicillin, associates this bacteria with epidemic outbreaks of severe nosocomial infection. The source of staphylococcal infection is a patient with a staphylococcal lesion or a career member of the hospital staff. We aimed to detect the frequency of S. aureus isolated from anterior nares and oral cavities among the hospital staff in Bauru - SP, and to determine the antibiotic susceptibility of the isolates. Within 213 of the staff members analyzed. S. aureus was found in 94 (44.13%) of careers, with 47 (50%) of nasal carriers, 23 (24.4%) of oral carriers and 24 (25.5%) of both carriers. The biochemical characteristics analyzed for the species identification were similar to S. aureus ATCC 29213. All strains identified as S. aureus showed varied sensibility to the antimicrobial agents tested. No vancomicin resistant strains and only 8 (8.5%) strains with oxacilin resistance were found.

The aim of this study was the assessment of isolation frequency and antimicrobial susceptibility pattern of nonfermenting Gram-negative bacilli. Ninety eight strains of nonfermenting Gram-negative bacilli, isolated from several clinical materials of patients admited at the Dr. Domingos Leonardo Cerávolo University Hospital and at Dr. Odilo Antunes Siqueira State Hospital, as well as from every outpatient; assisted at Laboratory of Clinical Analysis of Unoeste University, Presidente Prudente, São Paulo, in the period of October 1999 to April 2001 were analyzed. The most frequent species were Pseudomonas aeruginosa (65.3%) and Acinetobacter baumannii (23.5%). The frequency of the other isolated species was smaller than 2.5%. In the antimicrobial susceptibility tests, the two species more prevalent showed high resistance. The antibiotic most active in vitro was the imipenem, with 79.6% in microdiluition method, and 76.6% in diffusion method, for Pseudomonas aeruginosa strains and 100.0% in both microdiluition and diffusion methods, for Acinetobacter baumannii. The cephalosporins of third generation, the ciprofloxacin and the aminoglycosides, presented percentage of susceptibility varying from 22.4 to 69.7%. These results bring implications to the emergency use of the antimicrobial agents in the treatment of patients with severe infection.

Für die Krebsentstehung sind nach heutigen Vorstellungen Mutationen in bestimmten Genen somatischer Zellen verantwortlich, die ihrerseits durch chemische Modifikationen der DNA (DNA-Schäden) verursacht werden. Von besonderem Interesse als DNA-schädigende Agentien sind reaktive Sauerstoffspezies (ROS), die endogen - wie z.B. in der Lipidperoxidation oder mitochondrialen Atmungskette - oder exogen - z.B. durch Arzneimittel oder andere aus der Umwelt stammende Chemikalien - gebildet werden können. Dem Schutz der DNA vor dieser Schädigung und ihren Folgen dienen antioxidative Systeme und DNA-Reparatur, deren Effizienz von verschiedenen Genen der Zelle abhängig ist. Ziel dieser Arbeit war die Untersuchung des Einflusses bestimmter Faktoren auf die Bildung, Inhibierung (antioxidative Wirkung) und Reparatur oxidativer DNA-Schäden. Als Beispiel für exogene Stoffe, die zur Bildung oxidativer DNA-Schäden führen, wurden die Gyrasehemmer Ofloxacin und Norfloxacin nach Bestrahlung mit UV-360 untersucht. Unter Verwendung spezifischer DNA-Reparatur-endonukleasen als Sonden (Fpg-Protein, Endonuklease III, Endonuklease IV, Exonuklease III, T4 Endonuklease V) wurden spezifisch DNA-Modifikationen an zellfreier und zellulärer DNA quantifiziert. Es zeigte sich, daß es sich im Falle des Ofloxacins bei der ultimal mit der DNA reagierenden reaktiven Spezies um Hydroxylradikale handelt, während durch Norfloxacin Singulettsauerstoff entsteht. Durch Zusatz verschiedener Antioxidantien konnten diese Ergebnisse bestätigt werden. Im Gegensatz zu Hydroxylradikalen ist über die DNA-Schäden und Mutationen durch Alkoxyl-Radikale, die endogen bei der Lipidperoxidation entstehen, nichts bekannt. Die Substanz BCBT ([4-[(tert-Butyldioxy)carbonyl]benzyl]-triethyl-ammonium-chlorid) generiert Alkoxyl-Radikale. BCBT + UV-360 induzierte vorwiegend Fpg-sensitive Modifikationen, die durch Bestimmung mittels HPLC/ECD zu 83 ± 18% als 8-Hydroxyguanin identifiziert wurden. Der Vergleich des Einflusses der Zusätze tert-Butanol und Natriumazid auf die Schädigung mit BCBT mit anderen reaktiven Spezies zeigte klar, daß die Schädigung hier über Alkoxyl-Radikale verläuft. Die Analyse und Sequenzierung der durch BCBT induzierten Mutanten zeigte, daß vorwiegend GC->TA Transversionen gebildet werden, die auf 8-oxoG zurückzuführen sind. Auf der Ebene der Antioxidantien wurde der Einfluß des zelleigenen Glutathions untersucht. Mit der Depletion des Glutathion-Gehalts durch Vorbehandlung mit L-Buthionin-SR-sulfoximin (BSO) ging eine Erhöhung des Steady-State-Spiegels in den untersuchten AS52 Zellen einher. Selen ist ein wichtiges Spurenelement und u.a. Bestandteil von Glutathionperoxidasen (GPx). Um die Bedeutung von Selen bei der oxidativer DNA-Modifikationen bzw. deren Inhibierung zu untersuchen, wurden Natriumselenit und Ebselen eingesetzt. Ab einer Konzentration von 50 µM trat durch Natriumselenit eine Erhöhung des Steady-State-Spiegels auf (prooxidativer Effekt). Parallel mit der oxidativen Schädigung der DNA ist eine Erhöhung des zellulären Gehalts an Glutathion feststellbar. Ebselen bedingte in Konzentrationen bis 200 µM keine Erhöhung des Steady-State-Spiegels oder des Gehalts an reduziertem Glutathion. Beide Substanzen waren in der Lage, die durch sichtbares Licht und UV-B-Strahlung induzierten Einzelstrangbrüche zu reduzieren. Mikrokerne, die durch Schädigung mit den Agentien Kaliumbromat, Ro 19-8022 + Licht und Wasserstoffperoxid entstanden, waren ebenso durch Natriumselenit und Ebselen reduzierbar. In dieser Arbeit konnte ferner gezeigt werden, daß p53 keinen Einfluß auf die Reparatur UV-induzierter Modifikationen (Nukleotidexzisionsreparatur) und die Reparatur Fpg-sensitiver Modifikationen (Basenexzisionsreparatur) hat.

Using a rabbit model of pneumococcal meningitis, we compared the pharmacokinetics and bactericidal activities in cerebrospinal fluid (CSF) of older (ciprofloxacin, ofloxacin) and newer (levofloxacin, temafloxacin, CP-116,517, and Win 57273) quinolones with those of the beta-lactam ceftriaxone. All quinolones penetrated into the inflamed CSF better than ceftriaxone, and the speed of entry into CSF was closely related to their degrees of lipophilicity. At a dose of 10 mg/kg.h, which in the case of the quinolones already in use in clinical practice produced concentrations attainable in the sera and CSF of humans, ciprofloxacin had no antipneumococcal activity (delta log10 CFU/ml.h, +0.20 +/- 0.14). Ofloxacin (delta log10 CFU/ml.h, -0.13 +/- 0.12), temafloxacin (delta log10 CFU/ml.h, -0.19 +/- 0.18), and levofloxacin (delta log10 CFU/ml.h, -0.24 +/- 0.16) showed slow bactericidal activity (not significantly different from each other), while CP-116,517 (delta log10 CFU/ml.h, -0.59 +/- 0.21) and Win 57273 (delta log10 CFU/ml.h, -0.72 +/- 0.20) showed increased bactericidal activities in CSF that was comparable to that of ceftriaxone at 10 mg/kg.h (delta log10 CFU/ml.h, -0.80 +/- 0.17). These improved in vivo activities of the newer quinolones reflected their increased in vitro activities. All quinolones and ceftriaxone showed positive correlations between bactericidal rates in CSF and concentrations in CSF relative to their MBCs. Only when this ratio exceeded 10 did the antibiotics exhibit rapid bactericidal activities in CSF. In conclusion, in experimental pneumococcal meningitis the activities of new quinolones with improved antipneumococcal activities were comparable to that of ceftriaxone.

Rifampin at a maximally effective dose was less active than ceftriaxone (both drugs at 10 mg/kg of body weight.h) in a rabbit model of pneumococcal meningitis (delta log10 CFU/ml.h, -0.40 +/- 0.13 versus -0.77 +/- 0.18; P < 0.01). The bactericidal activity of rifampin decreased at concentrations in cerebrospinal fluid greater than those that are clinically achievable, and use of rifampin in combination with ofloxacin had no synergistic or additive effect.

The activity of moxifloxacin was compared with ofloxacin and doxycycline against bacteria associated with periodontitis within a biofilm (single strain and mixed population) in vitro. Minimal inhibitory concentrations (MICs) and minimal bactericidal concentrations (MBCs) of moxifloxacin, ofloxacin and doxycyline were determined against single strains and mixed populations in a planktonic state. Single-species biofilms of two Porphyromonas gingivalis and two Aggregatibacter actinomycetemcomitans strains and a multi-species biofilm consisting of 12 species were formed for 3 days. The minimal biofilm eradication concentrations (MBECs) were determined after exposing the biofilms to the antibacterials (0.002 - 512 µg ml-1) for 18 h, addition of nutrient broth for 3 days and subsequent subcultivation. Photographs were taken by using confocal laser scanning microscopy and scanning electron microscopy. The MICs and MBCs did not differ between ofloxacin and moxifloxacin against A. actinomycetemcomitans, moxifloxacin was more active than the other tested antibacterials against anaerobes and the mixed population. The single-species biofilms were eradicated by moderate concentrations of the antibacterials, the lowest MBECs were always found for moxifloxacin (2-8 µg ml-1). MBECs against the multi-species biofilms were 128 µg ml-1, >512 µg ml-1 and >512 µg ml-1 for moxifloxacin, ofloxacin and doxycycline, respectively. In summary, moxifloxacin in a topical formulation may have potential as an adjunct to mechanical removal of the biofilms.

The incidence of human brucellosis in Kyrgyzstan has been increasing in the last years and was identified as a priority disease needing most urgent control measures in the livestock population. The latest species identification of Brucella isolates in Kyrgyzstan was carried out in the 1960s and investigated the circulation of Brucella abortus, B. melitensis, B. ovis, and B. suis. However, supporting data and documentation of that experience are lacking. Therefore, typing of Brucella spp. and identification of the most important host species are necessary for the understanding of the main transmission routes and to adopt an effective brucellosis control policy in Kyrgyzstan. Overall, 17 B. melitensis strains from aborted fetuses of sheep and cattle isolated in the province of Naryn were studied. All strains were susceptible to trimethoprim-sulfamethoxazole, gentamicin, rifampin, ofloxacin, streptomycin, doxycycline, and ciprofloxacin. Multilocus variable number tandem repeat analysis showed low genetic diversity. Kyrgyz strains seem to be genetically associated with the Eastern Mediterranean group of the Brucella global phylogeny. We identified and confirmed transmission of B. melitensis to cattle and a close genetic relationship between B. melitensis strains isolated from sheep sharing the same pasture.
