Purpose: To explore the knowledge, attitudes, practice and perceived barriers of community pharmacists regarding provision of pharmaceutical care as well as provide recommendations on how to advance the service during the early stage of development in Macao. Methods: A questionnaire comprising 10 items was used to collect respondents’ demographic information and to evaluate their understanding of pharmaceutical care, attitude towards service provision, current practice and perceived barriers. Descriptive and comparative analysis of the results was conducted. Results: While 95 % of the participating pharmacists agreed that patients’ health was their primary responsibility, only 57 % believed that they can provide better pharmaceutical care in the future. The majority spent most of their work time counselling patients (90 %) and checking prescription (70 %). Only a small portion monitored adverse drug reaction and drug compliance (44 %), engaged in health screening or drug safety promotion (20 %) or maintained patient medication records (4 %). Insufficient communication with physicians (90 %), lack of time (79 %) and lack of physical space at the pharmacy (76 %) were considered the most significant barriers. Conclusion: A suboptimal level of pharmaceutical care is provided by pharmacists in Macao. Considering the barriers identified and integrating other country experiences, establishing an enabling atmosphere using policy and regulatory measures is the fundamental element for advancing pharmaceutical care by community pharmacists.

Ein sorgfältiges INR-Monitoring ist wichtig für eine sichere und effektive Vitamin-K-antagonistische Therapie (VKA). Einer von vielen Faktoren, welcher die Einhaltung des INR-ZSB beeinflusst, ist die Compliance. Im Rahmen der vorliegenden Arbeit wurde erstmals die Effektivität einer intensiven medizinisch-pharmazeutischen Betreuung von Patienten mit Phenprocoumon-Therapie, unter besonderer Berücksichtung der Compliance, untersucht. 71 Patienten mit VKA-Therapie (Interventionsgruppe (IG) und Kontrollgruppe (KG)) wurden in die prospektive Studie eingeschlossen und über 6 Monate beobachtet. Patienten der IG erfuhren eine intensive medizinisch-pharmazeutische Betreuung im Thrombosedienst. Die Patienten der Kontrollgruppe wurden vom Hausarzt betreut. Zusätzlich fand eine Compliance-Messung mittels OtCM-Blistern (Objective-therapy-compliance-measurement-system) statt. Während die Compliance der Kontrollgruppe retrospektiv am Ende des Beobachtungszeitraums ausgewertet wurde, erfuhren die Patienten der Interventionsgruppe ein „real-time“ Monitoring der Phenprocoumon-Entnahme aus den Blistern. Nach per Protokoll-Analyse konnten die Daten von 58 Patienten ausgewertet werden. Patienten der IG zeigten eine signifikant bessere INR-Einhaltung im ZSB (Time in Therapeutic Range (TTR) 74%) im Vergleich zur Kontrollgruppe (TTR 53%; p= 0,001). Die verschiedenen gemessenen Compliance-Raten (Dosing-, Taking-, Timing Compliance und Drug Holidays) waren in der IG höher als in der KG. Patienten mit einer höheren TTR waren tendenziell besser compliant. Spezialisierte medizinisch-pharmazeutische Betreuung hat einen positiven Effekt auf das Outcome der VKA-Therapie.rnrn

In Deutschland wird zur oralen Vitamin-K-Antagonistentherapie überwiegend der Wirkstoff Phenprocoumon (PPC) eingesetzt und die meisten Patienten werden durch ihren Hausarzt betreut. In einer deskriptiven, nicht-interventionellen Studie wurde die Ist-Situation der Versorgung von PPC-Patienten im ambulanten Sektor untersucht. Ziel war es, die Qualität und Effektivität der bisherigen Standardtherapie zu evaluieren. In Anbetracht der Einführung der neuen oralen Antikoagulantien (NOAC) ist die Untersuchung der PPC-Therapie von besonderem Interesse. Dem „Throughput-Modell“ folgend sollten „Input“- und „Outcome“-Parameter analysiert werden. rnIn einer klinischen Studie wurden 50 ambulant behandelte Patienten mit PPC-Therapie jeweils über einen Zeitraum von 3 Jahren retrospektiv beobachtet. In 5 niedergelassenen Arztpraxen in Rheinland-Pfalz wurden dazu 10 Patienten pro Praxis rekrutiert. Anhand der Patientenakte wurde eine Dokumentenanalyse durchgeführt. Die Selbstmedikation wurde mit einem eigens erstellten Fragebogen erfasst. rnIm Studienkollektiv wurden im Median 3 Comorbiditäten ermittelt. Die mediane Wochendosis betrug 4,0 Tabletten à 3 mg PPC. Die Patienten wurden im Median mit weiteren 15 verschiedenen Wirkstoffen therapiert, einer davon wurde in Selbstmedikation eingenommen. Im gesamten Beobachtungszeitraum fanden pro Patient im Median 57 Arztbesuche statt, die durch die Phenprocoumon-Therapie bedingt waren. INR (International normalized ratio)-Messungen (Median 47) waren der häufigste Grund für die Arztbesuche, so dass ein 3-Wochen-Rhythmus vom Gesamtkollektiv zu 97% erreicht wurde. Die „stabile“ INR-Einstellung wurde im Median nach 94 Tagen erreicht. Die prozentuale Rate (INR (%)) für die Einhaltung des INR-Zielbereiches (ZSB) erreichte internationale Benchmark-Werte, was auf eine gute Versorgungsqualität hindeutete. Die genauere Analyse ergab jedoch große interindividuelle Schwankungen. Während der „stabilen“ INR-Einstellung wurden bessere Ergebnisse als im Gesamtbeobachtungszeitraum erzielt. Drei Patienten (6%) erreichten die „stabile“ INR-Einstellung innerhalb von 3 Jahren nie. Die Auswertung für den erweiterten ZSB (ZSB ± 0,2) ergab bessere INR (%)-Ergebnisse als für den ZSB. Die Zeit im INR-ZSB (TTR (%)) erreichte mit 75% höhere Werte als INR (%) im ZSB mit 70%. Tendenziell war das Patientenkollektiv eher unter- als übertherapiert (Median „Under-INR“ 18% bzw. „Over-INR“ 8%). Erkrankungen und Impfungen stellten die wichtigsten der zahlreichen Einflussfaktoren für INR-Shifts hin zu Werten außerhalb des ZSB dar. Patienten, die Comedikation mit hohem Interaktionspotential einnahmen, erreichten in den INR-Qualitätsindikatoren schlechtere Ergebnisse als Patienten ohne potentiell interagierende Comedikation (Mann-Whitney-U-Test; p-Wert=0,003 für TTR (%), p=0,008 für INR (%)). In Zeitintervallen der „stabilen“ INR-Einstellung war der Unterschied nur für TTR (%) auffällig (Mann-Whitney-U-Test; p=0,015). Für den erweiterten ZSB waren die Unterschiede bezüglich beider INR-Qualitätsindikatoren nicht auffällig. Insgesamt wurden 41 unerwünschte Ereignisse (UAW) beobachtet, davon 24 (59%) in der Phase der „stabilen“ INR-Einstellung (21 leichte Blutungen, 1 schwere Blutung, 2 thromboembolische Ereignisse (TE)). Je 4 leichte Blutungen (19%) wurden in einen möglichen bzw. sicheren kausalen Zusammenhang mit der VKA-Therapie gebracht, wenn ein Zeitintervall von 3 Tagen zwischen der INR-Messung und Auftreten der UAW geprüft wurde. Ein TE wurde als sicher kausal gewertet. Von insgesamt 5 Krankenhausaufenthalten waren 3 bzw. 2 durch Blutungen bzw. TE veranlasst. Des Weiteren wurde im 3-Tage-Zeitintervall für 4 INR-Shifts hin zu Werten außerhalb des ZSB eine Interaktion mit verordneter CM als in sicherem oder möglichem kausalen Zusammenhang bewertet. Bei 49% der beobachteten Grippeimpfungen wurde ein INR-Shift festgestellt, der in ca. 60% der Fälle zu einem subtherapeutischen INR-Wert führte. Insgesamt war das klinische Ergebnis nicht optimal. rnDas „Outcome“ in Form der gesundheitsbezogenen Lebensqualität (LQ) wurde retrospektiv-prospektiv mittels SF-36-Fragebogen ermittelt. Die Patienten zeigten gegenüber der Normalbevölkerung einen Verlust an LQ auf körperlicher Ebene bzw. einen Gewinn auf psychischer Ebene. Das humanistische Ergebnis erfüllte bzw. übertraf damit die Erwartungen. rnInsgesamt wiesen die Ergebnisse darauf hin, dass Qualität und Effektivität der Antikoagulationstherapie mit PPC im ambulanten Sektor weiterer Optimierung bedürfen. Mit intensivierten Betreuungsmodellen lässt sich ein besseres Outcome erzielen. rn

OBJECTIVE: Pharmaceutical assistance is essential in health care and a right of citizens according to Brazilian law and drug policies. The study purpose was to evaluate aspects of pharmaceutical assistance in public primary health care. METHODS: A cross-sectional study using WHO drug indicators was carried out in Brasília in 2001. From a random sample of 15 out of 62 centers thirty exiting patients per center were interviewed. RESULTS: Only 18.7% of the patients fully understood the prescription, 56.3% could read it, 61.2% of the prescribed drugs were actually dispensed, and mean duration of pharmaceutical dispensing was 53.2 seconds. Each visit lasted on average 9.4 minutes. Of prescribed and non-dispensed drugs, 85.3% and 60.6% were on the local essential drug list (EDL) respectively. On average 83.2% of 40 essential drugs were in stock, and only two centers had a pharmacist in charge of the pharmacy. The mean number of drugs per prescription was 2.3, 85.3% of prescribed drugs were on the EDL, 73.2% were prescribed using the generic denomination, 26.4% included antibiotics and 7.5% were injectables. The most prescribed groups were: cardiovascular drugs (26.8%), anti-infective drugs (13.1%), analgesics (8.9%), anti-asthmatic drugs (5.8%), anti-diabetic drugs (5.3%), psychoactive drugs (3.7%), and combination drugs (2.7%). CONCLUSIONS: Essential drugs were only moderately available almost 30 years after the first Brazilian EDL was formulated. While physician use of essential drugs and generic names was fairly high, efficiency was impaired by the poor quality of pharmaceutical care, resulting in very low patient understanding and insufficient guarantee of supply, particularly for chronic diseases.

Background: Treatment of depression, the most prevalent and costly mental disorder, needs to be improved. Non-concordance with clinical guidelines and non-adherence can limit the efficacy of pharmacological treatment of depression. Through pharmaceutical care, pharmacists can improve patients' compliance and wellbeing. The aim of this study is to evaluate the effectiveness and costeffectiveness of a community pharmacist intervention developed to improve adherence and outcomes of primary care patients with depression. Methods/design: A randomized controlled trial, with 6-month follow-up, comparing patients receiving a pharmaceutical care support programme in primary care with patients receiving usual care. The total sample comprises 194 patients (aged between 18 and 75) diagnosed with depressive disorder in a primary care health centre in the province of Barcelona (Spain). Subjects will be asked for written informed consent in order to participate in the study. Diagnosis will be confirmed using the SCID-I. The intervention consists of an educational programme focused on improving knowledge about medication, making patients aware of the importance of compliance, reducing stigma, reassuring patients about side-effects and stressing the importance of carrying out general practitioners' advice. Measurements will take place at baseline, and after 3 and 6 months. Main outcome measure is compliance with antidepressants. Secondary outcomes include; clinical severity of depression (PHQ-9), anxiety (STAI-S), health-related quality of life (EuroQol-5D), satisfaction with the treatment received, side-effects, chronic physical conditions and sociodemographics. The use of healthcare and social care services will be assessed with an adapted version of the Client Service Receipt Inventory (CSRI). Discussion: This trial will provide valuable information for health professionals and policy makers on the effectiveness and cost-effectiveness of a pharmaceutical intervention programme in the context of primary care. The objectives of this study were to ascertain consumer knowledge and behaviour about hypertension and treatment and to compare these with health care providers' perceptions (of 'most' consumers). The design for the study was a problem detection study (PDS): focus groups and then survey. Focus groups and survey participants were convenience samples of consumers, doctors, nurses and pharmacists. The main outcome measures were agreement on a 5-point Likert scale with statements about consumers' knowledge and behaviour about high blood pressure and medication. The survey identified areas of consensus and disagreement between consumers and health providers. While general knowledge and concordance with antihypertensive therapy among consumers was good, consequences such as eye and kidney disease, interactions with herbal medicines, and how to deal with missing a dose were less well known. Side effects were a problem for over one-quarter of participants, and cost was a problem in continuing therapy. Half the consumers had not received sufficient written information. Providers overall disagreed that most consumers have an adequate understanding of the condition. They agreed that most consumers adhere to therapy and can manage medicines; and about their own profession's role in information provision and condition management. Consumers confirmed positive provider behaviour, suggesting opportunities for greater communication between providers about actions taken with their consumers. In conclusion, the PDS methodology was useful in identifying consumer opinions. Differences between consumer and provider responses were marked, with consumers generally rating their knowledge and behaviour above providers' ratings of 'most' consumers. There are clear gaps to be targeted to improve the outcomes of hypertension therapy.

Substantial complexity has been introduced into treatment regimens for patients with human immunodeficiency virus (HIV) infection. Many drug-related problems (DRPs) are detected in these patients, such as low adherence, therapeutic inefficacy, and safety issues. We evaluated the impact of pharmacist interventions on CD4+ T-lymphocyte count, HIV viral load, and DRPs in patients with HIV infection. In this 18-month prospective controlled study, 90 outpatients were selected by convenience sampling from the Hospital Dia-University of Campinas Teaching Hospital (Brazil). Forty-five patients comprised the pharmacist intervention group and 45 the control group; all patients had HIV infection with or without acquired immunodeficiency syndrome. Pharmaceutical appointments were conducted based on the Pharmacotherapy Workup method, although DRPs and pharmacist intervention classifications were modified for applicability to institutional service limitations and research requirements. Pharmacist interventions were performed immediately after detection of DRPs. The main outcome measures were DRPs, CD4+ T-lymphocyte count, and HIV viral load. After pharmacist intervention, DRPs decreased from 5.2 (95% confidence interval [CI] =4.1-6.2) to 4.2 (95% CI =3.3-5.1) per patient (P=0.043). A total of 122 pharmacist interventions were proposed, with an average of 2.7 interventions per patient. All the pharmacist interventions were accepted by physicians, and among patients, the interventions were well accepted during the appointments, but compliance with the interventions was not measured. A statistically significant increase in CD4+ T-lymphocyte count in the intervention group was found (260.7 cells/mm(3) [95% CI =175.8-345.6] to 312.0 cells/mm(3) [95% CI =23.5-40.6], P=0.015), which was not observed in the control group. There was no statistical difference between the groups regarding HIV viral load. This study suggests that pharmacist interventions in patients with HIV infection can cause an increase in CD4+ T-lymphocyte counts and a decrease in DRPs, demonstrating the importance of an optimal pharmaceutical care plan.

Para identificar mecanismos de compatibilização entre a lei e as normas técnicas, foram considerados o conceito de saúde e as características do Estado Democrático de Direito. Tomando-se o exemplo brasileiro das normas da política de assistência farmacêutica, concluiu-se que racionalidade jurídica impõe verificar se sua elaboração obedeceu ao requisito constitucional que exige a "participação da comunidade", instaurando um controle democrático e judicial.

Para se conhecer a prática profissional de farmacêuticos que atuam em farmácias e drogarias, seus conhecimentos e percepções acerca da Atenção Farmacêutica (AF), realizou-se estudo descritivo com 91 farmacêuticos do município de Jundiaí-SP. A maioria era jovem (62,6% entre 20 e 29 anos), do sexo feminino (63,7%), graduada em instituições privadas (90,1%) e não proprietária do estabelecimento (87,9%). Desenvolviam atividades administrativas, técnicas e de atenção ao usuário, principalmente dispensação de medicamentos e orientação; 67,0% acompanhavam o tratamento farmacoterapêutico dos usuários, mas sem registrar informações. Para 62,7%, AF relacionava-se apenas à orientação e atendimento dispensados, mas tais atividades não eram realizadas de forma sistemática e organizada, como preconizado. Muitos (91,2%) consideravam necessário realizar trabalho mais intenso com os usuários, porém apontaram dificuldades como falta de tempo e de apoio dos proprietários e desinteresse dos usuários. Várias dessas dificuldades têm sido verificadas também em outros países, sugerindo que a prática da AF: (a) requer uma mudança estrutural e rearranjo de funções, uma vez que, atualmente, a estrutura e as atividades são adequadas à atividade comercial; (b) reflete uma crise de identidade profissional e, em consequência, falta de reconhecimento social e pouca inserção na equipe multiprofissional de saúde. O conhecimento sobre AF mostrou-se limitado, mas a situação pode vir a alterar-se à medida que as mudanças curriculares em curso surtam efeito na formação dos novos farmacêuticos.

No Brasil, apesar dos avanços da assistência farmacêutica, permanecem falhas na garantia do acesso dos cidadãos aos medicamentos pelo Estado. Nos últimos anos, vem crescendo a reivindicação de medicamentos por parte do cidadão via sistema judiciário. Os objetos dessas solicitações são tanto os medicamentos em falta na rede pública como aqueles ainda não incorporados pelo Sistema Único de Saúde. Este fenômeno pode ser analisado sob diferentes perspectivas, inclusive a sanitária, entendida aqui como os desfechos sobre a saúde dos indivíduos que demandam estes medicamentos. O presente texto busca discutir as principais características das demandas judiciais frente aos seguintes aspectos: o uso racional de medicamentos, o uso de evidências científicas para a indicação terapêutica proposta e o quanto as demandas se justificam diante do conceito de acesso adotado pelo campo da assistência farmacêutica. Ponderações podem ser feitas no sentido de minimizar os riscos à saúde dos demandantes de medicamentos por via judicial, sobretudo quando o objeto da ação são medicamentos não pertencentes às listas de fornecimento público, ou com uso off label, ou desprovidos de registro no país. Considera-se que o Judiciário, a partir do fornecimento de medicamentos, busca garantir a saúde dos demandantes, e assim a dignidade da pessoa humana. Cabe ressaltar que este objetivo só será atingido quando a garantia da saúde estiver associada aos aspectos que certificam a segurança do paciente, inclusive no uso de medicamentos.

Incidentes com medicamentos geram problemas aos pacientes e custos adicionais ao sistema de saúde. A variedade de termos utilizada para comunicá-los propicia divergências nos resultados de pesquisas e confundem notificadores. Objetivou-se revisar os termos utilizados para descrever estes incidentes confrontando-os com as conceituações/definições oficiais disponíveis. Pesquisaram-se as bases PubMed, MEDLINE, IPA e LILACS para selecionar estudos publicados entre janeiro de 1990 e dezembro de 2005. Selecionaram-se 33 publicações. Verificou-se que a terminologia supranacional recomendada para descrever incidentes com medicamentos é insuficiente, mas que há consenso de uso das expressões em função do gênero do incidente. O termo Reação Adversa a Medicamento é mais utilizado quando não se verifica intencionalidade. A expressão Evento Adverso a Medicamento foi mais usada quando se descreviam incidentes durante a hospitalização; e Problema Relacionado a Medicamento foi mais utilizada em estudos que avaliaram atenção/cuidados farmacêuticos (uso/falta do medicamento). Ainda assim, a linha divisória entre essas três categorias não é clara e simples. Futuros estudos das relações entre as categorias e investigações multidisciplinares sobre erro humano podem subsidiar a proposição de novas conceituações
